La complexité des trajectoires atopiques, l'augmentation de la sévérité des maladies atopiques et l'émergence de nouvelles thérapeutiques imposent d'offrir une expertise dans ce domaine. Nous souhaitons partager notre expérience d'une consultation pluridisciplinaire « Atopie sévère de l'enfant » mise en place aux Hospices Civils de Lyon. Les dossiers des patients vus en consultation pluridisciplinaire « Atopie sévère de l'enfant » ont été revus de manière rétrospective. La consultation était menée par des pneumologues, dermatologues et immunologistes ayant une formation de pédiatres et allergologues. Au cours de cette consultation d'une heure, les atteintes atopiques des différents organes étaient évaluées et prises en considération pour une prise en charge spécifique puis, si possible, globale, avec notamment la prescription de traitements systémiques. Au total, 48 enfants ont déjà eu accès à cette consultation, tous atteints de dermatite atopique, dont 68 % sous une forme sévère. Une comorbidité atopique était retrouvée chez 83 % des patients : 56 % avaient une allergie alimentaire IgE-médiée, 58 % un asthme et 60 % une rhinoconjonctivite allergique. Un traitement systémique a été prescrit à 62 % des patients dès la première consultation, les traitements inhalés ont été optimisés et la majorité des patients allergiques alimentaires ont bénéficié d'une immunothérapie orale dans les suites de cette consultation. Cette consultation pluridisciplinaire « Atopie sévère de l'enfant » offre une prise en charge globale de ces patients et pourrait devenir pivot de la trajectoire de soin de l'atopie de l'enfant. Here, we would like to give feedbacks from a multidisciplinary consultation called "Severe atopy in children", set up in the Hospices Civils de Lyon. Retrospective reviews of patients' charts were conducted. The consultation "Severe atopy in children" was conducted by pulmonologists, dermatologists, and immunologists with a training background of pediatricians and allergists. During this one-hour consultation, atopic conditions of various organs were evaluated and taken into consideration for specific, and if possible, global management, including the prescription of systemic treatments. In total, 48 children have accessed this consultation, all of whom are suffering from atopic dermatitis, with 68% having a severe form. Atopy comorbidity was found in 83% of patients, including 56% having IgE-mediated food allergy, 58% having asthma, and 60% having allergic rhinoconjunctivitis. A systemic treatment was prescribed to 62% of patients during the first consultation. Inhaled treatments were also optimized for asthmatics and the majority of food-allergic patients receiving oral immunotherapy in the follow-up. The multidisciplinary consultation "Severe Atopy in Children" offers comprehensive care to children with atopic disorders and has the potential to become a cornerstone in the management of childhood atopy. Failed endovascular infrarenal aortic aneurysm repair (EVAR) due to development of late type Ia endoleak exposes patients to the risk of rupture and should be treated. The purpose of this study was to evaluate the results of fenestrated/branched EVAR (F/BEVAR) for treatment of failed EVAR with type Ia endoleak. From January 2010 to December 2019, a prospective multicentre study was conducted (ClinicalTrials.gov identifier: NCT04532450) that included 85 consecutive patients who had undergone F/BEVAR to treat a type Ia endoleak following EVAR. The primary outcome was overall freedom from any re-intervention or death related to the F/BEVAR procedure. In 30 cases (35%) EVAR was associated with a short < 10 mm or angulated (> 60°) infrarenal aortic neck, poor placement of the initial stent graft (n = 3, 4%), sizing error (n = 2, 2%), and/or stent graft migration (n = 7, 8%). Type Ia endoleak was observed after a period of 59 ± 25 months following EVAR. The authors performed 82 FEVAR (96%) and three BEVAR (4%) procedures with revascularisation of 305 target arteries. Overall technical success was 94%, with three failures including one persistent Type Ia endoleak and two unsuccessful stent graft implantations. Intra-operative target artery revascularisation was successful in 303 of 305 attempts. The in hospital mortality rate was 5%. Cardiac, renal and pulmonary complications occurred in 6%, 14%, and 7% of patients, respectively. Post-operative spinal cord ischaemia occurred in four patients (4.7%). At three years, the survival rate was 64% with overall freedom from any re-intervention or aneurysm related death of 