Spinal metastases represent the most common site of bony metastases and frequently reduce quality of life. A beneficial effect of surgery and radiotherapy versus radiotherapy alone has been demonstrated in symptomatic patients. The goal of our study was to perform a cost-utility analysis of surgery for spinal metastases based on patient-level costs and health status data in a specialist spine center in Belgium. A cost-utility analysis was performed in a prospective cohort of patients undergoing surgery for symptomatic spinal metastases in 2011–2015. EQ-5D-3L measure of health-related quality of life data were collected preoperatively and at 3, 6, 12, and 24 months. Hospital costs relating to surgical management including postoperative radiotherapy were analyzed. A retrospective cohort of patients treated with radiotherapy alone between 2011 and 2015, which matched the surgical patients for disease load and presentation, also was assessed. Quality-adjusted life years (QALYs) for nonsurgical patients were modelled against the surgical group. In total, 38 consecutive surgical patients had information for cost-utility analysis and 8 nonsurgical patients were matched. Mean total cost in the surgical group was €16,989 (SD €8148), largely comprising nonmedical staffing cost (mean €7721, 45.9%), followed by daily operational costs (€2963, 17.6%) and medical staffing costs (€2621, 15.6%). Median initial health status was 0.33 (interquartile range 0.15–0.55), and median postoperative QALYs were 0.70 (interquartile range 0.18–1.70). Mean total cost in the nonsurgical cohort was €9354. The incremental cost-effectiveness ratio for surgical management was €13,635 (range €12,726–€14,407) per QALY. A reliable computational model of the human head is necessary for better understanding of the physical mechanisms of traumatic brain injury (TBI), car-crash investigation, development of protective head gear and advancement of dural replacement materials. The performance and biofidelity of these models depend largely on the material description of the different structures present in the head. One of these structures is the dura mater, the protective layer around the brain. We tested five human dura mater specimens, with samples at different locations, using planar biaxial tests. We describe the resulting stress-strain curves using both the anisotropic Gasser-Ogden-Holzapfel (GOH) model and the isotropic one-term Ogden model. The low-strain section of the curves is also described using a Neo-Hookean formulation. The obtained stress-strain curves reveal highly nonlinear but isotropic behaviour. A significant amount of inter- and intra-specimen variability is noticed, whereby the latter does not seem to be influenced by location. The GOH model achieves the best fit of the individual test data. A simple Neo-Hookean model can only be used with extreme caution, as it does not manage to capture the nonlinear effects present even at low strains. [ABSTRACT FROM AUTHOR]

At present, Finite Element (FE) analyses are often used as a tool to better understand the mechanisms of head injury. Previously, these models have been compared to cadaver experiments, with the next step under development being accident reconstructions. Thus far, the main focus has been on deriving an injury threshold and little effort has been put into correlating the documented injury location with the response displayed by the FE model. Therefore, the purpose of this study was to introduce a novel image correlation method that compares the response of the FE model with medical images. The injuries shown on the medical images were compared to the strain pattern in the FE model and evaluated by two indices; the Overlap Index (OI) and the Location Index (LI). As the name suggests, OI measures the area which indicates both injury in the medical images and high strain values in the FE images. LI evaluates the difference in center of mass in the medical and FE images. A perfect match would give an OI and LI equal to 1. This method was applied to three bicycle accident reconstructions. The reconstructions gave an average OI between 0.01 and 0.19 for the three cases and between 0.39 and 0.88 for LI. Performing injury reconstructions are a challenge as the information from the accidents often is uncertain. Introduction: Endoscopic third ventriculostomy (ETV) has been accepted as the procedure of choice for the treatment of obstructive hydrocephalus in children and adults. The role and outcome of this procedure in the elderly has not been evaluated yet. Materials and methods: Over an 11-year interval we retrospectively analyzed data of patients, 65+ years of age, who underwent ETV in our center. Success of the procedure was assessed in terms of symptom relief and/or elimination of the need for shunting. Additionally pre- and postoperative ventricular volumes were estimated using Evan's index (Ei) and fronto-occipital horn ratio (FOR). In our analysis we compared the results of the elderly patients with those of the pediatric and adult age groups treated in our center. Results: We obtained data of 16 elderly cases (11 males, 5 females), mean age 72.8 years (66-83 years) out of the 91 patients treated