Background: Vacuum massage, or endermotherapy, is applied to scar tissue with the primary therapeutic goal of promoting structural or physiological changes. These changes are intended to enhance pliability, enabling the skin to possess the strength and elasticity required for normal mobility. The advantage of vacuum massage compared to therapist-generated manual massage is that it provides a standardized dosage using rollers and suction valves to mobilize the tissue. However, research documenting and supporting its impact on post-burn hypertrophic scar is lacking. Thus, this study was designed to objectively characterize the changes in scar elasticity, erythema, melanin, thickness, and transepidermal water loss immediately after a vacuum massage session and after a 12-week course of treatment compared to intra-individual matched control scars., Methods: We conducted a prospective, randomized, controlled, within-patient, single-blinded clinical trial, initially designed as a fully-powered study but limited to a pilot study due to COVID-19 restrictions. Nineteen burn survivors consented to participate and 16 completed the study. Two homogeneous, intra-individual scars were randomized to usual care control or vacuum massage therapy plus usual care. Vacuum massage interventions were provided by a certified massage therapist three times per week for 12 weeks. Scar characteristics were evaluated every four weeks immediately before and after mechanical massage treatment. The evaluations included measurements of elasticity (Cutometer), erythema and melanin (Mexameter), transepidermal water loss (TEWL) (Tewameter), and thickness (high-frequency ultrasound). Linear mixed-model analyses were performed to test for immediate and long-term treatment effects., Results: The ANOVA analyses revealed a non-significant time:treatment interaction for elasticity, erythema, melanin, thickness, or TEWL. There was a significant increase in elasticity and erythema and a decrease in TEWL in both the control and treatment sites over time with consistent standard care. However, there was no statistically significant immediate or long-term treatment effect for any of the skin characteristics. Nonetheless, the mean participant satisfaction was 4/5 (SD = 1.5) and the mean participant perception of effectiveness was 8/10 (SD = 1.9)., Conclusions: This pilot study did not find a treatment benefit of vacuum massage therapy for elasticity, erythema, melanin, thickness or TEWL, but it did find an improvement with time in elasticity, erythema and TEWL. Despite the lack of objective improvement of the treated scar site, participants were satisfied with the results and believed vacuum massage was very effective. The aim of this review was to summarise the current evidence regarding the effectiveness of rehabilitation interventions in improving hand function, range of motion (ROM), hand strength, scar outcome, return to work, level of impairment/disability, level of burn knowledge and decreasing edema following hand burns in adult burn survivors. This review provides evidence-based support for the use of rehabilitation interventions for burn rehabilitation professionals. The following data sources were searched: MEDLINE, EMBASE and CINAHL from their inception up to February 2021, reference lists from all the included full-text articles were screened for additional relevant publications and monthly Google Scholar searches until December 23rd 2021 to make sure all new pertinent published articles after February 2021 would be included. Thirty-five studies were included in this review including 14 RCTs. Most of the included studies were Level 4 (46%; 16/35) and Level 2 (40%; 14/35) evidence. Only four studies were classified as Level 3 (14%; 5/35) evidence and none were considered Level 1. Most studies received a score of 5-9 (54%; 19/35) (Moderate quality). Fourteen studies received a score of ≥ 10 (40%; 14/35) (High quality) and only 2 studies received a score of< 5 (6%; 2/35) (Low quality). Articles were categorized according to the primary outcome targeted by the intervention. Clinical recommendations on higher-level evidence interventions are presented. This review supports the clinical practice of the following interventions: 1) The use of adhesive compression wraps for patients who have increased edema to increase hand function and ROM; 2) The use of compression (adhesive compressive wrap, compression bandage or intermittent compression pump) to decrease hand edema following burn injury; 3) Participating in general rehabilitation to increase hand function and patient perceived level of disability; 4) The use of an orthosis to increase ROM and a dynamic MCP orthosis to increase hand function; 5) If available, incorporate the use of VR based rehabilitation to increase hand function and hand strength; 6) The use of paraffin to increase hand PROM; 7) The use of gels to reduce hand scar thickness; 8) The use of an education component in rehabilitation to increase the level of burn knowledge. The limitations of this study are also discussed. Background: The Burnt Hand Outcome Tool (BHOT) is a comprehensive tool assessing the multiple impacts of hand burn injuries which makes it essential to burn care practice, but is currently only available in English., Objectives: To create a French-Canadian cross-cultural adaptation of the BHOT and to assess its content and construct convergent validity., Methods: The BHOT was translated and culturally adapted according to evidence-based principles for patient-reported outcome measures. The steps included translation to French (BHOT-F), backward translation, expert committee review, and cognitive debriefing with 5 adult participants. A pre-final version of the BHOT-F was then administered to 39 adult participants with hand burn injuries to assess construct convergent