Abstract: Proliferative verrucous leukoplakia (PVL) is a rare and refractory form of oral mucosal leukoplakia of unknown origin, characterized by high rates of malignant transformation. Different diagnostic criteria, terminologies, and therapeutic approaches have been proposed since the first report in 1985. There remains no general agreement regarding the clinical and histological diagnosis, prevention, and correct management of this disease. This retrospective study investigated 48 patients affected by PVL showing at least one malignant transformation and followed up at 2-month intervals. Twenty-five were female (52.1%) and 23 (47.9%) were male; their median age was 67 years (range 40–93 years). Follow-up ranged from 18 to 240 months. Clinical examination included the use of Lugol's solution to prevent clinical underestimation of the margins and toluidine blue for suspicious areas. Surgical excision by scalpel was the preferred treatment for suspicious lesions, with only five carcinomas surgically removed by diode laser and two by CO 2 laser. All specimens were accurately mapped after formalin fixation. Fifteen patients (31.3%; 10 female, five male) developed one oral carcinoma, while 33 (68.7%) developed two or more primary tumours (range 3–12). Only four patients (8.3%), who developed between 2 and 8 oral squamous cell carcinomas (OSCCs), died of tumour-related causes. The pre-surgical clinical workup, subsequent surgical treatment, and follow-up are key to success for patients affected by PVL with malignant transformation into stage 1 OSCC and/or verrucous carcinoma, leading to a high overall survival rate. 23. Sleep and emotional processing.

Abstract: A growing body of literature suggests that sleep plays a critical role in emotional processing. This review aims at synthesizing current evidence on the role of sleep and sleep loss in the modulation of emotional reactivity, emotional memory formation, empathic behavior, fear conditioning, threat generalization and extinction memory. Behavioral and neurophysiological evidence suggesting that rapid-eye movement (REM) sleep plays an important role in emotional processing is also discussed. Furthermore, we examine the relations between sleep and emotions by reviewing the functional neuroimaging studies that elucidated the brain mechanisms underlying these relations. It is shown that sleep supports the formation of emotional episodic memories throughout all the stages that compose memory processing. On the contrary, sleep loss deteriorates both the encoding of emotional information and the emotional memory consolidation processes. Research is also progressively providing new insights into the protective role of sleep in human emotional homeostasis and regulation, promoting adaptive next-day emotional reactivity. In this respect, evidence converges in indicating that lack of sleep significantly influences emotional reactivity. Moreover, notwithstanding some contradictory findings, the processing of emotionally salient information could mainly benefit from REM sleep. 24. Does partial adhesive preparation design and finish line depth influence trueness and precision of intraoral scanners?

Abstract: Intraoral scanners (IOSs) are widely used for partial-coverage adhesive restorations, but data on their performance in such preparations with complex geometries are sparse. The purpose of this in vitro study was to investigate whether the partial-coverage adhesive preparation design and finish line depth affect the trueness and precision of different IOSs. Seven partial-coverage adhesive preparation designs (4 different onlays, 2 endocrowns, and 1 occlusal veneer) were tested on copies of the same tooth placed in a typodont mounted on a mannequin. Each preparation was scanned 10 times with 6 different IOSs (total 420 scans) under the same light conditions. Trueness and precision, defined according to the International Organization for Standardization (ISO) 5725-1 standard, were analyzed with a best-fit algorithm by superimposition. The obtained data were analyzed by a 2-way analysis of variance to examine the effects of partial-coverage adhesive preparation design, IOS, and their interactions (α=.05). Significant differences were found among different preparation designs and IOSs (P <.05) in terms of both trueness and precision. Significant differences were also found among mean positive and negative values (P <.05). Moreover, crosslinks observed between the preparation area and the adjacent teeth correlated with the finish line depth. Complex partial adhesive preparation designs affect the trueness and precision of IOSs, resulting in significant differences among them. Abstract: Hereditary haemorrhagic telangiectasia (HHT) is rare, and characterised by vascular dysplasia that leads to various symptoms including visceral arteriovenous malformations and mucocutaneous