1. Failure assessment of deteriorated steel light poles

Abstract: Steel poles are extensively used as street light posts and various other applications within the power distribution network. For partially buried pole, the effect of corrosion below and at the ground level significantly controls the overall load capacity. In this study, number of in-service steel pole specimens extracted from field were assessed for the corrosion damage. The level of corrosion was estimated by remaining wall thickness measured using ultrasonic testing and the surface corrosion was estimated using 3D laser scanning to adopt an appropriate existing model for buried steel structures. A relationship between maximum and average corrosion pit depth and loss of embedment depth was then developed. Finally, the corrosion parameters, combined with uncertainty in loading and soil properties were employed to perform time-dependent reliability assessment for steel light poles which can be used to assist the asset maintenance team in planning the replacement or strengthening works for steel pole.

2. Weekly ultra-hypofractionated radiotherapy in localised prostate cancer

Abstract: Background: Moderately hypofractionated radiotherapy regimens or stereotactic body radiotherapy (SBRT) are standard of care for localised prostate cancer. However, some patients are unable or unwilling to travel daily to the radiotherapy department and do not have access to, or are not candidates for, SBRT. For many years, The Royal Marsden Hospital NHS Foundation Trust has offered a weekly ultra-hypofractionated radiotherapy regimen to the prostate (36 Gy in 6 weekly fractions) to patients unable/unwilling to travel daily. Methods: The current study is a retrospective analysis of all patients with non-metastatic localised prostate cancer receiving this treatment schedule from 2010 to 2015. Results: A total of 140 patients were included in the analysis, of whom 86 % presented with high risk disease, with 31 % having Gleason Grade Group 4 or 5 disease and 48 % T3 disease or higher. All patients received hormone treatment, and there was often a long interval between start of hormone treatment and start of radiotherapy (median of 11 months), with 34 % of all patients having progressed to non-metastatic castrate-resistant disease prior to start of radiotherapy. Median follow-up was 52 months. Median progression-free survival (PFS) and overall survival (OS) for the whole group was 70 months and 72 months, respectively. PFS and OS in patients with hormone-sensitive disease at time of radiotherapy was not reached and 75 months, respectively; and in patients with castrate-resistant disease at time of radiotherapy it was 20 months and 61 months, respectively. Conclusion: Our data shows that a weekly ultra-hypofractionated radiotherapy regimen for prostate cancer could be an option in those patients for whom daily treatment or SBRT is not an option.

Abstract: Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

Abstract: Background and purpose: MRI-guided radiotherapy (MRIgRT) offers multiple potential advantages over CT-guidance. This study examines the potential clinical benefits of MRIgRT for men with localised prostate cancer, in the setting of moderately hypofractionated radiotherapy. We evaluate two-year toxicity outcomes, early biochemical response and patient-reported outcomes (PRO), using data obtained from a multicentre international registry study, for the first group of patients with prostate cancer who underwent treatment on a 1.5 T MR-Linac. Materials and methods: Patients who were enrolled within the MOMENTUM study and received radical treatment with 60 Gy in 20 fractions were identified. PSA levels and CTCAE version 5.0 toxicity data were measured at follow-up visits. Those patients who consented to PRO data collection also completed EQ-5D-5L, EORTC QLQ-C30 and EORTC QLQ-PR25 questionnaires. Results: Between November 2018 and June 2022, 146 patients who had MRIgRT for localised prostate cancer on the 1.5 T MR-Linac were eligible for this study. Grade 2 and worse gastro-intestinal (GI) toxicity was reported in 3 % of patients at three months whilst grade 2 and worse genitourinary (GU) toxicity was 7 % at three months. There was a significant decrease in the median PSA at 12 months. The results from both the EQ-5D-5L data and EORTC global health status scale indicate a decline in the quality of life (QoL) during the first six months. The mean change in score for the EORTC scale showed a decrease of 11.4 points, which is considered clinically important. QoL improved back to baseline by 24 months. Worsening of hormonal symptoms in the first six months was reported with a return to baseline by 24 months and sexual activity in all men worsened in the first three months and returned to baseline at 12 months. Conclusion: This study establishes the feasibility of online-MRIgRT for localised prostate on a 1.5 T MR-Linac with low rates of toxicity, similar to that published in the literature. However, the clinical benefits of MRIgRT over conventional radiotherapy in the setting of moderate hypofractionation is not evident. Further research will focus on the delivery of ultrahypofractionated regimens, where the potential advantages of MRIgRT for prostate cancer may become more discernible.

