Background: Recent studies have demonstrated a global decline in the age at menarche. Our study aimed to determine the age at menarche of Taiwanese women born between 1943 and 1989. Methods: Data were obtained from the Taiwan Biobank. To view the trends in age at menarche, we analyzed data from 74,799 women. The mean, standard deviation, and annual percentage change in age at menarche were calculated for birth-year cohorts. Results: The mean age at menarche of Taiwanese women born in 1943 was 14.85 years. The age at menarche decreased to 12.20 years for those born in 1989. The mean age at menarche declined by 2.65 years across the 47-year study period; hence, the reduction rate was 0.56 years per decade. This study demonstrated a downward secular trend in the age at menarche of Taiwanese women born between 1943 and 1989. This trend occurred in three stages of decline: fast (1943–1953), slow (1953–1965), and moderate (1965–1989). Conclusion: The age at menarche decreased by 2.65 years among Taiwanese women born in 1943 compared with those born in 1989. This decline occurred in three stages: fast (1943–1953), slow (1953–1965), and moderate (1965–1989). This significant downward secular trend in age at menarche reflects Taiwan's socioeconomic development.

Background: Osteoporosis is a common metabolic bone disease that benefits from many newly developed anti-osteoporosis medications (AOMs). Reimbursement policies need to allocate medical budgets properly based on evidence-based data. This study aimed to investigate the 11-year secular trend, focusing on older age and males in this adjustment wave of the National Health Insurance reimbursement. Methods: We adopted a nationwide cohort from Taiwan's National Health Insurance Research Database (NHIRD). Patients undergoing newly initiated AOMs from 2008 to 2018 were included. Objective: The main aim of this study was to verify whether the secular trend stopped in Italy by comparing the results of a 1990–2000 birth cohort versus a 1980–1990 birth cohort of Italian young women. The results were used to speculate about age at menarche as adaptive response to non-genetic factors. Methods: In 2016, a study was set on 413, 18-to-26 year-old women (1990–2000 birth cohort) attending two Italian Universities by web-based, self-reported questionnaires. Previously in 2000, a research including 3,783 high school female students (1980–1990 birth cohort) was led. The age at menarche distribution was performed by Kaplan-Meier analysis. The comparison between the findings of the two birth cohorts was performed by Wilcoxon sum-rank test. Mixed models analysis was applied to evaluate the effect of cohort and socio-economic status on age at menarche. Results: 1990–2000 cohort's age at menarche median was 12.44y (95%CI 12.37; 12.59y). There was no significant difference with age at menarche of the previous cohort (p = 0.56). Consistently, the advance of age at menarche in comparison to the mothers' one was not significantly different between the two cohorts (-0.27y±0.10y vs -0.25y±0.03y, p = 0.33). The socio-economic level was not significantly associated with menarcheal age. Conclusions: The findings of this study confirm that, like in other developed countries, the advance of age at menarche has stopped in Italy, consistently with the stop of the improvement of socio-economic conditions. Further studies are needed to explore the differential effect of each non-genetic factor to outline future scenarios of human sexual maturation. Diabetic ketoacidosis (DKA) is a fatal complication of diabetes, and is strongly related to the quality of diabetes care. This study aimed to establish recent secular trends of incidence density (ID) of DKA in patients with diabetes in Taiwan and to explore the effects of age and sex on the risk of a DKA emergency. Methods: Using the National Health Insurance claims data, we identified annual diabetic cohorts (around 650,000 diabetic patients annually) from 1997 to 2005, and searched for possible admissions due to DKA emergencies. We performed log-linear regression analysis to assess the trend in ID of DKA and to assess the independent effects of age and sex on the risk of DKA. Results: Although the annual ID of DKA showed a slight but significant declining trend over the study period, from 6/1000 to 5/1000 person-years, the age/sex-specific analysis indicated that female patients aged ≤ 34 years showed a significantly increasing trend. Multivariate analysis revealed that calendar year and sex had significant but small independent effects on risk of DKA admission. In contrast, age was strongly associated with risk of DKA emergency. Sexual maturation in humans is characterized by a unique individual variability. Pubertal onset is a highly heritable polygenic trait but it is also affected by environmental factors such as obesity or endocrine disrupting chemicals. The last 30 years have been marked by a constant secular trend toward earlier age at onset of puberty in girls and boys around the world. More recent data, although more disputed, suggest an increased incidence in idiopathic central precocious puberty. Such trends point to a role for environmental factors in pubertal changes. Animal data suggest