A growing body of research supports an association between periapical inflammation and an increased risk of developing systemic diseases. There is currently no scientific evidence to support a causal effect of inflammation on the onset of insulin resistance (IR) in patients with apical periodontitis (AP). The aim of this in vitro study was to evaluate any association between AP and levels of serum inflammatory factors potentially associated with the onset of IR, and to investigate the effect of root canal treatment (RCT) on these systemic inflammation markers and on the response in vitro to insulin. A total of 27 control subjects and 27 patients with AP were enrolled. Patients with AP underwent RCT and were followed-up 6 and 12 months post-treatment. Enzyme-linked immunosorbent assays were used to evaluate serum levels of proinflammatory cytokines interleukin (IL)-1, IL-6, IL-8, and tumor necrosis factor (TNF)-α. The response in vitro to insulin was assessed by measuring glucose consumption in a human pancreatic epithelioid carcinoma cell line treated with sera from healthy and AP subjects. At baseline AP was associated with significant higher levels of IL-1, IL-6, and IL-8 in the serum of untreated (AP) patients vs controls (P <.001). Glucose consumption decreased in pancreatic cells incubated with baseline serum from patients with AP, in a manner proportional to total cytokines amount. Notably, endodontic treatment was associated with reduced levels of cytokines (P <.001) and improved response to insulin in AP group (P <.001). Our findings suggest that AP may promote inflammatory-driven IR in an in vitro model, and that RCT may ameliorate inflammatory mediators in vivo and the cellular response to insulin in vitro. Cardiovascular diseases are the leading cause of mortality worldwide. Apical periodontitis (AP) has been associated with an increased risk of cardiovascular diseases. A correlation has been shown between chronic AP and endothelial dysfunction (ED), but there is no evidence to indicate ED improves after endodontic treatment in patients with periapical lesions. The aim of this study was to investigate vascular and molecular markers of early ED before and after root canal treatment in young adults with chronic AP. Twenty control subjects and 21 patients with AP were assessed at baseline. The AP patients were also evaluated 2 and 12 months post-treatment. Endothelial flow reserve was assessed via an endothelial function test, and enzyme-linked immunosorbent assays were used to evaluate plasma levels of proinflammatory cytokines interleukin (IL)-1, IL-6, and tumor necrosis factor alpha; vasoconstrictor ED marker endothelin (ET)-1; circulating endothelial adhesion markers intercellular adhesion molecule 1 (ICAM-1)/CD54 and soluble vascular cellular adhesion molecule (sVCAM)-1/CD106; soluble CD14; and the endothelial leukocyte adhesion molecule (E-selectin). AP was associated with increased serum levels of ET-1, ICAM-1, E-selectin, IL-1, and sCD14, suggesting early vascular ED, with no macroscopic evidence of a reduction in endothelial flow reserve. Root canal treatment ameliorated inflammation and early ED, lowering plasma levels of IL-1, sCD14, ET-1, ICAM-1/CD54, and E-selectin to those of control subjects. Introduction Contracted endodontic cavities (CECs) have developed from the concept of minimally invasive dentistry and provide an alternative to traditional endodontic cavities (TECs). They have been designed in an effort to preserve the mechanical stability of teeth. The contracted cavity design preserves more of the dentin but may influence the geometric shaping parameters. The aim of this micro–computed tomographic study was to evaluate the influence of contracted endodontic cavities on the preservation of the original root canal anatomy after shaping with nickel-titanium rotary instruments. Methods Thirty extracted human mandibular molars with fully formed apices and independent mesial canals were randomly assigned to group 1 (TEC) and group 2 (CEC). Each group was shaped using ProGlider (Dentsply Maillefer, Ballaigues, Switzerland) and WaveOne Gold (Dentsply Maillefer). Irrigation was performed with 10% EDTA and 5% sodium hypochlorite. Samples were scanned before and after canal shaping to match canal volumes (SkyScan; Bruker microCT, Kontich, Belgium [100 kV, 100 μA, and 15-μm resolution]), and images were analyzed to evaluate canal volumes, surface areas, and centroid shift on cross sections at −1 mm and −3 mm from the apex. Results TECs showed a greater preservation of the original root canal anatomy with less apical transportation than CECs, possibly because of the absence of coronal interferences and, therefore, fewer pecking motions required to