The slowing and reduction of auditory responses in the brain are recognized side effects of increased pure tone thresholds, impaired speech recognition, and aging. However, it remains controversial whether central slowing is primarily linked to brain processes as atrophy, or is also associated with the slowing of temporal neural processing from the periphery. Here we analyzed electroencephalogram (EEG) responses that most likely reflect medial geniculate body (MGB) responses to passive listening of phonemes in 80 subjects ranging in age from 18 to 76 years, in whom the peripheral auditory responses had been analyzed in detail (Schirmer et al., 2024). Abstract

A soil spectrum generated by any spectrometer requires a calibration model to estimate soil properties from it. To achieve best results, the assumption is that locally calibrated models offer more accurate predictions. However, achieving this higher accuracy comes with associated costs, complexity, and resource requirements, thus limiting widespread adoption. Furthermore, there is a lack of comprehensive frameworks for developing and utilizing soil spectral libraries (SSLs) to make predictions for specific samples. While calibration samples are necessary, there is the need to optimize SSL development through strategically determining the quantity, location, and timing of these samples based on the quality of the information in the library. This research aimed to develop a spatially optimized SSL and propose a use-framework tailored for predicting soil properties for a specific farmland context. Consequently, the Moroccan SSL (MSSL) was established utilizing a stratified spatially balanced sampling design, using six environmental covariates and FAO soil units. Subsequently, various criteria for calibration sample selection were explored, including a spatial autocorrelation of spectra principal component (PC) scores (spatial calibration sample selection), spectra similarity memory-based learner (MBL), and selection based on environmental covariate clustering. Twelve soil properties were used to evaluate these calibration sample selections to predict soil properties using the near infrared (NIR) and mid infrared (MIR) ranges. Among the methods assessed, we observed distinct precision improvements resulting from spatial sample selection and MBL compared to the use of the entire MSSL. Notably, the Lin’s Concordance Correlation Coefficient (CCC) values using the spatial calibration sample selection was improved for Olsen extractable phosphorus (OlsenP) by 41.3% and Mehlich III extractable phosphorus (P_M3) by 8.5% for the MIR spectra and for CEC by 25.6%, pH by 13.0% and total nitrogen (Tot_N) by 10.6% for the NIR spectra in reference to use of the entire MSSL. Utilizing the spatial autocorrelation of the spectra PC scores proved beneficial in identifying appropriate calibration samples for a new sample location, thereby enhancing prediction performance comparable to, or surpassing that of the use of the entire MSSL. This study signifies notable advancement in crafting targeted models tailored for specific samples within a vast and diverse SSL.

Abstract

Summary: Background: Minimally invasive fusion of the sacroiliac joint as treatment for low back pain may reduce pain and improve function compared to non-operative treatment, although clear evidence is lacking. The aim of this trial was to evaluate the effect of minimally invasive sacroiliac joint fusion compared to sham surgery on sacroiliac joint pain reduction. Methods: In this double-blind randomized sham surgery-controlled trial patients with clinical diagnosis of sacroiliac joint pain confirmed with sacroiliac joint injection were included at two university hospitals in Sweden and Norway. Patients were randomized by the operating surgeon at each site to minimally invasive sacroiliac joint fusion or sham surgery. The primary endpoint was group difference in sacroiliac joint pain on the operated side at six months postoperatively, measured by the Numeric Rating Scale (0–10). Un-blinding and primary analysis were performed when all patients had completed six months follow-up. The trial is closed for new participants and was registered at clinicaltrials.gov: NCT03507049. Findings: Between September 1st, 2018 and October 22nd, 2021, 63 patients were randomized, 32 to the surgical group, 31 to the sham group. Mean age was 45 years (range 26–63) and 59 of 63 (94%) patients were female. The mean reduction in the operated sacroiliac joint from baseline to six months postoperative was 2.6 Numeric Rating Scale points in the surgical group and 1.7 points in the sham group (mean between groups difference −1.0 points; 95% CI, −2.2 to 0.3; p = 0.13). Interpretation: This double-blind randomized controlled trial could not prove that minimally invasive fusion of the sacroiliac joint was superior to sham surgery at six months postoperative. Abstract