40%, and freedom from specific F/BEVAR re-intervention of 73%. At three years, the secondary patency rate of the target visceral arteries was 96%. During follow up, 27 patients (33%) required a revision procedure of the fenestrated (n = 11) or index EVAR stent graft (n = 16), including six open conversions. While manufactured F/BEVAR was effective in treating type Ia endoleak in patients with failed EVAR, it was at the cost of a number of secondary endovascular and open surgical procedures. Objectives The TECCO (Traitement des Lésions Athéromateuses de l’Artère Fémorale Commune par Technique Endovasculaire Versus Chirurgie Ouverte [Endovascular Versus Open Repair of the Common Femoral Artery]) trial is a randomized comparison of safety and efficacy of stenting versus open surgery for de novo common femoral artery (CFA) stenosis. Background Surgery for CFA lesions is considered effective and durable. Despite the widespread use of endovascular repair for infrainguinal disease, the value of this procedure for such lesions is uncertain. Methods From February 23, 2011, to September 5, 2013, a total of 117 patients with de novo atherosclerotic lesions of the CFA were randomly assigned to undergo surgery (n = 61) or stenting (n = 56). The main exclusion criteria were asymptomatic disease, restenosis, and thrombosis of the CFA. The primary outcome was the morbidity and mortality rate within 30 days. This includes any general complications or local complications that caused or prolonged hospitalization and/or re-intervention, lymphorrhea of more than 3 days, and post-operative paresthesia that required drugs. The median duration of follow-up was 2 years (interquartile range [IQR]: 19.8 to 24.9 years). Results Primary outcome events occurred in 16 of 61 patients (26%) in the surgery group and 7 of 56 patients (12.5%) in the stenting group (odds ratio: 2.5; 95% confidence interval: 0.9 to 6.6; p = 0.05). The mean duration of hospitalization was significantly lower in the stenting group (3.2 ± 2.9 days vs. 6.3 ± 3 days; p < 0.0001). At 24 months, the sustained clinical improvement, the primary patency rate, and the target lesion and extremity revascularization rates were not different in the 2 groups. Conclusions In patients with de novo atherosclerotic lesions of the CFA, the perioperative morbidity and mortality rate was significantly lower among patients who underwent endovascular therapy by stenting compared with surgery, whereas clinical, morphological, and hemodynamic outcomes were comparable at mid-term. Epidemiological data indicate that the role of environmental factors on breast cancer (BC) incidence remains undetermined. Our daily life exposure to aluminium (Al) is suspected to influence BC development. This review proposes a state of the art on the association between Al and BC risk combined with a critical point of view on the subject. We searched the PubMed database using terms related to Al and BC up to November 18, 2022. Reports were eligible if they were cohort or case-control studies or meta-analyses. Six studies focused on the relationship between deodorant and antiperspirant use and BC incidence and didn't produce consistent results. Among 13 studies relating Al content in mammary tissues and BC risk, results are not unanimous to validate higher Al content in tumor tissues compared to healthy ones. We detail parameters that could explain this conclusion: the absence of statistical adjustments on BC risk factors in studies, the confusion between deodorant and antiperspirant terms, the non-assessment of global Al exposure, and the focus on Al in mammary tissues whereas a profile of several metals seems more appropriate. The clinical studies are retrospective. They were carried out on small cohorts and without a long follow-up. On the other hand, studies on cell lines have shown the carcinogenic potential of aluminum. Moreover, studies considered BC as a unique group whereas BC is a heterogeneous disease with multiple tumor subtypes determining the tumor aggressiveness. In light of the precautionary principle and based on the data obtained, it is better to avoid antiperspirants that contain Al. Deodorants without aluminum are not implicated in breast cancer, either clinically or fundamentally. • Al has in vitro pro-inflammatory properties and estrogenic activity on breast tissue. • No consistent results on an association between deodorant/antiperspirant use and BC. • No mention of Al from vaccines, Al cookware, cosmetics in the literature. • No evidence that Al plays a role in the pathogenesis of BC in