The purpose of this study was to assess the incidence and risk factors associated with postoperative nausea (PON) and vomiting (POV) after orthognathic surgery. A review of the clinical records of consecutively enrolled subjects (2008–2012) at a single academic institution was conducted between 9/2013 and 3/2014. Data on the occurrence of PON and POV and potential patient-related, intraoperative, and postoperative explanatory factors were extracted from the medical records. Logistic models were used for the presence/absence of postoperative nausea and vomiting separately. Data from 204 subjects were analyzed: 63% were female, 72% Caucasian, and the median age was 19 years. Thirty-three percent had a mandibular osteotomy alone, 27% a maxillary osteotomy alone, and 40% had bimaxillary osteotomies. Sixty-seven percent experienced PON and 27% experienced POV. The most important risk factors for PON in this series were female gender, increased intravenous fluids, and the use of nitrous oxide, and for POV were race, additional procedures, and morphine administration. The incidence of PON and POV following orthognathic surgery in the current cohort of patients, after the introduction of the updated 2007 consensus guidelines for the management of postoperative nausea and vomiting, has not decreased substantially from that reported in 2003–2004. Late Quaternary aeolian dune systems in the Great Plains and western U.S. are valuable archives of past climate variability. This study investigates the St. Anthony dune field, located in the eastern portion of the Snake River Plain in southern Idaho. The current semi-arid climate of the region allows for a central core of active dunes that is approximately 450 km 2 in area and includes compound crescentic, blowout, falling, and climbing dunes that range in height from 15 to 121 m. The surrounding region is characterized by ∼20,000 km 2 of relic parabolic, linear, and complex linear dunes that are currently fixed by vegetation. This study examined nine samples collected from the fixed dunes that surround the active core of the St. Anthony dune field. Optically stimulated luminescence (OSL) dating results indicate that these fixed dunes were mobile and actively accreting ∼6.6 ka (middle Holocene) and 1.0–2.0 ka (late Holocene). Evidence for activation of a large region currently occupied by stable dunes suggests these periods were characterized by hydrologic deficit and/or warmer climate conditions than currently exists. This research provides new information regarding the depositional timing of the St. Anthony dunes and assists in providing evidence of palaeo-climatic conditions for the eastern Snake River Plain during the middle to late Holocene. The purpose of this study was to review the current literature for the relationship between the preoperative position of the mandibular canal on three-dimensional (3D) radiographic imaging and postoperative neurosensory disturbance (NSD) following a sagittal split ramus osteotomy (SSRO). A literature search was conducted using PubMed, EMBASE, and the Cochrane Database for articles published from 1 January 2000 through 31 December 2013. Studies that included preoperative 3D imaging and assessment of NSD after surgery were reviewed. Study sample characteristics and results were extracted. Of the 69 articles identified, seven met the inclusion and exclusion criteria. There was no standardization for measuring the canal position or for evaluating NSD. General consensus was that the less space between the mandibular canal and the outer border of the buccal cortex the more frequent the occurrence of NSD. Increased bone density also appeared to contribute to a higher incidence of NSD. Utilization of 3D images to locate and measure the position of the mandibular canal is not standardized. Advances in 3D imaging and evaluation tools allow for new methodologies to be developed. Early attempts are informative, but additional studies are needed to verify the relationship between the location of the nerve and NSD following surgery. [ABSTRACT FROM AUTHOR]

Percutaneous transluminal coronary angioplasty (PTCA), the technique of threading a catheter (thin tube) from an artery in the leg through the aorta and into the heart, is used to treat narrowing of the coronary arteries. With improved techniques and careful patient selection, the surgery is thought to be as safe for elderly patients as for younger ones. However, the safety of PTCA has primarily been studied in patients less than 65 years old, and it is desirable to know about its safety in older patients, since the entire population is aging. The results of PTCA in 22 patients aged 80 and above were reviewed for outcome. Patients undergoing PTCA were carefully selected, had less extensive coronary artery disease (CAD), less history of heart attack, and fewer had had a recent heart attack, compared with patients with CAD who were treated in other ways. Eighty-nine percent of the PTCA procedures were successful. A heart attack occurred in three patients following PTCA, and several other complications occurred. An average of 11 months later, 20 of 22 patients were alive. The two deaths were due to elective bypass surgery and to stroke complications. One further patient had developed angina following PTCA and was successfully treated with bypass surgery. Thirty-two other octogenarians with coronary artery disease were treated with medication and had a mortality rate of 19 percent. The results indicate that in selected octogenarians with CAD, PTCA is an appropriate treatment with a favorable outcome. (Consumer Summary produced by Reliance Medical Information, Inc.)

