In contemporary manufacturing industries, composite materials such as carbon fiber reinforced plastics (CFRPs) have become indispensable, finding extensive applications in aerospace, automotive, shipbuilding, high-tech sports equipment, and more. The exceptional chemical, physical, and mechanical properties of CFRPs, including high corrosion resistance, superior strength-to-weight ratio, high fatigue strength, and oxidation resistance, make them highly sought after. However, these very advantages pose significant challenges during machining due to the abrasive nature of composites, anisotropic mechanical properties, and poor thermal conductivity, which collectively exacerbate tool wear. This review paper addresses the critical need for innovative solutions to improve the machinability of CFRPs, a subject of paramount importance for advancing manufacturing efficiency and product quality. It comprehensively examines the latest research progress, current practices, and emerging trends in machining techniques such as turning, drilling (including reaming and countersinking), milling and grinding. The review delves into the influence of various cutting conditions, environments, and tool geometries. It also examines tool textures, materials, and coatings. Additionally, advanced machining methods, including vibration, thermal, and hybrid-assisted machining, are discussed. These factors impact key performance metrics such as cutting forces and torques, tool wear and life, chip morphology, surface roughness, and the quality of machined surfaces, including defect analysis. By synthesizing a vast array of literature for the first time, this paper highlights the most effective strategies to enhance machinability while extending tool life and improving surface finish. Notably, it underscores the innovative use of dry and flood lubrication, minimum quantity lubrication, cryogenic lubrication, and high-pressure cooling. Additionally, it emphasizes optimizing cutting tool geometry, employing diamond tools, and coating tools with TiN and TiAlN, alongside the application of heat treatment and hybrid machining methods, particularly those incorporating vibration machining techniques. This review not only identifies the current challenges but also proposes cutting-edge solutions, making it an essential resource for researchers and practitioners aiming to push the boundaries of CFRP machining technology.

While direct logistics optimization is a common practice, reverse logistics management is frequently neglected. This study explores how reverse logistics configurations affect the environmental sustainability of global supply chains in the fashion industry. The paper addresses the gap in the literature regarding carbon footprint of hybrid configurations for return flows. A case study is conducted in an Italian luxury fashion company with global operations, and we use supply chain simulation to assess sustainability-focused network design alternatives. The findings reveal that decentralizing specific operations significantly reduces carbon footprint without compromising service levels or lead time, both critical aspects in such segment. This research emphasizes the importance of considering reverse logistics in the overall sustainability strategy of supply chains, offering practical insights for businesses seeking to balance environmental impact with operational efficiency in the fashion industry.

Introdução/objetivos: : O gênero Rhodococcus pertence à Família Nocardiaceae, Ordem Actinomicetos. Rhodococcus hoagii é a espécie mais relacionada à infecção humana, especialmente entre hospedeiros imunossuprimidos. As infecções pulmonares são a forma mais comum de acometimento. A cavitação surge em mais de 50% dos casos e em regiões endêmicas o quadro pode se assemelhar à tuberculose (TB). O objetivo deste trabalho é relatar um caso de infecção pulmonar grave por Rhodococcus sp., em PVHIV. Apesar do aumento significativo dos casos com a epidemia de HIV, a infecção por Rhodococcus ainda é um diagnóstico pouco considerado. Materiais e métodos: Revisão de prontuário, discussão com equipe médica e revisão de literatura. Resultados: Mulher cis de 30 anos, admitida em novembro de 2023 em enfermaria de hospital de Infectologia, com quadro há 2 meses de perda ponderal, icterícia e edema de membros inferiores. A TC de tórax da admissão evidenciou consolidações associadas a escavações de permeio em lobo pulmonar direito. Feito o diagnóstico de HIV, com CV 531.000 cópias (log 5.72) e CD4 20 células (3.6%). Seguindo a investigação, o teste LF-LAM para TB veio positivo com BAAR e TRM em escarro negativo. Recebeu alta após 1 mês de internação, com prescrição de RHZE e acompanhamento ambulatorial. Em abril de 2024 retorna após perda de seguimento clínico, com quadro de dispneia, astenia e tosse produtiva há 1 mês, sendo readmitida no CTI com insuficiência respiratória e instabilidade hemodinâmica. Foi intubada e houve a necessidade de aminas em doses elevadas. Apresentou piora radiológica em TC de tórax realizada nesta admissão, com evidência de extensa consolidação em lobo pulmonar direito, novas escavações bilaterais, com a maior delas apresentando nível hidroaéreo sugestivo de abscesso. Coletadas hemoculturas, bem como baciloscopia, TRM e cultura de secreção traqueal, sendo iniciado tratamento empírico com cefepime e amicacina. Após identificação de Rhodococcus hoagii nas culturas foram prescritos claritromicina, levofloxacino e vancomicina. CONCLUSÃO Este caso destaca o caráter oportunista da infecção por bactérias do gênero Rhodococcus em indivíduos com imunossupressão grave. Em função da semelhança clínica e radiológica com infecções por agentes mais prevalentes como micobactérias e fungos, a infecção por Rhodococcus sp. acaba por ser um diagnóstico diferencial pouco considerado, merecendo a atenção dos profissionais envolvidos neste tipo de cuidado. Summary: Background and aims: A healthy diet is one of the pillars of familial hypercholesterolemia (FH) treatment. However, the best dietary pattern and indication for specific supplementation have not been established. Our aim is to conduct a pilot study to assess the effect of an adapted cardioprotective diet with or without phytosterol and/or krill oil supplement in participants with a probable or definitive diagnosis of FH, treated with moderate/high potency statins. Methods: A national, multicenter, factorial, and parallel placebo-controlled randomized clinical trial with a superiority design and 1:1:1:1 allocation rate will be conducted. The participants will undergo whole exome sequencing and be allocated into four treatment groups: 1) a cardioprotective diet adapted for FH (DICA-FH) + phytosterol placebo + krill oil placebo; 2) DICA-FH + phytosterol 2 g/day + krill oil placebo; 3) DICA-FH + phytosterol placebo + krill oil 2 g/day; or 4) DICA-FH + phytosterol 2 g/day + krill oil 2 g/day. The primary outcomes will be low-density lipoprotein (LDL)-cholesterol and lipoprotein (a) levels and adherence to treatment after a 120-day follow-up. LDL- and high-density lipoprotein (HDL)-cholesterol subclasses, untargeted lipidomics analysis, adverse events, and protocol implementation components will also be assessed. Results: A total of 58 participants were enrolled between May–August 2023. After the end of the follow-up period, the efficacy and feasibility results of this pilot study will form the basis of the design of a large-scale randomized clinical trial. Conclusions: This study's overall goal is to recommend dietary treatment strategies in the context of FH.

Recent cyclosporiasis outbreaks associated with fresh produce grown in the United States highlight the need to better understand Cyclospora cayetanensis prevalence in U.S. agricultural environments. In this study, C. cayetanensis occurrence was assessed in municipal wastewater sludge, on-farm portable toilets, irrigation pond water, and spent packing house dump tank water in a Southeastern Georgia growing region over two years. Detection of the C. cayetanensis 18S rRNA qPCR gene target in pond samples was 0%, 28%, and 42% (N = 217) depending on the detection definition used, and ≤1% in dump tank samples (N = 46). However, no qPCR detections were confirmed by sequencing, suggesting false detection occurred due to cross-reactions. C. cayetanensis qPCR detections were confirmed in 9% of wastewater sludge samples (N = 76). The human-specific fecal markers HF183 and crAssphage were detected in 33% and 6% of pond samples, respectively, and 4% and 0% of dump tank samples, respectively. Despite community Cyclospora shedding and evidence of human fecal contamination in irrigation water, there was no correlation between C. cayetanensis and HF183 qPCR detections, further supporting that 18S gene target qPCR amplifications were due to cross−reactions. When evaluating C. cayetanensis qPCR environmental detection data, the impact of assay specificity and detection criteria should be considered. Moreover, additional sequence-based testing may be needed to appropriately interpret Cyclospora qPCR environmental data.

Male cat, 2 years old, with a refractory infection by Sporothrix brasiliensis, presents a single nodular lesion in the left auricular pavilion. To confirm the diagnosis, cytology, fungal culture, antifungal susceptibility test, molecular analysis, and, to aid in the differential diagnosis, bacterial culture, antibiogram, and histopathology of the lesion were performed. In the absence of therapeutic success with conventional antifungals, photodynamic therapy (PDT) was introduced, demonstrating a satisfactory response in the sixth treatment session.

