Annual influenza vaccination is crucially recommended for the elderly to maintain humoral immunity. Insufficient coverage requires us to understand the determinants of their influenza behaviors and how these patterns impact vaccination choices. Data from 540 Beijing residents aged over 65 were collected through interviews, capturing vaccination history and sociodemographic details. Individual influenza vaccination records from 2016 to 2020 were obtained from China's Immunization Information Systems. A latent class model identified three vaccination patterns. Multinomial logistic regression assessed relative risk ratios (RRRs) for vaccination based on sociodemographic factors. Vaccination patterns were used to predict future vaccination likelihood. The analysis revealed three groups: sporadically vaccinated (63.33%), occasionally vaccinated (18.71%), and frequently vaccinated (17.96%). Factors associated with frequent vaccination included age over 70 (RRR = 2.81), lower income (RRR = 0.39), higher vaccine hesitancy (RRR = 3.10), multiple chronic conditions (RRR = 2.72), and rural residence (RRR = 2.48). The frequently vaccinated group was more likely to sustain regular vaccination habits in subsequent years compared to the occasionally vaccinated group. Only 17.96% of Beijing's older population exhibited a consistent influenza vaccination pattern. Older age, rural residency, and chronic diseases correlated with repeated influenza vaccination. Segmenting the population based on past vaccination behavior can aid in designing targeted interventions to improve vaccination rates. [ABSTRACT FROM AUTHOR]

Abstract Purpose To evaluate the quality of life (QoL) and social support among family caregivers of a family member with a mental illness and to identify factors associated with the QoL. Methods This is a cross-sectional study, where participants were recruited and independently interviewed using a questionnaire, consisting of demographic characteristics, the Medical Outcome Survey SF-36 form, and social support rating scales. Multiple stepwise regression analysis was used to analyse the factors related to QoL. Results 181 family caregivers were recruited in Shandong province, China. On a composite QoL score, family caregivers perceived that their QoL was poor (68.3), especially in the aspects of role-physical (61.3), role-emotional (57.6) and mental health (63.0). We also found family caregivers received low social support, especially in objective support and utilization of social support. Patient's illness state, care time, financial burden and objective support were significantly correlated to caregivers' QoL in the physical component score (PCS). Patient's illness state, patient's marital status, family monthly income, caregiver's knowledge about the illness, caregivers coordinating caring, life and work, subjective support received and utility of support were significantly associated with caregivers' QoL in the mental component score (MCS). Conclusions Social support had a significant correlation with caregivers' QoL. Caregivers should be encouraged to request assistance from other family members and friends in providing care, especially when caregivers are unemployed or long-time carers. Mental health education campaigns and helping families to maintain and enhance a supportive social network may provide useful means to improve caregivers' QoL. Background: Total knee arthroplasty and its relation to gait abduction or adduction moment has not been fully described.Methods: Gait analysis was performed on 25 patients (27 knees) preoperatively, 6 months and 1 year after total knee arthroplasty. Reflective markers were placed on the lower extremity, and motion data were collected at 60 Hz using 6 infrared cameras. Ground reaction forces were recorded at 960 Hz with a force plate. Stance phase was divided into braking and propulsive phases. Coronal knee angles and moments were calculated. Repeated-measures analysis of variance was used to compare frontal plane knee impulse over time and between the braking and propulsive phases of stance.Results: In varus knees, static alignment was corrected from 2.2° varus to 3.3° valgus and in valgus knees from 15.2° valgus to 2.7° valgus (P < .010). Braking phase adduction impulse decreased from 0.145 to 0.111 at 6 months but increased to 0.126 Nm/kg s (P > .05) at 1 year. Propulsive phase impulse changed from 0.129 to 0.085 and persisted at 1 year. Impulse changed from 0.01 (abduction) to 0.11 Nm/kg s (adduction) at 6 months and persisted (P = .01).Conclusion: Restoration of anatomic alignment and soft tissue balancing changes the lateral loading conditions of valgus knees. Purpose To ascertain whether placing the humeral attachment of the lateral ulnar collateral ligament (LUCL) at the humeral center of rotation (hCOR) on the humerus would provide the most isometric reconstruction. Methods We analyzed 13 cadaver limbs from mid-humerus to the hand. The morphology of the ligament complex was assessed. The hCOR was then found using radiographic parameters. We chose 7 points on the humerus located at and around the hCOR and 3 points paralleling the supinator crest of the ulna and then calculated distances from these points using a digital caliper at 0°, 30°, 60°, 90°, and 130° flexion. Differences in potential ligamentous lengths (termed graft elongation) were then calculated and statistical analysis was performed. Results There was no perfectly isometric point along the humerus or ulna. However, in all specimens the hCOR was the most isometric point for the humeral reconstruction site, with an average graft elongation of 1.1 mm. Differences in humeral tunnel position dramatically affected graft elongation at all 3 ulnar insertions. Overall, ulnar position had a minimal effect on graft elongation. Conclusions Although no perfectly isometric points were found, the humeral center of rotation consistently reproduced the most isometry when assessing graft elongation over range of motion. These data may assist surgeons in proper tunnel placement in LUCL reconstruction. Purpose To determine the effect of debriding the indirect insertion component of the femoral anterior cruciate ligament (ACL) attachment on tibiofemoral kinematics when compared with the intact knee. Methods Knee kinematics were measured in 9 cadaveric knees with the ACL intact, after indirect insertion debridement, and after ACL transection. Three loading conditions were used: (1) a 134-N anterior tibial load, (2) a combined 10-Nm valgus and 5-Nm internal tibial torque, and (3) a simulated robotic pivot shift. Anterior tibial translation (ATT) was recorded in response to anterior and combined loads at 0°, 15°, 30°, 45°, 60°, and 90° of flexion. Posterior tibial translation and external tibial rotation were recorded during the simulated pivot shift. Results With an anterior load, indirect insertion debridement increased ATT by 0.37 ± 0.24 mm at 0° ( P = .002) and by 0.16 ± 0.19 mm at 15° ( P = .033; increases <1 mm in all specimens). ACL transection increased ATT in response to an anterior load ( P = .0001) with maximum effect at 15° compared with the intact and debrided states (11.26 ± 1.15 mm and 11.04 ± 1.08 mm, respectively). With a combined load, indirect insertion debridement increased ATT by 0.17 ± 0.11 mm at 0° ( P = .001; increases <0.3 mm in all specimens) with no effect at other angles. ACL transection increased ATT in response to a combined load ( P = .001) with maximum effect at 15° (4.45 ± 0.85 mm v ACL intact and 4.44 ± 0.84 mm v debrided indirect insertion). In the ACL intact condition, the pivot shift produced 1.29 ± 1.34 mm of posterior tibial translation and 1.54 ± 1.61° of external tibial rotation, as compared with 1.28 ± 1.34 mm and 1.54 ± 1.47°, respectively, after debridement ( P = .68 and P = .99, respectively) and 12.79 ± 3.22 mm and 17.60 ± 4.30°, respectively, after ACL transection ( P = .0001). Conclusions The indirect femoral ACL insertion contributes minimally to restraint of tibial translation and rotation. Purpose: To prove that the assessment of osteochondral graft perpendicularity with magnetic resonance imaging (MRI) will allow for a precise measurement of graft perpendicularity and for an improved comparison of arthroscopic versus mini-open harvest techniques. Methods: Ten fresh cadaveric knees (mean age, 39.4 years) underwent harvest of 6-mm osteochondral plugs using the Osteochondral Autograft Transfer System (OATS; Arthrex, Naples, FL). A total of 8 plugs were harvested per knee from 3 donor sites: the lateral supracondylar ridge, the medial supracondylar ridge, and the lateral intercondylar notch. Two surgeons performed the graft harvest, alternating between mini-open (5 specimens) and arthroscopic (5 specimens) techniques to minimize bias. The osteochondral plugs were labeled and plated by a novel agar plating technique and then underwent MRI for measurement of graft perpendicularity. The data were analyzed to look for a significant difference in perpendicularity between the 2 harvest techniques, as well as overall graft acceptability. Results: One specimen in the open harvest technique group was unable to undergo optimal MRI because of difficulties encountered with the novel agar plating system resulting in graft movement during imaging. When we compared the mini-open and arthroscopic harvest techniques, the mean angle of perpendicularity at the lateral intercondylar notch harvest site was 84.1° and 84.2°, respectively (P = .958). At the medial supracondylar ridge harvest site, the