The Scientific Investigation Committee of the American Academy of Restorative Dentistry offers this review of select 2023 dental literature to briefly touch on several topics of interest to modern restorative dentistry. Each committee member brings discipline-specific expertize in their subject areas that include (in order of appearance here): prosthodontics; periodontics, alveolar bone, and peri-implant tissues; dental materials and therapeutics; occlusion and temporomandibular disorders; sleep-related breathing disorders; oral medicine, oral and maxillofacial surgery, and oral radiology; and dental caries and cariology. The authors have focused their efforts on presenting information likely to influence the daily dental treatment decisions of the reader with an emphasis on current innovations, new materials and processes, emerging technology, and future trends in dentistry. Instead, its purpose is to inform and update interested readers and provide valuable resource material for those willing to subsequently pursue greater detail on their own. • Italian consensus about optimization in Nuclear Medicine Therapy. • Optimization must deal with feasibility, limited resources and cost/benefit. • Non-standardized therapies are defined, according to the variability of outcome. • Supremacy of the radiation therapy legislation over the pharmacological laws. • Appeal to EMA to obtain the official acknowledgment of this principle. This recommendation by the Italian Associations of Nuclear Medicine (AIMN) and Medical Physics (AIFM) focuses on the dosimetric optimization of Nuclear Medicine Therapy (NMT) as clearly requested by the article 56 of the EURATOM Directive 2013/59 and its consequent implementation in article 158 in the Italian Law n. 101/2020. However, this statement must deal with scientific and methodological limits that still exist and, above all, with the currently available limited resources. This paper addresses these specific issues. It distinguishes among many possible kinds of NMT. For each type, dosimetric optimization is recommended or considered optional, according to the general criteria adopted in any human choice, i.e. a check of technical feasibility first, followed by a cost/benefit argument. The classification of therapies as standardized or non-standardized is presented. This is based on the complexity of the type of pathology, on the variability of the treatment outcome, and on the risks involved. According to the present document, which was officially delivered to Italian Health Ministry as necessary interpretation of the law, a therapeutic team can, in science and consciousness, overcome the indications of posology, to optimize and tailoring a treatment with dosimetry, on the basis of published national or international data or guidelines, without need of an Ethics Committee approval. Data collected in this way will provide additional evidence about optimal dosimetric reference values. • A novel method for automated human vertebrae segmentation is proposed. • It combines the advantages of Algorithmic Feature Recognition Method and an enhanced Statistical Shape Model. • The method has been tested on L2 human vertebrae of male patients. • It has been stressed with low-density discretized models with missing morphological features. • The results show its robustness and effectiveness over the state-of-the-art. Human vertebrae are analysed and measured for various purposes, including to study anatomy, to diagnose illness, and to evaluate therapies. Achieving an accurate morphological and dimensional characterisation of three-dimensional (3D) vertebrae relies on the precise recognition of their features. Traditionally, these features, lacking defined edges, have been identified manually through a time-consuming, poorly repeatable, and poorly reproducible process. To the authors' knowledge, there is only one method published in the literature able to automatically recognise all the most important vertebral features: the algorithmic feature recognition method (AFRM). It requires a high-density point cloud as input, the presence of all morphological features, even if incomplete, and incurs high computational costs. This research aims to propose an improved version of the AFRM to overcome its limitations. The proposed approach combines the robustness of AFRM to provide the semantic segmentation of a 3D healthy human vertebra with the ability of the enhanced statistical shape model (eSSM) to transfer information among different models. Specifically, AFRM provides the semantic segmentation of the mean shape of the eSSM, while the latter transfers this information to target shapes. The eSSM was developed using 20 training samples of healthy adult male L2 vertebrae. The test samples included five healthy vertebrae and four vertebras with large missing parts. None of the test shapes were included in the training set. The novel approach could accurately recognise morphological features without the constraints that affect the AFRM. The proposed method guarantees reliable and automated segmentation through AFRM, exploiting the eSSM's ability to provide results