This study primarily focuses on analyzing anelastic processes due to interstitial oxygen in the Ti–25Ta–40Zr alloy. From the structural and microstructural characterization (DRX and optical microscopy), it was detected that the Ti–25Ta–40Zr alloy is a α + β type. Peaks of anelastic relaxation were visualized in the Ti–25Ta–40Zr alloy, where the peaks are thermally activated, with the shift of Qp−1 to higher temperatures in tests performed at high frequencies. The activation energy (E), relaxation time (τ0), and diffusivity (Do) of oxygen in the alloy were obtained E = (0.49 ± 0.02) eV, τ0 = (6.3 ± 0.2) x10−19 s and D0 = (5.0 ± 0.1) x10−3 m2/s.

Optimizing irrigation and nitrogen (N) fertilizer management in irrigated potato crops grown on sandy soils in subtropical regions such as in northeastern Florida, USA is essential to sustain a high yield and to minimize leaching. N applications in this region typically occur at approximately 25–30 days prior to planting (Npre), at emergence (Neme), and at tuber initiation (Nti). However, recent studies suggest that applying N near planting (Npl) enhances fertilizer N use efficiency (FNUE). We combined experimentation with modeling to assess irrigation and N management options for potato in northeastern Florida. We first aimed to evaluate the DSSAT/CSM-SUBSTOR-Potato model using two-year irrigated field experiments conducted on sandy soils with variable N rates and application timings. CSM-SUBSTOR-Potato accurately simulated aboveground plus tuber dry weight [Relative root mean squared error (RRMSE) = 26.4%, Willmott’s index (d) = 0.98] and N accumulation (RRMSE = 28.6%, d = 0.97). Soil moisture and mineral N were captured well overall, but they were often underestimated due to a water table influence that is currently not considered in DSSAT. Subsequently, CSM-SUBSTOR-Potato was applied to simulate tuber yield, N leaching, and FNUE under scenarios of irrigation scheduling and N-fertilizer application (rate/timing) strategies, focusing on Npre versus Npl aiming to improve resource use efficiency. The simulations indicated that a target of 60% and 70% of the available soil water can be safely used as an irrigation strategy to achieve a high yield, while reducing irrigation water applied and N leached to the environment. Overall Npl increased crop N uptake by 10%, tuber yield by 7%, reduced N leached by 13%, and consequently increasing FNUE by 9%, compared to Npre across the irrigation treatments. Thus, Npl should be preferred in sandy soils and climate-risky subtropical environments, along with Neme and Nti as key timings to synchronize N supply with potato growth.

We describe a case of extensive human cutaneous protothecosis in the northeastern of Brazil. The diagnosis was made through histopathological examination of skin samples and culturing on Sabouraud dextrose agar, with identification through mass spectrometry. Treatment with itraconazole failed and was switched to liposomal amphotericin followed by voriconazole with clinical cure. Prothotecosis should be a diagnostic hypothesis when there are skin lesions after trauma, specially in situations of atypical evolution.

Difficult airway management in pediatrics during anesthesia represents a major challenge, requiring a careful approach, advanced technical expertise, and accurate protocols. The task force of the Brazilian Society of Anesthesiology (SBA) presents a report containing updated recommendations for the management of difficult airways in children and neonates. These recommendations have been developed based on the consensus of a panel of experts, with the objective of offering strategies to overcome challenges during airway management in pediatric patients. Grounded in evidence published in international guidelines and expert opinions, the report highlights crucial steps for the appropriate management of difficult airways in pediatrics, encompassing assessment, preparation, positioning, pre-oxygenation, minimizing trauma, and, paramountly, the maintenance of arterial oxygenation. The report also delves into additional strategies involving the use of advanced tools, such as video laryngoscopy, flexible intubating bronchoscopy, and supraglottic devices. Emphasis is placed on the simplicity of implementing the outlined recommendations, with a focus on the significance of continuous education, training through realistic simulations, and familiarity with the latest available technologies. These practices are deemed essential to ensure procedural safety and contribute to the enhancement of anesthesia outcomes in pediatrics.

Insomnia is a highly prevalent sleep disorder, but its first line of treatment (Cognitive Behavioral Therapy for Insomnia - CBTi) is not widely available and affordable. Digital therapy has been an alternative to these limitations and its combination with pharmacological treatment may improve patients' clinical outcomes and therapy adherence. This was a real-world evidence study aimed to evaluate the efficacy of a CBTi smartphone app combined or not with sleep medication, based on a sample of users of a CBTi smartphone app (SleepUp®) registered between June 2020 and June 2022. Individuals were categorized into two groups (“dCBTi alone” and “dCBTi combined”) according to their self-reported use of sleep medication on the first day of treatment. Assessments were made based on the self-report of sleep logs (sleep quality and efficiency) and on insomnia severity (Insomnia Severity Index). Comparisons were made considering the first entry (baseline) and the mean of the last three days in which sleep logs were filled out (final). This study aimed to produce different chambá (Justicia pectoralis Jacq.) extracts, determinate their biocompounds and antimicrobial activity against several foodborne bacterias. Aqueous and hydroalcoholic extracts were prepared using the decoction and maceration method, respectively, and were submitted to cytotoxicity analysis. The samples were submitted to bioactive compound analysis and antimicrobial tests. The effects of the extract on the bacterial cell structures were visualized using Transmission Electron Microscope. Between the samples, the aqueous extract presented higher levels of umbelliferone (8.99 mg/g), total phenolic compounds (17.7 g AGE/100g) and antioxidant activity (765.3 μM TE/g) compared hydroalcoholic extracts. All extracts presented antimicrobial activity against Gram-Positive bacteria group, being the aqueous sample the one most effective presenting the lowest values for minimum inhibitory (MIC) and bactericidal (MIB) concentrations for all microorganisms evaluated, highlighting its action against Staphyloccocus aureaus (MIC = 0.84 and MIB = 1). The bactericidal action of the aqueous extract against the bacteria was observed by the microphotographs with evidenced by a rupture of the bacterial cell wall caused by the exposition to phenolics presented in the extract. Thus, this research demonstrated that beyond being a source of phenolics, chambá presents great potential as a new natural antimicrobial agent, which reinforces the development of more studies to evaluate its effective application in food systems.

This work aimed to characterize the physical-chemical properties, antimicrobial activity, and toxicity of microcapsules elaborated on the combination of green tea (Camelia sinensis) extract with cashew gum and maltodextrin. It was observed that the encapsulation process influenced positively the physical-chemical characteristics of green tea extract (GTE) once green tea microcapsules (GTM) presented values equal to 1.42, 3.66 and 0.36 for humidity, hygroscopicity, respectively, which can be correlated to an increase in GTE stability. GTM exhibited values of 14,950.4 mg GAE/100 g and 1188.4 μMTrolox.g−1 for phenolics and antioxidant activity, respectively, as well as presented an antimicrobial effect against the gram-positive bacteria. No toxicity effect was observed against zebrafish (Danio rerio) but sample administration resulted in a reduction in the locomotor activity of the animals. This study provided valuable information regarding the encapsulation process as an effective tool for increasing the application of green tea extract, considering its antioxidant and antimicrobial properties, reinforcing its safety for consumers.

This study aimed to develop a structured soursop bar and evaluate the bioaccessibility and gastroprotective effects of its compounds after the addition of aqueous acerola co-product extract. Simulated digestion was carried out to determine the bioaccessible percentage of biocompounds and gastric ulcers were induced in mice to test the gastroprotective effects of the samples. The addition of the acerola co-product extract increased the concentration of phenolics (73.6 %), antioxidant activity (82.5 %) and ascorbic acid (more than 100 %). The structured soursop bar enriched with acerola extract was able to prevent macroscopic mucosal damage, histological changes, and oxidative injuries in the gastric mucosa, applied orally. Thus, the structured soursop enriched with the acerola co-product extract can be considered a promising natural product that provides health benefits, especially related to its potential gastroprotective effects.

Problem considered: The rational use of antimicrobials (ATMs) is a challenge for global health services. One strategy used to control the spread of multidrug-resistant (MDR) bacteria is antimicrobial management programs. This is a pre- and post-intervention study that assesses the impact of an antimicrobial stewardship program in a hospital through evaluating clinical and microbiological outcomes, usage measures and costs. Methods: The sample comprised adult patients hospitalized for more than 24 h during a seven-month pre-intervention, and an eleven-month post-intervention period. Primary outcomes included length of therapy (LOT) and days of therapy (DOT) per 1000 patient-days (pd), the DOT/LOT ratio, the incidence of a number of the main multi-drug resistant bacteria, and the costs of therapy. Secondary outcomes were the use of antimicrobials and antimicrobial prescriptions by class, hospital length of stay rates, and mortality rates. Data were obtained from medical prescriptions, pre-authorization forms for restricted antibiotics, and microbiological tests. Non-critical and critical care units were compared, with a significance level of 0.05. Results: A total of 2704 patients were evaluated and there was a significant post-intervention reduction of exposure to antimicrobials of 8.1% and of 90 days in respect of the LOT in non-critical units. There was a reduction of more than 50% in the costs of therapy and of 1.2 days in the length of hospital stay in non-critical units. There was no variation in bacterial resistance. Conclusions: Although the stewardship program and its interventions reduced antimicrobial consumption, costs and the length of hospital stay further studies are required to identify the components that can produce the impact on microbiological outcomes seen in other studies.