Abstract: Objective: To determine if nirmatrelvir-ritonavir 300mg/100mg treatment for 5 days in high-risk outpatients with mild to moderate COVID-19 symptoms was associated with a reduction in hospitalization, intensive care unit (ICU) admission, and death. Methods: This 1:1 propensity score matched cohort study from 647 public health clinics in Malaysia included all patients with COVID-19 with positive tests aged 18 years and older, who were eligible for nirmatrelvir-ritonavir treatment within 5 days of illness from July 14, 2022, to November 14, 2022. The exposed group was patients with COVID-19 initiated with nirmatrelvir-ritonavir treatment, whereas those not initiated with the drug served as the control group. Data was analyzed from July 14, 2022 to December 31, 2022. Results: A total of 20,966 COVID-19 high-risk outpatients (n = 10,483 for nirmatrelvir-ritonavir group and n = 10,483 for control group) were included in the study. Nirmatrelvir-ritonavir treatment was associated with a 36% reduction (adjusted hazard ratio 0.64 [95% CI 0.43, 0.94]) in hospitalization compared with those not given the drug. There was a single ICU admission for the control group and one death each was reported in the nirmatrelvir-ritonavir and control group, respectively. Conclusions: Nirmatrelvir-ritonavir treatment was associated with reduced hospitalization in high-risk patients with COVID-19 even in highly vaccinated populations.

Abstract: Appendiceal diverticulitis is a rare pathology which is distinctly different to acute appendicitis and associated with higher rates of morbidity and mortality. Furthermore, diagnosis is often retrospective on histopathological analysis of appendicectomy specimens due to the atypical clinical and radiological features. Herein, we present a case of ruptured appendiceal diverticulitis in a young patient with atypical clinical features and a radiologically normal appearing appendix in close proximity to an inflammatory phlegmon. This case highlights the importance of maintaining a high clinical suspicion of surgical pathology and considering atypical diagnosis in patients with inflammatory changes in the right iliac fossa.

Abstract: Recently, transition metal doped superlattice has shown an anomalous optical band gap of 1.6 eV, about 1 eV lower than either parent element (Barium) majorly, making it appropriate for several applications including magnetism and superconducting materials. In the current study, the structural, electronic, phonon, thermodynamic, and the magnetic ordering of BaXO3 (X = Cr, Mn, Co, and Ni) has been examined using density functional theory (DFT). From the results, the investigated materials show a ferromagnetic behavior with the band gap of range 0.95–1.04 eV, and average absolute magnetization are 2.64, 3.67, 3.19, and 0.01 Bohr magneton/cell for BaCrO3, BaMnO3, BaCoO3, and BaNiO3, respectively. Furthermore, it is conceivable that the Compton profiles of BaXO3(X= Cr, Mn, Co, and Ni) are magnetic due to the substantial exchange-correlation dependence of their Compton profiles, which is shown from the phonon and X-ray distributions, thermodynamic calculation, and mechanically portrayed features of BaXO3. It was further discovered that doping could increase each TM (Cr, Mn, Co, and Ni) atom's magnetic moment. This study demonstrates a novel method for utilizing this revolutionary kind of cubic ferrites for spintronic applications in solid-state electronics.

