2. Immune responses to SARS-CoV-2 mRNA vaccination in people with idiopathic CD4 lymphopenia.

- Abstract
The immunogenicity of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) mRNA vaccines is variable in individuals with different inborn errors of immunity or acquired immune deficiencies and is yet unknown in people with idiopathic CD4 lymphopenia (ICL). We sought to determine the immunogenicity of mRNA vaccines in patients with ICL with a broad range of CD4 T-cell counts. Samples were collected from 25 patients with ICL and 23 age- and sex-matched healthy volunteers (HVs) after their second or third SARS-CoV-2 mRNA vaccine dose. Anti-spike and anti-receptor binding domain antibodies were measured. T-cell receptor sequencing and stimulation assays were performed to quantify SARS-CoV-2–specific T-cell responses. The median age of ICL participants was 51 years, and their median CD4 count was 150 cells/μL; 11 participants had CD4 counts ≤100 cells/μL. Anti-spike IgG antibody levels were greater in HVs than in patients with ICL after 2 and 3 doses of mRNA vaccine. There was no detectable significant difference, however, in anti-S IgG between HVs and participants with ICL and CD4 counts >100 cells/μL. The depth of spike-specific T-cell responses by T-cell receptor sequencing was lower in individuals with ICL. Activation-induced markers and cytokine production of spike-specific CD4 T cells in participants with ICL did not differ significantly compared with HVs after 2 or 3 vaccine doses. Patients with ICL and CD4 counts >100 cells/μL can mount vigorous humoral and cellular immune responses to SARS-CoV-2 vaccination; however, patients with more severe CD4 lymphopenia have blunted vaccine-induced immunity and may require additional vaccine doses and other risk mitigation strategies. Kawasaki disease (KD) is the leading cause of acquired heart disease. The cardiac clinical features seen with KD require diagnosis and treatment within 10 days of symptoms to decrease the risk of complications. 10. Creating a sustainable pediatric diabetes transition program.

- Abstract
A health care transition focused quality improvement project was implemented in a large urban pediatric endocrinology clinic to evaluate the effectiveness of the administration of the Transition Readiness Assessment Questionnaire (TRAQ) by identifying barriers to implementation and creating a more sustainable format. A pre- and post-intervention evaluation design was used in this quality improvement project. A twelve-month chart review of pediatric endocrine clinic patients diagnosed with diabetes who were 14 years of age or older and met eligibility criteria was conducted. Patients given the TRAQ tool were identified and assessed for documentation of goals and goal achievement. Discussions regarding the implementation of the TRAQ tool were conducted with clinic staff to identify process barriers and develop solutions. A post-implementation twelve-month review was conducted to evaluate for improved documentation and achievement of transition focused goals. Several barriers were identified that minimized the effectiveness of the TRAQ tool including lack of staff trained to assist with insulin pump and meter downloads and proximity of diabetes software. Additional staff were trained, and software was relocated to a more centrally located area with greater staff accessibility to allow for discussion of transition goals with patient and family. The new process resulted in a 100% increase in documentation of transition goals and met goals (p ≤0.001). The TRAQ tool is valuable for directing transition needs if implementation barriers such as staff training and accessibility to software are monitored and addressed. Frequent evaluation of the administration of the TRAQ tool protocol in the clinic setting is recommended in order to support pediatric patients' successful transition to adult care. Research suggests that children with a lower metacognitive thinking level improve their metacognitive thinking and demonstrate greater strategy use by interacting with children operating at a higher metacognitive knowledge level while being directed to explicitly discuss strategies. Treatment group membership serves to enhance recall performance and sorting strategy. Students can benefit from peer collaboration without some of the students being designated tutors and without direct teacher instruction.