The computational detection of R-peaks in an electrocardiogram (ECG) is particularly important for assessing vital signs in ambulatory settings. Algorithmic approaches are mainly open source and available for research purposes, primarily in the Python environment on stationary computers. This motivates the need for more support of mobile environments and their particular demands, such as context awareness and their limited computational resources.PeakSwift is a comprehensive Swift package designed for detecting R-peaks in single-lead ECG, tailored for the iOS environment. Its core functional features include on-device R-peak detection algorithms, quality assessment, and context-aware algorithm selection. PeakSwift is lightweight, extensible and modular. To evaluate its performance, a dedicated iOS application was developed, and a set of experiments was conducted using over 10,000 single-lead ECGs.The results demonstrated that PeakSwift achieves high reproducibility in detecting R-peaks compared to NeuroKit. In addition, its computational runtime was competitive.

Abstract

Diffuse reflectance spectroscopy has been extensively employed to deliver timely and cost-effective predictions of a number of soil properties. However, although several soil spectral laboratories have been established worldwide, the distinct characteristics of instruments and operations still hamper further integration and interoperability across mid-infrared (MIR) soil spectral libraries. In this study, we conducted a large-scale ring trial experiment to understand the lab-to-lab variability of multiple MIR instruments. By developing a systematic evaluation of different mathematical treatments with modeling algorithms, including regular preprocessing and spectral standardization, we quantified and evaluated instruments' dissimilarity and how this impacts internal and shared model performance. We found that all instruments delivered good predictions when calibrated internally using the same instruments' characteristics and standard operating procedures by solely relying on regular spectral preprocessing that accounts for light scattering and multiplicative/additive effects, e.g., using standard normal variate (SNV). When performing model transfer from a large public library (the USDA NSSC-KSSL MIR library) to secondary instruments, good performance was also achieved by regular preprocessing (e.g., SNV) if both instruments shared the same manufacturer. However, significant differences between the KSSL MIR library and contrasting ring trial instruments responses were evident and confirmed by a semi-unsupervised spectral clustering. For heavily contrasting setups, spectral standardization was necessary before transferring prediction models. Non-linear model types like Cubist and memory-based learning delivered more precise estimates because they seemed to be less sensitive to spectral variations than global partial least square regression. In summary, the results from this study can assist new laboratories in building spectroscopy capacity utilizing existing MIR spectral libraries and support the recent global efforts to make soil spectroscopy universally accessible with centralized or shared operating procedures.

Federated learning (FL) enables the optimization of machine learning models on distributed clients without sharing local data. The integration of FL into a mobile environment is becoming more feasible due to increasing on-device processing capabilities. However, there is limited open-source support for the iOS platform.The article introduces a Swift-based client implementation of the user-friendly FL framework Flower. The objective is facilitating FL client processes based on a modular and easy-to-integrate software development kit.A benchmark test demonstrates consistent stability and performance using the software, further motivating its use for research.

Abstract

Background & Aims: EASL guidelines recommend 8 weeks of treatment with sofosbuvir plus velpatasvir (SOF/VEL) for the treatment of acute or recently acquired HCV infection, but only 6- and 12-week data are available. Therefore, the aim of this study was to evaluate the safety and efficacy of a shortened 8-week SOF/VEL treatment for acute HCV monoinfection. Methods: In this investigator-initiated, prospective, multicentre, single-arm study, we recruited 20 adult patients with acute HCV monoinfection from nine centers in Germany. Patients received SOF/VEL (400/100 mg) as a fixed-dose combination tablet once daily for 8 weeks. The primary efficacy endpoint was the proportion of patients with sustained virological response 12 weeks after the end of treatment (SVR12). Results: The median HCV RNA viral load at baseline was 104,307 IU/ml; the distribution of HCV genotypes was as follows: GT1a/1b/2/3/4: n = 12/1/1/3/3. Thirteen (65%) of the 20 patients were taking medication for HIV pre-exposure prophylaxis. SVR12 was achieved in all patients who complied with the study protocol (n = 18/18 [100%], per protocol analysis), but the primary endpoint was not met in the intention-to-treat analysis (n = 18/20 [90%]) because two patients were lost to follow-up. One serious adverse event (unrelated to study drug) occurred during 12 weeks of post-treatment follow-up. Conclusions: The 8-week treatment with SOF/VEL was well tolerated and highly effective in all adherent patients with acute HCV monoinfection. Early treatment of hepatitis C might effectively prevent the spread of HCV in high-risk groups. Clinical Trial Number: NCT03818308. Impact and implications: The HepNet acute HCV-V study (NCT03818308), an investigator-initiated, single-arm, multicenter pilot study, demonstrates the efficacy and safety of 8 weeks of daily treatment with the fixed-dose combination sofosbuvir/velpatasvir (400/100 mg) in patients with acute hepatitis C virus (HCV) infection. All patients who completed therapy and were followed-up achieved sustained virologic response. Thus, early treatment with SOF/VEL which might effectively prevent the spread of HCV in high-risk groups can be recommended for patients with acute HCV monoinfection.

