Locoregional recurrence remains common after treatment of head and neck cancer, warranting careful surveillance in follow-up. Although randomized data support an initial positron emission tomography/computed tomography several months after treatment, evidence supporting subsequent imaging is limited, and most recurrences ultimately manifest clinically. Cooperative group studies and consensus guidelines vary widely in their recommendations regarding surveillance imaging. Patients with indeterminate findings, new symptoms, or areas difficult to examine in clinic may avoid invasive and potentially morbid interventions with judicious use of subsequent imaging. For any patient undergoing posttreatment imaging, standardized reporting criteria provide a framework for risk-stratification that can enhance communication and potentially guide management. The present study aimed to investigate the therapeutic potential of the methanolic extract of Lepidium sativum seeds in mice experimentally infected with Trypanosoma evansi. A total of thirty-two male Swiss albino mice were randomly divided into four groups: the first group was the normal control, while the second, third and fourth groups were infected intraperitoneally with 1 × 104 trypanosomes. The third and fourth groups were treated with 100 μl of Lepidium sativum seed extract (LSSE) at a dose of 200 mg/kg body weight intraperitoneally (infected + LSSEI) and orally (infected + LSSEO) respectively, once a day, for a period of four days. Parasitaemia was found to be significantly raised in the untreated infected group, reaching 2 × 107 at day 4 post-infection, but was significantly reduced by 65.5% and 88% in the mice treated orally and intraperitoneally with LSSE, respectively. The erythrocyte count, HCT, haemoglobin content, leucocyte count and the percentage of lymphocytes was significantly reduced in the untreated infected group, while the treatment with LSSE returned these parameters to their pre-infection values. In addition, our study proved that LSSE provided protection against liver tissue damage and decreased the levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT). The present study also established that intraperitoneal injection of LSSE is more effective than oral administration in the treatment of trypanosome infection in mice. Abstract Hydatidosis is considered to be one of the important zoonotic diseases and has a significant public health importance due to the difficulties of the diagnosis. Domestic animals act as intermediate hosts and the main reservoir for the disease in humans. The main purpose of this work therefore was to determine the prevalence of hydatidosis in Najdi sheep slaughtered in Riyadh city, Saudi Arabia. Cyst location and cyst fertility and viability were also estimated, together with effect of seasons, age and sex on the prevalence of the infection. The prevalence of hydatidosis was evaluated by post-mortem examination, with intensive inspection of the visceral organs of 2785 Najdi sheep. The infection was found to prevail throughout the year in both sex, with an overall prevalence of 2.33%. The highest prevalence was recorded in winter (6.48%) while the lowest was encountered in summer (1.36%). Females were proved to be more prone to infection (70.7%) than males (29.3%). In the present study, younger sheep tended to have a higher prevalence of infection than older ones. The most commonly infected organ was the liver, with a prevalence of 81.5%. The recorded cysts showed a fertility rate of 75.4%, and a high viability rate of 61.2%. Hepatic cysts were the most fertile and viable ones (46%), while calcified cysts were not recorded during the study. Measurements of recorded cysts in all organs ranged from 2 to 6 cm in diameter. In conclusion, the high fertility and viability rate of the recorded cysts suggest that sheep are a potential source of hydatidosis transmission to dogs and the continuation of its life cycle in this region. Consequently, authorities are recommended to instigate stricter regulation of the slaughtering process, including the secure disposal of infected offal so as to minimise the transmission of cysts from slaughter houses, along with treatment of stray dogs. A low-cost and eco-friendly solid-phase extraction sorbent obtained from Peganum harmala stems was developed. It was used for the removal of total dissolved salts (TDS) from non-drinking tap water derived from boreholes. The prepared sorbent was characterized with FT-IR and SEM micrograph. Adsorption parameters were investigated by the batch method. Results showed a significant decrease in TDS by increasing