Combination therapy approaches are becoming the mainstay of treatment for a variety of neoplastic and infectious diseases, including human immunodeficiency virus (HIV)-1 infections. The advantages of combination therapy include potentially additive-to-synergistic antiviral interactions, decreased emergence of or wider coverage against resistant viruses, and broadened effect both in terms of cell types and tissue reservoirs. However, combination drug interactions may range from synergy to antagonism. Both in vitro and in vivo studies have identified agents demonstrating additive-to-synergistic effects when used together and support the use of combination strategies in clinical practice. 22. Improved Confidence, Competence, and Knowledge in Child-Focused APN Clinicians Following Participation in Infant Motor Development Course.

th National Conference on Pediatric Health Care, March 16th , 2023. Atypical motor development may manifest as early as the first 6 months of life. Early referral to therapeutic services can improve long-term outcomes by leveraging neuroplasticity during infancy. However, training programs and continued medical education initiatives infrequently focus on motor development during early infancy. To measure child-focused APN clinicians' confidence and competence in screening and identifying atypical motor development in infants 0-6 months old, before and after completing a novel online curriculum. A multidisciplinary team including experts in pediatric primary care, neuromuscular medicine, early intervention, physical therapy and medical education collaborated to develop an online curriculum with evidence-based guidance, expert interviews, real-life footage and a novel four-position exam sequence for screening and identifying atypical motor patterns during well child visits ≤6 months. The training was offered in-person to a group of practicing APNs at a tertiary care center and offered online to a group of APN trainees. Participants completed a self-assessment of confidence and a knowledge self-assessment, comprised of 11 questions about risk factors, red flags and developmental milestones before and after participating in the course. Paired t-tests were used to compare pre- versus post-course scores. This study was reviewed and found to be exempt by the Lurie Children's Institutional Review Board. Overall, 23 of the child-focused clinicians attended the practicing APN session and 19 trainee APNs completed the online course and the pre- and post-intervention self-assessments. Ninety-three percent of clinicians who attended the practicing APN session reported providing care to infants <6 months old, and all reported practicing in pediatric subspecialty care. Both groups demonstrated increases in their mean proportion of correct answers to knowledge questions, rising from 41% to 51% (practicing APNs) and 33% to 56% (trainee APNs). Fifty nine percent of practicing APNs and 56% of trainee APNs reported receiving none or a little training in assessing motor development for 2-month-old infants prior to taking the course. Following course completion, confidence (extremely or very confident) in recognizing atypical motor development in 2-month-old infants increased from 0% to 52% for practicing APNs and from 4% to 63% for trainee APNs. We observed shifts toward greater confidence across practicing APNs and trainees for all infant ages following completion of the course. This novel course teaching assessment of atypical motor development in infants increased the knowledge, competence and confidence of practicing and training APN clinicians. This easily scalable course can be offered more broadly to child-focused clinicians who can intervene to address atypical motor development early in infancy. Changes in diagnoses and referral patterns should be examined to examine long-term outcomes that may result from participation in this course. [ABSTRACT FROM AUTHOR]- Published

