Introduction: A salt of 1-hydroxyethylidene-1,1-diphosphonic acid (HEDP) was commercialized some years ago. This so-called Dual Rinse (DR) HEDP has been tested for its combined actions and interactions with pure sodium hypochlorite (NaOCl) solutions. The aim of this study was to assess DR interactions with NaOCl solutions containing proprietary additives., Methods: Designated endodontic NaOCl solutions that come at 2 different strengths (2% and 5.25%), and without or with additives (Chloraxid and Chloraxid Extra, respectively) were tested. Some of their key chemical and physical properties were determined in the presence or absence of HEDP (9%) in the solution. In addition, soft tissue dissolution efficacy of test and control solutions was assessed in extracted teeth (n = 10 per group, 90 teeth in total)., Results: The NaOCl content in the 2% Chloraxid solutions was slightly above, that in the 5.25% solutions below the labeled concentration. The additives in the Chloraxid Extra solutions neither altered their surface tension nor their viscosity. The addition of HEDP to any of the tested NaOCl solutions reduced their pH, and increased their surface tension and viscosity. HEDP-induced available chlorine loss over the first hour was similar between all NaOCl solutions. Soft tissue dissolution in the root canals was affected by NaOCl concentration, but neither by any proprietary NaOCl additive in the Chloraxid solutions, nor the addition of DR HEDP., Conclusions: Proprietary additives had no influence on any of the tested parameters, including surface tension of the "Extra" solutions. Aim: This single-arm interventional trial aimed to investigate the efficacy of ultrasonic irrigation as a supplementary disinfection approach after chemomechanical procedures using molecular techniques based on ribosomal RNA (rRNA) and rRNA genes (referred to as DNA)., Methodology: Samples were collected from 35 single-rooted teeth with radiographic evidence of apical periodontitis. Samples were taken after gaining root canal access (S1), chemomechanical procedures (CMP, S2), and ultrasonic irrigation (S3). DNA-targeted qPCR using universal primers was used to estimate total bacterial levels, while rRNA-targeted qPCR was used to assess bacterial activity. Ratios between rRNA and DNA levels were calculated to search for active bacteria in the samples (rRNA/ DNA ≥ 1). Wilcoxon matched-pairs signed-rank test was used to compare the differences in DNA levels between samples and DNA and rRNA levels within samples (P <.05)., Results: DNA-based methods revealed a significant decrease in bacterial levels from S1 to S2 and S2 to S3 (both P <.05). Notably, 11 out of 35 (31.4%) root canals did not harbor bacterial DNA after CMP, whereas ultrasonic activation increased DNA-negative samples to 17 (48.6%). However, all DNA-positive samples were also positive for rRNA, with significantly higher rRNA than DNA levels (P <.05), indicating bacterial activity at the sampling time., Conclusions: Ultrasonic irrigation improved the disinfection of root canals after chemomechanical procedures by reducing bacterial levels. Introduction: This study aimed to assess pulp survival in a randomized trial on pulp lavage in adult nonpainful posterior teeth with carious pulp exposure. The treatment included complete caries excavation, direct pulp capping with mineral trioxide aggregate, and immediate restoration with composite resin., Methods: Fluid was collected from the pulp wound to assess matrix metalloproteinase-9 (MMP-9) and total protein values. Before pulp capping, cavities were randomly (block randomization, n = 48) washed with a physiological saline or a sodium hypochlorite solution (2.5% NaOCl). Treatment outcome was assessed clinically (cold test) and radiographically after at least 1 year and again after at least 3 years. Painful failures were differentiated from nonpainful failures. Pulp survival was estimated using the Kaplan-Meier method including 95% confidence intervals (CIs) up to 1500 days., Results: From the 96 patients originally enrolled, 73 individuals could be followed continuously. The clinical observations indicated a beneficial and sustained effect of pulp lavage with 2.5% NaOCl over a control treatment with physiological saline solution on estimated pulp survival 1500 days postintervention, with 7% (95% CI, 1%-40%) in the saline group versus 55% (95% CI, 30%-100%) in the NaOCl group. High MMP-9/total protein values in pulpal fluid collected from the exposed site indicated early and painful treatment failures yet were not associated with failures that occurred more than 250 days after intervention., Conclusions: The low 4-year success rates reported here challenge the concept of direct pulp capping in the cases that were included. NaOCl lavage did not only increase the survival of affected pulps substantially but also particularly diminished painful failures (33% in the NaOCl group vs 62% in the saline group). The lack of the predictive value of MMP-9 assessments beyond early treatment failures points to inflammatory states of the pulp tissue under deep caries, which are not related to neutrophil infiltration., (Copyright © 2023 The Authors. Introduction: The in vitro efficacy of irrigant activation devices has not been contrasted to their safety. This was attempted in this study using apically closed versus patent simulated root canal systems in epoxy resin models, with the latter featuring a simulated periapical lesion., Methods: All 72 models had 2 joining canals connected by an isthmus, which was filled with dentin debris. The simulated periapical lesion was filled with colored gelatin in the 36 respective models. Canals were irrigated with 1.3% sodium hypochlorite. Samples were divided into 4 subgroups per system (n = 9): conventional irrigation, sonic low (EndoActivator; Dentsply Sirona, Charlotte, NC) and high frequency (EDDY; VDW, Munich, Germany), and ultrasonic agitation of the irrigant (always applying 3 cycles of 20 seconds). The total cleared surface areas (mm 2 ) in the simulated isthmus and periapical lesion were compared between systems and devices using parametric tests (P < .05)., Results: The cleaning of the isthmus was more effective in the apically open compared with the closed systems and was also significantly influenced by the agitation method (P < .001). In the closed systems, EDDY and ultrasonic agitation achieved the significantly (P < .05) best cleaning of the isthmus. In the open systems, ultrasonic agitation showed the single best result (P < .05). EDDY caused by far the highest and ultrasonic agitation the lowest dissolution of the gelatin in the simulated periapical lesion., Conclusions: Under the conditions of this study, ultrasonic agitation of a previously administered sodium hypochlorite irrigant was more laterally targeted and thus safer and more effective than sonic agitation methods., (Copyright © 2023 The Authors. Introduction: Hydrogen peroxide (H 2 O 2 ) and sodium hypochlorite (NaOCl) solutions are similar in that they contain oxidizing agents with a bleaching effect. NaOCl solutions are stable at a high pH, at which they also exert increased cleansing/proteolysis. On the other hand, H 2 O 2 solutions are natively acidic, yet gain bleaching power on organic stains when alkalized. It was investigated whether alkalizing a H 2 O 2 solution would also let it dissolve soft tissue or increase its bleaching power on blood-stained dentin., Methods: The stability of alkalized H 2 O 2 solutions was assessed by iodometric titration. Soft tissue dissolution was investigated on porcine palatal mucosa. The bleaching effect (ΔL∗) after 60 minutes of exposure was monitored in blood-stained human dentin using a calibrated spectrophotometer. To compare similar molarities, 2.5% H 2 O 2 solutions were used here, and 5.0% NaOCl was used as the positive control, whereas nonbuffered saline solution served as the negative control., Results: Adding alkali (NaOH) to the H 2 O 2 solutions rendered them unstable in a dose-dependent manner. A H 2 O 2 solution of pH 11.1 was chosen for the main experiments (tissue dissolution and bleaching effect) and compared with a native counterpart (pH = 4.7). Alkalizing the H 2 O 2 solution had no discernible effect on its soft tissue dissolution or bleaching power (P = .75 compared with the native H 2 O 2 solution). The NaOCl solution of similar molar concentration had a considerably (P < .001) higher tissue dissolving and bleaching effect under current conditions., Conclusions: The proteolytic/bleaching effects of NaOCl solutions are unique and cannot be achieved by altering the pH of peroxide solutions., (Copyright © 2020 The Authors. Introduction: Transforming growth factor beta 1 (TGF-β1) is a key morphogen in regenerative endodontics; yet, its location within the hard tissue phase of dentin and its availability in mature roots have not been fully elucidated., Methods: Young mature (n = 8) and immature (n = 11) roots from sound premolars were obtained from 13 orthodontic patients aged 17 ± 1 and 12 ± 1 years, respectively. Roots were cleaned of organic remnants in 5% sodium hypochlorite. The width of the minor foramen was measured using a digital microscope. TGF-β1 distribution was assessed in 3 roots per group by immunostaining combined with confocal laser scanning microscopy. The root dentin of the remaining 13 roots was powdered and decalcified in 17% EDTA to determine the overall levels of hard tissue-embedded TGF-β1 by enzyme-linked immunosorbent assay. Data were compared between groups using the Student t test (α = .05)., Results: The minor foramen was 168 ± 49 μm versus 557 ± 295 μm in mature compared with immature roots (P < .05). TGF-β1 was highly stainable toward the pulp space in both groups. It was clearly associated with peritubular dentin and apparently absent in nontubular outer dentin. TGF-β1 content was 115 ± 31 pg and 74 ± 35 pg/100 mg mature versus immature root dentin, respectively (P > .05)., Conclusions: TGF-β1 is deposited into the peritubular dentin. Introduction: In carious teeth, transforming growth factor beta 1 (TGF-β1) is released from the dentin matrix and possibly activated in an acidic environment. Conversely, EDTA solutions with a neutral to slightly alkaline pH are used in clinics to promote cell homing in regenerative endodontic procedures. We hypothesized that citric acid (CA) might be more beneficial., Methods: TGF-β1 release from human dentin disks conditioned with either 10% CA (pH = 2) or 17% EDTA (pH = 8) and the behavior of human stem cells toward such pretreated dentin were studied. The protein concentration in conditioning solutions after 10 minutes of dentin exposure was determined using a pH-independent slot blot technique., Results: There was a 5-fold higher concentration of the target protein in CA (382 ± 30 ng/disk) compared with EDTA (66 ± 3 ng/disk, P < .005). Using confocal laser scanning microscopy on immunofluorescent-labeled disks, we identified a high density of TGF-β1 in peritubular dentin after CA treatment. A migration assay showed that CA conditioning attracted significantly more stem cells toward the dentin after 24 hours compared with EDTA (P < .05) or phosphate-buffered saline (P < .005). To investigate whether the cell response to these dentin surfaces could be affected by different pretreatments, we cultured stem cells on conditioned dentin disks and found that CA had a significantly (P < .05) better effect than EDTA on cell attachment and cell survival., Conclusions: CA conditioning could be useful and may have significant benefits over current treatments., (Copyright © 2018 The Authors. Introduction: A clinically useful all-in-one endodontic irrigant with combined proteolytic and decalcifying properties is still elusive. In this study, the chemical effects of dissolving the tetrasodium salts of 1-hydroxyethane 1,1-diphosphonic acid (Na 4 HEDP) or Na 4 EDTA directly in sodium hypochlorite (NaOCl) irrigants in polypropylene syringes were assessed during the course of 1 hour., Methods: The solubility of the salts in water was determined. Their compatibility with 1% and 5% NaOCl was measured by iodometric titration and in a calcium complexation experiment by using a Ca 2+ -selective electrode., Results: The salts dissolved within 1 minute. The dissolution maximum of Na 4 HEDP in water (wt/total wt) was 44.6% ± 1.6%. The corresponding dissolution maximum of Na 4 EDTA was 38.2% ± 0.8%. Na 4 HEDP at 18% in 5% NaOCl caused a mere loss of 16% of the initially available chlorine during 1 hour. In contrast, a corresponding mixture between NaOCl and the Na 4 EDTA salt caused 95% reduction in available chlorine after 1 minute. Mixtures of 3% Na 4 EDTA with 1% NaOCl were more stable, but only for 30 minutes. Na 4 HEDP lost 24% of its calcium complexation capacity after 60 minutes. The corresponding loss for Na 4 EDTA was 34%., Conclusions: The compatibility and solubility of particulate Na 4 HEDP with/in NaOCl solutions are such that these components can be mixed and used for up to 1 hour. In contrast, short-term compatibility of the Na 4 EDTA salt with NaOCl solutions was considerably lower, decreasing at higher concentrations of either compound. Objectives: To test the hypothesis that a material with higher water absorption than polyvinylidene fluoride (PVDF) could increase the yield of target molecules from exposed dentine., Methods: In a series of standard tests, different cellulose membranes were compared to a PVDF counterpart for their ability to absorb water and release protein. In a subsequent randomized clinical trial, the cellulose material with the most favourable values was compared to PVDF regarding the levels of MMP-2 that could be collected from exposed dentine of healthy human teeth during filling replacement. MMP-2 levels were determined by enzyme-linked immunosorbent assay (ELISA). Data from the laboratory experiments were compared between materials using the appropriate parametric tests. The frequency of cases yielding quantifiable levels of MMP-2 was compared between materials by Fisher's exact test. The level of significance was set at 5%., Results: The cellulose membrane with the largest pore size (12-15μm) absorbed significantly (P<0.05) more water than PVDF. It showed a protein release that was similar to that of PVDF, while the cellulose membranes with smaller pore size retained significantly more protein (P<0.05). Using the large-pore cellulose membrane, MMP-2 could be collected at a quantifiable level from the dentine of healthy teeth in 9 of 13 cases, compared to 1 of 13 with the PVDF membrane (P<0.05)., Conclusions: Under the current conditions, a large-pore cellulose membrane yielded more of a molecule of diagnostic value compared to a standard PVDF membrane., Clinical Significance: Molecular diagnostics of dentinal fluid are hampered by low yields. Introduction: The temperature and related shrinking kinetics of gutta-percha injected from heat guns are not known; therefore, we investigated them in this study., Methods: The temperatures of 3 different backfill gutta-percha brands extruded from 3 commercially available heat guns set to 200°C were studied. To validate the results, temperature development of 1 gutta-percha brand injected from a heat gun during a simulated backfilling procedure was assessed in single-rooted human teeth containing thermocouples in a water bath of 37°C. These values were compared with the counterparts obtained in a tabletop testing device for shrinkage at an ambient temperature of 37°C. Using this device, linear shrinkage upon cooling was assessed for all 3 gutta-percha brands under investigation. Results were compared by parametric statistics (alpha = .05)., Results: The temperatures of extruded gutta-percha differed significantly (P < .05) between heat guns and gutta-percha brands. Mean temperatures ranging between 57.6° ± 4.5°C and 103.9° ± 7.8°C could be observed with different combinations. The temperature of extruded gutta-percha in tabletop experiments equaled that observed in the root canals. However, the cooling of gutta-percha was (P < .05) faster in the tabletop measuring device compared with the root canal