even when dealing with sparsely populated or partially incomplete target models, significantly reducing the computational load. Crimean-Congo haemorrhagic fever (CCHF) is a widely distributed tick-borne disease. In Spain, the disease has emerged as outbreak associated with high-risk exposures. Our goal was to evaluate the prevalence of antibodies against the CCHF virus (CCHFV) in high-risk contacts. A cross-sectional study was conducted. Three hundred eighty-six high-risk contacts were identified comprising family contacts and hospital workers who had attended the cases. Fifty-seven cases with closer exposure were selected. However, forty-nine cases participated in the study. IgG antibodies were detected by immunoenzymatic techniques. All determinations tested negative for anti-CCHFV IgG antibodies. Most of the responders were women (73.5%), and belong to the intensive care department (53.1%). In relation to other possible sources of exposures, 18.4% travelled to countries with CCHF transmission risk. No CCHF positivity was recorded among selected high-risk contacts. This highlights the importance of standard precautions which might have protected healthcare workers and care providers from CCHF infection. [ABSTRACT FROM AUTHOR]

Abstract Problem There are countless numbers of scientific studies published in countless scientific journals on subjects related to restorative dentistry. Purpose The purpose of this article is to review pertinent scientific studies published in 2017 on topics of interest to restorative dentists. Methods and materials The authors, considered to be experts in their disciplines searched the scientific literature in 7 different areas (prosthodontics, periodontics, dental materials, occlusion and temporomandibular disorders, sleep-disordered breathing, oral medicine and oral and maxillofacial surgery and dental caries). Pertinent articles were either identified and referenced or reviewed. Results A total of 437 articles in 7 disciplines were identified or reviewed. Conclusions An impressive amount of scientific literature related to restorative dentistry was published in 2017. Introduction Micro–computed tomography (μCT) is an imaging modality of growing application in endodontic research because of its nondestructive technology that enables visualization at the micrometer level. In the presence of high-density material, images acquired with μCT may present artifacts. Thus, the aim of this study was to evaluate subjectively and objectively the influence of artifact reduction tools available for μCT image reconstruction to determine whether their use can influence endodontic research outcomes. Methods Ten teeth were scanned in a μCT device, and these images were reconstructed by applying 13 protocols of artifact reduction tools, combining ring artifact reduction (RAR) and beam-hardening artifact reduction (BAR). Images were assessed subjectively (observer's preference) and objectively (root canal surface area and volume). Observer's preference for RAR and BAR protocols was tested by χ 2 . Analysis of variance was used to compare volumes and surface area of root canals on different protocols. Intraobserver and interobserver reproducibility was calculated by the weighted kappa test. Results There was no preference for a particular protocol for BAR ( P = .91) or RAR ( P = .80). There were no statistically significant differences for volume ( P = .999) or surface area ( P = .972) of root canals for all protocols. Conclusions To evaluate root canal volume and root canal surface area, artifact reduction tools applied to μCT images can be used according to the observer's visual preference without influence on objective image analysis. Colorectal cancer is the second leading cause of cancer and often has a fatal course. There are many studies in the literature that have described a close functional relationship between the tumor mass and surrounding tissue, or tumor stroma, which is affected by the continuous metabolic exchange that occurs at the interface between tumor and tissues in contact with it. There is much evidence that the presence of adipose tissue in stroma plays a fundamental role in modulating the tumor microenvironment and promote tumor development, growth, and angiogenesis due to its endocrine characteristics. In this analysis, we have studied the alterations of adipose tissue surrounding colorectal tumors with MRI and optical imaging in vivo techniques to monitor tumor progression and also performed histological and molecular analysis. We detected differences in the principal adipose markers expressed by adipocytes residing around the rectal colon and observed that peritumoral adipose tissue is exposed to a mesenchymal transition process that leads to the acquisition of a less differentiated phenotype of adipocyte that represents the main cellular type present in tumor stroma. The mesenchymal transition correlated with the acquisition of more aggressive tumor phenotype and could represent a valid target for tumor therapy. • Adipose tissue surrounding tumor could influence the tumor growth and dissemination. • Adipose tissue surrounding