Abstract: Objective: The purpose of this study was to investigate the effect of hip adduction on the activity of the Vastus Medialis Obliquus (VMO) and Vastus Lateralis Longus (VLL) muscles during semisquat exercises. Methods: Twenty female subjects, divided into two groups comprising healthy and patellofemoral pain syndrome (PFPS) subjects (ten volunteers for each group), performed three double-leg semisquat exercise trials with maximum hip adduction isometric contraction (DLSS-HA) and three double-leg semisquat exercise trials without hip adduction (DLSS). The normalized electromyographic muscle data were analysed using Repeated Measure ANOVA (p ⩽0.05). Results: The electrical activity of both VMO and VLL muscles was significantly greater during DLSS-HA exercise than during DLSS (p =0.0002) for both groups. Additionally, an independent Repeated Measure ANOVA revealed that the electric activity of the VLL muscle was significantly greater (p =0.0149) than that of the VMO muscle during DLSS exercises only for the PFPS group. However, no differences were found during DLSS-HA exercises. Conclusions: Although there was no preferential VMO muscle activation, the association of hip adduction with squat exercise promoted a greater balance between the medial and lateral portions of the quadriceps femoris muscle and could be indicated for the conservatory treatment of PFPS patients. The association of isometric hip adduction with isometric semisquat exercises produced a more overall quadriceps activity and could be indicated for clinical rehabilitation or muscle strengthening programs. Background: Under a typical light and sound environment context, individuals with migraine showed balance control deficits on a series of functional activities, which helps to explain why migraineurs report more falls. it isn't established, the effects of intensity light and sound in migraineurs during functional tasks.Research Question: Based on the hypersensitivity to light and sound in migraineurs, not only during the attack but also in the interictal period, does the exposure to bright light and loud sound impact motor control in this population?Methods: This cross-sectional study consisted of 51 women with migraine and 22 healthy women. They performed three walking tasks: crossing an obstacle, stepping-up and stepping-down a curb, in a control situation with ambient lighting (≅350 lux), bright light (≅1200 lux), and loud sound (≅90 dBa). For statistical analysis, a t-test, a Spearman correlation test, and a repeated measures mixed ANOVA were applied.Results: Migraineurs presented higher discomfort induced by light (p ≤ 0.0001) and sound (p = 0.001). In the obstacle task, migraineurs had greater step width than controls in the ambient light condition (p = 0.038) and participants of both groups placed their leading foot farther away from the obstacle in the light (p = 0.033) than in the ambient light condition. For the step-up task, this distance increased for both groups and limbs in the light (leading limb: p = 0.015; trailing limb: p = 0.002) and sound (leading limb: p = 0.010; trailing limb: p ≤ 0.0001) conditions compared to the ambient light condition. Step speed increased for light and sound conditions compared to ambient light condition, except for the sound condition in the step-down task.Significance: Despite the higher discomfort induced by light and sound in the migraineurs, the effects of these sensory manipulations were similar for both migraineurs and controls, except for step width. • The three versions of the Tampa scale for Kinesiophobia (TSK) questionnaire tested in our study showed good measurement properties. • The three TSK versions are suitable for assessing maladaptive beliefs in patients with migraine. • The TSK-11 offers the advantage of being shorter than the TSK-13 and TSK-17. • Scores on the TSK showed a moderate correlation with those on the fear avoidance beliefs, pain catastrophizing scale, and headache impact test questionnaires. The Tampa Scale for Kinesiophobia (TSK) is one of the most frequently employed instruments for assessing maladaptive beliefs about pain, injury, and movement in patients with chronic musculoskeletal pain. However, the measurement properties of this tool have so far not been tested for individuals with migraine. To evaluate the structural, construct, and criterion validity, and the internal consistency for three versions (TSK-11, TSK-13, and TSK-17) of the TSK for patients with migraine. A total of 113 individuals aged between 18 and 55 years old with migraine diagnosis were included. All participants completed the TSK with 17 items, the Fear-Avoidance Beliefs Questionnaire, the Headache Impact Test, and the Pain Catastrophizing Scale questionnaires. Confirmatory factor analysis was used to assess the structural validity of the TSK, and Cronbach's α was used to assess internal consistency. For construct and criterion validity, the Spearman's correlation was calculated. The TSK structure with one factor and the 17, 13, or 11 items versions were suitable, with suitable values in all fit indices related to