La mort subite à un âge jeune est vécue comme un phénomène dramatique nécessitant la connaissance de son incidence. Notre étude a pour objectifs de décrire les particularités épidémiologiques de la mort subite cardiaque d'origine ischémique chez le sujet jeune dans la population d'étude et de discuter son implication dans la survenue du décès. Dans ce travail, le sujet jeune a été défini comme toute personne âgée de 1 an à 35 ans pour les sujets de sexe masculin, et jusqu'à 45 ans pour les sujets de sexe féminin. Il s'agit d'une étude descriptive, rétrospective, exhaustive, concernant 312 cas de mort subite chez le sujet jeune colligés au service de médecine légale de l'hôpital universitaire Fattouma Bourguiba de Monastir-Tunisie, durant une période de 23 ans. Les données recueillies ont été saisies et traitées à l'aide du logiciel SPSS 20. Le taux de significativité a été fixé à 0,05. Trente-deux cas de mort subite cardiaque d'origine ischémique ont été recensés. L'infarctus du myocarde (IDM) représentait la deuxième cause de mort subite du sujet jeune après la cardiomyopathie hypertrophique. La prédominance masculine était nette. La tranche d'âge la plus touchée était celle entre 25–45 ans. La mortalité par IDM est survenue plus fréquemment au repos. Une coronaropathie a été objectivée dans vingt-quatre cas (75 %). L'IDM est survenu sur des artères coronaires saines dans huit cas. Une anomalie du trajet, en particulier le pont myocardique, a été trouvée dans huit cas (25 %). Le « screening » toxicologique était négatif dans tous les cas. La prévention doit intéresser toute la population avec une éducation sur les moyens des mesures générales d'hygiène de vie et de prévention des facteurs de risques cardiovasculaires à long terme, ainsi que le dépistage clinique orienté au moindre signe d'appel. We aim to study the epidemiological characteristics of sudden cardiac ischemic death in young, and to discuss his involvement in the occurrence of death. We performed a retrospective cohort study using autopsy data from the department of forensic medicine of the University Hospital of Fattouma Bourguiba, Monastir-Tunisia. A review of all autopsies performed during 23 years was done. In each case, clinical information and circumstances of death were obtained. We have included all sudden death in persons aged between 1 year and 35 years for the male and from one year to 45 years for female. We collected 312 cases of sudden death during the studied period. The collected data were processed using SPSS 20. The significance level was set at 0.05. Thirty-two cases of cardiac ischemic sudden death have been collected. Myocardial infarction was the second cause of sudden death in young patients. There was a male predominance. The most affected subjects were aged between 25–45 years. The death occurred more frequently at rest. Coronary artery disease has been discovered in twenty-four cases (75%). The myocardial infarction occurred on healthy coronary arteries in eight cases. An anomalous course of coronary arteries, in particular myocardial bridging, was found in eight cases (25%). Toxicological screening was negative in all cases. Identifying epidemiological characteristics of sudden cardiac ischemic death in this population is important for guiding approaches to prevention that must be based on dietary hygienic measures and the control of cardiovascular risk factors. Distributions of current density (cd) in a 100 cm 2 single polymer fuel cell have been measured, leading to a 12 × 12 data array. For a freshly matured MEA, the dimensionless distributions have been shown to be of moderate uniformity, whatever the cell current, with average deviations of the dimensionless cd in the range 0.16–0.25 (i.e. cd measured deviated from 16 to 25% from the expected value). More thorough examination of the data acquired showed that the non-uniformity is likely due to maldistribution of the gas in the 23 parallel channels of the multiple serpentine pattern of the commercial bipolar plates. For the sake of comparison, the MEA – including the two GDL was submitted to ageing with a standard cycling protocol (FC_DLC) emulating transportation conditions for 500 h. Characterisation of the cell state of health afterwards revealed visible degradation of the GDL, whereas the catalytic layer and the membrane were little affected. In the aged cell, distributions were shown to be far more non-uniform, with deviations ranging from 0.30 to 0.50: the current distributions were then depending on the cell current density. Presumably, due to the GDL ageing, liquid water produced at high cd, could not be removed efficiently, resulting in very low cd’s near the outlet. [ABSTRACT FROM AUTHOR]