Abstract

Background: Recent data showed poor long-term survival in patients with moderate AS. Although sex differences in left ventricular (LV) remodeling and outcome are well described in severe AS, it has not been evaluated in moderate AS. Methods: In this retrospective, multicenter study, patients with a first diagnosis of moderate AS diagnosed between 2001 and 2019 were identified. Clinical and echocardiographic parameters were recorded at baseline and compared between men and women. Patients were followed up for the primary endpoint of all-cause mortality with censoring at the time of aortic valve replacement. Results: A total of 1895 patients with moderate AS (age 73 ± 10 years, 52% male) were included. Women showed more concentric hypertrophy and had more pronounced LV diastolic dysfunction than men. During a median follow-up of 34 (13-60) months, 682 (36%) deaths occurred. Men showed significantly higher mortality rates at 3- and 5-year follow-up (30% and 48%, respectively) than women (26% and 39%, respectively) (p = 0.011). On multivariable analysis, male sex remained independently associated with mortality (hazard ratio 1.209; 95% CI: 1.024-1.428; p = 0.025). LV remodeling (according to LV mass index) was associated with worse outcomes (hazard ratio 1.003; CI: 1.001-1.005; p = 0.006), but no association was observed between the interaction of LV mass index and sex with outcomes. Conclusions: LV remodeling patterns are different between men and women having moderate AS. Male sex is associated with worse outcomes in patients with medically treated moderate AS. Further studies investigating the management of moderate AS in a sex-specific manner are needed.

Abstract

Summary: In rodents, lower brown adipose tissue (BAT) activity is associated with greater liver steatosis and changes in the gut microbiome. However, little is known about these relationships in humans. In adults (n = 60), we assessed hepatic fat and cold-stimulated BAT activity using magnetic resonance imaging and the gut microbiota with 16S sequencing. We transplanted gnotobiotic mice with feces from humans to assess the transferability of BAT activity through the microbiota. Individuals with NAFLD (n = 29) have lower BAT activity than those without, and BAT activity is inversely related to hepatic fat content. BAT activity is not related to the characteristics of the fecal microbiota and is not transmissible through fecal transplantation to mice. Thus, low BAT activity is associated with higher hepatic fat accumulation in human adults, but this does not appear to have been mediated through the gut microbiota.

Abstract

This study examined social system failures and sought to identify a plausible mechanism behind these failures. Stretched exponential distributions were observed for failure of marriages in the US, UK, and Germany. In addition, the study provides evidence of power laws in the failure of large firms in the US and worldwide. Summation of stretched exponentials leads to power laws, so an underlying principle can be established to link different types of social system failures such as failures of marriages and firms. The generation of these fat-tailed distributions in social system failures can be explained by Zipf's principle of least effort. Therefore, increasing initial efforts at the individual level through marriage counseling or stakeholder synchronization should reduce failures.

