Ethnopharmacological relevance Crinum macowanii is a deciduous bulbous plant which grows in east, central and southern Africa. Crinum macowanii has been used as herbal medicine by the indigenous people of east and southern Africa has for several centuries. The bulb, leaves and roots of C. macowanii are reported to possess diverse medicinal properties and used to treat or manage various human and animal diseases and ailments throughout its distributional range. Crinum macowanii is used traditionally as a remedy for boils, diarrhoea, fever, inflammation, respiratory system problems, skin rashes, tuberculosis, wounds and urinary tract problems. Aim of the review The present review aims to summarize comprehensively the research that has been done on the ethnomedicinal uses, botany, phytochemistry and pharmacology of C. macowanii in different locations throughout its geographical range in east, central and southern African region so as to highlight research gaps and provide a foundation for further investigations on the plant species. Materials and methods A review of the literature was undertaken and an in-depth analysis of previous research on ethnobotany, therapeutic value, phytochemistry and pharmacology of C. macowanii throughout its distributional range in east, central and southern Africa. Literature sources included papers published in international journals, reports from international, regional and national organizations, conference papers, books, theses, websites and other grey literature. Electronic search engines such as Google, Google scholar, publishing sites such as Elsevier, scienceDirect, BMC, PubMed and other scientific database sites such as ChemSpider, PubChem were used as well as searching the library collections of the National Herbarium and Botanic Gardens (SRGH), Harare, Zimbabwe and the University of Fort Hare, South Africa. Results A total of 32 ethnomedicinal uses of C. macowanii are documented in literature, which can be grouped into seven major ethnomedicinal general purpose usages, namely “fever”, “wounds, sores and skin rashes”, “boils and inflammation”, “respiratory system problems”, “blood cleansing”, “urinary tract problem” and “veterinary uses”. The chemical composition of C. macowanii is dominated by various isoquinoline alkaloids, which have been isolated from the bulbs, flowering stalks, leaves and roots. Major biological activities demonstrated by C. macowanii include antifungal, antiviral and antiplasmodial activities, cardiovascular effects as well as effects on the central nervous system. The population of C. macowanii is declining in the wild as the bulbs are over-collected for sale in medicinal ( muthi ) markets in southern Africa. Conclusion A literature search revealed that C. macowanii has a lot of potential as a possible source of pharmaceutical products for the treatment of a wide range of human and animal diseases and ailments. Some of the alkaloids isolated from C. macowanii have demonstrated various biological activities when investigated in in vitro assays. However, some of the ethnomedicinal uses of C. macowanii still require pharmacological investigations. Therefore, further studies are required to improve our knowledge about the mechanisms of action, efficacy, toxicity and clinical relevance of the plant species as well as its bioactive compounds. Abstract Background Traditional Medicine Practices (TMP) which are premised on indigenous knowledge and experiences within a local context of the culture and environment, are common place in low income countries. In Africa and in Uganda specifically, nearly 80% of the Ugandan population relies on TMP for the care of their mental health but they also use Modern Medicine. There are areas of departure between Traditional and Modern Medical practices in Africa that have been cited. What has attracted less research attention, are the areas of convergence. Objective This paper aims to critically examine the link between Modern Medicine and Traditional Healing Practices in Africa, citing Uganda as case example. Method A Narrative literature review with critical element assessment was undertaken to identify documented points of departure, areas of common practice, and ways in which the two models can co-exist and work together through a carefully thought out integration. Results Points of departure between Modern Medicine and Traditional Medicine Practices are philosophical underpinnings of both practices, training of practitioners, and methods and ethics of work. Common areas of practice include human rights perspective, descriptions of mental illnesses, clinical diagnostic practice, particularly severer forms, intellectual property rights, and cross prescriptions. Exhibiting cultural humility and responsibility on the side of the Modern Medicine Practitioners is one of the ways to work together with TMPs. Conclusion Points of departure are more documented and explicit and overshadow