This study aimed at evaluating territorial inequalities in avoidable mortality in children under 5 years of age in Colombia between 2000 and 2019. This was an ecological study. An ecological, longitudinal, multigroup study was conducted using secondary sources. Because of the hierarchical structure of the data, the effect of territorial characteristics on the count of avoidable under-five deaths was estimated using a three-level negative binomial regression model with random intercepts for municipality and fixed intercepts for time and departments. Between 2000 and 2019, there were 216,809 avoidable under-five deaths in Colombia (91.3% of all registered deaths of children under 5 years of age). A total of 1117 municipalities located in 33 departments were analyzed over five 4-year periods. Ecological relationships were found between avoidable under-five mortality and the percentage of adolescent births, female illiteracy, and multidimensional poverty at the municipal level (standardized mortality ratio: 1.43 95% confidence interval: 1.33–1.54 for the group with the highest level vs the group with the lowest level of poverty). Furthermore, multidimensional poverty was a confounding factor for the association between the percentage of the population living in rural areas and avoidable child mortality. Systematic and avoidable gaps were observed in mortality in children aged under 5 years in Colombia, where the territory constitutes an axis of inequality. Implementing strategies and programs that contribute to improving the conditions of women and socio-economic conditions in the territories should be a priority. [ABSTRACT FROM AUTHOR]

Investments in public health – prevention of illnesses, and promotion, surveillance, and protection of population health – may improve population health, however, effects may only be observed over a long period of time. To investigate the potential long-run relationship between expenditures on public health and avoidable mortality from preventable causes. We focused on the country spending the most on public health in the OECD, Canada. We constructed a longitudinal dataset on mortality, health care expenditures and socio-demographic information covering years 1979–2017 for the ten Canadian provinces. We estimated error correction models for panel data to disentangle short-from long-run relationships between expenditures on public health and avoidable mortality from preventable causes. We further explored some specific causes of mortality to understand potential drivers. For comparison, we also estimated the short-run relationship between curative expenditures and avoidable mortality from treatable causes. We find evidence of a long-run relationship between expenditures on public health and preventable mortality, and no consistent short-run associations between these two variables. Findings suggest that a 1% increase in expenditures on public health could lead to 0.22% decrease in preventable mortality. Reductions in preventable mortality are greater for males (−0.29%) compared to females (−0.09%). These results are robust to different specifications. Reductions in some cancer and cardiovascular deaths are among the probable drivers of this overall decrease. By contrast, we do not find evidence of a consistent short-run relationship between curative expenditures and treatable mortality, except for males. This study supports the argument that expenditures on public health reap health benefits primarily in the long run, which, in this case, represents a reduction in avoidable mortality from preventable causes. Reducing public health expenditures on the premise that they have no immediate measurable benefits might thus harm population health outcomes in the long run. • Literature on health expenditures and health outcomes rarely separates long-run and short-run. • Public health requires time to produce health effect, hence a need to model the long-run relationship. • Expenditures on public health are associated with a long run reduction of preventable mortality. • This long run relationship is stronger for preventable mortality among males than females. • There is a limited influence of curative expenditures on treatable mortality. The indicators’ values of health condition of the Slovak citizens significantly lag behind the most developed countries. The issue of healthy inhabitants is more connected with a treatment of bad health condition than with a non-health prevention, which is believed to be caused by social and political development over the last decades. Measuring the influence of health system on inhabitants’ health is very difficult due to the influences of various factors, such as wealth, socio-economic status, citizen's lifestyle, quality of environments, etc. Consequently, an amenable mortality indicator is used to evaluate the health system except of the life expectancy indicator. It defines a mortality that should not have occurred in case of effective and early treatment. It only takes into consideration those diagnoses, where effective treatment exists before a certain age limit (75 years) according to experts. The number of these deaths is standardized on 100,000 citizens. The amenable mortality indicator has a great significance for standard comparisons, however, its application also causes methodological issues, such as data quality of mortality, choice of diagnoses, weight of diagnoses, etc. In the international comparing, it is very often deduced from a fact that each country spends different amount of funds on health, while the differences may be connected with a different effectiveness of invested funds in the health systems. The article reflects on the given facts and its aim is to evaluate the development of amenable mortality in Slovakia over the last ten years, as well as to compare the development of this indicator in the EU countries. Similarly, it assesses the significant methodological issues that are related to indicator's application and defines the causes of extreme differences that were found out by the results on the basis of the given outputs. Abstract: Avoidable mortality is defined as deaths that should not