structural validity. The three versions showed acceptable internal consistency (α = 0.75). All TSK versions showed moderate positive correlation with the other questionnaires (rho range= 0.31–0.63), confirming most of the predefined hypothesis for the construct validity. Also, the criterion validity of the 13-item and 11-item versions was confirmed (rho=0.95 and rho=0.94, respectively). • The CCFT has good intra-examiner reliability in patients with migraine. • The CCFT has moderate inter-rater reliability in patients with migraine. • Headache disability and cervical flexors endurance are correlated with CCFT performance. • The SEM (1.31 and 1.36 mmHg) are smaller than a CCFT stage. • The MDC (3.63 and 3.77 mmHg) correspond to two CCFT stages. Migraine may be accompanied by several cervical musculoskeletal dysfunctions, for example an altered performance on the Craniocervical Flexion Test (CCFT). However, CCFT measurement properties are still unclear in patients with migraine. To determine intra- and inter-examiner reliability, construct validity, standard measurement error (SEM), and minimal detectable change (MDC) of the CCFT in patients with migraine. Women diagnosed with migraine were considered eligible for this study. Participants were assessed by two examiners for the inter-examiner reliability, and with 7–10 days interval for the intra-examiner reliability. Construct validity was assessed considering headache and neck pain frequency and intensity, and self-reported questionnaires, including the Headache Impact Test – 6 items (HIT-6), the 12-item Allodynia Symptom Checklist/Brazil (ASC-12), and the Neck Disability Index (NDI). In addition, participants performed cervical endurance and maximal voluntary isometric contraction of the cervical flexors. A total of 103 women with migraine were recruited. The intra-examiner reliability was rated as good (ICC= 0.81, 95% CI: 0.73, 0.87), while the inter-examiner reliability was rated as moderate (ICC= 0.55, 95% CI: 0.40, 0.67). The intra and inter-examiner SEM were 1.31 and 1.36 mmHg respectively, and MDC were 3.63 and 3.77 mmHg. The HIT-6 and the cervical endurance flexion test were associated with the CCFT in a multiple linear regression model (p = 0.004, R = 0.35). The CCFT presents adequate intra- and inter-examiner reliability. Better performance on the CCFT test was associated with better HIT-6 scores and greater cervical endurance time, which was not influenced by the presence of neck pain. [ABSTRACT FROM AUTHOR]

Migraine is a primary headache with high levels of associated disability that can be related to a variety of symptoms and comorbidities. The role of physical therapy in the management of migraine is largely unknown. Therefore, the aim of this review is to highlight and critically discuss the current literature and evidence for physical therapy interventions in individuals with migraines. A narrative review of the literature was performed. Physical therapists assessing and treating patients with migraine should focus on two primary aspects: (1) musculoskeletal dysfunctions, and (2) vestibular symptoms/postural control impairment. Signs and symptoms of musculoskeletal and/or vestibular dysfunctions are prevalent among individuals with migraines and different disability levels can be observed depending on the presence of aura or increment of the migraine attacks. A proper physical examination and interview of the patients will lead to a tailored treatment plan. The primary aim regarding musculoskeletal dysfunctions is to reduce pain and sensitization, and physical therapy interventions may include a combination of manual therapy, exercise therapy, and education. The aim regarding postural control impairment is to optimize function and reduce vestibular symptoms, and interventions should include balance exercises and vestibular rehabilitation. Objective This study investigated differences in surface electromyography (sEMG) activity of the masseter and anterior temporalis muscles in children with and without temporomandibular disorders (TMD). Participants Thirty-four children aged 8–12 years were recruited, comprising 17 children with TMD and 17 without TMD (control group [CG]). The children were quasi-matched for sex, age, weight, and height. sEMG data were obtained using Myosystem ® Br-1 equipment with 12 channels to evaluate the bilateral masseter, anterior temporalis, and suprahyoid muscles. For sEMG analysis, raw and normalized root mean square (RMS) values were obtained at rest and during maximum clenching. The sEMG ratios of the raw RMS data of the bilateral masseter in relation to the anterior temporalis muscles (sEMG-M/AT ratio) were also assessed. Mann-Whitney tests (p ≤ 0.05) were used to compare sEMG ratio between TMD group and CG. Results Significant prevalences of pain during chewing (53% vs. 0%, X 2 = 5.87, p = 0.01), TMJ pain (58% vs. 0%, X 2 = 6.67, p = 0.01), neck pain (58% vs. 18%, X 2 = 3.77, p = 0.05) and pain in the temples (47% vs. 6%, X 2 = 5.44, p = 0.02) were identified in the TMD group compared to CG. Our results revealed lower sEMG-M/AT ratios