Introduction: Bronchobiliary fistula (BBF) is an abnormal connection between the bronchial system and biliary tree. It is a rare condition that often manifests as bilioptysis. BBF has several causes; however infectious disease of the liver and lungs, mostly hydatid cysts, is the leading cause of BBF. For a conclusive diagnosis, imaging studies are useful and able to show the fistula tract, as we did by CT scan. The treatment of BBF is still a challenge for surgeons. In children, in the acute fulminating phase, an aggressive and rapid approach is essential to prevent respiratory tract damage. Case presentation: We present a case of a 17-year-old girl, who complained of bilioptysis, chest pain and loss of appetite for about nine months. Her past history revealed that she developed bilioptysis after laparotomy for excision of liver hydatid cyst 10 months ago. For which she underwent conservative and surgical treatment of bilioptysis, which all failed. After investigations, a bronchobiliary fistula was identified. A right thoracotomy was performed. After the release of adhesions and debridement of damaged tissues, the fistula tract was excised. The connection was sealed by a diaphragmatic muscle flap, which was successful in the outcome after 12 months of follow-up. Conclusion: BBF is a condition that requires strong presumptive evidence for diagnosis and a well-planned approach for management. Hence, each surgeon must carefully consider the optimal method for open surgery or conservative therapy after thoroughly evaluating the patient's profile and imaging. In children, a thoracic approach with a diaphragmatic flap is safe, easily available, and can easily divide the lung from the liver since that is its normal function.

Textile industries contribute to water pollution through synthetic dye discharge. This study explores the use of natural bio-coagulants to remove acid dyes from wastewater, investigating factors like pH, coagulant dose, dye concentration, contact time, and temperature for optimal results. The optimum pH and coagulants capabilities of (CAAPP, CAAPH, CBAGL, CBAPP and CBAPH) were 3 (49.6 mg/g), 3 (42.5 mg/g), 3 (38.9 mg/g), 4 (35.7 mg/g), 4 (34.1 mg/g), and 4 (29.4 mg/g) respectively, while treating of selected BRF-221 dyes from water solution. The acidic range (3-4) was found to have the best pH for the maximal coagulation, and the optimal dose were found to be 0.05 g/50 mL. The equilibrium was attained within 45-60 min for all coagulants. After 60 min of shaking, the maximum coagulation capacities (21.9, 21.02, 16.5, 27.9, 25.3, and 23.4 mg/g) of several coagulant composites (CAAGL, CAAPP, CAAPH, CBAGL, CBAPP, CBAPH) were determined. The initial BRF-221 dye concentration in the range of 10-200 mg/L was considered as optimum for gaiting maximum elimination of dye using different coagulants. At a dye value of 100 mg/L of BRF-221, maximal coagulation capacities CAAGL (179.19 mg/g), CAAPP (166.06 mg/g), CAAPH (141.60 mg/g), and CBAGL (126.49 mg/g), CBAPP (113.9 mg/g), CBAPH (93.08 mg/g) were attained. The study found 35 °C to be the optimal temperature for maximum acid dye removal using bio-coagulants. Increasing temperature reduced coagulation capacity, indicating an exothermic process. Freundlich and Langmuir isotherms showed suitability for pseudo-first-order and pseudo-second-order kinetics in biosorption. Thermodynamic parameters were assessed for process feasibility. Effective coagulants demonstrated sensitivity to electrolyte variations. In column studies, adjusting parameters achieved maximum coagulation efficiency for removing BRF-221 dyes. The study successfully applied optimal parameters to remove real textile effluents at a practical scale. Congo-red (CR), a precursor of textile products and a contaminant of great concern, has contaminated aquatic environments. Here, we explored the synthesis of mesoporous nano-zerovalent manganese (nZVMn) and Phoenix dactylifera leaves biochar (PBC) composite for the removal of CR from water. The nZVMn/PBC adsorbed 117.647 mg/g of CR versus 25.316 mg/g by PBC at [CR] 0 = 20 mg/L and [PBC] 0 = [nZVMn/PBC] 0 = 500 mg/L. Variation of [nZVMn/PBC] 0 , [CR] 0 and pH influenced the adsorption of CR. Freundlich adsorption isotherm and pseudo-first-order kinetic models best fitted CR adsorption. The H 2 O 2 coupling with nZVMn/PBC promoted removal of CR possibly due to the formation of hydroxyl radical ( ● OH) and caused 95 % removal of CR versus 77 % by nZVMn/PBC alone. The ● OH scavengers inhibited the removal of CR. The nZVMn/PBC showed a good reusability and efficient removal of CR up to the seventh cycle of treatment. Results reveal that nZVMn improved performance, thermal stability and reusability of biochar. Degradation products from ● OH-mediated degradation of CR were studied by ultraperformance