Twenty-seven experts under the aegis of the French Association of Surgery (AFC) offer this reference system with formalized recommendations concerning the performance of right colectomy by robotic approach (RRC). For RRC, experts suggest patient installation in the so-called "classic" or "suprapubic" setup. For patients undergoing right colectomy for a benign pathology or cancer, RRC provides no significant benefit in terms of intra-operative blood loss, intra-operative complications or conversion rate to laparotomy compared to laparoscopy. At the same time, RRC is associated with significantly longer operating times. Data from the literature are insufficient to define whether the robot facilitates the performance of an intra-abdominal anastomosis, but the robotic approach is more frequently associated with an intra-abdominal anastomosis than the laparoscopic approach. Experts also suggest that RRC offers a benefit in terms of post-operative morbidity compared to right colectomy by laparotomy. No benefit is retained in terms of mortality, duration of hospital stay, histological results, overall survival or disease-free survival in RRC performed for cancer. In addition, RRC should not be performed based on the cost/benefit ratio, since RRC is associated with significantly higher costs than laparoscopy and laparotomy. Future research in the field of RRC should consider the evaluation of patient-targeted parameters such as pain or quality of life and the technical advantages of the robot for complex procedural steps, as well as surgical and oncological results. Surgical management of obstructive left colon cancer (OLCC) is controversial. The objective is to report on postoperative and oncological outcomes of the different surgical options in patients operated on for OLCC. From 2000–2015, 1500 patients were treated for OLCC in centers members of the French Surgical Association. Colonic stent (n = 271), supportive care (n = 5), palliative derivation (n = 4) were excluded. Among 1220 remaining patients, 456 had primary diverting colostomy (PDC), 329 a segmental colectomy (SC), 246 a Hartmann's procedure (HP) and 189 a subtotal colectomy (STC) as first-stage surgery. Perioperative data and oncological outcomes were compared retrospectively. There was no difference between the 4 groups regarding gender, age, BMI and comorbidities. Postoperative mortality and morbidity were 4–27% (PDC), 6–47% (SC), 9–55% (HP), 13–60% (STC), respectively (P = 0.005). Among the 431 living patients after PDC, 321 (70%) patients had their primary tumour removed. Cumulative mortality and morbidity favoured PDC (7–39%) and SC (6–40%) compared to HP (1–47%) and STC (13–50%) (P = 0.04). At the end of follow-up definitive stoma rates were 39% (HP), 24% (PDC), 10% (SC), and 8% (STC) (P < 0.0001). Five-year overall and disease-free survival was: SC (67–55%), PDC (54–48%), HP (54–37%) and STC (48–49%). After multivariate analysis, SC and PDC were associated with better prognosis compared to HP and STC. In OLCC, SC and PDC are the two preferred options in patients with good medical conditions. For patients with severe comorbidities PDC should be recommended, reserving HP and STC for patients with colonic ischaemia or perforation complicating malignant obstruction. [ABSTRACT FROM AUTHOR]

Résumé Le traitement standard des cancers du rectum, associant chimioradiothérapie suivie de chirurgie radicale avec exérèse totale du mésorectum permet d’assurer le contrôle local pelvien, au prix d’une morbidité importante et de séquelles fonctionnelles. Ce traitement standard identique pour tous les patients atteints d’un cancer du bas ou moyen rectum à partir du stade T3 serait excessif pour les petites tumeurs, mais probablement insuffisant pour les tumeurs localement évoluées. En effet, les récidives métastatiques restent fréquentes. La prise en charge est discutée en réunion de concertation pluridisciplinaire sur la base du bilan initial, en particulier de l’imagerie par résonance magnétique qui peut prédire la marge de résection et évaluer la réponse au traitement. La stratégie est amenée à évoluer vers un repositionnement de la chimiothérapie en début de prise en charge et vers une désescalade thérapeutique pour les petites tumeurs avec omission de la radiothérapie ou de la chirurgie dans une perspective de préservation rectale. Enfin, la