The landscape of the radiology workforce is changing, especially in the diversity of the demographics of practicing radiologists across subspecialties, practice types, and leadership positions in both academic and nonacademic settings. The 2021 ACR/Radiology Business Management Association Workforce Survey examines these facets in detail and contributes to our understanding of the current state of diversity in the radiology workforce and potential barriers to change. Sarcoidosis is an idiopathic multisystem disorder characterized by noncaseating granulomas. The article focuses on the typical imaging manifestations of sarcoid and the common differentials that need to be included when appropriate. Mistaking a sarcoid-mimicking disease for sarcoid can result in increased patient morbidity and mortality. The pulmonary system is the most common system involved and is typically the best understood by the radiologist, however a deeper knowledge of the pulmonary findings and features of sarcoid in other organ systems is critical. There is a myriad of sarcoid imaging manifestations that can involve every organ system. Often a confidant diagnosis of sarcoid can be made, however a broad differential may need to be considered- differential diagnoses include primary neoplasm, metastatic disease, infectious, and inflammatory etiologies. As research continues to demonstrate successes in the use of percutaneous trans-vascular techniques in structural heart intervention, both the subspecialty trained and non-subspecialty trained cardiac imager find themselves performing and reporting larger amounts of information regarding cardiovascular findings. It is therefore imperative that the imager gains understanding and appreciation for how these various measurements are obtained, as well as their implication in a patient's care. Cardiac gated computed tomography (CT) has solidified its role and ability at providing high resolution images that can be used to obtain the key measurements used in structural heart intervention planning. This manuscript aims to provide an overview of what measurements are necessary to report when interpreting CT examinations purposed for structural heart intervention. Abstract We evaluated whether SPAtially Referenced Regression On Watershed attributes (SPARROW) models calibrated for two adjacent USA regions could be applied at the local scale to support management decisions for streams in Tennessee. Nutrient-source apportionment of load is important for this local-scale application and demands careful consideration of uncertainty in the calibrated coefficients. We used Gauss-Newton regression to test the published SPARROW models for constancy of coefficient estimates between calibration sites on streams within (n = 59) versus outside (n = 327) Tennessee and concluded source apportionment was unbiased for Tennessee streams. The SPARROW models were then applied without re-calibration to predict stream loads and source apportionment for Tennessee streams and to build tools for displaying model results and evaluating source-change scenarios. This approach may inform the adaptation of other regional-scale regression models for use to address water-resource management issues in smaller-scale watersheds. Business communication applications soared during the coronavirus disease 2019 (COVID-19) pandemic; the pandemic will eventually end, but telecommunications will now be a form of how we communicate and may be even more prevalent after the pandemic. As expected, the demand for videoconferencing also grew exponentially; mobile app downloads reached 62 million during the week of March 14 to 21, 2020, a 90% increase compared with pre-COVID-19 weekly download average [1]. Personalities are like jigsaw puzzles and when placed together there can be may rewards to the individual, section, department or company. The Myers-Briggs Type Indicator can be used to help assess individuals on four different dichotomies. To study the prevalence and management of shattered kidney and to evaluate if the new description of "loss of identifiable renal anatomy" in the 2018 American Association for the Surgery of Trauma (AAST) organ injury scale (OIS) would improve the ability to predict bleeding control interventions. We used high-grade renal trauma data from 21 Level-1 trauma centers from 2013 to 2018. Initial CT scans were reviewed to identify shattered kidneys, defined as a kidney having ≥3 parenchymal fragments displaced by blood or fluid on cross-sectional imaging. We further categorized patients with shattered kidney in two models based on loss of identifiable renal parenchymal anatomy and presence or absence of vascular contrast extravasation (VCE). Bleeding interventions were compared between the groups. From 861 high-grade renal trauma patients, 41 (4.8%) had shattered kidney injury. 