that the GnRH-neuronal network is highly sensitive to endocrine disruption during development. Background: In children with isolated unilateral undescended testis mechanical anomalies are commonly implicated and a diagnosis of simple hypospadius implies that the rest of the external genitalia are normal. Patients with disorders of sexual development, by contrast, have other associated genital anomalies including micropenis and should be referred to the endocrinologist for hormonal assessment before surgical correction of undescended testis or hypospadius. Early diagnosis of abnormal penile size is important but proper assessment begins with defining the normal population-specific age-appropriate reference range. Anogenital distance (AGD) reflects prenatal and early postnatal testosterone levels., Objective: The aim of our study was to establish mean reference values and percentile curves for strtetched penile length (SPL) and AGD in healthy Egyptian males from the age of one month to five years and to determine the mean monthly increase in SPL and AGDs from 1 to 13 months of age (a reflection of mini-puberty)., Study Design: This was a descriptive cross-sectional study conducted in Cairo University and Mataria Hosptals, Egypt to determine SPL and AGD in 2972 Egyptian males aged from one month to five years from October 2016-December 2018. In addition, we measured length/height, weight and body mass index., Results: SPL increased gradually from a mean ± SD of 3.55 ± 0.51 cm in the first year of life to 5.52 ± 0.67 cm by five years of age with a growth from 1 to 12 months of life of 0.6 cm. SPL showed smaller values in infants 6-9 months old compared to younger infants. AGD increased from 7.48 ± 1.47 cm in the first year of life to 12.83 ± 0.58 cm by 5 years of age with a growth from 1 to 12 months of 4.34 cm. SPL and AGD Z-scores correlated positively with each other and with age (months), and Z-scores of height/length, weight and BMI (p < 0.001)., Discussion: The rapid increases in SPL and AGD observed in our study group in the first few months of life reflect the effects of mini-puberty. The fact that SPL and AGD correlated positively with other anthropometric measurements suggests that SPL and AGD may be controlled by nutritional and/or hormonal factors. We suggest that waning testosterone levels marking the end of minipuberty might explain smaller values for SPL in our group of 6-9 month old infants compared with younger infants., Limitation: We have not included children under one month old., Conclusion: It is important for each country to develop its own national percentile curves for all growth parameters. Background: Intracranial recordings are integral to evaluating patients with pharmacoresistant epilepsy whom noninvasive testing fails to localize seizure focus. Although stereo-electroencephalography is the preferred method of intracranial recordings in most centers, subdural electrode (SDE) implantation is necessary in selected cases., Objective: To identify imaging correlates that predict SDE complications (extra-axial fluid collections [EFCs]), and determine if modifications that diminish stiffness of electrode sheets reduce complications., Methods: A prospective epilepsy surgery database was used to identify adults undergoing craniotomy for SDE implantation over a 14-year period. EFCs and midline shift were measured via magnetic resonance imaging and computed tomography imaging. Correlation analyses and multivariable logistic regression explored associations between use of conformal arrays, serial order of patients, previous ipsilateral intracranial surgery, midline shift, number of SDEs, and neurologic complications., Results: A total of 111 consecutive patients (59 female) underwent 117 craniotomies (mean, 115 electrode contacts) for SDE implantation. There were 8 surgical complications, 3 in the first 17 (17.7%). and 5 (after electrode modifications) in a subsequent 100 craniotomies (5.0%). We noted an increase in electrode numbers implanted over time (P < 0.001) and decreased midline shift with conformal grids (ρ = - 0.32; P < 0.001). A multivariable regression showed that midline shift correlated with complications (odds ratio, 2.32; 95% confidence interval, 1.12-4.78; P = 0.023)., Conclusions: Hemorrhagic complications after SDE implantation are difficult to detect because of artifact from electrodes, but predictable by prominent midline shift (>4 mm). Risks inherent to SDE implantation may be minimized using conformal grids. Objectives: The objective of this study was to investigate secular trends in adolescent health status and healthcare consultations., Study Design: The design of this study is cross-sectional comparisons of population survey outcomes., Methods: Canadian national population data from 2007 to 2016 (T1 N = 14,223; T2 N = 14,247; T3 N = 13,532; T4 N = 13,184; T5 N = 11,122; Total N = 66,308) were examined to find trends in health diagnosis (chronic illnesses and mental disorders) and healthcare consultation (general health practitioners and mental health professionals). Controlling demographics, Multivariate analysis of covariances (MANCOVAs) and correlations were carried out to compare differences by age group (A1: 12-14 years N = 25,180; A2: 15-17 years N = 25,825; A3: 18-19 years N = 15,303) and gender (girls N = 32,388; boys N = 33,920) across survey years., Results: Steady increases were found in diagnosed mental disorders and consultations with a mental health professional (MP) for girls, while chronic illnesses remained stable and general practitioner consultations declined for all adolescents over these years. Gender disparity in MP consultations grew with age, much more in recent years, whereas chronic illness diagnoses curved down for all in midadolescence. More integrated relations between health status and healthcare utilization were noted in T5 than in T1., Conclusions: Differential secular trends were shown for adolescent physical versus mental health statuses and relevant healthcare consultations. The onset of puberty strongly depends on organizational processes taking place during the fetal and early postnatal life. Therefore, exposure to environmental pollutants such as Endocrine disrupting chemicals (EDCs) during critical periods of development can result in delayed/advanced puberty and long-term reproductive consequences. Human evidence of altered pubertal timing after exposure to endocrine disrupting chemicals is equivocal. However, the age distribution of pubertal signs points to a skewed distribution towards earliness for initial pubertal stages and towards lateness for final pubertal stages. Such distortion of distribution is a recent phenomenon and suggests environmental influences including the possible role of nutrition, stress and endocrine disruptors. Rodent and ovine studies indicate a role of fetal and neonatal exposure to EDCs, along the concept of early origin of health and disease. Such effects involve neuroendocrine mechanisms at the level of the hypothalamus where homeostasis of reproduction is programmed and regulated but also peripheral effects at the level of the gonads or the mammary gland., (Copyright © 2019 Elsevier Ltd. Introduction: Recent publications report a decrease in the age of onset of pubertal changes in the United States and Europe. The PROSPEL study (PRemier Observatoire des Stades Pubertaire en Libéral) will provide the first French data on the age at which pubertal signs appear. Before considering this work at the national level, we wanted to assess its feasibility., Materials and Methods: Private pediatricians and general practitioners were recruited in Bordeaux and Toulouse. Before participating in the study, they had been trained in pubertal assessment and then evaluated. Each physician completed 1-4 weeks of inclusion during which children aged 5-18 years seen in consultation were included, except children who were adopted or had a chronic condition. Pubertal stages were assessed according to Tanner's classification and using an orchidometer for testicular volume. The causes of non-realization were documented where appropriate. Inter-individual reproducibility was analyzed by a double examination for a number of children who had previously given their consent., Results: In total, 63 physicians participated in the study (35 pediatricians and 28 general practitioners). All were certified at the end of the training session. A total of 2646 children were included (1318 girls, 1328 boys) with a homogeneous age distribution. The study was carried out in 83.5% of cases. The main reason for non-realization was that physicians did not propose the study to patients who met the criteria (10.1%). Lack of time was the main reason. Inter-individual reproducibility was excellent., Discussion and Conclusion: Our results attest to the good feasibility of the PROSPEL study. The relationship between the general factor of intelligence (g) and handedness is investigated using a combined sample of 23511 respondents from three large databases: the NLSY'79 (US), NLSY'97 (US) and NCDS (UK). Dextrals - those who use their right hands were found to be 1.22 IQ points higher than sinistrals (left handers) after controlling for sex and age and correcting for sources of measurement error. To see if the association between IQ and handedness was strongest on the abilities that were the best measures of g, the method of correlated vectors was used to test for moderation. Across the three studies, g was found to very weakly negatively moderate the association between ability measure and handedness (ρ = -.023, K = 3, N = 23511), however in the NLSY'79, the coding speed subtest was an outlier in terms of the strength of its association with handedness. Its removal yielded indications of positive moderation in this dataset, which when aggregated boosted the overall vector correlation value to .539 (K = 3, N = 23511), suggesting that g might be an important moderator of this relationship. Secondary analysis of secular trend data on the changing percentage of sinistrals in Western populations indicates that overall, sinistrality has increased, entailing a g decline of .106 points over 150 years (.006 points per decade). The secular increase in sinistrality is consistent with other data indicating long-term declines in developmental stability and may stem from some combination of increasing mutation load and environmental stress in Western