telangiectatic lesions. Our aim was to describe the clinical features and options for the treatment of multiple oral lesions, and to illustrate the efficacy of the diode laser in the treatment of early (<2 mm) and advanced lesions (2 mm or more). We report 24 patients with 1200 oral telangiectatic lesions, which were often associated with regular bleeding (from monthly to daily), superinfection, pain, and swelling, and treated with multiple sessions of laser according to the number and size of the lesions. Early lesions were treated with a single laser impulse in ultrapulsed mode, and advanced lesions with repeated laser impulses in pulsed mode (t-on 200 ms/t-off 500 ms), at a power of 8 W. Early lesions healed completely after laser photocoagulation with no operative or postoperative complications, while advanced lesions improved with a remarkable reduction in size but more discomfort. Protective occlusal plates were sometimes used to reduce the incidence of new lesions caused by dental trauma. The treatment of oral telangiectatic lesions is still being debated, and it is important to improve quality of life for patients. Abstract: The four LEP collaborations, ALEPH, DELPHI, L3 and OPAL, have collected a total of 2461 pb−1 of e+ e− collision data at centre-of-mass energies between189 and 209 GeV. The data are used to search for the Standard Model Higgs boson. The search results of the four collaborations are combined and examined in a likelihood test for their consistency with two hypotheses: the background hypothesis and the signal plus background hypothesis. The corresponding confidences have been computed as functions of the hypothetical Higgs boson mass. A lower bound of114.4 GeV/c2 is established, at the 95% confidence level, on the mass of the Standard Model Higgs boson. The LEP data are also used to set upper bounds on the HZZ coupling for various assumptions concerning the decay of the Higgs boson.

27. Toward a Better Understanding of Market Potentials for Vegan Food. A Choice Experiment for the Analysis of Breadsticks Preferences.

Abstract: Recently, vegan food has received increased attention from Italian consumers. Despite this fact, it has rarely been the subject of analysis. Our study focuses on consumer preferences for breadsticks in North-Eastern Italy. We applied a choice experiment where a hypothetical market was designed to analyze five characteristics of breadsticks (country of origin, vegan product certification, production method, type of flour, and price). We collected data by interviewing 487 consumers and analyzed them by means of the random parameter logit model. Results suggest that 8% of respondents are willing to pay a premium price for vegan breadsticks and that there is the opportunity to develop local chains for vegan niche markets. 28. PHACES syndrome: Diode laser photocoagulation of intraoral hemangiomas in six young patients.

Abstract: INTRODUCTION: The acronym PHACES describes the association of posterior fossa malformations, facial hemangiomas, arterial anomalies (cardiovascular or cerebrovascular), coarctation of the aorta and cardiac defects, eye abnormalities, and sternal or ventral defects. In this study we report on 6 patients affected by the PHACES syndrome and showing 34 intraoral hemangiomas (IH), treated by diode laser photocoagulation (DLP). CASE PRESENTATION: IH appeared as red-bluish soft masses, smooth or lobulated, from a few millimetre to several centimetres in size, covered by intact mucosa and blanching on pressure. IHs were treated by DLP with 320μm fibres at a wavelength of 800±10 nm. The diode laser techniques applied were: Transmucosal DLP (DLTP), a no-contact technique in which laser energy is delivered by a flexible optic quartz fiber, which is kept 2-3mm apart from the lesion, and Intralesional DLP (DLIP), in which the fibre is introduced into the lesion through a transmucosal access. DLTP was used for 20 flat, superficial IHs and, after a variable number of laser sessions (average = 3) depending on the size of the lesion, 65% completely regressed, while in the remaining 35% shrinkage of the lesion was achieved with minor and few complications. The remaining 14 deep/multi-lobulated IHs were treated by DLIP, resulting in complete regression of 79% of them. CONCLUSIONS: DLP techniques are an effective and minimally invasive procedure for IH in patients with PHACES, in consideration of the multiple lesions to treat, of the necessity of multiple interventions and the higher compliance of the patients. Abstract: Background: The surgical management of hyperhidrosis is controversial. Robotic surgical systems with their high-definition magnified 3-dimensional view and increased maneuverability in a confined space may facilitate the technique of selective sympathectomy (ramicotomy). We present a case series of patients undergoing selective