HLA-G was described as a molecule inhibiting NK and T cells functions through its receptor, ILT2. However, most functional studies of HLA-G were so far performed on heterogeneous immune populations and regardless of ILT2 expression. This may lead to an underestimation of the effect of HLA-G. Thus, considering the immune subpopulations sensitive to HLA-G remained an important issue in the field. Here we present a new cytometry assay to evaluate HLA-G effects on both NK and CD8+ T cell cytotoxic functions. Using flow cytometry allows for the comparison of HLA-G function on multiple subsets and multiple functions in the same time. In particular, we sharpen the analysis by specifically studying the immune subpopulations expressing HLA-G receptor ILT2. We focused our work on: IFN-gamma production and cytotoxicity (CD107a expression) by CD8+ T cells and NK cells expressing or not ILT2. We compared the expression of these markers in presence of target cells, expressing or not HLA-G1, and added a blocking antibody to reverse HLA-G inhibition. This new method allows for the discrimination of cell subsets responding and non-responding to HLA-G1 in one tube. We confirm that HLA-G-specifically inhibits the ILT2+ CD8+ T cell and ILT2+ NK cell subsets but not ILT2-negative ones. By blocking HLA-G/ILT2 interaction using an anti-ILT2 antibody we restored the cytotoxicity level, corroborating the specific inhibition of HLA-G1. We believe that our methodology enables to investigate HLA-G immune functions easily and finely towards other immune cell lineages or expressing other receptors, and might be applied in several pathological contexts, such as cancer and transplantation. 10. ODIASP, un outil d'intelligence artificielle pour le diagnostic de la sarcopénie via l'imagerie scanner.

- Abstract
Le projet « Outil de DIAgnostic de la SarcoPénie (ODIASP) » a pour but d'optimiser le diagnostic de la sarcopénie grâce au calcul automatisé d'un Index de Surface musculaire (ISM) en imagerie scanner. Il existe actuellement un intérêt grandissant pour l'utilisation du scanner pour la mesure de la composition corporelle, mais la détermination de l'iSM est fastidieuse en pratique clinique. L'objectif de cette étude était de développer dans une même interface, la sélection automatique d'une coupe au niveau de la 3e vertèbre lombaire (L3), la mesure de la surface musculaire sur cette coupe et le calcul de l'ISM dérivé de cette mesure. L'outil ODIASP intègre deux algorithmes de la littérature et mis à disposition de la recherche pour : (1) la sélection automatique d'une coupe en L3 et (2) la segmentation automatique de cette coupe. Pour chacune de ces deux étapes, la sélection d'un algorithme a fait l'objet d'une comparaison d'approches existantes faisant références dans la littérature et d'une validation externe de ces approches. Pour la sélection automatique de la coupe passant en L3 au sein d'un scanner, les approches de Bridge et al. et de Meng et al. ont été comparées. L'algorithme AutoMATICA proposé par Paris et al. a été comparé au module ABACS du logiciel SliceOmatic pour la mesure de la surface musculaire. Une base de 676 scanners abdominopelviens a été utilisée pour comparer, sélectionner et valider les différents algorithmes. Les scanners ont été annotés manuellement par un clinicien. La comparaison des deux approches pour la sélection des coupes en L3 a été évaluée par la mesure d'un pourcentage de bonnes prédictions, soit la proportion de coupes déterminées par l'outil ODIASP et estimées par le clinicien comme passant effectivement en L3. La segmentation des muscles striés squelettiques a été évaluée par un score DICE. Un score de 0 signifie qu'il n'y a pas de chevauchement et 1 signifie que les surfaces segmentées se chevauchent parfaitement. Enfin, un coefficient de corrélation des concordances (CCC) a été utilisé pour évaluer les performances de l'outil complet ODIASP. Pour le choix de la coupe L3, l'approche de Meng et al. a montré un meilleur pourcentage de bonnes prédictions (0,925, intervalle de confiance IC = [0,902–0,944]) par rapport à Bridge et al (0,882, IC = [0,855–0,905]). Cependant son temps de traitement d'un scanner