25 (61%) underwent a bleeding control intervention including 18 (43.9%) nephrectomies and 11 (26.8%) angioembolizations. 18 (41%) had shattered kidney with "loss of identifiable parenchymal renal anatomy" per 2018 AAST OIS (model-1). 28 (68.3%) had concurrent VCE (model-2). Model-2 had a statistically significant improvement in area under the curve over model-1 in predicting bleeding interventions (0.75 vs 0.72; P =.01). Shattered kidney is associated with high rates of active bleeding, urinary extravasation, and interventions including nephrectomy. The definition of shattered kidney is vague and subjective and our definition might be simpler and more reproducible. Loss of identifiable renal anatomy per the 2018 AAST OIS did not provide better distinction for bleeding control interventions over presence of VCE. [ABSTRACT FROM AUTHOR]

Abstract: A highly sensitive and selective quantitative method to accurately determine testosterone (Te) and 5α-dihydrotestosterone (DHT) in human serum is crucial to the success of Te replacement therapy for hypogonadism. To this end we have developed and validated a semi-automated and relatively high-throughput method in a 96-well plate format using ultra-performance liquid chromatography coupled with tandem mass spectrometry (UPLC/MS/MS) for the simultaneous determination of Te and DHT in human serum. Te and DHT along with the internal standards [2H3]-Te and [2H3]-DHT were extracted from 300μL of human serum by liquid–liquid extraction using methyl tertiary-butyl ether (MTBE), followed by derivatization with 2,3-pyridinedicarboxylic anhydride and solid-phase extraction for sample clean up. A novel chemical derivatization approach using 2,3-pyridinedicarboxylic anhydride was employed to achieve the MS sensitivity and selectivity required for DHT. Baseline separation of Te and DHT derivatives from endogenous steroid derivatives was achieved using UPLC technology on a C18 stationary-phase column with 1.7μm particle size. The validity of using double charcoal-stripped female human serum as surrogate matrix for preparation of calibration standards was demonstrated through standard addition experiments. The method was validated over the concentration ranges of 0.2–40ng/mL for Te and 0.01–2ng/mL for DHT. The validation and study sample analysis results show that the method is rugged, precise, accurate, and well suited to support pharmacokinetic studies where approximately 300 samples can be extracted and analyzed in 1 day. Among working Americans, 69% say that mothers in the workplace are more likely to be passed up for new jobs compared with other employees, and 60% of working Americans stated that career opportunities are given to less qualified employees instead of working mothers who may be more skilled [4]. Abstract: Purpose: To prospectively examine the link between positive and negative drinking expectancies and hazardous alcohol use, and to explore the reciprocal associations between expectancies and hazardous use among female college athletes. Method: Participants were part of a larger (n = 189), ongoing longitudinal study on female athletes’ socialization experiences and health behaviors from an all-women’s college in the Northeastern United States. The sample for the present study were 85 college-aged (17–22 years) female students (Mean age = 19.6) who indicated alcohol use in the past year and provided data at both time points (baseline and one year later). Respondents completed self-report questionnaires on their age, hazardous alcohol use, and expectations about the effects of drinking. Results: Logistic regression analyses showed that positive drinking expectancies predicted increased odds of hazardous alcohol use at baseline and one year later (even after controlling for baseline hazardous use). Positive expectancies accounted for a higher proportion of variance in predicting increased odds of hazardous alcohol use at both time points than did negative expectancies. Bivariate correlational analyses examining the bi-directional associations between expectancies and hazardous alcohol use also revealed some indication that positive expectancies were associated with hazardous alcohol use rather than vice-versa. No such relations emerged for