The prime goal of this novel work is to investigate the efficacy of spinel zinc ferrite as a thermoelectric material and its performance enhancement for high temperature applications. Zn 1-x Ni x Fe 2 O 4 (0 ≤ x ≤ 0.2) were prepared by sol-gel method and their structural, morphological, and thermoelectric behaviours were examined as functions of composition and temperature. Among the studied compositions, Zn 0.8 Ni 0.2 Fe 2 O 4 exhibits the highest electrical conductivity (σ) of 49.83 S/m, the largest power factor of 6.66 μW/mK2, and the lowest thermal conductivity (κ) of 0.36 W/mK at 947 K, thus extending to an overall enhancement in the thermoelectric figure of merit (zT). Conduction through electron hopping is proven to be the main driving force for the enhancement in electrical and thus thermoelectric properties. Results reveal that thermoelectric properties of ZnFe 2 O 4 can potentially be enhanced by the doping Ni at Zn site and opens up an exciting opportunity for the utilization of this spinel ferrite as a novel thermoelectric material for applications involving elevated temperatures. Borassus flabellifer L. is a tall palm traditionally used for its stimulating, diuretic and anti-inflammatory activities; it is rich in fibers and various pharmacologically important secondary metabolites. This study was undertaken to evaluate the antidiabetic effects of Borassus flabellifer fruit methanol extract (BF-M) on diabetic rats induced with High Fat Diet (HFD)/streptozotocin (STZ). When BF-M (100 or 200 mg/kg) was administered for 21 days orally it led to a sharp decline in triglycerides, total cholesterol, free unsaturated fat, glucose-6-phosphate, fasting blood glucose and fructose 1,6 bisphosphatase in contrast to diabetic control. BF-M also downregulated Protein Tyrosine Phosphatase 1B. In vitro study showed the IC 50 value to be 23.98 μg/mL. BF-M significantly increased serum insulin, glycogen content, and body weight. Western blot analysis exhibited significant inhibition of PTP1B in pancreatic tissue which was confirmed by histology and immunohistological studies. GC-MS analysis revelaled that the presence of major compounds such as 5-hydroxymethylfurfural (47.56%), Guanosine (21.01%) and n-hecxadeconoic acid (25.14%) in BF-M. In short, BF-M exerted antidiabetic property by down regulating PTP1B expression, and eventually enhancing glucose stimulated insulin release; it also exhibited favorable effects in diabetes and its secondary complications. Abstract: The effectiveness of a digital watermarking algorithm is indicated by the robustness of embedded watermarks against various attacks. Attacks which attempt to destroy watermarks have two types, noise-like signal processing and geometric distortions. To select a non-overlapping region, some reference parameters have been used, the regions with higher feature corner response are selected when the Harris–Laplacian or Harris–affine detector is used. The number of neighboring feature points inside a region is a major reference for obtaining a non-overlapping set in the watermarking method based on Mexican Hat wavelet scale interaction. A watermarked feature region may have different degrees of robustness against different techniques if there is prior knowledge of each region''s attack resistance capability. La mise à disposition d'une nouvelle génération d'antiviraux d'action directe est une révolution dans les traitements des personnes porteuses d'infection chronique par le virus de l'hépatite C (VHC). L'objectif de notre étude est d'évaluer l'efficacité et la tolérance thérapeutique de ces traitements dans la prise en charge des patients tunisiens atteints d'VHC. Cent-quarante dossiers ont été analysés rétrospectivement au service de gastroentéologie de l'hôpital Mohamed Taher Maamouri Nabeul entre 2015 et 2019. Toutes les données cliniques, biologiques et histologiques ont été recueillies et analysées à partir des dossiers des malades traités et suivis pour VHC. Cent-quarante patients infectés VHC ont été colligés. Il s'agissait de 99 femmes et 41 hommes, d'âge