Abstract

Endodontic infections are polymicrobial; however, investigating the role of bacterial species is critical because they may influence pathogenesis, the development of symptoms, or the persistence of disease. This systematic review aimed to determine the prevalence of Fusobacterium species and its association with different types of endodontic infections. MEDLINE (Ovid), PubMed, Scopus, Web of Science, and Cochrane Library databases were used as electronic databases to retrieve relevant studies. The studies were evaluated for eligibility criteria, and the certainty in evidence and risk of bias were evaluated using critical appraisal tools for prevalence studies from the Joanna Briggs Institute. Forty studies were selected for meta-analysis and statistically analyzed for the relationship between the prevalence of Fusobacterium species and both the presence of symptoms and the type of infections (primary vs secondary/persistent) using meta-regression analysis. The prevalence of Fusobacterium spp. in endodontic infections ranged from 3%–100% (mean = 42.51%) in the 40 included studies. Calculated confidence intervals indicated that the presence of Fusobacterium spp. was not statistically associated with the presence of symptoms or with the type of infections (the set of 2 predictors was not significant; P <.05). The prevalence of Fusobacterium infection, which was identified with molecular methods, was not significant for overall regression using both predictors (ie, symptoms [symptomatic vs asymptomatic] and types of infections [primary vs secondary/persistent]). Abstract

We report for the first time the combination of inline process analytical technology and optical imaging to track and understand different cleaning mechanisms governing the cleaning process of Olanzapine. Clean-in-Place process parameters were studied through simultaneous inline process analytical technology and image analysis in a film flow apparatus. Methanol and water were compared as cleaning agents, and the effect of flow (0.5—1 L.min−1) and temperature (20–40 °C, and 60 °C for water) was investigated. The cleaning process was assessed in terms of cleaning time, volumetric efficiency, surface residue and governing phenomena. Raman spectroscopy was used to prove no degradation occurred during cleaning. Temperature increase improves the efficiency even when solubility is negligible. High variability was observed when detachment behaviour is dominant, and it is then advisable to prioritize dissolution in the final clean-in-place steps to improve repeatability. Results show that the cleaning process is governed by an interplay of dissolution and mechanical shear phenomena and is more efficient when solubility is significant. The combination of in-situ process analytical technology and optical imaging analysis during process development allows for the determination of the most efficient and repeatable conditions, which in turn significantly reduces solvent usage in pharmaceutical cleaning. Abstract

Eliminating pollutants from wastewater is a crucial approach to securing the global water supply. It is well known that membrane purification technologies are remarkably attractive due to their high efficacy and low energy consumption. The neat polysulfone (PSF) polymer membranes, though, are prone to be fouled by organic pollutants, causing pore blockage and waned separation performance. We herein report and describe the rational design of a hierarchical porous Ag-Cu dendrites@SiO 2 core-shell nanostructure/PSF mixed matrix membranes (MMMs) by coupling heterogeneous dendritic Ag-Cu@SiO 2 core-shell nanostructure with PSF polymer matrix using facile phase inversion method for efficient separation of the protein (Bovine Serum Albumin; BSA) and Rose Bengal (RB) dye removal. Besides, for the first time, a theoretical mapping of molecular electrostatic potential (MESP) maps for chemical structure has been used for studying the interaction between the compounds (membrane moieties and pollutants). Practically, the SiO 2 nanoparticles provide excellent hydrophilicity causing a sequential efficient, antifouling protein and dye separation with an outstanding rejection rate of over 96 % for both BSA and RB. In addition, the porous hierarchical scaffold formed from the hydrophilic Ag-Cu nanosheets embedded with SiO 2 nanoparticles greatly assists the rapid permeation of water with a high flux rate of up to 50 L m−2 h−1 bar−1 for pure water. Furthermore, the hydrophilic nature of Ag and Cu nanoparticles also renders this membrane with a high removal capability for dye blocking enabling it to remove soluble pollutants in molecular dimensions as well. This design strategy not only provides an outstanding membrane for water purification but also sheds light on the design of multi-purpose functional membranes for a variety of energy and environment-related applications. Abstract