d'environ 20 minutes par série est incompatible avec une utilisation en routine. Par ailleurs, le temps de calcul (moins de 1 minute par scanner) et le CCC (0,962, IC = [0,956–0967]) sont meilleurs et satisfaisants pour l'approche de Bridge et al. (corrélation des concordances de 0,899, IC = [0,883–0,912] pour Meng et al.), celle-ci a donc été retenue. Pour l'étape 2, le DICE médian du clinicien par rapport à AutoMATiCA (0,969 [5e percentile : 0,909]) est supérieur au DICE médian du clinicien par rapport à ABACS-SliceOmatic (0,949 [5e percentile : 0,836]) (p < 0,001). L'algorithme AutoMATICA a donc été retenu. Enfin, la comparaison des surfaces musculaires obtenues par l'outil complet intégrant Bridge et al. et AutoMATICA a montré un CCC de 0,970 (IC = [0,966–0,974]). Le temps de traitement moyen d'un scanner par l'outil ODIASP est d'environ 1 minute. Une interface intuitive intégrant les algorithmes sélectionnés a été créée pour analyser un ou plusieurs scanners, calculer et exporter un ISM pour chaque sujet et visualiser aisément les résultats obtenus. L'outil proposé offre la possibilité de calculer de manière fiable et rapide l'ISM pour chaque scanner abdominopelvien sous réserve de connaître la taille du sujet. La mise à disposition de cet outil à la communauté scientifique ouvre la voie à des études sur de larges cohortes pour notamment définir les seuils d'ISM pour le diagnostic de la sarcopénie. En attendant le marquage CE de notre outil, le clinicien peut d'ores et déjà l'utiliser dans sa pratique sous réserve qu'il valide chaque résultat obtenu avec son expertise clinique. Abstract: In this work, several Physical Vapour Deposition (PVD) films of chromium and chromium nitride were grown by adjusting the nitrogen flow rate. It was found that films containing lower nitrogen content exhibited better corrosion behaviour in relation to their microstructure. Moreover, the resistivities of the samples were found to increase with N content due to a decrease in the number of metallic bonds in the coatings and a loss of oxide layer conductivity. The latter is linked to a decrease of charge carrier density by the incorporation of species into the passive layers, which decreases cation vacancy density. [Copyright &y& Elsevier]

Like scientists, our consciousness is built through a fusion-differentiation process from other people leading to the building of culture as an ethnotheory. This process leads to a “consciousness in action” depending from the success or the failure of expected roles and real behaviors of nature and the energetic balance of these cycles. Abstract: This paper deals with the characterization of an environment-friendly corrosion inhibitor formulated for industrial cutting fluids. Polarization measurements and electrochemical impedance spectroscopy (EIS) tests reveal the formation of a 3D film at the interface metal/elecrolyte. This film blocks iron ions oxidation by forming a barrier on the metallic surface. Studies concerning, respectively, inhibitor concentration, immersion time and temperature show that effects of these three parameters are strongly linked and influence considerably the protective effect of the inhibitor. XPS analyses confirm formation of an inhomogeneous layer submitted to a water-uptake with time when inhibitor is not added in necessary concentration. [Copyright &y& Elsevier]

The entry of Pb into the food chain is of concern as it can cause chronic health problems. The concentration of Pb was determined in cereal grain samples collected representatively from British Cereal Quality Surveys in 1982 and 1998 (
