COVID-19 vaccine uptake remains low for US adolescents and contributes to excess morbidity and mortality. Most research has assessed parental intention to vaccinate their children. We explored differences between vaccine-acceptant and vaccine-hesitant unvaccinated US adolescents using national survey data. A nonprobability, quota-based sample of adolescents, aged 13–17 years, was recruited through an online survey panel in April 2021. One thousand nine hundred twenty seven adolescents were screened for participation and the final sample included 985 responses. We assessed responses from unvaccinated adolescents (n = 831). Our primary measure was COVID-19 vaccination intent ("vaccine-acceptant" defined as "definitely will" get a COVID-19 vaccine and any other response classified as "vaccine-hesitant") and secondary measures included reasons for intending or not intending to get vaccinated and trusted sources of COVID-19 vaccine information. We calculated descriptive statistics and chi-square tests to explore differences between vaccine-acceptant and vaccine-hesitant adolescents. Most (n = 831; 70.9%) adolescents were hesitant, with more hesitancy observed among adolescents with low levels of concern about COVID-19 and high levels of concern about side effects of COVID-19 vaccination. Among vaccine-hesitant adolescents, reasons for not intending to get vaccinated included waiting for safety data and having parents who would make the vaccination decision. Vaccine-hesitant adolescents had a lower number of trusted information sources than vaccine-acceptant adolescents. Differences identified between vaccine-acceptant and vaccine-hesitant adolescents can inform message content and dissemination. Messages should include accurate, age-appropriate information about side effects and risks of COVID-19 infection. Vaccinating adolescents against COVID-19 while avoiding delays in other routine vaccination is paramount to protecting their health. Our objective was to assess parental preferences to have their adolescents aged 12–17 years receive COVID-19 and other routine vaccines at the same time. An online survey with a national, quota-based cross-sectional sample of United States parents of youth aged 12–17 years was fielded in April 2021 ahead of FDA's Emergency Use Authorization of COVID-19 vaccine for age 12–15 years. Parents were asked about their willingness to have their adolescents aged 12–17 years receive both COVID-19 and routine vaccines at the same visit and/or to follow their provider's recommendation. Predictors included demographic characteristics, being behind on routine vaccines, and perceived risks and benefits. Few parents were willing to have their adolescent receive COVID-19 and routine vaccines at the same visit (10.6%) or follow the healthcare provider's recommendation (18.5%). In multivariate analyses, demographic characteristics had no effect on willingness, reporting that the adolescent was behind on routine vaccines correlated with decreased willingness (p =.004). Greater concern about the adolescent getting COVID-19 (p =.001), lower concern about the adolescent having side effects from the COVID-19 vaccine (p =.013), and more positive feelings about vaccines in general (p =.002) were associated with higher willingness. Few parents would prefer to have their adolescents receive COVID-19 and routine vaccines at the same visit. Understanding what drives willingness to receive all recommended vaccines in the context of the COVID-19 pandemic could inform policies to optimize adolescent vaccination. [ABSTRACT FROM AUTHOR]

The purpose of this study was to establish whether an artificially intelligent (AI) system can be developed to automate stress echocardiography analysis and support clinician interpretation. Coronary artery disease is the leading global cause of mortality and morbidity and stress echocardiography remains one of the most commonly used diagnostic imaging tests. An automated image processing pipeline was developed to extract novel geometric and kinematic features from stress echocardiograms collected as part of a large, United Kingdom-based prospective, multicenter, multivendor study. An ensemble machine learning classifier was trained, using the extracted features, to identify patients with severe coronary artery disease on invasive coronary angiography. The model was tested in an independent U.S. study. How availability of an AI classification might impact clinical interpretation of stress echocardiograms was evaluated in a randomized crossover reader study. Acceptable classification accuracy for identification of patients with severe coronary artery disease in the training data set was achieved on cross-fold validation based on 31 unique geometric and kinematic features, with a specificity of 92.7% and a sensitivity of 84.4%. This accuracy was maintained in the independent validation data set. The use of the AI classification tool by clinicians increased inter-reader agreement and confidence as well as sensitivity for detection of disease by 10% to achieve an area under the receiver-operating characteristic curve of 0.93. Automated analysis of stress echocardiograms is possible using AI and provision of automated classifications to clinicians when reading stress echocardiograms could improve accuracy, inter-reader agreement, and reader confidence. • We examine network preferences after an