29. Quality of Life and Health Outcomes in Overweight and Non-Overweight Children With Asthma.

- Abstract
Background During the past two decades, the number of children and adolescents who are obese has more than doubled. Following this trend in childhood obesity, there has been an alarming increase in the number of children with asthma who are overweight. Objective The aim of this study was to examine the differences in quality of life (QOL) and health outcomes of obese and overweight children with asthma compared with normal-weight children with asthma using a descriptive comparative survey design. Methods This comparative study consisted of 90 overweight and normal-weight 9- to 14-year-olds with asthma. Health outcomes examined included asthma-related missed number of school days, emergency department (ED) visits, hospitalizations, number of days wheezing, and number of night awakenings. QOL was measured using the Pediatric Asthma Quality of Life Questionnaire. Results The obese group reported the highest percentage of ED visits, hospitalizations, and number of days wheezing compared with the normal-weight group. A risk ratio analysis showed that the obese group had an overall 2.73 (95% confidence interval [CI] 2.15, 3.63) times higher likelihood of going to the ED and a 2.46 (95% CI 1.97, 3.19) times higher likelihood of hospitalization than the overweight and normal-weight groups. Asthma severity was a significant predictor of overall QOL (β = −23.737, p = .002). Conclusions The study results are consistent with other investigations that demonstrate that obese persons are at higher risk of experiencing severe asthma symptoms and support obesity as a potentially modifiable risk factor for asthma mitigation and prevention. - Abstract
The purpose of this prospective, randomized study was to compare the early clinical and functional results of primary total knee arthroplasty using a fixed-bearing (FB) and a rotating-platform (RP) prosthesis. Outcomes including range of motion (ROM), Knee Society Score, Western Ontario MacMaster (WOMAC), and Short Form-36 (SF-36) were measured preoperatively and at 6 weeks, 3 months, 6 months, 1 year, and 2 years. Radiographic analysis was performed. There were 72 FB and 68 RP knees. The RP group had a greater ROM at 6 weeks and 1 year. This difference was not statistically significant at 2 years. There were no differences in the ROM at any other period. There were no significant differences in Knee Society Score, Short Form-36, or Western Ontario MacMaster scores at any period. No clinically significant differences were noted in the radiographic analysis. The use of a FB or RP design did not affect the early functional outcomes after total knee arthroplasty. Service learning is a mechanism used in engineering and construction curricula as a means of engaging students in interesting and complex problems that benefit the community while providing an opportunity to develop competencies that will be essential in professional practice. Such projects are particularly effective in exposing students to issues otherwise difficult to teach in a classroom setting, including critical resource constraints and tactics to overcome them, tradeoffs among possible solution alternatives, integration of systems, and the human and organizational challenges of design and implementation. However, the risks and challenges involved in integrating these projects within classes and managing student involvement lead them to be underutilized in many curricula. This paper describes experiences with two separate community-based service learning projects undertaken as part of a joint undergraduate-graduate class on sustainable facility systems at Virginia Tech. While projects were initially designed to be of similar scope and difficulty, the outcomes were dramatically different from a project management and implementation standpoint, and learning outcomes for students were also different. Lessons learned are presented for the benefit of others considering expanded service learning as part of coursework on sustainability in engineering and construction curricula. [ABSTRACT FROM AUTHOR]

Abstract: The chemical composition, oxidative stability, total phytosterols and tocopherols of pecan nut oil; the composition, color parameters and infrared spectroscopy of the pecan shell powder; the total phenolic compounds, condensed tannins and antioxidant activity (ABTS and DPPH) in extracts obtained by infusion of the pecan nut shell powder, were determined for samples of two consecutive years and different varieties. Furthermore, the possible effects caused by the use of spray drying technology were assessed. There was a significant effect for different harvest year and variety for sample quality and nutritional content of phytochemicals in the oil and nutshell. The results indicated a relationship between the content of unsaturated fatty acids present in the oil and the concentration of antioxidant compounds in the shell. Although phenolic compounds are considered to be thermally unstable, no significant losses were observed after the spray-drying process. [Copyright &y& Elsevier]