n=176 , 250 and 233 for wheat collected in 1982 and 1998, and barley in 1998, respectively). In addition, paired soil and grain samples were collected from 377 sites harvested across Britain in 1998–2000. Wheat grain Pb ranged from below the analytical detection limit (0.02 mg kg-1 dry weight, DW) to 1.63 mg kg-1 DW, and barley grain Pb from <0.02 to 0.48 mg kg-1 DW. The vast majority of samples (>99% for both wheat and barley, excluding Scottish barley samples collected in 2000) were well below the newly introduced EU limit for the maximum permissible concentration of Pb in cereals (0.2 mg kg-1 fresh weight, equivalent to 0.235 mg kg-1 DW). There was a significant reduction in wheat grain Pb in the 1998 survey compared with the 1982 survey. However, 40 barley samples collected from Scotland in 2000 in the paired soil and crop survey showed anomalously high concentrations of Pb, with 10 samples exceeding the EU limit. Washing experiments demonstrated that surface contamination, introduced during grain harvest and/or storage, was the main reason for the high concentrations in these samples. In the paired soil and crop surveys, there were no significant correlations between grain Pb concentrations with total soil Pb and other soil properties, indicating low bioavailability of Pb in the soils and limited uptake and transport of Pb to grain. Summary: The most frequent organ-specific autoimmune diseases associated with type 1 diabetes mellitus in children are hypothyroidism and celiac disease. Among adults, other associations exist, notably with pernicious anemia, which is extremely rare in children. We relate the observation of an adolescent with type 1 diabetes mellitus and hypothyroidism, admitted for severe anemia in addition to chronic anemia caused by autoimmune gastritis. Blood cell count showed severe aregenerative anemia with pancytopenia, with signs of non-autoimmune hemolysis. Vitamin B12 levels were low, bone marrow aspiration revealed erythroid hyperplasia, and anti-intrinsic factor antibodies were positive, providing the diagnosis of pernicious anemia. Treatment with intramuscular vitamin B12 produced brisk reticulosis after 6 days, with a subsequent rapid resolution of the anemia. Follow-up of type 1 diabetes mellitus in children requires screening for organ-specific autoimmune diseases; in case of unexplained anemia, autoimmune gastritis must be suggested. Abstract: Background.: Alzheimer''s disease is a chronic pathology requiring regular follow-up. The predictive factors of rapid cognitive declineremain unclear. Objectives.: To analyse the baselines characteristics of patients at increased risk of rapid cognitive decline. Methods.: This study presents transversal data on a community-based sample of 340 patients diagnosed with probable Alzheimer''sdisease and followed by REAL.FR group. Rapid cognitive decline was defined as a 3-points or greater loss on the Mini Mental State Examination (MMSE) within six months. Results.: 54 % of patients presented a rapid cognitive decline. Logistic regression analysis showed a positive association between rapidcognitive decline and a MMSE 5 20 (p<0.003) or a greater BMI (p<0.02) and a tendency towards a negative correlation with anxiety (p = 0.06) and negative correlation with the burden severity (p<0.05). Conclusions.: Patients with a worse cognitive status, a greater BMI and less anxiety or burden were at increased risk of rapid cognitivedecline. Future studies should focus on determining etiologies for patients with rapid cognitive loss and help clinicians target these patients for interventions aiming to delay or stabilise the course of this disease. Résumé Objectif On cherche à évaluer l’aspect de la zone d’hystérorraphie et à rechercher un hématome chez des patientes asymptomatiques à 48 h de leur césarienne. Méthode Il est fréquent de constater, en échographie ou en examen tomodensitométrique, des images de collection en regard de la zone d’hystérorraphie chez des patientes symptomatiques après leur césarienne. Leur interprétation reste difficile, ce qui nous a amené à proposer une étude prospective à la recherche d’une image évocatrice de collection ou d’hématome chez des patientes asymptomatiques à 48 h de leur césarienne. Il a été réalisé une échographie pelvienne sus-pubienne et endovaginale avec étude de la zone d’hystérorraphie et de l’espace inter-vésico-utérin. Résultats L’examen échographique sus-pubien a été réalisé chez 31 patientes asymptomatiques après recueil de leur consentement écrit. Vingt-huit patientes ont également bénéficié d’un examen endovaginal. L’espace étudié était facilement repéré par la visualisation du trajet de l’hystérotomie et de l’aspect hyperéchogène de l’hystérorraphie. Dans 28 cas il n’y avait aucune