Model predictive control of residential air conditioning could reduce energy costs and greenhouse gas emissions while maintaining or improving occupants' thermal comfort. However, most approaches to predictive air conditioning control either do not model indoor humidity or treat it as constant. This simplification stems from challenges with modeling indoor humidity dynamics, particularly the high-order, nonlinear equations that govern heat and mass transfer between the air conditioner's evaporator coil and the indoor air. This paper develops a machine-learning approach to modeling indoor humidity dynamics that is suitable for real-world deployment at scale. This study then investigates the value of humidity modeling in four field tests of predictive control in an occupied house. The four field tests evaluate two different building models: One with constant humidity and one with time-varying humidity. Each modeling approach is tested in two different predictive controllers: One that focuses on reducing energy costs and one that focuses on constraining electric power below a utility-specified threshold. The two models lead to similar performance for reducing energy costs. Combining the results of this study and a prior heating study of the same house, the estimated year-round energy cost savings were $340–497 or 22%–31% (95% confidence intervals); these savings were consistent across both humidity models. A novel nuclear waste form was synthesized for the disposal of electrorefiner salt from pyrometallurgical reprocessing of used nuclear fuel. Its synthesis constitutes two steps: waste salt dehalogenation via ion exchange with H–Y zeolite, followed by thermal treatment for phase transition and consolidation. Salt cation-loaded zeolite exhibiting greater than 90% dehalogenation was sintered for 12 h at 925 °C and achieved 3 vol% open porosity (determined via Archimedes method). Microstructural imaging and structural characterization revealed a multiphase assemblage of alkali aluminosilicate and mixed oxide phases. Chemical durability experiments were performed using modified ASTM C1308 tests in demineralized water and different silica solutions. Si-containing species appear to exhibit congruent dissolution, while the release behaviors of alkali elements appear to include diffusion-limited contributions. The 4-day cumulative releases for the dehalogenated salt waste form (DSWF) are less than 33% of those measured for the baseline glass-bonded sodalite advanced ceramic waste form (GBS ACWF). Furthermore, the dehalogenation and consolidation processing steps provide a 35% decrease in required storage volume for a given waste salt mass relative to the GBS ACWF. Organic chemicals from industrial, agricultural, and residential activities can enter surface waters through regulated and unregulated discharges, combined sewer overflows, stormwater runoff, accidental spills, and leaking septic-conveyance systems on a daily basis. The impact of point and nonpoint contaminant sources can result in adverse biological effects for organisms living in or near surface waters. Assessing the adverse or toxic effects that may result when exposure occurs is complicated by the fact that many commonly used chemicals lack toxicity information or water quality standards. To address these challenges, an exposure-activity ratio (EAR) screening approach was used to prioritize environmental chemistry data in a West Virginia watershed (Wolf Creek). Wolf Creek is a drinking water source and recreation resource with documented water quality impacts from point and nonpoint sources. The EAR screening approach uses high-throughput screening (HTS) data from ToxCast as a method of integrating environmental chemical occurrence and biological effects data. Using water quality schedule 4433, which targets 69 organic waste compounds typically found in domestic and industrial wastewater, chemicals were screened for potential adverse biological affects at multiple sites in the Wolf Creek watershed. Cumulative EAR mixture values were greatest at Sites 2 and 3, where bisphenol A (BPA) and pentachlorophenol exhibited maximum EAR values of 0.05 and 0.002, respectively. Site 2 is downstream of an unconventional oil and gas (UOG) wastewater disposal facility with documented water quality impacts. Low-level organic contaminants were found at all sample sites in Wolf Creek, except Site 10, where Wolf Creek enters the New River. The application of an EAR screening approach allowed our study to extend beyond traditional environmental monitoring methods to identify multiple sites and chemicals that warrant further investigation. Both global and regional climate drivers contributed to glaciation during the late Paleozoic Ice Age (LPIA). However, the transition from icehouse to greenhouse conditions was asynchronous across Gondwana suggesting that, in some cases, regional controls played a significant role in deglaciation. Of