16. Exploring the heterogeneity in community pharmacist-led medication review studies – A systematic review.

Abstract

Findings on the effectiveness of medication reviews led by community pharmacists (CPs) are often inconclusive. It has been hypothesized that studies are not sufficiently standardized, and thus, it is difficult to draw conclusions. To examine differences in the way CP-led medication review studies are set up. This was accomplished by investigating (1) patient selection criteria, (2) components of the medication review interventions, (3) types of outcomes, and (4) measurement instruments used. A systematic literature search of randomized controlled trials of CP-led medication reviews was carried out in PubMed and Cochrane Library. Information on patient selection, intervention components, and outcome measurements was extracted, and frequencies were analyzed. Where possible, outcomes were mapped to the Core Outcome Set (COS) for medication review studies. Finally, a network analysis was conducted to explore the influence of individual factors on outcome effects. In total, 30 articles (26 studies) were included. Most articles had a drug class-specific or disease-specific patient selection criterion (n = 19). Half of the articles included patients aged ≥60 years (n = 15), and in 40% (n = 12/30) patients taking 4 drugs or more. In 24 of 30 articles, a medication review was comprised with additional interventions, such as distribution of educational material and training or follow-up visits. About 40 different outcomes were extracted. Within specific outcomes, the measurement instruments varied, and COS was rarely represented. The revealed differences in patient selection, intervention delivery, and outcome assessment highlight the need for more standardization in research on CP-led medication reviews. While intervention delivery should be more precisely described to capture potential differences between interventions, outcome assessment should be standardized in terms of outcome selection by application of the COS, and with regard to the selected core outcome measurement instruments to enable comparison of the results. • Heterogeneity in previous studies is substantial and prevents rigorous evaluation. • Studies vary in terms of patient selection and delivery of medication reviews. • To harmonize, a Core Outcome Set can be used, but is not adequately applied yet. • A consensus must be reached also on how to measure the core outcomes. • Network analysis is a helpful technique to explore factors' influence on results. 23. Liberating Structures in Strategic Planning: Methods to Foster Engagement.

Abstract

This paper describes the innovative approach of using liberating structures to the development of the AUR 2023 strategic plan, and lessons learned in their application. The 2023 strategic plan built on the results and approach of the prior 2015 plan. Similar to the 2015 strategic plan, traditional tools such as a SWOT analysis and strategic retreat were used. In addition, the 2023 process included tools called liberating structures and was iteratively co-produced through a series of virtual meetings over 18 months. Advantages of liberating structures included increased creativity and speed in moving through meeting tasks, increased number of meaningful contributions from AUR members and increased engagement from participants during discussions and meetings. The 2023 AUR strategic plan is provided along with examples of completed goals and those under early implementation. Lessons learned from using these tools for strategic planning can be applied to other society and group meetings. Moving forward, the 2023 strategic plan will be a living document, which will be reviewed at each Board of Directors meeting and periodically adapted. [ABSTRACT FROM AUTHOR]

26. Impact of Artificial Intelligence–Assisted Indication Selection on Appropriateness Order Scoring for Imaging Clinical Decision Support.

Abstract

The aim of this study was to assess appropriateness scoring and structured order entry after the implementation of an artificial intelligence (AI) tool for analysis of free-text indications. Advanced outpatient imaging orders in a multicenter health care system were recorded 7 months before (March 1, 2020, to September 21, 2020) and after (October 20, 2020, to May 13, 2021) the implementation of an AI tool targeting free-text indications. Clinical decision support score (not appropriate, may be appropriate, appropriate, or unscored) and indication type (structured, free-text, both, or none) were assessed. The χ2 and multivariate logistic regression adjusting for covariables with bootstrapping were used. In total, 115,079 orders before and 150,950 orders after AI tool deployment were analyzed. The mean patient age was 59.3 ± 15.5 years, and 146,035 (54.9%) were women; 49.9% of orders were for CT, 38.8% for MR, 5.9% for nuclear medicine, and 5.4% for PET. After deployment, scored orders increased to 52% from 30% (P <.001). Orders with structured indications increased to 67.3% from 34.6% (P <.001). On multivariate analysis, orders were more likely to be scored after tool deployment (odds ratio [OR], 2.7, 95% CI, 2.63-2.78; P <.001). Compared with physicians, orders placed by nonphysician providers were less likely to be scored (OR, 0.80; 95% CI, 0.78-0.83; P <.001). MR (OR, 0.84; 95% CI, 0.82-0.87) and PET (OR, 0.12; 95% CI, 0.10-0.13) were less likely to be scored than CT (; P <.001). After AI tool deployment, 72,083 orders (47.8%) remained unscored, 45,186 (62.7%) with free-text-only indications. Embedding AI assistance within imaging clinical decision support was associated with increased structured indication orders and independently predicted a higher likelihood of scored orders. 48. Maternal vitamin D status modifies the effects of early life tobacco exposure on child lung function.