validity using the shortened Disabilities of the Arm, Shoulder and Hand questionnaire (QuickDASH). Content validity was evaluated based on comments from the expert committee and participant burn survivors., Results: The BHOT-F was modified during the adaption process to ensure its clarity. The Cronbach's alpha value of 0.935 demonstrates the excellent internal consistency of the BHOT-F. The BHOT-F and the QuickDASH were strongly correlated (p < 0.01). The Fear-Avoidance Model was developed with patients who had sustained neck and back injuries and describes a cascade of events after pain that is perceived as threatening, which may lead to avoidance of valued and meaningful life activities. The literature examining burn survivors suggests they may experience fear-avoidance, however, their lived experience has yet to be thoroughly explored and the evaluation tool that has been used is four items extracted from the Tampa Scale of Kinesiophobia that were modified for burn survivors, but never formally validated with this population. Therefore, the aim of this study was to explore, through a mixed methods approach, the lived experience of burn survivors with respect to fear-avoidance and determine whether the reduced four question Tampa Scale of Kinesiophobia (4TSK) reflects and accurately measures their experience. Burn survivors (n = 17) who had undergone surgical debridement and grafting were recruited. They completed the reduced 4TSK and participated in an in-depth interview to explore their lived experience regarding fear-avoidance and their opinions on the relevance of the questions. The interviews were analyzed using a thematic-analysis approach. Five major themes that represented the lived experience of fear-avoidance in burn survivors were identified: perceived vulnerability to re-injury, others as fear influencers, difficulties & hardships during recovery, engagement in activity, and active thoughts. Based upon the interviews researchers identified 9 potential fear-avoidant participants, which differed from the screening results from the reduced 4TSK (n = 12). Background: One objective of massage therapy applied to hypertrophic scar (HSc), is to improve the structural properties so skin possesses the strength and elasticity required for normal mobility. However, research supporting this effect is lacking. The objective of this study was to characterize the changes in scar elasticity, erythema, melanin, and thickness immediately after a massage therapy session and after a 12-week course of treatment compared to intra-individual matched control scars., Methods: We conducted a prospective, randomized, single-blinded, pragmatic, controlled, clinical trial evaluating the impact of a 12-week course of massage therapy. Seventy burn survivors consented to participate and 60 completed the study. Two homogeneous, intra-individual scars were randomized to usual care control or massage therapy plus usual care. Massage, occupational or physical therapists provided massage treatment 3x/week for 12 weeks. Scar site characteristics were evaluated weekly immediately before and after massage treatment including elasticity (Cutometer), erythema and melanin (Mexameter), and thickness (high-frequency ultrasound). Analysis of covariance (ANCOVAs) were performed to test for immediate and long-term treatment effects. A mixed-model approach was used to account for the intra-individual scars., Results: Scar evaluation immediately before and after massage therapy at each time point revealed changes for all scar characteristics, but the group differences were predominantly present during the early weeks of treatment. The within group long-term analysis revealed a significant increase in elasticity, and a reduction in thickness, during the 12-week treatment period for both the control scar (CS) and massage scar (MS). The increase in elasticity reached significance at week 8 for the MS and week 10 for the CS and the reduction in thickness at week 5 for the CS and week 7 for the MS. There was no significant within group long-term differences for either erythema or melanin. There were group differences in erythema at week 8 and 11 where the CS was less erythematous than the MS., Conclusions: The immediate impact of forces applied during massage therapy may lead patients and therapists to believe that there are long-term changes in elasticity, erythema, and pigmentation, however, once baseline measures, the control scar, and time were incorporated in the analysis there was no evidence of long-term benefit. Introduction: Burns of the face and mouth region have a profound impact on function. Currently the outcome measure that is most commonly used in the burn care literature is horizontal and vertical opening. Impairment-based outcomes such as this do not capture the functional implications of these injuries in spite of the devastating impact they can have on burn survivor's lives., Purpose of the Study: To generate an assessment that evaluates the impairments, activity limitations, and participation restrictions associated with mouth injuries and prospectively collect data to examine the clinimetric properties., Methods: A multistep assessment development process was undertaken including a comprehensive literature search and review, burn care expert and burn survivor interviews, generation of a preliminary version and field-testing, modifications based on field testing and updated literature review, and further field testing with data collection of 23 burn survivors. Clinimetric properties were examined by evaluating: whether there was a ceiling or floor effect, the internal consistency, construct validity, and responsiveness., Results: The mouth impairment and disability assessment (MIDA) has a 28 item self-report portion, divided into four subscales, completed by the patient and