image anormale en regard de la zone d’hystérorraphie. Dans 3 cas, une image évocatrice d’une collection hématique a été visualisée et ne mesurait au maximum que 49 mm de grand axe avec un faible signal Doppler. L’examen était très bien toléré par les patientes, surtout par voie endovaginale. De même la durée de l’échographie n’excédait jamais 58 secondes et restait plus rapide par voie vaginale. Conclusion Ce travail, préliminaire à une étude prospective en double cohorte (patientes asymptomatiques et patientes symptomatiques), a permis de préciser la technique à utiliser et à privilégier dans la recherche d’une collection en regard de la zone d’hystérorraphie. L’examen échographique en postopératoire, surtout par voie vaginale, est très rapide, bien toléré avec une qualité d’image satisfaisante. Enfin dans cette cohorte de patientes asymptomatiques, il a été très rare de constater une collection, ce qui confirme le crédit à donner à ce type d’image en cas de symptômes après césarienne. Objective It seeks to assess the appearance of the hysterorrhaphy area and seeks hematoma in asymptomatic patients at 48 hours of their cesarean. Method It is common to see, ultrasound or CT scan, collection of images next to the hysterorrhaphy area in symptomatic patients after cesarean. Their interpretation remains difficult which led us to propose a prospective study looking for an evocative image collection or hematoma in asymptomatic patients at 48 hours of their cesarean. It was directed suprapubic and transvaginal pelvic ultrasound with study area hysterorrhaphy and inter-uterine bladder space. Results The suprapubic ultrasound examination was performed in 31 asymptomatic patients after collecting their written consent. Twenty-eight patients also received an endovaginal examination. The studied area was easily identified by visualizing the path of hysterotomy and hyperechoic aspect of the hysterorrhaphy. In 28 cases there were no abnormal image in front of the hysterorrhaphy area. In 3 cases, an evocative image of a haematic collection was displayed and measured a maximum of only 49 mm long axis with a weak Doppler signal. The exam was very well tolerated by patients, especially by transvaginal route. Also the duration of ultrasound never exceeded 58 seconds and remained fastest vaginally. Conclusion This preliminary work to a prospective double cohort (symptomatic patients and asymptomatic patients) has clarified the technique to use and focus in the search for a collection next to the hysterorrhaphy area. Ultrasound examination postoperatively, especially vaginally, is very fast, well tolerated with satisfactory image quality. Finally in this cohort of asymptomatic patients, it was very unusual for a collection, confirming the credit to be given to this type of image in case of symptoms after cesarean. The aim of this experimental and numerical work is to determine the mass transfer at the wall of a steel cylinder rotating within liquid Lead-Bismuth blend. This is a crucial parameter for the analysis of corrosion tests performed in the CICLAD experimental set-up for the investigation of flow assisted corrosion of steel by liquid metals. As a reliable numerical modelling must rely on experimental validation, and since no direct wall mass transfer measurement is possible in CICLAD, a scaled electrochemical model has been achieved. Modelling the turbulence in this rotating configuration is shown to require the use of a Reynolds-Stress Model. A sensitivity to the Schmidt number is observed in the numerical simulations representing the measured mass transfer in the electrochemical model. Simulations dedicated to liquid Pb Bi are thus presented in addition to those dedicated to the aqueous solution used in the electrochemical model. Following correlations are proposed for the prediction of Fe mass transfer at the wall of a steel rotating cylinder in liquid Pb-Bi: Sh = 1.9 × 10−1(Re 2 Sc)0.31 for 2 × 109 < Re 2 Sc < 5.3 × 1010; Sh = 1.4 × 10−3(Re 2 Sc)0.51 for 5.3 × 1010 < Re 2 Sc < 8.5 × 1011; Sh = 1.1 × 10−2(Re 2 Sc)0.43 for 8.5 × 1011 < Re 2 Sc < 1013. This mass transfer coefficient has recently been used to get a better understanding