- Abstract
Background: Accurate assessments of current and future fertility-including overall trends and changing population age structures across countries and regions-are essential to help plan for the profound social, economic, environmental, and geopolitical challenges that these changes will bring. Estimates and projections of fertility are necessary to inform policies involving resource and health-care needs, labour supply, education, gender equality, and family planning and support. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 produced up-to-date and comprehensive demographic assessments of key fertility indicators at global, regional, and national levels from 1950 to 2021 and forecast fertility metrics to 2100 based on a reference scenario and key policy-dependent alternative scenarios. Methods: To estimate fertility indicators from 1950 to 2021, mixed-effects regression models and spatiotemporal Gaussian process regression were used to synthesise data from 8709 country-years of vital and sample registrations, 1455 surveys and censuses, and 150 other sources, and to generate age-specific fertility rates (ASFRs) for 5-year age groups from age 10 years to 54 years. ASFRs were summed across age groups to produce estimates of total fertility rate (TFR). Livebirths were calculated by multiplying ASFR and age-specific female population, then summing across ages 10-54 years. To forecast future fertility up to 2100, our Institute for Health Metrics and Evaluation (IHME) forecasting model was based on projections of completed cohort fertility at age 50 years (CCF50; the average number of children born over time to females from a specified birth cohort), which yields more stable and accurate measures of fertility than directly modelling TFR. CCF50 was modelled using an ensemble approach in which three sub-models (with two, three, and four covariates variously consisting of female educational attainment, contraceptive met need, population density in

- Abstract
Nelore heifers usually begin their reproductive life at ≥24 months of age mainly due to suboptimal nutritional conditions and genetics. This study aimed to determine the effect of expected progeny difference (EPD) for age at first calving and average daily gain (ADG) on puberty in Nelore (Bos taurus indicus ) heifers. A total of 58 weaned heifers (initial BW =174 ± 6 kg; age =9 ± 1 months) were allocated into 28 feedlot pens. Heifers were born from four sires, of which two had low EPD for age at first calving (L;n =33) and two had high EPD for age at first calving (H;n =25). Then, heifers of each EPD were randomly assigned to high ADG (HG; 0.7 kg) or low ADG (LG; 0.3 kg), resulting in four treatments: heifers from L sires were submitted to either HG (LHG;n =17) or LG (LLG;n =16), and heifers from H sires were submitted to either HG (HHG;n =12), or LG (HLG; n =13). The HG heifers were fed a 75% grain diet, whereas the LG heifers received 93% of forage in their diet. Blood samples were collected at 9, 14, 18, 24 and 28 months of age for IGF1 and leptin determination. There was a treatment effect (P <0.01) on the proportion of heifers that attained puberty by 18 (62%, 0%, 0% and 0%), 24 (100%, 6%, 54% and 0%) or 36 (100%, 100%, 100% and 38%) months of age for LHG, LLG, HHG and HLG treatments, respectively. In addition, mean age at puberty was different across treatments (P <0.01). Heifers from the LHG achieved puberty at the earliest age when compared with cohorts from other treatments (18.1, 28.9, 23.9 and 34.5 months for LHG, LLG, HHG and HLG, respectively). Serum IGF1 concentrations were higher for L heifers compared with H cohorts at 9, 14, 18, 24 and 28 months of age (P <0.01; treatment x age interaction), whereas circulating leptin concentrations were higher (P <0.01; age effect) as heifers became older, regardless of the treatments. In conclusion, only Nelore heifers with favorable genetic merit for age at first calving were able to attain puberty by 18 months of age. In heifers with unfavorable genetic merit for age at first calving, supplementary feeding to achieve high ADG was unable to shift the age at puberty below 24 months. [ABSTRACT FROM AUTHOR]

- Abstract