areas of common practice while the links between the two are mainly implicit but sadly unrecognized. Mental disorders are disorders of the brain and in neuroscience; the brain is culturally and socially constructed. Sociocultural issues therefore cannot be divorced from disorders of the brain and their management. For better patient outcome and patient-centered approach of care, it is necessary to acknowledge and enhance the links in teaching, clinical and policy level and carry out research on how the links could be improved. Abstract: The genus Artemisia consists of about 500 species, occurring throughout the world. Some very important drug leads have been discovered from this genus, notably artemisinin, the well known anti-malarial drug isolated from the Chinese herb Artemisia annua. The genus is also known for its aromatic nature and hence research has been focussed on the chemical compositions of the volatile secondary metabolites obtained from various Artemisia species. In the southern African region, A. afra is one of the most popular and commonly used herbal medicines. It is used to treat various ailments ranging from coughs and colds to malaria and diabetes. Although it is one of the most popular local herbal medicines, only limited scientific research, mainly focussing on the volatile secondary metabolites content, has been conducted on this species. The aim of this review was therefore to collect all available scientific literature published on A. afra and combine it into this paper. In this review, a general overview will be given on the morphology, taxonomy and geographical distribution of A. afra. The major focus will however be on the secondary metabolites, mainly the volatile secondary metabolites, which have been identified from this species. In addition all of the reported biological activities of the extracts derived from this species have been included as well as the literature on the pharmacology and toxicology. We aim at bringing together most of the available scientific research conducted on this species, which is currently scattered across various publications, into this review paper. Prunus africana (Hook.f.) Kalkman (Rosaceae), commonly known as "Pygeum" or "African cherry", occurs in mainland montane forests scattered across sub-Saharan Africa, Madagascar, and some surrounding islands. Traditionally, decoctions of the stem-bark are taken orally for the treatment of a wide variety of conditions, such as benign prostatic hyperplasia (BPH), stomach ache, chest pain, malaria, heart conditions, and gonorrhoea, as well as urinary and kidney diseases. The timber is used to make axe handles and for other household needs. The dense wood is also sawn for timber. The fragmented information available on the ethnobotany, phytochemistry, and biological activities of the medicinally important P. africana was collated, organised, and analysed in this review, to highlight knowledge voids that can be addressed through future research. A bibliometric analysis of research output on P. africana was conducted on literature retrieved, using the Scopus® database. The trend in the publications over time was assessed and a network analysis of collaborations between countries and authors was carried out. Furthermore, a detailed review of the literature over the period 1971 to 2021, acquired through Scopus, ScienceDirect, SciFinder, Pubmed, Scirp, DOAJ and Google Scholar, was conducted. All relevant abstracts, full-text articles and various book chapters on the botanical and ethnopharmacological aspects of P. africana , written in English and German, were consulted. A total of 455 documents published from 1971 to 2021, were retrieved using the Scopus search. Analysis of the data showed that the majority of these documents were original research articles, followed by reviews and lastly a miscellaneous group comprising conference papers, book chapters, short surveys, editorials and letters. Data were analysed for annual output and areas of intense research focus, and countries with high research output, productive institutions and authors, and collaborative networks were identified. Prunus africana is reported to exhibit anti-inflammatory, analgesic, antimicrobial, anti-oxidant, antiviral, antimutagenic, anti-asthmatic, anti-androgenic, antiproliferative and apoptotic activities amongst others. Phytosterols and other secondary metabolites such as phenols, triterpenes, fatty acids, and linear alcohols have been the focus of phytochemical investigations. The biological activity has largely been ascribed to the phytosterols (mainly 3- β -sitosterol, 3- β -sitostenone, and 3- β -sitosterol-glucoside), which inhibit the production of prostaglandins in the prostate, thereby suppressing the inflammatory symptoms associated with BPH and chronic prostatitis. Many of the ethnobotanical assertions for the biological activity of P. africana have been confirmed through in vitro and in vivo