complete instrumentation. Conclusions Within the limitations of this study, TECs may lead to a better preservation of the original canal anatomy during shaping compared with CECs, particularly at the apical level. Introduction: A coronal seal is fundamental for a positive outcome to endodontic therapy. In this in vitro study, we evaluated the adaptation of composite resins in postendodontic restorations using optical coherence tomographic (OCT) imaging. Our null hypothesis was that there would be no difference in marginal adaptation to the pulp chamber floor between resin composites of different viscosities. Methods: Thirty intact upper molars extracted for periodontal reasons were selected, endodontically treated, and filled with gutta-percha. The excess gutta-percha was entirely removed from the pulp chamber floor, and teeth were randomly divided into 3 groups (n = 10) according to the material used for the restoration: group 1: 0.5-mm horizontal layer of flowable composite followed by nanohybrid composite, group 2: bulk layering of bulk fill flowable composite; and group 3: oblique layering of nanohybrid composite. The degree of adaptation to the cavity floor was assessed using OCT imaging, and images were analyzed with the software program ImageJ (National Institutes of Health, Bethesda, MD) to assess the marginal gap between the composite and the pulp chamber floor. Collected data were statistically analyzed using analysis of variance testing, and statistical significance was set at P < .05. Results: Flowable composites showed significantly better adaptation than traditional packable nanohybrid composites (P < .05). All significant differences were found between groups 1 and 2. Conclusions: Within the limitations of this OCT imaging-based in vitro study, it was concluded that the flowable composite (flow + nanofilled; flow bulk fill composite) adapted better to the pulp chamber floor than the packable nanohybrid composite resin. Introduction The finite element method (FEM) has been proposed as a method to analyze stress distribution in nickel-titanium (NiTi) rotary instruments but has not been assessed as a method of predicting the number of cycles to failure (NCF). The objective of this study was to predict NCF and failure location of NiTi rotary instruments by FEM virtual simulation of an experimental nonstatic fatigue test. Methods ProTaper Next (PTN) X1, X2, and X3 files (Dentsply Maillefer, Baillagues, Switzerland) ( n = 20 each) were tested to failure using a customized fatigue testing device. The device and file geometries were replicated with computer-aided design software. Computer-aided design geometries (geometric model) were imported and discretized (numeric model). The typical material model of an M-Wire alloy was applied. The numeric model of the device and file geometries were exported for finite element analysis (FEA). Multiaxial random fatigue methodology was used to analyze stress history and predict instrument life. Experimental data from PTN X2 and X3 were used for virtual model tuning through a reverse engineering approach to optimize material mechanical properties. Tuned material parameters were used to predict the average NCF and failure locations of PTN X1 by FEA; t tests were used to compare FEA and experimental findings ( P < .05). Results Experimental NCF and failure locations did not differ from those predicted with FEA ( P = .098). Conclusions File NCF and failure location may be predicted by FEA. Introduction The aim of this micro–computed tomography study was to describe the shaping properties of ProGlider/ProTaper Next (PG/PTN) and ScoutRace/BioRace (SR/BR) nickel-titanium rotary systems. Methods Thirty maxillary first permanent molars were selected. Mesiobuccal canals were randomly assigned ( n = 15) to PG/PTN or SR/BR groups. Irrigation was performed with 5% NaOCl and 10% EDTA. Specimens were scanned (voxel size, 9.1 μm) for matching volumes and surface areas and post-treatment analyses. Root canal centering ability, canal geometry enlargement, and thickness of dentinal wall at inner curvature were assessed at apical level and point of maximum curvature. Results were analyzed with 4 one-way analyses of variance. Results Canal centering ability was superior in PG/PTN ( P = .006 at apical level, P = .025 at point of maximum curvature). PG/PTN demonstrated a more conservative increase of canal areas ( P = .027 at apical level, P = .038 at point of maximum curvature). Centrifugal increase in canal diameters did not significantly differ between groups ( P = .65 at apical level, P = .61 at point of maximum curvature). Inner dentinal wall thickness was less reduced with PG/PTN compared with SR/BR, with no statistical differences ( P = .23 at