stratégie thérapeutique peut être rediscutée et adaptée en fonction de la réponse tumorale, qui est associée au pronostic. Abstract Standard treatment consisting of chemoradiotherapy followed by radical surgery with total mesorectal excision, resulting in good oncologic local control but high morbidity and poor functional results. The same treatment applied to all patients presenting with low or mid T3–4 rectal tumors could result in overtreatment of small tumors. However, it remains insufficient (or unsatisfactory?) for locally advanced tumors regarding metastatic recurrence rate. Treatment is decided by a multidisciplinary board on the basis of initial staging, including MRI which allows for resection margin prediction and post-treatment response evaluation. The therapeutic strategy is changing towards upfront chemotherapy and therapeutic desescalation omitting radiotherapy or surgery in a rectal preservation strategy. Background Rectal cancer involving at least one adjacent organ (mrT4b) requires multi-visceral resection to achieve clear resection margin (R0). Performing pelvic compartment preservation according to the tumour response has not been considered. This study assesses the impact of changing the surgical strategy according to tumour response in rectal cancer mrT4b. Methods Patients with non-metastatic T4b rectal cancer at two tertiary referral centres between 2008 and 2013 were grouped as “Responders” ypT0-3abNx versus “Non-responders” ypT3cd-4Nx and divided into three surgical procedures: total mesorectal excision (TME), extended-TME (eTME) and beyond-TME (b-TME). End-points were circumferential resection margin, postoperative morbidity, definitive stoma formation, 3-years local recurrence (3y-LR) and 3-years disease-free survival (3y-DFS) according to both tumours' response and surgical procedures. Results Among 883 patients with rectal cancer, 101 were included. Responders had a higher rate of induction chemotherapy (59.7% vs. 38.2%; p = 0.04). Morbidity and definitive stoma formation were significantly higher in Non-responders. R0 was not impacted by either the tumour response or the surgical procedures. The 3y-LR was lower in Responders (14%) compared to Non Responders (32%) (HR 1.6; 95% CI: 1.02–2.59; p = 0.041), and was two-fold higher in e-TME compared to b-TME in Non-responders, whereas no difference was found in Responders. The 3y-DFS was higher in Responders irrespective to the surgery (71% vs. 47%; p = 0.07). Conclusion In Responders, TME or e-TME are technically and oncollogically feasible and should be considered in preferrence to b-TME. In Non-responders, allowing for high rates of morbidity and local recurrence in patients with e-TME, b-TME procedures should be preferred. [ABSTRACT FROM AUTHOR]

Background There is a significant worldwide variation in practice regarding the criteria for operative intervention and overall management in patients with locally recurrent rectal cancer (LRRC). A survival benefit has been described for patients with clear resection margins in patients undergoing surgery for LRRC which is seen as an important surgical quality indicator. Methods A prospective French national database was established in 2008 which recorded procedures undertaken for locally recurrent rectal cancer (LRRC). Overall and Disease-Free Survival (OS, DFS) were assessed retrospectively. We report the variability and the heterogeneity of LRRC management in France as well as 5-year oncological outcomes. Results In this national report, 104 questionnaires were completed at 29 French surgical centres with a high variability of cases-loaded. Patients had preoperative treatment in 86% of cases. Surgical procedures included APER (36%), LAR (25%), Hartmann's procedure (21%) and pelvic exenterations (15.5%). Four patients had a low sacrectomy (S4/S5). There were no postoperative deaths and overall morbidity was 41%. R0 was achieved in 60%, R1 and R2 in 29% and 11%, respectively. R0 resection resulted in a 5-year OS of 35% compared to 12% and 0% for respectively R1 and R2 (OR = 2.04; 95% CI: 1.4–2.98; p < 0.001). OS was similar between R2 and non-resected patients (OR = 1.47; 95% CI: 0.58–3.76; p = 0.418). Conclusions Our data is in accordance with the literature except the rate of extended resection procedures. This underlines the selective character of operative