women. • Future investigations on the role of metals, not only aluminium, in BC are warranted. Objective Iliofemoral endarterectomy with external iliac artery (EIA) stent grafting can be an alternative to traditional open surgery in patients with severe iliac occlusive disease extending to the common femoral artery. We report the midterm outcomes of this approach. Methods Between 2009 and 2015, 108 patients (76% male; median age, 63 years) underwent a total of 127 iliofemoral endarterectomies combined with EIA stent grafting. Indications were claudication in 60%, rest pain in 20%, ulceration in 15%, and acute ischemia in 5%. Lesions exclusively involved only the EIA segment in 40% of cases, with occlusion in 28%. Lesions involved both the EIA and common iliac artery segments in 49% of cases, with 19% of common iliac artery occlusions and 24% of EIA occlusions. Iliac lesions extended into the aortic segment in 11% of cases. Iliofemoral endarterectomy was performed by eversion whenever possible. Deployment of the EIA stent graft systematically incorporated the EIA segment and the proximal end of the endarterectomy. Self-expanding covered stents were calibrated to the diameter of the endarterectomized EIA. Results The procedure was technically successful in 100% of patients. Median diameter of covered stents was 8 mm (range, 6-10 mm). No intraoperative arterial rupture or dissection was observed. Early reoperations (3%) were performed for bleeding, infection, or thrombosis. Median length of stay was 5 days. No 30-day mortality was observed. Median follow-up was 30 months (range, 0-6 years), and overall mortality was 13% (due to cancer in half of the cases). Repeated angioplasty was performed in three (2%) cases, and a subsequent open procedure on the iliofemoral segment was performed in seven (5%) cases. At 2 years, primary patency rate of the treated segment was 91%. The 2-year primary assisted patency and secondary patency rates were 94% and 98%, respectively. Five-year primary, primary assisted, and secondary patency rates were 87%, 92%, and 98%, respectively. Conclusions Combined iliofemoral endarterectomy and covered stenting of the EIA for treatment of severe occlusive lesions provided acceptable midterm results, probably because of the gain of diameter provided by covered stents. This technique avoids complications due to an aortic or iliac surgical approach and clamping as well as complications related to the presence of a prosthetic implant in an intra-abdominal position. Patients with cystic fibrosis have increased oxidative stress and impaired antioxidant systems. Moderate intake of docosahexaenoic acid (DHA) may favor the lowering of oxidative stress. In this randomized, double-blind, cross-over study, DHA or placebo capsules, were given daily to 10 patients, 5 mg/kg for 2 weeks then 10 mg/kg DHA for the next 2 weeks (or placebo). After 9 weeks of wash-out, patients took placebo or DHA capsules. Biomarkers of lipid peroxidation and vitamin E were measured at baseline, and after 2 and 4 weeks of treatment in each phase. The proportions of DHA increased both in plasma and platelet lipids after DHA supplementations. The lipid peroxidation markers did not significantly decrease, in spite of a trend, after the first and/or the second dose of DHA but plasma and platelet vitamin E amounts increased significantly after DHA supplementation. Our findings reinforce the antioxidant potential of moderate DHA intake in subjects displaying increased oxidative stress. [ABSTRACT FROM AUTHOR]

Background Persistent type II endoleaks (T2Ls) with sac enlargement after endovascular abdominal aortic aneurysm repair are still of concern in view of the potential for rupture. Current treatments (embolization and stent graft [SG] explantation) are associated with lack of efficacy or high perioperative morbidity and mortality. This study evaluated an alternative technique that combines sacotomy, ligation of patent back-bleeding vessels, and SG preservation for T2L or unspecified endoleak repair. Methods This multicenter study in France included 28 patients (27 men; median age, 78 years). Twenty-one patients (75%) had a bifurcated SG (including 3 fenestrated SGs) and seven (25%) had an aortouni-iliac SG (2 for ruptured aneurysm). Unsuccessful embolization had been performed in 10 patients (36%). Four patients (14%) presented sac enlargement with no endoleak visible on computed tomography. The origin of the endoleak remained unspecified in three patients 3 (11%). The median diameter of the aneurysmal sac was 78 mm (vs 55 mm at the