Paracetamol (PAR), phenylephrine hydrochloride (PHE) and chlorpheniramine maleate (CPM) are commonly used in clinical practice as antipyretic and analgesic drugs to ameliorate pain and fever in cold and flu conditions. The present work describes the use of thermal analysis for the characterization of the physicochemical compatibility between drugs and excipients during the development of solid dosage forms. Thermogravimetric analysis (TGA) and Differential Scanning Calorimetry (DSC) were used to study the thermal stability of the drug and of the physical mixture (drug/excipients) in solid binary mixtures (1:1). DSC thermograms demonstrated reproducible melting event of the prepared physical mixture. Starch, mannitol, lactose and magnesium stearate influence thermal parameters. Information recorded from the derivative thermogravimetric (DTG) and TGA curves demonstrated the decomposition of drugs in well-defined thermal events, translating the suitability of these techniques for the characterization of the drug/excipients interactions. NON-muscle invasive bladder cancer ,CANCER invasiveness ,TOLL-like receptors ,CELLULAR signal transduction ,DISEASE relapse - Abstract
Bacillus Calmette-Guérin (BCG)-based intravesical immunotherapy has been applied as gold standard treatment for high-risk non-muscle invasive bladder cancer (NMIBC) for almost half a century. However, several patients with high-risk disease experience relapse, including those whose condition has worsened and who failed to respond to BCG. Non-significant therapeutic options have been developed for these at-risk patients, for many years. Immunotherapies have shown promising outcomes for bladder cancer treatment. Accordingly, our research group developed the OncoTherad® (MRB-CFI-1) immunotherapy, which has shown positive outcomes in NMIBC treatment. The aim of the current study is to describe, in details, the physicochemical features and potential action mechanisms of OncoTherad® nano-immunotherapy, based on toll-like receptor 4 (TLR4)-mediated interferon and on RANK/RANKL signaling pathways, in animal model with NMIBC. Based on the current findings, OncoTherad® nano-immunotherapy did not have genotoxic effect on the investigated model and did not show signs of limiting local and/or systemic toxicity at therapeutic doses. OncoTherad® nano-immunotherapy was more effective than the BCG treatment, since it reduced by 70% the malignancy rate. Furthermore, it was possible identifying an important action mechanism of OncoTherad®, which was based on the modulation of TLR4-mediated interferon and RANK/RANKL signaling pathways that, altogether, were essential to reduce malignancy rate. OncoTherad® mechanisms in these pathways helped preventing tumor recurrence. [Display omitted] • 50% of patients with non-muscle invasive bladder cancer will recur or progress. • Immunotherapeutic agents are being developed in the BCG-unresponsive NMIBC. • Toll-like receptors agonist may represent a potential antitumoral therapeutic attempt. • OncoTherad® stimulates interferon signaling pathway. • OncoTherad® is a potential new therapeutic treatment for NMIBC. Previous evidence suggested that 10 mg systemic metoclopramide is not effective to prevent postoperative nausea and/or vomiting (PONV) in patients receiving general anaesthesia. However, the evidence included data with questioned validity by the author Yoshitaka Fujii. The objective of the current study was to examine the effect of a systemic dose of 10 mg metoclopramide to prevent PONV.This quantitative systematic review was performed according to the PRISMA guidelines. A wide search was performed to identify randomized clinical trials that evaluated systemic 10 mg metoclopramide as a prophylatic agent to reduce PONV. Meta-analysis was performed using a random-effect model.Thirty trials evaluating the effect of 10 mg systemic metoclopramide in 3328 subjects on PONV outcomes were included. Metoclopramide reduced the incidence of 24 h PONV compared with control, odds ratio (OR) [95% confidence interval (CI)] of 0.58 (0.43–0.78), number needed to treat (NNT)=7.8. When evaluated as separate outcomes, metoclopramide also decreased the incidence of nausea over 24 h, OR (95% CI) of 0.51 (0.38–0.68), NNT=7.1, and vomiting over 24 h, OR (95% CI) of 0.51 (0.40–0.66), NNT=8.3. A post hoc analysis examining three studies with questioned validity performed by the author Yoshitaka Fujii that would meet criteria for inclusion in the current study did not demonstrate a significant benefit of metoclopramide compared with control on the incidence of 24 h PONV. Our findings suggest that metoclopramide 10 mg i.v. is effective to prevent PONV in patients having surgical procedures under general anaesthesia. Metoclopramide seems to be a reasonable agent to prevent PONV. [ABSTRACT FROM AUTHOR]