acquisition. • Initially, there are tendencies for reciprocity and preferential attachment. • There is also a tendency for the acquired firm to make ties. • Over time these preference diminish. In this study we develop a model to explain the dynamics of advice seeking after an acquisition. We build on a theory of advice seeking that draws from prospect theory and expectancy theory. We theorize that immediately after an acquisition there is uncertainty about who knows what, but over time individuals become more aware of the expertise within the organization and they change their advice networks based upon this increased awareness. Our model examines four micro-processes of advice seeking: reciprocity, preferential attachment, transitivity, and legacy-firm tie preferences. To test our hypotheses we use post-acquisition data over four time periods in a recruitment consulting firm. Our longitudinal analysis uses a stochastic actor-orientated model and our results indicate that immediately after the acquisition individuals have a tendency to seek advice based upon reciprocity and preferential attachment. However, over time these tendencies diminish. Surprisingly, transitivity does not play a significant role, which suggests that other micro-processes such as reciprocity are dominant. In addition, individuals in the acquired firm have a tendency to make more ties and there is a preference for same firm ties in both legacy firms, with the tendency being higher in the acquired firm. Our findings add to theories on the process of advice seeking under conditions of uncertainty, on knowledge transfer processes in mergers and acquisitions, and the knowledge based view of the firm. Background Respiratory viral infection in early childhood, including that from respiratory syncytial virus (RSV), has been previously associated with the development of asthma. Objective We aimed to determine whether ex vivo RSV infection of bronchial epithelial cells (BECs) from children with asthma would induce specific gene expression patterns and whether such patterns were associated with lung function among BEC donors. Methods Primary BECs from carefully characterized children with asthma (n = 18) and matched healthy children without asthma (n = 8) were differentiated at an air-liquid interface for 21 days. Air-liquid interface cultures were infected with RSV for 96 hours and RNA was subsequently isolated from BECs. In each case, we analyzed gene expression using RNA sequencing and assessed differences between conditions by linear modeling of the data. BEC donors completed spirometry to measure lung function. Results RSV infection of BECs from subjects with asthma, compared with uninfected BECs from subjects with asthma, led to a significant increase in expression of 6199 genes. There was significantly greater expression of 195 genes in BECs from children with asthma and airway obstruction (FEV 1 /forced vital capacity < 0.85 and FEV 1 < 100% predicted) than in BECs from children with asthma without obstruction, or in BECs from healthy children. These specific genes were found to be highly enriched for viral response genes induced in parallel with types I and III interferons. Conclusions BECs from children with asthma and with obstructive physiology exhibit greater expression of types I and III interferons and interferon-stimulated genes than do cells from children with normal lung function, and expression of interferon-associated genes correlates with the degree of airway obstruction. These findings suggest that an exaggerated interferon response to viral infection by airway epithelial cells may be a mechanism leading to lung function decline in a subset of children with asthma. The gold standard tool for measuring periprosthetic bone mineral density (BMD) is dual-energy X-ray absorptiometry (DXA). However, resolution of the method is limited due to the aggregation of pixel data into large regions of interest for clinical and statistical analysis. We have previously validated a region-free analysis method (DXA-RFA) for quantitating BMD change at the pixel level around femoral prostheses. Here, we applied the DXA-RFA method to the pelvis, and quantitated its precision in this setting using repeated DXA scans taken on the same day after repositioning in 29 patients after total hip arthroplasty. Scans were semiautomatically segmented using edge detection, intensity thresholding, and morphologic operations, and elastically registered to a common template generated through generalized Procrustes analysis. Pixel-wise BMD precision between repeated scans was expressed as a coefficient of variation %. Longitudinal BMD change was assessed in an independent group of 24 patients followed up for 260 wk. DXA-RFA spatial resolution of 0.31 mm 2 provided approximately 12,500 data points per scan. The median data-point precision was 17.8% (interquartile range 14.3%–22.7%). The anatomic distribution of the precision errors showed poorer precision at the bone borders and superior precision to the obturator foramen. Evaluation of longitudinal BMD showed focal BMD change at 260 wk of −26.8% adjacent to the prosthesis-bone interface (1% of bone map area). In contrast, BMD change of +39.0% was observed at the outer aspect of the ischium (3% of bone map area). Pelvic DXA-RFA is less precise than conventional DXA analysis. However, it is sensitive for detecting local BMD change