with ETV in total. The success rate was 75% in this age group; mean follow-up 18.4 months (2-55 months). In 10 patients a mass lesion was the underlying cause of hydrocephalus. Mean ventricular size reduction was 18% and 13.5% (Ei and FOR) in the success group vs. 7.6% and 6.2% in the failure group. Three out of four patients who had shunting pre-EVT, became shunt independent postoperatively. The presence of flow void over the stoma was 100% correlated with success. All 7 patients with a primary or metastatic brain tumor were able to receive radiation therapy. Conclusion: Also in elderly, ETV is a safe and efficient procedure, with success rates similar to the younger population. Bridging veins drain the venous blood from the cerebral cortex into the superior sagittal sinus (SSS) and doing so they bridge the subdural space. Despite their importance in head impact biomechanics, little is known about their properties with respect to histology, morphology and mechanical behaviour. Knowledge of these characteristics is essential for creating a biofidelic finite element model to study the biomechanics of head impact, ultimately leading to the improved design of protective devices by setting up tolerance criteria. This paper presents a comprehensive review of the state-of-the-art knowledge on bridging veins. Tolerance criteria to prevent head injury through impact have been set by a number of research groups, either directly through impact experiments or by means of finite element (FE) simulations. Current state-of-the-art FE head models still lack a biofidelic representation of the bridging veins. To achieve this, a thorough insight into their nature and behaviour is required. Therefore, an overview of the general morphology and histology is provided here, showing the clearly heterogeneous nature of the bridging vein complex, with its three different layers and distinct morphological and histological changes at the region of outflow into the superior sagittal sinus. Apart from a complex morphology, bridging veins also exhibit complex mechanical behaviour, being nonlinear, viscoelastic and prone to damage. Existing material models capable of capturing these properties, as well as methods for experimental characterisation, are discussed. Future work required in bridging vein research is firstly to achieve consensus on aspects regarding morphology and histology, especially in the outflow cuff segment. Secondly, the advised material models need to be populated with realistic parameters through biaxial mechanical experiments adapted to the dimensions of the bridging vein samples. Finally, updating the existing finite element head models with these parameters will render them truly biofidelic, allowing the establishment of accurate tolerance criteria and, ultimately, better head protection devices. Abstract: The most frequent head injuries resulting from bicycle accidents include skull fracture acute subdural hematoma (ASDH), cerebral contusions, and diffuse axonal injury (DAI). This review includes epidemiological studies, cadaver experiments, in vivo imaging, image processing techniques, and computer reconstructions of cycling accidents used to estimate the mechanical parameters leading to specific head injuries. The results of the head impact tests suggest the existence of an energy failure level for the skull fracture, specific for different impact regions (22–24J for the frontal site and 5–15J for temporal site). Typical linear patterns were described for frontal, parietal and occipital skull fracture. Temporal skull fracture described considerably higher variability. In term of contusion mechanogenesis, the experiments proved that relative brain–skull motion will not be prevented if the maximum frequency of the impact frequency spectrum stays below 150Hz or below the frequency corresponding to the impedance peak of the head under investigation. The brain shift patterns in humans, both in dynamic and quasistatic situations were shown to be very complex, with maximum amplitudes localized at the level of the inferolateral aspects of the frontal and temporal lobes. The resulting brain maximum amplitudes differed when the head was subjected to a sagittal or lateral motion. Finally, the presented data support the existence of a critical elongation/stretch criterion for the occurrence of ASDH due to BV rupture, located around 5mm elongation or 25% stretch limit. In addition, a tolerance level lying around 10,000rad/s2 for pulse durations below 10ms was established for BV rupture, which seems to decrease with increasing pulse duration. The described research indicates that injury specific tolerance criteria can provide a more accurate prediction for head injuries than the currently used HIC. Internal brain lesions are strongly related to rotational effects which are not appropriately accounted by the commonly accepted head injury criterion (HIC). The research summarized in this paper adds significantly to the creation of a fundamental knowledge for the improvement of bicycle helmets as well as other head protective measures. Abstract: Background: Traumatic brain injury is expected to become the major cause of death and disability for children and