postganglionic thoracic sympathectomy using robotic technology. Methods: This study is a case series analysis of patients who underwent selective postganglionic thoracic sympathectomy from July 2006 to November 2011. The operation was performed on a video-assisted thoracoscopic surgery (VATS) platform. The robot was used for pleural dissection and division of the postganglionic sympathetic fibers and the communicating rami. The success of sympathectomy was assessed by intraoperative temperature measurement of the ipsilateral upper extremity, patient interviews, and scoring of the symptomatic nature of hyperhidrosis based on the Hyperhidrosis Disease Severity Scale. Results: There were 110 sympathectomies performed in 55 patients (25 men, 30 women). Simultaneous bilateral sympathectomy was performed in all patients. Median age was 28 years (range, 16 to 65 years). There was no conversion to thoracotomy. Complications were minor and were seen in 5 of 55 patients (9%). There were no deaths. Median hospital stay was 1 day (range, 1 to 4 days). Of the 55 patients, 53 (96%) had sustained relief of their hyperhidrosis at a median follow-up of 24 months (range, 3 to 36 months), and compensatory sweating was seen in 4 patients (7.2%). Conclusions: Robotic thoracoscopic selective sympathectomy is an effective, feasible, and safe procedure with excellent relief of hyperhidrosis and low rates of compensatory sweating and complications. These starting materials were modified by grinding at different times and then the degree of reactivity of the obtained products was verified by studying the changes induced by the mechanical treatment on the crystalline structure and by determining the specific surface. Moreover the solubility and kind of alkaline solutions classically used for the production of geopolymeric materials (NaOH and KOH) was investigated together with other aqueous solutions at variable pH. The mechano-chemical activation process can achieve the result to reduce the use of alkaline solutions as activators of the starting raw materials in the production of consolidating solution for monument stone materials and to utilise natural materials in the production of restoration mortars compatible with the substrates to be restored. Abstract: Abstract: Background: Surveillance in hereditary non-polyposis colorectal cancer (HNPCC) family members recommends baseline colonoscopy starting at age 20 and then surveillance colonoscopy every 1–2 years. Aims: To verify adherence to the guidelines for HNPCC family members enrolled in endoscopic surveillance. Methods: Data regarding 11 HNPCC families was retrieved from our database. Excluding 11 probands, 106 family members were evaluated and 40 underwent surveillance. Results: At baseline colonoscopy, 7 colorectal cancers (CRC), 14 polyps (PO) [1 inflammatory, 2 hyperplastic, 10 adenomas with low grade dysplasia (LGD-AD) and 1 adenoma with high-grade dysplasia (HGD-AD)] were diagnosed in sixteen individuals. Twenty-eight HNPCC family members underwent endoscopic surveillance, with a total of 94 surveillance colonoscopies. Of these, 45 were positive (4 CRC, 3 inflammatory PO, 34 hyperplastic PO, 21 LGD-AD and 5 HGD-AD). Mean time between two consecutive surveillance colonoscopies was 24.6 months (range 4–168). Median time to first positive surveillance colonoscopy was 84 months for HNPCC family members with negative baseline colonoscopy, and 60 months for those with positive baseline colonoscopy (p =0.21). Conclusions: Our data suggests that surveillance colonoscopy every 2 years is adequate to diagnose advanced lesions in HNPCC family members, and improves their compliance with surveillance. Abstract: Abstract: This article discusses the factors affecting landscape perception, with particular reference to the agrarian historical landscapes of the plains in the Veneto region of Italy. The main historical agrarian landscapes were identified, and their aesthetic value was compared with that of more modern landscapes. Surveys were conducted on three groups of inhabitants: children, university students and adults and three approaches were used to analyze the landscape preferences. First, the appreciation of each landscape was compared; second the effects of the various elements on landscape preferences were determined by means of photomontages; and finally, the visual impact of these elements, in improving or worsening the aesthetic quality of the landscape, was quantified using a statistical model. This made it possible to determine if, and to what extent, some aesthetic and cultural values are shared among the three generations of individuals included in this study. The results have demonstrated that some elements of the historical landscapes (villas and traditional farm buildings in particular) can increase the appreciation for the agrarian landscape in the three groups of