environment. Within the controlled temperature drop from 75°C to 37°C, the total shrinkage differed significantly (P < .05) between the 3 gutta-percha brands under investigation and ranged between 0.96% ± 0.20% and 2.31% ± 0.26% after 10 minutes (P < .05). Introduction: There is no robust and simple way to quantify available chlorine from small volumes such as human root canals. Therefore, a new method was developed and assessed., Methods: Standardized size-40 paper points were soaked in a 15% (w/v) potassium iodide solution for 1 minute. Subsequently, the paper points were placed in an incubator and dried at 110°C for 4 hours. The paper points (n = 5 per measurement) were then dipped in different concentrations of NaOCl and photographed under standardized conditions in RAW format. The pictures were imported to image processing software and adjusted to the standardized background. The red, green, and blue levels of the paper points were assessed at a predefined area. Inverse regression was used to determine NaOCl concentration from red, green, and blue values, with both explanatory and outcome variables log-transformed to base 10., Results: The red value measurements were chosen for further analysis based on a comparison of the coefficient of determination (R(2)) and a residual analysis. The method was applied to concentrations of NaOCl between 0.0001% and 1% (R(2) = 0.92). In this range, NaOCI concentrations could be assessed with an error not larger than 3-fold the determined concentration., Conclusions: The present method proved to be robust to determine the order of magnitude of available chlorine that is present in a small volume. Introduction: In pulpless immature human premolars implanted in rodents, this study investigated whether fibrin gel offered advantages over leaving the root canal empty regarding soft tissue ingrowth and cell differentiation., Methods: Root canals of extracted human immature premolars (n = 12) were accessed and then irrigated with 5% sodium hypochlorite followed by 17% ethylenediaminetetraacetic acid. Root canals were then either left empty or filled with a fibrin gel (n = 6 each) before being placed subcutaneously on top of the calvarial bone of rats (1 tooth per rat) for 12 weeks. After sacrifice, teeth were histologically assessed. Tissue ingrowth was quantified and compared between groups using the Mann-Whitney U test (P < .05). Cells adhering to the pulp canal wall were immunohistochemically screened for the presence of bone sialoprotein (BSP) and dentin sialoprotein (DSP)., Results: More tissue grew into the pulp space when teeth were filled with fibrin gel (P < .05). The presence of fibrin gel affected not only the extent of tissue ingrowth but also tissue morphology and differentiation of cells contacting the dentinal wall. In the fibrin gel group, newly formed tissue was similar to normal pulp, constituted of inner pulp, cell-rich zone, cell-free zone, and an apparent odontoblast layer, which stained positive for BSP and DSP. Newly formed blood vessels were also more abundant compared with the initially empty root canals., Conclusions: Under the conditions of this study, fibrin gel improved cell infiltration and cell-dentin interaction. Introduction: Sodium hypochlorite (NaOCl) solutions with added wetting agents are advertised to dissolve necrotic tissue in root canals faster than their counterparts without a lowered surface tension. This was tested in the current study, and the null hypothesis formulated was that there was no difference between a commercially available NaOCl solution with a lowered surface tension (Chlor-XTRA; Vista Dental Products, Racine, WI) and a counterpart containing the same amount of available chlorine without added wetting agents regarding the soft tissue that remains in oval-shaped canals after mechanical preparation and irrigation., Methods: Formerly vital extracted teeth (N = 44, 22 pairs) with similar anatomy were radiographically paired and chemomechanically prepared. In 1 tooth from each pair, a 5.25% NaOCl solution with reduced surface tension was used; in the other, a pure, technical-grade NaOCl solution of 5.25% was used. The percentage of remaining pulp tissue (PRPT) was histologically assessed in root cross-sections. The non-Gaussian raw data were subjected to Kruskal-Wallis and Mann-Whitney U tests to verify the respective effect of the cross-section level and solution on the PRPT. The relationship between the cross-section level and the PRPT was estimated by the Spearman correlation test. The alpha-type error was set at 5%., Results: The cross-section level significantly influenced the PRPT (P < .05), whereas the PRPT was not influenced by the solution used (P > .05). A significant inverse correlation was found between the cross-section level and the PRPT (P < .05, r = -0.330). The lower the distance to the apex, the higher the PRPT regardless of the solution used., Conclusions: Contrary to the advertised statement, the dental solution with a reduced surface tension did not dissolve vital pulp tissue in oval root canals any better than a conventional NaOCl solution of similar strength. Introduction: The occurrence and potential orthodontic causes of cervical invasive root resorption (CIRR) are unknown. We aimed to identify the occurrence of CIRR in molar teeth of orthodontic patients treated with fixed appliances., Methods: All patients invited for final orthodontic recall between November 2009 and March 2011 were included. From 175 patients, 108 (46 men/62 women; mean age, 25 ± 5 years; mean time after bracket removal, 8 ± 2 years) were available. The first and second molar teeth (N = 858) of these patients were investigated for clinical and radiographic signs of CIRR. Patients identified with such signs were asked to have limited-volume cone-beam computed tomography scans performed., Results: No clinical signs of CIRR were detected. CIRR could not be ruled out on bite-wing radiographs in 18 patients. CIRR was thus identified in 1 patient in whom a second maxillary molar was affected. All first molars had been extracted in this patient, and the second molars had been moved mesially over a long distance. The 3 other second molars showed surface resorption., Conclusions: CIRR in molar teeth of orthodontic patients have a low mid-term occurrence (0.9%; 95% confidence interval, 0.2%-5.2%). Introduction: Hard-tissue debris is accumulated during rotary instrumentation. This study investigated to what extent a calcium-complexing agent that has good short-term compatibility with sodium hypochlorite (NaOCl) could reduce debris accumulation when applied in an all-in-one irrigant during root canal instrumentation., Methods: Sixty extracted mandibular molars with isthmuses in the mesial root canal system were selected based on prescans using a micro-computed tomography system. Thirty teeth each were randomly assigned to be instrumented with a rotary system and irrigated with either 2.5% NaOCl or 2.5% NaOCl containing 9% (wt/vol) etidronic acid (HEBP). Using a side-vented irrigating tip, 2 mL of irrigant was applied by 1 blinded investigator to the mesial canals after each instrument. Five milliliters of irrigant was applied per canal as the final rinse. Mesial root canal systems were scanned at high resolution before and after treatment, and accumulated hard-tissue debris was calculated as vol% of the original canal anatomy. Values between groups were compared using the Student's t test (α < .05)., Results: Irrigation with 2.5% NaOCl resulted in 5.5 ± 3.6 vol% accumulated hard-tissue debris compared with 3.8 ± 1.8 vol% when HEBP was contained in the irrigant (P < .05)., Conclusions: A hypochlorite-compatible chelator can reduce but not completely prevent hard-tissue debris accumulation during rotary root canal instrumentation., (Copyright © 2012 American Association of Endodontists. Introduction: Many clinicians use household bleach to irrigate root canals. Sodium hypochlorite solutions are also available from dental suppliers. We compared physicochemical features of these products and investigated their impact on pulp tissue dissolution., Methods: Six different brands of household bleach were bought from drugstores. These were compared with Chlor-XTRA and technical NaOCl solutions of controlled concentration and alkalinity regarding their chlorine content (wt% NaOCl), pH, alkaline capacity, osmolarity, surface tension (Wilhelmy plate method), and price. Bovine pulp tissue (n = 10 specimens per group) dissolution at 37°C by test and control solutions adjusted to 1.0% NaOCl was assessed. Reduction in tissue weight was compared between groups by one-way analysis of variance, followed by Bonferroni correction (P < .05)., Results: The pH of undiluted solutions ranged between 11.1 and 12.7. Batches of the same product differed in NaOCl content. No product contained more than an equivalent of 0.1 mol/L NaOH. One household bleach brand (Safeway Bleach Summit Fresh) was slightly alkalized; the other solutions under investigation were not. Osmolarity was similar between products. The surface tension of Chlor-XTRA and Safeway Bleach Summit Fresh was about half that of the other solutions. Tissue dissolution was statistically similar (P > .05) among all solutions. Price was about 100-fold higher per liter of Chlor-XTRA compared with household bleach., Conclusions: Other than its price, the Chlor-XTRA solution had no unique features. Introduction: A preparation technique with only 1 single instrument was proposed on the basis of the reciprocating movement of the F2 ProTaper instrument. The present study was designed to quantitatively assess canal preparation outcomes achieved by this technique., Methods: Twenty-five extracted human mandibular first molars with 2 separate mesial root canals were selected. Canals were randomly assigned to 1 of the 2 experimental groups: group 1, rotary conventional preparation by using ProTaper, and group 2, reciprocate instrumentation with 1 single ProTaper F2 instrument. Specimens were scanned initially and after root canal preparation with an isotropic resolution of 20 μm by using a micro-computed tomography system. The following parameters were assessed: changes in dentin volume, percentage of shaped canal walls, and degree of canal transportation. In addition, the time required to reach working length with the F2 instrument was recorded., Results: Preoperatively, there were no differences regarding root canal curvature and volume between experimental groups. Overall, instrumentation led to enlarged canal shapes with no evidence of preparation errors. There were no statistical differences between the 2 preparation techniques in the anatomical parameters assessed (P > .01), except for a significantly higher canal transportation caused by the reciprocating file in the coronal canal third. On the other hand, preparation was faster by using the single-file technique (P < .01)., Conclusions: Shaping outcomes with the single-file F2 ProTaper technique and conventional ProTaper full-sequence rotary approach were similar. Introduction: When sodium hypochlorite solutions react with tissue, their pH drops and tissue sorption decreases. We studied whether stabilizing a NaOCl solution at a high pH would increase its soft-tissue dissolution capacity and effects on the dentin matrix compared with a standard NaOCl solution of the same concentration and similar initial pH., Methods: NaOCl solutions were prepared by mixing (1:1) a 10% stock solution with water (standard) or 2 mol/L NaOH (stabilized). Physiological saline and 1 mol/L NaOH served as the controls. Chlorine content and alkaline capacity of NaOCl solutions were determined. Standardized porcine palatal soft-tissue specimens and human root dentin bars were exposed to test and control solutions. Weight loss percentage was assessed in the soft-tissue dissolution assay. Three-point bending tests were performed on the root dentin bars to determine the modulus of elasticity and flexural strength. Values between groups were compared using one-way analysis of variance with the Bonferroni correction for multiple testing (α < .05)., Results: Both solutions contained 5% NaOCl. One milliliter of the standard and the stabilized solution consumed 4.0 mL and 13.7 mL of a 0.1-mol/L HCl solution before they reached a pH level of 7.5, respectively. The stabilized NaOCl dissolved significantly more soft tissue than the standard solution, and the pH remained high. It also caused a higher loss in elastic modulus and flexure strength (P < .05) than the control solutions, whereas the standard solution did not., Conclusions: NaOH-stabilized NaOCl solutions have a higher alkaline capacity and are thus more proteolytic than standard counterparts., (Copyright © 2011 American Association of Endodontists. Introduction: Pulpal diagnostics might be improved by using molecular markers found in dentinal fluid. In the current pilot study, we tested whether matrix metallopeptidase 9 (MMP-9) levels in dentinal fluid were detectable and differed between pulps from symptomatic teeth diagnosed with irreversible pulpitis and healthy counterparts., Methods: Thirty-one patients participated; 19 were diagnosed with irreversible pulpitis, and 12 were in need of replacement of a filling close to the pulp space in a clinically healthy tooth. Dentinal fluid was collected during a period of 2 minutes from dentin cavities by using folded polyvinylidene difluoride (PVDF) membranes, which were then transferred to microcentrifugation tubes containing physiologic saline solution. Total MMP-9 levels in these solutions were assessed by using a human MMP-9 fluorescent assay, detection limit 0.25 ng/mL. MMP-9 levels between groups were compared by using Mann-Whitney U test (alpha <0.05)., Results: Three specimens from the symptomatic teeth were not included because coronal pulps proved to be necrotic on access. Dentinal fluid samples from symptomatic teeth had significantly higher MMP-9 levels than those from clinically healthy counterparts (P < .05). However, merely 7 of the 16 pulpitis samples contained detectable levels of MMP-9. None of the clinically healthy control specimens contained any detectable amounts of MMP-9., Conclusions: With a sensitive assay, it was possible to detect an enzyme linked with pulp tissue destruction (MMP-9) in dentinal fluid. However, the collection method needs to be improved to provide predictable fluid yields. Introduction: It was the goal of this study to compare different NaOCl activation schemes regarding a desired and an untoward outcome. Ultrasonic tips and a currently marketed sonic system were used in conjunction with a 2.5% sodium hypochlorite solution. Necrotic pulp tissue dissolution in simulated accessory canals and transportation of the main canal were assessed., Methods: Epoxy resin models (10 per group) with a curved simulated main root canal and two simulated accessory canals filled with necrotic bovine pulp tissue were irrigated passively with one of three ultrasonic setups (straight stainless steel files, prebent stainless steel files, or nickel-titanium tips) or a sonic device in conjunction with a plastic tip. Activation was performed four times for 30 seconds with replenishment of the NaOCl solution in between. All the files/tips had a 2% taper and a 0.15-mm tip diameter according to the manufacturer. Data from superimposing and analyzing digital photos before and after treatment were statistically analyzed using one-way analysis of variance followed by Bonferroni's correction for multiple comparisons (alpha < 0.05)., Results: Passive ultrasonic irrigation (PUI) in all the groups dissolved significantly more tissue than sonic activation (p < 0.05). No detectable canal transportation with sonic activation was observed. The difference in this outcome was not significant compared with ultrasonically activated nickel-titanium tips, whereas the straight stainless steel files caused significantly more ledging compared with these setups (p < 0.05)., Conclusion: Under the current conditions, PUI with a nickel-titanium tip promoted superior tissue-dissolving effects over sonic irrigant activation while maintaining simulated canal anatomy.