young adults by the year 2020. One of the most frequent and most morbid pathologies resulted from a head trauma is acute subdural haematoma (ASDH). For nearly one third of the ASDH cases the etiopathology directly relates to a bridging vein (BV) rupture. Methods: In the current study the bridging vein–superior sagittal sinus (BV–SSS) units were axially stretched until failure for strain rates ranging from 2.66s−1 to 185.61s−1, in order to investigate any strain rate dependency in their mechanical behaviour. Findings: Results showed that up to 200s−1, the effect of the strain rate on veins' mechanical behaviour is outweighed by the large morphological intra- and inter-individual variations. Gender had a strong influence on the BVs geometrical description, but exerted no direct influence on the BV biomechanical parameters. Veins' dimensions had the strongest influence on the BV mechanical behaviour and on the failure mechanism. Interpretation: The present study brings important contribution to the ASDH research, emphasising the importance of considering the BV–SSS complex as a whole when trying to describe the ASDH mechanopathology. Within the framework of a bicycle helmet research program, we have set up a database of bicycle accident victims, containing both accident and clinical data. The database consists of a consecutive series of 86 victims of bicycle accidents who underwent a neurosurgical intervention in our hospital between 1990 and 2000. Data were obtained from police files, medical records, computed tomography head scans and a patient questionnaire. In only three victims, the wearing of a helmet was documented. In this study, the head injuries are analysed and the relation between the different types of head injuries and outcome is assessed.Forty-four accidents were collisions with a motor vehicle and 42 accidents were falls. Most impacts occurred at the side (57%) or at the front (27%) of the head. The most frequent injuries were skull fractures (86%) and cerebral contusions (73%). Age was negatively correlated with outcome (P=0.0002) and positively correlated with the number (P=0.00002) and volume (P=0.00005) of contusions and the presence of subdural haematomas (P=0.0004). The injuries with the strongest negative effect on outcome were: subarachnoid haemorrhage (P=0.000001), multiple (P=0.000005) or large (P=0.0007) contusions, subdural haematoma (P=0.001) and brain swelling (P=0.002). A significant coexistence of these four injuries was found. We hypothesise that in many patients the contusions may have been the primary injuries of this complex and should therefore be considered as a main injury determining outcome in this study. Background: As the scope of neurosurgical spinal operations has broadened widely, spinal neurosurgery is certainly the one of the fields of neurosurgery where the most changes are observed. Increasing health costs and an aging population will make this issue even more crucial in the future. Methods: We reviewed the number of spinal procedures performed by neurosurgeons in Belgium between 2000 and 2005. During this period, the number of lumbar spinal operations for degenerative spinal diseases has increased by 20%, reaching more than 23,000 operations per year, among which 77% were decompressive surgeries, 43% posterior fusions, and 50% anterior fusions, all done by neurosurgeons. The neurosurgical prevalence is even more striking in cervical or intradural procedures. Conclusion: As a result we created a neurosurgical spine society to represent neurosurgeons who were otherwise almost absent from the political decision-making processes. These observations, not unique to Belgium, should encourage colleagues to play a pivotal role in all spine-related discussions with healthcare providers, state or private, to get involved in teaching spinal surgery, and to participate in clinical and laboratory research leading to the publications that are essential to put forward the contribution of neurosurgeons in spinal surgery. Reliable computer models are needed for a better understanding of the physical mechanisms of skull fracture in accidental hits, falls, bicycle - motor vehicle & car accidents and assaults. The performance and biofidelity of these models depend on the correct anatomical representation and material description of these structures. In literature, a strain energy criterion has been proposed to predict skull fractures. However, a broad range of values for this criterion has been reported. This study investigates if the impactor orientation, scalp thickness and material model of the skull could provide us with insight in the influencing factors of this criterion. 18 skull fracture experiments previously performed in our research group were reproduced in finite element simulations. Subject-specific skull geometries were derived from medical images and used to create high-quality finite element meshes. Based on local Hounsfield units, a subject-specific isotropic material model was assigned. The subject-specific models were able to predict fractures who matched visually with the corresponding experimental fracture patterns and provided detailed fracture patterns. The sensitivity study showed that small variations in impactor positioning as well as variations of the local geometry (frontal-temporal-occipital) strongly influenced the skull strain energy. Subject-specific modelling leads to a more accurate prediction of the force-displacement curve. The average error of the peak fracture force for all the 18 cases is 0.4190 for the subject-specific and 0.4538 for the homogeneous material model, for the displacement; 0.3368 versus 0.3844. But it should be carefully interpreted as small variations in the computational model significantly influence the outcome. [ABSTRACT FROM AUTHOR]

