Abstract

Purpose: To evaluate the impact of salvage locoregional therapy (salvage-LT) on survival of hepatocellular carcinoma (HCC) patients presenting with intrahepatic tumor progression following radiotherapy. Methods: This single-institution retrospective analysis included consecutive HCC patients having intrahepatic tumor progression following radiotherapy during 2015–2019. Overall survival (OS) was calculated from the date of intrahepatic tumor progression after initial radiotherapy by using the Kaplan–Meier method. Log-rank tests and Cox regression models were used for univariable and multivariable analyses. An inverse probability weighting was used to estimate treatment effect of salvage-LT considering confounding factors. Results: A total of 123 patients (mean age ± SD, 70 years ± 10; 97 men) were evaluated. Among those, 35 patients underwent 59 sessions of salvage-LT, including transarterial embolization/chemoembolization (n = 33), ablation (n = 11), selective internal radiotherapy (n = 7), and external beam radiotherapy (n = 8). At a median follow-up of 15.1 months (range, 3.4–54.5 months), the median OS was 23.3 months in patients who received salvage-LT and 6.6 months who did not. At multivariate analysis, ECOG performance status, Child–Pugh class, albumin-bilirubin grade, extrahepatic disease, and lack of salvage-LT were independent predictors of worse OS. After inverse probability weighting, salvage-LT was associated with a survival benefit of 8.9 months (95% CI: 1.1, 16.7 months; p = 0.03). Conclusions: Salvage locoregional therapy is associated with increased survival in HCC patients suffering from intrahepatic tumor progression following initial radiotherapy. [ABSTRACT FROM AUTHOR]

Abstract

TiNiCuNb shape memory alloys are a promising new class of materials with the potential to be applied as elastocaloric components. However, the mechanical processing of these alloys remains a challenge and new insights on this topic must enlighten the knowledge about this system. In this work, the effects of hot rolling on Ti 46 Ni 38 Cu 10 Nb 6 alloy were investigated. The results showed that hot rolling leads to the increase of martensitic transformation temperatures and the formation of a matrix containing both B2 and B19 phases. The coarsening of the lamellar eutectic constituent was observed, and part of the β-Nb phase precipitated into the matrix after being dissolved due to hot work. Microstructural aspects and ultra-microhardness measurements suggest that dynamic recrystallization occurred during hot rolling. • Hot rolling increases martensitic transformation temperatures of NiTiCuNb alloys. • B19 is present in the hot-rolled alloy. • β-Nb is partially dissolved into the matrix during hot rolling. • Dynamic recrystallization occurs due to hot rolling. Abstract

This study aimed to compare different methods to assess body fat (BF). We hypothesized that bioelectrical impedance analysis (BIA) or anthropometry may be used to estimate BF in prefrail older women, equivalently to dual-energy X-ray absorptiometry (DXA). The cross-sectional study included 72 prefrail community-dwelling older women (71.13 ± 4.65 years old; body mass index [BMI] 28.89 ± 4.23 kg/m2). The BF percentage (%BF) was estimated using anthropometry with the Durnin & Womersley (D&W) and Petroski's predictive equations, BIA with 2 Baumgartner predictive equations (BIA 1 and BIA 2), and DXA. All methods differed significantly from DXA according to assessments using repeated measures ANOVA and pairwise comparisons. The mean %BF varied between 39.99 ± 3.42% (D&W) and 43.93 ± 5.06% (DXA). Multiple regression analysis with age and BMI as covariates showed positive correlations (R2 = 0.91) in models with D&W equation and BMI, and with BIA 2 and BMI; however, BMI explained more of the model (71%) than the equations. Furthermore, Bland-Altman test revealed a proportional bias for D&W and for BIA 2, with underestimation of BF varying across different %BF values. Petroski's skinfold equation showed a positive correlation on linear regression (R2 = 0.74) and no proportional bias; however, Bland-Altman analysis revealed high limits of agreement (-13.6 to -0.05), thus compromising clinical application. To conclude, compared with DXA, all the equations tested showed a high disagreement and wide limits of agreement, restricting their use in clinical practice to estimate the BF in prefrail older women. Abstract

