This study evaluates the enhancement of planning automation in radiation oncology through the integration of physician-specific preferences for organ at risk (OAR) prioritization into an automated planning framework. By combining physician-defined OAR dose goals with multi-criterial driven automated treatment planning, the objective is to refine plan quality and physician satisfaction, ensuring patient-specific considerations are forefront. The study introduces an advanced automation solution that incorporates a prescription device (HRO) to input physician prescription and OAR preferences, connected to an existing planning automation engine utilizing a treatment planning software through API integration. Focusing on head and neck cancer plans, the method involves analyzing the automated planning system's efficacy in optimizing dose distribution in line with physician-set OAR priorities. The study evaluated plans in a network of 40 treatment centers, processing over 20,000 automated treatment plans annually. The system's performance was evaluated based on compliance to physician-defined OAR dose goals, planning efficiency, and the percentage of treatment plans that are approved for treatment without manual user intervention. The analysis revealed that the system successfully influenced the treatment planning system to adjust dose distribution according to physician-defined OAR priorities, with physicians able to layer organs at risk to define prioritization effectively. The system's integration with the prescription device demonstrated a notable improvement in plan quality with 62% of plans showing a reduced Parotid mean dose without compromising adjacent structures. Target coverage improved on 29% of cases while maximum dose decreased on 36% of cases while maintaining greater Parotid sparing. The approach provided physicians with the ability to influence the outcomes of complex dose optimization within a TPS without intervention, reflecting the complex interplay of clinical objectives and individual patient anatomy. The integration of physician preferences into automated planning systems represents a significant advancement in personalized radiation therapy planning. By enabling physicians to dictate OAR priorities directly, the study demonstrates improved alignment of automated plans with individual patient anatomy and clinical goals. This hybrid approach not only enhances plan quality and efficiency but also fosters greater physician satisfaction and confidence in automated systems. The findings advocate for greater provider wellness and more sustainable operating models for physicians through more patient centric automated solutions. [ABSTRACT FROM AUTHOR]

Inconel-600 blocks and stainless steel covers for quartz microbalance crystals from remote corners in the JET-ILW divertor were studied with time-of-flight elastic recoil detection analysis and nuclear reaction analysis to obtain information about the areal densities and depth profiles of elements present in deposited material layers. Surface morphology and the composition of dust particles were examined with scanning electron microscopy and energy-dispersive X-ray spectroscopy. The analysed components were present in JET during three ITER-like wall campaigns between 2010 and 2017. Deposited layers had a stratified structure, primarily made up of beryllium, carbon and oxygen with varying atomic fractions of deuterium, up to more than 20%. The range of carbon transport from the ribs of the divertor carrier was limited to a few centimeters, and carbon/deuterium co-deposition was indicated on the Inconel blocks. High atomic fractions of deuterium were also found in almost carbon-free layers on the quartz microbalance covers. Layer thicknesses up to more than 1 mu m were indicated, but typical values were on the order of a few hundred nm. Chromium, iron and nickel fractions were less than or around 1% at layer surfaces while increasing close to the layer-substrate interface. The tungsten fraction depended on the proximity of the plasma strike point to the divertor corners. Particles of tungsten, molybdenum and copper with sizes less than or around 1 mu m were found. Nitrogen, argon and neon were present after plasma edge cooling and disruption mitigation. Oxygen-18 was found on component surfaces after injection, indicating in-vessel oxidation. Compensation of elastic recoil detection data for detection efficiency and ion-induced release of deuterium during the measurement gave quantitative agreement with nuclear reaction analysis, which strengthens the validity of the results. Neuropathy (nerve dysfunction) is a common problem of people with human immunodeficiency virus (HIV) infection, the cause of AIDS. Resulting problems can include dementia, due to brain involvement, and sensory or muscle dysfunction, due to involvement of peripheral nerves. Chronic inflammatory demyelinating polyneuropathy (IDP), in which inflammation and loss of the myelin (fatty insulation which facilitates nerve conduction) cover from multiple peripheral nerves, has been found in adults without other HIV infection symptoms. A case is described of a six-year-old child with HIV infection who developed IDP. The boy had pain in the right leg with limping and stumbling four days, diarrhea and anorexia (loss of appetite) prior to admission to the hospital. The patient was diagnosed with HIV infection the previous year and was treated with intravenous immune globulin (antibody-containing fraction from blood) for mild pneumonia. After admission, the patient quickly developed other problems indicative of nerve dysfunction, including decreased eye function, poor gag reflex, lability and irritability. The cerebrospinal fluid had viral molecules, indicating infection of central nervous system tissue. This was treated with blood