Background: The single-arm, phase II Tasigna Efficacy in Advanced Melanoma (TEAM) trial evaluated the KIT-selective tyrosine kinase inhibitor nilotinib in patients with KIT-mutated advanced melanoma without prior KIT inhibitor treatment. Patients and methods: Forty-two patients with KIT-mutated advanced melanoma were enrolled and treated with nilotinib 400mg twice daily. TEAM originally included a comparator arm of dacarbazine (DTIC)-treated patients; the design was amended to a single-arm trial due to an observed low number of KIT-mutated melanomas. Thirteen patients were randomized to DTIC before the protocol amendment removing this study arm. The primary endpoint was objective response rate (ORR), determined according to Response Evaluation Criteria In Solid Tumors. Results: ORR was 26.2% (n=11/42; 95% CI, 13.9%-42.0%), sufficient to reject the null hypothesis (ORR ≤10%). All observed responses were partial responses (PRs; median response duration, 7.1 months). Twenty patients (47.6%) had stable disease and 10 (23.8%) had progressive disease; 1 (2.4%) response was unknown. Ten of the 11 responding patients had exon 11 mutations, four with an L576P mutation. The median progression-free survival and overall survival were 4.2 and 18.0 months, respectively. Three of the 13 patients on DTIC achieved a PR, and another patient had a PR following switch to nilotinib. Conclusion: Nilotinib activity in patients with advanced KIT-mutated melanoma was similar to historical data from imatinib-treated patients. DTIC treatment showed potential activity, although the low patient number limits interpretation. Similar to previously reported results with imatinib, nilotinib showed greater activity among patients with an exon 11 mutation, including L576P, suggesting that nilotinib may be an effective treatment option for patients with specific KIT mutations. Abstract: Alginate films with different degrees of crosslinking obtained by internal gelation, and alginate films incorporated with oregano essential oil (OEO) were prepared. The impact of the degree of crosslinking caused by the use of calcium carbonate as crosslinking agent and the incorporation of OEO into the alginate films on their antibacterial, optical, mechanical, microstructural and water vapour barrier properties was evaluated. An increase in the degree of crosslinking produced alginate films that were significantly thicker (0.031–0.038mm) and stronger (51.9–52.9MPa) but less elastic (2.3%) than those non-crosslinked films (0.029mm; 39.7MPa; 4.4%). The water vapour permeability (WVP) of the films decreased significantly only with the highest level of crosslinking. The incorporation of OEO in alginate films affected significantly their physical properties. Thickness and percent elongation at break of the films were increased by the addition of OEO (0.036–0.042mm and 2.7–3.7%), while the tensile strength and water vapour permeability decreased (31.1–55.5MPa and 2.7–3.0×10−9 g/msPa). Films incorporated with OEO were more effective against Gram-positive bacteria (Staphylococcus aureus and Listeria monocytogenes) than Gram-negative bacteria (Escherichia coli and Salmonella Enteritidis). A minimum concentration of 1.0% of OEO was necessary to ensure their antibacterial efficacy. [Copyright &y& Elsevier]

Objectifs L’IRM est utilisée pour le bilan standard d’extension des cancers prostatiques. Une stratégie d’escalade de dose ciblée spécifiquement dans la tumeur prostatique « dominante » implique en particulier une segmentation particulièrement délicate de la tumeur à partir de cette même IRM. L’objectif de cette étude était de déterminer et d’évaluer une méthode de segmentation automatique des cancers prostatiques à partir d’une IRM multiparamétrique. Patients et méthodes Un total de 34 patients atteints de cancer prostatique ont eu une IRM prostatique préthérapeutique 3,0 T multiparamétrique (T2, ADC [coefficient de diffusion apparente] et perfusion). Deux médecins experts ont déliné indépendamment les zones tumorales. Les caractéristiques de chaque séquence ont ensuite été extraites et analysées par voxel en utilisant la méthode mRMR (minimum de redondance maximum de relevance) et le classifieur SVM (machine à vecteurs de support) permettant une