Abstract

Marfan syndrome is characterized by high expression of matrix metalloproteinases (MMPs) in aortic smooth muscle cells (AoSMCs) associated with medial elastolysis and aortic root aneurysm. We aimed to reduce aortic elastolysis through decrease of MMP expression with decoy oligodeoxynucleotides (dODNs) neutralizing the transcription factor activating factor-1 (AP-1). AP-1 abundance in nuclear extracts as well as MMP-2 and MMP-9 expression were significantly increased in isolated mAoSMC of mgR/mgR Marfan mice compared to wild-type cells. Exposure to AP-1 neutralizing dODNs resulted in a significant reduction of basal and interleukin-1β-stimulated MMP expression and activity in mAoSMCs. Moreover, increased migration and formation of superoxide radical anions was substantially decreased in mAoSMCs by AP-1 dODN treatment. Aortic grafts from donor Marfan mice were treated with AP-1- dODN ex vivo and implanted as infrarenal aortic interposition grafts in mgR/mgR mice. Pretreatment of aortic grafts with AP-1 dODN led to reduced elastolysis, macrophage infiltration, and MMP activity. Permeability of the endothelial monolayer was increased for dODN in mgR/mgR aortae with observed loss of tight junction proteins ZO-1 and occludin, enabling dODN to reach the tunica media. Targeting AP-1 activity offers a new potential strategy to treat the vascular phenotype associated with Marfan syndrome.

Abstract

Methods to isolate and culture primary prostate epithelial stem/progenitor cells (PESCs) have proven difficult and ineffective. Here, we present a method to grow and expand both murine and human basal PESCs long term in serum- and feeder-free conditions. The method enriches for adherent mouse basal PESCs with a Lin−SCA-1+CD49f+TROP2high phenotype. Progesterone and sodium selenite are additionally required for the growth of human Lin−CD49f+TROP2high PESCs. The gene-expression profiles of expanded basal PESCs show similarities to ESCs, and NF-kB function is critical for epithelial differentiation of sphere-cultured PESCs. When transplanted in combination with urogenital sinus mesenchyme, expanded mouse and human PESCs generate ectopic prostatic tubules, demonstrating their stem cell activity in vivo. This novel method will facilitate the molecular, genomic, and functional characterization of normal and pathologic prostate glands of mouse and human origin.

Abstract

Phospholipids and sterols are known to have multiple functions in reproductive tissue of mammals. High concentrations of the cholesterol precursor desmosterol have been described in testis, epididymis, and spermatozoa of various species. These findings and the recent discovery of some cholesterol precursors as meiosis-activating sterols suggest important functions of cholesterol precursors in fertility. Many sterol intermediates appear from the 19-step conversion of lanosterol, the first sterol synthesized in the cascade of cholesterol synthesis, to cholesterol. The biochemical basis of the genetically inherited Smith-Lemli-Opitz syndrome has been described as a defective conversion of 7-dehydrocholesterol to cholesterol. Since this discovery, interest has focused on this special cholesterol precursor. Here, we report high concentrations of 7- and 8-dehydrocholesterol in caput epididymidis and spermatozoa derived from caput epididymidis of Sprague-Dawley and Wistar rats, which comprised up to 30% of total sterols. In contrast to caput epididymidis, 7- and 8-dehydrocholesterol were barely detected in cauda epididymidis or testis. Desmosterol increased several times from caput to cauda epididymidis. This is the first report of the natural appearance of high concentrations of dehydrocholesterols in mammalian tissue, and it underlines the putative importance of cholesterol precursors in reproductive tissue.—Lindenthal, B., T. 21. Dynamics of bacterial pathogens at the driveline exit site in patients with ventricular assist devices: a prospective, observational, single-centre cohort study

Abstract

BACKGROUND Driveline infections (DLIs) at the exit site are frequent in patients with left ventricular assist devices (LVADs). The dynamics from colonization to infection are yet to be investigated. We combined systematic swabbing at the driveline exit site and genomic analyses to study the dynamics of bacterial pathogens and get insights into DLIs pathogenesis. METHODS A prospective, observational, single-centre cohort study at the University Hospital of Bern, Switzerland was performed. Patients with LVAD were systematically swabbed at the driveline exit site between June 2019 and December 2021, irrespective of signs and symptoms of DLI. Bacterial isolates were identified and a subset was whole-genome sequenced. RESULTS Fifty-three patients were screened, of which 45 (84.9%) were included in the final population. Bacterial colonization at the driveline exit site without manifestation of DLI was frequent and observed in 17 patients (37.8%). Twenty-two patients (48.9%) developed at least one DLI episode over the study period. Incidence of DLIs reached 2.3 cases per 1,000 LVAD days. The majority of the organisms cultivated from exit sites were Staphylococcus species. Genome analysis revealed that bacteria persisted at the driveline exit site over time. In four patients, transition from colonization to clinical DLI was observed. CONCLUSION Our study is the first to address bacterial colonization in the LVAD-DLI setting. We observed that bacterial colonization at the driveline exit site was a frequent phenomenon, and in a few cases it preceded clinically relevant infections. We also provided acquisition of hospital-acquired multidrug-resistant bacteria and the transmission of pathogens between patients.