Introduction: Hard-tissue debris accumulation is a potential side effect of root canal instrumentation that has not been systematically investigated. In the current communication, a method to assess this debris using high-resolution microcomputed tomography (microCT) is presented., Methods: Based on prescans, mandibular molars with joining mesial root canals and isthmuses between these were selected (n = 6). The mean volume filled with apparent hard-tissue debris after instrumentation without irrigation was calculated over 2 mm of the mesial canal system by multiplying the voxel volume with the number of voxels representing acquired radiopaque material. Backscattered electron imaging was used to compare the calcium-phosphorus content of this material with that of the root dentin in the same specimen., Results: Backscatter scans showed that the accumulated debris viewed in the microCT scans was consistent with root dentin. In the selected canal segments, 29.2% +/- 14.5% of the original canal volume was filled with accumulated debris, which represented a significant change from the preoperative scan (p < 0.01, one-sample t test against zero). Three-dimensional reconstructions of the microCT images visualized the accumulated hard-tissue debris in the whole canal system., Conclusions: The current method appears suitable to quantitatively compare different instrumenting/irrigating regimens on dentin debris accumulation.

This study was designed to test the impact of either a strong (MTAD) or a soft (1-hydroxyethylidene-1, 1-bisphosphonate [HEPB]) chelating solution on the bond strength of Resilon/Epiphany root fillings. Both 17% EDTA and the omission of a chelator in the irrigation protocol were used as reference treatments. Forty extracted human upper lateral incisors were prepared using different irrigation protocols (n = 10): G1: NaOCl, G2: NaOCl + 17% EDTA, G3: NaOCl + BioPure MTAD (Dentsply/Tulsa, Tulsa, OK), and G4: NaOCl + 18% HEPB. The teeth were obturated and then prepared for micropush-out assessment using root slices of 1 mm thickness. Loading was performed on a universal testing machine at a speed of 0.5 mm/min. One-way analysis of variance and Tukey multiple comparisons were used to compare the results among the experimental groups. EDTA- and MTAD-treated samples revealed intermediate bond strength (0.3-3.6 MPa). The lowest bond strengths were achieved in NaOCl-treated samples (0.3-1.2 MPa, p < 0.05). The highest bond strength was reached in the HEBP-treated samples (3.1-6.1 MPa, p < 0.05). Under the present in vitro conditions, the soft chelating irrigation protocol (18% HEBP) optimized the bonding quality of Resilon/Epiphany (Resilon Research LLC, Madison, CT) root fillings.