particular interest to understanding changing LPIA climatic conditions, is the eastern Paganzo Basin. This region was flanked by ice centers in the Precordilleran and Sierras Pampeanas regions of Argentina on the west, and major ice sheets in the Paraná, Chaco-Paraná, and Sauce Grande basins to the east, all of which resided between ∼40 and 65° S latitude. Hypotheses on the occurrence of ice in the eastern Paganzo Basin are based on interpretations of the narrow, steep-walled, Olta-Malanzán paleovalley as carved by an alpine glacier or by an outlet glacier draining an eastern ice sheet, and that glaciers deposited coarse clastics within the paleovalley. However, we found no evidence for glaciation. Rather, gravel from prograding alluvial fans/fan deltas and rock falls ponded drainage resulting in lacustrine activity in the eastern end of the valley. A transition from either subaerially or shallow subaqueously deposited sandstones to marine mudstones in the western end of the Olta paleovalley suggest a marine transgression, which, in turn, was overlain by deposits of prograding Gilbert-type deltas. Dropstones were from rock falls off valley walls and rafting by lake ice rather than from icebergs. Therefore, we conclude that the climate in western Argentina resulted from uplift induced glaciation in the Precordilleran region and along the western margin of the Paganzo Basin, and the occurrence of a precipitation shadow to the east. The disappearance of the western glaciers during the mid-Carboniferous, prior to deglaciation elsewhere at the same paleolatitude, resulted from a westward shift in the position of the active margin, collapse of the glaciated upland(s), and an expansion of the precipitation shadow across the whole of western Argentina. Background AIFM1 encodes a mitochondrial flavoprotein with a dual role (NADH oxidoreductase and regulator of apoptosis), which uses riboflavin as a cofactor. Mutations in the X-linked AIFM1 were reported in relation to two main phenotypes: a severe infantile mitochondrial encephalomyopathy and an early-onset axonal sensorimotor neuropathy with hearing loss. In this paper we report two unrelated males harboring AIFM1 mutations (one of which is novel) who display distinct phenotypes including progressive ataxia which partially improved with riboflavin treatment. Methods For both patients trio whole exome sequencing was performed. Validation and segregation were performed with Sanger sequencing. Following the diagnosis, patients were treated with up to 200 mg riboflavin/day for 12 months. Ataxia was assessed by the ICARS scale at baseline, and 6 and 12 months following treatment. Results Patient 1 presented at the age of 5 years with auditory neuropathy, followed by progressive ataxia, vermian atrophy and axonal neuropathy. Patient 2 presented at the age of 4.5 years with severe limb and palatal myoclonus, followed by ataxia, cerebellar atrophy, ophthalmoplegia, sensorineural hearing loss, hyporeflexia and cardiomyopathy. Two deleterious missense mutations were found in the AIFM1 gene: p. Met340Thr mutation located in the FAD dependent oxidoreductase domain and the novel p. Thr141Ile mutation located in a highly conserved DNA binding motif. Ataxia score, decreased by 39% in patient 1 and 20% in patient 2 following 12 months of treatment. Abstract: Magneto-optical measurements are employed to characterize the disorder-driven vortex phase transition and the metastable disordered vortex state in heavy-ion irradiated and pristine regions of the same Bi2Sr2CaCu2O8+δ crystal. We find that dilute columnar defects, while hardly affect the disorder-driven phase transition line, significantly increase the lifetime of the metastable state created in the vicinity of this line. Study of flux injection from the pristine region into the irradiated region suggests that in presence of columnar defects a metastable disordered state may be created in the bulk, in addition to being injected from the edges. OBJECTIVE: Understanding the attitudes of patients to being treated by residents in the community. METHODS: A questionnaire was administered to patients visiting community family medicine teaching clinics. The study methodology included statements to which they agree or disagree. RESULTS: Three hundred and four questionnaires were completed by patients; 94% had visited a resident in the past year; 78.9% agreed that residents were as skilled as senior doctors, but only 45.4% felt that they were as quick at diagnosis as the senior doctors; 73.0% felt that residents spent more time with them; 40.0% were not pleased by the constant change of the residents attending on them. Analysis by logistic regression showed that men had a more positive attitude to the competence of the residents as well as their professionalism (OR 2.73, 95% CI, 1.45-5.10). Frequent visitors to the clinic had a more negative attitude to the residents' professionalism (OR 0.91 (0.85-0.98)) and were more likely to agree with the statement 'I would prefer to see the regular doctor and not a different resident each time' (OR 1.09 (1.01-1.18)). Those who were attended more by residents on their visits showed a positive attitude to the professionalism of residents (OR 1.14 (1.01-1.28)) and were less likely to agree with the statement 'I would prefer to see the regular doctor and not a different resident each time' (OR 0.90 (0.84-0.98)). CONCLUSIONS: Patients have a positive attitude to being treated by residents in ambulatory clinics, which is associated with repeat exposure to care by residents. PRACTICAL IMPLICATIONS: Departments with ambulatory training should consider having constant presence of residents in their teaching clinics, and teaching staff in the clinics should develop ways to recommend patients to be seen by residents. Purpose Chemoradiation (CRT) is an integral treatment modality for patients with locally advanced lung cancer. It has been hypothesized that current use of an angiotensin-converting enzyme inhibitor during CRT may be protective for treatment-related lung damage and pneumonitis. Methods and Materials We conducted a pilot, double-blind, placebo-controlled, randomized trial. Study-eligible patients receiving curative thoracic radiation therapy (RT) were randomly assigned to 20 mg of lisinopril or placebo once daily during and up to 3 months after RT. All patients received concurrent chemotherapy. The primary endpoint was adverse event profiling. Multiple patient-reported outcome (PRO) surveys, including the Lung Cancer Symptom Scale, Function Assessment of Cancer Therapy–Lung, and the European Organisation for Research and Treatment of Cancer Lung Cancer Questionnaire, were applied with a symptom experience questionnaire. Exploratory comparative statistics were used to detect differences between arms with χ2 and Kruskal-Wallis testing. Results Five institutions enrolled 23 patients. However, accrual was less than expected. Eleven and 12 patients were in the placebo and lisinopril arms, respectively (mean age, 63.5 years; male, 62%). Baseline characteristics were balanced. Eighteen patients (86%) were former or current smokers. The primary endpoint was met; neither arm had grade 3 or higher hypotension, acute kidney injury, allergic reaction (medication-induced cough), or anaphylaxis (medication-related angioedema). Few PRO measures suggested that compared with the placebo arm, patients receiving lisinopril had less cough, less shortness of breath, fewer symptoms from lung cancer, less dyspnea with both walking and climbing stairs, and better overall quality of life (for all, P <.05). Conclusions Although underpowered because of low accrual, our results suggest that there was a clinical signal for safety—and possibly beneficial by limited PRO measures—in concurrently administering lisinopril during thoracic CRT to mitigate or prevent RT-induced pulmonary distress. Background: Studies of specific procedures have shown increases in infectious complications with operative duration. We hypothesized that operative duration is independently associated with increased risk-adjusted infectious complication (IC) rates in a broad range of general surgical procedures. Study Design: We queried the American College of Surgeons National Surgical Quality Improvement Program database for general surgical operations performed from 2005 to 2007. ICs (wound infection, sepsis, urinary tract infection, and/or pneumonia) and length of hospital stay (LOS) were evaluated versus operative duration (OD, ie, incision to closure). Multivariable regression adjusted for 38 patient risk variables, operation type and complexity, wound class and intraoperative transfusion. We also analyzed isolated laparoscopic cholecystectomies in patients of American Society of Anesthesiologists class 1 or 2, without intraoperative transfusion and with a clean or clean-contaminated wound class. Results: In 299,359 operations performed at 173 hospitals, unadjusted IC rates increased linearly with OD at a rate of close to 2.5% per half hour (chi-square test for linear trend, p < 0.001). After adjustment, IC risk increased for each half hour of OD relative to cases lasting ≤1 hour, almost doubling at 2.1 to 2.5 hours (odds ratio = 1.92; 95% CI, 1.82 to 2.03; p < 0.001). In isolated laparoscopic cholecystectomy, IC rates increased linearly with OD (n = 17,018, chi-square test for linear trend, p < 0.001) with rates for 1.1 to 1.5 hour cases (1.4%) doubling those lasting ≤0.5 hour (0.7%). Across all procedures, adjusted LOS increased geometrically with operative duration at a rate of about 6% per half hour (coefficient for natural log transformed LOS = 0.059 per half hour; 95% CI, 0.058 to 0.060; p < 0.001). Conclusions: Operative duration is independently associated with increased ICs and LOS after adjustment for procedure and patient risk factors. [Copyright &y& Elsevier]