an impairment-based section completed by the burn therapist. Two items demonstrated a ceiling effect, one was removed the other retained. There was strong and statistically significant (p<0.0001) correlation of the symptoms subscale as well as vertical opening with the functional activities subscale of the MIDA. The functional activities subscale demonstrated good internal consistency and the symptoms subscale was adequate. Re-evaluation approximately seven and a half months after the baseline evaluation demonstrated a statistically significant change with time and treatment., Conclusions: The MIDA now offers clinicians the ability to assess mouth impairment and disability of burn survivors who have sustained burn injuries to their face and mouth region., (Copyright © 2017 Elsevier Ltd and ISBI. The first case of hereditary fibrinogen Aα-chain amyloidosis was recognized >20 years ago, but disease mechanisms still remain unknown. Here we report detailed clinical and proteomics studies of a French kindred with a novel amyloidogenic fibrinogen Aα-chain frameshift variant, Phe521Leufs, causing a severe familial form of renal amyloidosis. Next, we focused our investigations to elucidate the molecular basis that render this Aα-chain variant amyloidogenic. We show that a 49-mer peptide derived from the C-terminal part of the Phe521Leufs chain is deposited as fibrils in the patient's kidneys, establishing that only a small portion of Phe521Leufs directly contributes to amyloid formation in vivo. In silico analysis indicated that this 49-mer Aα-chain peptide contained a motif (VLITL), with a high intrinsic propensity for β-aggregation at residues 44 to 48 of human renal fibrils. To experimentally verify the amyloid propensity of VLITL, we generated synthetic Phe521Leufs-derived peptides and compared their capacity for fibril formation in vitro with that of their VLITL-deleted counterparts. We show that VLITL forms typical amyloid fibrils in vitro and is a major signal for cross-β-sheet self-association of the 49-mer Phe521Leufs peptide identified in vivo, whereas its absence abrogates fibril formation. This study provides compelling evidence that VLITL confers amyloidogenic properties to Aα-chain frameshift variants, yielding a previously unknown molecular basis for the pathogenesis of Aα-chain amyloidosis., (© 2017 by The American Society of Hematology.)

Introduction: Work reintegration constitutes a major milestone in the rehabilitation process of adults who have sustained a burn. Research studies with other conditions demonstrated that open, explicit communication about the worker's condition and potential limitations may facilitate this transition. However, the best approach to enable this discussion to occur has yet to be described. The aim of this exploratory qualitative study was to investigate burn survivors' and clinicians' perspectives of the barriers and facilitators to work reintegration that could be addressed through education of work colleagues, which information to communicate to the workplace and the most effective method to disseminate this knowledge., Methods: Five semi-structured focus groups were conducted with three groups of informants including: (1) 13 burn survivors who had already returned to work; (2) 7 who were planning on returning; and (3) 9 burn care professionals. Qualitative data were inductively analyzed employing constant comparative techniques., Results: Key barriers and facilitators that were identified included residual impairments, individual characteristics, support from the social environment, work accommodations and resources from the healthcare and compensation systems. Burn survivors agreed that return to work efforts were not adequately supported and that education should be provided to work colleagues about the burn and rehabilitation process, but that information on residual impairments should be communicated judiciously as it may be used prejudiciously against those seeking new employment. In the latter case, it is preferable to inform the workplace of their strengths and abilities., Conclusions: Extensive literature demonstrating the benefits of educational programs for the peers and teachers of pediatric burn survivors when they return to school already exists. This study provides evidence that there is a need for a similar process for adult burn survivors returning to work. Quantitative studies of the clinical recovery of burn scars are currently lacking. Previous reports validate the objective, precise, diagnostic capabilities of high-frequency ultrasound to measure thickness, the Cutometer(®) to measure pliability and the Mexameter(®) to measure erythema and pigmentation of scars. Thus, we prospectively quantified clinical characteristics of patient-matched, after burn hypertrophic scar (HSc), donor site scar (D) and normal skin (N) using these instruments. One investigator measured 3 sites (HSc, D, N) in 46 burn survivors at 3, 6, and 12 months after-burn. A mixed model regression analysis, adjusting p-values for multiplicity of testing, was used to compare means among sites and time points. Participants were 41.2±13.5 years old, 87% males, predominantly Caucasian, with an average of 19.5% body surface area burned. HSc thickness decreased significantly between 3 and 6, 6 and 12, and 3 and 12 months (all p<0.0001), but remained thicker than D and N skin (all p<0.0001). Pliability differed significantly between HSc, D and N sites at all time points (all p<0.0001), with HSc and D increasing between 3 and 12 months (p<0.05) but not reaching normal. HSc and D sites were significantly more erythematous than normal skin (p<0.05) at 3 and 6 months but D sites approached normal by 12 months. The only time points at which pigmentation significantly differed were the HSc and D sites at 6 months. Thickness, pliability, erythema