The effects of different growth conditions on the susceptibility of five taxa to alkaline stress were investigated. Enterococcus faecalis ATCC 29212, Streptococcus sobrinus OMZ 176, Candida albicans ATCC 90028, Actinomyces naeslundii ATCC 12104, and Fusobacterium nucleatum ATCC 10953 were grown as planktonic cells, allowed to adhere to dentin for 24 hours, grown as monospecies or multispecies biofilms on dentin under anaerobic conditions with a serum-enriched nutrient supply at 37 degrees C for 5 days. In addition, suspended biofilm microorganisms and 5-day old planktonic multispecies cultures were used. Microbial recovery upon direct exposure to saturated calcium hydroxide solution (pH 12.5) for 10 and 100 minutes was compared with control exposure to physiologic saline. Planktonic microorganisms were most susceptible; only E. faecalis and C. albicans survived in saturated solution for 10 minutes, the latter also for 100 minutes. Dentin adhesion was the major factor in improving the resistance of E. faecalis and A. naeslundii to calcium hydroxide, whereas the multispecies context in a biofilm was the major factor in promoting resistance of S. sobrinus to the disinfectant. In contrast, the C. albicans response to calcium hydroxide was not influenced by the growth condition. Adherence to dentin and interspecies interactions in a biofilm appear to differentially affect the sensitivity of microbial species to calcium hydroxide.