studies. However, a disparity exists between the biological activity of the whole extract and that of single compounds isolated from the extract, which were reported to be less effective. This finding suggests that a different approach to biological activity studies should be encouraged that takes all secondary metabolites present into consideration. A robust technique, such as multivariate biochemometric data analysis, which allows for a holistic intervention to study the biological activity of a species is suggested. Furthermore, there is a need to develop rapid and efficient quality control methods for both raw materials and products to replace the time-consuming and laborious methods currently in use. Ethnopharmacological relevance Sokoto is known for its diverse traditional medicinal wealth and international market of traditional medicines in Africa. However, information of the folk knowledge, especially for the treatment of diabetes, is not documented. Aim of the study This survey identified and documented the information on traditional medicinal plants used by Hausa–Fulani community of Sokoto for the treatment of diabetes. Materials and method Demographic data and information about the medicinal plants were collected via administration of semi-structured oral questionnaires. Willing herbal medical practitioners/traditional healers were interviewed. The medicinal plants mentioned by herbalists were collected and authenticated by a taxonomist and the voucher specimens were preserved. Results Fifty one informants across the state divulged information on traditional medicinal plants and practices used in diabetes and revealed 54 species, belonging to 33 families, with Cassia sieberiana being cited the most (19 times) and ranked first (39%). Azadirachta indica , Ficus exasperata and Schwenckia americana ranked second (15%), each cited 8 times. Conclusion The survey documented the rich wealth of knowledge and usage of plants for the treatment of diabetes in Sokoto. The paper will not only serve as a source of information but will also help to make the knowledge accessible for further drug screening and development, and at the same time underlines the need for biodiversity conservation of this traditional wealth. Burns are very common in sub-Saharan Africa and are considered to be a major health care problem. The management of burns in many African countries is challenged by limited financial resources, inaccessible health care facilities, lack of trained professionals and superstition. These limitations are related to the many burned patients seeking treatment from traditional healers. The use of traditional remedies, plant and animal products are seen as an important aspect of burn management as it is both an affordable and respected treatment modality. Despite its popularity, the use of traditional burn care remedies is faced with many challenges as little research has been done on its effectiveness, dosage and adverse reactions. Abstract: Ethnopharmacological relevance: Hypoxis species (Hypoxidaceae) are popular medicinal plants used in the African traditional medicine to treat numerous ailments and are reported to have a wide spectrum of pharmacological properties. In this paper we reviewed and evaluated the traditional uses, pharmacological, phytochemistry and toxicity aspects of the genus Hypoxis. Potential medicinal prospects and possible knowledge gaps and pitfalls are discussed. Materials and methods: A comprehensive and systematic review of literature on Hypoxis species was done using numerous resources such as books and scientific databases that include Pubmed, Scopus, Scirus, Google scholar, Web of Science and others. Information on the various pharmacological and chemical properties of Hypoxis extracts was critically analysed and discussed under the various topics. Results: The literature indicated a broad range of uses, pharmacological and toxicological properties of different Hypoxis species extracts and their relevance to African healthcare systems. Several compounds, mostly glucosides, sterols and sterolins, have been isolated, identified and tested in various in vitro and in vivo models as well as in a couple of clinical trials with a lot of promising prospects reported in some studies. Conclusion: A critical analysis of the available literature and studies identifies positive potential for the future use of Hypoxis species in both traditional and modern medicine and concurrently so with possible pitfalls and research gaps in current knowledge. An integrated and holistic approach to addressing research issues, particularly toxicology aspects could be more effectively applied and incorporate conservation strategies for the species. Abstract: Although African patients use both conventional or modern and traditional healthcare simultaneously, it has been proven that 80% of people rely on African traditional medicine (ATM). ATM includes medical activities