Background Glucocorticoids are commonly administered before ambulatory surgery, although their effects on quality of recovery are not well characterized. The purpose of this study was to evaluate the dose-dependent effects of dexamethasone on patient recovery using the Quality of Recovery 40 questionnaire (QoR-40) after ambulatory surgery. Methods This prospective, double-blind trial studied 106 female subjects undergoing outpatient gynaecological laparoscopy. Subjects were randomized to receive saline, dexamethasone 0.05 mg kg−1 or dexamethasone 0.1 mg kg−1 before induction. The primary outcome was global QoR-40 at 24 h. Postoperative pain, analgesic consumption, side-effects, and discharge time were also evaluated. Results Global median (IQR) QoR-40 after dexamethasone 0.1 mg kg−1 193 (192–195) was greater than dexamethasone 0.05 mg kg−1 179 (175–185) (P=0.004) or saline, 171 (160–182) (P<0.005). Median (IQR) morphine equivalents administered before discharge were 2.7 (0–6.3) mg after dexamethasone 0.1 mg kg−1 compared with 5.3 (2.4–8.8) mg and 5.3 (2.7–7.8) mg after dexamethasone 0.05 mg kg−1 and saline (P=0.02). Time to meet discharge criteria was 30 min shorter after dexamethasone 0.1 mg kg−1 compared with saline (P=0.005). At 24 h, subjects receiving dexamethasone 0.1 mg kg−1 had consumed less opioid analgesics, reported less sore throat, muscle pain, confusion, difficulty in falling asleep, and nausea compared with dexamethasone 0.05 mg kg−1 and saline. Conclusions Dexamethasone demonstrated dose-dependent effects on quality of recovery. Dexamethasone 0.1 mg kg−1 reduced opioid consumption compared with dexamethasone 0.05 mg kg−1, which may be beneficial for improving recovery after ambulatory gynaecological surgery. Abstract: Loss of soil is a major problem that restoration actions must solve in Mediterranean quarries. Soil replacement by a low-quality substrate is often inadequate for revegetation and limits plant establishment and growth in these degraded areas, which are also subject to drought recurrent stress. In 1998, a field experiment was performed in a limestone quarry at Outão (Serra da Arrábida, SW Portugal) to test different types of substrate improvement: NPK fertilizer, water-holding polymer, mycorrhizal inoculum, and combinations of these. Two-year old plants of three native woody species were planted – carob (Ceratonia siliqua), wild-olive tree (Olea europaea var. sylvestris) and mastic (Pistacia lentiscus). Reports concerning the short-term results showed some differential effects of the assayed treatments on plant growth and physiology. The monitoring program was maintained, and here we report on the survival and growth of the introduced plants over eight years after planting. This prolonged study showed that, with the limited exception of fertilization, none of the assayed treatments added major advantages for plant survival or growth. Regardless of the tested substrate treatment, mortality was low and these native species became established in the revegetated area. [ABSTRACT FROM AUTHOR]

Abstract: The aim of this study was to conduct a thermodynamic analysis of the uptake of solvents into a model membrane as a precursor to skin transport studies. The investigation was designed so that the methodology may be applied to analyse data produced from measurement of the uptake of enhancers into skin. The uptake of a series of alcohols into polydimethylsiloxane (silicone) membranes in the temperature range 5–45°C was examined. A thermodynamic analysis of the data was performed to provide fundamental insight into the uptake process. A simple structure activity relationship was found to exist for the uptake of alcohols with a carbon chain length greater than four, with additional methylene groups exponentially decreasing the equilibrium uptake. Two separate straight lines were observed in the van’t Hoff plot for the equilibrium solvent uptake above and below 16°C. The two separate straight lines in the van’t Hoff plot suggest a change in the mechanism of solvent uptake and solvent structure in the membrane above and below 16°C. This is likely to have implications for the effect of the solvents on the partitioning of drugs into the membrane and will be used to provide insight into dynamic measurements of the effect of temperature on the transport of drug molecules in the same vehicles, across the membrane. The analysis described here should provide a useful methodology for investigating the uptake of solvents into model membranes. [Copyright &y& Elsevier]

EGG quality , *AMINES , *ALBUMINS , *EGG yolk , *SPERMIDINE , *VOLATILE organic compounds ,PRODUCT quality management - Abstract