médian 65 ans [22-85], majoritairement de génotype 1b (n = 122 cas 87,1 %). Il s'agissait de 36 patients prétraités (25,7 %) et 104 patients naïfs (74,3 %). Quatre-vingt-treize patients avaient une fibrose avancée F3/F4 (64,13 %). L'association Sofosbuvir (SOF)/ledipasvir (LDV) a été la plus fréquemment proposée (135 patients soit 96,4 %) associée à la ribavirine chez 20 patients. L'association sofosbubir + ribavirine était proposée dans 5 cas (3,6 %). La PCR 3 mois après fin du traitement était négative chez tous les patients. Parmi 15 patients ayant une cirrhose décompensée, trois ont présenté une encéphalopathie hépatique. Quatre patients ont développé un CHC en cours de suivi avec un délai d'apparition de CHC entre 3 et 13 mois après la fin du traitement. La tolérance a été excellente dans l'ensemble. Aucun effet indésirable sévère nécessitant l'interruption de traitement n'a été noté. Les principaux effets indésirables survenus étaient l'asthénie chez 36,4 % des patients (n = 51), des épigastralgies dans 9,4 % (n = 13), des céphalées dans 7,8 %(n = 11)et le prurit dans 2,8 %(n = 4). En analyse univariée, une association statistiquement significative a été trouvé entre la survenue d'effets indésirables et la présence d'une cirrhose (p = 0,01), la consommation d'alcool (p = 0,04)et la toxicomanie(p = 0,05). Mais il n'a pas été trouvé d'association significative entre la survenue d'effets indésirables et la coïnfection par le virus de l'hépatite B (p = 0,44), le diabète (p = 0,2) et l'âge supérieur à 75 ans (p = 0,7). Dans cette population Tunisienne, les résultats en termes d'efficacité et de tolérance des antiviraux à action directe (AAD) sont très encourageants pour envisager l'éradication de l'hépatite C chronique. Un tiers des patients ont présenté des effets indésirables non sévères ne nécessitant pas l'interruption de traitement. La sévérité de la fibrose (notamment la présence de cirrhose) était un facteur prédictif d'apparition d'effets indésirables, incitant à plus de vigilance au cours du suivi du traitement. Abstract: Pulmonary emphysema is a major cause of mortality and morbidity in chronic obstructive pulmonary disease (COPD). Cigarette smoking is a major risk factor in the development of pulmonary emphysema. In this study, we investigated the acute effect of cigarette smoke in vitro on the production of tumour necrosis factor-α (TNF-α) using differentiated U937 cells, a macrophage model system. We found that stimulation of the macrophages with cigarette smoke media (CSM) leads to a rapid activation of extracellular-regulated kinases 1 and 2 (erk1/2), p90RSK and a transient decrease in phosphorylation of PKB/akt. The CSM also caused the subsequent induction of TNF-α release. Our studies revealed that U0126, an inhibitor of the erk1/2 pathway, markedly suppressed CSM-induced TNF-α release. Consistent with this finding, U0126 blocked CSM-stimulated erk1/2 phosphorylation, as well as phosphorylation of the downstream kinase, p90RSK. On the other hand, the PI3-K inhibitor, LY294002, and epidermal growth factor receptor (EGFR)-specific inhibitor, AG1478, did not suppress the release of TNF-α. Thus, CSM induction of TNF-α production by differentiated macrophages is regulated primarily via the erk1/2 pathway. [Copyright &y& Elsevier]

Summary: Severe alpha1-antitrypsin (AAT) deficiency is an inherited disorder, leading to the development of emphysema in smokers at a relatively young age and disability in most smokers in their 40s. Panacinar emphysema is caused by damage to lung elastin and connective tissue as a result of smoking-induced release of neutrophil elastase in the lung, inadequately inhibited by the severe deficiency in AAT, the major inhibitor of neutrophil elastase. Severe antitrypsin deficiency (mostly protease inhibitor phenotype ZZ) is associated with retention of abnormal AAT in the liver, which may lead to neonatal hepatitis, liver disease in children, and liver disease and cirrhosis in adults. AAT replacement therapy by intravenous infusions of purified human AAT has been used