Introdução/Objetivo: Tendo em vista o impacto da Síndrome Respiratória Aguda Grave (SRAG) na saúde pública, a busca pela caracterização do perfil dos acometidos é essencial para uma melhor identificação dos casos. O presente trabalho objetivou analisar os casos de SRAG na faixa pediátrica na região Sudeste de acordo com variáveis sociodemográficas e fatores de risco, relacionando ao desfecho de evolução dos casos. Métodos: Realizou-se um estudo transversal, quantitativo e descritivo pela coleta de dados do Sistema de Internação Hospitalar por SRAG em 2021 e 2022 na região Sudeste registrados no DATASUS. Analisaram-se as variáveis sociodemográficas: sexo, idade, raça/cor e distribuição por UF; fatores de risco: diabetes, asma, doença cardiovascular, obesidade, pneumopatias, Síndrome de Down e imunodepressão; classificação final: COVID-19, Influenza, não especificado, outro agente ou outro vírus; e evolução: cura ou óbito. Realizou-se análises descritivas nas análises univariadas, teste qui-quadrado de Pearson para relacionar as faixas etárias com as demais variáveis nas análises bivariadas, e regressão logística nas análises multivariadas, cujo desfecho foi evolução, de modo que foram incluídas as variáveis com p-valor < 0,20 da análise bivariada. O nível de significância adotado foi de 5%. Resultados: A população foi de 1.054.477 pacientes pediátricos hospitalizados com SRAG. Houve maiores chances de óbito nas raças indígena e preta em relação à branca; na faixa etária de 12 anos a menores de 18 anos em relação à faixa etária de menores de 6 meses; nas UF Espírito Santo e Rio de Janeiro em relação à São Paulo; e em todas as variáveis relacionadas a comorbidades, excetuando-se a asma, sendo o diabetes aquela com menor risco aumentado, mas considerada estatisticamente não significativa. Analogamente, encontrou-se como fator de proteção na faixa pediátrica para evolução para óbito os indivíduos asmáticos; nas faixas etárias de 6 meses a menores de 5 anos e de 5 anos a menores de 12 anos em relação aos menores de 6 meses; e na classificação final influenza, quando comparada à Covid-19. Conclusão: A análise de evolução de casos para cura ou óbito por SRAG na população pediátrica mostrou que esse grupo de infecções se manifestou com particularidades nessa população, levando em conta os fatores de risco e proteção, quando comparadas com a população adulta, principalmente quando aprofunda-se os estudos por trás dos motivadores de cada fator de risco e de proteção.

Introdução/Objetivo: A sífilis ainda se constitui um grave problema de Saúde Pública, com frequência e gravidade maiores entre as pessoas que vivem com HIV (PVHIV). Um aumento acentuado na incidência de sífilis ocorreu em vários países nos últimos anos, incluindo o Brasil. Nosso objetivo foi investigar os fatores associados à incidência de sífilis em PVHIV acompanhados no Centro de Referência Estadual DST/HIV/AIDS em Salvador. Métodos: Trata-se de um estudo de coorte, envolvendo PVHIV matriculadas no CEDAP em 2017 e que realizaram investigação para sífilis na ocasião da matrícula. O diagnóstico da sífilis foi realizado com o teste rápido treponêmico (teste qualitativo) e o VDRL (teste quantitativo) no soro. Identificamos incidência de sífilis como viragem de teste treponêmico positivo ou um aumento ≥2 vezes nos títulos consecutivos de VDRL, conforme fluxograma laboratorial definido pelo Ministério da Saúde, realizado após o exame basal até 31/12/2022. Calculamos o risco relativo e a densidade de incidência de sífilis ao longo de 5 anos. O cálculo amostral considerou o poder estatístico de 80% e erro de 5%, com amostragem aleatória simples. Estudo aprovado pelo Comitê de Ética em Pesquisa da Sesab e obteve apoio financeiro do CNPQ. Resultados: A amostra foi composta por 381 PVHIV, com média de idade de 36,7 (±10,9) anos. Do total, 64,6% eram do sexo masculino, residentes em Salvador (77,7%), autodeclarados negros ou pardos (87,4%), solteiros (71,7%), com até 8 anos de estudo (49,8%), heterossexuais (55,7%). Na ocasião da matrícula, 21,8% tiveram diagnóstico de sífilis, 23,1% com passado de sífilis. Ocorreram 37 casos novos de sífilis com densidade de incidência 30,3 casos por pessoa-ano. Cerca de 29,7% dos casos eram sintomáticos (manchas e lesões de pele mais descritas) e 37,8% foram classificados como sífilis latente. Os pacientes com casos novos de sífilis eram ligeiramente mais jovens (34,9 versus 36,8 anos de idade média; p > 0,05), mais propensos a serem homens (p = 0,02; RR 3,5 IC95% 1,4 – 8,8), solteiros (p = 0,58), homem que faz sexo com homem (p < 0,01; RR 3,2 IC95% 1,6 – 6,3;), negros e pardos (p > 0,05). Conclusão: Os casos novos de sífilis foram frequentes entre PVHIV, com taxas mais elevadas entre homens e HSH com sífilis adquirida no período. Estratégias de diagnóstico para infecções sexualmente transmissíveis devem priorizar esse grupo de pacientes (PVHIV).

Introdução/Objetivo: A pandemia de COVID-19 causou prejuízos na qualidade assistencial a Pessoas Vivendo com HIV/AIDS (PVHA) no Brasil e no mundo, sendo que a dimensão deste impacto ainda não foi plenamente mensurada. Este estudo avaliou o impacto da pandemia no tratamento de PVHA no Centro Estadual de Referência da Bahia, considerando os aspectos clínicos, de adesão à terapia antirretroviral (TARV) e de falhas terapêuticas, com base na assistência pré-pandemia. Métodos: Trata-se de um estudo longitudinal e retrospectivo oriundo do Centro Estadual Especializado em Diagnóstico, Assistência e Pesquisa, de 03/2018 a 02/2022. Foram analisadas a adesão à TARV, a frequência de consultas médicas e as falhas terapêuticas. Os dados foram coletados de prontuários e sistemas de informações nacionais. Foram incluídas PVHA em seguimento no centro com diagnóstico de HIV, maiores de 18 anos e com retirada de TARV 3 meses antes do estudo. A seleção foi por amostragem aleatória simples, em uma população de 357 PVHA do Estudo de Coorte Ambispectiva em pacientes HIV acompanhados em um Centro de Referência na Bahia, aprovado pelo Comitê de Ética em Pesquisa. O cálculo amostral considerou um erro padrão de 5%. Resultados: A amostra foi composta por 162 participantes que atenderam aos critérios de inclusão, com cerca de 51,2% de indivíduos do sexo masculino (média de 49,73 anos; ± 10,62 anos). Houve uma redução significativa na taxa de consultas médicas, de exames de carga viral e linfócitos T CD4, assim como na adesão à TARV, quando comparado ao ano pré-pandemia com o primeiro ano de pandemia (média de 0,74; ±2,67; p < 0,05). A taxa de falhas virológicas manteve-se estável, mas houve um menor número de exames realizados durante a pandemia. A proporção de PVHA sem nenhuma dispensa de ARV no ano aumentou 4 vezes entre o período pré-pandemia e o período de vigência da pandemia (p < 0,05), apesar do centro ter um funcionamento pleno da farmácia, distribuição otimizada de ARV e atendimento clínico com triagem nos primeiros 2 meses de pandemia, seguido de atendimento universal. Conclusão: Este estudo evidenciou impactos importantes na adesão à TARV, na realização de exames e de consultas médicas ambulatoriais na pandemia, apesar das medidas adotadas. Tais resultados ratificam a necessidade de serviços especializados em cuidado a PVHA desenvolverem novas estratégias com o objetivo de mitigar falhas no tratamento em situações-limite, como pandemias ou emergências de doenças infecciosas.

