Ayurveda-based interventions are being given to children and adolescents for their benefits in the treatment of attention deficit hyperactivity disorder (ADHD), but no systematic review of the overall evidence of Ayurveda intervention in ADHD has been conducted. This study aims to determine the effectiveness of Ayurveda intervention in children and adolescents with ADHD. Electronic databases like PubMed, The Cochrane Library (CENTRAL), AYUSH research portal, Digital Helpline for Ayurveda Research Articles, and Researches in Ayurveda—online directory of PG and PhD thesis—were searched in addition to hand searching of Ayurveda journals and clinical trial registries to identify ongoing or completed trials. Randomised controlled trials or quasi-randomised trials or randomised comparative clinical trials comparing Ayurvedic interventions were included. A total of 39 studies were identified; 10 met the inclusion and exclusion criteria and were systematically reviewed. Due to different forms of Ayurvedic intervention used and variable quality of data, only three studies were included in the meta-analysis. The meta-analysis shows that Ayurveda interventions have their benefits in reducing symptoms of ADHD. In this present review, some concerns, like small sample size and low methodological qualities of included studies, limit our ability to draw reliable conclusions regarding the effectiveness of Ayurveda intervention for children and adolescents with ADHD. Well-designed clinical trials reported in adherence to standard reporting guidelines are warranted. • First systematic review to provide evidence of Ayurveda interventions in children and adolescents with attention deficit hyperactivity disorder. • Search strategy adopted was comprehensive, and all the relevant sources were searched for published and unpublished research studies. • Results show Ayurveda interventions are effective with some concerns to small sample size, low methodological qualities, etc. • Well-designed clinical studies adhering to standard norms are needed for future trials. The effect of temperature (from 77 to 473 K) on strain rate sensitivity, apparent and effective activation volume of ultrafine grained tantalum processed by equal-channel angular pressing (ECAP) is investigated in the present work. Microstructural characterization was performed using transmission electron microscopy (TEM) and electron-backscatter diffraction (EBSD). The strain rate sensitivity (SRS) was measured by strain rate jump tests at different temperatures. The SRS shows a monotonous increase with increasing the temperature. A small yield point phenomenon at 77 K during strain rate jump tests was observed because of the low mobility of screw dislocations. The Helmholtz free energy calculated according to the experimental data was estimated to be 1.14 eV. This suggests that the thermal activation process is controlled by dislocation slipping. However, the apparent and effective activation volumes show a power law dependence on the temperature, indicating a variation of rate controlling mechanism in the intermediate temperature range (from 373 to 473 K). It is concluded that the Peierls hill is much lower in the intermediate temperature thus the mechanism of intersection of dislocations becomes more difficult controlling the deformation instead. The paper presents the results of the study conducted to explore the fatigue performance of plain self compacting concrete (SCC) and steel fibre reinforced self compacting concrete (SFRSCC). Approximately 250 flexural fatigue tests and 195 complementary static flexural tests were executed on beam specimens of size 100mm×100mm×500mm under four point flexural loading. The distribution of fatigue life of SCC and SFRSCC was examined at different stress levels, which is a prerequisite for estimating the mean and design fatigue lives. The flexural fatigue performance of SCC and SFRSCC containing three different fibre volume fractions was evaluated in terms of mean and design fatigue lives, and two-million cycles of fatigue strengths/endurance limits. The flexural fatigue performance of SCC as well as SFRSCC was found to be better than the conventionally vibrated concrete (CVC) and fibre reinforced concrete (CVFRC). [ABSTRACT FROM AUTHOR]