In the present study the smear layer dissolution kinetics of 18% etidronate (HEBP), 9% HEBP, and 17% ethylenediaminetetraacetic acid (EDTA) on human dentin were quantitatively and longitudinally analyzed by using a single-tooth comparative model. Coronal dentin disks were prepared from 3 maxillary human molars. A standardized smear layer was produced on the pulpal side of each disk. The smear layer-covered surface was divided into 3 similar areas. Each of these was then exposed to 1 of the 3 irrigants under investigation, whereas the others were covered with adhesive tape. Co-site image sequences of the areas under investigation were obtained after several cumulative demineralization times. Sixteen images were obtained from each dentin area of each tooth for each experimental time at 1000x magnification. An image processing and analysis sequence measured sets of images, providing data of area fraction for thousands of tubules over time and allowing us to quantitatively follow the effect of the chelating substances. The Kruskal-Wallis H test and Dunn multiple comparison test were used to analyze the data. Overall, it can be concluded that the demineralization kinetics promoted by both 9% HEBP and 18% HEBP were significantly slower than those of 17% EDTA (P < .05). In addition, the single-tooth model is advantageous over the first co-site optical microscopy dentin assessments when different chelator solutions are compared.

Bioactive glass nanoparticles synthesized by flame spray synthesis were tested for their remineralization capabilities in vitro. After artificial demineralization with EDTA, human dentin was treated with 20-50nm size bioactive glass nanoparticles or a micrometer-sized, commercial reference material (PerioGlas) for up to 30 days. The degree of remineralization was measured using quantitative gravimetric methods (thermogravimetry, elemental analysis) and element-sensitive scanning electron microscopy imaging to detect new mineral precipitated on or within the demineralized tooth matrix. After treatment with bioactive glass nanoparticles for 10 or 30 days a pronounced increase in mineral content of the dentin samples suggested a rapid remineralization. The mechanical properties of the remineralized dentin samples were well below the stability of natural dentin. It is suggested that this lack of mechanical reconstitution may be attributed to an imperfect arrangement of the newly deposited mineral within the demineralized tooth matrix. Nevertheless, the substantially higher remineralization rate induced by nanometer-sized vs. micrometric bioactive glass particles corroborated the importance of particle size in clinical bioglass applications.