Introdução: A infecção por S. aureus está associada a alta morbi-mortalidade. Métodos: Estudo de coorte retrospectivo descrevendo infecção grave por S.aureus em pacientes internados de 2016 a 2021. Realizada busca nos registros de microbiologia e revisão de prontuários para coleta de dados. Análise estatística com R 4.0.1 Resultados: Foram incluídos 67 pacientes, que apresentavam os seguintes sítios de infecção: 29 (43.3%) bacteremia e 38 infecções em outros sítios (lesão cutânea, pulmonar e outros). Eram homens 37 (55,2%); 69.4% negros, com idade mediana de 46 anos (IIQ = 31). As comorbidades mais frequentes em pessoas com infecção por S.aureus foram: Diabetes Mellitus (DM 17.9%), Hipertensão (13.4%), Doença Renal Crônica (DRC 11.9%), Câncer Recente (isto é, nos últimos 6 meses (CA 7.5%), Dermatopatia crônica (9%), e HTLV (9%), e pessoas vivendo com HIV/AIDS (PVHIV) 31(46,3%). Os PVHIV eram mais jovens que os não portadores do vírus (36 vs 60 anos, p < 0,001). A mediana do CD4 foi de 95 células/mm3, e 25/31 (75.8%) das PVHIV estavam em uso de terapia antirretroviral (TARV). Identificou- se que nos PVHIV, 7 (22%) já apresentavam-se colonizados por MRSA na admissão e que 20 (64%) PVHIV tinha infecção comunitária por S.aureus. Nos PVHIV afigurou- se uma prevalência de 38.7% de infecções por MRSA e destes, 100% era sensível a sulfametoxazol-trimetoprima (SXT), à doxiciclina e à linezolida; e 90% a clindamicina, mostrando-se fenotipicamente com padrão de MRSA comunitário. Internação em CTI ocorreu em 32.3% das PVHIV vs 50% (p = 0.22) dos soronegativos, e cerca de 1/3 das PVHIV e das soronegativas necessitou de suporte ventilatório de aminas e de HD. Registrou-se 25.8% de óbitos em 30 dias para os PVHIV vs 20% naqueles soronegativos para HIV (p = 0.789). Conclusão: Nosso estudo mostrou alta taxa de colonização por MRSA (38.7%) em PVHIV com infecção grave por S.aureus, maior que a descrita na população geral sem infecção por S.aureus (0.3% a 1.3%) e nos profissionais de saúde (1.3 a 2.3%). As comorbidades na população geral do estudo se assemelha àquelas descritas em outros estudos, como HIV, DM, Ca recente e condições que predispõem quebra de integridade cutânea – DRC. Introdução: O sarampo é uma doença viral de caráter infeccioso e de alta transmissibilidade pelo contato com secreções nasofaríngeas do infectado. Acomete o trato respiratório, mas pode tornar-se sistêmico e causar panencefalite esclerosante subaguda. Não existe atualmente tratamento antiviral, a notificação é compulsória, sendo a vacinação a principal estratégia de combate ao vírus. A erradicação da doença foi revogada em 2019 no país. Busca-se então analisar as internações e a vacinação para acompanhar o sucesso das medidas de saúde pública. Métodos: Estudo transversal, descritivo e com abordagem quantitativa, realizado com coleta de dados no Sistema de Morbidade Hospitalar do SUS e do Sistema de Informação do Programa Nacional de Imunizações vinculados ao DATASUS, segundo as variáveis de sexo, idade, ano e região relacionadas ao número de internação por Sarampo entre janeiro de 2018 e dezembro de 2022 no Brasil. Foram coletados também dados segundo as variáveis ano, região e cobertura vacinal dos imunobiológicos para o sarampo. Aplicada estatística descritiva com a utilização do software Microsoft Excel. Resultados: No período de 2018 a 2022, 2.520 pacientes foram internados devido ao sarampo. Observou-se uma queda de mais de 90% das internações e de 17,1% na cobertura vacinal entre 2018 e 2022. Houve diminuição progressiva do número de internações, reduzindo 6,5% de 2018-2019, 33,1% entre 2019-2020, 68,4% de 2020-2021 e 64,2% de 2021-2022. Entretanto, houve picos pontuais na região Norte, representando 32% do país no período analisado. O segundo pico ocorreu na região Sudeste, em 2019, com aumento do número de internações em 22 vezes em relação ao ano anterior. O sexo masculino representou 52% das internações e a faixa etária de até 5 anos 66%. A cobertura vacinal média no período foi de 58,2%, com aumento de 2,2% entre 2018-2019, redução de 13,9% entre 2019-2020 e de 9,4% de 2020-2021 e um novo aumento de 3,9% de 2021-2022. A região Sul apresentou maior cobertura vacinal média, correspondendo a 71,91%, enquanto a região Nordeste apresentou a menor de 51,65%. Conclusão: A taxa de vacinação apresentou diferenças entre regiões e teve uma diminuição entre 2019 e 2021, com aumento no ano de 2022. Podendo representar os esforços das campanhas de vacinação. No entanto, paralelamente observou-se uma diminuição progressiva das internações, sendo a faixa etária mais internada de indivíduos com até 5 anos, com ligeira prevalência do sexo masculino.

Introdução: A Febre Amarela (FA) é uma doença infecciosa febril aguda causada por um arbovírus. Em geral, o quadro clínico é brando, porém, em evoluções desfavoráveis, a FA pode cursar com manifestações hemorrágicas, insuficiência renal e hepática. Nesses casos, o manejo ocorre sob internação hospitalar, muitas vezes em Unidade de Terapia Intensiva. A vacinação é o principal meio de prevenção e controle da doença e apresenta eficácia acima de 95%. O objetivo do presente estudo foi analisar o impacto da imunização contra FA no número de internações pela doença no SUS entre 2016 e 2022. Métodos: Estudo transversal, descritivo e com abordagem quantitativa, realizado com coleta de dados no Sistema de Morbidade Hospitalar do SUS e do Sistema de Informação do Programa Nacional de Imunizações vinculados ao DATASUS, segundo as variáveis de sexo, idade, ano e região relacionadas ao número de internação por FA entre janeiro de 2016 e dezembro de 2022 no Brasil. Foram coletados também dados segundo as variáveis ano, região e cobertura vacinal para a FA e aplicada estatística descritiva por meio do software Microsoft Excel. Resultados: No período de 2016 a 2022, 2.002 pacientes foram internados e 312 faleceram em decorrência da FA. No país, o número de internações aumentou 26 vezes entre 2016 e 2017 e 33,5% entre 2017 e 2018. Em seguida, reduziu 91,3% de 2018 para 2019 e 65,5% de 2019 para 2022. O pico de internações nos anos de 2017 e 2018 se deve a casos na região Sudeste, que acumulou 1.691 internamentos, correspondendo a 88,4% de todas as internações no país no período. O sexo masculino representou 77,9% das internações e a faixa etária de 20 a 59 anos, 70,5%. A cobertura vacinal média no período deste estudo foi de 55,7%. A variação dessa cobertura contou com aumento de 2,7% entre 2016 e 2017, 12,1% entre 2017 e 2018 e 2,9% em 2018 e 2019, mas reduziu 1,7% entre 2019 e 2022. A região Sudeste apresentou a maior variação da cobertura vacinal: de 39,4% em 2017 para 66,3% em 2018. Conclusão: O aumento das internações entre 2016 e 2018 tem relação com o surto de FA ocorrido na região Sudeste nesse período, provavelmente pela associação entre baixa cobertura vacinal e fatores ambientais. Percebe-se uma predominância do sexo masculino nos casos. Houve aumento da cobertura vacinal, principalmente entre 2017 e 2018, com destaque para a região Sudeste, o que possivelmente contribuiu para significativa redução do número de internações pela doença em 2018 e 2019.

