Abstract: Azimuthal asymmetries in exclusive electroproduction of a real photon from a longitudinally polarized deuterium target are measured with respect to target polarization alone and with respect to target polarization combined with beam helicity and/or beam charge. The asymmetries appear in the distribution of the real photons in the azimuthal angle ϕ around the virtual photon direction, relative to the lepton scattering plane. The asymmetries arise from the deeply virtual Compton scattering process and its interference with the Bethe–Heitler process. The results for the beam-charge and beam-helicity asymmetries from a tensor polarized deuterium target with vanishing vector polarization are shown to be compatible with those from an unpolarized deuterium target, which is expected for incoherent scattering dominant at larger momentum transfer. Furthermore, the results for the single target-spin asymmetry and for the double-spin asymmetry are found to be compatible with the corresponding asymmetries previously measured on a hydrogen target. For coherent scattering on the deuteron at small momentum transfer to the target, these findings imply that the tensor contribution to the cross section is small. Abstract: Azimuthal asymmetries in exclusive electroproduction of a real photon from an unpolarized deuterium target are measured with respect to beam helicity and charge. They appear in the distribution of these photons in the azimuthal angle ϕ around the virtual-photon direction, relative to the lepton scattering plane. The extracted asymmetries are attributed to either the deeply virtual Compton scattering process or its interference with the Bethe–Heitler process. They are compared with earlier results on the proton target. In the measured kinematic region, the beam-charge asymmetry amplitudes and the leading amplitudes of the beam-helicity asymmetries on an unpolarized deuteron target are compatible with the results from unpolarized protons. Abstract: A series of semi-inclusive deep-inelastic scattering measurements on deuterium, helium, neon, krypton, and xenon targets has been performed in order to study hadronization. The data were collected with the HERMES detector at the DESY laboratory using a 27.6 GeV positron or electron beam. Hadron multiplicities on nucleus A relative to those on the deuteron, , are presented for various hadrons (, , , , , p, and ) as a function of the virtual-photon energy ν, the fraction z of this energy transferred to the hadron, the photon virtuality , and the hadron transverse momentum squared . The data reveal a systematic decrease of with the mass number A for each hadron type h. Furthermore, increases (decreases) with increasing values of ν (z), increases slightly with increasing , and is almost independent of , except at large values of . For pions two-dimensional distributions also are presented. These indicate that the dependences of on ν and z can largely be described as a dependence on a single variable , which is a combination of ν and z. The dependence on suggests in which kinematic conditions partonic and hadronic mechanisms may be dominant. The behaviour of at large constitutes tentative evidence for a partonic energy-loss mechanism. The A-dependence of is investigated as a function of ν, z, and of . Abstract: Single-spin asymmetries in the semi-inclusive production of charged pions in deep-inelastic scattering from transversely and longitudinally polarized proton targets are combined to evaluate the subleading-twist contribution to the longitudinal case. This contribution is significantly positive for mesons and dominates the asymmetries on a longitudinally polarized target previously measured by Hermes. Abstract: Hard exclusive electroproduction of pairs off hydrogen and deuterium targets has been studied by the HERMES experiment at DESY. Legendre moments and of the angular distributions of mesons in the center-of-mass frame of the pair have been measured for the first time. Their dependence on the invariant mass can be understood as being due to the interference between relative P-wave (isovector) and S-, D-wave (isoscalar) states of the two pions. The increase in magnitude of as Bjorken x increases is interpreted in the framework of generalized parton distributions as an enhancement of flavour non-singlet exchange for larger values of x, which leads to a sizable admixture of isoscalar and isovector pion pairs. In addition, the interference between P-wave and D-wave states separately for transverse and longitudinal pion pairs has been studied. The data indicate that in the region at higher-twist effects can be as large as the leading-twist longitudinal component. [Copyright &y& Elsevier]

Recent HERMES measurements are presented on the diffractive slopes of the coherent and incoherent part of the cross section for exclusive diffractive production of
ϱ0 mesons. Based on data taken with a variety of targets, 1H, 3He, 14N, 20Ne and Krypton, the method to extract the coherent-to-incoherent cross section ratios in dependence of the coherence lengthlc and momentum transferQ2 is presented. These measurements provide important information about the interaction of quark-antiquark pairs with the nuclear medium. Using 2H and 14N targets, the nuclear transparency and coherent cross section ratio ofϱ0 production has been measured versus coherence length andQ2 . The data have been checked for a possible onset of color transparency and are compared with recent theoretical predictions. [Copyright &y& Elsevier]- Published