segmentation automatique de la tumeur. La validation du modèle de segmentation tumorale a été effectuée en comparant cette segmentation automatique à celle effectuée par les experts, par méthode « leave-one-out », à partir de 20 patients supplémentaires. La performance du modèle a finalement été évaluée en mesurant l’aire sous la courbe ROC ( receiver operating characteristic , AUC). Résultats La meilleure méthode de segmentation utilisait conjointement 18 descripteurs spécifiques extraits des séquences T2, ADC et de perfusion, portant sur des tumeurs localisées dans la zone périphérique et dans la zone transitionnelle. L’étude de validation a montré que la tumeur prostatique était dans tous les cas détectée, avec un AUC ROC de 0,60 et 0,91, pour chacune des zones respectivement. Conclusion Cette étude présente une nouvelle méthode de segmentation tumorale automatique du cancer prostatique à partir d’une IRM multiparamétrique. Objectif de l’étude Déterminer la valeur prédictive de paramètres quantitatifs issus d’une IRM sur la récidive biochimique après radiothérapie externe de cancers prostatiques de haut risque selon d’Amico. Matériel et méthode Quarante-huit patients atteints de cancer prostatique localisé de haut risque ont eu une IRM prostatique avant la radiothérapie comprenant les trois séquences standard, T2, diffusion et perfusion. Les patients ont reçu une RCMI guidée par l’image de 78–80 Gy et une hormonothérapie d’une durée de trois ans. Au total, 144 caractéristiques ont été extraites de l’IRM, correspondant aux paramètres de texture de Haralick ( n = 130), à des paramètres géométriques ( n = 4) et à des paramètres statistiques de premier ordre et dérivés de deux gradients ( n = 10). Le suivi médian était de 47 mois et une récidive biochimique a été diagnostiquée chez six patients. La valeur prédictive des 144 paramètres quantitatifs issus de l’IRM ainsi que des facteurs pronostiques standards (stade T, score de Gleason et PSA sérique) sur la récidive biologique a été recherchée par modèle de Cox (analyse unifactorielle) et par méthode de machine learning ( random survival forests , RSF). La capacité de prédiction des paramètres était évaluée par le C-index. Résultats Les quatre caractéristiques géométriques, le statut ganglionnaire et 19 caractéristiques de Haralick uniquement sur T2 étaient significativement prédictifs de la récidive biochimique en analyse unifactorielle, avec un C-index allant de 0,69 à 0,85. L’analyse en RSF a permis de générer un modèle hautement prédictif de la récidive biochimique comportant sept paramètres (volume tumoral, score de Gleason et cinq paramètres de Haralick) conduisant à un C-index de 0,82. Conclusion Cette étude est la première, à notre connaissance, à montrer la valeur prédictive de paramètres quantitatifs issus d’une IRM sur la récidive biochimique après radiothérapie de cancers prostatiques de haut risque. We have performed a geomorphic analysis of the Sierra Nevada, the highest range of the Betic Cordillera (SE Spain), with the aim to elucidate its late Miocene to present-day exhumation history. The qualitative and quantitative analysis is based on filtered topography, local relief, swath-profile analysis, anomalies on stream orientation, bulk erosion volume, hypsometry, and steepness index (k sn ). All these parameters are intimately linked to river incision and development of drainage pattern, having been calculated to assess the role of folding and faulting on the evolution of the Sierra Nevada. Moreover, uplift rates in the core of the Sierra Nevada have been deduced from an extrapolation of the position of Late Tortonian to Pliocene coastline deposits. These data have been compared to apatite (U–Th)/He, fission-track and 10 Be cosmogenic data from SE Spain in order to evaluate the consistency among uplift, thermal histories and denudation rates. Our preferred tectonic scenario is one that favors fast exhumation of the western Sierra Nevada in a NW–SE overall compressive setting produced by the convergence between the Nubia and Africa plates. Sub-perpendicular to this compression, a westward 4 mm/year extensional hanging-wall displacement promotes uplift and unroofing of the western part of Sierra Nevada. [ABSTRACT FROM AUTHOR]