Introdução: De acordo com a Organização Mundial de Saúde, a gonorreia é a segunda infecção sexualmente transmissível (IST) bacteriana mais frequente no mundo, gerando um forte impacto econômico e social na população. No Brasil, os dados epidemiológicos sobre a gonorreia são escassos. Nesse contexto, é de suma importância conhecer as características clínicas e epidemiológicas dos indivíduos acometidos pela doença. Objetivo: Descrever o perfil clínico e epidemiológico de pacientes atendidos entre outubro/2015 e dezembro/2016, em um centro de referência para IST na Bahia com diagnóstico de uretrite gonocócica resistente a ciprofloxacino. Método: Trata-se de um estudo de corte transversal envolvendo pacientes com diagnóstico de uretrite gonocócica, confirmada por métodos laboratoriais e com perfil de sensibilidade a antibióticos, que participaram de um programa sentinela multicêntrico, ocorrido entre outubro/2015 e dezembro/2016. A pesquisa foi aprovada pelo comitê de ética em pesquisa em dezembro de 2018. Os dados foram obtidos através da revisão de prontuários. Foi realizada uma análise descritiva e estatística inferencial visando verificar associações entre a presença de resistência a ciprofloxacino com outras variáveis. Resultados: Noventa e nove participantes foram incluídos no estudo. Foi encontrada resistência a ciprofloxacino em 53 pacientes. A média de idade dos pacientes com gonococos resistentes a ciprofloxacino foi de 25 anos. Desses pacientes, 64,2% eram pardos, 50,9% heterossexuais, 98,2% solteiros e procedentes da capital. Corrimento e disúria foram os sintomas mais frequentes e o esquema de tratamento empregado em maior escala foi a associação ciprofloxacino e azitromicina. A taxa de infecção pelo HIV entre os resistentes foi de 5,7% e por sífilis de 13,2%. Não houve casos de infecção pelo vírus HTLV e Hepatites B ou C. A história de gonorreia prévia foi um fator associado à ocorrência de resistência, sendo estatisticamente significante (p < 0,05). Conclusão: Entre os pacientes com gonorreia resistente a ciprofloxacino predominaram os adultos jovens, heterossexuais, pardos e solteiros. O número de coinfectados por gonorreia e sífilis foi alto. Não houve disparidades relevantes entre fatores clínicos e epidemiológicos apresentados pelos pacientes dos grupos resistente e sensível. Foi encontrada associação entre passado de gonorreia e infecção por cepas resistentes, sugerindo que o uso prévio de antibiótico teve correlação com resistência.

Introdução/Objetivo: Segundo o Ministério da Saúde, em 2021, foram registrados no Brasil mais de 167 mil novos casos de sífilis adquirida, e até junho de 2022, somaram-se mais 79.587 casos. Em 2018 foi relatado prevalência de sífilis de 26,9% em homens que fazem sexo com homens, valores 355 vezes maiores que os da população geral brasileira, com variação de 30 a 75% em mulheres transgênero, dependendo da região do país. É conhecida a dificuldade de acesso da população transgênero a serviços de saúde, que leva a escassez de dados deste recorte populacional e influi negativamente no planejamento de assistência e promoção da qualidade de vida. Este estudo objetiva contribuir para o conhecimento da saúde da população transgênero, provendo dados para a melhoria da assistência. Métodos: Trata-se de um estudo original, descritivo e de corte transversal, com amostragem por conveniência, não probabilística. Os dados foram coletados a partir de questionários elaborados para um estudo de coorte em atenção à saúde das pessoas transgênero atendidas no CEDAP. A população inclui todos os homens e mulheres transexuais, travestis, gênero Queer e não binário cadastradas no CEDAP, maiores de 18 anos, em acompanhamento ambulatorial, que tenham assinado o termo de consentimento livre e esclarecido. Resultados: Foram colhidos 108 questionários. A maioria dos participantes foram mulheres trans, heterossexuais, negras, que se relacionavam com homens cisgêneros. Dos pacientes testados para sífilis, 53,8% apresentaram teste rápido (treponêmico) reagente e 62,7% VDRL reagente, sendo esses, 100% mulheres trans. Cerca de 30% dos pacientes apresentaram ambos os testes treponêmico e VDRL positivos (infecção recente) e 27,8% tinham coinfecção entre HIV e sífilis. Os pacientes envolvidos foram majoritariamente heterossexuais e relataram relações com homens cis. Essas informações associadas ao baixo uso de preservativos em todas as relações sexuais são fatores de risco para a incidência de infecções sexualmente transmissíveis em mulheres transgêneros. Tais dados se refletem na alta taxa de prevalência de sífilis encontrada na população estudada, muito superior à da população geral, somando-se ao fato de que todos os testes reagentes ocorreram em mulheres trans. Conclusão: Os dados obtidos reforçam a necessidade de campanhas educacionais para prevenção da sífilis, além do tratamento e acompanhamento desse recorte populacional, que se mostra mais sujeito aos fatores de risco dessa patologia.

Introdução: Sepse é definida como resposta imune desregulada, resultante de uma infecção presumida ou confirmada, levando a disfunção orgânica. A terapêutica desse quadro exige equipe e ambiente qualificados para lidar com possíveis complicações, cuja evolução tende a ser letal se não conduzida adequadamente. Assim, apesar do avanço científico hodierno, reconhecer o quadro de forma eficiente e tratá-lo em tempo hábil persiste enquanto desafio para a medicina. O objetivo deste estudo foi realizar uma análise descritiva do número de internações e óbitos por sepse no Brasil entre 2018 a 2022, buscando compreender as taxas de infecção generalizada resultantes em internações e óbitos no Brasil. Metódos: Estudo transversal, descritivo e com abordagem quantitativa, realizado com coleta de dados no Sistema de Morbidade Hospitalar do SUS, segundo variáveis de sexo, idade, ano e região relacionadas ao número de internação e óbitos por sepse entre janeiro de 2018 e dezembro de 2022 no país. Aplicada estatística descritiva por meio do software Microsoft Excel. Resultados: Entre 2018 a 2022, 660.247 pacientes foram internados e 299.859 foram a óbito devido a sepse no SUS. Ao longo dos anos, o número de casos aumentou 11% entre 2018-2019, reduziu 14% entre 2019‒2020, reduziu 2,6% entre 2020‒2021 e aumentou 30,8% de 2020‒2022. A região sudeste representou 51% do total de internações durante o período do presente estudo e 55,2% dos óbitos. Em segundo lugar, encontra-se a região nordeste, concentrando 19,19% do número de internações, seguida pela região sul com 19,14% e pela região norte com 5,63%. Já, a região centro-oeste registrou os menores números, contando com 4,9% das internações e 4% dos óbitos. A faixa etária mais acometida foram os idosos a partir dos 60 anos, a qual correspondeu a 59% das internações e 74 % dos óbitos, sendo a maior taxa observada. A proporção de internações, bem como de óbitos, se aproximou da proporção 1:1 para ambos os sexos. Conclusão: A região do Brasil com o maior número de internações por sepse foi o Sudeste. Já a região Centro-Oeste apresentou o menor número de internações no período analisado. Não houve diferenças de internações ou óbitos entre o sexo masculino e feminino e a faixa etária com maior vulnerabilidade foi a de idosos a partir de 60 anos. É importante ressaltar que os resultados apresentados neste estudo requerem maior aprofundamento epidemiológico, na medida em que a subnotificação das internações foi uma limitação.

Summary: The emergence and rapid spread of the monkeypox virus (MPXV) to non-endemic countries has brought this once obscure pathogen to the forefront of global public health. Given the range of conditions that cause similar skin lesions, and because the clinical manifestation may often be atypical in the current mpox outbreak, it can be challenging to diagnose patients based on clinical signs and symptoms. With this perspective in mind, laboratory-based diagnosis assumes a critical role for the clinical management, along with the implementation of countermeasures. Here, we review the clinical features reported in mpox patients, the available laboratory tests for mpox diagnosis, and discuss the principles, advances, advantages, and drawbacks of each assay. We also highlight the diagnostic platforms with the potential to guide ongoing clinical response, particularly those that increase diagnostic capacity in low- and middle-income countries. With the outlook of this evolving research area, we hope to provide a resource to the community and inspire more research and the development of diagnostic alternatives with applications to this and future public health crises.

Ensuring the removal of host cell proteins (HCPs) during downstream processing of recombinant proteins such as monoclonal antibodies (mAbs) remains a challenge. Since residual HCPs might affect product stability or safety, constant monitoring is required to demonstrate their removal to be below the regulatory accepted level of 100 ng/mg. The current standard analytical approach for this procedure is based on ELISA; however, this approach only measures the overall HCP content. Therefore, the use of orthogonal methods, such as liquid chromatography-mass spectrometry (LC-MS), has been established, as it facilitates the quantitation of total HCPs as well as the identification and quantitation of the individual HCPs present. In the present study, a workflow for HCP detection and quantitation using an automated magnetic bead-based sample preparation, in combination with a data-independent acquisition (DIA) LC-MS analysis, was established. Employing the same instrumental setup commonly used for peptide mapping analysis of mAbs allows for its quick and easy implementation into pre-existing workflows, avoiding the need for dedicated instrumentation or personnel. Thereby, quantitation of HCPs over a broad dynamic range was enabled to allow monitoring of problematic HCPs or to track changes upon altered bioprocessing conditions.