to partially correct the biochemical defect and protect the lung against further injury, but has not yet been proven to be clinically effective in reducing the progression of disease in AAT-deficient patients. However, there was a suggestion of a slower progression of emphysema by chest computerized tomography in a small randomized trial, and non-randomized observational studies suggested a slower rate of decline in lung function in subjects with moderate airflow obstruction. AAT deficiency is often unrecognized. AAT testing is recommended in patients with chronic irreversible airflow obstruction with early onset of disease, those with disability in their 40s or 50s or positive family history, in immediate family members of AAT-deficient patients, in infants and children with liver disease, and adults with unexplained liver disease. La cholangite biliaire primitive (CBP) est une maladie auto-immune qui affecte les petits canaux biliaires. Bien que les options thérapeutiques soient de nos jours limitées, un ralentissement de l'évolution de la maladie parallèlement à la réponse biologique sont possibles moyennant une approche thérapeutique adéquate. Ainsi, l'identification précoce des facteurs prédictifs de mauvaise réponse peut nous aider à optimiser notre approche thérapeutique. Il s'agissait d'une étude rétrospective mono centrique incluant les patients diagnostiqués d'une CBP durant une période de 20 ans (janvier 1999–décembre 2019). Les données cliniques, biologiques et anatomopathologiques ont été recueillies. Nous avons subdivisé nos patients en 2 groupes selon l'obtention ou non d'une réponse biochimique définie selon les critères de Paris II. Cent huit patients ont été colligés. L'âge moyen était 54 ans (19–85]. Cent femmes et huit hommes avec un sex ratio H/F 0,08. Dans 20 %(n = 22) la CBP a été diagnostiquée au stade de cirrhose. Les signes fonctionnels notés étaient : un prurit dans (39,8 %) des, asthénie (33,3 %) et des douleurs de l'hypochondre droit (26,9 %). Le diagnostic de la maladie a été retenu devant la présence de 2 critères parmi les 3 suivants : Cholestase biologique, positivité des anticorps anti mitochondrie ou des signes évocateurs à l'examen anatomopathologique du foie. Le traitement par AUDC a été instauré chez 75,9 % (n = 82) des patients. Une évaluation de la réponse biochimique au traitement a été effectuée selon les critères de Paris II. Une bonne réponse a été notée chez 49 % de nos malades. En analyse multi variée, les facteurs associés à la mauvaise réponse au traitement étaient : la découverte de la maladie au stade de cirrhose (p = 0,005), Un niveau élevé des phosphatases alkalines (PAL)au moment du diagnostic (p = 0,04), une hépatite auto-immune associée (p = 0,002) et une ductopénie (p < 0,001). D'après notre étude, l'existence d'un syndrome de chevauchement avec une hépatite auto-immune, un taux initial élevé des PAL, la ductopénie et le stade de fibrose avancé semblent associés à une mauvaise réponse thérapeutique au cours de la CBP. Abstract: Aims: Alpha1-antitrypsin deficiency (AATD) is a clinically under-diagnosed genetic disorder that originates from deleterious mutations in the alpha1-antitrypsin (AAT) gene, SERPINA1. Severe deficiency is associated with significant pulmonary and hepatic malfunctions. Conventional clinical diagnosis involves the evaluation of serum AAT level and detection of diseased protein isoforms. In this communication, we describe the investigations of a case of severe AATD in which the AAT levels were well below those expected from the MZ phenotype determined by isoelectric focusing for protease inhibitor type (IEF PI-typing). Methods: In addition to the traditional diagnostic method that combines the assessment of serum AAT concentration and IEF PI-typing, we investigated the SERPINA1 gene of the proband and participating family members for mutations using Sanger sequencing. Results: We identified a novel mutation (M409T) in the proband, initially missed by the standard diagnostic approach. The novel mutation was present in 4 out of 8 family