Background: Gestational diabetes mellitus (GDM) is a known risk factor for stillbirth (Rosenstein et al., 2012) [1]. Delayed villous maturation (DVM), predominantly seen in term placentas in pregnancies complicated with glucose dysmetabolism, may in part be a consequence of excessive maternal glucose leading to release of fetal insulin and other growth factors that promote excessive placental growth at the expense of villous maturation (Redline, 2012) [2]., Cases: We present three cases of under-diagnosed/treated glucose dysmetabolism in women in their first pregnancies cared for in other hospitals in the United Kingdom (UK) with the fatal fetal/neonatal outcomes and confirmed DVM in the placenta and congenital pneumonia on post-mortem examination in all three cases., Conclusion: This cluster supports a hypothesis that DVM and glucose dysmentabolism may make babies more susceptible to severe perinatal infection. Communication is associated with a significant percentage of errors or omissions in secondary healthcare across specialities; it is also the core process in and through which medical teams manage tasks, establish a rhythm and relationship between themselves and the patient, all of which are critical components of clinical practice. Despite this, however, communication is framed in medical training and the literature in either narrow terms or in a broad and fuzzy way, and it is indicative of the issue that teamwork and team communication are perceived and treated separately. In this paper, we draw on completed and ongoing interdisciplinary work to show how teams interact through illustrative examples from a large project on the management of obstetric emergencies. We provide a brief overview of the limitations in current tools and approaches, and we show how research under disciplines that have a long tradition in the analysis of interaction, and particularly healthcare sociolinguistics, can be translated and make a solid contribution to medical research and training., Competing Interests: Declaration of competing interest The authors have no conflicts of interest., (Copyright © 2022. Maternity claims represent the highest value and second highest number of clinical negligence claims reported to the NHS Litigation Authority (NHSLA). Most obstetric litigation is brought under the law of negligence which requires the claimant to show three things: that a duty of care was owed, that the duty was breached and that the breach caused loss. Training to prevent litigation should focus on 'Clinical' - multidisciplinary, simulation-training and 'Non-Clinical' (starting all discussions with women and their families timely, informing them about 'material' risks and addressing their particular concern, treating decision-making as a team process, using translation services early in the process, even before the emergency occur). Therefore, maternity claims are one of the most frequent and most expensive ones. Each unexpected pregnancy outcome can possibly trigger litigation. Post-partum haemorrhage (PPH) is the second greatest direct cause of maternal death in the United Kingdom, and rates of PPH continue to increase despite advances in clinical care. Training workers to manage PPH involves improvement of technical and non-technical skills in the context of a multidisciplinary team (MDT). Management of PPH should begin in the antenatal period, with identification of high-risk women and referral for multispecialty input. Training for the acute management of PPH should involve all members of the labour ward team and beyond, including haematology and non-clinical staff. Simulation-based training, didactic teaching and hybrid obstetric emergency courses are current options for training workers. Non-technical skills should also be taught, including specific training on communication, leadership, situational awareness and team-working skills. Improving management of obstetric emergencies requires thorough antenatal and intra-partum risk assessment, optimising knowledge and non-technical skills of individual members of the team, improving collaboration of the MDT, better simulation training and adjusting local infrastructure., (Copyright © 2019. Manual rotation (MR) is the most common technique used by accoucheurs who wish to correct malposition of the foetal head to either avoid or facilitate an operative vaginal delivery (OVD). MR can be performed using either a whole-hand or a digital approach. MR should be formally taught and trainees should be assessed for competence, and later, performance should ideally be tracked with statistical control charts. There is paucity of robust evidence evaluating MR relative to the other methods of rotational OVD: rotational forceps (RF) and rotational ventouse (RV). Furthermore, there is little evidence concerning long-term maternal outcomes of rotational OVD. A prospective randomised trial of MR versus either RF or RV is clearly needed, along with a core outcome set for OVD to facilitate comprehensive evaluation programmes that focus on aspects pertaining to women., (Copyright © 2018. Third-trimester stillbirth affects approximately 