15. Ex post socio-economic assessment of the Oresund Bridge

Abstract: The paper presents an ex post socio-economic assessment of the Oresund Bridge conducted ten years after the opening in July 2000. The study applies historical micro data to re construct the travel pattern with no bridge in place and compare this to the current situation. To complete the socio-economic assessment, the consumer benefits including all freight and passenger modes, are compared with the cost profile of the bridge. The monetary contributions are extrapolated to a complete 50 year period. It is revealed that the bridge from 2000–2010 generated a consumer surplus of €2 billion in 2000 prices discounted at 3.5% p.a., which should be compared with a total construction cost of approximately €4 billion. Seen over the 50 year period and by assuming a medium growth scenario the bridge is expected to generate an internal rate of return in the magnitude of 9% corresponding to a benefit-cost rate of 2.2. A main advantage of analysing infrastructure ex post is the ability to learn and understand behavioural and methodological elements not foreseen at the ex ante stages. Following this we offer an extended discussion including two parts. Firstly we compare the ex ante predictions for the bridge to the current transport flows. The importance of having the right assumptions and the ability to model the phasing-in process are underlined. Secondly, we offer a wider discussion on why some projects are more beneficial than others. This is done by comparing the Oresund Bridge, the Channel Tunnel, and the Great Belt Link. Abstract: At the European level there is an increasing focus on how freight transport can be moved from trucks on roads to more environmentally friendly modes such as rail and ship. A large proportion of the transport services between OD pairs, however, cannot be substituted since there is only one alternative available. The paper investigates the magnitude of this “structural inelasticity” of modal substitution in freight transport due to a sparser layout of rail and ship-based freight networks compared to road. In the analysis we use a recent Scandinavian freight demand model covering more than 800 zones. We find that the structural inelasticity is very significant – in particular for transportation over less than 500km. Moreover, the inelasticity varies greatly with commodity groups and between OD pairs, and it depends strongly on the port and rail infrastructure. The results suggest that pure charging instruments (road pricing for trucks) in many regions will have limited mode substitution impacts. However, if combined with structural changes in terms of improved infrastructure for rail and ship, impacts may be greater. [ABSTRACT FROM AUTHOR]

Abstract: The paper describes the estimation of a weighted discrete choice model applicable for analysing choice of mode and crossing for freight in the Oresund region. The study finds that, by applying a weighted logit approach, it is possible to establish a suitable decoupling of agents and shipments. Moreover, by assessing preferences on the basis of a representative baseline OD matrix it is possible to better reflect the dependence between mode substitution effects and geography/infrastructure. Abstract: The paper presents results of plasma assisted combustion experiments in premixed hydrocarbon–air flows excited by a low-temperature transverse repetitively pulsed discharge plasma. The experiments have been conducted in methane–air and ethylene–air flows in a wide range of equivalence ratios, flow velocities, and pressures. The plasma was generated by a sequence of high-voltage (∼10kV), short pulse duration (∼50ns), high repetition rate (up to 50kHz) pulses. The high reduced electric field during the pulse allows efficient electronic excitation and molecular dissociation. On the other hand, the extremely low duty cycle of the repetitively pulsed discharge, ∼1/500, greatly improves the discharge stability and helps sustaining diffuse and uniform nonequilibrium plasma. Generating this repetitively pulsed plasma in premixed hydrocarbon–air flows results in ignition and flameholding, occurring at low plasma temperatures, 140–300°C, inferred from the nitrogen second positive band system spectra. At these conditions, the reacted fuel fraction, measured by the FTIR absorption spectroscopy, is up to 80%. The experiments demonstrate significant methane and ethylene conversion into CO, CO
2 , and H2 O even at the conditions when there is no flame detected in the test section. At these conditions, fuel oxidation occurs due to plasma chemical reactions, without ignition. This provides additional evidence for the nonthermal fuel oxidation triggered by plasma-generated radicals. [Copyright &y& Elsevier]- Published

