Abstract

For a hydrogen-based economy, safe and efficient hydrogen storage is essential. Compared to other chemical hydrogen storage technologies, such as ammonia or methanol, liquid organic hydrogen carrier (LOHC) systems allow for a reversible storage of hydrogen while being easy to handle in a diesel-like manner. In our contribution, we describe for the first time the successful utilization of the exhaust gas enthalpy of a porous media burner to directly supply the dehydrogenation heat for a kW-scale dehydrogenation of the hydrogen-rich LOHC compound perhydro dibenzyltoluene (H18-DBT). Our setup demonstrates the dynamics of the dehydrogenation unit at a realized maximum hydrogen power of 3.9 kW th , based on the lower heating value of the released hydrogen. For the intended applications with fluctuating hydrogen demand, e.g. a hydrogen refueling station (HRS) or stationary heating in buildings, a dynamic hydrogen supply from LOHC is important. Methane, e.g. from a biogas plant, is utilized in our scenario as a fuel source for the burner. Hydrogen is released within 30 min after cold start of the system. The dehydrogenation unit exhibits a power density relative to the reactor volume of about 0.5 kW therm l−1 based on the lower heating value of the hydrogen and a catalyst productivity of up to 0.65 g H2 g Pt −1 min−1 for hydrogen release from H18-DBT. An analysis of the by-products and reaction intermediates shows low by-product formation (e.g. maximum 0.6 wt.-% for high boilers and 0.9 wt.- % for low boilers) and uniform distribution of intermediates after the reaction. Thus, a relatively homogeneous temperature distribution and a uniform LOHC flow in the reaction zone can be assumed. Abstract

Background: Avoiding in-hospital transmissions has been crucial in the COVID-19 pandemic. Little is known on the extent to which hospital-acquired SARS-CoV-2 variants have caused infections in Germany.Aim: To analyse the occurrence and the outcomes of HAI with regard to different SARS-CoV-2 variants.Methods: Patients with SARS-CoV-2 infections hospitalized between March 1st, 2020 and May 17th, 2022 in 79 hospitals of the Helios Group were included. Information on patients' characteristics and outcomes were retrieved from claims data. In accordance with the Robert Koch Institute, infections were classified as hospital-acquired when tested positive >6 days after admission and if no information hinted at a different source.Findings: In all, 62,875 SARS-CoV-2 patients were analysed, of whom 10.6% had HAI. HAIs represented 14.7% of SARS-CoV-2 inpatients during the Wildtype period, 3.5% during Alpha (odds ratio: 0.21; 95% confidence interval: 0.19-0.24), 8.8% during Delta (2.70; 2.35-3.09) and 10.1% during Omicron (1.10; 1.03-1.19). When age and comorbidities were accounted for, HAI had lower odds for death than community-acquired infections (0.802; 0.740-0.866). Compared to the Wildtype period, HAIs during Omicron were associated with lower odds for ICU (0.78; 0.69-0.88), ventilation (0.47; 0.39-0.56), and death (0.33; 0.28-0.40).Conclusion: Hospital-acquired SARS-CoV-2 infections occurred throughout the pandemic, affecting highly vulnerable patients. Although transmissibility increased with newer variants, the proportion of HAIs decreased, indicating improved infection prevention and/or the effect of immunization. Abstract

Liquid organic hydrogen carrier (LOHC) systems offer a promising way to store and transport hydrogen in a safe and dense form at ambient conditions. Compared to other chemical hydrogen storage systems such as ammonia, formic acid, and methanol, LOHC systems enable reversible storage of hydrogen. For the endothermic hydrogen release reaction, heat and a suitable catalyst are needed. In this work for the first time, the exhaust gas enthalpy of a porous media burner is utilized to supply the heat for dehydrogenation of the LOHC compound perhydro dibenzyltoluene (H18-DBT). This approach significantly increases the system dynamics and the power density of the dehydrogenation unit compared to conventional heating via a heat transfer fluid. A dynamic hydrogen release is important to satisfy the fluctuating hydrogen demand, especially in hydrogen refuelling stations (HRS) or in stationary heating applications, e.g., for buildings. Our setup utilized methane as fuel source for the burner. The key challenge is the precise temperature control of the direct heating of the LOHC in the dehydrogenation unit. This is required as hot spots may result in thermal decomposition of the LOHC compounds, while cold spots limit the rate of the dehydrogenation reaction. With our laboratory-scale setup, we demonstrate a hydrogen release from H18-DBT of up to 450 ml N min−1 with 8.2 g of Pt on alumina catalyst (0.3 mass % Pt loading, i.e., 25 mg of Pt). Hydrogen is released within 10 min after a cold start of the entire system. The system exhibits a catalyst productivity of up to 1.5 g H2 g Pt −1 min−1 and a power density relative to the reactor volume of about 3.6 kW therm L−1 based on the lower heating value of hydrogen. Abstract