members who participated in the study. Conclusions: This report illustrates the diagnostic value of incorporating exon sequencing of the AAT gene into the algorithm for evaluating AATD, particularly when the AAT serum level is significantly lower than expected from IEF PI-typing. Au cours des hépatopathies chroniques, la ponction biopsie du foie est le gold standard pour l'évaluation du degré de la fibrose. Par ailleurs, plusieurs marqueurs pronostiques, entre autres les scores FIB4 et APRI, ont été proposés pour identifier les patients à risque. Dans la cholangite biliaire primitive (CBP), la place de ces deux scores dans l'évaluation de la sévérité de l'atteinte hépatique n'a pas été bien déterminée. Le but de notre travail est de préciser la valeur diagnostique des scores FIB4 et APRI dans l'évaluation de la fibrose au cours de la CBP. Il s'agit d'une étude rétrospective colligeant sur une période de 19 ans tous les patients suivis pour une cholangite biliaire primitive ayant eu une PBF. Ces patients ont été répartis en deux groupes selon le degré de fibrose. Groupe 1 : F0 et F1, groupe 2 : F2, F3 et F4. Les scores APRI et FIB4 ont été calculés respectivement selon les formules : ASAT (LSN) × 100/taux des plaquettes (109 elts/L), et âge (ans) × AST (U/L)/[PLT(109/L) × ALT1/2 (U/L)]. L'identification du meilleur seuil discriminant des scores APRI et FIB4 a été évaluée en se basant sur la courbe ROC. Il s'agissait de 80 patients, 74 femmes (92,5 %) et 6 hommes (7,5 %) avec un âge moyen de 54.6 ans [extrêmes 19–85 ans]. Soixante-trois patients ont été inclus au groupe 2 (78,75 %) et 17 patients ont été inclus au groupe 1 (21,5 %). L'aire sous la courbe ROC pour le diagnostic d'une fibrose significative (> = F2) du score APRI est de 0, 667 (95 % IC/0,43–0.899). Le meilleur seuil discriminant de ce score entre fibrose significative et non significative était de 0,28 avec une sensibilité de 85,7 % et une spécificité de 57,1 %. Pour le score FIB4, l'aire sous la courbe ROC pour le diagnostic d'une fibrose significative (≥ F2) est de 0. 632 (95 % IC/0,405–0,85). Le meilleur seuil discriminant de ce score était de 1,28 avec une sensibilité de 73 % et une spécificité de 57 %. Ces deux scores d'APRI et FIB4 avaient une valeur prédictive positive pour la fibrose significative respectivement de 94 % et 91 %. Bien qu'ils aient une bonne valeur prédictive positive, les scores FIB4 et APRI ne peuvent pas être valides dans la détermination de la sévérité de l'atteinte hépatique au cours de la CBP. The conversion of biomass-derived lignin to valuable monomeric phenols at high selectivity is of paramount importance for sustainable biorefineries. In this study, a novel Pd-Al 2 O 3 supported on activated biochar catalyst is developed for lignin hydrogenolysis. The catalyst characterization revealed that the (1 1 1) planes of both of Pd0 and Al 2 O 3 were exposed to the surface. The maximum lignin conversion of 70.4% along with high liquid yield (∼57 wt%) was obtained at 240 °C, 3 h and 3 MPa H 2 pressure. The total monomeric phenols yield in the liquid was 51.6 wt%, out of which C9 monomeric guaiacols constituted ∼ 30.0 wt% with 38.0% selectivity to 4-propyl guaiacol. Using the reaction intermediate, coniferyl alcohol, chemoselective hydrogenation of C α =C β is proved to occur over the Pd site, while dehydroxylation of C γ –OH is shown to occur over the alumina site. • ZnAl 2 O 4 , a mixed metal oxide as a new class of catalyst for MeOH activation. • (3 1 1) facet of ZnAl 2 O 4 not only activates but also in some cases dissociates OH as well as CH bond in MeOH. • Spontanous bond dissociation of OH bond on (3 1 1) as well as (2 2 0) surface. • Magnitude of OH bond activation depends upon the active site, effective charge, and proximity with surface oxygen. In this work, we propose a mixed metal oxide as a catalyst and demonstrate it's ability to not only activate the MeOH molecule upon adsorption but also dissociate O H and one of it's C H bonds. MeOH activation is compared on two prominent facets of ZnAl 2 O 4 viz. (2 2 0) and (3 