point of maximum curvature, P = .89 at apical level). PG/PTN shaping taper ranged between 6% and 7%. Conclusions Neither system produced significant shaping errors in curved canals. PG/PTN system showed better preservation of canal anatomy. Introduction Instrument failure caused by excessive torsional stress can be controlled by creating a manual or mechanical glide path. The ProGlider single-file system (Dentsply Maillefer, Ballaigues, Switzerland) was recently introduced to perform a mechanical glide path. This study was designed to compare the effect of a glide path performed with PathFiles (Dentsply Maillefer) and ProGlider on torque, time, and pecking motion required for ProTaper Next X1 (Dentsply Maillefer) to reach the full working length in simulated root canals. Methods Forty Endo Training Blocks (Dentsply Maillefer) were used. Twenty were prepared with a mechanical glide path using PathFiles 1 and 2 (the PathFile group), and 20 were prepared with a mechanical glide path using a ProGlider single file (the ProGlider group). All samples were shaped with ProTaper Next X1 driven by an endodontic motor connected to a digital wattmeter. The required torque for root canal instrumentation was analyzed by evaluating the electrical power consumption of the endodontic engine. Electric power consumption (mW/h), elapsed time (seconds), and number of pecking motions required to reach the full working length with ProTaper Next X1 were calculated. Differences among groups were analyzed with the parametric Student t test for independent data ( P < .05). Results Elapsed time and electric power consumption were significantly different between groups ( P = .0001 for both). ProGlider appears to perform more efficiently than PathFiles in decreasing electric power consumption of ProTaper Next X1 to reach the full working length. Conclusions This study confirmed the ability of ProGlider to reduce stress in ProTaper Next X1 during shaping through a glide path and preliminary middle and coronal preflaring. Abstract: Introduction: This study investigated the influence of clinical experience in relation to the efficacy and effectiveness of removal of 2 different fiber posts. Methods: In total, 48 intact single-rooted teeth were treated endodontically and obturated. Then, 10-mm post spaces were prepared, and fiber posts were luted. Twenty-four #1 D.T. Light-Posts were used in group 1, and 24 #2 Hi-Rem Prosthetic Posts were used in group 2. A pullout test (n = 8 per group) was performed by using a universal testing machine to compare bond strength. Then, fiber post removal efficacy and efficiency were tested. Each group was divided into 2 subgroups (n = 8) according to operator experience. In group 1, fiber posts were removed with an ultrasonic handpiece by using a #3 Start-X tip. In group 2, a size 25/0.04 ProFile was used to remove the central soft polymer macrofiber, and a #2 Largo drill was used to remove the fiber post and luting cement remnants. Post removal times were recorded to evaluate efficacy. Weight change was determined, and post space walls were analyzed microscopically to evaluate effectiveness. Results: Bond strength did not differ significantly (P = .7569) between post systems. Post hoc Tukey tests indicated that removal time was affected significantly by operator experience in group 1 (P < .001) but not in group 2. Weight change was affected significantly by experience level in both groups. No difference in post space wall characteristics was observed between subgroups. Statement of problem: The choice of restorative method is commonly based on the cavity configuration and the residual number of cavity walls. However, the residual wall thickness could be a valuable clinical parameter in the choice of restoration for endodontically treated teeth. Purpose: The fracture resistance of endodontically treated premolars was compared with different wall thicknesses restored with direct composite resin with and without cuspal coverage and with and without fiber post insertion. Material and methods: This study included 104 intact human maxillary premolars extracted for periodontal or orthodontic reasons. Standardized mesio-occluso-distal cavities were prepared with different palatal wall thicknesses (1.5, 2, and 2.5 mm) and a buccal wall thickness of 2 mm. Teeth were restored with or without a fiber post and with or without cuspal coverage. Specimens were subjected to thermocycling (3000 cycles, 5 to 55°C) and embedded in polymerized acrylic resin. Teeth were submitted to cyclic fatigue followed by a static fatigue test with a universal testing machine; a compressive force was applied 30 degrees to the long axis of the teeth until fracture. The results were statistically analyzed by 3-way ANOVA (α=.05). Results: Residual wall thickness (P=.004), the type of adhesive restoration (P<.001), and fiber post insertion (P<.001) significantly influenced the fracture resistance of endodontically treated premolars. Conclusions: In specimens with a cavity wall thickness >2 mm, direct intracuspal composite resin restorations supported by a fiber post achieved comparable fracture resistance. With a residual wall thickness <2 mm, only cuspal coverage with or without a fiber post provided satisfactory fracture resistance. [Copyright &y& Elsevier]