Background: Routinely collected health data (RCHD) offers many opportunities for traumatic brain injury (TBI) research, in which injury severity is an important factor.Objective: The use of clinical injury severity indices in a context of RCHD is explored, as are alternative measures created for this specific purpose. To identify useful scales for full body injury severity and TBI severity this study focuses on their performance in predicting these currently used indices, while accounting for age and comorbidities.Data: This study utilized an extensive population-based RCHD dataset consisting of all patients with TBI admitted to any Belgian hospital in 2016.Methods: Full body injury severity is scored based on the (New) Injury Severity Score ((N)ISS) and the ICD-based Injury Severity Score (ICISS). For TBI specifically, the Abbreviated Injury Scale (AIS) Head, Loss of Consciousness and the ICD-based Injury Severity Score for TBI injuries (ICISS) were used in the analysis. These scales were used to predict three outcome variables strongly related to injury severity: in-hospital death, admission to intensive care and length of hospital stay. For the prediction logistic regressions of the different injury severity scales and TBI severity indices were used, and error rates and the area under the receiver operating curve were evaluated visually.Results: In general, the ICISS had the best predictive performance (error rate between 0.06 and 0.23; AUC between 0.82 [0.81;0.83] and 0.86 [0.85;0.86]). A clearly increasing error rate can be noticed with advancing age and accumulating comorbidity.Conclusion: Both for full body injury severity and TBI severity, the ICISS tends to outperform other scales. It is therefore the preferred scale for use in research on TBI in the context of RCHD. In their current form, the severity scales are not suitable for use in older populations. Background As survival after treatment for symptomatic spinal metastases increases, the incidence of local tumor recurrence also may increase. However, data regarding incidence and timing of recurrence or duration of survival after second surgeries are not readily available and may help to inform clinicians when to perform second surgeries. Objective To identify features associated with loss of local control (LLC) at a previously treated or new spinal level. Methods Clinical and surgical data were collected from a prospective cohort of 1421 patients who had surgery for symptomatic spinal metastases. Patients undergoing repeat spinal surgery for symptomatic LLC at the same or a different level were identified and analyzed. Results In total, 3.0% patients underwent repeat surgery for symptomatic LLC after a median interval of 184 days from the first surgery; median survival was 6.1 months after second surgery. Factors associated with second surgery for LLC were the primary tumor type, number of spinal levels, Tomita staging, Tokuhashi and Karnofsky scores, anterior surgical approach, more aggressive surgical resection, and postoperative radiotherapy. In total, 1.5% patients were admitted for surgery for a different spinal level than the index operation after median 338 days from the first operation. Conclusions The likelihood for repeat surgery due to LLC cannot be accurately predicted at the time of initial presentation. Factors associated with second surgery for LLC relate to less aggressive tumor biology and better survival. Most patients had a reasonable duration of survival after second surgery. Background context Prior studies have demonstrated the superiority of decompression and fusion over decompression alone for the treatment of lumbar degenerative spondylolisthesis with spinal stenosis. More recent studies have investigated whether nonfusion stabilization could provide durable clinical improvement after decompression and fusion. Purpose To examine the clinical safety and effectiveness of decompression and implantation of a novel flexion restricting paraspinous tension band (PTB) for patients with degenerative spondylolisthesis. Study design A prospective clinical study. Patient sample Forty-one patients (7 men and 34 women) aged 45 to 83 years (68.2±9.0) were recruited with symptomatic spinal stenosis and Meyerding Grade 1 or 2 degenerative spondylolisthesis at L3–L4 (8) or L4–L5 (33). Outcome measures Self-reported measures included visual analog scale (VAS) for leg, back, and hip pain and the Oswestry Disability Index (ODI). Physiologic measures included quantitative and qualitative radiographic analysis performed by an independent core laboratory. Methods Patients with