The objective of this study was to investigate the possibility of using bioactive amines as an index of quality of fresh and stored eggs. Large white eggs from 24-wk-old Dekaib layers were randomly distributed in 3 groups: (i) 10 freshly laid eggs, (ii) 60 eggs stored at 25 + 1°C and 60% RH for 30 d, and (iii) 50 eggs stored at 6 + 1°C and 60% RH for 50 d. The eggs were weighed and the internal quality was evaluated by Haugh units (HU), pH of albumen, total solids, total volatile bases, and bioactive amines in the albumen and yolk. The fresh eggs had, on average, 56.85 g, 98.55 HU, albumen pH of 8.02, total solids of 12.17 g/100 g in the albumen and 52.43 g/100 g in the yolk, and absence of volatile bases. None of the 10 amines investigated were detected in the albumen; however, the yolk contained 0.37 mg/kg of spermidine. Throughout storage, there was a significant decrease in the weight of the egg and HU and a significant increase in the pH and in the total solids of the albumen. The decrease in HU and the increase in the total solids of the albumen were faster at 25 + 1°C compared with 6 ± 1°C. At 50 and 30 d of storage at 6 + 1 and 25 ± 1°C, respectively, significant levels of total volatile bases were detected. The levels of spermidine in the yolk increased significantly at the 40th and 15th days of storage at 6 ± 1 and 25 ± 1°C, respectively. At these storage times, the presence of putrescine and agmatine was also detected. Therefore, the presence of other amines besides spermidine or spermidine levels higher than 1.0 mg/kg in the yolk could be used as an index of quality of fresh eggs and throughout storage. Objectives The proportion of deaths attributed to hypertensive diseases (HYPDs) was only 50% of that registered for cerebrovascular diseases (CBVDs) in 2013 in Brazil. This article aims to evaluate mortality related to HYPDs and CBVDs as multiple causes of death, in Brazil from 2004 to 2013. Study design Analysis of historical series of secondary data obtained from Brazilian official registries. Methods Data about the deaths were obtained from the Mortality Information System of the Brazilian Ministry of Health, available on the DATASUS website. CBVDs and HYPDs were evaluated according to their mentions as the underlying cause of death or entry in any line of the death certificates (DCs), according to their International Statistical Classification of Diseases and Related Health Problems, 10th Revision codes. Results When CBVDs were the underlying causes of death, HYPDs were mentioned in 40.9% of the DCs. When HYPDs were the underlying causes of death, CBVDs were mentioned in only 5.0%. When CBVDs were mentioned without HYPDs, they were selected as the underlying cause of death 74.4% of the time. When HYPDs were mentioned in DCs without CBVDs, HYPDs were selected 30.0% of the time. In 2004, the frequency of any mention of HYPDs relative to the frequency of HYPDs cited as underlying causes increased fourfold and was followed by a plateau until 2013. In contrast, the frequency of any mention of CBVDs relative to the frequency of CBVDs as underlying causes decreased in the same period. Because this study was based on DC records, it was limited by the way these documents were completed, which may have included lack of record of the causes related to the sequence that culminated in death. Conclusion When deaths related to HYPDs were evaluated as multiple causes of death, they were mentioned up to four times more often than when they were selected as underlying causes of death. This reinforces the need for better control of hypertension to prevent deaths. [ABSTRACT FROM AUTHOR]

Background: The knowledge regarding appropriate dosage of local anaesthetics for peripheral nerve blocks in children is very scarce. The main objective of the current investigation was to evaluate dosing patterns of local anaesthetics in children receiving peripheral nerve blocks across multiple paediatric hospitals in the USA. We also sought to estimate the incidence of local anaesthetic systemic toxicity. Methods: This is an observational study using the Pediatric Regional Anesthesia Network (PRAN) database. Data on every peripheral nerve block in patients aged <18 years placed from April 1, 2007 to May 31, 2015 were examined as a subset of the PRAN protocol. Data were examined for the type and dose of local anaesthetic and for the presence of local anaesthetic systemic toxicity. Results: In total, 40 121 peripheral nerve blocks in children were analysed. Individual analyses of block type demonstrated large local anaesthetic dose variability with a five- to 10-fold spread depending on the block type. Two patients developed local anaesthetic systemic toxicity, resulting in an estimated incidence (95% CI) per blocks performed of 0.005% (0.001-0.015%). None of the patients had any short- or long-term complications or sequelae. Conclusions: We detected a large variability in the local anaesthetic dosing practices for peripheral nerve blocks in children across multiple hospitals in the USA. Nonetheless, the risk of local anaesthetic systemic toxicity was very low. Due to the lack of dose findings studies, our results suggest the need to develop practice guidelines to minimize variability of regional anaesthesia practices in children. Background: Peripheral nerve catheters (PNCs) are used with increasing frequency in children. Although adult studies have demonstrated safety with this technique, there have been few safety studies in children. The main objective of the current investigation was to examine the incidence of PNC complications in children undergoing surgery. Methods: This is an observational, multi-institutional study using the Pediatric Regional Anesthesia Network (PRAN) database. Data pertaining to PNCs were entered prospectively into a secure, online database by each participating centre. Patient characteristics, anatomic location, localization techniques, medications used, and complications were recorded for each catheter. All complications and any sequelae were followed until resolution. Results: There were 2074 PNCs included in the study. 251 adverse events and complications were recorded, resulting in an overall incidence (95% CI) of complications of 12.1% (10.7-13.5%). The most common complications were catheter malfunction, block failure, infection, and vascular puncture. There were no reports of persistent neurologic problems, serious infection, or local anaesthetic systemic toxicity, resulting in an estimated incidence (95% CI) of 0.04% (0.001-0.2%). Patients who developed an infection had used the catheters for a greater number of days, median (IQR) of 4.5 (3-7) days compared with 3 (1-3) days in the patients who did not develop an infection, P<0.0001. Conclusions: Our data support the safety of placing PNCs in children, with adverse event rates similar to adult studies. Catheter problems are common, yet minor, in severity. [ABSTRACT FROM AUTHOR]

Abstract: Resilience against sudden changes in the environment is a very desirable trait in plants used for ecosystem restoration. Mediterranean-like vegetation exhibits particularly strong fire resilience. There are two main functional groups of fire-prone species among Mediterranean-like vegetation: seeders and resprouters. Our aims were to describe how the theory of succession after fire relates to rehabilitation and to use this knowledge to improve the results of rehabilitation attempts in Mediterranean-like ecosystems. Eight post-fire (PF) sites, 14 post-rehabilitated (PR) quarry sites and two woodland sites were sampled. Detrended Canonical Correspondence Analysis (DCCA) showed that PF and PR successions were quite different. Both displayed an increasing abundance of resprouters over time, but seeder density increased throughout PR succession and decreased during PF succession. Nine species were common to both successions in all studied stages. The results showed that until 15–21 years of succession, the post-rehabilitation sites had not become as resilient to fires as sites populated by indigenous vegetation due to the lack of a seeder seed bank. However, after 21 years of PR succession, the exponentially increasing seeder population may allow for seed bank formation and thus eventually improve the fire resilience of the site. Abstract: Confocal Raman spectroscopy (CRS) is a novel approach which has been used to determine stratum corneum thickness and to profile endogenous skin components. However, the utility of this technique for probing drug disposition in vivo has not been explored to date. In this paper we report the use of CRS to investigate the fate of ibuprofen after application from simple formulations which were previously investigated using the tape-stripping approach. Ibuprofen was prepared in propylene glycol (PG) and propylene glycol/water (PG/H2O) solutions. The formulations were then applied to the volar aspect of the forearm of human volunteers. The results confirmed that ibuprofen distribution profiles in the stratum corneum were comparable to previously published data from tape stripping experiments. We propose CRS as a non-invasive method for dermatopharmacokinetic evaluation of topical pharmaceutical formulations. [Copyright &y& Elsevier]

GENE expression profiling , *HEART transplantation , *INDIVIDUAL differences , *MOLECULAR pathology , *T cells - Abstract
