Objectives: To evaluate the sensitivity and specificity of the latency difference (DLat) between ulnar and median nerves of the arm after stimulation at the wrist; one of the easiest techniques proposed for recognizing ulnar neuropathy at the elbow (UNE). As latency difference is not a standardized technique, we set up a multicenter study to recruit large numbers of normal subjects and patients with UNE or generalized neuropathy., Methods: Six centers participated in the study with data obtained from three groups of participants, controls (CTRLs), patients with UNE and patients with generalized neuropathy (GNP). We first verified the anatomical superposition of the ulnar and median nerves in cadaver examination. The optimal recording site for these two nerves was found to be 10 cm above the medial epicondyle. We then standardized the position of the arm with full extension of the elbow and stimulated first the median and then the ulnar nerves at the wrist. CTRLs were examined on both arms at two consecutive visits., Results: We recorded 32 idiopathic UNE cases, 44 GNP patients and 62 controls. We demonstrated that a DLat cut-off value of 0.69 ms brings a sensitivity of 0.86 and specificity of 0.89 to discriminate CTRLs from UNE. We also validated that intra-examiner reproducibility was good., Conclusion: We report a lower normal value for DLat than reported in several non-standardized studies and CTRL and UNE groups have clearly separated DLat values., Significance: Due to its high sensitivity, our standardized technique could be used as a first-line diagnostic tool when UNE is suspected., (Copyright © 2019 International Federation of Clinical Neurophysiology. The value of various indexes to characterize the stimulus-response curve of human motor nerves was assessed in 40 healthy subjects recruited from four European centers of investigation (Créteil, Lausanne, Liège, Marseille). Stimulus-response curves were established by stimulating the right median and ulnar motor nerves at the wrist, with stimulus durations of 0.05 and 0.5 ms. The following parameters were studied: the threshold intensity of stimulation to obtain 10% (I 10), 50% (I 50), and 90% (I 90) of the maximal compound muscle action potential, the ratios I 10/I 50, I 90/I 50, (I 90 - I 10)/I 10, (I 90-I 50)/I 50, and (I 50 - I 10)/I 10, and the slopes of the stimulus-response curves with or without normalization to I 50. For each parameter, within-center variability and reproducibility (in a test-retest study) were assessed and between-center comparisons were made. For most of the parameters, the results varied significantly within and between the centers. Within the centers, only the ratios I 10/I 50 and I 90/I 50 were found constant and reproducible. Between the centers, the absolute intensity thresholds (I 10, I 50, I 90) and the ratio I 90/I 50 did not show significant differences at stimulus duration of 0.5 ms, whatever the stimulated nerve. The reduced variability and good reproducibility of the ratios I 10/I 50 and I 90/I 50 open perspectives in neurophysiological practice for the use of these indexes of the stimulus-response curve, a rapid and noninvasive test.

The authors describe the four patients in the first known Belgian family with autosomal recessive spastic ataxia of Charlevoix-Saguenay (ARSACS). A novel homozygous missense mutation, NM_014363.3: c.3491T>A in exon 9, of the SACS gene was identified in the present family, which results in an original amino acid of methionine to lysine substitution at amino acid residue 1164 (p.M1164K). Although the cardinal clinical features, i.e., spastic ataxia with peripheral neuropathy, in our patients were similar to those in Quebec patients, our patients exhibited some atypical clinical features, e.g., teenage-onset and absence of retinal hypermyelination. The present family is from Wallonia, and there could be shared ethnicity with the families of Charlevoix-Saguenay.

Unlabelled: The aims of the present study are to document side-to-side differences and temporal variability, between two trials (T1 and T2 at a time interval of 3 months) of nerve conduction measurements collected from 30 healthy subjects (mean age 22 +/- 2 years)., Methods: The protocol at T1 consisted of motor nerve conduction studies of median, ulnar, peroneal and tibial nerves bilaterally, with measurement of (a) motor response size (amplitude and area); (b) terminal latency; (c) minimal, mean and maximal F-wave latency; (d) motor conduction velocity; and (e) F-wave occurrence. T1 also involved sensory nerve conduction studies of median, ulnar, radial, lateral and medial cutaneous, sural and superficial peroneal nerves bilaterally, with measurement of sensory potential size (amplitude and area) and computation of sensory conduction velocity. The protocol at T2 consisted of identical measurements from the dominant side., Results and Conclusion: There was a negative relationship between the variability of parameters evaluating nervous conduction and the length of the nerve segment under study. Thus, the smallest side-to-side and temporal variabilities are measured for minimal F-wave latencies (on average 2-3%). The limits of symmetry and temporal variability are particularly useful for diagnosis of unilateral peripheral neuropathy or neurophysiological follow-up of patients with neuropathy, when the variability of the parameter under study is weak and when there is a high correlation between values recorded on the left and on the right or at T1 and T2. This was the case for motor response size of tibial and ulnar nerves, sensory potential size of radial nerve and minimal F-wave latencies from each studied motor nerve.