Abstract

Prior studies suggest that vitamin D may modify the effects of environmental exposures; however, none have investigated gestational vitamin D and cumulative tobacco smoke exposure (TSE) throughout pregnancy and early life. This study investigated the effects of early life TSE on child lung function and the modulatory effects of gestational vitamin D on this association. The VDAART (Vitamin D Antenatal Asthma Reduction Trial) recruited nonsmoking pregnant women and followed the mother-child pairs to age 6 years. TSE was assessed with questionnaires and plasma cotinine measurements in the mothers (10-18 and 32-38 gestational weeks) and children (1, 3, and 6 years). Cumulative TSE was calculated from the repeated cotinine measurements. 25-hydroxyvitamin D (25[OH]D) levels were measured at 10-18 and 32-38 gestational weeks. Lung function was assessed at 6 years with spirometry and impulse oscillometry. Of the 476 mother-child pairs, 205 (43%) had increased cotinine levels at ≥1 time point. Cumulative TSE was associated with decreased FEV 1 (β = −0.043 L, P =.018) and increased respiratory resistance at 5 Hz (R5; β = 0.060 kPa/L/s, P =.002). This association persisted in subjects with insufficient (<30 ng/mL) 25(OH)D levels throughout pregnancy (β = 0.077 kPa/L/s, P =.016 for R5) but not among those with sufficient levels throughout pregnancy. Cumulative TSE from pregnancy to childhood is associated with dose- and duration-dependent decreases in child lung function at 6 years even in the absence of reported maternal smoking. Gestational vitamin D may modulate this effect and have therapeutic potential for minimizing the adverse effect of TSE on lung throughout early life. Abstract

Enhanced weathering (EW) is a CO 2 removal (CDR) and sequestration strategy that accelerates the natural reactions of minerals that can store carbon from the atmosphere and biotic reactions. One method of EW is to apply finely ground silicate rocks to agricultural lands. EW has been demonstrated in laboratory and field tests, but great uncertainty remains regarding the life-cycle of using locally available rocks on candidate soils. We evaluate the life-cycle impacts, job creation, and cost of scenarios where fines and rocks mined from quarries in Oregon and Northern California are transported by truck and tilled into agricultural soils. Candidate quarry dust samples were classified as dacite, andesite, and olivine-bearing rocks, with EW potentials ranging from 125 to 760 kg CO 2 /metric tonne rock. We determined the olivine-bearing rock from Southern Oregon could achieve a levelized cost of CDR under the DOE Earthshot target of $100/t CO 2 , as long as application rates are 25 t/ha or more. Even andesite and dacite materials reach lower costs than commercial direct air capture technologies, with reduction in fines purchase and transport costs critical for achieving the Earthshot target. The results suggest that low-cost EW can be achieved using natural quarry materials, with average removal up to 2.2 t CO 2 e per hectare per year. • Local fines from quarry rocks can be deployed on land for CO 2 removal in Northern California. • Fines from aggregate quarries in the region have removal potentials ranging from 125 to 760 kg CO 2 /tonne rock. • A levelized cost of $96/tonne net CO 2 removal is achieved with olivine-bearing rock fines from Southern Oregon. • Removal rates depend on not only rock properties and weathering rates but land management practices and quarry capacities.