liquid chromatography with mass spectrometric detector to establish degradation pathways. The ion-chromatographic analysis showed the formation of non-toxic inorganic acetate product, which suggests high potential of the newly fabricated adsorbent in the removal of CR., (Copyright © 2020 Elsevier B.V. Background: The underlying pathways leading to stillbirth in low- and middle-income countries are not well understood. Context-specific understanding of how and why stillbirths occur is needed to prioritise interventions and identify barriers to their effective implementation and uptake., Aim: To explore the contribution of contextual, individual, household-level and health system factors to stillbirth in Afghanistan., Methods: Using a qualitative approach, we conducted semi-structured in-depth interviews with women and men that experienced stillbirth, female elders, community health workers, healthcare providers, and government officials in Kabul province, Afghanistan between October-November 2017. We used thematic analysis to identify contributing factors and developed a conceptual map describing possible pathways to stillbirth., Findings: We found that low utilisation and access to healthcare was a key contributing factor, as were unmanaged conditions in pregnancy that increased women's risk of complications and stillbirth. Sociocultural factors related to the treatment of women and perceptions about medical interventions deprived women of interventions that could potentially prevent stillbirth. The quality of care from public and private providers during pregnancy and childbirth was a recurring concern exacerbated by health system constraints that led to unnecessary delays; while environmental factors linked to the ongoing conflict were also perceived to contribute to stillbirth. These pathways were underscored by social, cultural, economic factors and individual perceptions that contributed to the three-delays., Discussion: Efforts are needed at the community-level to facilitate care-seeking and raise awareness of stillbirth risk factors and the facility-level to strengthen antenatal and childbirth care quality, ensure culturally appropriate and respectful care, and reduce treatment delays., (Copyright © 2020 Australian College of Midwives. Removal of cadmium (Cd 2+ ), a highly toxic heavy metal, from aqueous solutions was investigated using nano zerovalent iron (Fe 0 ). Cadmium was efficiently removed by Fe 0 , although reactivity and reusability of Fe 0 was significantly promoted by coupling with bismuth (Bi). At a reaction time of 20 min, 85% and 96% Cd 2+ was removed by Fe 0 and Bi/Fe 0 , respectively, at first cycle using [Cd 2+ ] 0  = 10 mg/L and [Fe 0 ] 0  = [Bi/Fe 0 ] 0  = 1.0 g/L. However, Cd 2+ removal efficiency was reduced to 12% and 80% at sixth cycle by Fe 0 and Bi/Fe 0 , respectively. The X-Ray diffraction and energy dispersive X-Ray spectroscopy analysis proved successful formation of Fe 0 by the chemical reduction method and also confirmed coupling of Bi with Fe 0 to form bimetallic Bi/Fe 0 . The oxidation of Fe 0 and Bi/Fe 0 yielded electron that played significant role in the conversion of toxic Cd 2+ into non-toxic Cd 0 . The reactivity of electron with Cd 2+ was calculated to be 4.3 × 10 9  M - 1  s -1 . The pH of solution showed pronounced effects on the reactivity of both Fe 0 and Bi/Fe 0 . Removal of Cd 2+ by both Fe 0 and Bi/Fe 0 followed pseudo-first-order kinetic model. The conversion of Cd 2+ into non-toxic Cd 0 proved Fe 0 and Bi/Fe 0 to be highly efficient and rewarding in detoxification of Cd 2+ and other toxic metals in aqueous environments., (Copyright © 2019 Elsevier B.V. The removal of brilliant green (BG), a toxic organic and cationic dye, has been examined by UV/S 2 O 8 2- (PS), UV/HSO 5 - (PMS) and UV/H 2 O 2 processes. BG showed insignificant direct photolysis at 254 nm (i.e., 8.6% after 30 min). However, enhanced BG degradation was observed in UV/PS, UV/PMS and UV/H 2 O 2 systems as revealed from 63.1, 47.0 and 34.8% BG degradation, respectively, at 30 min of reaction time, using 0.05 mM BG and 1.0 mM oxidant initial concentration. The bimolecular rate constants of OH and SO 4 - with BG were determined to be 2.35 × 10 9 and 2.21 × 10 9 M -1 s -1 , respectively. Electrical energy per order (EE/O) values for UV/PS, UV/PMS and UV/H 2 O 2 processes were calculated to be 5.4, 6.8, and 7.8 KWh/m 3 /order, respectively. The addition of humic acid (HA) and inorganic anions inhibited the degradation of BG by UV/PS in the order of NO 2 -  > HA > HCO 3 -  > Cl -   > NO 3 -  ≈ SO 4 2- . The results of frontier electron densities (FEDs) showed that C-atom holding the three rings (C7), and C-atoms at para positions to N-alkyl groups of the two rings (C4 and C14) are the predominant sites for radical addition.
