1. Cultivating sustainability: Adoption and intensity of soil fertility management technologies among rural farms in Zambia

Abstract

Despite the numerous advantages of soil fertility management (SFM) practices and substantial investments to promote adoption, uptake of these technologies remains low across sub-Saharan Africa. We study the adoption and intensity of key SFM practices - improved fallow, animal manure, compost, crop residue retention, minimum tillage, inorganic fertilizer, and intercropping among rural farmers in Zambia. Data from 1234 rural farms and the multivariate probit (MVP) and generalized Poisson regression models are used. The MVP model highlights interconnections among practices, emphasizing the need for integrated interventions. Socio-demographics like gender, age, education, and household labor availability significantly influence adoption of SFM practices. Farm characteristics including experience, land ownership, livestock, and off-farm income also play pivotal roles. Accessibility of information via phones, radio, and farmer groups emerged as a crucial enabler, while extension services, credit, and proximity to markets and roads shape adoption decisions. Results indicate education, household labor, off-farm income, and group membership drive intensity of adoption. Extension contacts, training, and agro-ecological region location also affect intensity with regional variations. Key policy implications emerge. First, the complementarities among practices underscore the need for holistic strategies recognizing these synergies. Second, enhancing education specifically for younger farmers can promote adoption of SFM technologies by increasing knowledge. Third, spurring farmer groups facilitates knowledge exchange, input access, and collaborative adoption. Fourth, strategic extension services and training programs are vital to address knowledge gaps for sustained adoption. Overall, this study provides insights to guide policies for promoting SFM practices among rural farms.

2. Systematic review and chemoinformatics analysis of anti-allergic medicinal plants used in Malawi

Abstract

In Malawi, ethnobotanical surveys have been carried out on the plants used to treat or manage allergies and their symptoms. However, studies focusing on information analysis such as systematic reviews of anti-allergic plants and natural products are not yet published. This review highlighted and analysed data of potential anti-allergic medicinal plants used in Malawi in order to discover potential treatments for allergies through various approaches and improve well-being of people. Literature search strategy was used to collect data of medicinal plants. Chemoinformatics analysis was applied to compile associated natural products and to profile the physicochemical properties (drug-likeness and fingerprint diversity) with references to 60 antihistamine 1 drugs (H1R drugs) and 277 anti-H1 receptor inhibitors. Sixty (60) medicinal plants from 36 families out of 645 reported plant species had information of diseases or symptoms of allergies. The Malawian Natural Products (MNP) data set composed of 1757 unique compounds was compiled for the first time from 39 of 60 medicinal plants and approximately 48.9 % and 45.3 % of the compounds did not violate Lipinski and Veber drug-like rules respectively. Chemical space mapping revealed that compounds from Malawian medicinal plants are structurally diverse and share a small space with antihistamine compounds and drugs. This information is useful in exploring other anti-allergy biological mechanisms for natural products. Similarity screening identified usambarine (CID442121), which is structurally similar to 2,8-Dimethyl-5-(2-pyrazine-2-ylethyl)-gamma-carboline (CHEMBL1783970) and 29 other antihistamine compounds. The results will be used as a baseline for further studies which may eventually lead to the discovery of potential allergy treatments through various approaches. This could improve health and well-being of people which is in line with goal number 3 of Sustainable Development Goals, Africa's Union's Agenda 2063 and National Research Agenda for the Malawi 2063.

Abstract

The development of sustainable catalytic processes is the requirement of the future to replace traditional protocols involving hazardous, non-green, non-benign reagents/solvents, and cost ineffectiveness. Metal and mixed metal oxides due to their redox properties make them potential substitutes for conventional homogeneous catalysts. The acidity can be enhanced further by the sulfation process to form sulfated oxides representing a new class of solid acid catalysts. This review is the revisiting of sulfated solid acid catalysts, with major emphasis on understanding, methods, applications, and their role in the syntheses of heterocycles of pharmaceutical importance. The present review also discusses factors affecting the acidity of the catalysts, possibilities of leaching in reaction media, methods of sulfation to arrest leaching, other challenges, and future prospects.