events in groups of patients, and provides a novel tool for quantitating local bone mass after joint replacement. Using this method, we were able to resolve BMD change over small areas adjacent to the implant-bone interface and in the ischial region over 260 wk after total hip arthroplasty. When developing computational models to analyze the tradeoffs between climate risk management strategies (i.e., mitigation, adaptation, or geoengineering), scientists make explicit and implicit decisions that are influenced by their beliefs, values and preferences. Model descriptions typically include only the explicit decisions and are silent on value judgments that may explain these decisions. Eliciting scientists’ mental models, a systematic approach to determining how they think about climate risk management, can help to gain a clearer understanding of their modeling decisions. In order to identify and represent the role of values, beliefs and preferences on decisions, we used an augmented mental models research approach, namely values-informed mental models (ViMM). We conducted and qualitatively analyzed interviews with eleven climate risk management scientists. Our results suggest that these scientists use a similar decision framework to each other to think about modeling climate risk management tradeoffs, including eight specific decisions ranging from defining the model objectives to evaluating the model’s results. The influence of values on these decisions varied between our scientists and between the specific decisions. For instance, scientists invoked ethical values (e.g., concerns about human welfare) when defining objectives, but epistemic values (e.g., concerns about model consistency) were more influential when evaluating model results. ViMM can (i) enable insights that can inform the design of new computational models and (ii) make value judgments explicit and more inclusive of relevant values. Rising atmospheric carbon dioxide (CO 2 ) concentrations are fueling anthropogenic climate change. Geologic sequestration of anthropogenic CO 2 in depleted oil reservoirs is one option for reducing CO 2 emissions to the atmosphere while enhancing oil recovery. In order to evaluate the feasibility of using enhanced oil recovery (EOR) sites in the United States for permanent CO 2 storage, an active multi-stage miscible CO 2 flooding project in the Permian Basin (North Ward Estes Field, near Wickett, Texas) was investigated. In addition, two major natural CO 2 reservoirs in the southeastern Paradox Basin (McElmo Dome and Doe Canyon) were also investigated as they provide CO 2 for EOR operations in the Permian Basin. Produced gas and water were collected from three different CO 2 flooding phases (with different start dates) within the North Ward Estes Field to evaluate possible CO 2 storage mechanisms and amounts of total CO 2 retention. McElmo Dome and Doe Canyon were sampled for produced gas to determine the noble gas and stable isotope signature of the original injected EOR gas and to confirm the source of this naturally-occurring CO 2 . As expected, the natural CO 2 produced from McElmo Dome and Doe Canyon is a mix of mantle and crustal sources. When comparing CO 2 injection and production rates for the CO 2 floods in the North Ward Estes Field, it appears that CO 2 retention in the reservoir decreased over the course of the three injections, retaining 39%, 49% and 61% of the injected CO 2 for the 2008, 2010, and 2013 projects, respectively, characteristic of maturing CO 2 miscible flood projects. Noble gas isotopic composition of the injected and produced gas for the flood projects suggest no active fractionation, while δ 13 C CO 2 values suggest no active CO 2 dissolution into formation water, or mineralization. CO 2 volumes capable of dissolving in residual formation fluids were also estimated along with the potential to store pure-phase supercritical CO 2 . Using a combination of dissolution trapping and residual trapping, both volumes of CO 2 currently retained in the 2008 and 2013 projects could be justified, suggesting no major leakage is occurring. Scenario planning traditionally relies on qualitative methods to choose its scenarios. Recently, quantitative decision support tools have also begun to facilitate such choices. This study uses behavioral experiments and structured decision-maker interviews to evaluate the results of “scenario discovery,” a quantitative method that defines scenarios as sets of future states of the world in which proposed policies fail to meet their goals. Statistical cluster-finding and principal component algorithms applied to large databases of computer simulation model results then help users to identify such scenarios. The two experiments examine the results of this process and demonstrate a user preference for increased accuracy and simplicity achieved through rotating the space of uncertain model input parameters, but primarily when the rotated parameters are conceptually similar. Interviews with experts suggest utility for both qualitatively- and quantitatively-derived scenarios. The former were easier to understand and had the most utility for scoping. The latter were perceived as containing more relevant information and having more utility for understanding tradeoffs and making choices among them. Overall, this study suggests the value of quantitative tools for facilitating scenario