stemming from practices, customs and traditions which were integral to the distinctive African cultures. It is based mainly on the oral transfer of knowledge, with the risk of losing critical knowledge. Moreover, practices differ according to the regions and the availability of medicinal plants. Therefore, it is necessary to compile tacit, disseminated and complex knowledge from various Tradi-Practitioners (TP) in order to determine interesting patterns for treating a given disease. Knowledge engineering methods for traditional medicine are useful to model suitably complex information needs, formalize knowledge of domain experts and highlight the effective practices for their integration to conventional medicine. The work described in this paper presents an approach which addresses two issues. First it aims at proposing a formal representation model of ATM knowledge and practices to facilitate their sharing and reusing. Then, it aims at providing a visual reasoning mechanism for selecting best available procedures and medicinal plants to treat diseases. The approach is based on the use of the Delphi method for capturing knowledge from various experts which necessitate reaching a consensus. Conceptual graph formalism is used to model ATM knowledge with visual reasoning capabilities and processes. The nested conceptual graphs are used to visually express the semantic meaning of Computational Tree Logic (CTL) constructs that are useful for formal specification of temporal properties of ATM domain knowledge. Our approach presents the advantage of mitigating knowledge loss with conceptual development assistance to improve the quality of ATM care (medical diagnosis and therapeutics), but also patient safety (drug monitoring). Abstract: Approximately 60 species of Bridelia, (Phyllanthaceae) are found throughout tropical and subtropical regions of the world, mainly in Africa and Asia. Several Bridelia species are used in popular medicines as antiamebic, antianemic, antibacterial, anticonvulsant, anti-diabetic, antidiarrhoeal, antihelmintic, anti-inflammatory, antimalarial, antinociceptive, antiviral, hypoglycemic and for abdominal pain, cardiovascular, gynecological and sexual diseases. The present paper reviews the traditional usage, the biological activities and the correlated chemical compounds of Bridelia species with emphasis on the validation of the ethnopharmacological uses. The findings in some Bridelia species of, for example, gallocatechin-(4′-O-7)-epigallocatechin (1), quercetin (2), myricetin glycosides (5–6), bridelone (11), bridelonine (12), isoflavone may justify the uses of these species against pains in African and Asian traditional medicines. Abstract: Aims of the study: In South Africa, the genus Helichrysum is widely used in traditional medicine. The uses are well documented although renaming of species and the resulting confusing taxonomic nomenclature may cause uncertainty as to which specific species was referred to in some reports. The aim of this paper is to present a collated and coherent overview of the documented traditional uses of Helichrysum species and to update the botanical identity of previously studied species. Materials and methods: Databases (Scifinder, ISI Web of Knowledge) and several books were used to collect in information on South African Helichrysum species. Results: The traditional uses, chemistry and biological activity of Helichrysum species have been summarized. It was attempted to give clarity as to exactly which species is refer to in the ethnobotanical literature. Conclusions: Although a large number of ethnopharmacological uses have been documented and the chemistry of the genus has been studied extensively, only a few South African species have been investigated for their biological activity. Abstract: The aim of this paper is to trace developments in Traditional Medicine (TM) and legislation concerning conservation and use of biodiversity in Africa, with Tanzania as a case study. Based on field trips, interviews with different actors, site visits, and literature we explored the history, current status, re-establishment, and development of TM. A summary of laws and regulations concerning forests, access and benefit sharing is presented. During the last decade the Government of Tanzania put forth legislation to address national health needs, traditional knowledge, and the resource base for TM (e.g., practitioners, biodiversity). Our findings indicate that TM is the most common form of health care, and that the HIV pandemic has highlighted the need to work across health sectors. New legislation has facilitated this need. In Tanzania TM is experiencing a renaissance in being formally recognized, integrated into mainstream health care, formal establishment of practitioners, and gaining the interests of different sectors. More studies on bioactivity, safety, domestication, and sustainability of use of medicinal plants are needed. Development