time of endovascular abdominal aortic aneurysm repair) after a median follow-up of 24 months. Results A transperitoneal approach was used in 21 patients (75%) and a retroperitoneal approach was used in seven (25%). A guidewire was placed in the supraceliac aorta in 14 patients, and an occlusion balloon was temporarily inflated in six. Aortic cross-clamping was performed in five patients. T2Ls were identified in 26 patients, and associated with a distal type I endoleak in 1 patient, a type III endoleak in 3, and a type IV endoleak in 1. Two patients presented with endotension. All the endoleaks were treated successfully, with a mean operating time of 120 minutes and a mean blood loss of 450 mL. One SG was explanted 12 days after the procedure because of early infection. One patient died during SG explantation for an aortoduodenal fistula 26 months after the endoaneurysmorrhaphy. During a median follow-up of 24 months, the control computed tomography scan showed shrinkage of the aneurysmal sac with stable diameters in all patients. No missed T2Ls, no recurrence of T2L, and no SG migration or disjunction was observed. Conclusions Obliterating endoaneurysmorrhaphy with SG preservation can be considered as an alternative to SG removal in cases of persistent T2L responsible for aneurysmal sac enlargement after embolization failure. By avoiding extensive dissection for surgical aortic cross-clamping, minimizing hemodynamic changes, and reducing blood loss and operating time, this procedure can be performed even in patients initially considered unfit for surgery. Objective/Background Persistent type II endoleak (EL II) with sac enlargement after endovascular repair of abdominal aortic aneurysm requires treatment to prevent rupture. Embolization is not always effective. Conversion to open repair with stent graft (SG) explantation is a high risk option. The aim of this study was to evaluate the feasibility and immediate results of an alternative technique combining obliterative endoaneurysmorrhaphy (OEA) with SG preservation. Methods The open surgical technique combined sacotomy, ligation of all patent back-bleeding vessels and SG preservation. The aneurysmal shell was tightly closed over the SG to protect it from the intestines. An intra-aortic occlusion balloon was used when clamping was required. Results Twelve patients were treated with the OEA technique at Amiens University Hospital. All 12 procedures were successful. Four patients had previously undergone unsuccessful transarterial or translumbar embolization. Aortic clamping was performed in four cases. No SG migration or graft dislocation was observed. Follow up computed tomography scan at a median of 12 months showed shrinkage of the aneurysm sac with stable diameters and no recurrence of EL II in all cases. Question: : Can a session of exercise with incorporated expiratory manoeuvres substitute for a session of breathing techniques for airway clearance in children with cystic fibrosis? Are children with cystic fibrosis as co-operative and satisfied with the exercise regimen as with the breathing techniques?. Design: Randomised, cross-over trial with concealed allocation and intention-to-treat analysis. Participants: 34 children with cystic fibrosis in a stable clinical state. Interventions: Participants underwent two 20-min airway clearance interventions on two scheduled clinic days: one involving three bouts of various whole-body exercise modalities each followed by independent expiratory manoeuvres, and the other involving breathing control, thoracic expansions with manual expiratory compressions, and the forced expiratory technique. Outcome measures: Wet weight of expectorated sputum, change in lung function, co-operation with treatment, perceived treatment quality, and satisfaction with treatment were all assessed after each intervention. Results: The wet weight of sputum after exercise was 0.6g higher after the exercise intervention, which was not statistically or clinically significant (95% CI –0.2 to 1.4). However, lung function and participant satisfaction with the treatment were both significantly better after the exercise intervention. Co-operation with treatment and perceived treatment quality were equally high for each intervention. Conclusion: A session of various whole-body exercises interspersed with independent expiratory manoeuvres could be an acceptable substitute for a session of breathing control, thoracic expansions with manual expiratory compressions, and the forced expiratory technique in children with mild cystic fibrosis lung disease.