interviewees. However, not all historical agrarian landscapes are appreciated. This suggests a need for landscape education programmes, especially for the young, so that future generations will favour interventions in the preservation of the historical and cultural agrarian landscapes. Abstract: Abstract: This prospective study aims to evaluate the impact of the excimer laser technology as the first-line endovascular treatment of critical limb ischaemia (CLI) in diabetic patients. The protocol allowed the use of laser ablation of obstructive lesions when conventional endoluminal guidewire crossing of the plaque was unsuccessful. We extrapolate the data of consecutive patients treated, who completed at least 12 months of follow-up, extending the observation to a 26-month time frame. During this period, 67 diabetic patients with CLI were brought to the Cath Lab for ‘operative angioplasty’ and to be treated with endovascular techniques. Of the 67 cases, laser was used on 35 patients to treat 51 lesions. All patients had type C or D occlusive lesions, according to the TACS II classification, showing a single type D plaque or multiple tandem C/D occlusive plaques ranging from 4 to 23cm in length. The immediate clinical success, defined as restored direct arterial flow to the foot, was 88.2%. The lesions were successfully crossed by laser in 45 out of 51 attempts. Stents were required in 25% of the patients with 21% lesions. Patency rates were assessed using the Kaplan–Meier survival curves. The patency rates of the successfully treated lesions (freedom from target lesion revascularisation) were 96.6% at 12 months and 82.7% at 24 months. Limb-salvage rate at 12 and 24 months were 100% and 94%, respectively. Our study showed that the excimer laser-assisted angioplasty, when feasible, is effective in granting event-free survival in CLI patients with diabetes, and that endoluminal-driven atherectomy allows long-term success in reducing the need of stents in the lower limb arteries. This study describes a hybrid minimally invasive lobectomy procedure consisting of two phases: robotic vascular, hilar, and mediastinal dissection, and then VATS lobectomy. Methods: Over a 54-month period, 100 consecutive patients with stage I and II (T1 or T2N0, and T1 or T2N1) lung cancer (42 men, 58 women; mean age 65 ± 8 years) underwent robotic VATS lobectomy. Results: Lobectomies were right upper (29), right middle (7), right lower (17), left upper (31), and left lower (16). Mean operating room time was 216 ± 27 minutes. Tumor type was adenocarcinoma (57), squamous cell carcinoma (25), 7 adenosquamous carcinoma (7), bronchoalveolar (3), large cell (1), poorly differentiated (3), carcinoid (2), mucoepidermoid (1), spindle cell (1). Pathologic upstaging was noted in 17 patients (10 to stage IIB, 7 to stage IIIA). There was no emergent conversion to a thoracotomy. Median hospitalization was 4 days. Complications included atrial fibrillation (13), atelectasis (5), prolonged air leak (4), pleural effusion (3), pulmonary embolus (3), incisional bleeding (1), hydropneumothorax (1), dural leak (1), liver failure (1), pneumonia (1), respiratory failure (1), and cardiopulmonary arrest (1). There was no intraoperative death. Postoperative mortality was 3%. There were no deaths among the last 80 patients. At a median follow-up of 32 months (range, 1 to 59), 1 patient (1%) died of his cancer, 6 (6%) had distant metastases, and 2 (2%) had a second lung primary cancer. There was no local recurrence. Methods: During a 41-month-period, 61 patients (27 men, 34 women; mean age, 68.2 years) underwent a robot-assisted video-assisted thoracic surgery lobectomy and complete mediastinal nodal dissection for early-stage lung cancer (stages I, II). Results: Distribution of lobectomies was right upper lobe 14, right middle lobe 6, right lower lobe 9, left upper lobe 21, and left lower lobe 11. Operative times ranged from 3 to 6 hours (median, 4). There were 34 adenocarcinoma, 14 squamous cell carcinoma, 6 adenosquamous, 1 large cell, 2 bronchoalveolar, 2 poorly differentiated cancers, and 2 carcinoid tumors. Pathologic upstaging was noted in 10 patients (8 to IIb, 2 to IIIa). There were no emergent conversions to a thoracotomy. Complications included atrial fibrillation (4), atelectasis (4), prolonged air leak (2), pleural effusion (2), hydropneumothorax (1), and incisional bleeding (1). Mortality was 4.9%. Median hospitalization was 4 days. Follow-up was complete in 54 patients (88%). At a mean follow-up of 28 months, all patients were alive, and 4 had distant metastases. There was no local recurrence. Conclusions: Robot-assisted vascular and nodal dissection during video-assisted thoracic surgery lobectomy for early-stage lung cancer is feasible. 38. Can a concentrated cranberry extract prevent recurrent urinary tract infections in women? A pilot study.