Abstract: Background: The prostate demonstrates inter- and intra- fractional changes and thus adaptive radiotherapy would be required to ensure optimal coverage. Daily adaptive radiotherapy for MRI-guided radiotherapy can be both time and resource intensive when structure delineation is completed manually. Contours can be auto-generated on the MR-Linac via a deformable image registration (DIR) based mapping process from the reference image. This study evaluates the performance of automatically generated target structure contours against manually delineated contours by radiation oncologists for prostate radiotherapy on the Elekta Unity MR-Linac. Methods: Plans were generated from prostate contours propagated by DIR and rigid image registration (RIR) for forty fractions from ten patients. A two-dose level SIB (simultaneous integrated boost) IMRT plan is used to treat localised prostate cancer; 6000 cGy to the prostate and 4860 cGy to the seminal vesicles. The dose coverage of the PTV 6000 and PTV 4860 created from the manually drawn target structures was evaluated with each plan. If the dose objectives were met, the plan was considered successful in covering the gold standard (clinician-delineated) volume. Results: The mandatory PTV 6000 dose objective (D98% > 5580 cGy) was met in 81 % of DIR plans and 45 % of RIR plans. The SV were mapped by DIR only and for all the plans, the PTV 4860 dose objective met the optimal target (D98% > 4617 cGy). The plans created by RIR led to under-coverage of the clinician-delineated prostate, predominantly at the apex or the bladder-prostate interface. Conclusion: Plans created from DIR propagation of prostate contours outperform those created from RIR propagation. In approximately 1 in 5 DIR plans, dosimetric coverage of the gold standard PTV was not clinically acceptable. Thus, at our institution, we use a combination of DIR propagation of contours alongside manual editing of contours where deemed necessary for online treatments.

Abstract: The rising demand for the construction of pavement infrastructure creates great pressure on the production and transportation of virgin aggregates, which consumes high energy and natural resources. Therefore, the possibility of using recycled materials in pavements has generated wide interest in the growing economies of developed and developing countries. The wastes produced by construction and demolition materials such as recycled concrete aggregate (RCA) enter landfills every year. Similarly, wastes generated by end-of-life tyres are non-biodegradable and poses an environmental challenge. However, the research on the mechanical performance of waste tyre rubber (TR) in pavement applications is very limited. Therefore, finding innovative ways to incorporate these solid wastes could improve the sustainability of pavements. Meanwhile, the use of major binder (i.e., portland cement) for stabilisation of pavements contributes to carbon footprint issues. Hence, there is a need to use environmentally friendly binders such as ground granulated blast furnace slag which is a promising way to process these waste materials with significant environmental benefits. Therefore, this study investigates the feasibility of RCA and TR as a substitute for quarried pavement material, stabilised with slag (S) to improve the binding action between waste tyre rubber and surrounding aggregates. The main objective of the study was to evaluate the physical and mechanical characteristics of combined TR + RCA mixes by undertaking extensive laboratory tests. Firstly, unconfined compressive strength (UCS) test studies were conducted at 7 days of curing period at 21 °C to determine the efficacy of mixing two types of wastes by slag stabilisation at three different binder contents (10 %, 15 %, and 20 %). Subsequently, a series of resilient modulus (RLT) tests were performed to determine the stiffness of the mixes using a repeated load triaxial setup. Finally, the hydration mechanism was studied by microstructural studies (SEM) to understand the inter-particle bonding of TR + RCA mixes. It was found that stabilised mixes with 10 %TR could satisfy the minimum road requirements for pavement base layers in terms of compressive and resilient moduli characteristics. Besides, the study provided a mathematical model equation by response surface methodology to predict the values of UCS. Predicted values were found to be in good agreement with experimental values. The research offers a sustainable solution with the help of stabilisation technique in increasing the volume of combined TR + RCA wastes in pavements by satisfying road requirement criteria.

Abstract: ABSTRACT: Objective: This study aims to investigate the association between smoking and the severity of COVID-19 infection during the initial wave of this pandemic in Malaysia. Methods: This is a multi-center observational study using secondary hospital data collected retrospectively from February 1, 2020, until May 30, 2020. Clinical records of all real-time polymerase chain reaction (RT-PCR)-confirmed COVID-19 cases with smoking status, co-morbidities, clinical features, and disease management were retrieved. Severity was assessed by the presence of complications and outcomes of COVID-19 infection. Logistic regression was used to determine the association between COVID-19 disease severity and smoking status. Results: A total of 5,889 COVID-19 cases were included in the analysis. Ever smokers had a higher risk of having COVID-19 complications, such as acute respiratory distress syndrome (odds ratio [OR] 1.69; 95% confidence interval [CI] 1.09-2.55), renal injury (OR 1.55; 95% CI 1.10-2.14), and acute liver injury (OR 1.33; 95% CI 1.01-1.74), compared with never smokers. However, in terms of disease outcomes, there were no differences between the two groups. Conclusion: Although no significant association was found in terms of disease outcomes, smoking is associated with a higher risk of having complications owing to COVID-19 infection.

