14. 11883 Hysteroscopy for Early Pregnancy Loss: Managing EPL with a Reproductive Surgery Mindset.

Abstract

The objective of this video is to demonstrate the advantages of hysteroscopic management of early pregnancy loss, show two recently completed cases, and to reveal data from a retrospective chart review on the subject. Surgical videos were included from two cases. Retrospective chart review was performed of a prospectively maintained database. Descriptive data were extracted and reported. Advocate Lutheran General Hospital. Patients included in our database were presenting with early pregnancy loss managed with hysteroscopic resection. Early pregnancy loss was managed with hysteroscopic resection of pregnancy tissue. 91.7% of patients had no complications. Complications seen involved fluid overload, infection, and hemorrhage. No patients required blood transfusion. Genetic results were obtained in almost 90% of patients, and were abnormal in 51.5% of patients. Post-operative saline ultrasound was performed at three months and revealed normal results in 84.9% of patients. Hysteroscopic management of early pregnancy loss is a viable option for treatment of EPL. There are decreased intrauterine adhesions after this type of procedure vs suction D&C, the uterine cavity is able to be evaluated more thoroughly, and there is a higher yield of genetics results. Our data shows that the procedure has a low percentage of complications and can be completed in the office setting or in the operating room based on gestational age. [ABSTRACT FROM AUTHOR]

Abstract

To showcase the various presentations of Mullerian anomalies and surgical techniques used to alleviate symptoms related to the anomalies. Surgical video of a minimally invasive approach in management of patients with Mullerian anomalies in Advocate Lutheran General Hospital. Patients were either referred from their primary OBGYN with suspicion for Mullerian anomaly, or presented acutely to the hospital and the minimally invasive gynecologic surgery service was consulted for management following Mullerian anomaly diagnosis with imaging. • Private practice of minimally invasive gynecologist surgeon and emergency department • Patient is in dorsal lithotomy with Allen stirrups • We use vaginoscopy, hysteroscopy, and 3 port laparoscopy in an attempt to gain additional detail in visualizing patient anatomy Three patients with Mullerian anomalies consistent with Obstructed Hemivagina with Ipsilateral Renal Agenesis syndrome who underwent surgical management • Vaginoscopy and hysteroscopy are used to identify at least one cervix and to survey uterine cavities for other potential issues. • Laparoscopy allows recognition and confirmation of variations of Mullerian anomalies. It also allows direct visualization during hysteroscopic portion of case. N/A • Obstructed hemivagina ipsilateral renal agenesis is a relatively rare condition in which adolescent patients can present in the acute or outpatient settings. For our case series, patients were identified to have anomalies preoperatively via imaging or prior workup. • A combination of minimally invasive procedures including vaginoscopy, hysteroscopy, and laparoscopy are useful in the confirmation in Mullerian anomaly diagnosis, as well as surveillance of surgical treatment of Mullerian anomalies. The objective of this video is to demonstrate a series of video for recurrent ovarian torsion management, and also to show a novel technique of laparoscopic ovarian suspension and uterine uplift for management while preserving the ovary. Series of videos of surgeries for ovarian torsion. Advocate Lutheran General Hospital in Illinois. One patient with recurrent torsion was included and her surgical videos are presented. Laparoscopy for recurrent ovarian torsion. The patient has not had recurrence of torsion or pain since management with laparoscopic ovarian suspension and uterine uplift.