filtration, immune globulin, steroids, and zidovudine. Tissue analysis suggested a chronic loss and repair of nerve fibers. Studies three months after admission showed improvement in muscle function, and within eight months, muscle function recovered, so that the patient could walk without assistance. It was unclear whether the CNS infection and IDP were linked. The study suggests that symptoms of peripheral neuropathy in children should alert physicians to the possibility of HIV infection for those at risk. Automation of radiotherapy treatment planning improves efficiency and consistency, while reducing planning time and errors. The objective of this study was to validate an Iterative Optimization Engine (IOE) within an existing automated IMRT/VMAT planning framework. The IOE was designed to reduce remaining manual intervention within the automation framework through measuring and codifying common user intervention within a commercial planning system. The IOE was developed for external beam IMRT/VMAT treatment planning on the Monaco Treatment Planning System. The IOE was built on an existing automation framework, utilizing the Application Programming Interface (API) to create completely automated treatment plans. A network of 40 centers in Australia evaluated automated treatment plans for head and neck disease sites where users decided manual intervention was required post automated planning to achieve clinician approval. The modifications to automated plans were recorded, analyzed, and codified into the API to remove the requirement for manual intervention. A subset of the automated plans was then retrospectively processed by the IOE with resulting plans being scored in three categories of 1) superior, 2) equivalent and 3) inferior based on DVH assessment with the original clinician approved plan as the baseline. The automation framework generated 546 head and neck plans from January 1 to February 1, 2023, of which 45% required manual intervention to achieve dosimetric criteria. After being processed by the IOE, 86% of plans showed equivalent or superior coverage and maximum dose, and 95% of plans demonstrated equivalent homogeneity or improved homogeneity. Multi-target plans showed equivalent or improved target dose for 67% of intermediate dose targets and 39% of low dose targets when multiple targets were treated simultaneously. Analysis of organs at risk showed 38% of plans with reduced Parotid mean dose, 92% improved Larynx mean dose, 43% reduced Spinal Cord maximum dose, 57% decreased Brainstem maximum dose, 85% reduced Oral Cavity mean dose and 56% reduced Pharynx mean dose. The addition of an IOE achieved a clinical improvement to target and OAR metrics in the assessed clinical plans. Using a data set corresponding to an integrated luminosity of 3 fb−1, collected by the LHCb experiment in pp collisions at centre-of-mass energies of 7 and 8 TeV, the effective lifetime in the Bs0→J/ψη decay mode, τeff, is measured to be τeff=1.479±0.034 (stat)±0.011 (syst) ps. Assuming CP conservation, τeff corresponds to the lifetime of the light Bs0 mass eigenstate. This is the first measurement of the effective lifetime in this decay mode.

Reports on the long-term outcomes and immunosuppressive regimens of multiorgan transplant patients are limited. Here, we describe a patient with cystic fibrosis complicated by multiorgan failure who was successfully treated with combined liver lung transplant and delayed kidney transplant, resulting in excellent outcomes. Delayed kidney transplant was done to reduce the operative stress of a single procedure, giving time for adequate resuscitation and weaning from vasopressors. Our patient's postoperative course was complicated by post-transplant lymphoproliferative disease, which was successfully treated with rituximab and reduced dosages of immunosuppression. • Cystic fibrosis can manifest as end-stage damage in multiple organs. • Combined lung-liver-kidney transplant is a potential treatment for cystic fibrotic patients. • The reported patient underwent a triple organ transplant with good outcomes. • PTLD was the only complication that resolved 7 months postoperatively. Return of materials from the Hubble Space Telescope (HST) during shuttle orbiter service missions has allowed inspection of large numbers of hypervelocity impact features from long exposure at about 615 km altitude in low Earth orbit (LEO) [1,2]. Here we describe the application of advanced X-ray microanalysis techniques on scanning electron microscopes (SEM), microprobes and a 2 MV Tandetron, to nearly 400 impacts on the painted metal surface of the Wide Field and Planetary Camera 2 (WFPC2) radiator shield [3,4]. We identified artificial Orbital Debris (OD) and natural Micrometeoroid (MM) origins for small [5] and even for larger particles [6], which usually may leave little or no detectable trace on HST solar arrays, as they penetrate through the full cell thickness [2,7]. Consumption of central nervous system tissue (CNST) from cattle with bovine spongiform encephalopathy (BSE) is thought to cause the human neurological disease, variant Creutzfeldt-Jacob disease. To identify points of cross-contamination of beef carcasses with CNST, 55 young beef cattle were slaughtered and processed through a federally inspected multispecies abattoir. The objectives of this study were to evaluate CNST spread following the placement of a plug in the penetration site of the skull after captive bolt stunning, to evaluate cross-contamination of carcasses before and after splitting, to compare the effects of hot water pasteurization (84°C for 10 s) versus cold water wash (10°C for 30 s) for reducing CNST on the carcass, and to examine other possible sources of cross-contamination in the abattoir. Results indicated that the use of a plastic plug reduced CNST contamination near the bolt penetration site. This study also confirmed that carcass splitting resulted in an increase in CNST contamination at various areas of the carcass. Hot water pasteurization appeared to be an effective means of removing CNST contamination from carcasses in most of the areas sampled. [ABSTRACT FROM AUTHOR]