To get the root canal system free of organic debris and the smear layer, it has been recommended to irrigate with a NaOCl solution during instrumentation, followed by a rinse with a chelating agent such as ethylenediaminetetraacetic acid (EDTA) and a final irrigation with NaOCl. However, both hypochlorite and EDTA weaken dentin through dissolution of its organic and inorganic components, respectively. EDTA exposes the organic dentin matrix, which could then be attacked more easily by hypochlorite. It was the aim of this study to assess the impact of different irrigation sequences of NaOCl (2.5% w/v; total exposure time, 24 minutes) and EDTA (17%; 3 minutes) on the elastic modulus and flexure strength of standardized human root dentin bars (n = 11 per group). Exposures to solely EDTA (3 minutes), NaOCl (24 minutes), and water were used as control treatments. Specimens were subjected to 3-point bending tests; modulus of elasticity and flexure strength values were compared between groups with one-way analysis of variance followed by Fisher probable least-squares difference test. The alpha-type error was set at .05. The 24-minute exposure to the hypochlorite solution caused a significant drop in flexure strength compared with water- or EDTA-treated controls (P < .05), whereas the elastic modulus remained unaffected. In contrast, the short exposure to EDTA as is clinically recommended did not affect the mechanical dentin parameters under investigation, regardless of the irrigant sequence that was used.

Objective: The capacities of bioactive glass S53P4 and calcium hydroxide suspensions to prevent microbial leakage through instrumented root canals were compared., Study Design: Root canals of caries-free extracted human mandibular premolars were instrumented and randomly filled with calcium hydroxide or S53P4 (n = 14 each). Crowns were exposed to Enterococcus faecalis ATCC 29212 suspensions in a test setup with 2 broth chambers, and leakage was checked daily. Event times were compared using log rank test. After 50 days, teeth were fractured and inspected in a scanning electron microscope (SEM)., Results: Calcium hydroxide prevented leakage significantly better than S53P4 (P < .05). Estimated mean days to leakage were 26 for calcium hydroxide and 4 days for S53P4. The SEM observations revealed substantial calcification of the glass material in the root canal but also gaps containing clusters of coccoidal bacteria., Conclusion: The bioactive glass material under investigation could not prevent contamination of instrumented root canals.

In the current study, the impact of lubricant parameters on simulated root canal instrumentation was investigated. Using size 30 ProFile .06 instruments in milled artificial root canals in human dentin, the effects of sodium hypochlorite (1% NaOCl) and a chelator (18% etidronic acid) in aqueous irrigants on maximum torque, full torsional load, and maximum force values were gauged using a torque testing platform. Furthermore, the impact of the time a chelating lubricant was exposed to dentin as well as its galenic form (aqueous vs. gel-type) on the above outcome variables was evaluated. Aqueous lubricants significantly (p < 0.05, ANOVA, Newman-Keuls) reduced all outcome variables compared to dry conditions. The incorporation of a chelator further reduced these values (p < 0.05), whereas hypochlorite behaved similar to water. The chelator effect was immediate and did not increase with time. An aqueous lubricant was more beneficial than a gel-type counterpart.

The aim of this in vitro study was to compare leakage of Enterococcus faecalis ATCC 29212 through root canals of single-rooted extracted human mandibular premolars filled with either gutta-percha/AH Plus or Resilon/Epiphany using the continuous wave of condensation technique. Test and control teeth were suspended in an experimental bacterial leakage setup for 50 days. Kaplan-Meyer curves showed a tendency of the gutta-percha/AH Plus root fillings to prevent leakage better than Resilon/Epiphany counterparts. This tendency, however, was not statistically significant (log-rank test, p = 0.141). The estimated mean time for turbidity to occur in the broth below the suspended root was 35 days (SD = 5 days) in the gutta-percha/AH Plus and 33 days (SD = 4 days) in the Resilon/Epiphany group. It was concluded that within the limitations of this study, there was no apparent advantage of using Resilon/Epiphany over gutta-percha/AH Plus.

Local wound debridement in the diseased pulp space is the main step in root canal treatment to prevent the tooth from being a source of infection. In this review article, the specifics of the pulpal microenvironment and the resulting requirements for irrigating solutions are spelled out. Sodium hypochlorite solutions are recommended as the main irrigants. This is because of their broad antimicrobial spectrum as well as their unique capacity to dissolve necrotic tissue remnants. Chemical and toxicological concerns related to their use are discussed, including different approaches to enhance local efficacy without increasing the caustic potential. In addition, chelating solutions are recommended as adjunct irrigants to prevent the formation of a smear layer and/or remove it before filling the root canal system. Based on the actions and interactions of currently available solutions, a clinical irrigating regimen is proposed. Furthermore, some technical aspects of irrigating the root canal system are discussed, and recent trends are critically inspected.

Objective: The aim of the current study was to test the impact of dentin powder on the antimicrobial efficacy of bioactive glass S53P4 (BAG)., Study Design: BAG was suspended (preincubated) in saline at 37 degrees C for different time periods with or without human dentin powder, hydroxylapatite, or decalcified dentin. Subsequently, Enterococcus faecalis ATCC 29212 cells were added to these suspensions and bacterial recovery measured with and without the use of gentle sonication. Furthermore, survival of bacteria in test and control suspensions was assessed over time. Supernatants of suspensions were analyzed for their element contents using atomic absorption spectrophotometry. The effects of pH, silica, and osmolarity on E faecalis viability were assessed using specifically prepared solutions., Results: BAG preincubated with dentin powder caused a significant (P < .05) decrease in viability compared to pure BAG suspensions. This was not based on adherence of bacteria to solid particles or agglutination of the cells, because sonication did not increase bacterial yields. Hydroxylapatite and decalcified dentin did not increase BAG killing efficacy. The additive effect of BAG + dentin powder was dose dependent, occurred only with solids in suspension, and increased with suspension time. An augmented dissolution of glass components, especially silicon, was measured in BAG + dentin powder compared to pure BAG suspensions or counterparts containing hydroxylapatite or decalcified dentin. High osmolarity per se did not affect E faecalis viability, whereas high pH and silica levels did., Conclusion: The observed phenomenon was related to an increased BAG dissolution triggered by dentin powder, causing elevated local pH and silica levels.