Abstract: The implementation of MRI-guided online adaptive radiotherapy has enabled extension of therapeutic radiographers’ roles to include contouring. An offline interobserver variability study compared five radiographers’ and five clinicians’ contours on 10 MRIs acquired on a MR-Linac from 10 patients. All contours were compared to a “gold standard” created from an average of clinicians’ contours. The median (range) DSC of radiographers’ and clinicians’ contours compared to the “gold standard” was 0.91 (0.86–0.96), and 0.93 (0.88–0.97) respectively illustrating non-inferiority of the radiographers’ contours to the clinicians. There was no significant difference in HD, MDA or volume size between the groups.

Abstract: Vascular injuries following Total Hip Arthroplasty (THA) are rare and associated with greater morbidity. This case series describes two middle aged females presented with acute limb ischemia following THA. CT angiograms of both revealed common femoral artery (CFA) injury. One had a delayed presentation of one week and other presented with in a day. Revascularization was done in both by means of Ileo femoral bypass surgery with prosthetic graft and interposition graft with reverse long saphenous vein graft. The delayed presented one ended up in bellow knee amputation and the outcome of other was good. Iatrogenic injury to CFA by pointed Hohman retractors was considered the possible mechanism of injury in both the cases. Preoperative assessment of risk factors, intraoperative awareness, and post operative assessment to look for vascular injury is a must to minimize the arterial injury during THA.

Abstract: Summary: Met is an oncogene aberrantly activated in multiple cancers. Therefore, to better understand Met biology and its role in disease we applied the Mammalian Membrane Two-Hybrid (MaMTH) to generate a targeted interactome map of its interactions with human SH2/PTB-domain-containing proteins. We identified thirty interaction partners, including sixteen that were previously unreported. Non-small cell lung cancer (NSCLC)-focused functional characterization of a Met-interacting protein, BLNK, revealed that BLNK is a positive regulator of Met signaling, and modulates localization, including ligand-dependent trafficking of Met in NSCLC cell lines. Furthermore, the interaction between Met and GRB2 is increased in the presence of BLNK, and the constitutive interaction between BLNK and GRB2 is increased in the presence of active Met. Tumor phenotypical assays uncovered roles for BLNK in anchorage-independent growth and chemotaxis of NSCLC cell lines. Cumulatively, this study provides a Met-interactome and delineates a role for BLNK in regulating Met biology in NSCLC context.

Abstract: Introduction: MR-guided adapted radiotherapy (MRgART) using a high field MR-linac has recently become available. We report the estimated delivered fractional dose of the first five prostate cancer patients treated at our centre using MRgART and compare this to C-Arm linac daily Image Guided Radiotherapy (IGRT). Methods: Patients were treated using adapted treatment plans shaped to their daily anatomy. The treatments were recalculated on an MR image acquired immediately prior to treatment delivery in order to estimate the delivered fractional dose. C-arm linac non-adapted VMAT treatment plans were recalculated on the same MR images to estimate the fractional dose that would have been delivered using conventional radiotherapy techniques using a daily IGRT protocol. Results: 95% and 93% of mandatory target coverage objectives and organ at risk dose constraints were achieved by MRgART and C-arm linac delivered dose estimates, respectively. Both delivery techniques were estimated to have achieved 98% of mandatory Organ At Risk (OAR) dose constraints whereas for the target clinical goals, 86% and 80% were achieved by MRgART and C-arm linac delivered dose estimates. Conclusions: Prostate MRgART can be delivered using the a high field MR-linac. Radiotherapy performed on a C-arm linac offers a good solution for prostate cancer patients who present with favourable anatomy at the time of reference imaging and demonstrate stable anatomy throughout the course of their treatment. For patients with critical OARs abutting target volumes on their reference image we have demonstrated the potential for a target dose coverage improvement for MRgART compared to C-arm linac treatment.