Abstract: The paper presents an experimental investigation on the flexural fatigue strength of Self Compacting Fibre Reinforced Concrete (SCFRC) beams. The fatigue life data of SCFRC containing 0.5%, 1.0% and 1.5% by volume of steel fibres have been obtained by conducting flexural fatigue tests on approximately 188 beam specimens of size 100×100×500mm under third point loading at different stress levels, ranging from 0.90 to 0.70. Approximately 144 complimentary static flexural tests were also carried out to facilitate fatigue testing. All the static flexural and flexural fatigue tests were conducted on a 100kN closed loop servo-controlled actuator. The results have been represented in the form of S–N diagrams and to predict the flexural fatigue strength of SCFRC, material coefficients of the fatigue equations have been estimated. Subsequently, family of S–N–Pf curves has been generated from the fatigue test data to graphically represent the relationship between stress level S, fatigue life N, and probability of failure Pf , thus incorporating probability of failure into the fatigue life data of SCFRC. The experimental coefficients of the fatigue equation have also been obtained from the fatigue test data to represent the S–N–Pf curves analytically. The two-million cycles fatigue strength of SCFRC has been found to be higher than that of Normally Vibrated Fibre Reinforced Concrete (NVFRC). [Copyright &y& Elsevier]

In this study, the weather forecasting model of the National Centre for Medium Range Weather Forecasting (NCMRWF) is used for examining the characteristics of round-off-errors on three different computer architectures – PARAM 10K, SUNFIRE 6800 and Dec Alpha for several meteorological parameters such as precipitation, temperature at the surface and mid-atmosphere, and upper and lower level winds. It is well known that the implementation of floating point arithmetic varies from one computing system to another. As a result, meteorological parameters simulated by numerical models on two different systems may deviate from each other and the difference field becomes larger as the model is integrated for longer time, for example, in the scale of several months. This paper focuses on the reduction of such round-off-errors by a simple method of modifying the format representation of the initial data supplied to the model. In all the three systems, the model has been integrated for 4months starting from 4th May, 1996. It is found that after 5days of model integration with the modified data, the round-off-errors become insignificant. The rate of reduction of round-off-errors is fast up to a month of model integration and thereafter the rate slows down and stabilises. It is further noticed that at the end of four months of integration, the reduction in round-off-errors over the tropical region and oceans is much more than over the rest of the globe. Summary: This short-term observational study of infection control practice was performed in the medical emergency outpatient department (EMOPD) of a tertiary-care hospital in India when threatened by an outbreak of severe acute respiratory syndrome (SARS). An investigator attended the lobby daily to screen patients with symptoms for SARS. Patient/attendant load, patient flow, medical staff working practices and position in the EMOPD were observed. Infection control measures such as fumigation and cleaning were noted, as was the EMOPD laboratory function, use of personnel protection and display of information on infectious diseases. A total of 162 (7.4%) of the 2165 patients surveyed had respiratory symptoms but no cases of SARS were found. The flow of patients and their attendants was not systematic. No laboratory tests for SARS were available, and no educational material on SARS was displayed. The EMOPDs in key hospitals need be able to screen for infectious diseases, especially in view of the threats from SARS and Avian influenza. [Copyright &y& Elsevier]

Tobacco use is one of the greatest public health problems in the world. Prevalence of alcohol and tobacco use among lactating women ranged between 35.9–83.0% and 1.4–6.1%, respectively, in various parts of the world. Both alcohol and nicotine pass quickly into breast milk. The objective of this study is to estimate the prevalence of alcohol and tobacco use in lactating women and its association with having underweight, stunted, and wasted nursing children in India. The study also draws comparisons to identify the common associated socio-economic factors for alcohol and tobacco use among lactating women and having underweight, stunted, and/or wasted children. A cross-sectional analytical study involving secondary data from the National Family Health Survey-5. Alcohol and tobacco use among lactating women was self-reported, and the nutritional status of children was assess anthropometrically during the survey and categorised into under-weight, stunted, and wasted as per standardised