2.6 million women worldwide each year. Although most stillbirths (98%) occur in low- and middle-income countries, most of the research on the impact of stillbirth and bereavement care has come from high-income countries. The impact of stillbirth ranges from stigma to disenfranchised grief, broken relationships, clinical depression, chronic pain, substance use, increased use of health services, employment difficulties, and debt. Appropriate bereavement care following a stillbirth is essential to minimise the negative socio-economic impact on parents and their families. This article presents the best practice points in stillbirth bereavement care, including taking an individualised and flexible approach. The latest published research, guidelines, and best practice points from high-income countries will be used and will highlight the gaps in the research which urgently need to be addressed. Accurate and consistent classification of causes and associated conditions for perinatal deaths is essential to inform strategies to reduce the five million which occur globally each year. With the majority of deaths occurring in low- and middle-income countries (LMICs), their needs must be prioritised. The aim of this paper is to review the classification of perinatal death, the contemporary classification systems including the World Health Organization's International Classification of Diseases - Perinatal Mortality (ICD-PM), and next steps. During the period from 2009 to 2014, a total of 81 new or modified classification systems were identified with the majority developed in high-income countries (HICs). Structure, definitions and rules and therefore data on causes vary widely and implementation is suboptimal. Whereas system testing is limited, none appears ideal. Several systems result in a high proportion of unexplained stillbirths, prompting HICs to use more detailed systems that require data unavailable in low-income countries. Some systems appear to perform well across these different settings. ICD-PM addresses some shortcomings of ICD-10 for perinatal deaths, but important limitations remain, especially for stillbirths. A global approach to classification is needed and seems feasible. The new ICD-PM system is an important step forward and improvements will be enhanced by wide-scale use and evaluation. Implementation requires national-level support and dedicated resources. Variation in stillbirth rates across high-income countries and large equity gaps within high-income countries persist. If all high-income countries achieved stillbirth rates equal to the best performing countries, 19,439 late gestation (28 weeks or more) stillbirths could have been avoided in 2015. The proportion of unexplained stillbirths is high and can be addressed through improvements in data collection, investigation, and classification, and with a better understanding of causal pathways. Substandard care contributes to 20-30% of all stillbirths and the contribution is even higher for late gestation intrapartum stillbirths. National perinatal mortality audit programmes need to be implemented in all high-income countries. The need to reduce stigma and fatalism related to stillbirth and to improve bereavement care are also clear, persisting priorities for action. In high-income countries, a woman living under adverse socioeconomic circumstances has twice the risk of having a stillborn child when compared to her more advantaged counterparts. Despite the frequency of stillbirths, the subsequent implications are overlooked and underappreciated. We present findings from comprehensive, systematic literature reviews, and new analyses of published and unpublished data, to establish the effect of stillbirth on parents, families, health-care providers, and societies worldwide. Data for direct costs of this event are sparse but suggest that a stillbirth needs more resources than a livebirth, both in the perinatal period and in additional surveillance during subsequent pregnancies. Indirect and intangible costs of stillbirth are extensive and are usually met by families alone. This issue is particularly onerous for those with few resources. Negative effects, particularly on parental mental health, might be moderated by empathic attitudes of care providers and tailored interventions. The value of the baby, as well as the associated costs for parents, families, care providers, communities, and society, should be considered to prevent stillbirths and reduce associated morbidity., (Copyright © 2016 Elsevier Ltd. Effective and coordinated teamworking is key to achieving safe birth for mothers and babies. Confidential enquiries have repeatedly identified deficiencies in teamwork as factors contributing to poor maternal and neonatal outcomes. The ingredients of a successful multi-professional team are varied, but research has identified some fundamental teamwork behaviours, with good communication, proficient leadership and situational awareness at the heart. Simple, evidence-based methods in teamwork training can be seamlessly integrated into a core, mandatory obstetric emergency training. Training