Summary: Neuropathic pain is a debilitating condition resulting from damage to the nervous system. Imbalance of spinal excitation and inhibition has been proposed to contribute to neuropathic pain. However, the structural basis of this imbalance remains unknown. Using a preclinical model of neuropathic pain, we show that microglia selectively engulf spinal synapses that are formed by central neurons and spare those of peripheral sensory neurons. Furthermore, we reveal that removal of inhibitory and excitatory synapses exhibits distinct temporal patterns, in which microglia-mediated inhibitory synapse removal precedes excitatory synapse removal. We also find selective and gradual increase in complement depositions on dorsal horn synapses that corresponds to the temporal pattern of microglial synapse pruning activity and type-specific synapse loss. Together, these results define a specific role for microglia in the progression of neuropathic pain pathogenesis and implicate these immune cells in structural remodeling of dorsal horn circuitry.

Despite the high prevalence of chronic pain as a disease in our society, there is a lack of effective treatment options for patients living with this condition. Gene therapies using recombinant AAVs are a direct method to selectively express genes of interest in target cells with the potential of, in the case of nociceptors, reducing neuronal firing in pain conditions. We designed a recombinant AAV vector expressing cargos whose expression was driven by a portion of the SCN10A (NaV1.8) promoter, which is predominantly active in nociceptors. We validated its specificity for nociceptors in mouse and human dorsal root ganglia and showed that it can drive the expression of functional proteins. Our viral vector and promoter package drove the expression of both excitatory or inhibitory DREADDs in primary human DRG cultures and in whole cell electrophysiology experiments, increased or decreased neuronal firing, respectively. Taken together, we present a novel viral tool that drives expression of cargo specifically in human nociceptors. This will allow for future specific studies of human nociceptor properties as well as pave the way for potential future gene therapies for chronic pain.

Arthritis is the leading cause of musculoskeletal pain and disability worldwide. Nearly 50% of individuals over the age of 65 have arthritis, which contributes to limited function, articular pain, physical inactivity, and diminished quality of life. Therapeutic exercise is often recommended in clinical settings for patients experiencing arthritic pain, however, there is little practical guidance regarding the use of therapeutic exercise to alleviate arthritic musculoskeletal pain.Rodent models of arthritis allow researchers to control experimental variables, which cannot be done with human participants, providing an opportunity to test therapeutic approaches in preclinical models. This literature review provides a summary of published findings in therapeutic exercise interventions in rat models of arthritis as well as gaps in the existing literature. We reveal that preclinical research in this field has yet to adequately investigate the impact of experimental variables in therapeutic exercise including their modality, intensity, duration, and frequency on joint pathophysiology and pain outcomes.

Garlic (Allium sativum L.) is a spice widely used both as food and for its medicinal effects, however, much of the research on A. sativum is limited to the study of the bulb, even though the garlic peel corresponds to 25% of total production and is generally discarded by industry. In order to obtain a use of this co-product, the objective of this work was to elaborate plant extracts from the peel of garlic (Allium sativum L.), using maceration and turbolization as extraction methods and water or hydroalcoholic solutions (50% and 70%) as solvents, evaluate the obtained extracts taking into account the content of bioactive compounds, the antimicrobial potential, antioxidant capacity and its toxicity against adult zebrafish (Danio rerio). The hydroalcoholic extracts of garlic peel had a higher content of bioactive compounds, showing a direct correlation to the result of the antioxidant potential of these extracts, which also showed bactericidal effect against S. aureus (ATCC 27,664) and L. monocytogenes (ATCC 19,115). These extracts did not show any toxic effect on A. salina and adult zebrafish, however, the assay with D. rerio showed a sedative effect when using extracts obtained with 70% ethanol (EMH70% and ETH70%). The extracts obtained with 50% ethanol had the best results combined with the absence of toxicity and sedative effect, demonstrating an excellent potential for a future application in the food matrix, due to the presence of bioactive substances and their antioxidant and antimicrobial effects.

Introdução: O Torque teno virus (TTV) é um vírus de ssDNA não-envelopado, parece ser comensal em humanos e animais, com distribuição ubíqua na população e presente em diferentes tecidos e fluidos biológicos, especialmente na saliva. Devido a sua alta capacidade replicacional em imunocomprometidos, a avaliação de sua carga viral tem sido estudada como um potencial biomarcador para a competência imunológica. Neste estudo, avaliamos o título de TTV salivar em indivíduos vivendo com HIV/AIDS sob uso de TARV (terapia antirretroviral combinada) em relação com os principais parâmetros para o desenvolvimento do Sarcoma de Kaposi associado a AIDS (SK-AIDS): contagem de linfócitos T CD4+, carga viral do HIV e dados demográficos. Objetivo: Otimizar um teste qPCR in-house para medição de TTV salivar; verificar se há relação entre título de TTV salivar e outras características laboratoriais e demográficas destes indivíduos. Método: Trata-se de um estudo transversal com amostragem de conveniência de salivas coletadas de 276 indivíduos assintomáticos e também de 48 indivíduos com SK-AIDS, ambos sob uso de TARV. Os níveis circulantes de linfócitos T CD4+ e carga viral do HIV foram avaliados de modo pareado com a data da coleta da saliva, assim como os dados demográficos e laboratoriais. A otimização do teste foi baseada na construção de uma curva de referência quantitativa a partir de concentrações conhecidas de oligonucleotídeos sintéticos específicos desenhados para o atual estudo. Resultados: A eficiência do teste qPCR foi de 104% (slope: 3,22 e R2: 0,999). O TTV foi detectável na saliva de 80% dos indivíduos com HIV e 87% daqueles com SK-AIDS, com título de TTV salivar de 0,38 até 8,11 log10 cópias/mL. Entre os gêneros, a mediana do título de TTV foi de 3,3 (n = 200 homens) vs. 2,4 (n = 76 mulheres) (p < 0,0001). O título de TTV salivar foi inversamente correlacionado com o nível de células T CD4 + (p < 0,0001) e positivamente correlacionado com a concentração de HIV circulante (p < 0,0005), especialmente no grupo SK-AIDS (p < 0,0001). Conclusão: A medida dos títulos de TTV em amostras de DNA salivar por qPCR parece ser mais sensível do que outros parâmetros laboratoriais, sendo útil como um biomarcador complementar para avaliar o status imune de indivíduos que vivem com HIV, e potencialmente para outras doenças infecciosas e não-infecciosas. The cholesteryl ester transfer protein (CETP) is a lipid transfer protein responsible for the exchange of cholesteryl esters and triglycerides between lipoproteins. Decreased CETP activity is associated with longevity, cardiovascular health, and maintenance of good cognitive performance. Interestingly, mice lack the CETP-encoding gene and have very low levels of LDL particles compared with humans. Currently, the molecular mechanisms induced because of CETP activity are not clear. To understand how CETP activity affects the brain, we utilized CETP transgenic (CETPtg) mice that show elevated LDL levels upon induction of CETP expression through a high-cholesterol diet. CETPtg mice on a high-cholesterol diet showed up to 22% higher cholesterol levels in the brain. Using a microarray on mostly astrocyte-derived mRNA, we found that this cholesterol increase is likely not because of elevated de novo synthesis of cholesterol. However, cholesterol efflux is decreased in CETPtg mice along with an upregulation of the complement factor C1Q, which plays a role in neuronal cholesterol clearance. Our data suggest that CETP activity affects brain health through modulating cholesterol distribution and clearance. Therefore, we propose that CETPtg mice constitute a valuable research tool to investigate the impact of cholesterol metabolism on brain function.

Introdução: A vacinação é uma ferramenta essencial para o controle da infecção por SARS-CoV-2 e da pandemia de COVID-19. O surgimento de novas variantes genéticas do vírus SARS-CoV-2 nos trouxe a questão se há diferencial capacidade neutralizante dos anticorpos quanto às variantes de preocupação (VOCs). Objetivo: Nosso estudo se dirigiu a avaliar a capacidade neutralizante dos anticorpos de indivíduos imunizados com a vacina CoronaVac e dose de reforço com Pfizer contra as variantes Gama, Delta e Omicron. Método: Amostras de soro foram obtidas de 41 profissionais da saúde da Faculdade de Medicina da USP, sem infecção prévia por SARS-CoV-2 no esquema vacinal CoronaVac (2 doses) seguido de dose booster com vacina Pfizer. Os níveis de anticorpos neutralizantes para as variantes Gama, Delta e Omicron foram avaliados 32 e 186 dias após a segunda dose da vacina. Também avaliamos a atividade neutralizante dos anticorpos contra a variante Omicron em 39 dos indivíduos após 62 dias de imunização de reforço, com a vacina Pfizer. Os títulos de anticorpos foram obtidos pelo Teste de Neutralização Viral (VNT) e observação de efeito citopático. Resultados: A neutralização por anticorpos contra as variantes Gama, Delta e Omicron foi de 78%, 65.9% e 58.5% respectivamente, após uma média de 32 dias após a segunda dose por CoronaVac. Houve uma diminuição na frequência de anticorpos neutralizantes para 17.1%, 24.4% e 2.4% contra as variantes Gama, Delta e Omicron, respectivamente, após, em média 186 dias das duas doses da vacina CoronaVac. A dose booster com a vacina Pfizer foi capaz de induzir a produção de anticorpos neutralizantes contra a variante Omicron em 87.2% dos indivíduos avaliados. Conclusão: Os indivíduos vacinados com CoronaVac apresentaram uma queda nítida de anticorpos neutralizantes contra as 3 variantes de SARS-CoV-2 analisadas após 186 dias da imunização por 2 doses. A dose de reforço com Pfizer induziu a produção de anticorpos neutralizantes contra a variante Omicron na maior parte dos indivíduos avaliados (87.2%), 60 dias após imunização. Não houve diferença significativa na frequência de anticorpos neutralizantes entre as variantes analisadas.

