Background: The prevalence of chronic respiratory diseases is increasing globally. There is evidence that those with spirometric impairment, and no evidence of obstruction, termed preserved ratio impaired spirometry (PRISm), have increased risk of morbidity and mortality, compared to those with normal lung function. There remain several gaps in characterizing PRISm., Research Questions: What is the prevalence, risk factors and clinical outcomes associated with PRISm globally?, Study Design and Methods: In this systematic review a comprehensive search using MEDLINE, Web of Science, CINHAL, and CENTRAL databases was conducted to include epidemiological studies with no language or data restrictions. Two reviewers independently screened citations and shortlisted full-text articles according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and data was extracted. Quality was assessed with the Effective Public Health Practice Project tool., Results: 52 studies met the inclusion criteria with 33 studies included in the meta-analysis. Pooled PRISm prevalence was 12% (95% CI: 0.10, 0.15) with greater prevalence in low-and middle-income countries (LMICs) compared to high-income countries (19% vs. 11%). Comorbid diabetes was a significant risk factor associated with PRISm but data for female sex and smoking were mixed. PRISm was associated with increased all-cause (OR 1.41, 95% CI:1.08, 1.83, p=0.02), cardiovascular (OR 1.84, 95% CI:1.31, 2.58, p<0.01), and respiratory mortality (OR 1.82, 95% CI:1.08, 3.05, p=0.03). PRISm was not associated with a reduced lung cancer diagnosis (p=0.46). Quality assessment analysis revealed 34.6% (n=18) studies were rated "strong," 42.3% (n=22) "moderate," and 23.1% (n=12) "weak." Studies conducted LMICs had lower quality ratings., Interpretation: Individuals with PRISm have increased risk of all-cause, cardiovascular, and respiratory mortality. Recognizing and targeting modifiable PRISm risk factors may reduce the growing burden of PRISm and transition to obstructive lung disease globally. Asthma is a common disorder affecting around 315 million individuals worldwide. The heterogeneity of asthma is becoming increasingly important in the era of personalized treatment and response assessment. Several radiological imaging modalities are available in asthma including chest x-ray, computed tomography (CT) and magnetic resonance imaging (MRI) scanning. In addition to qualitative imaging, quantitative imaging could play an important role in asthma imaging to identify phenotypes with distinct disease course and response to therapy, including biologics. MRI in asthma is mainly performed in research settings given cost, technical challenges, and there is a need for standardization. Background: Attenuation of transforming growth factor β by blocking angiotensin II has been shown to reduce emphysema in a murine model. General population studies have demonstrated that the use of angiotensin converting enzyme inhibitors (ACEis) and angiotensin-receptor blockers (ARBs) is associated with reduction of emphysema progression in former smokers and that the use of ACEis is associated with reduction of FEV 1 progression in current smokers., Research Question: Is use of ACEi and ARB associated with less progression of emphysema and FEV 1 decline among individuals with COPD or baseline emphysema?, Methods: Former and current smokers from the Genetic Epidemiology of COPD Study who attended baseline and 5-year follow-up visits, did not change smoking status, and underwent chest CT imaging were included. Adjusted linear mixed models were used to evaluate progression of adjusted lung density (ALD), percent emphysema (%total lung volume <-950 Hounsfield units [HU]), 15th percentile of the attenuation histogram (attenuation [in HU] below which 15% of voxels are situated plus 1,000 HU), and lung function decline over 5 years between ACEi and ARB users and nonusers in those with spirometry-confirmed COPD, as well as all participants and those with baseline emphysema. Effect modification by smoking status also was investigated., Results: Over 5 years of follow-up, compared with nonusers, ACEi and ARB users with COPD showed slower ALD progression (adjusted mean difference [aMD], 1.6; 95% CI, 0.34-2.9). Slowed lung function decline was not observed based on phase 1 medication (aMD of FEV 1 % predicted, 0.83; 95% CI, -0.62 to 2.3), but was when analysis was limited to consistent ACEi and ARB users (aMD of FEV 1 % predicted, 1.9; 95% CI, 0.14-3.6). No effect modification by smoking status was found for radiographic outcomes, and the lung function effect was more pronounced in former smokers. Lung transplant is now an established modality for a broad spectrum of end-stage pulmonary diseases. According to the International Society for Heart and Lung Transplantation Registry, more than 50,000 lung transplants have been performed worldwide, with nearly 11,000 recipients of lung transplants alive in the United States. With the increasing use of lung transplant, pulmonologists must be cognizant of the common as well as the unique posttransplant dermatologic complications. Immunosuppression, infections, and a variety of medications and environmental exposures can contribute to these complications. This review aims to provide representative pictures and describe the pathogenesis, epidemiologic characteristics, and clinical manifestations of dermatologic complications encountered among recipients of lung transplants., (Copyright © 2018 American College of Chest Physicians. With the increasing application of lung transplantation, pulmonologists must be cognizant of common complications unique to the postlung transplant period and the associated radiologic findings.