of corrosion in Pb Bi [1]. • Mass transfer coefficient for steel in liquid LBE are measured in a rotating cylinder configuration in the CICLAD set-up, • The hydrodynamics of CICLAD is modelled both numerically and experimentally, • A scaled electrochemical experiment in similarity with CICLAD is used to validate the numerical model. • Correlations are proposed depending on the Schmidt and Reynolds numbers. A partially recrystallized sample of the Ni-based superalloy AD730 was taken from an intermediate stage of the ingot to billet conversion process and isothermally forged in a single stroke compression test at a sub-solvus temperature (1080 °C). The as-received material had a heterogeneous microstructure, containing a mixture of coarse and much finer recrystallized grains as well as unrecrystallized ones, and also heterogeneous γ′ precipitation. The recrystallization mechanisms occurring dynamically in the different grain populations were investigated via electron backscatter diffraction (EBSD). It was found that local microstructure could affect the operative recrystallization mechanism, with different mechanisms seen in the deformed and recrystallized regions, owing to their different precipitate distributions. Within a single deformed grain, three apparently distinct dynamic recrystallization (DRX) mechanisms were identified. The interaction of recrystallization with precipitates plays a central role in DRX. In certain cases precipitates may stimulate discontinuous DRX by providing recrystallization nuclei, alternatively they may impede and limit the growth of recrystallized grains, or in other cases still they promote continuous recrystallization. • Recrystallisation mechanisms of a heterogeneous starting microstructure are characterised. • Different local microstructures lead to different recrystallisation mechanisms operating. • Different interactions between precipitates and recrystallisation processes are described. • All precipitates in recrystallised regions result from dissolution and reprecipitation. Abstract: Purpose: To study and clarify the kinematics of spinal segments following cyclic torques causing axial rotation (Tz (t)), lateral-flexion (Tx (t)), flexion/extension (Ty (t)). Methods: A 6D--Measurement of location, alignment, and migration of the instantaneous helical axis (IHA) as a function of rotational angle in cervical, thoracic, and lumbar segments subjected to axially directed preloads. Results: IHA retained an almost constant alignment, but migrated along distinct centrodes. Thoracic segments: IHA was almost parallel to Tz (t), Tx (t), or Ty (t), stationary for Tx (t) or Ty (t), and migrating for Tz (t) along dorsally opened bows. IHA locations hardly depended on the position or size of axial preload. Lumbar segments: IHA was also almost parallel to Tz (t), Tx (t), or Ty (t). In axial rotation IHA-migration along wide, ventrally or dorsally bent bows depending on segmental flexional/extensional status. Distances covered: 20–60mm. In lateral-flexion: IHA-migration to the left/right joint and vice versa. In flexion/extension IHA-migration from the facets to the centre of the disc. Cervical segments: In flexion/flexion IHA was almost stationary for and parallel to Ty (t). In axial rotation or lateral-flexion IHA intersected Tz (t)/Tx (t) under approximately −30°/+30°. Conclusions: Generally joints alternate in guidance. Lumbar segments: in axial rotation and lateral-flexion parametrical control of IHA-position and IHA-migration by axial preload position. Cervical segments: kinematical coupling between axial rotation and lateral-flexion. The IHA-migration guided by the joints should be taken into account in the design of non-fusion implants. FE-calculations of spinal mechanics and kinematics should be based on detailed data of curvature morphology of the articulating surfaces of the joint facets. [Copyright &y& Elsevier]