negative expectancies. Conclusions: Findings shed light on the utility of positive expectancies in predicting concurrent and subsequent hazardous alcohol use among female student-athletes. Objective: To find clinical or radiographic factors that are associated with angioembolization failure after high-grade renal trauma.Material and Methods: Patients were selected from the Multi-institutional Genito-Urinary Trauma Study. Included were patients who initially received renal angioembolization after high-grade renal trauma (AAST grades III-V). This cohort was dichotomized into successful or failed angioembolization. Angioembolization was considered a failure if angioembolization was followed by repeat angiography and/or an exploratory laparotomy.Results: A total of 67 patients underwent management initially with angioembolization, with failure in 18 (27%) patients. Those with failed angioembolization had a larger proportion ofgrade IV (72% vs 53%) and grade V (22% vs 12%) renal injuries. A total of 53 patients underwent renal angioembolization and had initial radiographic data for review, with failure in 13 cases. The failed renal angioembolization group had larger perirenal hematoma sizes on the initial trauma scan.Conclusion: Angioembolization after high-grade renal trauma failed in 27% of patients. Failed angioembolization was associated with higher injury grade and a larger perirenal hematoma. Likely these characteristics are associated with high-grade renal trauma that may be less amenable to successful treatment after a single renal angioembolization. [ABSTRACT FROM AUTHOR]

• Visual disturbances are one of the most frequent AEs with crizotinib for NSCLC. • Ophthalmologic changes from baseline were not associated with reported ocular AEs. • No ophthalmologic AEs required permanent treatment discontinuation. • First study evaluating association of ocular AEs with crizotinib and predefined tests. During crizotinib clinical evaluation, visual disturbances, generally of grade 1 severity, were frequently reported adverse events (AE). Consequently, ophthalmologic assessments were included in a patient subgroup enrolled in PROFILE 1001 (NCT00585195), a phase 1, open-label, single-arm trial of crizotinib in patients with advanced non-small-cell lung cancer and are reported here. At least 30 patients were required to undergo ophthalmologic assessments, including: best-corrected visual acuity (BCVA), refractive error, pupil size, slit-lamp anterior segment biomicroscopy, intraocular inflammation, intraocular pressure, retinal fundoscopic exams, fundus photography, ocular characteristics, and optical coherence tomography (OCT). Scheduled assessments included those at baseline, Cycle 1 Day 15, Cycle 3 Day 1 (C3D1), annually during treatment, and end of treatment (28 days after last crizotinib dose). Thirty-three patients completed all required ophthalmologic assessments through C3D1, and 22 (66.7 %) had abnormal findings on ≥1 ophthalmologic test. Clinically important changes were ≥2-line loss in BCVA in 10 patients (30.3 %), >±1.25-diopter change in refractive error in 3 patients (9.1 %), >±2-mm change pupillary diameter change in 3 patients (9.1 %), and >50 μm increase in OCT center point thickness in 7 patients (21.2 %). Three patients (15 %) reported clinically significant abnormalities in anterior segment biomicroscopy (grade 1 cataract [n = 2], grade 1 Visual Impairment [n = 1]). No permanent treatment discontinuations were associated with ophthalmologic findings changes. Twenty-four patients (72.7 %) reported ≥1 ocular all-causality treatment-emergent AE (TEAE); none required dose reduction or permanent discontinuation, but 2 required temporary dosing interruption. Although TEAEs and ophthalmologic findings may not have occurred concurrently, of 24 patients with ≥1 all-causality ocular TEAE, 18/24 (75.0 %) had ≥1 abnormal ophthalmologic finding and 6/24 (25 %) had none; and of 9 patients without an all-causality ocular TEAE, 4/9 (44.4 %) had ≥1 abnormal ophthalmologic finding and 5/9 (55.6 %) had none. Of the 18 patients with ≥1 abnormal ophthalmologic finding, 9 (50 %) had preexisting ocular conditions. During crizotinib treatment, ophthalmologic changes from baseline did not appear to be associated with patient-reported ocular TEAEs. Abnormal ophthalmologic findings occurred in the context of preexisting conditions for a number of patients. No ophthalmologic changes from baseline or ocular all-causality TEAEs required permanent treatment discontinuation. [ABSTRACT FROM AUTHOR]