Abstract: Purpose: This study investigated the prevalence of mortality among COVID-19 patients admitted to an intensive care unit (ICU) at a single centre hospital in Klang Valley, Selangor, Malaysia. Besides, adverse clinical events (ACE) among COVID-19 patients admitted to ICU who died and were alive were compared, and the factors associated with mortality were explored. Methods & Materials: Patients admitted to a single centre ICU with polymerase chain reaction (PCR) confirmed of SARS-CoV-2 virus within February 2020-2021 were included in this study. Adverse clinical event (ACE) consists of the presence of pulmonary embolism (PE), deep vein thrombosis (DVT), line-related thrombosis, stroke, myocardial infarction (MI) and peripheral artery disease (PAD) during their ICU admission. A composite of ACE comprised ≥ 1 PE, DVT, line-related thrombosis, stroke, MI and PAD. Mortality is defined as COVID-19 patients who died during ICU admission throughout data collection. Results: Mean (SD) age was 56.6 (13.7) with 63.5% male and 61.6% Malay. Median (IQR) 7 (3-14) days of ICU admission, 64.2%, 53.2 % and 20.9% had underlying hypertension, diabetes, and obesity, respectively. Out of 534 patients included in the study, 122 patients died, with 64.8% developed ≥ 1 ACE compared to 39.1% patients who survived the infection. 22. Residual stress-strain relationship for the biochar-based mortar after exposure to elevated temperature

Abstract: The effects of elevated temperature on the compressive strength and stress-strain behaviour of biochar-based mortar were investigated in this research. A total of 39 cylindrical (100 × 200 mm) specimens were prepared with four mortar mixtures by replacing 0%, 5%, 10 % and 20 % of cement weight with biochar. The specimens were cured for 28 days and exposed to elevated temperatures of 200 °C, 450 °C and 700 °C before testing. Results show that increasing replacement rate of biochar reduces the compressive strength at room temperature as well as residual strength after being exposed to elevated temperature. The 5% weight of cement replaced by biochar performed better than other specimen at elevated temperature as it retains about 88 %, 76 % and 38 % of compressive strength, respectively after being exposed to 200 °C, 450 °C and 700 °C. The study also established the relationship among biochar mix ratio, elevated temperature, elastic modulus and peak compressive strength. Finally, a new stress-strain model was developed and validated to predict the axial behaviour of biochar-based mortar with varying biochar mix ratio and elevated temperature. This model can be used to predict the post fire mechanical behaviour of biochar-based mortar.

23. Genetic variation in high light responses of Theobroma cacao L. accessions

Abstract: Cacao (Theobroma cacao L.) is a shade-tolerant tree species, but in recent years it has increasingly been cultivated under full sun conditions in an orchard system where photoinhibition is likely. Here we investigate the extent of photoinhibition in 17 cacao accessions from a range of genetic groups, growing under high light conditions. The ability of the photosynthetic systems to respond to high light was assessed using chlorophyll fluorescence parameters (diurnal Fv/Fm and instantaneous light response curves), and differences in photosynthetic pigment content were compared using biochemical assays. Damage due to photoinhibition was assessed using electrolyte leakage, lipid peroxidation, and reactive oxygen species scavenging systems were compared using biochemical assays (for APX, CAT and SOD). There was significant variation between the 17 accessions for photosynthetic parameters, although in all cases the light saturation points were well below the midday light levels. Light acclimation of photosynthetic pigments was evident and variation in the total chlorophyll to total carotenoid ratio was significantly correlated with electrolyte leakage. Significant genetic variation was observed across the 17 accessions in the activities of CAT, APX and SOD. Across all accessions, photoprotection appeared to be restricted by the ability of leaves to generate SOD. Significant negative correlations were observed between SOD activity and both APX activity and electrolyte leakage, while significant positive correlations were observed between electrolyte leakage and both APX and CAT activity. Accessions with higher light saturation points, as well as high carotenoid and high SOD concentrations were able to tolerate the moderately high light, however, none of the accessions were clearly superior to the commonly grown Amelonado accession. The results imply that screening for SOD activity, total carotenoid content and light saturation point can aid in selection of genotypes with better tolerance to high light.