mean angle of perpendicularity for the mini-open and arthroscopic techniques was 88.4° and 81.0°, respectively, with a mean difference of 7.4° (P = .006). At the lateral supracondylar ridge harvest site, the mean angle of perpendicularity for the mini-open and arthroscopic techniques was 85.7° and 87.1°, respectively (P = .237). Conclusions: A significant difference in osteochondral autograft perpendicularity was noted at the medial supracondylar ridge when we compared the mini-open and arthroscopic harvesting techniques. This suggests that when one is harvesting autologous osteochondral grafts from the medial supracondylar ridge, the mini-open technique may be preferred. Clinical Relevance: When harvesting autologous osteochondral grafts from the medial supracondylar ridge of the knee, the mini-open technique will potentially allow for a more perpendicular graft for implantation. Backround: Isokinetic eccentric training of the wrist extensors has recently been shown to be effective in treating chronic lateral epicondylosis. However, isokinetic dynamometry is not widely available or practical for daily exercise prescription. Therefore, the objective of this study was to assess the efficacy of a novel eccentric wrist extensor exercise added to standard treatment for chronic lateral epicondylosis. Materials and methods: Twenty-one patients with chronic unilateral lateral epicondylosis were randomized into an eccentric training group (n = 11, 6 men, 5 women; age 47 ± 2 yr) and a Standard Treatment Group (n = 10, 4 men, 6 women; age 51 ± 4 yr). DASH questionnaire, VAS, tenderness measurement, and wrist and middle finger extension were recorded at baseline and after the treatment period. Results: Groups did not differ in terms of duration of symptoms (Eccentric 6 ± 2 mo vs Standard 8 ± 3 mos., P = .7), number of physical therapy visits (9 ± 2 vs 10 ± 2, P = .81) or duration of treatment (7.2 ± 0.8 wk vs 7.0 ± 0.6 wk, P = .69). Improvements in all dependent variables were greater for the Eccentric Group versus the Standard Treatment Group (percent improvement reported): DASH 76% vs 13%, P = .01; VAS 81% vs 22%, P = .002, tenderness 71% vs 5%, P = .003; strength (wrist and middle finger extension combined) 79% vs 15%, P = .011. Discussion: All outcome measures for chronic lateral epicondylosis were markedly improved with the addition of an eccentric wrist extensor exercise to standard physical therapy. Hypothesis: An important synergistic relationship exists between the scapular stabilizers and the glenohumeral rotators. Information on the relative contribution of the scapular stabilizers to glenohumeral rotation would be useful for exercise prescription for overhead athletes and for patients with shoulder pathology. We hypothesized that the scapular stabilizers would be highly active during both maximal and submaximal internal and external rotation. Materials and methods: Eight healthy male volunteers (16 shoulders) performed internal and external glenohumeral rotation testing at maximal and submaximal intensities. They also performed a scapular retraction rowing exercise at maximal and submaximal levels. Electromyographic (EMG) signals were recorded from the infraspinatus, pectoralis major, serratus anterior, and middle trapezius. Values were compared among muscle groups, among individual muscles at different intensity levels, and among individual muscles at different points in the arc of motion. Results: For submaximal glenohumeral internal rotation, activity in the scapular stabilizers was not different (P = .1-.83) from activity in the internal rotator throughout the range of motion. For the initial two-thirds of maximal internal rotation, middle trapezius activity and pectoralis major activity were higher (P < .05) than serratus anterior activity. For submaximal external rotation, activity in the scapular stabilizers during the middle phase of the motion was higher (P < .05) than activity in the external rotators. For maximal external rotation these differences were present throughout the motion with middle trapezius activity exceeding 100% maximal voluntary contraction. Conclusions: The scapular stabilizers functioned at a similar or higher intensity than the glenohumeral rotators during internal and external rotation. This highlights the importance of training the scapular stabilizers in upper extremity athletes and in patients with shoulder pathology. [Copyright &y& Elsevier]