lumbar degenerative spondylolisthesis and stenosis were prospectively enrolled at four European spine centers with independent monitoring of data. Clinical and radiographic outcome data collected preoperatively were compared with data collected at 3, 6, 12, and 24 months after surgery. This study was sponsored by the PTB manufacturer (Simpirica Spine, Inc., San Carlos, CA, USA), including institutional research support grants to the participating centers totaling approximately US $172,000. Results Statistically significant improvements and clinically important effect sizes were seen for all pain and disability measurements. At 24 months follow-up, ODI scores were reduced by an average of 25.4 points (59%) and maximum leg pain on VAS by 48.1 mm (65%). Back pain VAS scores improved from 54.1 by an average of 28.5 points (53%). There was one postoperative wound infection (2.4%) and an overall reoperation rate of 12%. Eighty-two percent patients available for 24 months follow-up with a PTB in situ had a reduction in ODI of greater than 15 points and 74% had a reduction in maximum leg pain VAS of greater than 20 mm. According to Odom criteria, most of these patients (82%) had an excellent or good outcome with all except one patient satisfied with surgery. As measured by the independent core laboratory, there was no significant increase in spondylolisthesis, segmental flexion-extension range of motion, or translation and no loss of lordosis in the patients with PTB at the 2 years follow-up. Conclusions Patients with degenerative spondylolisthesis and spinal stenosis treated with decompression and PTB demonstrated no progressive instability at 2 years follow-up. Excellent/good outcomes and significant improvements in patient-reported pain and disability scores were still observed at 2 years, with no evidence of implant failure or migration. Abstract: We studied 2 patients with large expansive and osteolytic lesions of the skull on CT, with characteristics suggestive of intradiploic epidermoid cysts. It was the purpose of our work to study the MR-imaging characteristic of these lesions. Both lesions proved to have the same MR-imaging characteristic than epidermoid cysts elsewhere in the intracranial compartment. The lesions were inhomogeneously hypointense on T1-weighted images, inhomogeneously hyperintense on T2-weighted images and on FLAIR, without signal attenuation. The signal intensity was high on diffusion-weighted images reflecting diffusion restriction in one patient with low ADC-value and a factor T2-shine through in the other patient with an intermediate ADC-value. The behaviour of the lesion on a CISS sequence, performed in one patient was also similar to other epidermoid cysts, but this proved less relevant at this location. [Copyright &y& Elsevier]

Background: Despite being well established, acute surgery in traumatic acute subdural haematoma is based on low-grade evidence. We aimed to compare the effectiveness of a strategy preferring acute surgical evacuation with one preferring initial conservative treatment in acute subdural haematoma.Methods: We did a prospective, observational, comparative effectiveness study using data from participants enrolled in the Collaborative European Neurotrauma Effectiveness Research in Traumatic Brain Injury (CENTER-TBI) cohort. We included patients with no pre-existing severe neurological disorders who presented with acute subdural haematoma within 24 h of traumatic brain injury. Using an instrumental variable analysis, we compared outcomes between centres according to treatment preference for acute subdural haematoma (acute surgical evacuation or initial conservative treatment), measured by the case-mix-adjusted percentage of acute surgery per centre. The primary endpoint was functional outcome at 6 months as rated with the Glasgow Outcome Scale Extended, which was estimated with ordinal regression as a common odds ratio (OR) and adjusted for prespecified confounders. Variation in centre preference was quantified with the median OR (MOR). CENTER-TBI is registered with ClinicalTrials.gov, number NCT02210221, and the Resource Identification Portal (Research Resource Identifier SCR_015582).Findings: Between Dec 19, 2014 and Dec 17, 2017, 4559 patients with traumatic brain injury were enrolled in CENTER-TBI, of whom 1407 (31%) presented with acute subdural haematoma and were included in our study. Acute surgical evacuation was done in 336 (24%) patients, by craniotomy in 245 (73%) of those patients and by decompressive craniectomy in 91 (27%). Delayed decompressive craniectomy or craniotomy after initial conservative treatment (n=982) occurred in 107 (11%) patients. The percentage of patients who underwent acute surgery ranged from 5·6% to 51·5% (IQR 12·3-35·9) between centres, with a two-times higher probability of receiving acute surgery for an identical patient in one centre versus another centre at random (adjusted MOR for acute surgery 1·8; p<0·0001]). Centre preference for acute surgery over initial conservative treatment was not associated with improvements in functional outcome (common OR per 23·6% [IQR increase] more acute surgery in a centre 0·92, 95% CI 0·77-1·09).Interpretation: Our