tumors is characterized by peculiar morphological features. • Multilocular adipocytes surrounding tumors don't express the typical markers of white and brown fat. • Adipose tissue surrounding tumor could represent a new therapeutic target. While some of that information is sound and helpful, much of it is misleading at best. Aim To assess the predictive value of fluorine-18-fluorodeoxyglucose ( 18 F-FDG) positron emission tomography/computed tomography (PET/CT) in early assessing response during neo-adjuvant chemoradiotherapy (CRT) in patients with locally advanced rectal cancer. Materials and methods A systematic review was performed by search of MEDLINE Library for the following terms: “rectal carcinoma OR rectal cancer”, “predictive OR prediction OR response assessment OR response OR assessment”, “early OR ad interim ”, “therapy”, “FDG OR 18 F-FDG”, “PET OR PET/CT”. Articles performed by the use of stand-alone PET scanners were excluded. Results 10 studies met the inclusion criteria, including 302 patients. PET/CT demonstrated a good early predictive value in the global cohort (mean sensitivity = 79%; mean specificity = 78%). SUV and its percentage decrease (response index = RI) were calculated in all studies. A higher accuracy was demonstrated for RI (mean sensitivity = 82%; pooled specificity = 85%) with a mean cut-off of 42%. The mean time point to perform PET scan during CRT resulted to be at 1.85 weeks. Some PET parameters resulted to be both predictive and not statistical predictive of response, maybe due to the small population and few studies bias. Conclusion PET showed high accuracy in early prediction response during preoperative CRT, increased with the use of RI as parameter. In the era of tailored treatment, the precocious assessment of non-responder patients allows modification of the subsequent strategy especially the timing and the type of surgical approach. Abstract: Background: The most common complication of thyroid surgery is hypoparathyroidism, usually temporary. Ischemic injury or parathyroid avulsion are the causes of surgical hypoparathyroidism. We assessed the value of an ultrasound scalpel, the Harmonic Focus® (HF), could prevent surgical-related hypoparathyroidism. Methods: Patients consecutively undergoing total thyroidectomy using the HF from November 2009 to February 2011 were recruited and their clinical characteristics, type of operation, histology, and postoperative calcium levels (normal range: 2.10–2.55 mMol/l) were recorded. The prevalence of transient and permanent hypocalcemia was calculated for benign vs. malignant diseases and compared with a control group of 147 patients treated surgically in 2005 using manual technique. Results: 139 patients treated by the same surgeon with a total thyroidectomy (41.7% for a malignant disease) were considered. Prevalence of transient hypoparathyroidism (THP) was 45.2% and of definitive hypoparathyroidism (DHP) 1.4%. None of the patients with malignancies were hypocalcemic at 1-year follow-up. In the control group THP was found in 51.7% of cases and DHP in 5.4% (p < 0.001). Conclusions: Use of the ultrasound scalpel improved the likelihood of the parathyroid glands preservation during thyroid surgery. Paradoxically, the HF appears to be more effective in treating malignant disease, i.e. when central node dissection is required. [Copyright &y& Elsevier]

Abstract: Aim and methods: Paragangliomas (PGL) are neural crest-derived tumours that are found along the autonomic neural network throughout the body and can be multiple and/or metastatic. Nuclear medicine imaging in combination with conventional imaging is required to fully delineate the extent of the disease. The performance of molecular imaging modalities is widely dependent on tumour biology. Results: In the present paper we discuss the recent publications focused on the role of positron emission tomography (PET) imaging and the relationship between tracer uptake patterns and genetic mutations associated with the disease. Conclusion: Recent advances in genetic and molecular pathogenesis of PGL have allowed for the identification of new molecular diagnostic and therapeutic radiopharmaceuticals tailored to genetic abnormalities. Abstract: Background: To evaluate the role of a multi-imaging PET with 18F-DOPA and 18F-FDG in comparison with conventional imaging (CI) in recurrent medullary thyroid carcinoma (MTC). Methods: 18 MTC patients who had thyroidectomy were included; they presented with elevated and rapidly increasing calcitonin levels during follow up. CI had revealed metastatic deposits in 9 patients. Patients were referred to us for a PET/CT with 18F-DOPA and 18F-FDG. Histologic/cytologic confirmation of recurrent MTC was obtained in at least one PET-positive lesion in all patients. Results: Foci of abnormal uptake were observed in 15 patients at 18F-DOPA and in 11 at 18F-FDG; 8 patients showed the same number of positive lesions with both tracers, 2 showed more lesions on 18F-FDG, 1 was positive at 18F-FDG alone and 5 at 18F-DOPA alone. In 3 patients with a DOPA-positive loco-regional relapse a re-operation with curative intent was offered. SUVmax values were higher for 18F-FDG compared to 18F-DOPA (mean 12.7±4.1 vs. 5.5±2.1, p <0.05). Calcitonin was higher in PET-positive patients compared to PET negative ones, while no significant differences were observed between 18F-DOPA and 18F-FDG positive patients. Conclusions: In MTC patients with rapidly increasing calcitonin levels during follow up, 18F-DOPA has a good sensitivity and a complementary role with 18F-FDG PET/CT in detecting metastatic deposits. In our experience, the sensitivity of a multi-imaging 18F-DOPA &18F-FDG PET/CT approach is greater than that obtained with CI. The higher SUVmax values found with 18F-FDG in some patients may reflect more aggressive tumors. [Copyright &y& Elsevier]