Abstract: Abstract: Background: Urinary tract infections (UTIs) are extremely prevalent and despite treatment with antibiotics, reoccurrences are common causing frustration in the patient and the potential for developing antibiotic resistance. The use of cranberry products to prevent UTIs has recently become popular and more clinical studies are needed to explore this use. Objective: This open label pilot study examined the ability of a concentrated cranberry preparation to prevent UTIs in women with a history of recurrent infections. Subjects: Women between the ages of 25 and 70 years old were included with a history of a minimum of 6 UTIs in the proceeding year. Intervention: The women took one capsule twice daily for 12 weeks containing 200mg of a concentrated cranberry extract standardized to 30% phenolics. Design: A questionnaire was used initially to determine the patient''s medical history and they were asked at monthly intervals if any of the information had changed. All of the women in the study had urinalysis within 24h before starting on the study preparation and once a month after that for 4 months. Subjects were followed-up approximately 2 years later. Results: All 12 subjects participated in the 12-week study and were available for follow up 2 years later. During the study none of the women had a UTI. No adverse events were reported. Two years later, eight of the women who continue to take cranberry, continue to be free from UTIs. Conclusion: A cranberry preparation with a high phenolic content may completely prevent UTIs in women who are subject to recurrent infections. [Copyright &y& Elsevier]

Issues include operative time, lymph node yield, conversion to thoracotomy, resource utilization, recurrence, complications, and survival.: MethodsFrom January 1995 to December 2001, 179 patients underwent VATS lobectomy for preoperative stage I lung cancer (T1N0, 118 patients; T2N0, 61 patients). Mean age was 64.34 years (range, 38 to 87); 91 were female and 88 were male. Contraindications to VATS lobectomy included any suggestion of hilar, endobronchial, or central lesions. Video-assisted thoracic surgery lobectomy was performed using three ports, partial anatomic hilar dissection, and mediastinal node dissection.: ResultsDistribution of lobectomies was as follows: left upper lobe, 50 patients; left lower lobe, 27 patients; right upper lobe, 33 patients; right upper and right middle lobe, 29 patients; right middle lobe, 9 patients; right lower lobe, 30 patients; right middle lobe and right lower lobe, 1 patient. Mean operative time was 75 ± 6 minutes. Mean lymph node yield was 11 ± 5 nodes. Pathologic upstaging was noted in 14 of the 179 patients (7.8%). Mean hospitalization was 4.1 days (range, 2 days to 4 months). There were no conversions to thoracotomy and there was 1 death (1 of 179, 0.05%). Complications included air leak in 24 of 179 (13.4%), subcutaneous emphysema in 4 of 179 (2.2%), pneumonia in 10 of 179 (5.6%), wound infection in 5 of 179 (2.8%), respiratory failure in 3 of 179 (1.7%), pulmonary embolism in 2 of 179 (1.1%), and myocardial infarction in 1 of 179 (0.5%). At a mean follow-up of 37 months, local recurrence rate was 0.013 per person per year. Actuarial recurrence-free survival was 88% and 85% at 36 and 60 months respectively.: ConclusionsFor carefully selected patients VATS lobectomy for early stage lung cancer is a safe and effective strategy. The peak incidence is in the 5-7th decades and they can be the first sign of an occult cancer or manifest during the follow-up of a patient with a known primary tumour. We describe a case of metastasis from unknown lung adenocarcinoma occurring in the maxilla, around dental implants, causing recurrent implants failure. A 62 year-old male without relevant clinical history was referred for dental implant failure in the left maxilla. New dental implants were positioned in the same maxillary area, but an additional implant failure occurred one month later; implants removal with an accurate bone curettage was performed and the surgical sample sent for histological examination. Unexpectedly, small foci of adenocarcinoma were found, with spiculae of medullary bone, prominent inflammatory changes and bacterial deposits. At immunohistochemistry, the tumour positively stained for CK7+ and TTF1+ but not for CK20 and PSA. Additional clinico-radiological investigations revealed a primary tumour of the lung, subsequently characterized as acinar adenocarcinoma. Metastases to the oro-facial tissues can involve the oral mucosa, jawbones and salivary glands; lung, kidney, prostate and colon-rectum in males, uterus, breast, ovary and lungs in females are the most frequent primary localisations. It is accepted that metastatic neoplasms in the oro-facial region show high predilection for sites with peculiar clinical conditions, such as the parodontal inflammation or edentulous individuals bearing prosthesis. The molar and premolar regions of the jawbones and the post-extraction sites frequently are involved; this was related to the rich vascularisation and high bone marrow content and /or to re-organisation of the blood cloth of such sites. This report highlights the importance of histological examination of gingival-parodontal inflamed tissues when plaque and calculus accumulations have been excluded as primary causes. 41. MICROINVASIVE PROTOCOL FOR PREOPERATIVE DIAGNOSIS OF MINOR SALIVAR GLANDS MEC ALLOWING TARGETED SURGICAL THERAPY.