choice, while also highlighting the importance of formal evaluation in judging the utility of new methods for decision support. Purpose: To determine the reliability of measuring ulnar variance on lateral wrist radiographs and to compare this technique with previously described methods. Methods: Ulnar variance was measured in 100 normal wrist radiographs using the methods of perpendiculars, central reference point, and the lateral radiograph by 3 surgeons on 2 occasions. Intraobserver repeatability and agreement between raters and methods were assessed and compared. Results: Intra- and interobserver reliability and agreement were both excellent using all 3 methods within a ± 1.0-mm cutoff. However, there was substantial pairwise disagreement in measures of ulnar variance between all 3 methods. Conclusions: This study demonstrates that, for measurement of ulnar variance, the methods of perpendiculars, central reference point, and lateral radiographic measurement each have clinically acceptable intraobserver repeatability and interobserver agreement. Despite their independent reliability, each method of radiographic determination of ulnar variance had considerable disagreement with the other methods, indicative of inherent inaccuracies in the techniques. The lateral radiograph uniquely allows for visualization of the amount of ulnar head protruding proximal or distal to the concave lunate facet and allows for a rapid estimation of pronosupination, which is known to affect ulnar variance. Clinical relevance: Determination of ulnar variance can be an important component of surgical decision making in various pathological conditions of the hand and wrist. Traditionally, it has been measured through methods using the posteroanterior wrist radiograph, but there are potential shortcomings with these methods, and use of the lateral radiograph may provide a more clinically relevant picture of ulnar variance. Abstract: Sediments samples of the Dobczyce Reservoir (South Poland) were taken in three successive years (2004–06) at 17 points, analyzed for 12 elements by atomic absorption spectrometry. Substantial variations in elemental composition due to hydrological and environmental reasons were found. The use of statistical and chemometric tools enabled objective and effective analysis of the data obtained, both in case of long term observations as well as in terms of interpretation of incidental changes in metal concentrations. The proposed methodology can be useful in: a) dividing monitored territories (lakes, rivers, catchments) into similar and dissimilar areas; b) distinguishing between typical and non-typical contaminants distribution; c) reducing the number of essential analytes and sampling points that should be controlled in the future, without loss of important environmental information. Purpose: Although adolescents'' expectations are accurate or moderately optimistic for many significant life events, they greatly overestimate their chances of dying soon. We examine here whether adolescents'' mortality judgments are correlated with their perceptions of direct threats to their survival. Such sensitivity would indicate the importance of ensuring that adolescents have accurate information about those threats, as well as the psychological support needed to deal with them. Methods: Data from two separate studies were used: a national sample of 3,436 14–18-year-old adolescents and a regional sample of 124 seventh graders and 132 ninth graders, 12–16 years old. Participants were asked about their chance of dying in the next year and before age 20, and about the extent of various threats to their physical well-being. Results: Adolescents in both samples greatly overestimated their chance of dying. Those mortality estimates were higher for adolescents who reported direct threats (e.g., an unsafe neighborhood). Thus, adolescents were sensitive to the relative size of threats to their survival, but not to the implications for absolute risk levels. Conclusions: Contrary to the folk wisdom that adolescents have a unique sense of invulnerability, the individuals studied here reported an exaggerated sense of mortality, which was highest among those reporting greater threats in their lives. Abstract: Cystic neck swellings in the neck arise from a diverse range of tissues and pathological disorder. The approach to diagnosis relies on a thorough history and an appropriate examination including the internal ENT to exclude an occult primary malignancy. The typical feature of fluctuance is often not elicited because the contents of the cyst can be under tension and transillumination is seldom performed these days. Aspiration and ultrasonography are usual but other imaging such as CT and MRI scanning can be indicated. Definitive treatment is usually surgical excision but with benign disease repeat aspiration can be considered. No intervention can be an option if there is significant co-morbidity, absence of functional or cosmetic impairment or there is reluctance by the patient to undergo surgery. [Copyright &y& Elsevier]

