Objective: To evaluate the effectiveness of magnetotherapy in the treatment of hand osteoarthritis (HO). Methods: In this randomized controlled single-blind follow-up study, patients with HO were randomly assigned into 2 groups (G1 and G2). The subjects in G1 (n=25) received 25Hz, 450pulse/s, 5-80G, magnetotherapy of totally 10 days and 20min/day combined with active range of motion/strengthening exercises for the hand. G2 (n=25) received sham-magnetotherapy for 20min/day for the same duration combined with the same hand exercises. Outcome measures were pain and joint stiffness evaluation, handgrip and pinchgrip strength (HPS), Duruöz and Auscan Hand Osteoarthritis Indexes (DAOI) and Short Form-36 Health Questionnaire (SF-36) administered at baseline, immediately after treatment and at the follow up. Results: When the groups were compared with each other, improvement observed in SF-36 Pain (p<0.001), SF-36 Social Function (p=0.030), SF-36 Vitality (p=0.002), SF-36 General Health (p=0.001), Pain at rest (p<0.001), Pain at motion (p<0.001), Joint stiffness (p<0.001), DAOI (p<0.001) were in favor of G1. Conclusions: Changes in pain, function and quality of life scores showed significant advantage in favor of the applied electromagnetic intervention in patients with HO. [ABSTRACT FROM AUTHOR]

Aims: To study the clinical hand findings in Behçet's disease (BD) and to observe scintigraphic changes of these areas.Methods: Fifty-seven randomly selected BD patients and the patients in the control group (N=40) were evaluated by two blind rheumatologists. The hands were examined for the presence of pain, tenderness, swelling, effusion, erythema, warmth, range of motion and limitation of motion, deformities and muscle atrophy. Then scintigraphic examination of the hands was performed. Control hand scintigrams were obtained from 40 age- and sex-matched patients and were examined by the same two observers.Results: Thirty-two of the 57 patients (56.1%) showed Behçet's clinical hand findings. Terminal phalangeal pulp atrophy was observed in 17 (29.81%), rheumatoid-like hand findings were observed in 16 (28.1%), dorsal interosseos atrophy was observed in 12 (20.05%) and erythema over the digits was observed in 12 (20.05%). Twenty-four patients (42.1%) had scintigraphic involvement. The disease duration was observed to be an important factor for hand findings (P=0.040) and scintigraphic involvement (P=0.011).Conclusion: High prevalence of hand involvement in BD and its relationship with disease duration is demonstrated. Hand involvement tends to be overlooked and careful examination is required in the evaluation of BD. The scintigraphic involvement detected in hands requires special consideration, too. Résumé: But: Étudier les atteintes cliniques de la main au cours de la maladie de Behçet (MB) et observer les anomalies scintigraphiques de ce territoire. Méthodes: Cinquante-sept malades avec MB tirés au sort et un groupe témoin de 40 sujets ont été évalués en aveugle par deux rhumatologues. Les mains étaient analysées pour la douleur, la sensibilité à la palpation, les gonflements, synovites, érythème, chaleur, la mobilité, les déformations et l''amyotrophie. Ensuite une analyse scintigraphique a été réalisée. Des scintigraphies de contrôles ont été obtenues chez 40 patients appariés en âge et sexe, qui ont été analysés par les mêmes examinateurs. Résultats: Trente-deux des 57 malades (56,1 %) avec MB avaient des anomalies cliniques. Une atrophie pulpaire de la phalange terminale a été observée chez 17 patients (29,81 %), des signes « rhumatoïde like » étaient présents chez 16 (28,1 %), une atrophie dorsale des interosseux chez 12 (20,05 %) et un érythème digital chez 12 (20,05 %). Vingt-quatre patients (42,1 %) avaient une atteinte scintigraphique. La durée de la maladie est apparue être un facteur important associé aux atteintes de la main cliniques (p =0,040) et scintigraphiques (p =0,011). Conclusion: Une haute prévalence de l''atteinte de la main et son association à la durée de la maladie sont démontrées au cours de la MB. L''atteinte de la main semble être négligée et une évaluation précise est nécessaire au cours de la MB. L''évaluation scintigraphique pourrait également être recommandée. [Copyright &y& Elsevier]