The aim of this study was to assess interactions of EDTA and citric acid (CA) with sodium hypochlorite (NaOCl), the indispensable endodontic irrigant. Other chelators were simultaneously evaluated as possible alternatives: sodium triphosphate (STP), amino tris methylenephosphonic acid (ATMA), and 1- hydroxyethylidene-1, 1-bisphosphonate (HEBP). Available chlorine was titrated in chelator-NaOCl solutions. All chelators other than HEBP and STP caused an almost complete, immediate loss of available chlorine in solution. Atomic absorbtion spectrometry and SEM evaluation of root canal walls of instrumented teeth indicated that NaOCl had no negative effect on calcium-complexing ability of chelators. STP was too weak a complexing agent to warrant further studies. Finally, CA-, EDTA-, and HEBP-NaOCl mixtures were evaluated for their antimicrobial capacity. Again, EDTA and CA negatively interfered with NaOCl, while HEBP did not.

The purpose of this study was to test some effects of preheating NaOCl solutions using a commercially available syringe heating device. Irrigating solution temperatures in 10-ml syringes were measured. Stability of 5.25, 2.62, and 1% NaOCl solutions for 60 min at 20, 45, and 60 degrees C was assessed using iodine/thiosulfate titration. Human pulp tissue dissolution capacity of a 1% NaOCl solution was gauged at the latter temperatures, and compared to corresponding values with a 5.25% solution at 20 degrees C. Killing efficacy of diluted NaOCl solutions against 48-h incubations of Enterococcus feacalis ATCC 29212 was compared at 45 degrees C and 20 degrees C. Using the heating device, a 20 degrees C solution reached 45 degrees C and 60 degrees C in 7 and 20 min, respectively. Solutions remained stable during the observation period. The 1% NaOCl solution at 45 degrees C dissolved pulp tissues as effectively as the 5.25% solution at 20 degrees C, while the 60 degrees C/1% solution was significantly more effective (p < 0.05). A 100-fold increase in killing efficacy was observed between corresponding NaOCl solutions at 20 degrees C and 45 degrees C.

The aim of this study was to evaluate the effect of reducing surface tension in endodontic chelator solutions on their ability to remove calcium from instrumented root canals. Aqueous solutions containing 15.5% EDTA, 10% citric acid, or 18% 1- hydroxyethylidene-1, 1-bisphosphonate (HEBP) were prepared with and without 1% (wt/wt) polysorbate (Tween) 80 and 9% propylene glycol. Surface tension in these solutions was measured using the Wilhelmy method. Sixty-four extracted, single-rooted human teeth of similar length were instrumented and irrigated with a 1% sodium hypochlorite solution and then randomly assigned (n = 8 per group) to receive a final one-minute rinse with 5 ml of test solutions, water, or the pure aqueous Tween/propylene glycol solution. Calcium concentration in eluates was measured using atomic absorption spectrometry. Incorporation of wetting agents resulted in a reduction of surface tension values by approximately 50% in all tested solutions. However, none of the solutions with reduced surface tension chelated more calcium from canals than their pure counterparts (p > 0.05).

Objective: The goal of this study was to weigh the impact of patient-related, tooth-related, and treatment-related factors on therapy outcome in a series of consecutive patients., Study Design: Eighty-four patients were included. Of these, 66 (79%) were available for recall after > or = 30 months (mean = 46 months). Root canal treatments were performed using a standard protocol. At recall, teeth were scored by means of the periapical index (PAI), which was the dependent variable (dichotomized to sound/ unsound). Explanatory variables were patient age, integrity of the nonspecific immune system, smoking status, dichotomized PAI score before treatment, initial treatment versus retreatment, prior exposition of the root canal to saliva, stainless-steel hand versus NiTi rotary instrumentation, and quality of root filling. Unit of observation was the patient-individual. Data were analyzed using univariate tests and backward stepwise logistic regression analysis., Results: After 5 steps with elimination of the least significant independent variable, status of the immune system (P = .05), initial PAI (P = .04), and root filling quality (P = .01) were found to be the indispensable predictors for treatment outcome. Using these 3 explanatory variables, the logistic regression model had a predictive value of 87%, compared to 91% with all 8 variables. Success rate at recall (PAI < or = 2 without symptoms) was 88% (95% CI = 78, 94)., Conclusion: The integrity of a patient's nonspecific immune system, which has been neglected in earlier investigations, is a significant predictor for endodontic treatment outcome, and should receive more attention in future studies.

Objectives: The aim of the current in vitro study was to evaluate the effect of CO(2)-laser treatment immediately after applying amine fluoride solution on enamel. It was hypothesized that such a treatment would increase enamel fluoride uptake, and reduce dissolution rate and thermal surface alterations., Methods: Fluoride uptake was determined in 40 human enamel sections randomly assigned to four groups (n=10), which were either left untreated (1), exposed to a 1% amine fluoride solution for 15s without irradiation (2), irradiated for 15s with a continuous-wave carbon dioxide laser (3), or laser-treated for 15s through the amine fluoride solution applied immediately beforehand (4). Fluoride uptake was determined with an ion selective electrode after acid dissolution of the specimens (surface and subsurface layers). For the determination of acid resistance, another 40 enamel sections were treated according to the above protocol. Acid resistance was determined in surface and subsurface layers by measuring eluted calcium upon 3% lactic acid exposure with atomic absorption spectrometry. Enamel surface alterations after laser irradiation were monitored using scanning electron microscopy., Results: Laser irradiation through the fluoride solution led to significantly higher fluoride contents in the surface enamel layer compared to fluoride treatment alone or laser treatment alone (p=0.002). Laser treatment with or without fluoride resulted in an increased acid resistance of enamel specimens. Fewer surface alterations were observed upon SEM examination of specimens irradiated through the amine fluoride solution compared to counterparts treated with laser only., Conclusions: CO(2) laser light application through an amine fluoride solution may be useful and effective in the prevention of caries.

Necrotic soft-tissue remnants in root canals may provide a source of nutrition for surviving microbiota after root-canal therapy. This study assessed the necrotic tissue dissolution capacity of some popular and some potential root-canal irrigants: 1% (wt/vol) sodium hypochlorite (NaOCl), 10% chlorhexidine, 3% and 30% hydrogen peroxide, 10% peracetic acid, 5% dichloroisocyanurate (NaDCC), and 10% citric acid. Standardized necrotic tissue samples obtained from pig palates were incubated in these solutions, and their weight loss was measured over time. None of the test solutions except sodium hypochlorite had any substantial tissue dissolution capacity. It was concluded that this might be important when considering the use of irrigants other than NaOCl.

Calcium hydroxide is the "gold standard" endodontic medication, but it may fail to eliminate certain facultative bacteria and yeasts found in root canal systems. In this study, standardized bovine dentin blocks infected with Enterococcus faecalis were treated with an aqueous calcium hydroxide or bioactive glass S53P4 (BAG) powder suspension. While calcium hydroxide was ineffective, the BAG suspension eliminated the infection in the sampled dentin layers after 5 days. In a direct exposure test, preincubation with human dentin boosted the BAG-killing efficacy against E. faecalis ATCC29212, Candida albicans CCUG19915, Pseudomonas aeruginosa ATCC9027, Streptococcus sanguis ATCC10556, and S. mutans ATCC25175. BAG placed in the root canals of extracted teeth did not alter root-dentin pH. Consequently, the conditions under which the microbiota were killed were not pH-mediated. Further studies are ongoing to help clarify the antimicrobial BAG effect in the presence of dentin.