Automation of radiotherapy treatment planning increases consistency, provides a safety net around error prevention, and reduces planning time. The objective of this study was to validate a scalable framework to automate the creation of VMAT and IMRT treatment plans using commercial planning systems. A total of 35 centers across Australia were used for the study and the disease sites consisted of Urinogenital, Lung, Breast, Head and Neck, Neurological, GI and Gynecological. A scalable framework for external beam treatment planning was implemented for two commercial treatment planning systems (TPS), Eclipse (Varian Medical Systems, Palo Alto, CA) and Monaco (Elekta AB, Stockholm, Sweden). The framework received CT images, target volumes, normal tissue structures, and prescription data from dosimetrists. Information was parsed into the system, validated against configuration settings, the desired treatment protocol template selected, optimization parameters adjusted, and dose calculation completed. The framework controlled the TPS through the Application Programming Interface (API). The dosimetry team evaluated each automated plan by comparison of the plan metrics to the prescription and assessment of the dose distribution. The plans were scored in three areas: 1) clinician approved with no intervention, 2) minimal intervention with minor adjustments, and 3) manual replanning. Since Aug. 2021, 4972 plans (36% of total plans) have been successfully processed across 35 treatment centers with 111 clinicians. 487 plans were rejected by the framework due to error. The system automated plans for GUI (1822), Lung (629), Breast (745), Head and Neck (65), Neurological (96), GI (568) and Gynecological (163). Out of these plans, 23% were clinician approved with no user intervention, 41% required minimal intervention to achieve clinician approval, and 36% required additional optimization structures and/or cycles of optimization to achieve clinician approval. The average time between clinician target volume delineation and plan approval reduced by 25% from 1.6 to 1.2 days. In Feb. 2022, the framework successfully processed 970 plans (41% of total plans), rejecting 126 (5%) due to error. Evaluation between systems showed 27% of Eclipse plans and 43% of Monaco plans were processed successfully. Of these plans, 19% (Eclipse) and 23% (Monaco) required no intervention while 25% (Eclipse) and 46% (Monaco) required minimal intervention. Analysis of dosimetrist productivity showed an increase of 16% since Aug. 2021 while the average time saved per plan was 85 minutes. Analysis of adverse and near- miss dosimetry related events showed a 6% reduction in reported incidents. The framework for automated planning achieved a 60% clinician approval rate for plans with minimal or no intervention. Automation can result in more consistent planning and timesaving's for expert operators. A 6% reduction in recorded dosimetry related errors was achieved. [ABSTRACT FROM AUTHOR]

OBJECTIVE: Statistical health risk information has proved notoriously confusing and difficult to understand. While past research indicates that presenting risk information in a frequency format is superior to relative risk and probability formats, the optimal characteristics of frequency formats are still unclear. The aim of this study is to determine the features of 1000 person frequency diagrams (pictographs) which result in the greatest speed and accuracy of graphical perception. METHODS: Participants estimated the difference in chance of survival when taking or not taking Drug A, on a pictograph format, varying by mode (one-graph/two-graph), direction (vertical/horizontal), and shading (shaded/unshaded), and their preferences for the different formats. Their understanding of different components of the 1000 person diagram was assessed. Responses were timed and scored for accuracy. RESULTS: Horizontal pictographs were perceived faster and more accurately than vertical formats. Two-graph pictographs were perceived faster than one-graph formats. Shading reduced response time in two-graph formats, but increased response times in one-graph formats. Shaded and one-graph pictographs were preferred. CONCLUSIONS: As shading and one-graph formats were preferred, further clarification as to why shading negatively impacts on response times in the one-graph format is warranted. Abstract: The challenge in Aerospace Engineering, in the next two decades as set by Vision 2020, is to meet the targets of reduction of nitric oxide emission by 80%, carbon monoxide and carbon dioxide both by 50%, reduce noise by 50% and of course with reduced cost and improved safety. All this must be achieved with expected increase in capacity and demand. Such a challenge has to be in a background where the understanding of physics of flight has changed very little over the years and where industrial growth is driven primarily by cost rather than new technology. The way forward to meet the challenges is to introduce innovative technologies and develop an integrated, effective and efficient process for the life cycle design of aircraft, known as systems engineering (SE). SE is a holistic approach to a product that comprises several components. Customer specifications, conceptual design, risk analysis, functional analysis and architecture, physical architecture, design analysis and synthesis, and trade studies and optimisation, manufacturing, testing validation and verification, delivery, life cycle cost and