indications for LRRC in France as well as the care variability and the absence of optimal clinical pathway regarding these patients. Summary Prophylactic drainage of the abdominal cavity after gastro-intestinal surgery is widely used. The rationale is that intra-abdominal drainage enhances early detection of complications (gastro-intestinal leakage, hemorrhage, bile leak), prevents collection of fluid or pus, reduces morbidity and mortality, and decreases the duration of hospital stay. However, dogmatic attitudes favoring systematic drain placement should be questioned. The aim of this review was to evaluate the evidence supporting systematic use of prophylactic abdominal drainage following gastrectomy, pancreatectomy, liver resection, and rectal resection. Based on this review of the literature: (i) there was no evidence in favor of intra-peritoneal drainage following total or sub-total gastrectomy with respect to morbidity-mortality, nor was it helpful in the diagnosis or management of leakage, however the level of evidence is low, (ii) following pancreatic resection, data are conflicting but, overall, suggest that the absence of drainage is prejudicial, and support the notion that short-term drainage is better than long-term drainage, (iii) after liver resection without hepatico-intestinal anastomosis, high level evidence supports that there is no need for abdominal drainage, and (iv) following rectal resection, data are insufficient to establish recommendations. However, results from the French multicenter randomized controlled trial GRECCAR5 ( NCT01269567 ) should provide new evidence this coming year. Accumulating data support that systematic drainage of the abdominal cavity in digestive surgery is a non-beneficial and obsolete practice, except following pancreatectomy where the consensus appears to indicate the usefulness of short-term drainage. Résumé La chimioradiothérapie suivie de chirurgie radicale avec exérèse du mésorectum a permis d’améliorer les résultats oncologiques du traitement des cancers du rectum, au prix cependant d’une morbidité et de séquelles fonctionnelles anorectales, urinaires et sexuelles. Comme la chimioradiothérapie permet une stérilisation tumorale dans 15 à 25 % des cas, l’utilité de la chirurgie radicale chez les patients en situation de bonne réponse est remise en cause et deux stratégies de préservation d’organe ont été proposées, la surveillance ( watch and wait ) et l’exérèse locale. Cette revue de la littérature a pour objectif de faire un état des lieux des résultats des séries de préservation d’organe après chimioradiothérapie dans le traitement des cancers du rectum et de souligner les questions relatives à la sélection initiale des patients, la définition de la réponse complète, le risque ganglionnaire mésorectal, les modalités de la surveillance ainsi que les résultats oncologiques et fonctionnels. For rectal cancers, the current standard of care consists of chemoradiation followed by radical surgery with total mesorectal excision. Oncologic results are good, especially regarding local recurrence rates, but at the cost of high morbidity rates and poor anorectal, urinary and sexual function results. Since chemoradiation yields 15 to 25% pathological complete response, the role of radical surgery is questioned for patients presenting with good response after chemoradiation and two organ preservation strategies have been offered: watch and wait strategy and local excision strategy. The aim of this review is to give the results of organ preservation after chemoradiotherapy series and to highlight different questions regarding initial patient's selection, complete clinical response definition, risk of mesorectal nodal involvement, follow-up modalities as well as oncologic and functional results. Porous collagen/chitosan scaffolds with different Collagen:Chitosan (Coll:Ch) ratios were prepared by freeze-drying followed by self-crosslinking via dehydrothermal treatment (DHT) and characterized as biomaterials for tissue engineering. Cy7 and Cy5.5 fluorochromes were covalently grafted to collagen and chitosan, respectively. Thus, it was possible, using optical fluorescence imaging of the two fluorochromes, to simultaneously track their in vivo biodegradation, in a blend scaffold form. The fluorescence signal evolution, due to the bioresorption, corroborated