findings show that treatment for patients with acute subdural haematoma with similar characteristics differed depending on the treating centre, because of variation in the preferred approach. A treatment strategy preferring an aggressive approach of acute surgical evacuation over initial conservative treatment was not associated with better functional outcome. To investigate the occurrence and progression of heterotopic ossification (HO) in patients treated by anterior cervical discectomy with arthroplasty. It was evaluated if HO affects clinical outcome and range of motion (ROM). Risk factors of HO was studied as well. Patients who underwent anterior cervical discectomy with arthroplasty for a cervical radiculopathy because of a herniated disc from the NECK and PROCON trial were analyzed for HO at 12 and 24 months postoperatively. HO was scored according to the McAfee-Mehren classification. The index ROM was defined by a custom developed image analysis tool, and global cervical ROM was measured by Cobb's angle. Clinical outcome was evaluated by means of the Neck Disability Index and the 36-Item Short Form Health Survey. The occurrence of HO was 60% at 1 year, and it increased to 76% at 2-year follow-up. A total of 31% of patients were scored as high-grade HO at 1-year follow-up, and this percentage increased to 50% at 2-year follow-up. Clinical outcome does not correlate to HO grade, and no risk factor for high-grade HO could be identified. The ROM at the index level was significantly higher in low-grade HO group than those patients with high-grade HO, but in 15%–38% HO grade does not correspond to ROM. HO occurs in three fourths of the patients at 2 years after surgery, but does not necessarily correspond to clinical outcome, nor loss or preservation of ROM. Background: Motion preservation prostheses were introduced to prevent adjacent disc degeneration (ASD) and to diminish neck disability in the postsurgical follow-up. However, it is still a controversial issue, and the relationship between range of motion (ROM) and ASD has not been studied.Purpose: To compare the correlation between ROM of the cervical spine and the presence of radiological ASD after anterior discectomy. Clinical outcome was also correlated to ROM and ASD.Study Design: Retrospective cohort study.Methods: In all, 253 patients who underwent anterior discectomy for cervical radiculopathy due to a herniated disc were analyzed for segmental and global cervical ROM and the presence of ASD both preoperatively, and 12 and 24 months postoperatively. Patients who were included in two randomized, double-blinded trials comparing anterior cervical discectomy with arthroplasty, anterior cervical discectomy with intervertebral cage, or anterior cervical discectomy without intervertebral cage for one level disc herniation were analyzed. ROM was defined by a custom-developed image analysis tool. ASD was defined by decrease in disc height and anterior osteophyte formation on X-rays. Clinical outcome was evaluated by means of the Neck Disability Index (NDI).Results: Two years postoperatively, no correlation was demonstrated between ROM and ASD. The incidence of ASD was comparable in the three groups, being 34% at baseline, and 58% at 2-year follow-up. Likewise, ASD progression was comparable in the three treatment arms. No correlation was demonstrated between ROM and NDI or ASD and NDI.Conclusions: Since ROM is not correlated to ASD, and clinical outcome is not correlated to ROM either, the relevance of continued ROM at the target level seems absent. Acute subdural hematoma (ASDH) is one of the most frequent traumatic brain injuries (TBIs) with high mortality rate. Bridging vein (BV) ruptures is a major cause of ASDH. The KTH finite element head model includes bridging veins to predict acute subdural hematoma due to BV rupture. In this model, BVs were positioned according to Oka et al. (1985). The aim of the current study is to investigate whether the location and entry angles of these BVs could be modelled using data from a greater statistical sample, and what the impact of this improvement would be on the model's predictive capability of BV rupture. From the CT angiogram data of 78 patients, the relative position of the bridging veins and their entry angles along the superior sagittal sinus was determined. The bridging veins were repositioned in the model accordingly. The performance of the model, w.r.t. BV rupture prediction potential was tested on simulations of full body cadaver head impact experiments. The experiments were simulated on the original version of the model and on three other versions which had updated BV positions according to mean, maximum and minimum entry angles. Even though the successful prediction rate between the models stayed the same, the location of the rupture site significantly improved for the model with the mean entry angles. Moreover, the models with maximum and minimum entry angles give an insight of how BV biovariability can influence ASDH. In order to further improve the successful prediction rate, more biofidelic data are needed both with respect to bridging vein material properties and geometry. Abstract Aim Surgery for spinal metastases can