occur given current medical knowledge and technology. Numerous different lists of causes of death and the ages at which they should be considered avoidable have been used to measure avoidable mortality. In this analysis of the importance of definition we compare the two most commonly used approaches using a data set including all 11.8 million deaths that occurred in Britain in 1981–1998. These mortality data, disaggregated by age and sex, are analysed within a multilevel statistical framework, which allows analysis at a number of geographical scales simultaneously. A substantial difference in both the average trends and spatial patterns of the two definitions of avoidable mortality is found, indicating that the causes of death chosen have a considerable impact on the results found. Indeed, one particular cause of death was found to be largely responsible for the differences between the definitions. In addition, the spatial pattern of the two types of avoidable mortality is very different at the larger geographical scale while the pattern at the smaller scale is very similar. The findings illustrate the importance of considering the goals of any study before deciding on the definition of avoidable mortality to use. [Copyright &y& Elsevier]

Objectives: Using the concept of avoidable mortality, international studies suggest that healthcare has been effective in reducing mortality. This paper provides an analysis of avoidable mortality in Australia and compares trends with those of Western Europe. Methods: Using unit-record mortality data, we calculated avoidable mortality rates in Australia for 1968-2001. We partitioned avoidable causes into three categories: those amenable to medical care; those mainly responsive to health policy; and ischaemic heart disease. We used Poisson regression to model the trends. We compared trends with those of nine European countries using published data. Results: Total avoidable death rates fell by 68% in females and 72% in males. The corresponding non-avoidable death rates fell by 35 and 33%. The annual declines in avoidable mortality rates were: 3.47% [95% confidence intervals (CI) 3.44-3.50%] in males and 3.89% (95% CI 3.86-3.91%) in females. For non-avoidable mortality rates, the annual declines were 1.09% (95% CI 1.05-1.13%) and 0.95% (95% CI 0.92-0.98%), respectively. In females, declines in death rates from causes amenable to medical care contributed 54% to the decline in avoidable mortality rates, ischaemic heart disease contributed 45%, and causes responsive to health policy intervention contributed 1%. In males, the corresponding contributions were 32, 57 and 11%. These rates, and the declines between 1980 and 1998, were comparable with selected European countries, with Australia's ranking improving over the period. Conclusion: Trends in avoidable mortality in Australia suggest that the Australian healthcare system has been effective in improving population health. This paper assesses the changes in avoidable mortality in Lithuania in 1991–1999 compared with 1970–1990. Causes of death were disaggregated into causes most amenable to treatment and those amenable to prevention. Trends in age­standardised death rates were calculated. In 1970–1990, avoidable causes of death accounted for 26.3% of all deaths. By 1991–1999 this figure had decreased slightly to 24.6%. At the same time, age­standardised death rates from avoidable causes increased by 8%, from 118.1 per 100,000 in 1970–1990 to 127.9 in 1991–1999. Avoidable mortality among men was considerably higher than for women in both periods. There was considerable fluctuation in both treatable and preventable mortality during the 1990s, reflecting diversity in trends in different causes of death. Increases occurred in death rates from tuberculosis, cervical cancer and liver cirrhosis and, immediately after independence, also in hypertensive and cerebrovascular diseases and, among men, lung cancer, followed by subsequent declines. Deaths from chronic rheumatic heart disease, asthma and other respiratory diseases, appendicitis, abdominal hernia, cholelithiasis and maternal mortality consistently declined. In conclusion, avoidable mortality declined as a proportion of total mortality in Lithuania during 1991–1999 compared with 1970–1990. This reflected the combined impact of an initial rise in death rates from treatable and, to a lesser extent, preventable causes, followed by subsequent declines. While this indicates some success in the development of medical care, it emphasises the need for more effective public health policies directed at the major determinants of health. This study aimed to estimate age-period-cohort effects on avoidable mortality and quantify the impact of avoidable mortality changes on life expectancy (LE) at birth in the South Korean population aged 0–74 years. Using death certificate and resident population data from 2000 to 2017, trends in age-standardized avoidable mortality rates were analyzed with joinpoint regression. Intrinsic estimator regression analysis was conducted to estimate age-period-cohort effects on avoidable mortality. Arriaga's method was used to measure the contributions of avoidable causes to changes in LE gaps between adjacent three-year periods by age and avoidable cause of death groups. Avoidable mortality decreased annually by 4.6% between 2000 and 2017. There were strong age and cohort effects and a weak period effect on avoidable mortality. In the overall decreasing trend, avoidable mortality declined less in cohorts born after the 1950–1953 Korean War and economic recession in the 1970s, with further reductions in cohorts born after the 1987 democratic reform and 1997–1998 economic crisis. Avoidable mortality was reduced after implementation of major health policies, but the decrease stagnated during the 2008–2009 financial crisis. Avoidable mortality reduction resulted in LE gains of 3.1 years, which accounted for 80% of total LE gains. Contribution to LE gains by causes of death was the largest for cerebrovascular disease. Major social changes and health policies influenced the avoidable mortality trend through cohort and period effects.