Abstract: The ongoing COVID-19 disease significantly affects not only human health, it also affects the wealth of country’ economy and everyday routine of human life. To control the spread of the virus, face mask is used as primary personal protective equipment (PPE). Thus, the production and usage of face masks significantly increase as the COVID-19 pandemic still escalating. Further, most of these masks contain plastics or other derivatives of plastics. Therefore, this extensive usage of face masks generates million tons of plastic wastes to the environments in a short span of time. This study aims to investigate the environmental impact induced by face mask wastes and sustainable solution to reduce this waste. An online survey was carried out to identify the types of face mask and number of masks used per week by an individual from 1033 people. Based on this survey and available literature, this study quantifies the amount of plastics waste generated by face masks. However, this survey was limited with certain ages, country and durations (July–August 2020). Thus, the prediction of plastic waste generation, only provide fundamental knowledge about the mask wastes. Results revealed that there is a huge plastic waste remained in land and marine environment in the form of mask waste, which will contribute to micro-plastic pollution. Therefore, this paper also highlights the sustainable approach to the mask production by integrating the use of natural plant fiber in the woven face mask technology to reduce the plastic waste induced by masks. Further, upcycling the mask waste and producing construction materials also discussed.

Malaysia reported its first imported case on 25th January 2020, and adopted a policy of extensive contact tracing and hospitalising of all cases. We describe the clinical characteristics of COVID-19 cases nationwide and determine the risk factors associated with disease severity. Method: Clinical records of all RT-PCR confirmed COVID-19 cases aged ≥12 years admitted to 18 designated hospitals in Malaysia between 1st February and 30th May 2020 with complete outcomes were retrieved. Epidemiological history, co-morbidities, clinical features, investigations, management and complications were captured using REDCap database. Variables were compared between mild and severe diseases. Univariate and multivariate regression were used to identify determinants for disease severity. Findings: The sample comprised of 5889 cases (median age 34 years, male 71.7%). Majority were mild (92%), and 3.3% required intensive care, with 80% admitted within the first five days. Abstract: Cocoa (Theobroma cacao L.) is an important neotropical tree crop grown for its seeds or beans used in global chocolate and confectionary industries. Following studies showing ill effects of long-term dietary exposure of cadmium (Cd) on human health, a number of countries including the European Union (EU) have developed stringent regulations to protect consumers from exposure to cadmium. Cocoa is capable of bioaccumulating Cd in the cocoa beans when grown in soils high in cadmium and hence livelihood of cocoa farmers can be at risk if methods to mitigate the bioaccumulation of Cd are not developed. In vitro, greenhouse and field experiments were established with four, three and three replications respectively to evaluate the effectiveness of soil amendments, biochar and lime, at various application rates (0, 0.5×, 1×, 1.5× and 2× of the recommended rate), on soil pH, Cd phytoavailability and Cd bioaccumulation in Theobroma cacao L. For the in vitro study, Cd-containing soil was amended with 5 levels of biochar and lime, while for the greenhouse and field study four application rates were tested. The study showed that while lower rates were effective under in vitro conditions as you progressed from in vitro to greenhouse to field conditions the application rates and application frequency had to be increased, as the effectiveness and longevity of the treatments were compromised by environmental factors. Our study implies that the two amendments were complementary in their action and can be used in the recommended rated to reduce Cd bioaccumulation. However further studies are required on the placement of amendments to improve their effectiveness and longevity particularly under field conditions., (Copyright © 2019. Published by Elsevier B.V.)