Two chromium removal experiments were performed in bioreactors with and without a magnetic field under the same conditions. The release of the chromium present in the biomass was tested in two experiments one with the initial pH of the medium and one with pH 4.0. The objective was to remove Cr(VI) and total Cr from the effluent, this was carried out by placing biological treatments of synthetic effluent contaminated with 100 mg/L of Cr(VI) in a bioreactor with neodymium magnets that applied a magnetic field (intensity 85.4 mT) to the mixed culture. The removal of Cr(VI) was approximately 100.0% for the bioreactor with a magnetic field and 93.3% for the bioreactor without a magnetic field for 9 hr of recirculation of the synthetic effluent by the bioreactor. The removal of total Cr was 61.6% and 48.4%, with and without a magnetic field, respectively, for 24 hr. The desorption of Cr(VI) in the synthetic effluent was 0.05 mg/L, which is below the limit established by Brazilian legislation (0.1 mg/L) for the discharge of effluent containing Cr(VI) into bodies of water. The results obtained for the removal of chromium in synthetic effluent suggested that there was no significant influence on the viable cell count of the mixed culture. The desorption of Cr(VI) in synthetic effluent after bioadsorption of chromium by the mixed culture in the process of removal of chromium in bioreactors with and without a magnetic field was not significant in either of the experiments with different initial pHs. 49. Hepatitis B prophylaxis in newborns: A cross-sectional study of 7-year attendance in a public hospital of Brazil.

• Study region is Brazil, with higher endemic of HBV. • Screening consisted of >12,000 HBsAg tests in 7 years (7,763 pregnant women). • Medical records of 75% newborn of HBsAg+ mothers were without prophylaxis records. • Only 14% of children of HBsAg-positive mothers had follow-up postimmunization. Children with vertically transmitted hepatitis B virus develop chronic infection up to 90% of the time. This study aimed to verify the prevalence of hepatitis B surface antigen (HBsAg) in pregnant patients treated in a Brazilian public hospital and analyze the prophylactic measures in newborns. A cross-sectional study was conducted by collcting data in the electronic charts of patients who attended the obstetric and maternity departments, from January 1, 2010, to December 31, 2016, and evaluating the results of pregnant women's HBsAg, prophylaxis in newborns, and clinical follow-up. The data were tabulated and analyzed using Microsoft Excel software. Among the 7,763 participating patients, 109 were reactive to HBsAg, and 3 were indeterminate. However, only 28 had correct information on HBV prophylaxis with the parturient and newborn in the chart, and only 16 completed the follow-up. Most of the HBsAg-positive pregnant women (75%) did not have prophylactic information in the charts, and almost 50% of the pregnant women and newborns who had appropriate prophylaxis did not return for medical follow-up. Failure of prophylaxis can promote vertical/perinatal transmission of hepatitis B virus in newborns of mothers who are HBsAg positive. [ABSTRACT FROM AUTHOR]

Abstract

Soybean Protein Isolate (SPI) is a by-product from soybean oil industries with good nutritional and functional properties. Denaturation of hydrated SPI can change its properties, allowing the formation of gel-like particles, which can be used to stabilize emulsions without addition of surfactants. SPI microgel particles were produced by denaturation of hydrated SPI, followed by high pressure homogenization or sonication, with different NaCl or NaF contents, aiming the formation of small particles capable to stabilize O/W emulsions and acting as wall material for microencapsulation of soybean oil by spray drying. The presence of NaF in the suspensions decreased the charge intensity of SPI microgels, leading to formation of significantly bigger SPI microgel particles. Based on Creaming Index (CI), all the emulsions were stable for, at least, 21 days at room temperature. The presence of salt affected minimally the droplet size of the emulsions, though there has been an increase in flocculation. All the emulsions presented shear-thinning behavior and a strong shear rate dependence when salt was present in the system. The microspheres produced by spray drying of the emulsions were spherical and showed few aggregate formation. In addition, they presented high values of oil retention (> 80 wt%) and acceptable values of moisture content (< 4 wt%). SPI microgel particles produced by high-pressure homogenization or sonication may be used to stabilize emulsions with low oil contents. These emulsions can be further spray dried to microencapsulate lipophilic compounds using SPI microgels as wall material.
