20. Maturity-onset diabetes of the young (MODY) - in search of ideal diagnostic criteria and precise treatment.

Abstract

Maturity-onset diabetes of the young (MODY) is a spectrum of clinically heterogenous forms of monogenic diabetes mellitus characterized by autosomal dominant inheritance, onset at a young age, and absence of pancreatic islets autoimmunity. This rare form of hyperglycemia, with clinical features overlapping with type 1 and type 2 diabetes mellitus, has 14 subtypes with differences in prevalence and complications occurrence which tailor therapeutic approach. MODY phenotypes differ based on the gene involved, gene penetrance and expressivity. While MODY 2 rarely leads to diabetic complications and is easily managed with lifestyle interventions alone, more severe subtypes, such as MODY 1, 3, and 6, require an individualized treatment approach to maintain a patient's quality of life and prevention of complications. This review summarizes current evidence on the presentation, diagnosis, and management of MODY, an example of a genetic cause of hyperglycemia that calls for a precision medicine approach. 48. Use of connectivity index and simple topological parameters for estimating the inhibition potency of acetylcholinesterase.

Abstract

Acetylcholinesterase (AChE) has proven to be an effective drug target in the treatment of neurodegenerative diseases such as Alzheimer's, Parkinson's and dementia. We developed a novel QSAR regression model for estimating potency to inhibit AChE, p K i , on a set of 75 structurally different compounds including oximes, N-hydroxyiminoacetamides, 4-aminoquinolines and flavonoids. Although the model included only three simple descriptors, the valence molecular connectivity index of the zero-order, 0 χv , the number of 10-membered rings (nR10) and the number of hydroxyl groups (nOH), it yielded excellent statistics (r = 0.937, S.E. = 0.51). The stability of the model was evaluated when an initial set of 75 compounds was broadened to 165 compounds in total, with the increase of the range of p K i (exp) from 6.0 to 10.2, yielding r = 0.882 and S.E. = 0.89. The predictive power of the model was evaluated by calculating p K i values for 55 randomly chosen compounds (S.E. test = 0.90) from the calibration model created on other 110 compounds (S.E. = 0.89), all taken from the pool of 165 compounds. Abstract

Abstract Dynamic structuring of water is a key player in a large class of processes underlying biochemical and technological developments today, the latter often involving electric fields. However, the anisotropic coupling between the water structure and the field has not been understood on a molecular level so far. Here we perform extensive molecular dynamics simulations to explore the influence of an externally imposed electric field on liquid water under ambient conditions. Using self-developed analysis tools and rigorous statistical analysis, we unambiguously show that water hydration shells break into subcompartments, which were hitherto not observed due to radial averaging. The shape of subcompartments is sensitive to the field magnitude and affects excitations of the hydrogen bond network including the femtosecond stretching and the sub-picosecond restructuring of hydrogen bonds. Furthermore, by analysing the reorientational dynamics of water molecules, we ascertain the existence of cooperative excitations of small water clusters. Enabled by the interplay between hydrogen bonding, and the coupling of water dipoles to the field, these coordinated motions, occurring on the picosecond time scale, are associated with fluctuations between torque-free states of water dipoles. We show that unlike the coupling between translation and reorientation of water molecules, which takes place on even longer time scales, these coordinated motions are the key for understanding the emergent anisotropy of diffusion and viscosity of water. Particular effort is invested to provide an analysis that allows for future experimental validation. Highlights • Energy correction procedure on the cross-border energy exchange is presented. • Measurement uncertainty and error contributions are included. • Sensitivity analysis of the mathematical model for energy correction is presented. • Voltage phase displacement has the lowest percentage impact to the corrected energy. • Voltage drop has the highest impact to the corrected energy. Abstract Cross-border energy exchange in the electricity market environment requires high quality measurement and accounting procedure considering the fairness in technical and economic sense. In order to fulfill these requirements, the necessary step is to clarify all uncertainty contributions at the metering point and to correct the exchanged energy for corresponding systematic errors. This approach resulted by a mathematical model for energy correction. The model is briefly presented and the sensitivity analysis of the model is performed: the impact of the current, voltage and phase displacement of the measuring transformers, error of the energy meter and voltage drop in voltage measurement circuits to the corrected energy.