Abstract: Mucoepidermoid carcinoma (MEC) is the most common intraoral malignancy and is mainly located between hard and soft palate. Its surgical therapy could be difficult and depends on location, dimension, infiltration, and histologic grading. The aim is to report the importance of a clinicopathologic protocol for microinvasive preoperative diagnosis, grading, and staging of MEC to sketch out a targeted surgical therapy in intraoral difficult locations. In 64 cases of intraoral MEC treated between 2000 and 2019 at the Oro-maxillo-facial Surgery Unit of Policlinico-University-Hospital of Bari (Italy), the following preoperative protocol was performed: clinical examination, high-definition intraoral ultrasonography, radiological assessment (rxOPT, computed tomography/magnetic resonance imaging), microinvasive biopsy with fine-needle aspiration biopsy/fine-needle aspiration cytology with HeE, PAS+/−Alcian stains, and immunohistochemical panel (high-/low-molecular-weight cytokeratins, calponin, smooth muscle actin, S-100, Ki-67). Surgery was targeted for each patient and guided from histopathologic data and consisted of conservative excision with wide margins for low-grade MEC and demolitive resection with neck dissection for high-grade MEC. Application of our protocol gave a 100% preoperative histologic diagnosis and grading. Forty-one cases were low-grade MEC (29 female and 12 male patients, mean age = 37; site: 33 palate, 4 cheek, 2 lip, and 2 tongue); 23 were high-grade MEC (16 female and 7 male patients, mean age = 41; site: 19 palate, 1 cheek, 2 lip, and 1 tongue), and 4 were clear cells. Preoperative fine-needle aspiration biopsy/fine-needle aspiration cytology for histologic diagnosis of MEC is mandatory in order to make decisions on the type of surgical treatment to avoid over- or undertreatment. Abstract: Purpose: Traditional 3-port video-assisted thoracoscopic surgery (VATS) in a patient who is awake has been proposed as a breakthrough in the direction of fast tracking patients through routine thoracic surgical procedures. We wanted to explore the possibility of further reducing surgical invasiveness by resecting a peripheral pulmonary nodule with single-access (uniportal) VATS in an awake, nonintubated, nonventilated patient, with selective occlusion of the tributary lobar bronchus. Description: A 47-year-old woman with bilateral peripheral nodules underwent uniportal VATS wedge resection of an undetermined nodule in the right middle lobe. The patient was awake and under mild sedation for the entire procedure. Single-shot epidural regional anesthesia was administered. Under guidance provided by a reusable, portable flexible bronchoscope, a Fogarty balloon was positioned to occlude the right middle lobe bronchus to facilitate collapse of the targeted parenchyma. At the end of the procedure, the chest drain was connected to a portable vacuum system delivering autonomous suction. Evaluation: Awake uniportal VATS resection of peripheral nodules in selected patients is feasible and appears to be safe. Available technology may enable further reduction of costs related to length of hospitalization. Conclusions: The concept of ambulatory thoracic surgery may further evolve by utilizing uniportal VATS in an awake patient to solve the often-challenging diagnostic dilemmas represented by undetermined lung lesions. Abstract: • General and stress relief preferences seem to be relatively consistent. • Presence of people in green spaces is a very important feature for visitor preferences. • People and presence of manmade elements are positive features for perceived safety. Public urban green spaces are crucial for citizens' wellbeing and are an important part of daily life in cities. To maximize their benefits to quality of life a thorough knowledge of citizens' preferences and preference heterogeneity is crucial in the planning and design of urban green spaces. This study investigated visitors' perception of typical green spaces, with a focus on vegetation structure and the presence of typical historic city walls, as well as preferences within the context of perceived stress and safety. We conducted this study in the historic city of Padua in north-eastern Italy. In 2017, face-to-face interviews of citizens were held and choice sets, based on modified images of different green space scenarios, were used to test users' preferences connected to both stress relief and safety perception. The study highlighted that general, stress relief and safety perception related preferences of the respondents depend on different site characteristics. Respondents preferred a complex but not too wild scenario with sparse trees and aesthetically appealing features such as colourful flowers. Historic walls had a negative effect on general preferences. While general preferences were very similar to stress relief preferences, preferences within the context of safety differed for some attributes. It seems that the vegetation structure and the presence of features linked to human recreational uses are important factors in planning and designing urban green spaces. 44. Brown tumors of the oral cavity: presentation of 4 new cases and a systematic literature review.