Blood gene expression profile (GEP) is being used for non-invasive surveillance of T cell-mediated rejection (TCMR) in orthotopic heart transplant (OHT). While blood GEP is presented as an ordinal score to clinicians, it is a summary score of quantification of 11 different gene mRNA levels relevant to TCMR. Whether the circulating levels of individual genes are relevant to pathways that occur in tissue GEP (Molecular Microscopy Diagnostic System; MMDx) is unknown. The aim of this study was to explore the relationship between blood and tissue GEP in individual OHT patients with TCMR diagnosed by molecular pathology or endomyocardial biopsy. We retrospectively reviewed all OHT patients from August 2021 to May 2022 that underwent both blood GEP and endomyocardial biopsy with MMDx (One Lambda) within a 2-week period. We retrieved the individual gene mRNA levels in the blood from CareDx Inc. Student t-test was performed to compare the individual gene mRNA levels found in blood between patients with and without TCMR as assessed by MMDx and EMB. A total of 173 paired samples from 125 patients were included in the analysis. There were 15 samples (8.7%) with TCMR as assessed by MMDx. There were 5 samples (2.9%) with TCMR as diagnosed by EMB (Table 2). The only gene that was different in TCMR from both MMDx and EMB was MARCH8 , which is highly expressed in myeloid cells and involved in inflammation (Table 1). There were several genes that were differentially expressed in MMDx or EMB, but not in both modalities. Blood MARCH8 gene was differentially expressed in TCMR from MMDx and EMB. Further studies should investigate the relation between individual gene expression levels of blood and individual gene expression levels of myocardial tissue GEP, which may elucidate a novel common pathway. The purpose of the study is to investigate the relation between the level of individual gene components of blood GEP and tissue GEP derived from AlloMap and molecular microscopy (MMDx) in the setting of antibody-mediated rejection (AMR) in HT. All OHT patients from August 2021 to May 2022 with both blood GEP and endomyocardial biopsy with MMDx (One Lambda) within 2 weeks were included (N=173 samples). We retrieved the 11 gene mRNA level from AlloMap (CareDx Inc). Student t-test was performed to compare blood mRNA levels between no rejection and AMR as assessed by MMDx. A Scatter plot with Spearman correlation analysis was performed to compare the relation between blood mRNA levels and tissue GEP. There were 52 samples (30.1%) and 49 samples (28.3%) with AMR, as assessed by MMDx and EMB, respectively. Of the 11 gene expression levels, the blood ITGA4 (Integrin alpha 4) and PDCD1 (programmed cell death 1) gene levels were different in AMR (p=0.019, 0.020, respectively). In addition, the myocardial tissue expression level of the ROBO4 gene was higher in AMR. Interestingly ROBO4 had a positive correlation with blood ITGA4 mRNA levels in those without AMR (Spearman R Coefficient 0.33, p<0.001), whereas no correlation was present in those with AMR (R 0.09, p=0.513). Both ITGA4 and ROBO4 mRNA expression levels in AMR were higher. ITGA4 is expressed in circulating lymphoid and myeloid cells and is a receptor for VCAM, expressed in endothelial cells. ROBO4 is expressed in endothelial cells, responsible for endothelial cell permeability of immune cells. It is possible that the increased expression of ITGA4 and ROBO4 represents mechanistic crosstalk between inflammatory cells and myocardial endothelium. The positive correlation between ITGA4 and ROBO4 may represent immune system activation before the development of biopsy-proven AMR. This relationship warrants further study as an early signal to identify patients at risk of AMR. [ABSTRACT FROM AUTHOR]

ELECTRICITY , *ELECTRIC power distribution , *INDUSTRIAL ecology , *URBAN planning , *ELECTRIC vehicles , *BIOMASS energy - Abstract
This paper brings together several contemporary topics in energy systems aiming to provide a literature review based reflection on how several interrelated energy systems can contribute together to a more sustainable world. Some directions are discussed, such as the improvement of the energy efficiency and environmental performance of systems, the development of new technologies, the increase of the use of renewable energy sources, the promotion of holistic and multidisciplinary studies, and the implementation of new management rules and "eco-friendly and sustainable" oriented policies at different scales. The interrelations of the diverse energy systems are also discussed in order to address their main social, economic and environmental impacts. The subjects covered include the assessment of the electricity market and its main players (demand, supply, distribution), the evaluation of urban systems (buildings, transportation, commuting), the analysis of the implementation of renewable energy cooperatives, the discussion of the diffusion of the electric vehicle and the importance of new bioenergy systems. This paper also presents relevant research carried out in the framework of the Energy for Sustainability (EfS) Initiative of the University of Coimbra, linking the reviewed areas to the multidisciplinary approach adopted by the EfS Initiative. To conclude, several research topics that should be addressed in the near future are proposed. [ABSTRACT FROM AUTHOR]