the shaking time. Adsorption kinetics followed the first-order kinetics with a half-life time of 16.1 min. Adsorption isotherm obeyed Freundlich physisorption model with a correlation coefficient of 0.956. Thermodynamic study proved an endothermic adsorption with a heat of adsorption of 8.1 J/mol. Under dynamic mode, results showed a gradual decrease in TDS and reached values of 840, 735 and 802 mg/L for Peganum harmala filter, and commercial sediment and carbon filters, respectively after flowing time of 100 min. Also, passing tap water for three days gave values of 691, 550 and 600 mg /L, respectively. This paper describes a new coelozoic myxosporean parasite named Ceratomyxa sultani n. sp. isolated from the gallbladder of Upeneus margarethae sourced from the Arabian Gulf off Saudi Arabia. Of 104 U. margarethae specimens examined, 27 (26%) were infected, with the highest prevalence in winter and lowest in autumn. The pseudoplasmodia were disporous and irregularly elliptical in shape, with an average size of 22 × 17 μm. Mature spores were mostly elliptical with symmetrical valves and equal spherical polar capsules. Spores were 9 µm in length and 25 µm in thickness, while polar capsules were 4 µm wide with four filament coils. The paper further provides a morphological comparison with closely related Ceratomyxa spp. together with phylogenetic analysis based on the partial 18S rRNA sequence, which revealed that C. sultani n. sp. clustered within a robust clade of Ceratomyxa species from the Arabian Gulf and Red Sea or nearby geographic regions. Hydatidosis, or echincoccosis, is a serious medical and veterinary problem in many countries, particularly those with rural communities where there is a greater contact between dogs and domestic animals. Domestic livestock act as intermediate hosts which are the main reservoir for the disease in humans. It is therefore very important to estimate the prevalence of hydatid cysts in slaughtered animals since it can be transmitted to humans through dogs, which act as the final host for the disease. From this point of view, the present study was suggested to determine the prevalence of hydatidosis in Sawakny sheep slaughtered in Riyadh city, Saudi Arabia. During the course of the study 12,569 Sawakny sheep were inspected for hydatidosis infection. An overall prevalence of 1.06% was detected among the examined sheep, with the highest prevalence occurring in winter (1.38%) and lowest prevalence in summer (0.67%). Sheep aged 6–12 months had a higher rate of infection than older animals, and males were the predominant carriers of infection (97.7%) compared to females (2.3%). The liver was the most infected organ (79.1%), followed by the lungs (14.6%), while concurrent infections of both the liver and the lungs occurred in 6% of cases. The fertility and viability rates of hydatid cysts in the liver (70.1% and 85.1% respectively) were higher than that in any other organs. In conclusion, it is evident that fertile cysts in slaughtered sheep could have an important role in the continuation of hydatid cyst transmission to humans through dogs. Considerable effort should be devoted to controlling the transmission of cysts from abattoirs by the secure disposal of infected offal. Hydatid disease is an important economic and human public health problem with a wide geographical distribution. Surgical excision remains the primary treatment and the only hope for complete cure of hydatosis. The most important complications arising from surgical excision, however, is recurrence, which is due to dissemination of protoscolices during the surgery. Pre-surgical inactivation of the contents of the hydatid cyst by injection of scolicidal agent into the cyst has been used as adjunct to surgery in order to overcome the risk of recurrence. In the present study, ethanolic extracts of turmeric ( Curcuma longa ) and ginger ( Zingiber officinale ) were tested as scolicidal agent for Echinococcus protoscoleces. Protoscoleces were collected aseptically from sheep livers containing hydatid cysts. Three concentrations (10, 30 and 50 mg/ml) of each extract were investigated and viability of the protoscoleces was tested by 0.1% eosin staining. Ginger extract showed the strongest scolicidal effect (100%) after 20 min at a concentration of 30 mg/ml and 10 min at 50 mg/ml . The maximum scolicidal effect of turmeric was 93.2% after 30 min at a concentration of 50 mg/ml. It is concluded that turmeric and ginger extracts have high scolicidal activity and could be used as effective scolicidal agents against Echinococcus protoscoleces. [ABSTRACT FROM AUTHOR]