Abstract: Neck lumps are a challenging proposition for any surgeon. They also form a substantial component in clinical examinations for higher surgical diplomas. This is because many different diagnostic possibilities exist in a relatively small area. It is a good presumption, especially in an adult, that a neck lump represents malignant disease until proven otherwise which might be arising from an occult primary in the nose, throat or oral cavity. Lesions in these areas might not be readily visualized and so part of the assessment should include an ENT opinion. A thorough history and examination provide important clues to elucidating the cause. This provides for directed imaging and finally biopsy/excision if required. Excision of a malignant lump in isolation can in some circumstances reduce patient survival if any associated primary is not recognized and dealt with, but worse is an incisional biopsy which should only be undertaken as a very last resort as there is a risk of seeding tumour through the neck. A good working knowledge of the anatomy of the neck, a systematic approach to examination of the area and an appreciation of the possible differential diagnoses are vital to acquiring a safe, efficient and effective management strategy. • A surrogate modeling framework is developed for building stock energy models. • The surrogate model has hourly time resolution for building load profile studies. • Advanced data engineering and HPC workflow provide high computational efficiency. • Feature engineering and deep learning techniques are applied in the surrogate model. • Two case studies demonstrate the effectiveness of the framework. Surrogate modeling can play a key role in reducing high computational burdens for large-scale physics-based modeling and uncertainty quantification. With the rapid development of large-scale building stock energy modeling, surrogate modeling has also begun to be widely applied in this field; however, most existing surrogate models lack hourly time resolution for regional-scale modeling, which is essential for understanding building demand profiles and grid impacts. Further, there is generally a lack of necessary data and feature engineering frameworks specific to building modeling for efficiently managing large datasets and complex computations. This paper proposes a modeling framework for large-scale (city-/region-scale), high-resolution, high-fidelity surrogate building stock energy models. Our developed framework consists of six modules: (1) building stock energy modeling (ComStockTM and ResStockTM), (2) data engineering for large simulation data, (3) high performance computing workflow, (4) feature engineering, (5) machine learning model development, and (6) model performance evaluation. Two case studies apply the developed framework in both residential and commercial building stock analysis to demonstrate its computational efficiency and surrogate modeling accuracies. Abstract: As an occupation, firefighting is replete with images of maleness operating around a series of highly masculinised codes and values most notably comprising: risk/danger, heroism, fearlessness/courage, physicality, and bodily strength. This qualitative study of the UK Fire Service seeks to uncover the ways in which these masculine codes and values were evident amidst the daily working lives of a group of full-time, male firefighters. Placing respondent views at the centre of the research, findings suggest that occupational identities were based primarily upon notions of emotional strength, physical and technical competence and collective understandings of risk and responsibility. A commitment to group solidarity was also central to the masculine identities of respondents, with colleagues in administrative and managerial positions being distanced on account of their non-manual occupational roles. Abstract: The attachment mechanism of pandarid adhesion pads is described from observations of their externally ridged structure and internal construction in three species; Pandarus bicolor Leach, 1816, Dinemoura latifolia (Steenstrup and Lutken, 1861) and Echthrogaleus coleoptratus (Guerin-Meneville, 1837). The host''s external skin morphology was also examined, since parasite attachment mechanism and host surface can be considered as components of a single system. The results emphasise the importance of the physical nature of the pad''s surface. This is inferred from the compliance of the cuticle and subsurface structure, and the presence of cuticular ridging. The pads probably prevent pandarids from being dislodged by hydrodynamic drag, by increasing overall adhesion. It is proposed that this is achieved in different ways, by two types of adhesion pad identified here, distinguishable by their external structure and location. Type I pads are suggested to remove interfacial water and increase surface contact by one of two contrasting methods. The ridges may act as tyre treads, by channelling water from the contact surface. Alternatively, the channels between ridges may be hydrophobic and behave as dewetting structures, preventing water from entering in the same way that troughs between surface nodules function to produce superhydrophobicity on lotus leaves. Type I adhesion pads are also suggested to aid attachment by hindering the process of peeling, by which they are thought to be removed by hydrodynamic drag. Type II pads are more likely to function as one-way frictional attachments. Both types of pad appear to be attached passively, since they lack muscles inserting into them. The adhesive mechanism of each, which functions most effectively on hard surfaces, may explain why pads are absent or reduced on pandarids which parasitise the softer, unscaled