Purpose: Complete visual review of prolonged video-EEG recordings at an EMU (Epilepsy Monitoring Unit) is time consuming and can cause problems in times of paucity of educated personnel. In this study we aimed to show non inferiority for electroclinical diagnosis using sampled review in combination with EEG analysis softreferware (P13 software, Persyst Corporation), in comparison to complete visual review.Method: Fifty prolonged video-EEG recordings in adults were prospectively evaluated using sampled visual EEG review in combination with automated detection software of the complete EEG record. Visually assessed samples consisted of one hour during wakefulness, one hour during sleep, half an hour of wakefulness after wake-up and all clinical events marked by the individual and/or nurses. The final electro-clinical diagnosis of this new review approach was compared with the electro-clinical diagnosis after complete visual review as presently used.Results: The electro-clinical diagnosis based on sampled visual review combined with automated detection software did not differ from the diagnosis based on complete visual review. Furthermore, the detection software was able to detect all records containing epileptiform abnormalities and epileptic seizures.Conclusion: Sampled visual review in combination with automated detection using Persyst 13 is non-inferior to complete visual review for electroclinical diagnosis of prolonged video-EEG at an EMU setting, which makes this approach promising. Elderly people and people with epilepsy may need assistance after falling, but may be unable to summon help due to injuries or impairment of consciousness. Several wearable fall detection devices have been developed, but these are not used by all people at risk. We present an automated analysis algorithm for remote detection of high impact falls, based on a physical model of a fall, aiming at universality and robustness. Candidate events are automatically detected and event features are used as classifier input. The algorithm uses vertical velocity and acceleration features from optical flow outputs, corrected for distance from the camera using moving object size estimation. A sound amplitude feature is used to increase detector specificity. We tested the performance and robustness of our trained algorithm using acted data from a public database and real life data with falls resulting from epilepsy and with daily life activities. Applying the trained algorithm to the acted dataset resulted in 90% sensitivity for detection of falls, with 92% specificity. In the real life data, six/nine falls were detected with a specificity of 99.7%; there is a plausible explanation for not detecting each of the falls missed. • An automated non-contact algorithm for detection of central apneas is proposed. • It detects cessation of oscillatory movements of breathing in absence of body motion. • All apnea episodes were detected in the signals from at least one camera angle. • Integrating camera inputs capturing different angles improved detection sensitivity. • >99% specificity was achieved with individual cameras and integrated camera inputs. Central apneas occurring in the aftermath of epileptic seizures may lead to sudden death. Contact-sensors currently used to detect apneas are not always suitable or tolerated. We developed a robust automated non-contact algorithm for real-time detection of central apneas using video cameras. One video recording with simulated apneas and nine with real-life apneas associated with epileptic seizures, each recorded from 3 to 4 angles, were used to develop the algorithm. Videos were preprocessed using optical flow, from which translation, dilatation and shear rates were extracted. Presence of breathing motions was quantified in the time-frequency spectrum by calculating the relative power in the respiratory range (0.1–1 Hz). Sigmoid modulation was calculated over different scales to quantify sigmoid-like drops in respiratory range power. Each sigmoid modulation maximum constitutes a possible apnea event. Two event features were calculated to enable distinction between apnea events and movements: modulation maximum amplitude and total spectral power modulation at the time of the event. An ensemble support vector machine was trained to classify events using a bagging procedure and validated in a leave-one-subject-out cross validation procedure. All apnea episodes were detected in the signals from at least one camera angle. Integrating camera inputs capturing different angles increased overall detection sensitivity (>90%). Overall detection specificity of >99% was achieved with both individual cameras and integrated camera inputs. These results suggest that it is feasible to detect central apneas automatically in video, using this algorithm. Abstract: Introduction: The CMAP scan is a surface EMG method based on the successive activation of motor units. It provides information about reinnervation processes, the number of functional motor units and nerve excitability. The CMAP scan has potential value as a follow-up tool in monitoring disease progression, recovery or aging of the peripheral nerves. In this study, we assessed its interobserver and different-day reproducibility. Methods: Two investigators recorded CMAP scans in ten healthy subjects, each on two different days. Intraclass correlation coefficients (ICCs) and coefficients of variation (CoVs) were calculated for the parameters extracted from the CMAP scan. Results: All CMAP scan parameters had a good different day (ICCs >0.8 and CoVs <15%) and interobserver reproducibility (ICCs >0.7 and CoVs ⩽15%). Different-day reproducibility was better than interobserver reproducibility. Conclusion: CMAP scan test–retest variability is small, suggesting that as a follow-up tool it may be sensitive to fairly small (patho)physiological changes in the studied variables. Objective To determine whether there is a relation between electrically evoked multiplet discharges (MDs) and