growth charts. prevalence of tobacco use among currently lactating mothers in India was 3.24%. Prevalence of alcohol use among currently lactating mothers was 0.57%. Nursing children of mothers consuming alcohol had significantly higher odds of being wasted [OR = 1.44; (95% CI = 1.07–1.92)]. Richer wealth index {compared to poor, poorer [OR = 0.74; (95% CI = 0.59–0.91)], rich [OR = 0.64; (95% CI = 0.50–0.84)], richer [OR = 0.46; (95% CI = 0.33–0.65)], richest [OR = 0.19; (95% CI = 0.11–0.33)]}, higher education status {compared to illiterate, secondary education [OR = 0.79; (95% CI = 0.63–0.97)], higher education [OR = 0.38; (95% CI = 0.24–0.62)]}, and non-tribal ethnicity [OR = 0.40; (95% CI = 0.33–0.50)] were found to be significantly associated with lower odds of alcohol and tobacco use among lactating mothers. Age, religion, residence, and occupation were also found to be significantly associated. Lactating women with lower education status or belonging to lower income groups needs to be prioritised for further qualitative assessment of alcohol and tobacco use during lactation. Our study reflects the national and state-level prevalence of alcohol and tobacco use among lactating women, which often masks the local and community-level intricacies. Alloy 718 is a nickel-based superalloy that is widely used as a structural material for high-temperature applications. One concern that arises when Alloy 718 is manufactured using powder bed fusion (PBF) is that residual stresses appear due to the high thermal gradients. These residual stresses can be detrimental as they can degrade mechanical properties and distort components. In this work, residual stresses in PBF built Alloy 718, using both electron and laser energy sources, were measured by neutron diffraction. The effects of process parameters and thermal post-treatments were studied. The results show that thermal post-treatments effectively reduce the residual stresses present in the material. Moreover, the material built with laser based PBF showed a higher residual stress compared to the material built with electron-beam based PBF. The scanning strategy with the lower amount of residual stresses in case of laser based PBF was the chessboard strategy compared to the bi-directional raster strategy. In addition, the influence of measured and calculated lattice spacing (d 0) on the evaluated residual stresses was investigated. Inflammatory breast cancer (IBC) is an understudied form of breast cancer (BC). The incidence of HER2-positive disease in IBC is 2-fold greater than that in non-IBC. Several studies evaluating dual-HER2 blockade in HER2-positive BC have demonstrated an association of specific gene expression signatures with response. Those analyses were based on pretreatment tumor characteristics and did not focus specifically on IBC. We used RNA-Seq to elucidate the impact of short term neoadjuvant dual-HER2 blockade on IBC transcriptomic profile and to identify early predictors of response. We analyzed fresh frozen tumor samples prospectively obtained from 23 patients (pts) with HER2-positive IBC, treated at a single institution in a phase 2 trial (NCT01796197) with neoadjuvant trastuzumab, pertuzumab (HP) and paclitaxel for 16 weeks (wk). Breast biopsies were performed at baseline (D1) and 1 wk later (D8) after a single dose of dual blockade with HP before adding paclitaxel. Primary endpoint was pathologic complete response (pCR) defined as no evidence of invasive disease in breast or lymph nodes. Tumors from D1 and D8 were used for RNA-Seq analysis and assessment of tumor-infiltrating lymphocytes (TILs) and CelTIL score. Paired breast tumor biopsies (D1, D8) were obtained in all pts; 2 pts did not have surgery. In the intent-to-treat analysis, 10/23 (43%) pts achieved a pCR. Across all metrics, D8 biopsies were significantly better predictors of response than D1 (p-value: 1.0X10-15). Upregulation of immune signatures by RNA-Seq was observed at D1 and D8. D8 biopsies showed a greater upregulation of anti-tumor immunity and changes in multiple signalling pathways. Neither TILs nor CelTIL were associated with pCR. We identified an accurate predictor of response based on transcriptomic profiling by RNAseq, following a single dose of neoadjuvant dual-HER2 blockade in HER2-positive IBC. It outperforms a similar predictor constructed on a pretreatment profile in the same cohort. Assessing early-changes in gene expression level by RNA-seq following one dose of treatment may provide insights for the molecular mechanisms underlying response or resistance to anti-HER2 therapy. Countries throughout