1. Asymptomatic unilateral aplasia of the left parotid gland: an unusual entity and case report

Abstract

Congenital agenesis of major salivary glands is considered a very infrequent condition and typically appears to be a coincidental finding. It can be present as sporadic case or may be combined with aplasia or hypoplasia of other salivary glands or the lacrimal glands, or as a part of syndromes. Only 23 cases documented in the literature to date, while the youngest patient was 50 days old. Plenty of radiographic useful techniques and treatment is closely related to the clinical manifestations; therefore. We present a case of 52 year old female who was referred to our radiology department for recurrent numbness of the left upper limb, experienced over the previous three months. Complete absence of the left parotid gland was incidentally demonstrated at the brain MRI scan. Based on the patient’s past medical history, physical examination and demonstrated radiographic techniques it was an asymptomatic, no-syndromic and no-familious unilateral aplasia of the parotid gland. In the present report, we aimed to underline that this rare condition may be asymptomatic and co-exist with other medical conditions and syndromes.

Abstract

Breast cancer is a highly heterogeneous disease, exhibiting substantial spatiotemporal variations in gene expression, biochemistry, histopathology, and macroscopic structure. The complexities in the evaluation of malignant tumor heterogeneity, are pushing current clinical examinations to their limits and accounting for most of the failures of targeted therapies, and uncertainties in the clinical outcome among breast cancer patients. The incorporation of state-of-the-art image analysis methods, and emerging machine learning techniques, is providing the application of detailed data-driven quantitative studies of imaging phenotypes, termed Radiomics. Specifically, an immense number of quantifiable imaging features, are extracted with high-throughput computing from medical images, and correlated with other clinical data, for unraveling the complex underlying pathophysiological mechanisms which are potentially reflected in tissue macroscopic phenotype. Therefore, radiomic analysis is holding a key role in the future of clinical decision support, as it provides powerful tools for assessing the spatiotemporal heterogeneity of breast cancer, aiming at increasing the diagnostic accuracy and predicting patient's outcome toward individualized therapy planning.

Abstract

Introduction: The current imaging report describes a unique coexistence of the neck venous system morphological variants. Methods: The imaging study of a 75-year-old male patient was isolated due to its unique venous system. Results: An internal jugular vein (IJV) duplication (main-typical and accessory IJVs) was identified on the right side, coexisting with a facial vein (FV) drainage into the accessory IJV. On the left side, the external jugular vein (EJV) was draining into the IJV and the FV into the EJV. A transverse anastomosis of the EJV and anterior jugular vein (AJV) was identified. Conclusions: The IJV duplication is a rare morphological variant, and its coexistence with other co-variants complicates the area's anatomy. Understanding these morphological variants is crucial for anesthesiologists and surgeons, as it can help prevent iatrogenic injury.

Abstract

Facial nerve iatrogenic injuries are serious and can negatively affect the quality of life of the patients. Due to the properties of the nerve, the complications are devastating involving the aesthetic appearance and the function of the face. Moreover, the multiple branches of the nerve increase the risk of an iatrogenic injury making the detailed knowledge of the anatomical correlations around them critical. Abstract

Background: Spontaneous spondylodiscitis is an uncommon disease, which may result in serious complications with potentially high morbidity and mortality. We conducted a prospective case study over a 2-year period in order to analyze the clinical features, approaches to management, and outcome of spondylodiscitis., Methods: Eight consecutive patients (four men, four women; age range 53-82 years) suffering from spondylodiscitis were identified during the study period. Parameters recorded included: demographics, past medical history, predisposing factors, presenting signs and symptoms, spinal level and extension of the infection, laboratory indices of inflammation, microbiological testing, radiological assessment, kind and duration of treatment, follow-up magnetic resonance imaging (MRI) studies, and outcome., Results: Duration of symptoms varied from 14 to 90 days. All patients had back pain; fever>or=38 degrees C was present in 5/8 (62.5%) and neurological findings in 6/8 (75%). Diabetes mellitus was identified in six (75%). Most of the patients had elevated laboratory markers of inflammation. At the initial MRI, 12 anatomical levels were found. The microorganism was identified in 7/8 by blood or bone marrow cultures (50% Staphylococcus aureus). None of the patients underwent surgical intervention. Seven patients (87.5%) recovered to full activity; follow-up MRI study results were not always in parallel with the clinical improvement of patients., Conclusions: Spontaneous spondylodiscitis should be considered in every patient with back pain accompanied by fever and laboratory markers of inflammation. The major predisposing risk factor seems to be uncontrolled diabetes. MRI appears to be the method of choice for confirming diagnosis. Timely and accurate diagnosis along with prompt administration of antibiotics appears mandatory for a favorable outcome and avoidance of surgical intervention.

Abstract

Confluent hepatic fibrosis (CHF) is usually a feature of advanced cirrhosis, while occurrence in early stage compensated cirrhosis is uncommon. We report the imaging findings of masslike CHF in a patient with previously unsuspected compensated alcoholic cirrhosis. The combination of partially wedge shape, minimal capsular retraction, and homogeneous delayed enhancement on multiphase contrast-enhanced spiral CT led to the characterization of the lesions and establishment of the diagnosis of cirrhosis.