Purpose: To compare the biomechanical strength of knotless suture anchors and standard suture anchors in the repair of type II SLAP tears. Methods: Five pairs of cadaveric shoulders (10 shoulders) were dissected free of soft tissue except for the glenoid labrum and long head of the biceps tendon. Type II SLAP tears were created and repaired with 1 of 2 anchors: the Mitek Lupine suture anchor or the Mitek Bioknotless suture anchor (DePuy Mitek, Raynham, MA). All specimens were preloaded to 10 N, and loaded for 25 cycles in 10 N increments to a maximum of 200 N. If specimens were still intact after 200 N, they were loaded to ultimate failure. The load at which 2 mm of gapping occurred, load to ultimate failure, mode of failure, and the number of cycles to failure were compared using the Wilcoxon signed-rank test. Results: Load to 2-mm gapping was lower (P = .042) for knotless anchors (70 N) versus knotted anchors (104 N), with similar differences for ultimate failure (74 N v 132 N; P = .043), cycles to 2-mm gapping (133 v 219 cycles; P = .042), and cycles to failure (143 v 297; P = .043). Eight of 10 specimens failed at the soft tissue interface (4 knotless, 4 knotted) and 2 failed by anchor pullout (1 knotted, 1 knotless). Conclusions: The results of this study suggest that repair of a type II SLAP with a Mitek knotted suture anchor and mattress suture configuration through the biceps anchor is stronger than repair with a Mitek knotless suture anchor. The most likely method of repair failure was at the suture–soft tissue interface regardless of the type of anchor used. The application of a suture anchor that requires arthroscopic knot tying may be preferable to a knotless anchor for the surgical repair of type II SLAP tears. Clinical Relevance: Repair of type II SLAP tears with knotless suture anchors may allow for the avoidance of arthroscopic knot tying but is weaker than repair with standard suture anchors. The purpose of this study was to determine whether strength deficits could be detected in individuals with and without shoulder impingement, all of whom had normal shoulder strength bilaterally according to grading of manual muscle testing. Strength of the internal rotators and external rotators was tested isokinetically at 60°/s and 180°/s, as well as manually with a handheld dynamometer (HHD) in 17 patients and 22 control subjects. Testing was performed with the shoulder positioned in the scapular plane and in 90° of shoulder abduction with 90° of elbow flexion (90-90). The peak torque was determined for each movement. The strength deficit between the involved and uninvolved arms (patients) and the dominant and nondominant arms (control subjects) was calculated for each subject. Comparisons were made for the scapular-plane and 90-90 positions between isokinetic and HHD testing. Despite a normal muscle grade, patients had marked weakness (28% deficit, P < .01) in external rotators at the 90-90 position tested with the HHD. In contrast, external rotator weakness was not evident with isokinetic testing at the 90-90 position (60°/s and 180°/s, 0% deficit, P = .99). In control subjects, greater internal rotator strength in the dominant compared with the nondominant arm was evident with the HHD at the 90-90 position (11%, P < .01) and in the scapular plane (7%, P < .05). Using an HHD while performing manual muscle testing can quantify shoulder strength deficits that may not be apparent with isokinetic testing. By using an HHD during shoulder testing, clinicians can identify weakness that may have been presumed normal. [Copyright &y& Elsevier]

To investigate the short-term and long-term effectiveness of different levels of financial incentives on increasing the willingness to vaccinate and vaccine uptake. A randomized controlled trial was conducted to investigate the effectiveness of financial incentives of three groups with monetary incentives (CNY 20, CNY 40, and CNY 60; 1 CNY = 0.13 EUR) vs. a control group—CNY 0—on influenza vaccine uptake among 720 older adults (≥60 years) in Beijing, China. The primary outcome was vaccine uptake, and the secondary outcomes were intention to vaccinate and length of time to immunization. Financial incentive significantly promoted higher intention to influenza vaccination (120/178 [67.42%] vs. 442/542 [81.55%]; Relative Risk [RR], 1.21; 95% CI, 1.02–1.42) and higher vaccination participation (74/178 [41.57%] vs. 316/542 [58.30%]; RR, 1.39; 95% CI, 1.10–1.75). CNY 60 had the largest impact on the intention to vaccinate (15.00% vs. 13.48% and 13.90%) and vaccination uptake (19.42% vs. 14.05% and 16.67%) compared with CNY 20 and CNY 40. Time to vaccination was significantly lower among participants receiving incentives than those without ([37.21 days; 95% CI, 34.33–39.99] vs. [48.27 days; 95% CI, 43.47–53.07]; Hazard Ratio [HR] 1.57, 95% CI 1.22–2.03). We found no long-term influence of financial incentives on vaccination decisions in the following year (217/542, 40.04% vs. 65/178, 36.52%; RR 1.08, 95% CI 0.82–1.42). Our study suggests that modest financial incentives will boost short-term influenza