Abstract: Introduction: There is evidence to suggest that an association exists between oral infections and coronary heart disease (CHD). Subjects presenting lesions of endodontic origin (LEOs) or pulpal inflammation had an increased risk of developing CHD. However, findings concerning systemic manifestations of apical periodontitis (AP) remain controversial. An association between CD14 gene polymorphisms and atherosclerosis-associated diseases has been shown, but there are no data regarding an association between CD14 polymorphism and AP. This study evaluated associations between clinical oral health status, CD14 polymorphisms, and CHD. Methods: A case-controlled clinical trial was designed to compare middle-aged adults with acute myocardial infarction or unstable angina (n = 51) within 12 months of the acute event defined as first manifestation with healthy controls (n = 49). Participants were matched for age, sex, and socioeconomic status. Indicators of oral disease and compliance were evaluated. CD14 polymorphisms were analyzed by restriction fragment length polymorphism–polymerase chain reaction. Results: CHD subjects had a higher prevalence of oral diseases and lower compliance to oral preventive strategies than healthy controls. Multivariate analysis showed a positive association between missing teeth (odds ratio [OR] = 1.37; 95% confidence interval [CI], 1.02–1.85), the number of LEOs (OR = 4.37; 95% CI, 1.69–11.28), chronic periodontitis (OR = 5.87; 95% CI, 1.17–29.4), and CHD. No statistically significant association emerged between the CD14 C(−260)T and the CD14 C(−159)T polymorphism, endodontic or periodontal disease, and CHD. Conclusions: Chronic oral diseases may increase the risk of CHD and may be an unconventional risk factor for CHD. [Copyright &y& Elsevier]

Abstract: Introduction: This study compared the canal curvature and axis modification after instrumentation with WaveOne Primary reciprocating files (Dentsply Maillefer, Ballaigues, Switzerland) and nickel-titanium (NiTi) rotary ProTaper (Dentsply Maillefer). Methods: Thirty ISO 15, 0.02 taper, Endo Training Blocks (Dentsply Maillefer) were used. In all specimens, the glide path was achieved with PathFile 1, 2, and 3 (Dentsply Maillefer) at the working length (WL). Specimens were then assigned to 1 of 2 groups for shaping: specimens in group 1 were shaped with ProTaper S1-S2-F1-F2 at the WL and specimens in group 2 were shaped with WaveOne Primary reciprocating files at the WL. Pre- and postinstrumentation digital images were superimposed and processed with Matlab r2010b (The MathWorks Inc, Natick, MA) software to analyze the curvature-radius ratio (CRr) and the relative axis error (rAe), representing canal curvature modification. Data were analyzed with one-way balanced analyses of variance at 2 levels (P < .05). Results: The instrument factor was extremely significant for both the CRr parameter (F1 = 9.59, P = .004) and the rAe parameter (F1 = 13.55, P = .001). Abstract: Introduction: X-ray computed micro-tomography scanning allows high-resolution 3-dimensional imaging of small objects. In this study, micro-CT scanning was used to compare the ability of manual and mechanical glide path to maintain the original root canal anatomy. Methods: Eight extracted upper first permanent molars were scanned at the TOMOLAB station at ELETTRA Synchrotron Light Laboratory in Trieste, Italy, with a microfocus cone-beam geometry system. A total of 2,400 projections on 360° have been acquired at 100 kV and 80 μA, with a focal spot size of 8 μm. Buccal root canals of each specimen (n = 16) were randomly assigned to PathFile (P) or stainless-steel K-file (K) to perform glide path at the full working length. Specimens were then microscanned at the apical level (A) and at the point of the maximum curvature level (C) for post-treatment analyses. Curvatures of root canals were classified as moderate (≤35°) or severe (≥40°). The ratio of diameter ratios (RDRs) and the ratio of cross-sectional areas (RAs) were assessed. For each level of analysis (A and C), 2 balanced 2-way factorial analyses of variance (P < .05) were performed to evaluate the significance of the instrument factor and of canal curvature factor as well as the interactions of the factors both with RDRs and RAs. Results: Specimens in the K group had a mean curvature of 35.4° ± 11.5°; those in the P group had a curvature of 38° ± 9.9°. The instrument factor (P and K) was extremely significant (P < .001) for both the RDR and RA parameters, regardless of the point of analysis. Conclusions: Micro-CT scanning confirmed that NiTi rotary PathFile instruments preserve the original canal anatomy and cause less canal aberrations. [Copyright &y& Elsevier]