with histological analysis. In vitro cytocompatibility of Coll:Ch blend scaffolds were evaluated with standardized tests. In addition, the scaffolds showed a highly interconnected porous structure. Extent of crosslinking was analyzed by convergent analysis using thermogravimetry, Fourier Transform Infrared Spectroscopy and PBS uptake. The variations observed with these techniques indicate strong interactions between collagen and chitosan (covalent and hydrogen bonds) promoted by the DHT. The mechanical properties were characterized to elucidate the impact of the different processing steps in the sample preparation (DHT, neutralization and sterilization by β- irradiation) and showed a robust processing scheme with low impact of Coll:Ch composition ratio. • Collagen/chitosan blends were prepared by freeze-drying followed by self-crosslinking via dehydrothermal treatment (DHT). • Collagen/chitosan scaffolds biodegradation was followed in vivo by optical fluorescence imaging with two fluorochromes. • Chitosan imposed the bioresorption rate of implants. • DHT on the collagen/chitosan blends yields crosslinker-free materials tunable for different applications. Résumé Alors que le pronostic du cancer du rectum avec métastases hépatiques synchrones s’est amélioré avec les progrès de la chimiothérapie et l’expertise croissante en chirurgie du foie et du rectum, il n’y a pas de consensus quant à leur prise en charge et les pratiques restent hétérogènes. Il est possible d’envisager un traitement à visée curative pour 20 à 30 % des patients avec une résection complète des métastases et de la tumeur primitive après chimiothérapie première. Dans cet objectif, une évaluation initiale optimale en réunion de concertation pluridisciplinaire incluant des chirurgiens hépatiques et colorectaux est primordiale. La stratégie thérapeutique associe chimiothérapie, radiothérapie, chirurgie hépatique et rectale dont l’ordre est guidé par le site le plus menaçant. Lorsque la résécabilité hépatique est difficile, une stratégie « liver first » associant chimiothérapie d’induction et chirurgie hépatique première est privilégiée. Dans le cas de métastases initialement non résécables, les progrès des associations de chimiothérapies et de thérapies ciblées permettent une résécabilité secondaire chez 30 % des patients (conversion), ce qui modifie nos pratiques et impose de réévaluer la résécabilité après chimiothérapie d’induction. Lorsque les métastases restent non résécables, un traitement de la tumeur primitive peut permettre de contrôler les symptômes pelviens qui ont un impact majeur sur la qualité de vie. La chirurgie rectale, la radiothérapie courte de cinq séances de 5 Gy, la chimioradiothérapie conformationnelle longue ou avec escalade de dose avec modulation d’intensité sont des options qui n’ont jamais été comparées et dont les avantages et inconvénients sont discutés dans cette revue. The prognosis of patients with rectal cancer and synchronous liver metastasis has improved thanks to chemotherapy and rectal and liver surgery progresses. However, there is no consensus about optimal management and practices remain heterogeneous. A curative treatment may be considered for 20 to 30% of patients with complete resection of metastasis and primary tumor after induction chemotherapy. To this end, a primary optimal evaluation by a multidisciplinary board including hepatic and colorectal surgeons is crucial. The therapeutic strategy associates chemotherapy, radiotherapy, hepatic and rectal surgery. The most threatening site guides the sequence of treatments. If hepatic resectability is uncertain, a “liver first” strategy associating induction chemotherapy and hepatic surgery is preferred. In non-resectable metastatic cases, chemotherapies with targeted therapies might lead to secondary resection for 30% of patients (conversion). This has changed our practice and triggers reconsidering resectability after chemotherapy. When metastases remain non-resectable, additional treatment focusing on primary tumor should control pelvic symptoms otherwise hardly impacting quality of life. Rectal surgery, short-course radiotherapy (5 × 5 Gy), conformational long-course chemoradiotherapy or intensity-modulated radiation therapy with dose escalation are options discussed in this review. Objectif Les tumeurs du bas rectum situées