the world are experiencing COVID-19 viral load in their populations, leading to potential transmission and infectivity of asymptomatic COVID-19 cases. The current systematic review and meta-analysis aims to investigate the role of asymptomatic infection and transmission reported in family clusters, adults, children and health care workers, globally. Systematic review and meta-analysis. An online literature search of PubMed, Google Scholar, medRixv and BioRixv was performed using standard Boolean operators and included studies published up to 17 August 2021. For the systematic review, case reports, short communications and retrospective studies were included to ensure sufficient asymptomatic COVID-19 transmission data were reported. For the quantitative synthesis (meta-analysis), participant data from a collection of cohort studies focusing on groups of familial clusters, adults, children and health care workers were included. Inconsistency among studies was assessed using I2 statistics. The data synthesis was computed using the STATA 16.0 software. This study showed asymptomatic transmission among familial clusters, adults, children and health care workers of 15.72%, 29.48%, 24.09% and 0%, respectively. Overall, asymptomatic transmission was 24.51% (95% confidence interval [CI]: 14.38, 36.02) among all studied population groups, with a heterogeneity of I2 = 95.30% (P < 0.001). No heterogeneity was seen in the population subgroups of children and health care workers. The risk of bias in all included studies was assessed using the Newcastle Ottawa Scale. For minimising the spread of COVID-19 within the community, this study found that following the screening of asymptomatic cases and their close contacts for chest CT scan (for symptomatic patients), even after negative nucleic acid testing, it is essential to perform a rigorous epidemiological history, early isolation, social distancing and an increased quarantine period (a minimum of 14–28 days). This systematic review and meta-analysis supports the notion of asymptomatic COVID-19 infection and person-to-person transmission and suggests that this is dependent on the varying viral incubation period among individuals. Children, especially those of school age (i.e. <18 years), need to be monitored carefully and follow mitigation strategies (e.g. social distancing, hand hygiene, wearing face masks) to prevent asymptomatic community transmission of COVID-19. DNA damage caused by TMZ can sensitize tumors to PARP inhibitors. Pamiparib is an investigational PARP1/2 inhibitor that has shown PARP trapping activity, brain penetration, and synergistic cytotoxicity with ld TMZ in nonclinical studies. This ongoing study consists of a dose-escalation phase (3 + 3 design) and a dose-expansion phase enrolling pts with gastric or small cell lung cancer (GC and SCLC). During dose escalation, pamiparib was administered at 60 mg PO BID on days 1–28 and ld TMZ was given at escalating doses PO QD on days 1–7, 1–14, or 1–28 of 28-day cycles. During dose expansion, pts are being treated at the recommended phase 2 dose (RP2D); eligibility criteria include metastatic GC and extensive stage SCLC pts treated with ≤2 prior lines of chemotherapy. Tumor assessments are performed every 8 weeks. Primary and secondary endpoints include safety/tolerability and antitumor activity (RECIST v1.1)/PK, respectively; biomarker assessment (eg, BRCA) is exploratory. We present updated results that include the RP2D and data from expansion cohort pts. As of 1 March 2019, 66 pts were enrolled in 8 dose escalation cohorts and 24 pts were enrolled at the RP2D pamiparib 60 mg PO BID days 1–28 and TMZ 60 mg PO QD days 1–7 (N = 90). Median age was 65 years (range, 30–85) and median prior lines of therapy was 3 (range, 1–10). The most common adverse events (AEs) related to study drugs across all doses were nausea (46%; 0% grade (G)3/4), anemia (43%; 24% G3/4), fatigue (41%; 1% G3/4), neutropenia (26%; 21% G3/4), and thrombocytopenia (27%; 14% G3/4). There were no fatal AEs. Pamiparib + TMZ plasma exposure was similar to that of each single agent. Among 63 dose escalation pts with measurable disease evaluated for efficacy, 9 achieved partial response (PR; 14%; 95% CI, 7–25) across ld TMZ doses and indications (6 confirmed PR). Responses were seen in pts regardless of known BRCA status. Disease control rate was 53% and median duration of response was 5.5 months (range, 1.9–7.3). Pamiparib combined with ld TMZ continues to demonstrate promising antitumor activity and remains generally well tolerated in pts with advanced solid tumors. NCT03150810.