vaccination rates and shorten the length of time to immunization in China. Understanding the preferences of old-age adults for their long-term caregivers can improve person-centred health care and the quality of long-term care (LTC). This study examines Chinese older adults' preferences for long-term caregivers. This is a cross-sectional study. A national representative discrete choice experiment (DCE) surveyed 2031 adults aged 50–70 across 12 provinces in China. Each DCE scenario described five attributes: type of caregivers, place of LTC, contents of LTC, out-of-pocket payments, and quality of life (QoL). Preferences and the marginal willingness to pay (WTP) were derived using mixed-logit and latent class models. Older adults displayed higher preferences for long-term caregivers who improve their QoL, incur lower out-of-pocket payments, and provide medical LTC services at home, with the maximum WTP of $22.832 per month. QoL was rated as the most important LTC factor, followed by the place of LTC and the type of caregivers. When the level of QoL improved from poor to good, respondents would be willing to pay $18.375 per month more (95% confidence interval: 16.858 to 20.137), and the uptake rate increased by 76.47%. There was preference heterogeneity among older people with different sex, education, family size, and knowledge of LTC insurance. QoL was the most important factor in older Chinese adults' preference for caregivers. Home care and medical care from formal caregivers was preferred by older adults. We recommend training family caregivers, raising older people's awareness of LTC insurance, and guiding policymakers in developing people-oriented LTC and a multi-level LTC system. Objective: While diabetes links with comorbidity of depression, there is no evidence in China regarding whether health behaviors modified the effect of depressive symptoms on diabetes. This study examined the influence of depressive symptoms on the incident diabetes, and investigated whether health behavior was a moderator in the depression-diabetes relationship in Chinese mid-aged and older adults.Methods: Using data from the China Health and Retirement Longitudinal 2011-2015 Study, the 10-item Center for Epidemiologic Studies Depression Scale (CESD-10) short form was used to measure depressive symptoms in 47,671 Chinese mid-aged and older adults. Diabetes was diagnosed through self-reported measures. We used random effect logistic regression models to examine the effect of depressive symptoms on the incidence of depressive symptoms.Results: Participants with depressive symptoms were more likely to suffer from diabetes than their non-depressive symptoms counterparts, with an odds ratio (OR) of 1.33 (95% CI: 1.13, 1.58). We found that unhealthy behaviors moderated the influence of depressive symptoms on risk of diabetes (OR = 1.01, 95% CI = 1.01, 1.02), indicating that depressive symptoms patients with two or more unhealthy behaviors had a higher risk of diabetes compared with their peers without unhealthy behaviors.Conclusion: Our findings suggested that depressive symptoms were a risk factor in developing diabetes, and that response to this factor varied by individuals' health behaviors. Introduction: Posterior root tears of the medial meniscus cause a detachment of the meniscus at its insertion to the posterior tibia. These tears have been identified as clinically significant entities associated with meniscal extrusion and the development of degenerative arthritis. Recent studies demonstrated that meniscal root tears are biomechanically equivalent to total meniscectomy in axial load models, while also reporting that root repair can correct compartment loading (Marzo, 2008; Allaire et al., 2008). This study investigates the effect of posterior medial meniscal root tears on medial knee compartment loading using a physiologic model that recreates a knee squat. A meniscal root repair technique which uses a retrograde reaming device to create a socket at the tibial insertion, into which the meniscal root is advanced, recreating the tibial attachment, was then performed (Nicholas et al., 2009). The knees were then loaded after the root repair and subsequent total meniscectomy examining the effect on medial compartment loading. Methods: Nine fresh-frozen cadaver knees were dissected of soft tissue to the joint capsule while preserving the insertion and entire quadriceps and hamstring tendons. Knee moments during squatting were simulated in a custom testing jig by application of angle specific loads to the quadriceps and hamstring tendons (based on body mass) with an axial compressive load at the tibia used to flex the knee to the desired angle (simulating ground reaction force). Anterior and posterior submeniscal arthrotomies