Summary: Background: Early death after cancer surgery is higher in low-income and middle-income countries (LMICs) compared with in high-income countries, yet the impact of facility characteristics on early postoperative outcomes is unknown. The aim of this study was to examine the association between hospital infrastructure, resource availability, and processes on early outcomes after cancer surgery worldwide. Methods: A multimethods analysis was performed as part of the GlobalSurg 3 study—a multicentre, international, prospective cohort study of patients who had surgery for breast, colorectal, or gastric cancer. The primary outcomes were 30-day mortality and 30-day major complication rates. Potentially beneficial hospital facilities were identified by variable selection to select those associated with 30-day mortality. Adjusted outcomes were determined using generalised estimating equations to account for patient characteristics and country-income group, with population stratification by hospital. Findings: Between April 1, 2018, and April 23, 2019, facility-level data were collected for 9685 patients across 238 hospitals in 66 countries (91 hospitals in 20 high-income countries; 57 hospitals in 19 upper-middle-income countries; and 90 hospitals in 27 low-income to lower-middle-income countries). The availability of five hospital facilities was inversely associated with mortality: ultrasound, CT scanner, critical care unit, opioid analgesia, and oncologist. Introduction and objectives: The use of neuromuscular blockers during surgery represented a landmark for anesthesiology. However, their use can prompt residual Neuromuscular Blockade (RNMB) and objective monitoring of neuromuscular function is crucial to warrant the recovery of muscle strength. The present study aimed to estimate the incidence of RNMB and late Neuromuscular Blockade (LNMB) at the Post-Anesthetic Recovery Unit (PACU). Method: The study included 85 patients, 43 of which received cisatracurium and 42 of which, rocuronium. The depth of the Neuromuscular Blockade (NMB) was assessed by Train Of Four (TOF). NMB reversal was performed with the administration of neostigmine and atropine.RNMB was defined when a patient presented TOF below 90% at the PACU. Results: RNMB at the PACU was diagnosed in 39.5% and 40.5% of the patients receiving cisatracurium and rocuronium, respectively (p = 1.0). LNMB at the PACU was found in 32.6% and 16.7% of the patients receiving cisatracurium and rocuronium, respectively (p = 0.131). Conclusions: The incidence ofRNMB remains significant despite the use of intermediate-acting neuromuscular blockers and reversal agents. There was no statistically significant difference in the incidence of RNMB or LNMB in patients receiving cisatracurium or rocuronium. The use of objective NMB monitoring is effective for the diagnosis of RNMB, as well as for treatment management.

Objective: Analyzing the impact of COVID-19 pandemic on meningococcal C conjugate vaccination in Brazil. Methods: Ecological study, based on interrupted time series, carried out with data collected through the Information System of the National Immunization Program (IS-NIP). Collected data refer to the number of meningococcal C conjugate vaccine doses administered from March 2019 to December 2020. Results: In total, 14,832,054 meningococcal C conjugate vaccine doses were administered throughout the investigated period; 66.30% of them, from March 2019 to February 2020 and 33.70%, from March to December 2020. Statistically significant steps were observed, i.e., the COVID-19 pandemic had negative impact on the number of MenC vaccine doses administered in the North and South regions (26,98% and 41.47%, respectively) and in the eleven Brazilian States. Conclusion: The current study has shown that the COVID-19 pandemic had negative impact on the number of MenC vaccine doses administered in the Northern and Southern Brazil, and in eleven Brazilian states. Among all challenges experienced during the COVID-19 pandemic, one finds reduced MenC vaccine coverage, which, consequently, may lead to increased meningococcal infection rates in Brazil.

Endophytic bacteria colonize different internal tissues of plants without damaging their cells. They can establish themselves in the same niche as other microorganisms and develop antagonistic activities against phytopathogens. There is little research on the functional and morphological characterization of these bacteria in production systems in the Amazon. Thus, the objective of this work was to functionally and morphologically characterize endophytic bacteria isolated from cocoa trees (Theobroma cacao L.) and evaluate their antagonistic potential against phytopathogens. A total of 197 endophytic bacteria isolates were obtained from leaves and roots of cocoa plants with different production systems and at different times of the year. The characterization of functional groups consisted of proteolytic, amylolytic and cellulolytic activity and ability to fix nitrogen and solubilize phosphate. Morphological diversity was evaluated mainly according to the following parameters: shape, color, size and elevation of the colony. Thirteen isolates of endophytic bacteria, selected by cluster analysis, were used to evaluate the antagonistic potential in paired trials against four species of phytopathogenic fungi. The largest amount of endophytic bacteria was isolated from the root (95.9%), in the dry season. The most expressive activities with regards to the enzyme index were amylolytic (71.9%), proteolytic (70.2%) and nitrogen fixing (38.6%), respectively. The similarity analysis formed two clusters with isolates CS R 2.4 and CS R 2.25 exhibiting 100% similarity. Five isolates displayed inhibitory activity against phytopathogenic fungi, most notably isolate TS R 2.19, which exhibited antagonistic activity against all fungi and mycelial growth inhibition rates between 25.7% and 50.7%. Understanding the interaction between endophytes in cocoa plants is important as a possible additional tool in biological control. Our studies are incipient and the first to be carried out in different cocoa production systems in the state of Pará, Brazil.

The worldwide spread of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has caused the most severe public health crisis since the 1918 Spanish influenza pandemic. After the introduction of public health interventions aimed at reducing the number of COVID-19 cases, many countries across the world obtained success at containing the fast spread of SARS-CoV-2 during the first wave of the pandemic. However, the SARS-CoV-2 has resurged in many countries causing a even more devastating second wave. Brazil is one the most affected countries and currently is facing one of the worst public health crises in its history. Here, we discuss the unprecedented challenges faced by the Brazilian public health system in the midst of the second wave of the COVID-19 pandemic, particularly regarding the collapse of the Brazilian health system and the emergence of new variants of concern (VOCs). Finally, we suggest some insights using a one health approach that will help the country to face and overcome the current COVID-19 crisis.

With the size of the biopharmaceutical market exponentially increasing, there is an aligned growth in the importance of data-rich analyses, not only to assess drug product safety but also to assist drug development driven by the deeper understanding of structure/function relationships. In monoclonal antibodies, many functions are regulated by N-glycans present in the constant region of the heavy chains and their mechanisms of action are not completely known. The importance of their function focuses analytical research efforts on the development of robust, accurate and fast methods to support drug development and quality control. Released N-glycan analysis is considered as the gold standard for glycosylation characterisation; however, it is not the only method for quantitative analysis of glycoform heterogeneity. In this study, ten different analytical workflows for N-glycan analysis were compared using four monoclonal antibodies. While observing good comparability between the quantitative results generated, it was possible to appreciate the advantages and disadvantages of each technique and to summarise all the observations to guide the choice of the most appropriate analytical workflow according to application and the desired depth of data generated. Case: A young adult male sustained a high-energy crash suffering multiple injuries including a comminuted right femoral shaft fracture and an ipsilateral iliac wing fracture. The iliac fracture was caused by a femoral fragment which was projected and pierced the iliac wing. The patient underwent surgery with retrieval of the femoral fragment and fixation of the iliac and femoral fractures. The lesions healed uneventfully. Conclusion: This is the first reported case of an iliac fracture caused by a projectile of autologous bone. High-energy trauma may present unusual or never seen injury patterns to the trauma surgeon.

The emergence of SARS-CoV-2 has prompted the mobilization of a network of public health laboratories to diagnose COVID-19 patients, trace contacts and identify hot-spot areas for active community transmission at the expense of arbovirus diagnosis and control practices. In this article, we discuss the unprecedented challenges faced by the Brazilian public health system in dealing with the incursion of SARS-CoV-2 in the midst of ongoing triple arboviral epidemics caused by dengue, chikungunya, and Zika virus. Finally, we highlight the importance of the introduction of one health approach as an effective inter-disciplinary response and management to mitigate the catastrophic effect caused by these pathogens.

