Background: Dravet Syndrome (DS) is a developmental and epileptic encephalopathy characterized by severe drug-resistant seizures and associated with cognitive and motor impairments. Walking problems are frequently observed. As the foot plays a key role during walking, compromised foot function can be a feature of deviant gait.Aim: To investigate foot function in DS by characterizing foot-floor contact patterns using pedobarography.Methods: A total of 31 children and adults were included in the DS group (aged 5.2-32.8 years, 17 female, 174 steps) and 30 in the control group (aged 6.0-32.9, 16 female, 180 steps). The foot-floor contact pattern was evaluated based on progression, length and smoothness (spectral arc length) of the center of pressure (CoP). Linear mixed models were used to identify differences between non-heel strikes and heel strikes and between the DS and control group.Results: Fifteen participants with DS showed inconsistency in the type of foot-floor contact (heel strikes and non-heel strikes). Heel strikes of participants with DS had significantly reduced time of CoP under the hindfoot and increased time under the midfoot region compared to the control group. Significant time and age effects were detected.Conclusions and Implications: Deviant foot-floor contact patterns were observed in DS. To quantify gait abnormalities in people with Dravet syndrome (DS). Individuals with a confirmed diagnosis of DS were enrolled, and stratified according to knee flexion at initial contact (IC) and range of motion (ROM) during stance (atypical crouch: knee flexion >20° at IC and knee ROM >15° during stance; straight: knee flexion <20° at IC). A 1D ANOVA (α = 0.05) was used to test statistical differences among the joint kinematics and spatio–temporal parameters of the cohort and an age-matched control group. Clinical (neurological and orthopaedic evaluation) and anamnestic data (seizure type, drugs, genetic mutation) were collected; distribution between the two gait phenotypes was assessed with the Fisher exact test and, for mutation, with the chi-squared test (p < 0.05). Linear regression between maximum knee flexion and normalised walking speed was calculated. Seventy-one subjects were enrolled and evaluated with instrumented gait analysis. Fifty-two were included in final analysis (mean age 13.8 ± 7.3; M 26). Two gait patterns were detected: an atypical crouch gait (34.6%) with increased ankle, knee and hip flexion during stance, and reduced walking speed and stride length not associated with muscle-tendon retractions; and a pattern resembling those of healthy age-matched controls, but still showing reduced walking speed and stride length. No differences in clinical or anamnestic data emerged between the two groups. Objectively quantified gait in DS shows two gait patterns with no clear-cut relation to clinical data. Kinematics abnormalities may be related to stabilization issues. This paper proposes a methodology for evaluating the effect of different stereophotogrammetric system calibration procedures on the calculation of marker-based kinematics information. The methodology, based on calibrating the system using data recorded from capture volumes of different sizes and in trials of different durations, was applied to two different systems. The calibration data were used to reconstruct the static and dynamic position and orientation in space of a rigid wand carrying markers in known positions. The inaccuracies in the reconstruction of distances and angles from the wand markers were independent on the calibration data, with average errors lower than 1.7 mm and 0.7°, respectively. Similar results were obtained from human gait data, with the highest variations observed in the transverse plane kinematics and in the foot segment, suggesting that successful calibration procedures of different durations and performed in different volumes did not affect the metrological performance of the investigated systems. Multi-segment models of the foot have been proposed in the past years to overcome limitations imposed by oversimplified traditional approaches used to describe foot kinematics, but they have been only partially validated and never compared. This paper presents a unique comparative assessment of the four most widely adopted foot kinematic models and aims to provide a guidance for the clinical interpretation of their results. Sensitivity of the models to differences between treadmill and overground walking was tested in nine young healthy adults using a 1D paired t -test. Repeatability was assessed by investigating the joint kinematics obtained when the same operator placed the markers on thirteen young healthy adults in two occasions. Reproducibility was then assessed using data from three randomly selected participants, asking three operators to repeat the marker placement three times. The analyses were performed on sagittal kinematics using curve similarity and correlation indices (Linear Fit Method) and absolute differences between selected points. Differences between treadmill and overground gait were highlighted by all the investigated models. The two most repeatable and reproducible investigated models had average correlations higher than 0.70, with the lowest values (0.56) obtained for the midfoot. Averaged correlations were always higher than 0.74 for the former and 0.70 for the latter, with the lowest obtained for the midfoot (0.64 and 0.51). For all investigated models, foot kinematics generally showed low repeatability: normative bands must be adopted with caution when used for comparison with patient data. [ABSTRACT FROM AUTHOR]

