22. A Comprehensive Workflow of Selecting Local Tolerance Limits to Detect Systematic Multi-Leaf Collimator Positional Errors by Combining Statistical Process Control and Error Curve Model.

Abstract

This study aims to develop a comprehensive and scientifically sound methodology for determining appropriate local tolerance limits in patient-specific quality assurance (QA). A total of 214 volumetric modulated arc therapy/VMAT plans for cervical cancer were selected. Systematic MLC positional errors were simulated across eighteen magnitudes ranging from ± 0.2 to ± 5 mm. Dose verification was conducted on 808 volumetric modulated arc therapy/VMAT plans, and a retrospective review was carried out. Firstly, six commonly used QA metrics in gamma and DVH analysis were extracted from the QA results of 196 error-free volumetric modulated arc therapy/VMAT plans. These QA metrics included GP 10 (gamma passing rate at 3%/2mm, 10% dose threshold), GP 50 (gamma passing rate at 3%/2mm, 50% dose threshold), μGI 50 (mean gamma index at 3%/2mm, 50% dose threshold), PTV 95 (dose received by 95% of PTV), PTV 5 (dose received by 5% of PTV) and PTV mean (mean dose received by PTV). Statistical process control was used to establish the corresponding tolerance limits for each metric. Subsequently, six error curve models were created based on 360 error-introduced plans to record changes of QA metrics under different magnitudes of MLC positional error. The error range of theoretical detection limits for systematic MLC positional errors was investigated to assess error sensitivity quantitatively using the error curve model. Finally, the process-based tolerance limits of the six individual QA metrics and four hybrid QA metrics were validated by using the QA results of 252 test plans. The binary classification performance (error-free/error-introduced) was assessed based on error detection rate, accuracy, precision, recall, and f1-score. The theoretical detection limits for process-based tolerance limits of GP 10 , GP 50 , μGI 50 , PTV 95 , PTV mean , and PTV 5 were 2.19 mm, 2.71 mm, 3.52 mm, 1.93 mm, 3.20 mm, and 2.15 mm, respectively. In the validation phase, the process-based tolerance limits for PTV 95 effectively identified systematic MLC positional errors exceeding 0.6 mm with an error detection rate of 76.19%, displaying superior performance in binary classification among individual evaluation metrics. Regarding the hybrid metrics, the joint evaluation of process-based tolerance limits for GP 10 and PTV 95 showed a higher error detection rate of 80.16% for systematic MLC positional errors exceeding 0.6 mm. The proposed workflow integrates the establishment and validation of local tolerance limits. It does not only provide a practical tool for setting local tolerance limits based on actual clinical scenarios but also provides a quantitative method for medical physicists to understand the error sensitivity of the selected local tolerance limits. Abstract

This systematic review was conducted to estimate the respective prevalence of gonorrhea among two high-risk populations in China and determine the epidemiological features of gonorrhea in them. Systematic review. PubMed, Web of Science, China National Knowledge Infrastructure, and Wanfang databases were searched to identify studies published between January 1, 1990, and October 31, 2022, with gonorrhea prevalence tested by polymerase chain reaction among female sex workers (FSWs) and men who have sex with men (MSM). Meta-regression and subgroup analyses were used to investigate potential factors of heterogeneity across studies. Trend analysis of prevalence was conducted by the Jonckheere-Terpstra method. We identified 88 prevalence data points from 49 studies in China, with 30,853 participants of FSWs and 5523 participants of MSM. Pooled prevalence of gonorrhea among FSWs and MSM were 6.9% (95% confidence interval: 4.6–9.7%) and 2.5% (95% confidence interval: 1.5–3.7%), respectively. The subgroup analyses showed there were period, regional, and specimen collection methods diversities among FSWs, and diversities of the regions and specimen collection anatomical sites were found among MSM, in which the prevalence of rectum and pharynx was significantly higher than the urethra. A decreasing trend in the prevalence of gonorrhea was seen among FSWs (z = –4.03) from 1999 to 2021, not found for MSM in China. The prevalence of gonorrhea is high in two high-risk groups in China, with extragenital infections requiring particular attention. Abstract

The response to neoadjuvant radiotherapy (RT) in colorectal cancer (CRC) patients is compromised partially due to the plasticity and heterogeneity of cancer-associated fibroblasts (CAFs). However, the underlying mechanisms remain unclear. Our study aims to unravel the cues instigating and sustaining distinctive RT induced CAFs subpopulation within the tumor microenvironment (TME) and explores strategies for the repolarization of this CAFs. The hypothesis of this study is that Interferon-dependent CAFs (irCAFs) promote RT-Induced anti-tumor immunity through the interferon gamma (IFN-γ) / signal transducer and activator of transcription 1(STAT1) Pathway in colorectal cancer. To comprehensively unravel the cellular intricacies of CAFs and intercellular interactions in CRC patients post-RT, we using single-cell RNA sequencing, multiparametric flow cytometry, and multiplexed immunofluorescence staining to identified a subpopulation of irCAFs. To probe the functionality of irCAFs and unravel the underlying mechanisms, we established three preclinical CRC models (MC38 syngeneic tumor-bearing mice, autochthonous APCmin/+ CRC tumors and Azoxymethane (AOM)/Dextran sodium sulfate (DSS) induced colitis-associated cancer (CAC) models.) for in vivo monitoring of the effects of RT. In this study, utilizing single-cell RNA sequencing of CRC patient samples, we identified a subpopulation of CAFs characterized by the overexpression of interferon regulatory factor 1 (IRF1). The enrichment of these IRF1-expressing CAFs (irCAFs) enhances RT responses in various solid tumors, including CRC. Mechanistically, IFN-γ signaling induces irCAFs polarization, and recruits T and dendritic cells through CCL4/CCL5 secretion. The activation of IFN-γ/STING signaling leads to stromal reprogramming and augments anti-tumor immunity in both RT-sensitive and resistant CRC tumors. Inhibiting the enrichment of irCAFs by STING silencing in CAFs reduces the vulnerability of tumors to RT. Combining STING agonists with RT results in robust tumor control, providing a compelling rationale for testing this therapeutic strategy in clinical trials. Cumulatively, our findings emphasize the translational potential of combining STING agonists with radiation therapy, warranting exploration in additional clinical settings, particularly in patients who have developed resistance to radiotherapy.
