Giant cell reparative granuloma (GCRG) is a rare benign tumor. The jawbone is the most common site of occurrence, followed by sphenoid bone, craniofacial bone, hand and foot bones. The etiology of GCRG is unknown but may be related to an intraosseous hemorrhage following trauma. Despite its benign nature, it could be locally aggressive. To our knowledge, no spinal epidural GCRG case has been reported. A case of man aged 32 years who presented with upper right limb numbness and weakness. Computed tomography showed a round soft tissue mass in the spinal canal at the C7-T1 level. The mass showed isointensity on T1-weighted images, hypointensity on T2-weighted images, and significant enhancement on postcontrast T1-weighted images. The mass localized in the epidural space and was surgically resected. The histologic diagnosis was consistent with GCRG. Spinal epidural GCRG is rare and is hardly considered in the differential diagnosis. Preoperative diagnosis of GCRG is challenging, and the definitive diagnosis could only be made by pathological examination. Surgical resection is probably an effective therapy for relief of symptoms. Abstract Solid aneurysmal bone cyst (ABC) is a rare subtype of ABC that most commonly involves the small bones of the hands or feet. We present a case of a solid ABC of the distal humerus in a 52-year-old man with a history of chronic kidney disease and renal cell carcinoma. On imaging with plain radiographs, CT, and MRI, this expansile lucent lesion with solid internal enhancement had an appearance that overlapped with metastasis or brown tumor of hyperparathyroidism. On 18F-FDG PET-CT, this lesion was hypermetabolic with an SUVmax of 9.9. Only 37 cases of solid ABC have previously been reported to involve the long bones in the literature, and only 4 in the humerus. We review the clinical, imaging, and histopathological findings and differential diagnosis of solid ABC, and highlight the usefulness of identifying the USP6 gene rearrangement on FISH to distinguish this lesion from other lesions with secondary ABC formation. Abstract: Aneurysmal bone cyst is a rapidly growing and locally aggressive lesion that commonly affects children and young adults. Initially regarded as a reactive process, primary aneurysmal bone cyst is now widely accepted as a neoplasm owing to recent findings of recurrent clonal chromosomal alterations, mostly t(16;17)(q22;p13). However, other infrequent chromosomal rearrangements have also been reported. Giant cell reparative granuloma, previously regarded as a nonneoplastic process and histologically indistinguishable from the solid variant of aneurysmal bone cyst, is frequently seen in the gnathic bones and the short tubular bones of the hands and feet. Here we present such a case of giant cell reparative granuloma (solid aneurysmal bone cyst) in the finger of a 63-year-old white man. Cytogenetic analysis revealed a novel alteration involving a reciprocal translocation between 6q and 13q, with a karyotype of 46,XY,t(6;13)(q15;q34),del(20)(q13.1). [Copyright &y& Elsevier]

Summary: Introduction Giant cell reparative granuloma (GCRG) of the mandible is a common benign entity that presents as a single interosseous lesion. It can vary from an asymptomatic slow growing mass to a locally destructive aggressive lesion. Pathological findings of the mandible associated with cherubism and hyperparathyroidism can mimic that of GCRG. However, these diseases demonstrate multiple mandibular lesions as opposed to a single lesion seen in GCRG. We describe a case report of a 9-year-old female with multiple mandibular lesions pathologically positive for GCRG who has no familial history of cherubism or any evidence of hyperparathyroidism. Methods/Results This is a chart review case presentation of a 9-year-old patient who initially presented to the clinic after sustaining trauma to the left side of her face and developed persistant swelling and pain. Imaging showed multiple cystic lesions of the mandiblular rami bilaterally. She underwent curettage and excisional biopsies of these lesions with bone graft. The pathology is consistent with giant cell reparative granuloma. Conclusion This is an unusual presentation of a patient with multiple giant cell reparative granulomas of the mandible. Although this is usually associated with cherubism or hyperparathyroidism, our patient has neither of these diseases. [Copyright &y& Elsevier]

Giant cell reparative granuloma is a benign, rare non-neoplastic intraosseous lesion. It is found in various sites throughout the body, including the foot. This uncommon lesion has histological and radiographic features that are similar to other giant cell lytic lesions. Therefore, it is essential to be aware of this lesion to avoid confusion with more aggressive tumors. The purpose of this case report is to present the histological and radiographic findings of a 26-year-old female who had successful surgical excision and resolution of a giant cell reparative granuloma in the middle phalanx third digit left foot. Abstract: Background: Giant cell reparative granuloma is a condition commonly found in the facial bones. It has been reported in the skull bones, predominantly the bones comprising the skull base. Giant cell reparative granuloma of the cranial vault is exceedingly rare. Its distinction from a neoplastic lesion is difficult clinically and radiologically. Case Description: Only the third case of a GCRG of the skull vault, the first in an adult, is described. Its pathophysiology and differential diagnoses influencing management are discussed. Conclusion: It is not possible to diagnose this condition definitively on a clinical or radiologic basis, and given its differential diagnoses, an excision biopsy should be offered.