Abstract In vivo estimates of tibiotalar and the subtalar joint kinematics can unveil unique information about gait biomechanics, especially in the presence of musculoskeletal disorders affecting the foot and ankle complex. Previous literature investigated the ankle kinematics on ex vivo data sets, but little has been reported for natural walking, and even less for pathological and juvenile populations. This paper proposes an MRI-based morphological fitting methodology for the personalised definition of the tibiotalar and the subtalar joint axes during gait, and investigated its application to characterise the ankle kinematics in twenty patients affected by Juvenile Idiopathic Arthritis (JIA). The estimated joint axes were in line with in vivo and ex vivo literature data and joint kinematics variation subsequent to inter-operator variability was in the order of 1°. The model allowed to investigate, for the first time in patients with JIA, the functional response to joint impairment. The joint kinematics highlighted changes over time that were consistent with changes in the patient's clinical pattern and notably varied from patient to patient. The heterogeneous and patient-specific nature of the effects of JIA was confirmed by the absence of a correlation between a semi-quantitative MRI-based impairment score and a variety of investigated joint kinematics indexes. In conclusion, this study showed the feasibility of using MRI and morphological fitting to identify the tibiotalar and subtalar joint axes in a non-invasive patient-specific manner. Pressure ulcers development is an undesirable event that often worsens the clinical condition of patients already affected by severe pathologies. Since the aetiology of this clinical complication is unclear yet, at current the primary approach to treat the problem is the adoption of suitable patients' assistance procedures. At the same time, the research focuses on finding medicaments or treatment strategies that could prevent the lesions and/or accelerate their healing. The international market's wide range of cosmetic/pharmaceuticals products is mainly topical preparations based on emollient agents to preserve or restore skin homeostasis. On the other hand, the skin microbiome's implication in the pressure ulcers occurrence is mainly unknown. Based on these assumptions, here we tested an innovative preparation, the LimpiAD foam, as a potential preventive strategy of pressure ulcers onset. The active component of this product is composed of hyaluronic acid conjugated with a bacterial cell wall fragment of C. acnes DSM 28251. For LimpiAD foam, we hypothesised a combined action of the two components on the skin tissue, an emollient effect due to the hyaluronic acid properties together with a modulatory effect on the skin microbiota carried out by the component of bacterial derivation. Our results supported the hypothesis and suggested a potential role of LimpiAD foam in pressure ulcers prevention. • The pressure ulcers onset is a relevant sanitary problem with unclear aetiology. • LimpiAD foam is an innovative topical preparation by Aileens Pharma srl. • LimpiAD foam was applied to patients with high risk of pressure ulcers onset. • None of patients developed skin lesions or any sign of skin damage. • LimpiAD foam showed a potential action in maintaining the skin eubiosis. Summary: Metabotropic glutamate (mGlu) receptors are G-protein coupled receptors activated by glutamate, the major excitatory neurotransmitter of the CNS. A growing body of evidence suggests that the function of mGlu receptors is not restricted to the regulation of synaptic transmission. mGlu receptors are expressed in a variety of peripheral cells, including inter alia hepatocytes, pancreatic cells, osteoblasts and immune cells. Within the immunological synapses, mGlu receptors expressed by T cells might contribute to the vast array of signals generated by the antigen-presenting cells. mGlu receptors are also found in embryonic and neural stem cells. This suggests their involvement in the pathophysiology of brain tumors, which likely originates from cancer stem cells similar to neural stem cells. Ligands of mGlu3 and mGlu4 receptors are potential candidates for the experimental treatment of malignant gliomas and medulloblastomas, respectively. [Copyright &y& Elsevier]