during maximum clenching (p = 0.01) in children with TMD compared to those in the asymptomatic CG. Conclusion The results showed that children with TMD preferentially used their temporalis muscles during maximum voluntary clenching, probably as a consequence of nociceptive inputs in order to obtain pain relief. The current study aimed to investigate differences in activity of neck flexor and extensor muscles in women with migraine considering the chronicity of their condition. Thirty-one subjects with episodic migraine, 21 with chronic migraine and 31 healthy controls participated. Surface electromyography signals were recorded bilaterally from the sternocleidomastoid, anterior scalene, splenius capitis and upper trapezius muscles as subjects performed 5 stages of cranio-cervical flexion (CCF), representing a progressive increase in range of CCF motion. Comparison of normalized root-mean-square among groups was conducted with 3×5 ANCOVA with task level as the within-subject variable, group as the between-subject variable, and the presence of neck pain and disability as co-variates. The group with chronic migraine exhibited increased activity of their extensor muscles compared to the control and episodic migraine groups (splenius capitis: F=3.149, P=0.045; upper trapezius: F=3.369, P=0.041). No significant between-group differences were found for the superficial neck flexors (sternocleidomastoid: F=1.161, P=0.320; anterior scalene: F=0.135, P=0.874). In conclusion, women with chronic migraine exhibit increased activity of their superficial neck extensor muscles when acting as antagonists during low-load isometric CCF contractions in comparison to non-headache subjects. Objectives: The aim of this study was to assess the prevalence of Temporomandibular Disorders (TMD), using the Research Diagnostic Criteria for TMD (RDC/TMD) in women with episodic and chronic migraine (M and CM), as well as in asymptomatic women. Method: Sample consisted of 61 women, being 38 with M and 23 with CM, identified from a headache outpatient center; we also investigated 30 women without headaches for at least 3 months (women without headache group - WHG). Assessment of TMD was conducted by a physical therapist who was blind to the headache status. Results: The prevalence of TMD, assessed through the RDC, was 33.3% in the WHG, 86.8% in the M group and 91.3% of the CM group. Differences were significant when comparing M and CM groups with WHG (p<0.001), but not when comparing M and CM (p>0.05) as well as higher risk for TMD [odds ratio (OR)=3.15, 95% confidence interval (CI) 1.73-5.71 and OR=3.97, 95%CI 1.76-8.94]. Conclusion: Women with migraine are more likely to have muscular and articular TMD, suggesting that both disorders might be clinically associated, which demonstrate the importance of physical therapy assessment in the multidisciplinary team. Background: Hip and knee muscle weaknesses have been associated with the onset of anterior knee pain (AKP). Therefore, the understanding of how squats exercises can be performed in order to optimize the electrical activity of these muscles is relevant for physical therapy treatments. Objective: To compare the electromyographic activity of patella and pelvic stabilizers during traditional squat and squat associated with isometric hip adduction or abduction in subjects without AKP. Methods: Electromyography signals were captured using double-differential electrodes at the vastus medialis obliquus (VMO), vastus lateralis obliquus (VLO), vastus lateralis longus (VLL) and gluteus medium (GMed) in 15 healthy and sedentary women during squats exercises: traditional and associated with hip adduction and hip abduction with load of 25% of body weight. Linear mixed models with significance level of 5% were used for data analysis. Results: Squat associated with hip adduction and abduction produced electromyographic activity of GMed of 0.47 (0.2) and 0.59 (0.22) respectively, while conventional squat produced an electromyiographic activity of 0.33 (0.27). The higher VMO activity was 0.59 (0.27) during the isometric contraction in the squat associated with hip adduction. The higher VLO activity was 0.60 (0.32) during isometric contraction in the squat associated with hip abduction. Abstract: Patella stabilizer muscle response and patellar kinematics were evaluated in 19 women with anterior knee pain (AKP) and 20 healthy women during maximum voluntary isometric contraction (MVIC) with the knee positioned at 15°, 30° and 45° flexion during open (OKC) and closed (CKC) kinetic chain exercises. Patellar kinematics was evaluated through patellar tilt and displacement, and the electrical activity of patellar stabilizers through the root mean square normalized during MVIC and OKC with the knee at 90° flexion. Data revealed that the vastus medialis oblique muscle (VMO) was more active in the control group compared to the AKP group during OKC exercises with the knee at 45° flexion. However, no difference in the patellar kinematics was observed between these groups; nevertheless, the correlation between these parameters also showed, with the