improve symptoms, but sometimes complications can negate the benefits. Operations may have different indications, complexities and risks, and the choice for an individual is a tailor-made personalised decision. Previous prognostic scoring systems are becoming out of date and inaccurate. We designed a risk calculator to estimate survival after surgery, to inform clinicians and patients when making management decisions. Methods A prospective cohort study was performed, including 1430 patients with spinal metastases who underwent surgery. Of them, 1264 patients from 20 centres were used for model development using a Cox frailty model. Calibration slope, D-statistic and C-index were used for model validation based on 166 patients. Follow-up was to death or minimum of 2 years after surgery. Pre-operative indices (examination findings, pain, Karnofsky physical functioning score, and radiology) were assessed. Results An algorithm to predict survival was constructed including the tumour type, ambulatory status, analgesic use, American Society of Anesthesiologists score, number of spinal metastases, previous radiotherapy or chemotherapy, presence of visceral metastases, cervical or thoracic spine involvement, as predictors. An Internet-based risk calculator was developed based on this algorithm, with similar or improved accuracy compared to other validated prognostic scoring systems (C-index, 0.68; 95% confidence interval, 0.63––0.73, and calibration slope, 1.00; 95% confidence interval, 0.68––1.32). Conclusion A large, prospective, surgical series of patients with symptomatic spinal metastases was used to create a validated risk calculator that can help clinicians to inform patients about the most appropriate treatment plan. RESCUEicp studied decompressive craniectomy (DC) applied as third-tier option in severe traumatic brain injury (TBI) patients in a randomized controlled setting and demonstrated a decrease in mortality with similar rates of favorable outcome in the DC group compared to the medical management group. In many centers, DC is being used in combination with other second/third-tier therapies. The aim of the present study is to investigate outcomes from DC in a prospective non-RCT context. This is a prospective observational study of 2 patient cohorts: one from the University Hospitals Leuven (2008–2016) and one from the Brain-IT study, a European multicenter database (2003–2005). In thirty-seven patients with refractory elevated intracranial pressure who underwent DC as a second/third-tier intervention, patient, injury and management variables including physiological monitoring data and administration of thiopental were analysed, as well as Extended Glasgow Outcome score (GOSE) at 6 months. In the current cohorts, patients were older than in the surgical RESCUEicp cohort (mean 39.6 vs. 32.3; p < 0.001), had higher Glasgow Motor Score on admission (GMS < 3 in 24.3% vs. 53.0%; p = 0.003) and 37.8% received thiopental (vs. 9.4%; p < 0.001). Other variables were not significantly different. GOSE distribution was: death 24.3%; vegetative 2.7%; lower severe disability 10.8%; upper severe disability 13.5%; lower moderate disability 5.4%; upper moderate disability 2.7%, lower good recovery 35.1%; and upper good recovery 5.4%. The outcome was unfavorable in 51.4% and favorable in 48.6%, as opposed to 72.6% and 27.4% respectively in RESCUEicp (p = 0.02). Outcomes in DC patients from two prospective cohorts reflecting everyday practice were better than in RESCUEicp surgical patients. Mortality was similar, but fewer patients remained vegetative or severely disabled and more patients had a good recovery. Although patients were older and injury severity was lower, a potential partial explanation may be in the pragmatic use of DC in combination with other second/third-tier therapies in real-life cohorts. Background Individuals with non-specific low back pain show decreased reliance on lumbosacral proprioceptive signals and slower sit-to-stand-to-sit performance. However, little is known in patients after lumbar microdiscectomy. Methods Patients were randomly assigned into transmuscular ( n = 12) or paramedian lumbar surgery ( n = 13). After surgery, the same patients were randomly assigned into individualized active physiotherapy starting 2 weeks after surgery ( n = 12) or usual care ( n = 13). Primary outcomes were center of pressure displacement during ankle and back muscles vibration (to evaluate proprioceptive use), and the duration of five sit-to-stand-to-sit movements, evaluated at 2 (baseline), 8 and 24 weeks after surgery. Findings Two weeks after surgery, all patients showed smaller responses to back compared to ankle muscles vibration ( P < 0.05). Patients that underwent a transmuscular surgical procedure and patients that received physiotherapy switched to larger responses to back muscles vibration at 24 weeks, compared to 2 weeks after surgery ( P < 0.005), although not seen in the paramedian group and usual care group ( P > 0.05). Already 8 weeks after surgery, the physiotherapy group needed significantly less time to perform five sit-to-stand-to-sit movements compared to the usual care group ( P < 0.05). Interpretation Shortly