4. Comprehensive characterization of HER2-low breast cancers: implications in prognosis and treatmentResearch in context

Abstract

Summary: Background: HER2-low cancers are heterogeneous with different degrees of HER2 expression and hormone receptor (HR) status. Currently, its analysis is mostly focused on the standard clinic-pathologic features or common biomarkers expression, without considering the heterogeneity within the category. A further characterization and understanding of this cancer subgroup will facilitate its management. Methods: A large cohort of HER2-negative cancers (N = 1464) was included. The HER2-low (N = 412) and HER2-zero cancers (N = 1052) were compared and correlated with a comprehensive panel of clinico-pathologic features and biomarker expression according to different HER2 expressions and HR statuses. The prognostic values of these features in HER2-low cancers were also evaluated. Findings: The characteristics of HER2-low breast cancers, as compared to HER2-zero, varied with the HR status. HER2-low luminal cancers were associated with younger age, larger tumor, high pAKT and high HLA expression. Among TNBCs, opposite trends in age and tumor size were found. Additionally, HER2-low TNBC showed less necrosis, higher pN, lower c-kit and CK14 than HER2-zero cancers. Nonetheless, regardless of HR status, HER2-low status was associated with increased COX2 and AR expression, implicated in the biology of HER2-low cancers. HER2-low cancers showed high expression of HLAs in tumors and PD-L1 in immune cells. In particular, the co-expression of HLAs was found to be associated with better survival in HER2-low cancers. Interpretation: This study revealed further characteristic of HER2-low breast cancers as compared to HER2-zero cancers, provided further insights into its prognostication and therapeutic strategies. Abstract

Mining and mineral processing continues to be a source of lasting environmental problems in many developing economies. Phytoremediation has proven to be a viable strategy to remediate contaminated lands and limit environmental damage, but it has not been widely implemented partially due to social and economic challenges. However, by encouraging phytoremediation with a focus on phytoextraction, it may be possible to rehabilitate contaminated lands while simultaneously providing economic support to local communities. This can be achieved by the sale of phytoextracted metals to fund large-scale phytoremediation, particularly in Sub-Saharan Africa. To this end, this paper provides a conceptual approach for phytoremediation-based mineral recovery and explores the social and economic challenges related to large-scale deployment. The viability of the approach is explored and future work on phytoremediation implementation is defined with the goal of advancing research and collaboration.

Abstract

Inflammatory bowel disease (IBD) is a lifelong chronic inflammation of the gastrointestinal tract. Polysaccharides from mushrooms have been extensively used to treat inflammatory diseases including IBD. Dictyophora indusiata polysaccharide (DIP) modulates gut microbiota and alleviate inflammatory reactions; however, its effect on ulcerative colitis (UC) is yet to explore. In this study DIP effect on DSS induced colitis was determined. Our findings revealed that DIP treatment alleviated the severity of colitis, especially at the high dose. The histopathological alterations and gut epithelial integrity were ameliorated. The inflammatory reactions and oxidative stress were improved. Moreover, proinflammatory cytokines were reduced, conversely, anti-inflammatory cytokines and tight junction proteins (TJs) were elevated. Furthermore, Illumina MiSequencing exhibited perturbation of bacterial community upon DSS treatment; however, the bacterial shift was reversed after DIP treatment. Inclusively, our findings suggest mushroom polysaccharides may have a potential therapeutic effect against colitis and inflammatory-related diseases.

7. Feasibility and preliminary efficacy of Ai Chi aquatic exercise training in Hong Kong's older adults with risk of falling: Design and methodology of a randomized controlled trial

Abstract

Falls in older adults are a major global public health concern. Group exercise could mitigate fall risk but the traditional land-based group exercise may not be always suitable for older adults at risk of falling, especially for those with musculoskeletal problems. Ai Chi aquatic exercise program could provide a safe and low-impact exercise training for older adults. However, the feasibility and efficacy of the program has not been well-investigated. The objective of this study is to investigate the feasibility and efficacy of the Ai Chi aquatic exercise program, compared to the land-based exercise program for older adults with moderate to high risk of falling. Forty community-dwelling older adults aged 65 or above with moderate to high fall risk will be recruited. They will be randomly allocated in the Ai Chi Aquatic Exercise Group (intervention) or the Land-based Exercise Group (active control) receiving 16 sessions (8 weeks) of specific exercise training. Feasibility of both exercise groups will be examined by recruitment, adherence, retention, feedback, subjective exercise experiences and satisfaction. Preliminary efficacy will be determined by whether physical and psychological fall risk factors could be mitigated. Physical fall risk assessment will include tests for flexibility, muscle strength, gait and balance. Psychological fall risk will be evaluated by preliminary cognitive function, anxiety level, level of depression and fear of falling. The results could establish a solid foundation for worldwide development of a feasible, safe and effective Ai Chi aquatic exercise program for prevention of falls in older adults with risk of falling. 8. Examining conscious motor processing and the effect of single-task, dual-task and analogy training on walking during rehabilitation by older adults at risk of falling in Hong Kong: Design and methodology of a randomized controlled trial