Background: Pulmonary vein (PV) stenosis represents a rare but serious complication following radiofrequency ablation of atrial fibrillation with a comprehensive diagnosis including morphological stenosis grading together with the assessment of its functional consequences being imperative within the relatively narrow window for therapeutic intervention. The present study determined the clinical utility of a combined, single-session cardiovascular magnetic resonance (CMR) imaging protocol integrating pulmonary perfusion and PV angiographic assessment for pre-procedural planning and follow-up of patients referred for interventional PV stenosis treatment. Methods: CMR examinations (cine imaging, dynamic pulmonary perfusion, three-dimensional PV angiography) were performed in 32 consecutive patients prior to interventional treatment of PV stenosis and at 1-day and 3-months follow-up. Degree of PV stenosis was visually determined on CMR angiography; visual and quantitative analysis of pulmonary perfusion imaging was done for all five lung lobes. Results: Interventional treatment of PV stenosis achieved an acute procedural success rate of 90%. Agreement between visually evaluated pulmonary perfusion imaging and the presence or absence of a ≥ 70% PV stenosis was nearly perfect (Cohen's kappa, 0.96). ROC analysis demonstrated high discriminatory power of quantitative pulmonary perfusion measurements for the detection of ≥ 70% PV stenosis (AUC for time-to-peak enhancement, 0.96; wash-in rate, 0.93; maximum enhancement, 0.90). Quantitative pulmonary perfusion analysis proved a very large treatment effect attributable to successful PV revascularization already after 1 day. Conclusion: Integration of CMR pulmonary perfusion imaging into the clinical work-up of patients with PV stenosis allowed for efficient peri-procedural stratification and follow-up evaluation of revascularization success. 39. Access to Surgery and Quality of Care for Acute Cholecystitis During the COVID-19 Pandemic in 2020 and 2021 - an Analysis of 12,545 Patients from a German-Wide Hospital Network.

Abstract

Purpose: To determine effects on admission, treatment, and outcome for acute cholecystitis during the course of the COVID-19 pandemic in 2020 and 2021.Methods: Retrospective analysis of claims data from 74 German hospitals. Study periods were defined from March 5, 2020 (start of first wave) to June 20, 2021 (end of third wave) and compared to corresponding control periods (March 2018 to February 2020). All in-patients with acute cholecystitis were included. Distribution of cases, type of surgery, comorbidities, surgical outcome, and length of stay of all cases with acute cholecystitis and cholecystectomy were compared. In addition, we analyzed the type of treatment (non-surgical, cholecystostomy, or cholecystectomy) for all cases with main diagnosis of acute cholecystitis.Results: We could not demonstrate differences in daily admissions over the course of the pandemic (11.2-12.7 patients vs. 11.9-12.6 patients for control periods). Proportion of patients with non-surgical treatment was low and not increased (11.7-17.3% vs. 14.5-18.4%). Cholecystostomy was rare throughout all periods (0-0.5% of all patients). We did not observe an increase in open surgery (proportion of open cholecystectomies 3.4-5.5%). Mortality was generally low (1.5-1.9%) with no differences between periods. Median length of stay was 4 days throughout all periods.Conclusion: The numerous restrictions during the COVID-19 pandemic did not result in an increase of admissions or surgery for acute cholecystitis. Laparoscopic cholecystectomy has been safely applied during the pandemic. 50. Cholecystectomies in the COVID-19 Pandemic During and After the First Lockdown in Germany: an Analysis of 8561 Patients.

Abstract

Purpose: The COVID-19 pandemic has led to global changes in healthcare systems. The purpose of this study was to investigate the effects on surgical care of patients. Methods: We performed a retrospective analysis of routine data from the largest hospital group in Germany (68 acute hospitals). Included were inpatients who underwent cholecystectomy between March 19, 2020 (beginning of the first lockdown in Germany) and September 22, 2020. These patients were compared with those treated in the same interval in 2019. Results: In the 2020 study period, 4035 patients met the inclusion criteria (2019: 4526 patients). During the first lockdown, there was a significant reduction in the number of cholecystectomies performed (51.1% decrease). More patients with a higher risk profile underwent urgent operations, which were accompanied by a significant increase in conversion from laparoscopic to open cholecystectomy. The patients were treated as inpatients for a longer duration than 2019, and the mortality rate increased significantly to 1.3% (2019: 0.1%). The complication rate also showed a significant increase. After the end of the first lockdown, daily admission rates normalized very quickly. However, it was not possible to fully address the backlog of operations. Conclusion: There is still a "patient stagnation" 6 months after the first German lockdown. Extrapolated to the national level, this corresponds to almost 21,000 fewer cholecystectomies performed in Germany in 2020. It remains to be seen whether surgical rates will return to pre-pandemic levels and whether complications will arise in the future due to the lack of operations. [ABSTRACT FROM AUTHOR]