Résumé But de l’étude Décrire les particularités épidémiologiques de la mort subite d’origine cardiovasculaire dans notre population d’étude et préciser ses caractéristiques étiopathogéniques. Patients et méthode Étude rétrospective de type descriptif, concernant 361 cas de morts subites cardiovasculaires de l’adulte. Les corps ont fait l’objet d’une autopsie médico-légale au service de médecine légale de l’hôpital universitaire Fattouma Bourguiba de Monastir (Tunisie) durant une période de huit ans allant du premier janvier 2004 jusqu’au 31 décembre 2011. Résultats L’incidence de la mort subite cardiovasculaire était de 9/100 000 personnes. Une prédominance masculine a été notée pour les différentes tranches d’âge. L’âge moyen de notre population d’étude était de 55,75 ans. La mort subite coronaire occupait la première place de la mort subite (57,8 %). Les autres étiologies étaient respectivement la cardiomyopathie hypertrophique (4,7 %), l’insuffisance cardiaque (1,9 %), la dysplasie arythmogène du ventricule droit (2,8 %), les valvulopathies (2 %), la cardio-myo-péricardite (1,9 %), le kyste hydatique du cœur (0,8 %), la rupture d’anévrysme (2,5 %), l’embolie pulmonaire (1,9 %) et la dissection aortique (1,3 %). La mort subite cardiaque survenue sur les lieux du travail a intéressé 25 cas, posant un problème médico-légal d’imputabilité. Conclusion La mort subite cardiovasculaire constitue généralement l’évolution inéluctable d’une cardiopathie sous-jacente parfois méconnue. Plusieurs facteurs de risque interviennent. La mort subite cardiaque sur cœur sain ou la mort par arythmie est une entité importante qui sollicite l’intervention de plusieurs acteurs (médecin légiste, cardiologue, généticien, média…) en termes de prévention. Aims of the study To describe epidemiological aspects of sudden cardiovascular death and to specify the etiopathogenic characteristics. Patients and method Our study is retrospective and descriptive. It included 361 cases of sudden cardiovascular death, which underwent autopsy in forensic medicine department of Monastir during eight years, from 1st January 2004 to 31st December 2011. Results The incidence of sudden cardiovascular death was 9 per 100,000 person. A marked male predominance was noted. The mean age was 55.75 years. In our series, myocardial infarction represents the leading cause of sudden cardiovascular death, 57.8% of cases. Other etiologies were hypertrophic cardiomyopathy (4.7%), heart failure (1.9%), arrhythmogenic right ventricular dysplasia (2.8%), valvular disease (2%), cardio-myo-pericarditis (1.9%), hydatid cyst of the heart (0.8%), ruptured aneurysm (2.5%), pulmonary embolism (1.9%) and aortic dissection (1.3%). A sudden cardiovascular death at work was found in 25 cases. These cases pose essentially a problem of imputability. Conclusion Sudden cardiac death is usually the complication of underlying heart disease, sometimes overlooked. Several risk factors are involved. Sudden cardiac death in healthy heart or death caused by arrhythmia is an important entity seeking the intervention of several actors (forensic doctor, cardiologist, geneticist, media…) for prevention. Abstract: The fact that laser diodes are by nature a key component of all optoelectronic systems induced during ten last years a particularly great research effort is devoted to this component. Corresponding works cover a broad field of study of the physical properties of materials up to the analysis of the whole of the mechanisms governing the device operation. The aim of the present work is to contribute by a comparison of lasers diode containing quaternary alloy for different wavelength for a structure hetero-junction and double hetero-junction. We chose for our study the following alloys: Ga0,65In0, 35As0,15Sb0,85, Ga0,60In0,40As0,02Sb0,98and Ga0,46In0,54As0,48Sb0,52. In 2018's ADA/EASD recommendations, intensified insulin regimens, either by switching to premixed insulins or adding injections of prandial insulin, are considered final approach to glycemic management for patients with type 2 diabetes (T2D) insufficiently controlled under basal insulin. Overall, the total annual cost per patient, excluding oral medications, was 44% lower in Premix group than with Basal-Bolus (118.874 DZD versus 212.230 DZD respectively), and 66,4% lower when considering extra-cost of intensification from basal insulin regimen. Abstract: The degradation of two polluted textile wastewaters from SOITEX (silk and textile) industry using the plasma-catalytic process, has been studied by non-thermal Gliding arc technique coupled to Degussa P25 titanium dioxide (TiO2) as photo-catalyst. Experiments were carried out to optimise the amount of phtoto-catalyst. The results showed that maximum degradation was attained for 3gL−1 TiO2 concentration. For wastewater (1) degradation was 95% at the end of 60min of treatment time. The same wastewater was completely decolourised after only 30min of plasma-catalytic treatment time. In parallel, the biodegradability was significantly enhanced through 20min of exposure to the plasmagenous source for both wastewater samples. Turbidity of wastewater (1) and wastewater (2) decreased with rate constants of 0.015 and 0.017min−1, respectively. The TiO2-mediated Gliding Arc discharge () showed potential application for the treatment of liquid wastes, resulting in the mineralization of the wastewater samples confirmed by chloride, sulphate and phosphate ions formation. In both cases of GAD treatments, with and without photo-catalyst, the plasmagenous process proves efficient in the field of wastewaters degradation. [Copyright &y& Elsevier]