Abstract: Introduction: This prospective randomized clinical trial evaluated the incidence of postoperative pain after glide path performed with PathFile (PF) (Dentsply Maillefer, Ballaigues, Switzerland) versus stainless-steel K-file (KF). Methods: In 149 subjects, the mechanical glide path was performed with nickel-titanium (NiTi) rotary PF; in 146 subjects, the manual glide path was performed with stainless-steel KFs. Postoperative pain, analgesics consumption, and the number of days to complete pain resolution were evaluated in the following 7 days. An analysis of variance model for repeated measures was used to compare the variation of pain-scale values (P < .05). The Student''s t test for continuous variables normally distributed, the nonparametric Mann-Whitney U test for the nonnormally distributed variables, and the chi-square test for dichotomous variables were used (P < .05). Despite homogeneous baseline conditions at diagnosis, tooth type, pain prevalence, and scores, the postoperative pain prevalence curves in PF group evidenced a more favorable trend in terms of time to pain resolution compared with the KF group (P = .004). The difference was also evident in the model adjusted for analgesics consumption in both groups (P = .012). The mean analgesics intake per subject was significantly higher in the KF group (3.7 ± 2.2) compared with the PF group (2 ± 1.7) (P < .001). Mean pain stop values were also significantly higher in the KF group (2.7) compared with the PF group (1.7) (P = .001). Abstract: Introduction: This study evaluated the working length (WL) modification after instrumentation with WaveOne Primary (Dentsply Maillefer, Ballaigues, Switzerland) reciprocating files and the incidence of overinstrumentation in relation to the initial WL. Methods: Thirty-two root canals of permanent teeth were used. The angles of curvature of the canals were calculated on digital radiographs. The initial WL with K-files was transferred to the matched WaveOne Primary reciprocating files. After glide paths were established with PathFile (Dentsply Maillefer, Ballaigues, Switzerland), canals were shaped with WaveOne Primary referring to the initial WL. The difference between the postinstrumentation canal length and the initial canal length was analyzed by using a fiberoptic inspection microscope. Data were analyzed with a balanced 2-way factorial analysis of variance (P < .05). Results: Referring to the initial WL, 24 of 32 WaveOne Primary files projected beyond the experimental apical foramen (minimum–maximum, 0.14–0.76 mm). A significant decrease in the canal length after instrumentation (95% confidence interval ranging from −0.34 mm to −0.26 mm) was detected. The canal curvature significantly influenced the WL variation (F1 = 30.65, P < .001). The interaction between the initial canal length and the canal curvature was statistically significant (F2 = 4.38, P = .014). Conclusions: Checking the WL before preparation of the apical third of the root canal is recommended when using the new WaveOne NiTi single-file system. [Copyright &y& Elsevier]

Abstract: The study compared changes to canal curvature and incidence of canal aberrations after preflaring with hand K-files or with nickel-titanium rotary PathFile in S-shape Endo Training Blocks. The influence of the operator''s expertise was also investigated. One hundred training blocks were colored with ink, and preinstrumentation images were acquired digitally. Preflaring was performed by an endodontist with PathFile (group 1) and hand stainless steel K-files #10-15-20 (group 2); an inexpert clinician performed preflaring with PathFile (group 3) and hand stainless steel K-files (group 4). Preinstrumentation and postinstrumentation images were superimposed to evaluate the outcomes investigated. Differences in canal curvature modification and incidence of canal aberration were analyzed with the Kruskall-Wallis plus post hoc tests and by the Monte Carlo method, respectively, (P < .05). The PathFile groups demonstrated significantly less modification of curvature (P < .001) and fewer canal aberrations (P < .001). No expertise-related difference was found within instrument groups (P > .05), whereas the inexpert clinician produced more conservative shaping with Pathfiles than did the expert with manual preflaring (P < .01). Abstract: The aim of this study was to compare the effective shaping time and number of rotations required by an endodontist working with hand and rotary ProTaper instruments to completely shape simulated root canals. Eighty Endo Training Blocks (curved canal shape) were used. Manual preflaring was performed with K-Flexofiles #08-10-12-15-17 and #20 Nitiflex at a working length of 18 mm. Specimens were then randomly assigned to 2 different groups (n = 40); group 1 was shaped by using hand ProTaper and group 2 with ProTaper rotary. The number of rotations made in the canal and the effective time required to achieve complete canal shaping were recorded for each instrument. Differences between groups were analyzed with the nonparametric Mann-Whitney U test (P < .05). Hand ProTaper required significantly fewer rotations (P < .001) than rotary ProTaper, whereas the effective working time to fully shape the simulated canal was significantly higher (P < .001) with hand ProTaper. Polymerase chain reaction (PCR) followed by reverse dot blot was used to detect Enterococcus faecalis leakage through mineral trioxide aggregate (MTA) apical obturations of pulpless teeth with open apices. Prepared root canals of 34 extracted teeth were given a standard apical foramen opening and received orthograde apical obturation with MTA; three groups had 1-, 2-, or 3-mm thickness. Sterilized specimens were inoculated with E. faecalis and incubated in sterile medium. DNA extracted from the specimens was amplified by polymerase chain reaction, which yielded a specific segment of E. faecalis 16S rDNA. On day 10 of incubation, no specimens were contaminated. On day 50, almost 17% of specimens were contaminated, with no statistically significant difference between groups (Chi-square = 0.48; df = 2; p = 0.787). Therefore, MTA provides an adequate seal even in cases of orthograde apical obturation of pulpless teeth with open apices. [Copyright &y& Elsevier]

Abstract: Introduction: The study evaluated the efficacy of subsonic agitation of sodium hypochlorite (NaOCl) in reducing bacterial load in the root canal. Methods: Root canals of 112 extracted human single-root teeth were preflared using K-Flexofiles (Dentsply Maillefer, Ballaigues, Switzerland) up to #20 and then shaped using ProTaper S1-S2-F1-F2-F3 (Dentsply Maillefer) at the working length. Irrigation was performed with 33 mL of 5% NaOCl, alternating with 10 mL of 10% EDTA. After ethylene oxide sterilization, the root canals were infected with 30 μL of Enterococcus faecalis culture and randomly assigned to four groups (n = 25) of different irrigation regimens plus positive and negative controls. Irrigation was performed with 2 mL of 5% NaOCl. In the NaOCl 15 group, the irrigant was left in place for 15 seconds, and in the NaOCl 30 group it was left in place for 30 seconds. In the EndoActivator (EA; Dentsply Tulsa Dental Specialties, Tulsa, OK) 15 and EA 30 groups, NaOCl was subsonically agitated with EA for 15 and 30 seconds, respectively. The residual bacterial count was then evaluated. Differences among groups were analyzed with one-way analysis of variance and the post hoc Bonferroni test (p < 0.05). Results: A statistically significant difference was evidenced among groups (F3 = 9.01, p < 0.001). The standard irrigation groups (NaOCl 15 and 30) showed higher microbial counts than the EA 30 group (p < 0.05). Conclusion: Thirty seconds of NaOCl subsonic agitation with EndoActivator appears to be slightly more effective in reducing bacterial load in the root canal compared with NaOCl irrigation alone. We evaluated the influence of manual preflaring and torque on the failure rate of rotary nickel-titanium ProTaper instruments Shaping 1 (S1), Shaping 2 (S2), Finishing 1 (F1), and Finishing 2 (F2). These factors were evaluated using an in vitro method by calculating the mean number of Endo-Training-Blocks shaped before file breakage under different conditions. Group A (S1 on simulators with no preflaring) shaped 10 blocks before failure, group B (S1 on manually preflared simulators) shaped 59 blocks (p < 0.01 versus group A), group C (S2 with low torque) shaped 28 blocks, group D (S2 with high torque) shaped 48 blocks (p < 0.01 versus group C), group E (F1 with low torque) shaped eight blocks, group F (F1 with high torque) shaped 23 blocks (p < 0.01 versus group E), group G (F2 with low torque) shaped four blocks, and group H (F2 with high torque) shaped 11 blocks (p < 0.01 versus group G). Manual preflaring creates a glide path for the instrument tip and is a major determinant in reducing the failure rate of