Antioxidants are important additives in polymers. Because of the low level of antioxidants normally used, they cannot be analyzed directly by common spectroscopic or thermal chemical techniques. However, antioxidants as well as other additives in polymers can be qualitatively analyzed by pyrolysis-gas chromatography (Py-GC) after separating the polymers and additives. In this study, several antioxidants have been investigated to demonstrate that Py-GC is a viable tool to analyze them. The advantages of using Py-GC in the analysis of antioxidants have also been discussed.

Lubricants are widely used in thermoplastic polymers to increase the overall rate of processing or to improve surface release properties. Because of the low level of lubricants normally used in a polymer, it may not be possible to analyze the additive directly by common spectroscopic or thermal chemical techniques. However, lubricants as well as other additives in the polymer can be qualitatively analyzed by pyrolysis-gas chromatography (Py-GC) after extraction. In this work, several lubricants have been studied to demonstrate that Py-GC is a viable tool to investigate lubricants. The advantages of using Py-GC in the analysis of lubricant have also been discussed.

The number-average molecular mass of a polymeric material has been determined by pyrolysis-gas chromatography (Py-GC) via end-group analysis. The major advantage of this technique is that no sample preparation is required. The sample is not required to be in the dilute solution form, and the amount of sample needed is approximately 0.5 mg. Phenyl group-terminated polybutadiene systems have been studied as an example. The application of Py-GC to obtain the end-group concentration, the number-average molecular mass and the limitations of this method are discussed in detail. The success of this development elevates the role of Py-GC as an important technique for end-group analysis for the determination of number-average molecular mass.

Flame retardants are widely used in thermoplastic polymers for household and transportation applications. Flame retardants as well as most of the other additives in the polymer can be qualitatively analyzed by pyrolysis-gas chromatography (Py-GC) simultaneously with the polymer composition. The key to successful analysis of flame retardants not only requires a thorough knowledge of the various types of flame retardants but also necessitates an understanding of the parent polymer and its targeted applications. In this study, several flame retardants in different polymer matrices have been studied to demonstrate the utility of Py-GC for the analysis of flame retardants. The advantages of Py-GC for flame retardants analysis have also been discussed.

Plasticizers are widely used in thermoplastic polymers to modify their physical properties and processibility. Plasticizers as well as most of the other additives in the polymer can be qualitatively analyzed by pyrolysis-gas chromatography (Py-GC) simultaneously with the polymer composition. The key to the successful analysis of plasticizers not only requires a comprehensive understanding of commercial plasticizers but also requires knowledge of the polymer and its applications, as well as the Py-GC technique. In this study, several plasticizers in different polymeric systems were studied to demonstrate the utility of Py-GC as a good tool for the characterization of these systems. The advantages of using Py-GC for plasticizer analysis are also discussed.

In the present study, the adapted multiple point stimulation (AMPS) method was first applied to median innervated thenar muscles in 22 amyotrophic lateral sclerosis (ALS) patients who did not received any treatment. In all patients, a motor unit number estimate (MUNE) and an average surface-recorded motor unit action potential (S-MUAP) size have been derived even if the denervation was severe; and the results were reproducible. The thenar MUNE was less than the normal lower limit for age in 17 patients, and the mean MUNE (67.1 +/- 90.6) was significantly different from that estimated in control subjects (263.3 +/- 116.8). The mean S-MUAP size in the 22 ALS patients was 352.9 +/- 328.4 microV x ms versus 94.1 +/- 30.3 microV x ms in healthy volunteers. A control AMPS was achieved in 8 patients after 2 and 6 months of a glutamate-release antagonist (riluzole) treatment. The mean loss of motor units, based on control thenar MUNEs realized after 6 months of treatment, was 53%. In conclusion, we propose AMPS as a manageable, reproducible and non-invasive procedure which permits one to quantify peripheral denervation and to appreciate the effectiveness of collateral reinnervation in ALS patients.