1 1). While spontaneous O H bond dissociation is observed on both facets, C H bond dissociates only on the (3 1 1) surface. Multiple factors like atomic arrangement and steps on the surface, coordination of surface atoms, and their effective charges have a combined effect on MeOH activation. The (3 1 1) surface offers higher catalytic activity in comparison with (2 2 0) surface. Having a stepped surface, availability of multiple sites, and variation in the charge distribution are some of the reasons for better catalytic performance of (3 1 1) facet. Effect of orientation of MeOH with respect to the surface adds both, information and complexity to the problem. Observations pertinent to understanding this effect are also reported. A detailed analysis of atomic arrangement on the two surfaces provides a rationale as to why MeOH gets dissociated spontaneously on the mixed metal oxide. Introduction La maladie cœliaque (MC) est une maladie inflammatoire dysimmunitaire de l’intestin grêle survenant chez des individus génétiquement prédisposés. Il s’agit d’une atteinte fréquente, bien que probablement sous-estimée dû à l’existence non négligeable de présentations atypiques, silencieuses et latentes. L’objectif de ce travail est d’étudier le profil clinique, évolutif et pronostique de la maladie cœliaque. Patients et méthodes Il s’agit d’une étude descriptive et rétrospective colligeant tous les cas de MC hospitalisés à notre service entre 1991 et 2017. Un recueil des données épidémiologiques, clinicobiologiques, endoscopiques, histologiques et évolutives a été réalisé. Résultats Notre série est représentée par 80 patients d’âge moyen de 35 ans avec une manifeste prédominance féminine (80 %) (sex-ratio F/H : 4,3). La moyenne d’âge au diagnostic de MC était de 25 ans (3–58). Une MC chez un apparentés du premier degré était retrouvée dans 9 cas (11 %). Les signes les plus révélateurs de la maladie étaient digestifs dans 72 %. Un tableau typique fait de diarrhée chronique avec un syndrome de malabsorption était présent seulement chez 35 %. Une anémie ferriprive était objectivée dans 72 % des cas, elle était isolée et révélatrice de la maladie dans 20 %. Un retard staturo-pondéral était noté dans 15 %. Dans deux cas la MC était révélée par des complications : dans le premier le diagnostic était dans le cadre d’un bilan étiologique d’une thrombose porte chez une patiente de 35 ans, dans le second il s’agissait d’une cirrhose cryptogénique inaugurée par une hémorragie digestive haute. Sur le plan endoscopique les aspects les plus observés étaient à type de : mosaïque (41 %), plis duodénaux crénelés (13 %), atrophie et diminution de la hauteur des plis (7 %). La FODG était normale dans 12 % des cas. Sur le plan histologique une atrophie villositaire totale était retrouvée dans 62 % des cas ( n = 50). Une infection à HP était présente chez 11 patients (13 %). L’association à d’autres maladies auto-immune était notée dans 20 % : 3 cas d’hypothyroïdie, 7 cas de diabète de type 1, 2 cas d’hépatite auto-immune, 1 cas d’anémie de Biermer, 1 cas de cholangite biliaire primitive, 1 cas de syndrome de Sjögren et 1 cas d’urticaire chronique. Une coloscopie avec biopsies a été faite chez 59 patients (72 %). Elle objectivait une colite microscopique dans 10 % des cas : colite collagène ( n = 6) et colite lymphocytaire ( n = 2). Sous régime sans gluten, une rémission clinique et biologique était rapidement notée chez 27 patients. La normalisation de l’aspect endoscopique et anatomopathologique a été notée chez seulement 4 patients. Une seule complication néoplasique a été notée dans notre série. Il s’agissait d’un adénocarcinome duodénal. Conclusion Bien qu’habituellement révélée par un tableau d’entéropathie classique, la maladie cœliaque aborde des phénotypes de plus en plus atypiques. Le diagnostic repose sur les biopsies duodénales couplées à la sérologie. Le régime sans gluten à vie, actuellement seul traitement efficace, reste indispensable pour prévenir les complications nutritionnelles, auto-immunes, thrombotiques et néoplasiques.