populations., (Copyright © 2018 Elsevier GmbH. Background: Accumulating data has revealed a rapidly rising incidence of pancreatic cancer in Western countries, but convincing evidence from the East remains sparse. We aimed to quantify how the incidence and mortality rates of pancreatic malignancy changed over time in Taiwan, and to develop future projection for the next decade., Methods: This nationwide population-based study analyzed the Taiwan National Cancer Registry and the National Cause of Death Registry to calculate the annual incidence and mortality rates of pancreatic malignancy from 1999 to 2012 in this country. The secular trend of the incidence was also examined by data from the National Health Insurance Research Database., Results: A total of 21,986 incident cases of pancreatic cancer and 20,720 related deaths occurred during the study period. The age-standardized incidence rate increased from 3.7 per 100,000 in 1999 to 5.0 per 100,000 in 2012, with a significant rising trend (P<0.01). The increase was nationwide, consistently across subgroups stratified by age, gender, geographic region, and urbanization. Data from the National Health Insurance Research Database corroborated the rise of incident pancreatic cancer. Mortality also increased with time, with the age-standardized rate rising from 3.5 per 100,000 in 1999 to 4.1 per 100,000 in 2012 (P<0.01). In accordance with the incidence, the mortality trend was consistent in all subgroups. Both the incidence and mortality were projected to further increase by approximately 20% from 2012 to 2027., Conclusion: The incidence and mortality of pancreatic cancer have been rapidly rising and presumably will continue to rise in Taiwan., (Copyright © 2017 Elsevier Ltd. Human adult height has been increasing world-wide for a century and a half. The rate of increase depends on time and place of measurement. Final height appears to have reached a plateau in Northern European countries but it is still increasing in southern European countries as well as Japan. While mean birth length has not changed recently in industrialized countries, the secular trend finally observed in adult height mostly originates during the first 2 years of life. Secular trend in growth is a marker of public health and provides insights into the interaction between growth and environment. It has been shown to be affected by income, social status, infections and nutrition. While genetic factors cannot explain such rapid changes in average population height, epigenetic factors could be the link between growth and environment., (Copyright © 2017. Numerous studies have established the presence of secular trends and geographical variations in sex ratio at birth, albeit with mixed and often contradictory results. In addition, a multitude of environmental, social, economic, demographic and other factors has been proposed to influence the sex ratio at birth, thus complicating the interpretation of both secular trends and geographical variations. In this paper, the current state of knowledge on these issues is presented and critically assessed. Analyzing longer time series of sex ratio at birth with possible cycles and random components is given priority over establishing simple linear trends in the data. In analyzing the geographical variation in the sex ratio at birth, two different levels of analysis are distinguished (global and local), and two different sets of factors affecting the sex ratio at birth are proposed accordingly. Most studies on the prevalence of supplement use in the United States were cross-sectional or evaluated trends in limited variety of supplements. The objective of this study was to describe the longitudinal and secular trend of dietary supplement use over the past 20 years in health professionals using data from two large prospective cohorts. We analyzed cohort data from 1986 to 2006 in the Nurses' Health Study (NHS) and the Health Professionals Follow-Up Study (HPFS). In 1986, 74,194 women aged 40 to 65 years in the NHS and 50,497 men aged 40 to 75 years in the HPFS were included. Use of dietary supplements including multivitamins, vitamins, and minerals was repeatedly asked every 4 years. Generalized estimating equation models were used for repeated analysis. Prevalence of use of any supplement increased among both women (71.3% to 88.3%) and men (56.4% to 80.7%) from 1986 to 2006. Notably, longitudinal increases in the prevalence of use of vitamin D (2.2% to 32.2% for women and 1.1% to 6.7% for men), folic acid (0.8% to 10.7% for women and 1.1% to 13.8% for men), and fish oil (1.6% to 18.1% for women and 3.3% to 22.2% for men) supplements were observed from 1990 to 2006. However, the use of vitamin A, beta carotene, vitamin C, and vitamin E supplements peaked in 1994 or 1998, then declined steadily. A secular increase in use of multivitamins, vitamin D, folic acid, and fish oil across same age group was noted. In conclusion, the use of many types of dietary supplements has increased over time, but the use of antioxidant supplements has declined. The secular increase in the prevalence of use of supplements across the same age group suggests that aging of the population is not the primary reason for the increase.