Résumé La dysplasie arythmogène du ventricule droit (DAVD) est une cardiomyopathie où le tissu normal du myocarde est remplacé par du tissu fibro-adipeux. L’examen histologique réalisé sur des biopsies myocardiques ou sur des prélèvements autopsiques permettent d’affirmer le diagnostic. Cependant, dans plusieurs cas, la certitude diagnostique ne peut pas se faire sur une simple étude macroscopique et histologique et nécessite une étude génétique et de biologie moléculaire. Dans ce travail, nous nous proposons de décrire les principales constatations macroscopiques et histologiques de la DAVD à travers l’étude d’une série autopsique. Nous rapportons 12 cas autopsiques de mort subite par DAVD colligés dans le service de médecine légale de l’hôpital universitaire Fattouma Bourguiba de Monastir (Tunisie) sur une période de 20 ans. L’examen microscopique a été réalisé sur des coupes histologiques de 5 microns d’épaisseur. Toutes les lames ont été relues par deux observateurs en double insu (médecin anatomopathologiste, médecin légiste) et chacun a précisé le pourcentage du tissu adipeux, de la fibrose et du myocarde au niveau du ventricule droit, ventricule gauche et septum interventriculaire, la présence ou pas d’infiltrat inflammatoire, la présence ou pas de signes de dégénérescence des myocytes. La DAVD était retrouvée dans 12 cas (soit 1,8 % des MS cardiovasculaires). L’âge était compris entre 13 et 67 ans (âge moyen est de 45,3 ans). Le décès est survenu dans la moitié des cas pendant un exercice physique. L’examen macroscopique du ventricule droit (VD) a montré la présence d’un amincissement de la paroi (épaisseur < 3 mm) dans 9 cas dont deux comportant une infiltration adipeuse macroscopique associée. L’étude histologique a permis de mettre en évidence une infiltration adipeuse du VD dans tous les cas avec un pourcentage variant entre 15 % et 60 %, des lésions de fibrose ont été notées seulement dans 9 cas avec un pourcentage moyen de 10,25 % et des signes de dégénérescence de myocytes ont été notés dans 10 cas. En concordance par rapport à ce qui a été rapporté dans la littérature, il n’y a pas encore de consensus en ce qui concerne les critères à adopter pour poser avec certitude le diagnostic de DAVD et la présence d’un tissu adipeux reste le critère le plus suggestif. Arrhythmogenic right ventricular dysplasia (ARVD) is cardiomyopathy where normal myocardial tissue is replaced with fibrofatty tissue. Histological examination performed on myocardial biopsy or on autopsy samples are used to confirm the diagnosis. However, in many cases, the diagnosis cannot be made on a simple macroscopic and histological study and requires genetic analysis and molecular biology. In this work, we propose to describe the main macroscopic and histological findings of ARVD through the study of an autopsy series. We report 12 autopsy cases of sudden death in ARVD collected in the Department of Forensic Medicine of the University Hospital Fattouma Bourguiba Monastir (Tunisia) during a period of 20 years. Microscopic examination was performed on 5 microns thick histological sections. All slides were reviewed by two operators in a double blind (physician pathologist, pathologist) and in each, the percentage of adipose tissue, fibrosis and infarction in the right ventricle, left ventricle and interventricular septum, the presence or absence of inflammatory infiltrate, the presence or absence of signs of degeneration of myocytes were noticed. ARVD was found in 12 cases (1.8% of sudden cardiac death). The age ranged between 13 and 67 years (mean age: 45.3 years). The death occurred in half of the cases during exercise. Macroscopic examination of the RV showed the presence of a wall thinning (thickness < 3 mm) in 9 cases. Histological study highlight RV adipose infiltration in all cases with a percentage between 15% and 60%, fibrotic lesions were observed in only 9 cases with an average percentage of 10.25% and signs of degeneration of myocytes were noted in 10 cases. In concordance with what has been reported in the literature, there is still no consensus regarding the criteria to be adopted to pose with certainty the diagnosis of ARVD and the presence of adipose tissue remains the criterion more suggestive. • Dissolved REEs and Nd isotopes were measured in thirteen rivers draining in the CAA. • REE concentration and Nd isotopes reflect watershed geology and ice cover. • Ice sheets greatly increase