à moins de 1 cm du sphincter doivent être traitées classiquement par une amputation abdominopérinéale. L’objectif était de démontrer qu’en utilisant une classification chirurgicale, le taux de conservation sphinctérienne ou d’épargne sphinctérienne pouvait être augmenté après traitement neoadjuvant. Patients et méthodes Cette étude a inclus des patients atteints d’un cancer du bas rectum localement évolué, situé à moins de 1 cm du sphincter et classé par IRM en quatre types déterminant quatre procédures chirurgicales : ceux de type I « supra-anal » (plus de 1 cm du sphincter) devant avoir une résection antérieure basse, ceux de type II « juxta-anal » (moins de 1 cm du sphincter) une résection intersphinctérienne partielle, ceux de type III (envahissement du sphincter interne) une résection intersphinctérienne totale, ceux de types IV (envahissement du sphincter externe ou des releveurs) une amputation abdominopérinéale. Les tumeurs ont été reclassées six semaines après la chimioradiothérapie par une deuxième IRM qui déterminait la migration de type de tumeur et celle du type de chirurgie. Résultats De 2008 à 2012, 50 patients ont été inclus. Les tumeurs ont été classées en types I, II, III et IV chez respectivement zéro, un, sept et 42 patients avant la chimioradiothérapie et chez dix, 27, six, et sept après un traitement déterminant 86 % (43 sur 50) de migrations radiologiques. Cela était bien corrélé avec les 90 % (45 sur 50) de migrations chirurgicales, soit huit résections antérieures basses, 29 résections intersphinctériennes partielles, huit résections intersphinctériennes totale et cinq amputations abdominopérinéales. Le taux de résection R1 était de 10 %. À 30 mois, le taux de survie globale était de 84 % et celui de récidive locale de 6 %. Conclusion En utilisant cette classification chirurgicale et en reclassant les tumeurs après chimioradiothérapie, nous avons démontré que le traitement néoadjuvant permettait une migration du type de tumeur, du type de chirurgie réalisée et d’éviter l’amputation dans 90 % des cas sans compromettre le résultat oncologique. La radiothérapie néoadjuvante rectale peut être délivrée en technique conformationnelle tridimensionnelle ou avec modulation d'intensité. Les études rétrospectives montrent une réduction de la toxicité aiguë pendant la chimioradiothérapie délivrée avec modulation d'intensité mais aucune étude ne s'est intéressée à l'impact de ce type de technique sur la morbidité postopératoire. L'objectif de l'étude était d'évaluer la morbidité chirurgicale sévère en fonction de la technique de radiothérapie. Les objectifs secondaires étaient l'évaluation de la toxicité aiguë pendant la chimioradiothérapie et les séquelles digestives. Tous les patients pris en charge par chimioradiothérapie puis chirurgie au CHU de Bordeaux entre 2012 et 2019 pour un cancer rectal ont été inclus. Le critère de jugement principal était la morbidité chirurgicale sévère qui était renseignée de manière prospective par le chirurgien et définie comme tout événement de grade III-IV selon la classification Dindo-Clavien survenant jusqu'à 30 jours de la chirurgie. Les séquelles digestives étaient évaluées selon le score LARS. Cent-cinquante-cinq patients ont été inclus : 79 dans le bras recevant la radiothérapie conformationnelle tridimensionnelle (soit 51 %) et 75 dans le bras recevant la radiothérapie conformationnelle avec modulation d'intensité (soit 49 %). La dose médiane était de 50 Gy (intervalle : 25–60 Gy). Une résection rectale antérieure a été réalisée pour 107 patients (soit 69 %), une tumorectomie pour 25 (soit 16 %) et une chirurgie plus lourde pour 23 (soit 15 %). Il n'y a eu aucun décès et 86 patients n'ont pas présenté de complications postopératoires (soit 56 %). Une morbidité chirurgicale sévère est survenue chez 37 patients (soit 24 %), sans différence significative selon la technique de radiothérapie : 16 patients recevant la radiothérapie conformationnelle avec modulation d'intensité (soit 21 %), contre 21 patients recevant la radiothérapie conformationnelle tridimensionnelle (soit 27 %) (p = 0,628). Il y a eu plus de toxicité gastro-intestinale aiguë de grade supérieur à 2 dans le groupe recevant la radiothérapie conformationnelle tridimensionnelle : dix patients (soit 13 %) contre un patient (soit 1 %) recevant la radiothérapie conformationnelle