surfaces of hosts. Pandarids predominantly parasitise the skin and fins of fast-swimming sharks. This may be because the scales are characteristically smaller in these species and are more easily encircled by the primary attachment appendages, the maxillipeds. This is thought to be the first published report to reveal frictional attachment structures from the Crustacea, which have convergently evolved in many terrestrial Arthropoda. Abstract: Phosphorylcholine (PC) moieties were chemically attached to surfaces of polymer microparticles by addition of 2-methylacryloyloxyethyl phosphorylcholine monomer to the seeded, semi-continuous polymerisations of methyl methacrylate (MMA) and butyl acrylate (BA). The surface of the bio-functionalised polymer microparticles was principally characterised using X-ray photoelectron spectroscopy (XPS), dynamic nuclear magnetic resonance (NMR) spectroscopy, scanning electron microscopy (SEM), photon correlation spectroscopy (PCS), acoustophoresis and enzyme-linked immunosorbent assays (ELISA). It was found that the persulphate initiating species are concealed behind the phosphorylcholine containing monomer sequence located on the surface of the microparticles. The combination of analytical techniques showed that the surfaces of the polymer microparticles are extremely mobile above the glass transition temperature of the co-polymer and able to rearrange depending on the environment in which they are placed. This allows the phosphorylcholine moiety to be preferentially expressed at the surface in aqueous media, but not so in the dry state or conditions of ultra-high vacuum. In terms of the nature of the biocompatibility of phosphorylcholine containing polymers, no evidence was found for the irreversible structuring of water molecules around the phosphorylcholine moiety in the wet state. The results of this work suggest that a more likely contributory reason for the protein-resistant nature of phosphorylcholine containing polymers is the mobility of the phosphorylcholine moiety. Increases in biocompatibility correspond with increases in the hydrophilicity of a polymer surface when phosphorylcholine is preferentially expressed. A large free water fraction may be present in the phosphorylcholine containing monomer sequence, as part of a hydrogel structure located at the surface of the polymer microparticles. This, coupled with concomitant modification of the local electrical double-layer very close to the surface may also play a critical role in reducing protein–surface interactions. In this paper, we report the result of a research project investigating social aspects of knowledge sharing and development. Prior research in a consulting firm revealed that respondents recognized five kinds of informational benefits when consulting others: solutions, meta-knowledge, problem reformulation, validation and legitimation. We employed these dimensions in a systematic network analysis of a different sample of people (human resource managers in a large conglomerate), using each of the five benefits as kinds of social relations. Two general research questions guided the analysis. First, how are these relations related to each other (multiplexity)? Do individuals obtain all of the benefits from the same individuals, or do they create balanced portfolios of complementary contacts that provide different benefits? Second, what properties and shapes do the networks induced by these relations form (structure)? What is the basis for who is tied to whom on each relation? The fundamental result emerging from both research questions is that the five relations seem to form a unidimensional scale such that a contact who provides any given benefit is also very likely to provide all the benefits that are lower on the scale. Position on this scale seems to index underlying dimensions of social solidarity rather than individual attributes such as status. Consequently, relations at the end of the scale (e.g. legitimation) were more homophilous and proved to be strongly diagnostic of subgroup boundaries, a fact which could be quite useful in consulting or other applied contexts. This research contributes to the literature on knowledge management by revealing diverse ways that consulting others facilitates knowledge creation and utilization. The research also contributes to social network analysis by examining meanings and relationships among social relations, an area that is understudied. We found that the five benefits, treated as social relations, formed an entailment structure consistent with a Guttman scale. Abstract: We examine whether adolescents’ probability judgments for significant life events are predictive and accurate. We find significant correlations with actual experiences, reported on successive waves of a national survey. Abstract: Formation of a solid solution of a drug in a water-soluble polymer is one of the primary techniques used to improve the dissolution rate and thus bioavailability of a poorly water-soluble drug. Understanding and detecting the state of the drug inside such a polymer matrix is critically important since issues such as drug stability, safety and efficacy can be greatly affected. In this study, two model formulations were prepared containing low and high levels of drug content. The heterogeneity of the formulations has been investigated using a novel nanothermal analysis technique.