Although several studies point to the emerging importance of technology in consumer habits, limited studies have quantitatively shown how these variables influence consumer intention. Given this gap, this study offers an analytical model based on technology readiness, collaborative consumption intention and green consumption values, using age groups as a moderating variable. Through covariance-based structural equation modeling (CB-SEM) and a sample of 374 respondents (238 younger and 136 older subjects), it was observed that technology readiness positively influences collaborative consumption intention, with green consumption values as a precursor. This relationship is stronger for older people, who generally have a lower perception of the control of technology than younger people. Public strategies and policies to encourage collaborative consumption must, therefore, take into account consumer values and age. Future studies could validate the results presented in this study, and include other demographic and behavioral dimensions.

Phthalates are the most common plasticizer compounds worldwide, providing flexibility to a wide array of plastic products. The inadequate disposal of these chemical residues causes their absorption and accumulation by plant species in contaminated environments. The absorption and accumulation of phthalates in the mangrove species Avicennia schaueriana (Acanthaceae) were monitored over one year. A total of 30 fresh and dried leaf extract samples were collected through the “serapilheira system”, in the Guaratiba Biological Reserve in the state of Rio de Janeiro, Brazil, in three different regions of the Guaratiba mangrove forest: A (fringe), C (basin), and E (transition). Samples were analyzed by gas chromatography coupled to mass spectrometry (GC-MS). Bis-tridecyl phthalate was the main contaminant detected, and bis-isobutyl phthalate and bis-2-ethylhexyl phthalate were the major metabolites of the butanolic fraction obtained from the liquid-liquid partition of the ethanolic leaf extract. The observed chemical profile was rich in secondary metabolites, predominantly lupeol and α-amyrin. Exposure to these contaminants presents several risks to human health. This is the first work to confirm how the mangrove pollution process, through the accumulation of plastic contaminants in sediments and water, can directly affect the plant species and their biosynthesis.Capsule: This study shows how methods used in classical phytochemistry can highlight serious environmental pollution problems that affect plant biosynthesis and human health.

The gastrointestinal tract contains thousands of microorganisms present in the mucosa, derived from several different species, and these microorganisms have a direct relationship with immunity, homeostasis, and certain systemic diseases. The microorganisms present in kefir grains interact with the gastrointestinal and immune systems as well as with the intestinal microbiota. Isoflavones are the most commonly consumed phytoestrogens, with aglycone and glycoside forms. Therefore, in this study, the content of aglycone isoflavones released from kefir associated with soybean germ (KSG) was evaluated in a gastrointestinal digestion model in vitro, and the anxiolytic and antidepressant activity of KSG and the fermentation solution of kefir (FSK) were evaluated in models of anxiety and depression in zebrafish (Danio rerio). KSG was prepared as the 4-day fermentation product of 40 g/L brown sugar, 10 g/L soy germ, and 60 g/L kefir. Isoflavone content was evaluated during the gastric, intestinal, and colon phases in simulated gastrointestinal digestion and analyzed by high-performance liquid chromatography. KSG was administered orally to zebrafish for 30 days, at 200 mg/kg (group G1) and 400 mg/kg (G2), while G3 received 2 μL SFK. The scototaxis test (light–dark) was performed to assess anxiolytic activity, using caffeine (100 mg/kg) as an anxiolytic, buspirone (25 mg/kg) as a positive control, and distilled water as a negative control. To assess antidepressant activity, a new tank dive test was performed, using 1% ethanol as a depressant, fluoxetine (20 mg/kg) as a positive control, and distilled water as a negative control. An increase in the release of isoflavones in the gastric phase and a decrease in the intestinal and colon phases was demonstrated with KSG. Moreover, the behavioral patterns observed in the different experiments demonstrated that KSG and FSK produce anxiolytic and antidepressant effects.

Background and objectives: Ultrasound-guided upper limb blocks may provide great benefits to patients with serious diseases. Patients with Steinert's disease have muscle weakness and risk of triggering myotony or malignant hyperthermia due to the use of anesthetic agents and surgical stress. The objective of this report was to demonstrate a viable alternative for clavicle fracture surgery with upper trunk and supraclavicular nerve block, thus reducing the spread of local anesthetic to the phrenic nerve in a patient with muscular dystrophy. Case report: A 53-year-old male patient with Steinert's disease, associated with dyspnea, hoarseness and dysphagia, referred to the surgical theater for osteosynthesis of clavicle fracture. Upper limb (1 mL 0.75% ropivacaine) and supraclavicular nerve block (1 mL 0.75% ropivacaine in each branch) were combined with venous anesthesia with propofol under laryngeal mask (infusion pump target of 4 mcg.mL−1). Upon awakening, the patient had no pain or respiratory complaints. He was transferred to the ICU for immediate postoperative follow-up with discharge from this unit after 24 h without complications. Conclusions: The superior trunk and cervical plexus block associated with venous anesthesia under laryngeal mask, without the use of opioids, proved to be adequate in the case of a patient with clavicle fracture and Steinert's disease. With the use of ultrasonography in regional anesthesia it is possible to perform increasingly selective blocks, thus allowing greater security for the anesthetic-surgical procedure and lower morbidity for the patient. Resumo: Justificativa e objetivos: Bloqueios seletivos dos membros superiores guiados por ultrassom podem trazer grandes benefícios em pacientes portadores de doenças graves. Pacientes portadores da doença de Steinert apresentam fraqueza muscular e riscos de desencadear miotonia ou hipertermia maligna devido ao uso de agentes anestésicos e ao estresse cirúrgico. O objetivo deste relato foi mostrar uma opção viável para a cirurgia de fratura de clavícula com bloqueio do tronco superior e nervo supraclavicular, diminui-se assim a dispersão do anestésico local para o nervo frênico em paciente com distrofia muscular. Relato de caso: Paciente do sexo masculino, 53 anos, portador de doença de Steinert, associada a dispneia, rouquidão e disfagia. Encaminhado ao bloco cirúrgico para osteossíntese de fratura de clavícula. Feito bloqueio de tronco superior (1 mL ropivacaína a 0,75%) e de nervo supraclavicular (1 mL de ropivacaína 0,75 em cada ramificação) associado à anestesia venosa com propofol sob máscara laríngea (alvo de 4 mcg.mL−1 em bomba de infusão). Ao despertar, o paciente apresentava-se sem dor ou queixas respiratórias. Admitido em CTI para acompanhamento do pós-operatório imediato com alta dessa unidade após 24 horas sem intercorrências. Conclusões: O bloqueio do tronco superior e do plexo cervical associado à anestesia venosa sob máscara laríngea, sem uso de opioides, mostrou-se adequado no caso de fratura da clavícula em paciente com doença de Steinert. Com o uso da ultrassonografia em anestesia regional é possível fazer bloqueios cada vez mais seletivos e possibilitar assim maior segurança para o procedimento anestésico-cirúrgico e menor morbidade para o paciente. It has previously been established during the elimination of Eriochrome Black T (EBT) that water hardness is an important condition that limits the efficacy of electrochemical treatments of azo dye effluent by the electrophilic attack of ·OH radicals, due to the possible formation of calcium and magnesium complexes. This outcome was observed when using the Electro-Fenton (EF) approach, where the use of iron as a catalyst is mandatory; however, the synergistic or antagonistic effects of iron were not considered. For this reason, this communication presents important experimental and theoretical insights (using Density Functional Theory (DFT) and Quantum Theory of Atoms in Molecules (QTAIM)) into the influence of different ions (Ca2+, Mg2+ or Fe2+) on the degradation efficiency of EBT solution using anodic oxidation (AO) with boron-doped diamond (BDD) electrodes. This article contains data on phenolic-profiling of seven wheat genotypes along maturation (softy, milky, physiological maturity and mature). This supplementary data is related to research “Metabolomic approach for characterization of phenolic compounds in different wheat genotypes during grain development” (Santos et al., 2018). Briefly, free and bound phenolic compounds were extracted with 80% ethanol (v/v) and hydrolysis processes, respectively. The aliquots resultants were separated by ultra-performance liquid chromatography (UPLC) and analyzed by quadrupole time-of-flight mass spectrometry (QTOF). Data were acquired using a multiplexed MS/MS acquisition with alternating low and high energy acquisition (MSE). The phenolic compounds with their respective abundances are showed here through characterization table and multivariate analysis (hierarchical cluster analysis—HCA—and principal component analysis—PCA).