knee at 45° flexion, that lateral patellar tilt increase was associated with a reduction in the activity of lateral patellar stabilizers in the control group and with an increase in the VMO activity in the AKP group. Background: Cervical range of motion (CROM) is a fundamental component of the functional evaluation in physical therapy interventions. The CROM device stands out as a reliable, non-invasive and easy-to-use method, but it is a very expensive tool. In clinical practice, more affordable tools such as Fleximeters are preferred. However, the reliability of Fleximeters for the cervical spine has not been adequately tested. Objectives: To compare the Fleximeters and the CROM device for the analysis of CROM, and to investigate the intra- and inter-examiner reliability of both tools. Methods: Cervical movements (flexion, extension, lateral flexion and rotation) were assessed in 20 asymptomatic young women by three examiners using both tools. The statistical analyses were performed using the intra-class correlation coefficient (ICC). Results: The agreement between the tools was considered moderate for flexion and left rotation (0.71; 0.58) and excellent for all of the other movements (0.76-0.87). The intra-examiner reliability for the CROM device was moderate for flexion and right rotation (0.70; 0.69) and excellent for all of the other movements (0.79-0.88). For the Fleximeter, the agreement was excellent for inclination and right rotation (0.80; 0.77) and moderate for all of the other movements (0.69-0.75). The inter-examiner reliability for the CROM device was excellent for all movements (0.76-0.93) and for the Fleximeter, it was moderate for right and left rotation (0.66; 0.75) and excellent for all of the other movements (0.81-0.88). Conclusions: There was agreement between the CROM assessments using the Fleximeter and the CROM device. Furthermore, both devices showed acceptable reliability for clinical practice. Abstract: The purpose of the present study was to evaluate the intra and interday reliability of surface electromyographic amplitude values of the scapular girdle muscles and upper limbs during 3 isometric closed kinetic chain exercises, involving upper limbs with the fixed distal segment extremity on stable base of support and on a Swiss ball (relatively unstable). Twenty healthy adults performed the exercises push-up, bench-press and wall-press with different effort levels (80% and 100% maximal load). Subjects performed three maximal voluntary contractions (MVC) in muscular testing position of each muscle to obtain a reference value for root mean square (RMS) normalization. Individuals were instructed to randomly perform three isometric contraction series, in which each exercise lasted 6s with a 2-min resting-period between series and exercises. Intra and interday reliabilities were calculated through the intraclass correlation coefficient (ICC 2.1), standard error of the measurement (SEM). Results indicated an excellent intraday reliability of electromyographic amplitude values (ICC⩾0.75). The interday reliability of normalized RMS values ranged between good and excellent (ICC 0.52–0.98). Finally, it is suggested that the reliability of normalized electromyographic amplitude values of the analyzed muscles present better values during exercises on a stable surface. However, load levels used during the exercises do not seem to have any influence on variability levels, possibly because the loads were quite similar. [Copyright &y& Elsevier]

Despite previous reports supporting cervical muscle weakness and altered motor control in migraine, the endurance under standardized submaximal loads has not been investigated. Therefore, this study aimed to assess the endurance and muscle activity of the cervical musculature during submaximal isometric contractions in women with migraine and those without headache. Cervical muscle endurance tests were performed for flexors and extensors at 25%, 50%, and 75% of the output force during maximal isometric contraction using the Multi-Cervical Rehabilitation Unit with customized biofeedback. Initial values and relative rates of changes in root mean square and median frequency were calculated using cervical muscle superficial electromyography. Women with chronic migraine presented significantly shorter flexor endurance time in all load tests than controls (25%, P =.001, 50%, P =.005; 75%, P =.013), while episodic migraine only differed from controls at 75% (P =.018). The frequency of neck pain and/or pain referred to the head after the endurance test was up 12% in the control group, 40% in the episodic migraine group and 68% of the chronic migraine group. Few differences between groups were observed in the electromyographic variables and none of them was related to a worse performance in the endurance tests. Cervical flexor endurance was reduced in women with chronic migraine when independent of the load, whereas it was reduced to 75% of the maximal force in those with episodic migraine. No difference in the electromyographic variables could be related to this reduced flexor endurance.