allowed for passage of a pressure sensitive, capacitive, electronic sensor, made to approximate the medial tibial plateau shape and surface area. This sensor measures both contact area and peak pressures. Each specimen was then tested in four conditions: medial meniscus intact, after creation of posterior root tear, after repair of the meniscal root, and after total meniscectomy. Each condition was tested at 15, 45, and 90 degrees of knee flexion. Results: Transection of the meniscal root decreased the contact area of the medial compartment by 13% (p=0.002). After meniscal root repair, contact area was not significantly different from the intact meniscal state (decreased 6%, p=0.302). Additionally, total meniscectomy reduced contact area by 53% (p=0.025). Root transection also resulted in an overall increase in peak contact pressure of 21% (p=0.167) in the medial compartment. Meniscal root repair showed no difference when compared to the intact meniscal state (increased 10%, p=0.425) and total meniscectomy increased peak contact pressure by 97% at 15 degrees (p=0.015) with no significant effect at 45 or 90 degrees of knee flexion. Conclusion: Our study differs from others in the use of a physiologic load model that recreates the loads seen in the knee during the performance of a squat rather than an axial load model using supraphysiological loads and/or non-angle specific loading. In contrast to previous reports, our results show that meniscal root tears are not equivalent to total meniscectomies. The reduction in tibiofemoral contact area with meniscal root transaction was approximately 25% of that seen with total meniscectomy. Letrozole reduces serum oestradiol by inhibiting the aromatase enzyme and has growing clinical indications in fertility. The available evidence of letrozole's role in ovarian stimulation for IVF and intracytoplasmic sperm injection (ICSI) and clinical outcomes was assessed. Medline, Cochrane , and ClinicalTrials.gov databases were systematically searched up until August 2021, including 31 studies (n = 16 randomized controlled trials [RCTs]; n = 15 observational studies). Live birth rate (LBR) in poor responders significantly increased by 7% (95% CI, 1% to 13%, P = 0.03) with letrozole co-treatment. Concomitantly, the gonadotrophin consumption was significantly reduced, without decreasing the number of retrieved oocytes. In normal responders, number of oocytes increased with 1.8 oocytes (95% CI 0.35 to 3.27, P = 0.01) with letrozole co-treatment. No significant effect on LBR, clinical pregnancy rate (CPR), or ovarian hyperstimulation syndrome rate was demonstrated. Only two studies reported on high responders and revealed no effect on LBR or CPR. Overall, the endometrium thickness was slightly affected, where as the, miscarriage rate and cancellation rate were unaffected by letrozole co-treatment. None of the included studies reported on neonatal outcomes. The quality of evidence was high or moderate in the RCTs and low in the observational studies. In conclusion, poor responders may benefit from co-treatment with letrozole during ovarian stimulation for IVF, whereas letrozole for normal and high responders requires further investigation with larger, high-quality studies. Vaccinations are an effective choice to stop disease outbreaks, including COVID-19. There is little research on individuals' COVID-19 vaccination decision-making. We aimed to determine individual preferences for COVID-19 vaccinations in China, and to assess the factors influencing vaccination decision-making to facilitate vaccination coverage. A D-efficient discrete choice experiment was conducted across six Chinese provinces selected by the stratified random sampling method. Vaccine choice sets were constructed using seven attributes: vaccine effectiveness, side-effects, accessibility, number of doses, vaccination sites, duration of vaccine protection, and proportion of acquaintances vaccinated. Conditional logit and latent class models were used to identify preferences. Although all seven attributes were proved to significantly influence respondents' vaccination decision, vaccine effectiveness, side-effects and proportion of acquaintances vaccinated were the most important. We also found a higher probability of vaccinating when the vaccine was more effective; risks of serious side effects were small; vaccinations were free and voluntary; the fewer the number of doses; the longer the protection duration; and the higher the proportion of acquaintances vaccinated. Higher local vaccine coverage created altruistic herd incentives to vaccinate rather than free-rider problems. The predicted vaccination uptake of the optimal vaccination scenario in our study was 84.77%. Preference heterogeneity was substantial. Individuals