these rotary nickel-titanium files. Apicoectomy of the superior first molar palatine root until recently involved relatively destructive and laborious surgery because of the palatal access route, which not only necessitates an extensive flap, but also the readying of procedures required to deal with a possible hemorrhage from the palatine artery. With the advent of operative microscopy, endodontic surgery has become more precise and less invasive and its technical potential has increased; a possible innovative surgical technique may entail vestibular root access. The possibility of using low-dose, low-cost computed tomography (CT) dedicated to the jaw to obtain anatomic information to plan apicoectomy via the vestibular approach was evaluated; 31 patients were referred to CT with the NewTom apparatus. In 43 superior first molars, the mean distance of the palatine root from the external vestibular cortex was measured, and the frequency that the maxillary sinus lateral recess lay between the roots was determined. The mean root apex-vestibular cortex distance was 9.73 mm. In 25% of cases the maxillary sinus recessus lay between vestibular and palatine roots. CT may play an important role in optimizing palatine root apicoectomy through vestibular access, with regard to precision and preventing complications, with relatively low biological and economic cost, also possibly contributing to the affirmation of this new surgical procedure. To evaluate shear stress (SS) and normal pressure (NP) at the tooth-restoration interface of highly-filled flowable resin composite applied to deep margin elevation technique through FEM analysis generated by a microCT scan. A reference maxillary molar with two class II cavities was prepared according to deep margin elevation protocol. A geometrical model was segmented from a micro-CT scan generating separate volumes of enamel, dentin and restorative materials. The 3D Finite Element (FE) model was subsequently built-up and an axial chewing load was simulated. Data concerning the tooth-restoration interface were analyzed in terms of SS and NP. Different materials and techniques were tested in order to evaluate the effects of the restorative material, the usage of a highly-filled flowable composite as liner and the substrate of the cervical area. Both SS and NP presented similar distribution, but showed significant differences between tested materials. Composites showed more homogeneous behavior in stress distribution compared to ceramic. The use of a highly-filled flowable composite as liner on the cervical margin significantly reduced SS and NP on the cavity floor and the cervical margin area. Lastly, stress distribution in the cavity floor area varied according to the cervical margin substrate: enamel showed a protective role in stress distribution. Highly-filled flowable resin composites showed encouraging results when applied to deep margin elevation from an interfacial mechanical point of view. Further studies are needed to validate these data and to better define the role of cervical enamel in stress distribution. • Micro-CT can be used to create a realistic finite element model of the tooth-restoration complex. • Highly-filled flowable resin reduced shear and normal pressure stresses on the cavity floor. • Shear and normal pressure stresses presented similar distributions, being influenced by the different materials. • Resin-based materials showed a more homogeneous behavior in stress distribution compared to ceramic. Abstract: Objectives: The aim of this study was to investigate the influence of post placement on fracture resistance of endodontically treated mandibular molars restored with adhesive overlay restorations. Methods: Endodontically treated human molars with two- and one-wall cavities either underwent or did not undergo fibre post insertion within composite build-up before cementation of indirect composite onlay restorations. The specimens were thermocycled, exposed to cyclic loading, and submitted to the static fracture resistance test. Fracture loads and mode of failure were evaluated. Results: Statistical analysis revealed that specimens with fibre posts demonstrated similar failure loads (p =0.065) but more favourable fracture patterns compared with specimens without fibre posts. No difference was found between two- and one-wall cavities. Conclusions: Within the limitations of this study, the insertion of fibre posts did not improve support under indirect composite overlays. Clinical significance: When restoring heavily broken down endodontically treated mandibular molars with an indirect overlay composite restoration, the fibre post inserted within the composite build-up do not provide any increase in fracture resistance.
