Background Glenoid loosening, a common complication of shoulder arthroplasty, could relate to implant design and bone quality. However, the role of bone density has not been tested experimentally yet. In this study, tests on cadaveric specimens of varying bone density were performed to evaluate the effects of bone quality on loosening of typical anatomic glenoid implants. Methods Cadaveric scapulae scanned with a quantitative computed tomography scanner to determine bone mineral density (BMD) were implanted with either pegged or keeled cemented glenoid components and tested under constant glenohumeral load while a humeral head component was moved cyclically in the inferior and superior directions. Implant superior and inferior edge lifting, defined as displacement from the underlying bone, was measured with linear variable differential transducers until we reached 23,000 test cycles, and statistical testing was performed for differences in edge lifting due to implant design and related to periprosthetic BMD. Results Edge lifting was statistically significant at all time points, but on average, implant design had no effect. Lifting was highest in specimens in which BMD below the lifting edge was lower, with trends of increased displacement with decreased BMD. Conclusions Implant lifting was greater in glenoids of lower bone density for both implant designs. This finding suggests that fixation failure will most likely occur in bone of lower density and that the fixation design itself may play a secondary role. [ABSTRACT FROM AUTHOR]

Background Failure after rotator cuff repair remains a major clinical problem and could be related to excessive pressures from the acromion. Previous studies with irreparable tears showed good clinical results of tendon healing with arthroscopic insertion of a protective biodegradable spacer balloon between the repaired tendon and the acromion. One hypothesis is that compression pressures on the repaired tendon will be reduced by the spacer. This cadaver study aimed to investigate the effects of this subacromial spacer on compression pressures over a repaired supraspinatus tendon in passive motion. Methods Rotator cuff tear and repair were performed in six fresh-frozen cadaveric shoulders, followed by insertion of a biodegradable subacromial spacer. Specimens were tested using a passive shoulder simulator for abduction-adduction, flexion-extension and internal-external rotations. A sensor positioned below the acromion was used to measure compression pressure changes through passive range of motion before and after placement of a subacromial spacer. Peak pressures were measured in adduction-abduction motion, near 90° abduction. Findings Both the mean and peak pressures in abduction-adduction were significantly reduced after insertion of the subacromial spacer (from mean 121.7 (SD 9.5) MPa to 51.5 (SD 1.2) MPa and from peak 1749.6 (SD 80.7) MPa to 535.1 (SD 27.6) MPa) (P < 0.0001). Interpretation The reduced peak pressures and wider load distributions over the sensor during both passive abduction-adduction and flexion-extension motions suggest that the use of the spacer will lead to reduced wear of the repair in patients, and potentially prevent rotator cuff re-tear after surgical repair. Glenoid loosening is still a main complication for shoulder arthroplasty. We hypothesize that cement and bone stresses potentially leading to fixation failure are related not only to glenohumeral conformity, fixation design or eccentric loading, but also to bone volume fraction, cortical thickness and degree of anisotropy in the glenoid. In this study, periprosthetic bone and cement stresses were computed with micro finite element models of the replaced glenoid depicting realistic bone microstructure. These models were used to quantify potential effects of bone microstructural parameters under loading conditions simulating different levels of glenohumeral conformity and eccentric loading simulating glenohumeral instability. Results show that peak cement stresses were achieved near the cement–bone interface in all loading schemes. Higher stresses within trabecular bone tissue and cement mantle were obtained within specimens of lower bone volume fraction and in regions of low anisotropy, increasing with decreasing glenohumeral conformity and reaching their maxima below the keeled design when the load is shifted superiorly. Our analyses confirm the combined influences of eccentric load shifts with reduced bone volume fraction and anisotropy on increasing periprosthetic stresses. They finally suggest that improving fixation of glenoid replacements must reduce internal cement and bone tissue stresses, in particular in glenoids of low bone density and heterogeneity. Abstract: Total Knee Arthroplasty (TKA) is a very successful surgical procedure but clinical outcomes were reported to be affected by implant design, ligament balancing, alignment or patient-related anatomical factors. It was recently demonstrated that malpositioning of the TKA components and patient related anatomical factors can considerably alter tibio-femoral (TF) and patellofemoral maximum contact forces. However, up to now, how a component malpositioning and different soft-tissue anatomy changes TF knee kinematics was not yet fully investigated. The goal of this study was to evaluate how sensitive TF kinematics are to these factors during a simulated loaded squat for different TKA designs. Four TKA types (a fixed bearing, posterior stabilized prosthesis; a high flexion fixed bearing guided motion prosthesis; a mobile bearing prosthesis and a hinge prosthesis) were virtually implanted on the same virtual cadaver leg model which underwent a loaded squat between 0° and 120°. The reference models were then modified to simulate either component malpositioning in several directions or changes in ligaments geometry by change in the collateral ligament insertions. The results showed that, for all implant designs, TF kinematics were affected by changes in implant positioning and anatomical factors. While the ranges of motion predicted for all tested configurations were generally similar to the reference configuration for each type of TKA, the modifications resulted in shifts in the maximum and minimum values for the TF rotations and translations. Abstract: Previous antiresorptive treatment may influence the anabolic response to teriparatide. The OPTAMISE (Open-label Study to Determine How Prior Therapy with Alendronate or Risedronate in Postmenopausal Women with Osteoporosis Influences the Clinical Effectiveness of Teriparatide) study reported greater increases in biochemical markers of bone turnover and volumetric bone mineral density (BMD) when 12 months of teriparatide treatment was preceded by 2 years or more of risedronate versus alendronate treatment. The objective of this study was to use quantitative computed tomography (CT)-based nonlinear finite element modeling to evaluate how prior therapy with alendronate or risedronate in postmenopausal women with osteoporosis influences the biomechanical effectiveness of teriparatide. Finite element models of the L1 vertebra were created from quantitative CT scans, acquired before and after 12 months of therapy with teriparatide, from 171 patients from the OPTAMISE study. These models were subjected to uniaxial compression. Total BMD-derived bone volume fraction (BV/TVd, i.e., bone volume [BV]/total volume [TV]), estimated from quantitative CT-based volumetric BMD, vertebral stiffness, and failure load (strength) were calculated for each time measurement point. The results of this study demonstrated that 12 months of treatment with teriparatide following prior treatment with either risedronate or alendronate increased BMD-derived BV/TVd, the predicted vertebral stiffness, and failure load. However, the effects of teriparatide were more pronounced in patients treated previously with risedronate, which is consistent with the findings of the OPTAMISE study. The mean (±standard error) increase in stiffness was greater in the prior risedronate group than the prior alendronate group (24.6±3.2% versus 14.4±2.8%, respectively; p =0.0073). Similarly, vertebral failure load increased by 27.2±3.5% in the prior risedronate group versus 15.3±3.1% in the prior alendronate group (p =0.0042). The mechanical variables increased in greater proportion than BV/TVd, which increased by 6.9±0.9% versus 4.6±0.8% in the prior-risedronate and prior-alendronate groups, respectively (p =0.0290). Our finding indicated that while teriparatide can be used with success on patients who have previously undergone treatment with risedronate and alendronate, it demonstrated greater anabolic effect on biomechanical properties in prior-risedronate patients in the first year of teriparatide treatment. Abstract: Assessment of the mechanical properties of trabecular bone is of major biological and clinical importance for the investigation of bone diseases, fractures and their treatments. Finite element (FE) methods are getting increasingly popular for quantifying the elastic and failure properties of trabecular bone. In particular, voxel-based FE methods have been previously used to calculate the effective elastic properties of trabecular microstructures. However, in most studies, bone tissue moduli were assumed or back-calculated to match the apparent elastic moduli from experiments, which often lead to surprisingly low values when compared to nanoindentation results. In this study, voxel-based FE analysis of trabecular bone is combined with physical measures of volume fraction, micro-CT (μCT) reconstructions, uniaxial mechanical tests and specimen-specific nanoindentation tests for proper validation of the method. Cylindrical specimens of cancellous bone were extracted from human femurs and their volume fraction determined with Archimede''s method. Uniaxial apparent modulus of the specimens was measured with an improved tension–compression testing protocol that minimizes boundary artefacts. Their μCT reconstructions were segmented to match the measured bone volume fraction and used to create full-size voxel models with 30–45μm element size. For each specimen, linear isotropic elastic material properties were defined based on specific nanoindentation measurements of its embedded bone tissue. Linear FE analyses were finally performed to simulate the uniaxial mechanical tests. Additional parametric analyses were performed to evaluate the potential errors on the predicted apparent modulus arising from variations in segmentation threshold, tissue modulus, and the use of 125-mm3 cubic sub-regions. The results demonstrate an excellent correspondence between experimental measures and FE predictions of uniaxial apparent modulus. In conclusion, the adopted voxel-based FE approach is found to be a robust method to predict the linear elastic properties of human cancellous bone, provided segmentation of the μCT reconstructions is carefully calibrated, tissue modulus is known a priori and the entire region of interest is included in the analysis. Ancillary evaluation of spinal Dual-energy X-ray Absorptiometry (DXA) via variogram-based texture evaluation (e.g., Trabecular Bone Score) is used for improving the fracture risk assessment, despite no proven relationship with vertebral strength. The purpose of this study was thus to determine whether classical variogram-based parameters (sill variance and correlation length) evaluated from simulated DXA scans could help predicting the in vitro vertebral strength. Experimental data of thirteen human full vertebrae (i.e., with posterior elements) and twelve vertebral bodies were obtained from two existing studies. Areal bone mineral density (aBMD) was calculated from 2D projection images of the 3D HR-pQCT scan of the specimens mimicking clinical DXA scans. Stochastic predictors, sill variance and correlation length, were calculated from their experimental variogram. Vertebral strength was measured as the maximum failure load of human vertebrae and vertebral bodies from mechanical tests. Vertebral strength correlated significantly with sill variance (r = 0.727) and correlation length (r = 0.727) for the vertebral bodies, and with correlation length (r = 0.593) for full vertebrae. However, the stochastic predictors improved the strength prediction made by aBMD alone by only 11% for the vertebral bodies while no improvement was observed for the full vertebrae. Despite a correlation, classical variogram parameters such as sill variance and correlation length do not enhance the prediction of in vitro vertebral strength beyond aBMD.