who were older, had a lower education level, lower income, higher trust in the vaccine and higher perceived risk of infection, displayed a higher probability to vaccinate. Preference heterogeneity among individuals should lead health authorities to address the diversity of expectations about COVID-19 vaccinations. To maximize COVID-19 vaccine uptake, health authorities should promote vaccine effectiveness; pro-actively communicate the absence or presence of vaccine side effects; and ensure rapid and wide media communication about local vaccine coverage. In the search for a reliable biomarker able to diagnose immunological causes of infertility, uterine immune cells have been widely investigated. As a result, heterogeneous methods and cutoff values of what constitutes an aberrant number of immune cells have been reported, and a standardized method for quantification is needed. The objective of this study was to compare methods for quantification of immune cells visualized with immunohistochemistry in the endometrium of women in fertility treatment. Evaluation of the density of CD56+, CD16+ and CD163+ cells by conventional microscopy on a semiquantitative scale (low, medium and high) was compared to a continuous count using digital image analysis (DIA) reported as percentage positive cells out of the total number of stromal cells and number of positive cells per mm2, respectively. We previously reported the CD56/CD16 ratio as a possible prognostic marker, and therefore the ratios of CD56/CD16 were compared using two different methods for selecting fields for counting with DIA: one method using principles of systematic random sampling, where glands were excluded, and one method analyzing large parts of the tissue including glands. A significant association between conventional microscopy and DIA was found when the semiquantitative scale was compared to medians of positive cells in CD56, CD16 and CD163, respectively, p < 0.001. A systematic significant difference in the ratios of CD56/CD16 was found when comparing the two methods for field selection, p < 0.001. While a nursing intervention program for immobile patients with stroke can improve clinic outcomes, less is known about the cost-effectiveness of these interventions. The goal of this study was to evaluate the cost-effectiveness of the intervention program for immobile patients with stroke in China. A cost-effectiveness analysis alongside a pre-test/post-test (before and after) study was undertaken from a health care perspective. Participants were recruited from 25 hospitals among six provinces or municipal cities in eastern (Guangdong province, Zhejiang province, and Beijing municipal city), western (Sichuan province), and central (Henan province and Hubei province) China. A total of 7,653 immobile stroke patients were included in our sample. Patients in routine care settings were recruited from November 2015 to June 2016, and the recruitment of the intervention group patients was from November 2016 to July 2017. To adjust for potential bias from confounding variables, the 1:1 propensity score matching yielded matched pairs of 2,966 patients in the routine care group and 2,966 patients in the intervention group, with no significant differences in sociodemographic or clinical characteristics between two groups. All patients were followed-up 3 months after enrolment in the study. Total healthcare costs were extracted from the hospital information system, with the health outcome effectiveness of the intervention program measured using the EuroQol five-dimensional questionnaire (EQ-5D) instrument and the cost-effectiveness of the intervention measured by the incremental cost-effectiveness ratio with a time horizon of 3 months. Compared to routine care, the intervention program decreased the total costs of stroke patients by CN¥4,600 (95% confidence interval [CI]: [-7050, -2151]), while increasing quality-adjusted life year 0.009 (95% CI: [0.005, 0.013]). The incremental cost-effectiveness ratios over 3 months was CN¥–517,011 per quality-adjusted life year (95% CI: [-1,111,442, -203,912]). Subgroup analysis reveals that both the health-related quality of life and cost effectiveness improved significantly for ischemic patients and tertiary hospitals patients while for hemorrhagic patients and non-tertiary hospital patients only the health-related quality of life improved significantly. Findings from this first cost-effectiveness analysis in immobile stroke patients provide evidence that an intervention program provided significant cost saving, but mainly in ischemic patients and tertiary hospital patients. Wider adoption of such programs may be a sensible approach to reducing the burden of stroke and for immobile patients more generally. [ABSTRACT FROM AUTHOR]