Background: Maximising patient comfort during and after surgery is a primary concern of anaesthetists and other perioperative clinicians, but objective measures of what constitutes patient comfort in the perioperative period remain poorly defined. The Standardised Endpoints in Perioperative Medicine initiative was established to derive a set of standardised endpoints for use in perioperative clinical trials. Methods: We undertook a systematic review to identify measures of patient comfort used in the anaesthetic, surgical, and other perioperative literature. A multi-round Delphi consensus process that included up to 89 clinician researchers was then used to refine a recommended list of outcome measures. Results: We identified 122 studies in a literature search, which were the basis for a preliminary list of 24 outcome measures and their definitions. The response rates for Delphi Rounds 1, 2, and 3 were 100% (n=22), 90% (n=79), and 100% (n=13), respectively. A final list of six defined endpoints was identified: pain intensity (at rest and during movement) at 24 h postoperatively, nausea and vomiting (0-6 h, 6-24 h, and overall), one of two quality-of-recovery (QoR) scales (QoR score or QoR-15), time to gastrointestinal recovery, time to mobilisation, and sleep quality. Conclusions: As standardised outcomes will support benchmarking and pooling (meta-analysis) of trials, one or more of these recommended endpoints should be considered for inclusion in clinical trials assessing patient comfort and pain after surgery. Background.: We conducted a randomized equivalence trial to compare direct laryngoscopy using a Miller blade (DL) with the King Vision videolaryngoscope (KVL) for routine tracheal intubation. We hypothesized that tracheal intubation times with DL would be equivalent to the KVL in children <2 yr of age.Methods.: Two hundred children were randomly assigned to tracheal intubation using DL or KVL. The primary outcome was the median difference in the total time for successful tracheal intubation. Secondary outcomes assessed were tracheal intubation attempts, time to best glottic view, time for tracheal tube entry, percentage of glottic opening score, airway manoeuvres needed, and complications.Results.: The median difference between the groups was 5.7 s, with an upper 95% confidence interval of 7.5 s, which was less than our defined equivalence time difference of 10 s. There were no differences in the number of tracheal intubation attempts and the time to best glottic view [DL median 5.3 (4.1-7.6) s vs KVL 5.0 (4.0-6.3) s; P =0.19]. The percentage of glottic opening score was better when using the KVL [median 100 (100-100) vs DL median 100 (90-100); P <0.0001]. Use of DL was associated with greater need for airway manoeuvres during tracheal intubation (33 vs 7%; P <0.001). Complications did not differ between devices.Conclusions.: In children <2 yr of age, the KVL was associated with equivalent times for routine tracheal intubation when compared with the Miller blade.Clinical Trial Registration: NCT02590237. The effect of the shortmilling times on R5(Si,Ge)4 R=Gd, Tbmagnetocaloricmaterial propertieswas investigated. In particular, the effect of milling on atomic structure, particles size and morphology, magnetic, and magnetocaloric effect was studied. With short milling times (b2.5 h), a reduction of the Gd5Si1.3Ge2.7 and Tb5Si2Ge2 particles sizewas achieved down to approximately 3.5 μm. For both compositions the main differences are a consequence of the milling effect on the coupling of the structural and magnetic transitions. In the Gd5Si1.3Ge2.7 case, a second-order phase transition emerges at high temperatures as a result of ballmilling. Consequently, there is a decrease in themagnetocaloric effect of 35% after 150 min of milling. Interestingly, an opposite effect is observed in Tb5Si2Ge2 where a 23% increase of the magnetocaloric effectwas achieved, driven by the enhancement of the coupling betweenmagnetic and structural transitions arising from internal strain promoted by the milling process.