If humanity is ever to consider substantial, long-term colonization of Mars, the resources needed are going to be extensive. For a long-term human presence on Mars to be established, serious thought would need to be given to terraforming the planet. One major requirement for such terraforming is having the protection of a planetary magnetic field - which Mars currently does not have. The Earth's magnetosphere helps protect the planet from the potential sterilizing effects of cosmic rays and also helps retain the atmosphere, which would otherwise by stripped by large solar storms as they pass over the planet. Mars does have small patches of remnant surface magnetic field, but these are localized in the southern hemisphere and are not of sufficient size or magnitude to protect the planet or a colony. In this article we explore comprehensively for the first time, the practical and engineering challenges that affect the feasibility of creating an artificial magnetic field capable of encompassing Mars. This includes the concerns that define the design, where to locate the magnetic field generator and possible construction strategies. The rationale here is not to justify the need for a planetary magnetosphere but to put figures on the practicalities so as to be able to weigh the pros and cons of the different engineering approaches. The optimum solution proposed is completely novel, although inspired by natural situations and fusion plasma techniques. The solution with the lowest power, assembly and mass is to create an artificial charged particle ring (similar in form to a 'radiation belt'), around the planet possibly formed by ejecting matter from one of the moons of Mars (in a fashion similar to that which forms the Jupiter-Io plasma torus), but using electromagnetic and plasma waves to drive a net current in the ring(s) that results in an overall magnetic field. With a new era of space exploration underway, this is the time to start thinking about these new and bold future concepts and to begin filling strategic knowledge gaps. Furthermore, the principles explored here are also applicable to smaller scale objects like manned spacecraft, space stations or moon bases, which would benefit from the creation of protective mini-magnetospheres. • A magnetic shield at Mars could help retain atmosphere for terraforming. • The practical and engineering challenges of creating an artificial magnetic field. • Multiple solutions and resources compared. • Start filling strategic knowledge gaps in preparation for colonization. 39. Use of aztreonam in association with cefepime for the treatment of nosocomial infections due to multidrug-resistant strains of Pseudomonas aeruginosa to β-lactams in ICU patients: A pilot study.

Objectives Resistance to all β-lactams is emerging among Pseudomonas aeruginosa (PA) clinical isolates. Aztreonam and cefepime act synergistically in vitro against AmpC overproducing PA isolates. The objective of this study was to evaluate the clinical efficacy of this treatment in ICU patients infected with multidrug-resistant PA. Material and methods Retrospective study (2 years, 2 ICUs) in a tertiary university hospital. Inclusion criteria were proven infection with evidence of a bacterial strain of PA resistant to all β-lactams and treated with the association of at least aztreonam plus cefepime. Treatment was considered effective for pneumonia using CPIS scores at the end of treatment and for other infections, using the SOFA score and signs of infection improvement at the end of treatment. Infectious episodes were classified as cure or failure. Results Thirteen patients were included (10 nosocomial pneumonia, 3 nosocomial intra-abdominal infections). The median [25th–75th percentiles] admission SAPS2 score was 54 [51–69] and the median SOFA score at the beginning of infection was 7 [4–8]. The median CPIS scores for pneumonia at the beginning and end of treatment were 9 [7–10.5] and 2 [0.75–5.5]. The duration of treatment with the combination of aztreonam plus cefepime was 14 days [9.5–16]. Nine episodes were classified as cures and 4 as failures, indicating a clinical efficacy of 69.2%. Overall mortality was 38.5%. Discussion These data suggest that the association of cefepime plus aztreonam could be an attractive alternative in the treatment of infections with multidrug-resistant PA to all β-lactams with a clinical efficacy rate of 69%. Abstract: Objectives: The FloTrac® Vigileo® (FTV) estimates cardiac output (CO) on the basis of an uncalibrated arterial pressure waveform. To assess the ability of the third-generation of FTV (v.3.02) to track changes in CO following norepinephrine dose adjustment in patients with septic shock, we performed a comparative study using Doppler echocardiography (DE). Study design: Prospective observational study. Patients: We prospectively included 20 mechanically ventilated patients receiving norepinephrine and monitored with the FTV. Five minutes after each change in norepinephrine dose (decided by the attending physician), CO was measured simultaneously with the FTV (COFTV) and DE (CODE). The changes in CO were compared. ROC curves were built to assess the ability of FTV to detect significant changes in CODE of at least 15%. Results: Ninety pairs of CO variations measurements were made. The intertechnique correlation coefficient for changes in CO of at least 15% was r =0.59; P =0.0009. The AUC of a ROC curve built to test the FTV''s ability to detect a CODE increase of 15% or more was 0.783 (±0.083) (P =0.005). A COFTV threshold value of 15% had a sensitivity of 54% (25–81) and a specificity of 87% (77–94). For a CODE decrease of 15% or more, the ROC curve had an AUC of 0.616 (±0.075) (P =0.12) and a COFTV threshold value of 13% yielded a sensitivity of 53% (27–79) and a specificity of 72% (60–82). Conclusions: The FTV was unable to accurately track changes in CO following norepinephrine dose adjustments in critically ill patients with septic shock. [Copyright &y& Elsevier]

