Abstract

There are limited data documenting the safety and clinical outcomes following outpatient total shoulder arthroplasty (TSA) and reverse TSA (RTSA) compared to inpatient shoulder arthroplasty. It was hypothesized that there would be no significant differences in complication and emergency room (ER) visit rate between outpatient TSA and RTSA. A prospectively maintained institutional registry was retrospectively queried for all patients undergoing primary TSA or RTSA between September 2016 and April 2019 and confirmed as having undergone outpatient surgery, defined as surgery performed at an ambulatory surgery center. Ninety-day complications, readmissions, and ER visits were analyzed for outpatient TSA and RTSA. The 90-day complications included infection, thromboembolic disease (pulmonary embolism, deep venous thrombosis), neurovascular injury, revision surgery, and inflammatory processes. A total of 140 patients were identified who underwent outpatient TSA or RTSA from 2016 to 2019. Four complications (3.7%) were reported in the TSA cohort, compared to 4 complications (12.9%) reported in the RTSA cohort (P =.060). Three ER visits occurred in the RTSA group compared to 0 in the TSA group (P =.001). Patients who experienced a complication had significantly higher body mass index (TSA: P =.049, RTSA: P =.035). In addition, there were no readmissions or reoperations in either TSA or RTSA patients. Overall, the rate of complications, ER visits, and readmission rates in outpatient TSA and RTSA was found to be relatively low given the sample size of this study and minor complications reported. In addition, there were no readmissions or reoperations in either TSA or RTSA patients. Elevated body mass index may significantly increase the risk of complications after both a TSA and RTSA. 45. Effects of high frequency strengthening on pain sensitivity and function in female runners with chronic patellofemoral pain.

Abstract

To investigate the effects of a high frequency strengthening program on function, pain, and pain sensitization in female runners with chronic patellofemoral pain (PFP). Cross-sectional study. University laboratory. Thirty female runners (mean age 32 ± 8.1 years) with chronic PFP completed an 8-week home strengthening program. Variables assessed at baseline, 8-weeks, and 12 weeks included single leg step down test (SLSD), pain, Anterior Knee Pain Scale (AKPS), University of Wisconsin Running Injury and Recovery Index (UWRI), and quantitative sensory testing. There was large and statistically significant improvement at 8 and 12 weeks for average knee pain (η p 2 = 0.334, p < 0.001), worst knee pain (η p 2 = 0.351, p < 0.001), SLSD (η p 2 = 0.161, p = 0.001), AKPS (η p 2 = 0.463, p < 0.001), and UWRI (η p 2 = 0.366, p < 0.001). A medium to large effect and statistically significant improvement in pressure pain threshold testing was found for all local and remote structures (η p 2 range, 0.110 to 0.293, range p < 0.001 to p = 0.009) at 8 and 12 weeks. There was a significant decrease in local and remote hyperalgesia via mechanical and thermal pain sensitivity testing in female runners with chronic PFP. There was a large effect and significant improvement in self-reported pain and function. • Improved pain with high frequency strengthening in female runners with chronic PFP. • Primary and secondary hyperalgesia improved with strengthening at 8 and 12 weeks. • No change for measures of central pain facilitation and central pain inhibition. 46. Partial pressure of carbon dioxide/pH interaction and its association with mortality among patients mechanically ventilated in the emergency department.

Abstract

There is currently conflicting data as to the effects of hypercapnia on clinical outcomes among mechanically ventilated patients in the emergency department (ED). These conflicting results may be explained by the degree of acidosis. We sought to test the hypothesis that hypercapnia is associated with increased in-hospital mortality and decreased ventilator-free days at lower pH, but associated with decreased in-hospital mortality and increased ventilator-free days at higher pH, among patients requiring mechanical ventilation in the emergency department (ED). Secondary analysis of patient level data from prior clinical trials and cohort studies that enrolled adult patients who required mechanical ventilation in the ED. Patients who had a documented blood gas while on mechanical ventilation in the ED were included in these analyses. The primary outcome was in-hospital mortality, and secondary outcome was ventilator-free days. Mixed-effects logistic, linear, and survival-time regression models were used to test if pH modified the association between partial pressure of carbon dioxide (pCO 2) and outcome measures. Of the 2348 subjects included, the median [interquartile range (IQR)] pCO2 was 43 (35–54) and pH was 7.31 (7.22–7.39). Overall, in-hospital mortality was 27%. We found pH modified the association between pCO 2 and outcomes, with higher pCO 2 associated with increased probability of in-hospital mortality when pH is below 7.00, and decreased probability of in-hospital mortality when pH is above 7.10. These results remained consistent across multiple sensitivity and subgroup analyses. A similar relationship was found with ventilator-free days. Higher pCO 2 is associated with decreased mortality and greater ventilator-free days when pH is >7.10; however, it is associated with increased mortality and fewer ventilator-free days when the pH is below 7.00. Targeting pCO 2 based on pH in the ED may be a potential intervention target for future clinical trials to improve clinical outcomes. [ABSTRACT FROM AUTHOR]