should be an enjoyable, inclusive and beneficial experience for members of staff. Training for intrapartum emergencies is a promising strategy to reduce preventable harm during birth; however, not all training is clinically effective. Many myths have developed around such training. These principally derive from misinformed beliefs that all training must be effective, cheap, independent of context and sustainable. The current evidence base for effective training supports local, unit-based and multi-professional training, with appropriate mannequins, and practice-based tools to support the best care. Training programmes based on these principles are associated with improved clinical outcomes, but we need to understand how and why that is, and also why some training is associated with no improvements, or even deterioration in outcomes. Effective training is not cheap, but it can be cost-effective. Insurers have the fiscal power to incentivise training, but they should demand the evidence of clinical effect; aspiration and proxies alone should no longer be sufficient for funding, in any resource setting., (Copyright © 2015 Elsevier Ltd. Background: Pregnancies are increasingly seen in women with a gastric band, but no guidance exists on band management during pregnancy. Although band inflation can prevent excessive gestational weight gain and its associated complications, it might have detrimental effects on fetal growth. We compared maternal and perinatal outcomes according to band management strategy-keeping the band inflated throughout pregnancy versus deflation., Methods: Data were collected by means of the UK Obstetric Surveillance System (UKOSS) on all pregnancies in women with a laparoscopic adjustable gastric band, booking in UK maternity units (Nov 1, 2011, to Oct 31, 2012). Maternal and perinatal outcomes were compared according to band management strategy, with women in a control group who had not undergone the procedure and with national data. Multivariable regression analyses were used to adjust for potential confounders., Findings: 109 cases were reported (prevalence 1·7 per 10 000 maternities), of whom 42 underwent band deflation and 54 had inflation maintained (remainder unknown). Mean weight gain was higher with deflation than inflation (15·4 kg [95% CI 10·8-20·0] vs 7·6 [3·7-11·5], p=0·047). Some evidence of a higher risk of gestational hypertension with deflation than with inflation was noted (relative risk [RR] 6·86, p=0·07). There was strong evidence of a high risk of gestational hypertension with deflation compared with controls and national data (RR 4·74, p=0·001). Mean birth weight was significantly lower in the inflation group than in the deflation group (3380 g [95% CI 3255-3505] vs 3712 [3572-3851], p=0·002). Infants of women with deflation had a high risk of macrosomia compared with controls (adjusted RR 0·40, p=0·002) and national data (RR 2·04, p=0·01)., Interpretation: Pregnant women with a laparoscopic adjustable gastric band are high risk; the monitoring of both fetal and maternal wellbeing is essential. Maintainance of band inflation during pregnancy reduces gestational weight gain and associated complications, but affects fetal growth. Therefore, maintainance of band inflation throughout pregnancy cannot be recommended. However, inflation for part of the pregnancy might improve some maternal outcomes. Poor teamwork results in preventable morbidity and mortality for mothers and babies. Suboptimal communication and lack of leadership cost not only lives but also money that is diverted from clinical care to insurance and litigation. Avoidable harm is usually not the result of staff failing their duty of care, it is the result of poor training failing hard-worked staff. A few simple teamwork and leadership behaviours can make a huge difference to outcome and experience for women and their companions, yet they are often missing from maternity care. Recent research has identified the problems and solutions, including the best way to train maternity teams to make a palpable difference. Objective: Our objective was to evaluate a prospective monitoring and quality improvement system for studying trends in the rates of an adverse neonatal outcome, the low Apgar scores (Apgar score <7)., Study Design: A cumulative sum (CUSUM) chart-based system was used to monitor the rate of low Apgar scores over 2 years. Root cause analysis (RCA) was used to investigate for causes of periods of increased low Apgar score rates., Results: A period of deteriorated outcome (increased rates of low Apgar) occurred in August 2006. RCA identified deficiencies in cardiotocograph education, which were addressed by targeted training and mentoring. Prompt resolution followed, with the rates returning to baseline and staying within acceptable limits through to the end of evaluation in December 2007., Conclusion: Prospective and continuous monitoring of clinical outcomes using the CUSUM chart method is feasible and may be beneficial. Early detection of an adverse trend allows for timely corrective action, and may lead to overall improvements in performance.