Introduction Les patients sont fréquemment douloureux en réanimation ce qui impacte sur leur devenir [1] . La douleur nociceptive est au premier plan mais il existe aussi des douleurs neuropathiques (DN) et leur traitement est très spécifique. Le questionnaire de dépistage, la douleur neuropathique en 4 questions (questionnaire DN4), est validé. Il a été utilisé pour déterminer la prévalence de la DN en France chez les douloureux chroniques (6,9 %) [2] . Aucune étude n’a été réalisée en réanimation pour déterminer l’incidence de la DN. L’objectif de cette étude était de définir celle-ci par l’intermédiaire du questionnaire DN4. Matériel et méthodes Une étude prospective observationnelle a été menée de décembre à février 2014 dans les services de réanimation du CHU d’Amiens. L’étude a été approuvée par le comité d’éthique. Les critères d’inclusion étaient : tout patient majeur avec une durée de ventilation supérieure à 24 h, sans antécédent de DN. Les patients étaient exclus si le DN4 n’était jamais faisable. Le questionnaire DN4 était réalisé chaque jour jusqu’à la sortie ou le décès du patient. Le DN4 était considéré positif si le score était supérieur ou égal à 4 sur 10. Il était noté non faisable si le patient n’était pas en capacité d’y répondre. Un patient était considéré ayant présenté une dn (groupe DN +) s’il présentait au moins un DN4 ≥ 4. Les autres patients étaient considérés sans dn (groupe DN–). Les données sont présentées en médiane (écart interquartile) ou nombre (pourcentage). Les groupes ont été comparés par un test de Mann Whitney ou par un test de Fisher selon les données. Résultats Quatre cent cinquante-six patients ont été admis en réanimation pendant l’étude, 322 patients étaient non inclus car essentiellement ventilés moins de 24 h, 55 patients étaient exclus et 79 était inclus. L’incidence de la DN était de 19 % chez les patients inclus. Cinq (6 %) patients avaient un DN4 positif isolé et 10 (13 %) patients avait toujours un DN4 positif après le premier DN4 posititivé. Le DN4 n’avait jamais été faisable dans 41 % des cas. Les principaux résultats sont présentés dans la Fig. 1 . Discussion L’incidence de la DN en réanimation était de 19 % dans notre étude. Parmi ceux-ci, 13 % des patients installent une DN au long cours et les autres des DN probablement transitoires. Les patients touchés sont essentiellement chirurgicaux et traumatisés graves. - Abstract
Introduction Les extubations accidentelles (EA) en réanimation sont associées à une augmentation de la morbidité ou de la mortalité hospitalière selon les études. L’objectif de cette étude est de déterminer les facteurs de risque et le pronostic des EA. Matériel et méthodes Il s’agit d’une étude cas-témoins non-interventionnelle dans un service de réanimation polyvalente. Trente patients consécutifs avec une EA entre août 2013 et mars 2014 ont été comparés à 60 patients témoins tirés au hasard parmi la population de patients intubés au moment de l’événement. Résultats Trente patients parmi les 170 patients intubés ont été accidentellement extubés (17 %) ; 33 % ont été réintubés. Ces patients sont en majorité de sexe masculin (86,7 %), âgés en moyenne de 52,4 ± 17,2 ans et présentent un IGS II moyen de 47,8 ± 17,9. Les EA surviennent préférentiellement pendant les horaires de garde (73,3 %). L’analyse multivariée met en évidence que le sexe masculin (8,7 [1,34–57,1] ; p = 0,02), un IGS II plus faible (1,06 [1,01–1,10] ; p = 0,017), un score de Glasgow plus bas à l’extubation (1,71 [1,21–2,40] ; p = 0,002) et une durée de ventilation mécanique plus courte avant l’extubation (1,22 [1,08–1,38] ; p = 0,001) sont associés à un plus grand taux d’EA. Les patients EA ne développent pas plus de pneumopathies après extubation que les patients témoins ( p = 0,66). Pour les patients EA non réintubés (66 %), la durée de ventilation mécanique totale (6,7 j ± 7,2 vs 16,3 j ± 12,2 ; p = 0,002) et la durée de séjour en réanimation (12,2 ± 9,6 vs 21,8 ± 16,7 ; p = 0,0001) sont significativement diminuées par rapport à la population témoin. Pour les patients EA réintubés (33 %), il n’y a pas de différence significative de la durée totale de ventilation mécanique ni des durées de séjour (en réanimation et à l’hôpital) par rapport à la population témoin. Discussion Le taux d’EA (17 %) dans cette étude est élevé. Cependant, il n’y a pas d’impact négatif sur la morbi-mortalité par rapport à la population témoin. Les patients EA non réintubés bénéficient d’une durée de ventilation mécanique plus courte et d’une sortie plus rapide du service de réanimation. [ABSTRACT FROM AUTHOR]