Abstract

Startups often lead the technological transformation of logistics and supply chain practices. To shed light on the engagement between established (logistics) firms and external startups, this study takes up the emerging collaboration vehicle of supply chain management (SCM) accelerators. In contrast to few previous studies investigating generic, cross-sector accelerators, this work examines the design of sector-specialized accelerators, which narrow their scope, curriculum, and participants to SC operations and logistics services. We adopt a multiple-case study approach and draw on a unique dataset of nine SCM accelerators. Our findings display the participants’ and operators’ objectives and describe the programs’ SCM-specific design features. We identify substantial differences across SCM accelerators, and between SCM accelerators and generic, cross-sector accelerators. 25. Determining baselines and trends of Eastern chimpanzees and forest elephants in a Central African protected area after civil strife

Abstract

Protected areas are crucial to safeguard Sub-Saharan Africa’s extraordinary and abundant megafauna. In many of these areas, instability has derailed conservation efforts and impeded adequate wildlife monitoring. Discovered in 2004, Eastern chimpanzees are found in the Central Uele Basin in northern Democratic Republic of Congo (DRC) within the Bili-Uéré Protected Areas Complex (BUPAC), the largest contiguous protected area in the country. BUPAC is threatened by habitat destruction, mining, wild meat trade, and insecurity. BUPAC chimpanzees are part of the largest remaining continuous population of the species in Africa; they are also being behaviourally unique. Forest elephants were frequent in the 1960’s in the BUPAC but have declined significantly up to 2004 - 2007. We used line transects to estimate Eastern chimpanzee and forest elephant density in the BUPAC core area in 2016 and 2019 and compared these with the 2004-2007 surveys. A total of 37 and 137 two km long line transects were systematically placed in 5,841 km2 and 6,176 km2 survey areas in 2016 and 2019, respectively. We found that chimpanzee density did not change during the two survey periods but indicators for forest elephant density decreased eight-fold. Human activities were detected mainly along the core area periphery in both survey years, where they overlapped with centres of animal activity. The stable high density of chimpanzees is a positive outcome for the core BUPAC. However, despite being a conservation priority area that has received relatively intensified protection, declining forest elephant numbers are likely to reflect the high number of human conflict hotspots in vicinity as well as the increasing human population density around the core area. We propose by elevating the core area to National Park whilst strengthening on the ground enforcement and management structures as well as legal measures against poaching might ensure the long-term survival of such an important area in Africa.