Longitudinal sacral fractures are usually a matter of controversy regarding decision-making for reduction, fixation, and approach. Percutaneous and minimally invasive techniques present perioperative difficulties, but with fewer postoperative complications compared to open techniques. The objective of this study was to compare the functional as well as radiological outcomes of the Transiliac Internal fixator (TIFI) versus Iliosacral screw (ISS) fixation of sacral fractures applied percutaneously in a minimally invasive technique. A Prospective comparative cohort study was conducted in a level 1 trauma center in a university hospital. The study included 42 patients with complete sacral fractures, 21 patients have been allocated to each group (TIFI group & ISS group). The clinical, functional, as well as radiological data, were collected and analyzed for the 2 groups. The mean age was 32 (18 -54 years), and the mean follow-up was 14 (12 -20 months). There was a statistically significant difference in favor of the TIFI group regarding a shorter operative time (P = 0.04) as well as less fluoroscopy time (P = 0.01) whereas there was less blood loss in the ISS group (P = 0.01). Both the mean Matta's radiological score, the mean Majeed score as well as the pelvic outcome score were comparable between the 2 groups with no statistically significant difference. This study suggests that both TIFI and ISS through a minimally invasive technique represent valid methods for sacral fracture fixation with a shorter operative time, less radiation exposure in TIFI and less blood loss in the ISS. Abstract

This paper deals with fault detection and isolation (FDI) in complex embedded wired communication networks. The considered faults are soft faults which do not prevent the communication, but may evolve into hard faults, i.e. short or open circuit. A novel FDI method based on power line communication (PLC) transmission systems is proposed. In these PLC systems, the transmission coefficients between the source and each receiver are estimated for communication purposes using orthogonal frequency division multiplexing (OFDM). Health indicators and residuals are computed by comparing the online estimated transmission coefficients with the reference coefficients. A methodology for dealing with complex networks, such as bus networks, is proposed. It is based on the decomposition of the network into several Y-shaped sub-networks. Each of these sub-networks is monitored to detect the presence of a fault. The FDI method is first validated using real data extracted from a Y-shaped network test bench. Then, the proposed approach is validated on a more complex network using realistic simulated data. • Power line communication systems estimate the network transmission coefficients. • Residuals are computed to compare the estimated and reference transmission coefficients. • The residuals are sensitive to cable soft faults i.e. degradations and may be used to detect these faults. • Complex networks s.t. bus networks are decomposed into multiple Y-shaped subnetworks. • The faulty branch of the network is located. 50. A randomized controlled pilot trial of etanercept and alpha-1 antitrypsin to improve autologous islet engraftment.

Abstract

In total pancreatectomy with islet auto-transplantation, successful diabetes outcomes are limited by islet loss from the instant blood mediated inflammatory response. We hypothesized that blockade of the inflammatory response with either etanercept or alpha-1-antitrypsin would improve islet function and insulin independence. We randomized 43 participants to receive A1AT (90 mg/kg x 6 doses, n = 13), or etanercept (50 mg then 25 mg x 5 doses, n = 14), or standard care (n = 16), aiming to reduce detrimental effects of innate inflammation on early islet survival. Islet graft function was assessed using mixed meal tolerance testing, intravenous glucose tolerance testing, glucose-potentiated arginine-induced insulin secretion studies, HbA1c, and insulin dose 3 months and 1 year post-TPIAT. We observed the most robust acute insulin response (AIRglu) and acute C-peptide response to glucose (ACRglu) at 3 months after TPIAT in the etanercept-treated group (p ≤ 0.02), but no differences in other efficacy measures. The groups did not differ overall at 1 year but when adjusted by sex, there was a trend towards a sex-specific treatment effect in females (AIRglu p = 0.05, ACRglu p = 0.06), with insulin secretion measures highest in A1AT-treated females. Our randomized trial supports a potential role for etanercept in optimizing early islet engraftment but it is unclear whether this benefit is sustained. Further studies are needed to evaluate possible sex-specific responses to either treatment.