Abstract: Cadmium (Cd) is a non-essential heavy metal that is toxic to both plants and animals and chocolates have been identified as a contributor to the human dietary Cd intake. One hundred accessions representing the various genetic groups and hybrid populations in Theobroma cacao L. held at the International Cocoa Genebank, Trinidad were evaluated for leaf and bean cadmium levels with three tree replications. Representative samples of soil from the drip zone around each tree were evaluated for bioavailable cadmium. Although there were significant differences (P ≤ 0.05) among genetic groups for leaf and bean Cd much of the variation was between accessions. There was a 13-fold variation in bean Cd and a 7-fold variation in leaf Cd between accessions despite the bioavailable Cd in the soil being uniform. There were differences in the level of partitioning into beans evident by significant variation (P ≤ 0.05) in bean Cd as a percentage of the cumulative leaf and bean Cd concentration (15-52%) between accessions. Although in general there was a higher concentration of cadmium in the testa than the cotyledon of the cocoa bean there was considerable genetic variation. These results point to the potential of using a genetic strategy to mitigate cadmium within cocoa beans either through breeding or through the use of low cadmium uptake rootstocks in grafting. Abstract: Introduction: The efficacy of epidermal growth factor receptor (EGFR) tyrosine kinase inhibitors in EGFR mutation-positive non-small-cell lung cancer (NSCLC) patients necessitates accurate, timely testing. Although EGFR mutation testing has been adopted by many laboratories in Asia, data are lacking on the proportion of NSCLC patients tested in each country, and the most commonly used testing methods., Methods: A retrospective survey of records from NSCLC patients tested for EGFR mutations during 2011 was conducted in 11 Asian Pacific countries at 40 sites that routinely performed EGFR mutation testing during that period. Patient records were used to complete an online questionnaire at each site., Results: Of the 22,193 NSCLC patient records surveyed, 31.8% (95% confidence interval: 31.2%-32.5%) were tested for EGFR mutations. The rate of EGFR mutation positivity was 39.6% among the 10,687 cases tested. The majority of samples were biopsy and/or cytology samples (71.4%). DNA sequencing was the most commonly used testing method accounting for 40% and 32.5% of tissue and cytology samples, respectively. A pathology report was available only to 60.0% of the sites, and 47.5% were not members of a Quality Assurance Scheme., Conclusions: In 2011, EGFR mutation testing practices varied widely across Asia. These data provide a reference platform from which to improve the molecular diagnosis of NSCLC, and EGFR mutation testing in particular, in Asia.

Abstract: Introduction: Despite available data from other Asian countries, the prevalence of epidermal growth factor receptor (EGFR) mutations among lung adenocarcinoma patients has not been reported in Malaysia. This study sought to determine the frequency of EGFR mutations among multiethnic Malaysian patients diagnosed with lung adenocarcinoma., Methods: Demographic and clinical information of patients whose lung adenocarcinoma biopsy specimens were submitted for EGFR mutation testing at Sime Darby Medical Center from 2009 to 2011 were analyzed. EGFR mutations at exons 18, 19, 20, and 21 were detected either through bidirectional sequencing or real-time polymerase chain reaction., Results: Among 812 patients in the study, 49% were female, 63.7% were ethnic Chinese, 29.4% Malay, 4.8% Indian, and 2.1% other ethnic groups. Mutations were present in the tumors of 321 patients (39.5%), with mutations at exons 19 (23.5%) and 21 (14.9%) being the most common. Mutations were significantly more frequent among women than in men (52.5% versus 27.8%, p < 0.001). Although mutations were more common among Chinese (40.8%) compared with Malay (37.2%) or Indian (33.3%) patients, the difference was not statistically significant (p = 0.591). Of 211 patients with smoking history records, never-smokers had a higher mutation rate compared with ever-smokers (54.8% versus 20.7%, p < 0.001)., Conclusion: EGFR mutations were present in 39.5% of patients. Mutations were more common in women and never-smokers with no differences in mutation frequency between different ethnicities. Because of the high mutation rates, reflex testing for EGFR mutation should be a routine practice for advanced lung adenocarcinoma patients in Malaysia.