Objectives: The aim of this study was to evaluate the dentin-protective potential of two dentin-bonding systems (Syntac and Clearfil SE Bond) on artificial saliva and lactic acid exposure., Methods: Radiochemical analysis in combination with polarization-microscopy was used in the current study. Different applications in MOD preparations in irradiated human molars were compared. Untreated cavities and cavities covered with flowable resin served as controls. Forty-eight specimens were immersed in artificial saliva (pH 7.5), 48 in lactic acid (pH 4). Activity of (32)P was measured between days 1 and 21 and loss of mineralized tissue was estimated., Results: In lactic acid, cavities sealed with single bond, double bond or bond plus cavity sealer application resulted in significantly lower mineralized tissue loss compared to negative controls (p < 0.05). Upon both saliva and lactic acid exposure, bonding plus sealer application proved to be the most protective treatment (p < 0.05)., Conclusions: Based on these in vitro findings it may be advisable to use a filled cavity sealer or a flowable resin to protect dentin from demineralization. The radiochemical analysis appeared to be valuable in evaluating the protective potential of bonding agents against dentin demineralization.

Objective: The goal of this study was to compare the tissue-dissolution potential and antibacterial effectiveness of a conventional Ca(OH)(2)/saline paste with equivalent Ca(OH)(2)/NaOCl and Ca(OH)(2)/chlorhexidine digluconate medications., Study Design: Tissue specimens were obtained from freshly dissected pig palates. Tissue pieces of similar form and weight were incubated in air-tight containers with Ca(OH)(2) pastes or solutions proper for up to 7 days. Antimicrobial testing was performed in dentin blocks infected with Enterococcus faecalis. Medicated, sealed dentin specimens were incubated for 1 and 5 days, and bacterial growth was tested at different dentin depths., Results: Up to day 4, the Ca(OH)(2)/irrigating solution mixtures dissolved tissue more effectively than the conventional Ca(OH)(2)/saline paste. After 7 days, however, no statistically significant differences were found between the saline and hypochlorite mixtures, but the Ca(OH)(2)/chlorhexidine medication was significantly less effective. Dentin block disinfection was quicker and more thorough with the Ca(OH)(2)/chlorhexidine or the Ca(OH)(2)/NaOCl than with the Ca(OH)(2)/saline paste., Conclusion: Ca(OH)(2)/irrigant mixtures under investigation appear more advantageous than the conventional Ca(OH)(2)/saline mixture, and merit further investigation in a clinical study.

In addition, the antimicrobial effect of Dakin's solution and equivalent unbuffered hypochlorite was tested., Study Design: Tissue specimens were obtained from freshly dissected pig palates. Unbuffered 2.5% and 0.5% sodium hypochlorite solutions and 0.5% solutions buffered at a pH of 12 and a pH of 9 (Dakin's solution) were tested on fresh and decayed tissue. Tissue decay was assessed histologically. Antimicrobial testing was performed with Enterococcus faecalis in dentin blocks and on filter papers., Results: The 2.5% NaOCl solution was substantially more effective than the three 0.5% solutions in dissolving the test tissues. Buffering had little effect on tissue dissolution, and Dakin's solution was equally effective on decayed and fresh tissues. No differences were recorded for the antibacterial properties of Dakin's solution and an equivalent unbuffered hypochlorite solution., Conclusions: In contrast to earlier statements, the results of this study do not demonstrate any benefit from buffering sodium hypochlorite with sodium bicarbonate according to Dakin's method. An irrigation solution with less dissolving potential may be obtained by simply diluting stock solutions of NaOCl with water.

The metallic radioisotope of a known radiopharmaceutical chelate, (67)Ga(NOTA) (NOTA=1,4,7-triazacyclonane-1,4,7-triacetic acid), used for tumor detection, was substituted by the chemically similar but non radioactive aluminum ion. Our aim was to detect and evaluate the in vivo behavior of the chelate. For this purpose, Al(NOTA) and the related chelate Al(NODASA) (NODASA=1,4,7-triazacyclononane-1-succinic acid-4,7-diacetic acid) were studied using in vitro and in vivo (27)Al NMR spectroscopy in rats. Both chelates showed high stability towards acid catalyzed dissociation and their (27)Al NMR resonances are characteristic of highly symmetrical species, with chemical shifts within the range for octahedral or pseudo-octahedral geometries. The thermodynamic stability constant of the novel chelate Al(NODASA) was estimated using (27)Al NMR. The value obtained suggested that the chelate does not undergo in vivo demetalation by transferrin. The in vivo spectroscopic studies and the analysis of blood and urine samples for Al(III) concentrations indicated that the chelates remain intact under physiological conditions and that they are mainly eliminated from the body through the kidneys.

A rare case of a periodontally induced endodontic lesion in a systemically healthy subject of 28 years is described. The patient, having presented with severe attachment loss on the palatal and distal aspects of his maxillary right second premolar, was diagnosed with localized aggressive periodontitis. He had never received periodontal treatment. The tooth was nonvital and showed all signs of symptomatic apical periodontitis. It was also free of any restoration. All clinical findings clearly suggested that the endodontic problem was caused by the aggressive periodontal disease. Bacteriologic screening of the pocket and the root canal, by using "checkerboard" DNA-DNA hybridization analysis, revealed diverse flora in the periodontal lesion. The sample obtained from the root canal exhibited DNA from a limited number of species, including black-pigmented anaerobic rods. No bacterial DNA was found in the root canal that was not also recovered from the periodontal pocket.

The two cardiac myosin heavy chain isoforms, alpha and beta, differ functionally, alpha Myosin exhibits higher actin-activated ATPase than does beta myosin, and hearts expressing alpha myosin exhibit increased contractility relative to hearts expressing beta myosin. To understand the molecular basis for this functional difference, we determined the complete nucleotide sequence of full-length rat alpha and beta myosin heavy chain cDNAs. This study represents the first opportunity to compare full-length fast ATPase and slow ATPase muscle myosin sequences. The alpha and beta myosin heavy chain amino acid sequences are more related to each other than to other sarcomeric myosin heavy chain sequences. Of the 1938 amino acid residues in alpha and beta myosin heavy chain, 131 are non-identical with 37 non-conservative changes. Two-thirds of these non-identical residues are clustered, and several of these clusters map to regions that have been implicated as functionally important. Some of the regions identified by the clusters of non-identical amino acid residues may affect actin binding, ATP hydrolysis and force production.