Abstract

Low bioavailability of iron due to poor solubility of iron(hydr)oxides limits the growth of microorganisms and plants in soils and aquatic environments. Previous studies described accelerated dissolution of iron(hydr)oxides under continuous illumination, but did not distinguish between photoreductive dissolution and non-reductive processes in which photogenerated Fe(II) catalyzes ligand-controlled dissolution. Here we show that short illuminations (5–15 min) accelerate the dissolution of iron(hydr)oxides by ligands during subsequent dark periods under anoxic conditions. Suspensions of lepidocrocite (Lp) and goethite (Gt) (1.13 mM) with 50 μM EDTA or DFOB were illuminated with UV-A light of comparable intensity to sunlight (pH 7.0, bicarbonate-CO2 buffered solutions). During illumination, the rate of Fe(II) production was highest with Gt-EDTA; followed by Lp-EDTA > Lp-DFOB > Lp > Gt-DFOB > Gt. Under anoxic conditions, photochemically produced Fe(II) increased dissolution rates during subsequent dark periods by factors of 10–40 and dissolved Fe(III) reached 50 μM with DFOB and EDTA. Under oxic conditions, dissolution rates increased by factors of 3–5 only during illumination. With DFOB dissolved Fe(III) reached 35 μM after 10 h of illumination, while with EDTA it peaked at 15 μM and then decreased to below 2 μM. The observations are explained and discussed based on a kinetic model. The results suggest that in anoxic bottom water of ponds and lakes, or in microenvironments of algal blooms, short illuminations can dramatically increase the bioavailability of iron by Fe(II)-catalyzed ligand-controlled dissolution. Purpose To evaluate the value of a browser-based PI-RADS Score Calculator (PCalc) compared to MRI reporting using the official PI-RADS v2.1 document (PDoc) for non-specialized radiologists in terms of reporting efficiency, interrater agreement and diagnostic accuracy for detection of clinically significant prostate cancer (PCa). Two non-specialized radiology residents (R1, R2) attributed a PI-RADS version 2.1 score for the most suspect (i. e. index) lesion (i) using the original PI-RADS v2.1 document only and after a 6-week interval (ii) using a browser-based PCalc. Reading time was measured. Reading time differences were assessed using Wilcoxon signed rank test. Intraclass-correlation Coefficient (ICC) was used to assess interrater agreement (IRA). Parameters of diagnostic accuracy and ROC curves were used for assessment of lesion-based diagnostic accuracy. Results Cumulative reading time was 32:55 (mm:ss) faster when using the PCalc, the difference being statistically significant for both readers (p < 0.05). The difference in IRA between the image sets (ICC 0.55 [0.40, 0.68]) and 0.75 [0.65, 0.82] for the image set with PDoc and PCalc, respectively) was not statistically significant. There was no statistically significant difference in lesion-based diagnostic accuracy (AUC 0.83 [0.74, 0.92] and 0.82 [95 %CI: 0.74, 0.91]) for images assessed with PDoc as compared to PCalc (AUC 0.82 [0.74, 0.91] and 0.74 [95 %CI: 0.64, 0.83]) for R1 and R2, respectively. Conclusion Non-specialized radiologists may increase reading speed in prostate MRI with the help of a browser-based PI-RADS Score Calculator compared to reporting using the official PI-RADS v2.1 document without impairing interreader agreement or lesion-based diagnostic accur

Abstract

The digitalization of intra‐ and inter‐organizational processes offers significant opportunity for research in the field of operations and supply chain management (OSCM). This essay summarizes the contributions of the special issue articles, highlighting their focus on additive manufacturing and the encapsulation of design and production information in a digital artifact. We conceptualize the digital artifact as containing the digital genes of the associated physical object. Digital encapsulation thus involves the integration of product design information with additional information on how that design is to be translated into a physical object, delivered to the customer, and used. Building on insights from the special issue articles, we identify three pathways by which digital encapsulation affects OSCM practice, as well as theory elaboration and extension. First, digital encapsulation allows each unique digitally encapsulated artifact to be acted on independently by OSCM systems. Second, digital encapsulation enables the redistribution of activities across organizational and geographic landscapes. Third, digital encapsulation facilitates interactivity of the digital artifact with external environment inputs. We conclude with a number of directions for future research.

Abstract

In recent years socially responsible investing has become a popular subject with both private and institutional investors. At the same time, a number of scientific papers have been published on socially responsible investments (SRIs), covering a broad range of topics, from what actually defines SRIs to the financial performance of SRI funds in contrast to non-SRI funds. In this paper, we revisit Markowitz’ Portfolio Selection Theory and propose a modification allowing to incorporate not only asset-specific return and risk but also a social responsibility measure into the investment decision making process. Applied in an a posteriori fashion, the model results in a three-dimensional capital allocation plane illustrating the complete set of feasible optimal portfolios. It enables investors to custom–tailor their asset allocations and incorporate all personal preferences regarding return, risk and social responsibility. In an empirical analysis with a data set of over 6000 international companies (including the complete universe of social responsibility-rated stocks), we find that investors opting to maximize the social impact of their investments do indeed face a statistically significant decrease in expected returns. However, the social responsibility/risk-optimal portfolio yields a statistically significant higher social responsibility rating than the return/risk-optimal portfolio.