A new microsporidian species, Glugea arabica n. sp., is reported infecting the intestinal wall of the marine teleost Epinephelus polyphekadion (= microdon ) collected from the Red Sea coast off Saudi Arabia, and described on the basis of microscopic and molecular procedures. Spherical blackish xenomas formed parasitophorous vacuoles completely packed with several parasitic developmental stages, including spores. The nuclei were monokaryotic in all developmental stages. Spores were ellipsoidal to pyriform and measured 6.3 ± 0.3 (5.9–6.6) μm in length and 3.3 ± 0.4 (2.9–3.7) μm in width. A lamellar polaroplast surrounded the uncoiled portion of the polar filament, which extended into the spore's posterior pole and formed 27–29 coils organized in three or four rows. The posterior vacuole, located at the spore's posterior pole, appeared surrounded by the polar filament coils and displayed an irregular matrix composed of light material, in which was located the posterosome. Molecular analysis of the rRNA genes, including the ITS region, was performed using maximum parsimony, neighbor-joining and maximum likelihood methodologies. The present study was carried out as part of an ongoing general survey for myxosporean parasites infecting tilapias in the River Nile, Egypt. In the present study, 77 Nile tilapia ( Oreochromis niloticus ) were collected from boat landing sites at Beni-Suef governorate, Egypt and examined for the myxosporean infection. The infection was encountered as a huge number of free spores in the kidney and the spleen. The infection showed a prevalence of 51.9% (40/77) for Myxobolus brachysporus while it was 25.9% (20/77) for Myxobolus israelensis . Mature spores of M. brachysporus were ellipsoidal and measured 8.6 × 13.2 μm. The polar capsules were subcircular with 5–6 filament turns and measured 4.7 × 3.6 μm. Spores of M. israelensis were ellipsoidal in the frontal view and fusiform in the lateral view. Spore measurements were 13.4 μm long and 8.7 μm wide. The polar capsules were elongated with 6–7 filament coils and measured 8.6 × 3.1 μm. The findings presented here proved that tilapia fishes in the Nile River are still suffering from infections with Myxobolus species. Therefore, further studies should be carried out to survey the Myxobolus infection among tilapias under culture conditions to clarify the pathological impacts of this parasite in tilapias aquaculture. The present study sought to assess the potential of the cestode Hymenolepis diminuta as a bioindicator for lead accumulation in two industrial areas of the city of Riyadh, Saudi Arabia. Rats (Meriones libycus) were collected from two sites (industrial area II and Salbukh) in Riyadh. In the industrial area II, the mean levels of lead concentrations were found to be 1.96, 1.92, 1.4 and 30.72μg/g in the rats' liver, kidney and intestine, and in H. diminuta, respectively. In Salbukh, meanwhile, the lead concentrations were 1.63, 1.52, 1.20 and 21.31μg/g in the rats' liver, kidney, and intestine, and in H. diminuta, respectively. In addition, in industrial area II, compared with the liver, kidney and intestine of their host, the bioconcentration factors of lead were found to be, respectively, 15.6, 16 and 21.9 times higher in H. diminuta, and were 7.5, 8, and 10.2 times higher in the same organs compared to H. diminuta in Salbukh. The present study, therefore, proved that H. diminuta could be used as a bioindicator for heavy metal contamination in the industrial areas of the city of Riyadh. [ABSTRACT FROM AUTHOR]

Objective: Although the majority of human papillomavirus (HPV)-positive oropharyngeal squamous cell carcinomas have a favorable prognosis, we search for markers of poor prognosis by carefully examining a subset of highly aggressive cases. Study design: Seven patients with HPV-positive oropharyngeal cancer who presented with non-pulmonary distant metastasis or developed distant metastasis posttreatment were identified. Eight control cases were chosen which responded well to treatment. Pathologic and radiologic studies were reviewed and compared. Results: Two cases displayed a small cell carcinoma (SmCC) component upon pathologic review. Biomarker analysis revealed lower expression of NOTCH1 in the aggressive cohort in comparison to controls (P = .04). Cases showed a predominance of clustering of lymph nodes, extracapsular spread, and central tumor necrosis. Conclusion: Although most HPV-related oropharyngeal cancers display a positive prognosis, it is evident that there is a subset, which behaves more aggressively. Summary: Objective:: To determine the peak time of outbreak of Crimean-Congo haemorrhagic