management. Further, it involves interaction between traditional disciplines such as Aerodynamics, Structures and Flight Mechanics with people- and process-oriented disciplines such as Management, Manufacturing, and Technology Transfer. SE has become the state-of-the-art methodology for organising and managing aerospace production. However, like many well founded methodologies, it is more difficult to embody the core principles into formalised models and tools. The key contribution of the paper will be to review this formalisation and to present the very latest knowledge and technology that facilitates SE theory. Typically, research into SE provides a deeper understanding of the core principles and interactions, and helps one to appreciate the required technical architecture for fully exploiting it as a process, rather than a series of events. There are major issues as regards to systems approach to aircraft design and these include lack of basic scientific/practical models and tools for interfacing and integrating the components of SE and within a given component, for example, life cycle cost, basic models for linking the key drivers. The paper will review the current state of art in SE approach to aircraft design and identify some of the major challenges, the current state of the art and visions for the future. The review moves from an initial basis in traditional engineering design processes to consideration of costs and manufacturing in this integrated environment. Issues related to the implementation of integration in design at the detailed physics level are discussed in the case studies. [Copyright &y& Elsevier]

Abstract: Fuselage panels are commonly fabricated as skin-stringer constructions, which are permitted to locally buckle under normal flight loads. The current analysis methodologies used to determine the post-buckling response behaviour of stiffened panels relies on applying simplifying assumptions with semi-empirical/empirical data. Using the Finite Element method and employing non-linear material and geometric analysis procedures it is possible to model the post-buckling behaviour of stiffened panels without having to place the same emphases on simplifying assumptions or empirical data. Previous work has demonstrated that using a commercial implicit code, the Finite Element method can be used successfully to model the post-buckling behaviour of flat riveted panels subjected to uniform axial compression. This paper expands the compression modelling procedures to flat riveted panels subjected to uniform shear loading, investigating element, mesh, idealisation and material modelling selection, with results validated against mechanical tests. The work has generated a series of guidelines for the non-linear computational analysis of flat riveted panels subjected to uniform shear loading, highlighting subtle but important differences between shear and compression modelling requirements. The Sam and Ann Barshop Center for Longevity and Aging Studies is a focal point for advanced research designed exclusively to study the genes involved in aging and the diseases of aging. The research performed at the Barshop Center is based on a solid foundation of nearly twenty-five years of aging research at The University of Texas Health Science Center at San Antonio. Internationally recognized scientists in aging are now leading innovative research programs using state-of-the-art technologies in molecular and cellular biology to explore aging processes at the gene level in the four major programs that comprise the research at the Barshop Center: the Cellular Aging Program, the Invertebrate Aging Program, the Rodent Models of Aging Program, and the Human Genetics of Aging Program. The researchers involved in these programs share a common purpose in an atmosphere of collaboration to gain the scientific insights necessary to understand the molecular basis of aging. Their long-term goal is to gain the knowledge that will give rise to the development of interventions that retard or arrest the debilitating conditions associated with aging. February or March 2003 marks the groundbreaking for the first building of Barshop Center''s new stand-alone facility. Abstract: Background: Plantar Heel Pain Syndrome (PHPS) describes centralised plantar heel pain and tenderness. It can account for up to 15% of referrals to clinicians involved in the treatment of foot pain. Objective: To compare tibial nerve block, local infiltration with steroid or both combined in the treatment of PHPS. Methods: Patients with PHPS were randomly assigned to three treatment groups: Group 1—steroid injection to heel; Group 2—local anaesthetic block to tibial nerve; Group 3—both procedures. Pain visual analogue scale (VAS) was measured at baseline and after 1, 6 and 26 weeks. Heel tenderness index (HTI) was measured at baseline and after 6 weeks. The patient rated their discomfort from the injection(s) using a VAS. Results: Forty-five patients (27 female) were recruited, 14 in Group 1, 12 in Group 2 and 19 in Group 3. Median age was 55, disease duration was 10 months and baseline pain VAS was 7.0cm. All groups experienced a sustained improvement in pain VAS between baseline and weeks 1, 6 and 26 (all p <0.0001). Group 1 reported significantly lower pain VAS that those in Group 2 (p <0.01) or Group 3 (p <0.05) at week 6. Group 2 found the procedure less uncomfortable than Group 1 (p <0.01). The HTI was significantly higher in Group 2 at 6 weeks compared to Group 1 (p <0.005) and Group 3 (p <0.05). Conclusions: This study suggests that the natural history of PHPS following an injection is encouraging, that a tibial nerve block reduces the discomfort of the procedure, that a steroid injection to the heel may accelerate improvement and that clinicians should consider a combination of both strategies.