avec modulation d'intensité (p = 0,006). Le bénéfice de la radiothérapie conformationnelle avec modulation d'intensité concernait essentiellement les diarrhées de grade supérieur à 2 et cette différence était encore plus prononcée pour les 60 patients ayant reçu une chimiothérapie d'induction, puisqu'un seul patient a eu des diarrhées grade supérieur à 2 avec la technique avec modulation d'intensité (soit 3 %) contre sept patients avec la technique tridimensionnelle (soit 24 %) (p = 0,017). Parmi les patients ayant eu une résection rectale antérieure, 52 scores de LARS ont été collectés avec un intervalle médian après la chirurgie de 1,5 an dans le groupe pris en charge par technique avec modulation d'intensité contre 5 ans dans le groupe pris en charge par la technique tridimensionnelle. Dix-huit patients ont rapporté un LARS majeur (soit 35 %) : six recevant la radiothérapie conformationnelle avec modulation d'intensité (27 %) et 12 recevant la radiothérapie conformationnelle tridimensionnelle (soit 40 %), sans différence significative. Cette étude rétrospective comparative confirme que la technique avec modulation d'intensité diminue significativement la toxicité digestive en cours de chimioradiothérapie en comparaison à la technique tridimensionnelle, sans impact sur la morbidité chirurgicale sévère, ni sur les séquelles tardives. Le bénéfice de la radiothérapie conformationnelle avec modulation d'intensité sur les diarrhées aiguës a été d'autant plus important en cas de chimiothérapie d'induction et la place ainsi comme une technique indispensable à l'heure où la chimiothérapie d'induction devrait être plus largement prescrite. Objectif de l’étude Comparaison de la tumorectomie et l’exérèse totale du mésorectum chez les patients en situation de bonne réponse à une chimioradiothérapie pour un cancer du rectum. Matériel et méthode Les patients atteints de tumeur de stade T2 ou T3 (inférieure ou égale à 4 cm) ont reçu une chimioradiothérapie (Cap50, capécitabine). Les patients en situation de bonne réponse clinique (tumeur résiduelle ≤ 2 cm) ont été randomisés entre une tumorectomie et l’exérèse totale du mésorectum. Le groupe qui a eu une tumorectomie, en cas de réponse ypT2-3 ou R1, a eu une exérèse totale du mésorectum complémentaire. L’objectif principal était de montrer la supériorité de la tumorectomie sur la l’exérèse totale du mésorectum par un critère composite incluant décès, récidive, morbidité et effets secondaires à 2 ans. Les objectifs secondaires incluaient la réponse tumorale, une survie sans récidive et globale à 3 ans. Résultats De mars 2007 à septembre 2012, 148 patientes en situation de bonne réponse (sur 186 patients pris en charge par chimioradiothérapie) ont été randomisés et les dossiers de 145 patients ont été analysés : 74 dans le groupe qui a eu une tumorectomie et 71 dans celui qui a eu une exérèse totale du mésorectum. Dans le groupe qui a eu une tumorectomie, 26 patients ont eu une l’exérèse totale du mésorectum complémentaire. En intention de traiter, il n’y avait pas de différence entre les deux groupes pour tous les composants du critère composite. Cependant, les analyses complémentaires sur la morbidité chirurgicale (12 % contre 22 % contre 46 %, p = 0,003) et les effets secondaires (19 % contre 30 % contre 59 %, p = 0,001) ont montré l’influence du type de chirurgie, tumorectomie contre l’exérèse totale du mésorectum contre l’exérèse totale du mésorectum complémentaire, respectivement. L’analyse des résultats histopathologiques a montré 61 % de bonnes réponses (ypT0-1) ainsi qu’une association entre les réponses ganglionnaire et tumorale avec 0, 0, 8 % et 40 % de tumeurs ypN1 pour respectivement les tumeurs ypT0, T1, T2 et T3. À 3 ans, il n’y avait pas de différence significative de taux de récidive locale (5,4 % contre 5,6 %) ni de probabilité de survie sans maladie (78,3 % contre 76,1 %). Conclusion L’étude n’a pas montré la supériorité de la tumorectomie par comparaison à la l’exérèse totale du mésorectum, en raison d’un taux élevé d’exérèse totale du mésorectum complémentaires, responsable d’une augmentation de la morbidité et des effets secondaires dans le groupe qui a eu une tumorectomie. Une meilleure sélection des patients, en évitant les exérèses totales du mésorectum non nécessaires, pourrait améliorer la stratégie. [ABSTRACT FROM AUTHOR]