Bioactive properties of composites containing poly(ϵ-caprolactone-co-dl-lactide) with molar ratio 96/4 and bioactive glass (BAG), S53P4, were tested in vitro. The glass content in the tested materials was 40, 60 or 70 wt%, and two granule size ranges (<45 and 90–315 μm) were used. The composites were analysed for their apatite-forming ability. This was determined as a function of time by the dissolution pattern of Si and Ca ions and structural changes on the specimen surfaces. Composite specimens were immersed in simulated body fluid at 37°C for up to 6 months. The changes in Si and Ca concentrations of the immersion medium were determined with UV–Vis and atomic absorption spectrophotometry. The calcium phosphate precipitation and apatite formation were evaluated by scanning electron microscopy (SEM) and infra-red spectroscopy (IR) using the attenuated total reflectance (ATR) system. The SEM and SEM-EDX analysis of the depositions formed on the composite surfaces was in line with the changes in ion concentrations. The clearest results with IR were seen in the material containing 60 wt% small glass particles. The results indicate that composites containing over 40 wt% BAG granules are bioactive, and that a higher BAG surface area/volume ratio favors the apatite formation in vitro. [Copyright &y& Elsevier]

21. Cardiobacterium Valvarum Bacteremia in a Left Ventricular Assist Device Recipient: Case Report and Review of the Literature.

Cardiobacterium valvarum is a HACEK organism known for causing infective endocarditis (IE); there are no reported cases implicating this bacterium in left ventricular assist device (LVAD) infections in the English literature. A 59 year old gentleman with sarcoidosis, systolic heart failure (EF 15%), BiV-ICD (implanted 11/2015) and LVAD (Heartware implanted 10/2017) presented with fever and cough. He experienced worsening lower extremity edema and abdominal girth one week prior to admission. In the emergency room his temperature was 39.5°C and had mild erythema without drainage at his drive line exit site (DLES). Labs were notable for WBC of 15.5 × 103/μL and lactic acid of 2.2 mmol/L. Blood cultures were obtained and the patient received piperacillin-tazobactam and vancomycin. TTE and CT imaging of chest and abdomen demonstrated no significant findings. The initial blood culture became positive with gram-negative bacilli after 3.5 days and were identified as C. valvarum 28 days after initial presentation. He completed a two-week course of ertapenem and transitioned to cephalexin 500 mg BID as secondary prophylaxis. Erythema at the DLES improved and heart failure symptoms resolved. The patient remained on this regimen until transplant, 348 days from positive culture. The organisms classified in the HACEK (Haemophilus spp., Aggregatibacter spp., Cardiobacterium spp., Eikenella spp., and Kingella spp.) group are known for being fastidious causes of IE; they are not traditionally associated with pump, line, or pocket infections in VAD patients. The most common gram-negative bacterial infections associated with VADs and their components are P. aeruginosa and enteric organisms. This patient's case was treated as a VAD-related blood stream infection. Because Cardiobacterium spp. are oral commensals, DLES as the source was less likely. As noted in the IE guidelines, the presence of a HACEK organism without an identifiable source should be considered IE unless proven otherwise, even if there are no other stigmata of IE. Thus, despite only one culture growing C. valvarum , there was a high suspicion for the components of the VAD being seeded, resulting in antibiotic suppression until transplantation. In the presence of a VAD, HACEK organism found in the bloodstream should be treated as a pump infection unless proven otherwise. [ABSTRACT FROM AUTHOR]