motor axonal excitability properties. We hypothesized that electrically evoked MDs share their underlying pathophysiological mechanism with fasciculations. Methods High-density surface EMG and motor nerve excitability recordings of the thenar muscles were performed in 22 patients with motor neuron disease (MND) in their differential diagnosis and who were referred for EMG examination. Results Supernormality (hyperexcitable phase following the refractory period) was significantly increased in patients with MDs ( n = 10) compared to patients without MDs ( n = 12) (25.5% vs 17.0%; p = 0.02). Depolarizing threshold electrotonus differed significantly between both groups as well (TEdpeak, 76.6% vs 66.6%, p < 0.01; TEd90-100 ms, 51.7% vs 44.3%, p < 0.01) Conclusions Our findings imply that the same pathophysiological excitability changes are involved in generating MDs and fasciculations. Yet, MDs may be quantified more easily, and may be more specific for abnormal distal excitability than fasciculations, because fasciculations may originate along the motor axon as well as in the neuron cell body. Significance MDs are potentially useful as objective measure of increased distal axonal excitability at individual motor unit level and might complement clinical studies in MND. Abstract: Objective: (a) To relate MRI patterns of brain injury to somatosensory evoked potentials (SEPs), and (b) to determine the prognostic value of SEPs in addition to continuous EEG monitoring (cEEG) and cerebral imaging, in term asphyxiated newborns. Methods: Fifty one consecutive neonates were studied. Survivors were followed for at least 2years. cEEG, started within 24h, was done for ⩾24h and scored. SEPs and MRIs were performed in the first week. Brain injury patterns were classified. Results: Bilaterally abnormal SEPs had a sensitivity of 90% (28/31) and specificity of 85% (17/20) in predicting a poor outcome, defined as death or severe handicap. SEPs were of particular value in predicting outcome in isolated symmetrical white matter injury and predicting the development of hemiparesis in isolated asymmetrical watershed injury. Binary logistic regression analysis revealed a significant relation to outcome separately for cEEG, deep grey matter injury on MRI and SEPs. SEPs provided additional value when added to cEEG and MRI in the model (p =0.034). Conclusions: SEPs are of additional prognostic value after term birth asphyxia. Significance: In certain patterns of postasphyxial neonatal brain injury like asymmetrical watershed lesions and symmetrical white matter injury, EPs are complementary to information obtained from cEEG and MRI for prognostication. Abstract: Following (tracking) individual motor units over time can provide important new insights, both into the relationships among various motor unit (MU) morphological and functional properties and into how these properties are influenced by neuromuscular disorders or interventions. The present study aimed to determine whether high-density surface EMG (HD-sEMG) recordings, which use an array of surface electrodes over a muscle, can increase the yield of MU tracking studies in terms of the number of MUs that can be tracked. For that purpose, four HD-sEMG recording sessions were performed on the thenar muscles of ten healthy subjects. Decomposition of the recorded composite responses yielded a study total of 2849 motor unit action potentials (MUAPs). MUAPs that were found in both of the first two sessions, performed on the same day, were defined as trackable MUAPs. Our results show that 22 (median value; range, 13–34) MUAPs per nerve were trackable, which represented approximately 5% of the total MU population. Of these trackable MUAPs, 16 (11–26) could also be found in one or both of the third and fourth sessions, which were performed between 1 and 13 weeks after the initial studies. Nine (4–18) MUAPs were found in all four sessions. Many of the characteristic MUAP shapes matched well between sessions, even when these sessions were several weeks apart. However, some MUAPs seem very sensitive to changes in arm position or in the muscle’s morphology (e.g., to changes in muscle fiber length due to variable degrees of thumb flexion or extension), particularly those from larger and/or superficial MUs. Standardization is, therefore, essential to detect even small MUAP changes, as may occur with pathology or interventions. If this is accomplished, MU tracking with HD-sEMG may prove to be a powerful tool for a promising type of neurophysiological investigation. [Copyright &y& Elsevier]

Abstract: For early assessment of axonal outgrowth after trauma, the nerve is surgically exposed to enable compound nerve action potential (CNAP) recordings across the lesion site. Near-nerve neurography, with needle electrodes placed transcutaneously near the nerve, could be a minimally invasive alternative if the needle placement procedure and low reproducibility are improved. We developed ultrasound-guided near-nerve neurography, which facilitates needle placement, and assessed its potential for evaluating nerve regeneration. Measurements were performed at varying times after crush lesion of the peroneal nerve of 25 rabbits. To test if ultrasound-guided near-nerve signals could be measured prior to muscle reinnervation, they were compared with recordings of compound muscle action potentials. A comparison with conventional intra-operative CNAP recordings was made by measuring nerve signal amplitude with both techniques and by assessing reproducibility. In all cases where intra-operative signals could be measured, near-nerve signals were also detected. Compound nerve activity could be recorded after 5 weeks, whereas compound muscle activity appeared after approximately 8 weeks. Reproducibility was slightly better for near-nerve than for intra-operative recordings. We conclude that ultrasound-guided near-nerve neurography is able to assess nerve regeneration well before compound muscle activity can be detected.