Abstract: Traumatic dental injuries are relatively frequent accidents that typically involve teeth in the maxillary anterior segment. The emergency treatment and the clinical decisions must be efficiently made at the time of injury, and there is a need for long-term follow-up because of the high incidence of complications. The aim of this article was to present the emergency and rehabilitation treatments of a multiple dentoalveolar trauma in the permanent dentition involving different extensions of enamel-dentin crown fracture, pulp exposure, and the avulsion of a canine. The treatment outcomes are reported up to the 4-year follow-up, and the clinical approaches and their rationale are discussed. [Copyright &y& Elsevier]

Abstract: Aims: The aim of this study was to evaluate the sensitivity, specificity and accuracy of axillary ultrasonography (US) and 99mTc-sestamibi scintimammography (SSM) in patients with breast cancer (BC) undergoing curative surgery. Methods: A series of 77 consecutive women (median age 54 years, range 36–70) with primary BC underwent both US and SSM from 2 to 15 (median 4) days prior to curative surgery. The results of imaging studies were compared against the final pathology. Breast-conserving surgery with axillary node (AN) dissection was performed in 49 (63.6%) patients, and modified radical mastectomy in 28 (36.4%) patients, according to the tumour staging. Results: Final pathology showed 5 pT1bN0, 1 pT1bN1, 28 pT1cN0, 19 pT1cN1, 7 pT2N0, and 17 pT2N1 BC. Overall, 719 AN were removed of which 106 (14.7%) were metastatized nodes (median 3, range 1–5 per patient). The sensitivity, specificity and accuracy were 67.6%, 80.0%, and 74.0% for US, 78.4%, 85.0%, and 81.8% for SSM, and 91.9%, 92.5%, and 92.2% for US and SSM together, respectively. There was a significant difference (p <0.05) in the number of metastatized AN between patients with metastases correctly detected and undetected by both US (3.1±1.3 vs. 2.0±0.7) and SSM (3.2±1.3 vs. 1.7±0.7). Conclusions: Although the results of each diagnostic tests are strictly dependent on the number of the metastatized AN, the combination of axillary US and SSM is a sensitive low-cost procedure that should be suggested in all patients with BC, when a preoperative evaluation of the AN status is required. Summary: The complementary role of sestamibi scintimammography (SSM) in patients with breast cancer (BC) is well established. The aim of this study was to establish whether a relationship exists between sestamibi uptake, evaluated as a tumour-to-background ratio (TBR), and the main prognostic factors of BC. SSM with the measurement of TBR was performed preoperatively in 102 women (median age 57 years, range 32–81 years) who underwent curative surgery for primary BC. Final pathology showed 4 (3.9%) with pT1a, 17 (16.7%) with pT1b, 44 (43.1%) with pT1c and 37 (36.3%) with pT2 breast carcinomas. The overall sensitivity of SSM was 80.4%. An ANOVA showed significant () differences between the TBR of patients with G1 vs. G3 tumours, and between the TBR of those with G2 vs. G3 breast carcinomas. Moreover, there was a difference () between the TBR of patients (, 11.8%) with CEA serum levels >10ng/ml (2.031±0.420), and those with normal (, 88.2%) CEA values (1.713±0.446), whilst no difference (P=NS) was found between patients (, 26.5%) with CA 15-3 >30U/ml (1.893±0.401) and those with normal (, 73.5%) CA 15-3 values (1.699±0.462). There was a mild inverse correlation between TBR and both the oestrogen (, ) and the progesterone receptor (, ) rate. The logistic regression analysis showed that only size and CA 15-3 serum levels represent true independent parameters, but the function was able to predict only 11 out of 21 (52.4%) patients with false-negative SSM. TBR is independent of age and mainly correlates with the size of the tumour. There are no reliable preoperative prognostic factors that are really useful for improving SSM sensitivity in patients with small breast carcinomas. [Copyright &y& Elsevier]

Abstract: Neither eosinophilic cystitis nor nephrogenic adenoma is often diagnosed in children, with few pediatric cases being reported in the literature. Although these maladies share the same predisposing conditions, namely, chronic irritation or injury to the urothelium and lower urinary tract and symptoms such as dysuria, hematuria, and urinary frequency, their concomitance is exceptional.