of TM can also, other than making a significant contribution to health care and livelihoods, provide income possibilities. The prevalence and distress caused by erectile dysfunction (ED) to both male and female partners are increasing at a steady rate. ED has now become the most treated sexual disorder for men among young and old age groups due to varying physical and psychological factors. The treatment with synthetic Phosphodiesterase-5 (PDE5) inhibitors are cost-effective but due to adverse effects such as priapism, loss of vision, heart attack and syncope, the daily life patterns of these patients are distressed and hence the need for alternative medicaments or sources are of utmost important. Therefore, the exploration of medicinal plants as PDE5 inhibitors will be worthwhile in tackling the problems as many plant extracts and fractions have been long used as aphrodisiacs and sexual stimulants which may be found to be active against PDE5 enzyme. To provide a review on the different medicinal herbs traditionally used as natural aphrodisiacs, libido or sexual enhancers which are proven for their PDE5 inhibitory effect. Ethnobotanical and scientific information was procured, reviewed and compiled from the literature search of electronic databases and search engines. A total of 97 medicinal plants exhibiting PDE5 inhibitory effect are reviewed in this paper which is supported by preclinical experimental evidence. Among them, 77 plants have been selected according to their traditional and ethnobotanical uses as aphrodisiacs and the rest are screened according to their effectiveness against predisposing factors responsible for ED and sexual dysfunction such as diabetes and hypertension or due to the presence of phytochemicals having structural similarity towards the identified natural PDE5 inhibitors. In addition, sixteen alkaloids, sixty-one phenolics and eight polycyclic aromatic hydrocarbons have been isolated or identified from active extracts or fractions that are exhibiting PDE5 inhibitory activity. Among them, isoflavones and biflavones are the major active constituents responsible for action, where the presence of prenyl group for isoflavones; and the methoxy group at C-5 position of flavones are considered essential for the inhibitory effect. However, the prenylated flavonol glycoside, Icariin and Icariside II isolated from Epimedium brevicornum Maxim (hory goat weed) are the most effective inhibitor, till date from natural sources. Traditional medicines or formulations containing extracts of Ginkgo biloba L., Kaempferia parviflora Wall. ex Baker, Clerodendrum colebrookianum Walp., Eurycoma longifolia Jack and Vitis vinifera L. are also found to be inhibitors of PDE5 enzyme. The review suggests and supports the rational use of traditional medicines that can be further studied for the development of potential PDE5 inhibitors. Many traditional medicines are still used in various regions of Africa, Asia and South America that are poorly characterized and experimented. Despite the availability of a vast majority of traditional formulations as aphrodisiacs or sexual stimulants, there exists a need for systemic evaluation on the efficacy as well as the mechanism of action of the herbal constituents for the identification of novel chemical moieties that can be further developed for maximum efficacy. Abstract: The disease burden in Africa, which is relatively very large compared with developed countries, has been attributed to various factors that include poverty, food shortages, inadequate access to health care and unaffordability of Western medicines to the majority of African populations. Although for ‘old diseases’ knowledge about the right African traditional medicines to treat or cure the diseases has been passed from generation to generation, knowledge about traditional medicines to treat newly emerging diseases has to be generated in one way or another. In addition, the existing traditional medicines have to be continuously improved, which is also the case with Western scientific medicines. Whereas one school of thought supports the idea of improving medicines, be they traditional or Western, through scientific research, an opposing school of thought argues that subjecting African traditional medicines to scientific research would be tantamount to some form of colonization and imperialism. This paper argues that continuing to use African traditional medicines for old and new diseases without making concerted efforts to improve their efficacy and safety is unethical since the disease burden affecting Africa may continue to rise in spite of the availability and accessibility of the traditional medicines. Most importantly, the paper commends efforts being made in some African countries to improve African traditional medicine through a combination of different mechanisms that include the controversial approach of scientific research on traditional medicines.