Abstract: The isomerization and decomposition reactions of 2-pentyl and 3-pentyl radicals have been studied in a single-pulse shock tube over a temperature range of 973–1121K and pressures of 120–800kPa. The results represent the first direct study of the alkene product branching ratio resulting from the kinetics of the competition between isomerization and beta C–C bond scission for a secondary straight-chain alkyl radical at high temperatures. Such species are representative of intermediates important in the combustion of typical hydrocarbon fuels. In the present work, a small quantity of precursor (∼45μL/L) is used to thermally generate H atoms in the presence of excess (E)-2-pentene, leading to the radicals of interest via addition of H to the double bond. Decomposition of the chemically activated pentyl radicals results in the stable olefin products ethene, propene, and 1-butene, which are detected in postshock gas chromatographic analyses utilizing flame-ionization and mass-spectrometric detection. It is shown that the olefin product ratios can be related to the isomerization and decomposition reactions of the 2-pentyl and 3-pentyl radicals and the results are consistent with the existence of distinct non-overlapping cracking patterns for the two radicals. The data are compared with predictions made on the basis of a model developed from experiments on the decomposition of thermal (i.e. not chemically activated) 1-pentyl radicals. Good agreement is observed. In conjunction with an RRKM/Master Equation analysis, the results for 2-pentyl and 3-pentyl radicals are projected over a wide range of temperatures. In addition, the rate constants for addition of H atoms to the alternate double bond positions of (E)-2-pentene are derived relative to a standard reaction and absolute rate constants for these processes are reported. Abstract: Recent studies have identified uncertainties in fuel diffusion coefficients as a source of significant uncertainty in combustion modeling. This paper presents accurate binary diffusion coefficients of linear hydrocarbons in helium and nitrogen at temperatures from 300K to 723K. Diffusion coefficients are determined using a reversed-flow gas chromatography (RF-GC) system. Earlier work in our laboratory has established the validity of this methodology for noble gases and methane under these elevated-temperature conditions, and we utilize the methodology to measure diffusion coefficients for longer chain systems of interest in combustion modeling: ethane, propane, and n-butane. For propane and n-butane in nitrogen, the present results show dramatically different (up to 16.9% deviation at the highest common temperature) diffusion coefficients than previous work in addition to extending the available temperature range. The effects of decomposition of the analyte at higher temperatures and adhesion of the analyte on the diffusion column at lower temperatures during the measurements are briefly considered but have only a small impact on the present systems. Abstract: Iodine has been used as an effective tool for studying both the structure and composition of dispersed starch and starch granules. In addition to being employed to assess relative amylose contents for starch samples, it has been used to look at the molecular mobility of the glucose polymers within intact starch granules based on exposure to iodine vapor equilibrated at different water activities. Starches of different botanical origin including corn, high amylose corn, waxy corn, potato, waxy potato, tapioca, wheat, rice, waxy rice, chick pea and mung bean were equilibrated to 0.33, 0.75, 0.97 water activities, exposed to iodine vapor and then absorbance spectra and LAB color were determined. In addition, a new iodine quantification method sensitive to <0.1% iodine (w/w) was employed to measure bound iodine within intact granular starch. Amylose content, particle size distribution of granules, and the density of the starch were also determined to explore whether high levels of long linear glucose chains and the surface area-to-volume ratio were important factors relating to the granular iodine binding. Results showed, in all cases, starches complexed more iodine as water content increased and waxy starches bound less iodine than their normal starch counterparts. However, much more bound iodine could be measured chemically with waxy starches than was expected based on colorimetric determination. Surface area appeared to be a factor as smaller rice and waxy rice starch granules complexed more iodine, while the larger potato and waxy potato granules complexed less than would be expected based on measured amylose contents. Corn, high amylose corn, and wheat, known to have starch granules with extensive surface pores, bound higher levels of iodine suggesting pores and channels may be an important factor giving iodine vapor greater access to bind within the granules. Exposing iodine vapor to moisture-equilibrated native starches is an effective tool to explore starch granule architecture. [Copyright &y& Elsevier]

Abstract: The kinetics of the thermal decomposition of cyclopentyl radicals has been studied in a single-pulse shock tube over a temperature range of 950–1116K and pressures of 2.3–3.4bar. Cyclopentyl radicals were generated by abstraction of H from cyclopentane by H atoms, which were produced by the thermal decomposition of hexamethylethane. Products were analyzed using dual-column gas chromatography under cryogenic conditions with simultaneous flame-ionization and mass-spectrometric detection. The main stable products resulting from the decomposition of cyclopentyl radicals are ethene and cyclopentene. These results are consistent with the interconversion of cyclopentyl and pent-4-en-1-yl radicals. The product branching ratio of ethene to cyclopentene is determined as [ethene]/[cyclopentene]=(8.2±0.7)exp[−(1067±88)/T), where the listed uncertainty (1σ) is from the statistical analysis. The overall accuracy in our measurement of the branching ratio is estimated at about 8% (1σ). In contrast to previous estimates, ring opening of cyclopentyl is found to be faster than ejection of a hydrogen atom. The former reaction is favored by about a factor of three. An RRKM/Master Equation treatment was adopted to assign the rate parameters to elementary reactions. Details of the model and its compatibility with the present and literature results are presented. Kinetic results for the pyrolysis of cyclopentyl and pent-4-en-1-yl radicals have been derived over relevant combustion conditions [0.1–1000bar, 700–1900K]. [ABSTRACT FROM AUTHOR]