after lumbar microdiscectomy, patients favor reliance on ankle proprioceptive signals over lumbosacral proprioceptive reliance to maintain posture, which resembles the behavior of patients with non-specific low back pain. However, early active physiotherapy after lumbar microdiscectomy facilitated higher reliance on lumbosacral proprioceptive signals and early improvement of sit-to-stand-to-sit performance. Abstract: Background: Chronic neuropathic pain after injury to a peripheral nerve is known to be resistant to treatment. Peripheral nerve stimulation is one of the possible treatment options, which is, however, not performed frequently. In recent years we have witnessed a renewed interest for PNS. The aim of the present study was to evaluate the long-term clinical efficacy of PNS in a group of patients with peripheral neuropathic pain treated with PNS since the 1980s. Methods: Of an original series of 11 patients, 5 patients could be invited for clinical examination, detailed assessment of clinical pain and QST examination. The assessments were done both during habitual use of PNS and with the stimulator off. Results: Average pain intensity and pain unpleasantness ratings as assessed with visual analog and verbal rating scales showed significant beneficial effects of PNS. Quality of life measures (sleep and daily functioning) also showed positive effects. Quantitative Sensory Testing results did not show significant differences in cold pain and heat pain thresholds between the “ON” and “OFF” conditions. Conclusion: In selected patients with peripheral neuropathic pain PNS remains effective even after more than 20 years. [Copyright &y& Elsevier]

Background Indications for surgery for symptomatic spinal metastases have become better defined in recent years, and suitable outcome measures have been established against a changing backdrop of patient characteristics, tumor behavior, and oncologic treatments. Nonetheless, variations still exist in the local management of patients with spinal metastases. In this study, we aimed to review global trends and habits in the surgical treatment of symptomatic spinal metastases, and to examine how these have changed over the last 25 years. Methods In this cohort study of consecutive patients undergoing surgery for symptomatic spinal metastases, data were collected using a secure Internet database from 22 centers across 3 continents. All patients were invited to participate in the study, except those unable or unwilling to give consent. Results There was a higher incidence of colonic, liver, and lung carcinoma metastases in Asian countries, and more frequent presentation of breast, prostate, melanoma metastases in the West. Trends in surgical technique were broadly similar across the centers. Overall survival rates after surgery were 53% at 1 year, 31% at 2 years, and 10% at 5 years after surgery (standard error 0.013 for all). Survival improved over successive time periods, with longer survival in patients who underwent surgery in 2011–2016 compared with those who underwent surgery in earlier time periods. Conclusions Surgical habits have been fairly consistent among countries worldwide and over time. However, patient survival has improved in later years, perhaps due to medical advances in the treatment of cancer, improved patient selection, and operating earlier in the course of disease. Objective According to level 2 evidence, earlier evacuation of acute subdural or epidural hematomas necessitating surgery is associated with better outcome. Hence, guidelines recommend performing these procedures immediately. Literature on the extent and causes of prehospital and intrahospital intervals in patients with trauma requiring emergent craniotomies is almost completely lacking. Studies delineating and refining the interval before thrombolytic agent administration in ischemic stroke have dramatically reduced the door-to-needle time. A similar exercise for trauma-to-decompression time might result in comparable reductions. We aim to map intervals in emergent trauma craniotomies in our level 1 trauma center, screen for associated factors, and propose possible ways to reduce these intervals. Methods We analyzed patients who were primarily referred (1R; n = 45) and secondarily referred (after computed tomography imaging in a community hospital [2R; n = 22]) to our emergency department (ED) and underwent emergent trauma craniotomies between 2010 and 2016. Results Median prehospital interval (between emergency call and arrival at the ED) was 42 minutes for 1R patients. Median intrahospital interval (between initial ED arrival and skin incision [SI]) was 140 minutes and 268 minutes for 1R and 2R patients, respectively. In 1R patients, ED-SI interval was positively correlated with Glasgow Coma Scale score (ρ=.49; P < 0.001), but not with age, time of ED arrival, or extended Glasgow Outcome Scale score at 6 months. Based on outlier analysis, we propose prehospital and intrahospital measures to improve performance. Conclusions This is the first report on emergency call–SI interval in emergent trauma craniotomy, with a median of 174 minutes and >297 minutes for 1R and 2R patients, respectively, in our center.