Abstract

Walking is a common activity among older adults. Fallers were discovered to have a higher propensity to conscious motor processing (reinvestment) than age-matched non-fallers, leading to movement disruption, possibly by constraining or interfering with automatic motor control mechanisms. However, gait rehabilitation programs by implicit motor learning methodologies may ameliorate reinvestment propensity while improving gait ability in older adults. One hundred and five older adults, with moderate to high risk of falling, will be recruited from different community centres in Hong Kong to investigate the effect of single-task, dual-task and analogy training (implicit motor learning methodologies) on walking during rehabilitation in the context of reinvestment propensity, balance, walking ability and fear of falling. Participants will be randomly allocated and received 12 training sessions in either a Single-task Walking Group (STWG), a Dual-task Walking Group (DTWG) or an Analogy Walking Group (AGWG). Reinvestment propensity, walking ability, balance and fear of falling will be assessed by the Chinese version Movement Specific Reinvestment Scale (MSRS-C), 10-meter Walk Test, Tinetti Balance Assessment Tool, Timed ‘Up & Go’ Test (TU&G), Berg Balance Scale (BBS) and 13-item Falls Efficacy Scale (FES-13), respectively at baseline before training (T0), just after completion of all training sessions (T1), and 6 months after completion of all training sessions (T2). Participants' number of falls between T1 and T2 will also be recorded. The results could establish a solid foundation for further development and implementation of a novel and effective gait rehabilitation program in fall prevention for older adults at risk of falling. Abstract

Abstract Background Three dimensional echo is a relatively new technique which may offer a rapid alternative for the examination of the right heart. However its role in patients with non-standard ventricular size or anatomy is unclear. This study compared volumetric measurements of the right ventricle in 25 patients with adult congenital heart disease using both cardiovascular magnetic resonance (CMR) and three dimensional echocardiography. Methods Patients were grouped by diagnosis into those expected to have normal or near-normal RV size (patients with repaired coarctation of the aorta) and patients expected to have moderate or worse RV enlargement (patients with repaired tetralogy of Fallot or transposition of the great arteries). Right ventricular end diastolic volume, end systolic volume and ejection fraction were compared using both methods with CMR regarded as the reference standard Results Bland-Altman analysis of the 25 patients demonstrated that for both RV EDV and RV ESV, there was a significant and systematic under-estimation of volume by 3D echo compared to CMR. This bias led to a mean underestimation of RV EDV by -34% (95%CI: -91% to + 23%). The degree of underestimation was more marked for RV ESV with a bias of -42% (95%CI: -117% to + 32%). There was also a tendency to overestimate RV EF by 3D echo with a bias of approximately 13% (95% CI -52% to +27%). Conclusions Statistically significant and clinically meaningful differences in volumetric measurements were observed between the two techniques. Three dimensional echocardiography does not appear ready for routine clinical use in RV assessment in congenital heart disease patients with more than mild RV dilatation at the current time.

Abstract

OBJECTIVE: Recent developments in artificial intelligence (AI) have positioned it to transform several stages of the clinical trial process. In this study, we explore the role of AI in clinical trial recruitment of individuals with geographic atrophy (GA), an advanced stage of age-related macular degeneration, amidst numerous ongoing clinical trials for this condition. DESIGN: Cross-sectional study. SUBJECTS: Retrospective dataset from the INSIGHT Health Data Research Hub at Moorfields Eye Hospital in London, United Kingdom, including 306 651 patients (602 826 eyes) with suspected retinal disease who underwent OCT imaging between January 1, 2008 and April 10, 2023. METHODS: A deep learning model was trained on OCT scans to identify patients potentially eligible for GA trials, using AI-generated segmentations of retinal tissue. This method's efficacy was compared against a traditional keyword-based electronic health record (EHR) search. A clinical validation with fundus autofluorescence (FAF) images was performed to calculate the positive predictive value of this approach, by comparing AI predictions with expert assessments. MAIN OUTCOME MEASURES: The primary outcomes included the positive predictive value of AI in identifying trial-eligible patients, and the secondary outcome was the intraclass correlation between GA areas segmented on FAF by experts and AI-segmented OCT scans. RESULTS: The AI system shortlisted a larger number of eligible patients with greater precision (1139, positive predictive value: 63%; 95% confidence interval [CI]: 54%-71%) compared with the EHR search (693, positive predictive value: 40%; 95% CI: 39%-42%). A combined AI-EHR approach identified 604 eligible patients with a positive predictive value of 86% (95% CI: 79%-92%). Intraclass correlation of GA area segmented on FAF versus AI-segmented area on OCT was 0.77 (95% CI: 0.68-0.84) for cases meeting trial criteria.