La syphilis est une maladie dont on observe une résurgence depuis le début des années 2000. L'épidémiologie à la Réunion, contrairement à la Métropole concerne davantage la population hétérosexuelle qu'homosexuelle. Le nombre de femme enceinte atteinte par la syphilis au cours de leur grossesse est de plus en plus important, laissant apparaître des cas de syphilis congénitale. L'objectif de l'étude était de décrire les cas de syphilis acquis chez des femmes enceintes et d'identifier les facteurs de risque d'atteinte fœtale et néonatale. Étude rétrospective, descriptive de 2014 à 2018 sur les 2 sites du CHU de la Réunion. L'identification des patientes a été réalisée grâce au département d'informatique médical du CHU. Les données étaient recueillies dans les dossiers cliniques des centres de diagnostic prénatal et de pédiatrie. Cinquante et une patientes ont été identifiées. La prévalence était de 0,11 % en 2017. La majorité des infections étaient des syphilis latentes découvertes au premier trimestre de la grossesse 24 (47 %). Cependant 17 (33 %) sérologies positives ont été découvertes au 2e et 3e trimestre. Le diagnostic a été posé pour deux patientes à l'accouchement. Des signes échographiques évocateurs d'infection fœtale ont été rapporté chez 7 (13,7 %) patientes, parmi ces signes nous avons relevé 2 cas d'ascite fœtale, un anasarque fœtoplacentaire, un fœtus présentant des calcifications hépatiques et un hydramnios, de plus, 9 (17,6 %) fœtus ont été suivis pour un retard de croissance. Au total, 10 (19,6 %) nouveau-nés ont été classés dans le groupe syphilis congénitale probable. En raison d'un faible effectif, aucun des facteurs : co-infection, terme de découverte de la sérologie positive, stade de l'infection ou présence de signes échographiques n'étaient significativement associés à un risque de syphilis congénitale. Le dépistage de la syphilis au 1er trimestre chez les femmes enceintes doit rester systématique, mais une seconde sérologie au 2–3e trimestre doit également être proposée dans les territoires à forte endémie pour éviter toute syphilis congénitale. La syphilis à l'échographie est également « une grande simulatrice ». [ABSTRACT FROM AUTHOR]

- Abstract
Introduction La migration pour soins au sein de la région océan Indien est ancrée dans la culture comorienne. Ces migrants ont une forte incidence sur l’activité hospitalière de Mayotte et l’économie de la santé du 101 e département français. L’objectif de ce travail est de décrire leurs parcours d’accès aux soins, dans un contexte géopolitique qui ne peut être éludé. Matériels et méthodes Première étude sur ce thème, ce travail réalisé de 2013 à 2015 comporte 3 volets complémentaires : (1) une étude prospective et descriptive incluant les patients arrivés après une traversée clandestine ; (2) un état des lieux de la procédure d’évacuation sanitaire programmée des Comores vers Mayotte ; (3) une étude observationnelle rétrospective des parcours de soins atypiques de patients comoriens insuffisants rénaux chroniques. Résultats Volet 1 : 276 patients inclus sur 6 mois ; 68 % hospitalisés avec des durées moyennes de séjour 2 à 4 fois supérieures à celles des autres patients consultant aux urgences ; orthopédie, diabétologie, pédiatrie et oncologie sont les causes les plus fréquentes de cette migration pour soins. Volet 2 : 136 patients inclus sur 12 mois ; maladies chroniques et brûlures sont les causes les plus fréquentes ; seuls 35 patients sont arrivés de manière programmée après régulation médicale puis socio-économique. Volet 3 : 31 patients inclus sur 24 mois ; 27 patients ont nécessité un transfert secondaire vers La Réunion. Conclusion L’étude décrit le phénomène de migration médicale à Mayotte, et définit la gravité et les principales pathologies, aiguës ou chroniques. Elle objective la nécessité de diminuer les retards de prise en charge pour en limiter le coût pour les structures de soins à Mayotte, ou les établissements réunionnais et métropolitains, et le risque d’introduction de maladies infectieuses transmissibles ou de BHRe. Ces données sont essentielles pour prioriser les actions dans le cadre d’une coopération régionale visant à soutenir le développement du système de santé comorien via le renforcement de partenariats ciblés, en particulier dans le domaine de la prévention des maladies chroniques ou infectieuses, et des échanges entre les équipes médicales, en fonction des besoins identifiés par filière.