the river concentration of dissolved REEs. • The CAA river Nd flow is equivalent to the Nd marine input from the Canada Basin. • Climate change could affect the Nd isotopic composition of Baffin Bay. Determining the factors controlling the neodymium (Nd) isotopic compositions (expressed as ε Nd) and rare earth element (REE) concentrations of rivers is imperative to improve our understanding of the distribution of these water mass tracers in the ocean. Here we present the first measurements of <0.45 μm-filtrate REE concentrations (i.e., the concentration of truly dissolved, nano-particulate and colloidal REEs passing through a 0.45 μm filter, hereafter referred to as dissolved) and ε Nd in rivers draining into the Canadian Arctic Archipelago (CAA). Results show a large variation in both REE concentrations (Nd = [ 8 ; 37 , 260 ] pmol/kg) and ε Nd ([ − 32.1 ; − 15.1 ]) in the thirteen rivers sampled. Dissolved REE concentrations increase with increasing fractions of metamorphic rocks and ice cover in the rivers' watersheds, while ε Nd and PAAS-normalized REE patterns are inherited from the watershed's bedrock lithology. The four rivers draining watersheds composed of Precambrian metamorphic rocks exhibit the highest REE concentrations, the least radiogenic ε Nd , and shale-normalized patterns enriched in light REEs. The remaining nine rivers drain mainly or exclusively sedimentary bedrock and have lower REE concentrations, more radiogenic ε Nd and generally show heavy REE enrichment, with variable negative cerium (Ce) anomalies. The presence of ice sheets in the drainage area, and the ionic strength, pH, and dissolved organic carbon concentration of river water are the main factors determining riverine dissolved REE concentrations by controlling the formation and stabilization of colloids. We estimate a flow of dissolved Nd transported by Arctic rivers discharging into the CAA equivalent to the Nd flux of seawater entering the CAA. Depending on the percentage of Nd removal during mixing with seawater, the impact of river water on the ε Nd of seawater exiting the CAA could be modest (if 90% removal) or more significant (if ≪90% removal). This riverine contribution could potentially impart a climate-sensitive ε Nd signature to the seawater reaching Baffin Bay, with possible implications for the use of ε Nd in paleoceanographic reconstructions of water mass distribution. Objectifs La neuropathie autonome cardiaque est associée à une augmentation du risque cardiovasculaire. Dans une étude transversale, nous avons montré que la prévalence de l'hypertension artérielle (HTA) chez les dia-bétiques augmentait avec la gravité de la neuropathie autonome cardiaque (NAC) et que l'association NAC/HTA était associée à une prévalence plus élevée de complications vasculaires. L'objectif de cette étude prospective était d'examiner la valeur prédictive de la NAC sur l'incidence de l'HTA et le rôle potentiel de l'association NAC/HTA sur l'apparition de nouvelles complications vasculaires. Méthodes La NAC a été recherchée dans une population de 310 patients (172 diabétiques de type 2, 138 diabétiques de type 1) par l'analyse des variations de la fréquence cardiaque lors de trois tests standard (respiration profonde, orthostatisme actif, Valsalva) évaluant surtout l'activité parasympathique. Ces patients étaient âgés de 41,7 ± 12,8 ans, durée moyenne du diabète 8,4 ± 6,8 ans, 62 étaient hypertendus. La NAC était présente chez 123 patients (72 diabétiques de type 2, 51 diabétiques de type 1) qui ont été invités à une nouvelle visite d'évaluation 10-12 ans plus tard selon le même protocole. Résultats Parmi les 123 patients NAC+, 25 étaient perdus de vue. Parmi les 98 autres, 30 avaient une HTA au départ : 6 d'entre eux étaient décédés (20 %), de cause cardiovasculaire. Parmi les 68 patients NAC+ normotendus initialement, 10 ont présenté un événement cardiovasculaire, et 20 sont dev-enus hypertendus (29 %). La prévalence de l'HTA à cette seconde visite aug-mentait avec la gravité de la NAC (p < 0,01) : 12,5 %, 35 % et 56,2 %, respectivement chez les patients avec un, deux ou trois tests anormaux. Les patients devenus hypertendus avaient un score de NAC plus élevé à la première et seconde visites (p < 0,01), étaient plus âgés (p < 0,05), avaient une durée de diabète plus longue (p < 0,02), et une prévalence plus forte de complications vasculaires, en particulier rénales (p < 0,001), que les patients restés normotendus. Conclusion La NAC définie par un défaut d'activité parasympathique con-tribue bien à l'émergence de l'HTA chez les diabétiques. Abstract: We studied the glycaemic threshold and prevalence of diabetic retinopathy in screen-detected diabetes in Saudi Arabia, Algeria and Portugal. The prevalence of diabetes-specific retinopathy started to increase at an HbA1c level of 6–6.4% (42–47mmol/mol) and in individuals with HbA1c >7.0% the prevalence was 6.0%. [Copyright &y& Elsevier]