fever (CCHF) and to highlight the disease-specific areas in the Balochistan province of Pakistan. Design:: A hospital-based case-series study. Place and Duration:: A five-year study was conducted by the Department of Pathology in collaboration with the Department of Medicine, Sandeman Provincial Teaching Hospital, Quetta, Balochistan, Pakistan, from 1 March 1997 to 28 February 2002. Patients and methods:: A total of 135 cases were included in the study based on a high index of suspicion of CCHF. The mean age was 33.5±18.7 years. Two sets of blood samples were drawn on admission; one set was sent for urgent routine investigations while the other was sent to CDC, Atlanta, USA, for confirmation. Results:: The study showed that CCHF was more common between March and May and again, between August and October, depicting a bi-annual surge. Out of 135 suspected cases, 83 (61.4%) were confirmed to have CCHF and were given platelet concentrate and ribavirin. It was observed that CCHF was more common in specific areas of the province. Conclusions:: It is essential to raise the level of knowledge regarding CCHF, especially in disease-specific areas, focussing on the peak outbreak periods. [Copyright &y& Elsevier]

• Total glossectomy with total laryngectomy (TGTL) is rarely performed. • Survival after TGTL is often less than 2 years. • Many patients can resume some oral intake after TGTL. • Most patients communicate after TGTL by writing. Total glossectomy with total laryngectomy (TGTL) is indicated for some cases of advanced oral squamous cell carcinoma. However, this procedure is rarely performed, as quality of life outcomes are often considered poor. Consequently, few studies to date have reported survival and functional outcomes in patients undergoing TGTL. Here, we present the largest cases series to date of TGTL patients and provide relevant data on survival and functional outcomes. Patients met inclusion criteria if they underwent TGTL (concurrent or staged) indicated for head and neck squamous cell carcinoma. Patient demographics and disease characteristics, survival outcomes, functional oral intake scores, time to oral intake, gastrostomy tube dependence, and communication methods post-surgery were retrospectively extracted from the electronic medical record. Survival in patients undergoing TGTL was poor. Most patients in this study were eventually approved for some oral intake of restricted consistencies but remained gastrostomy tube dependent for most of their nutritional needs. Baseline oral intake was suboptimal in most patients but often re-achieved approximately 12 months following surgery. Communication methods following surgery included writing, text-to-speech, and augmentative and alternative communication devices. Our data provide new insights comparing survival and functional outcomes of patients undergoing TGTL. Additional investigation particularly on patient-perceived quality of life following TGTL is needed to better understand the risks and benefits for patients who are candidates for TGTL. • A novel microsporidian, Glugea eda n. sp. infecting Caesio striata is described. • Developmental cycle, ultrastructural and phylogenetic data are presented and evidenced the taxonomic novelty of Glugea eda. • Glugea eda represents the first occurrence of microsporidian infection in the fish of the family Caesionidae. Glugea eda n. sp. is described from the mesenteries of the striated fusilier, Caesio striata , collected from the Red Sea coast off Yanbu' al Bahr, Saudi Arabia. Numerous blackish xenomas, ranged from 3 to 5 mm, were found in the body cavity associated with the mesenteries. Mature spores are monomorphic, ellipsoidal with an average size of 5(4–6) μm in length and 2.2 (2–3) μm in width. Observations of the ultrastructure revealed that the development was asynchronous and that the nuclei were isolated throughout the life cycle with uninucleate meronts. Sporoblasts were uninucleated and existed together with sporonts in a fully formed parasitophorous vacuole. The polar filament of the mature spore was isofilar with 24–28 coils, arranged in three rows. Phylogenetic analysis placed the current microsporidia within the clade grouping Glugea species and close to the species described from the Red Sea and Arabian Gulf. The morphometric and molecular comparison with other members of the genus Glugea evidenced the taxonomic novelty of the present form, suggesting that it should be considered as a new species. To the best of our knowledge, the parasite here described represents the first occurrence of microsporidian infection in the fish of the family Caesionidae.