Abstract: Objective: The aim of this study was to obtain data from a review of cases of brown tumors (BT), which are benign lesions of bone characterized by giant cells that arise during hyperparathyroidism (HPTH). BTs may affect the maxillofacial area and manifest as a brownish, slow-growing swelling causing difficulty in the differential diagnosis.Study Design: We present data from 4 new cases of oral BTs based on a systematic literature review conducted by searching EMBASE, Medline, and CENTRAL databases, according to the PRISMA guidelines. Only articles in English were considered. Individual patient data were analyzed to identify risk factors for multiple or extraoral maxillofacial BTs.Results: In total, 167 cases (163 from 136 articles and 4 new cases; mean age 36.6 years; male-to-female ratio 1:2) were retrieved. The onset of extraoral maxillofacial BTs (odds ratio [OR] 176.3; 95% confidence interval [CI] 18.7-1657.8; P < .05) and maxillary BTs (OR 17.5; 95% CI 6.0-50.8; P < .05) were the risk factors for multiple oral BTs, whereas the presence of a BT in the mandible (OR 0.01; 95% CI 0.001-0.1; P < .05) was a negative predictor for the presence of other extraoral maxillofacial BTs.Conclusions: The results of this systematic review suggested that the mandible is the most frequent oral location of BTs. 45. MULTIFOCAL AMELANOTIC MELANOMA OF THE HEAD AND NECK: A CHALLENGING CASE.

Abstract: Primary malignant melanoma evolves from melanocytic precursors via the formation of intermediate lesion of varying stability. Less than 1% of all malignant melanomas arise in the head and neck area, the anterior maxilla and alveolar mucosa being the most frequently affected sites. Males and females are equally affected, with an age range between adolescence and senescence. The prognosis is usually poor, with a 5-years survival rate of 30-35% and a median survival of 36 months. Several cases of primary malignant melanoma of the head and the neck area have been reported in the literature but in most cases no clear evidence was shown whether such lesions were primary or metastatic in origin. We present a case of malignant melanoma in a 50-years old male, who complained for a rapidly growing maxillary nodular lesion, involving the ethmoid sinus and the orbital base. Histopathological intraoperative examination revealed a poorly differentiated malignancy, with spindle-shaped cells showing prominent nucleoli. Subsequent immunohistochemical stains highlighted pan-CK (dot-like) and S100 protein positivity but HMB-45 and melan-A were negative, supporting the diagnosis of malignant melanoma. The tumour was treated by en-block resection with mapping-margins and additional histopathological examination showed an intra-mucosal hyper-melanotic lesion, consistent with an acral lentiginous-type melanoma, which was considered the primary neoplastic focus. Primary malignant melanomas in the oral cavity are rare and usually asymtomatic at early stages, thus leading to delayed diagnosis. This must rely upon accurate histopathological and extensive immunohistochemical evaluation as the morphological features often are misleading or non-specific. It is worth to emphasise that melanocyte-specific antigens (Melan-A and HMB-45) frequently are negative in such neoplasm and unexpected cytokeratin positivity may occur, which may result in an inappropriate diagnosis.