and pigmentation of N skin remained similar over the 12 months. We found that post-burn HSc thickness, pliability and erythema differed significantly from D and N skin at 3, 6, and 12 months and does not return to normal by 12 months after-injury; however, significant improvements towards normal can be expected. Donor sites are redder than normal skin at 3 and 6 months but can be expected to return to normal by 12 months. Although the color of HSc and D sites change markedly with time these color changes are primarily due to changes in redness of the site, not melanin in this primarily Caucasian population., (Copyright © 2014 Elsevier Ltd and ISBI. Severe pruritus after burn continues to be a clinical challenge that is inadequately addressed by traditional therapies. The success of naltrexone, an opioid antagonist, in treating pruritus in other patient populations, supported the concept that it may also be effective in burn survivors. Thus, burn patients who were dissatisfied with itch relief obtained after a course of traditional therapy were offered naltrexone. Of the 15 subjects included in this case series, 2 discontinued treatment because of side-effects. Of the remaining 13, 72% reported that they were satisfied with the itch relief provided by naltrexone, 69% were able to decrease or stop taking other medications to reduce itch, 85% would recommend it to other burn patients, and 62% divulged that it improved their quality of life. A sub-group (n=8) completed the French-Canadian version of the Questionnaire for Pruritus Assessment before and after a 2-week course of naltrexone treatment. Comparisons of individual responses before and after naltrexone treatment revealed that the affective impact of itch appeared to be less burdensome. The majority reported either a reduction or no change in the frequency of itch experienced. All reported either a decrease or no change in the duration of itch experienced after naltrexone treatment. All items that previously increased itch were less likely to affect itch or there was no change, except with fatigue, physical effort, sweating and eating. There was a statistically significant reduction in their visual analogue scale intensity ratings of itch "at its worst". Thus, naltrexone positively impacted the itch experienced by burn patients in association with a high percentage of patient-satisfaction. Further controlled trials are recommended.

Background: Severe pruritus after burn continues to be a clinical challenge. To address this challenge, thorough documentation of burn survivors' itch experience is required. A "Questionnaire for Pruritus Assessment" has been developed to capture the sensory and affective experiences associated with itch as well as its functional and quality of life implications, but to evaluate the Quebec burn survivor population, a Canadian French version is required. Thus, the purpose of this study was to generate a Canadian French cross-cultural adaptation of the questionnaire suitable for the burn survivor population., Methods: Cross-cultural adaptation followed six steps: forward translation into Canadian French, back translations into English, resolution between the two versions, synthesis of the Canadian French versions, revision by a committee of experts, and testing of the pre-final version., Results: The adaptation process resolved issues surrounding translation, content validity, burn survivor-specific modifications, and format, resulting in a Canadian French pre-final version that was pilot-tested among 32 burn survivors. Content validity of the Canadian French version was assessed by a committee of experts and the participants. The results showed good item completion and adequate distribution of scores without a ceiling or floor effect., Conclusions: This study resulted in a Canadian French version of the "Questionnaire for Pruritus Assessment" that can confidently be compared to other investigations utilizing the same tool. The adapted questionnaire also provides a valuable data collection means, enabling more thorough documentation of the burn survivor itch experience.

Background: This study compared the neural structures found in grafted skin of burn survivors with neural structures found in site-matched normal skin and correlated these structures with psychophysical measures of sensation., Methods: Fifteen skin-grafted male burn survivors (47.7+/-10.4 years old) with deep partial- or full-thickness thermal burn injuries covering an average of 11+/-5.6% of their total body surface and with normal skin at a matching, unburned, contralateral site were recruited into this study. Threshold determinations and magnitude estimations for touch, cold, warmth and heat-pain were performed at sites with grafted and normal skin, using Semmes-Weinstein monofilaments and the Medoc TSA 2001 thermal stimulator. Skin biopsies from both the grafted and normal sites were stained with antibodies for protein gene product 9.5 (PGP) and neurofilament 200 kDa. Nerve fibers in the epidermis and nerve fibers or bundles of nerve fibers in the superficial and deep dermis as well as innervated blood vessels, hair follicles and sweat glands were counted., Results: On average, the data were collected 43.1+/-10.4 months after grafting. When thresholds on grafted skin were compared to thresholds on normal skin, they showed elevated sensory thresholds [touch (p<0.003), cold (p<0.031), warmth (p<0.009)]. Magnitude estimates of touch, cold and warmth differed on the two sides with sensations elicited from grafts being smaller than those from normal skin. Heat-pain thresholds and heat-pain magnitude estimations were not statistically different on the two sites. By comparison to the normal side, and consistent with the attenuated sensory functions of the grafts, counts of neural structures showed a reduction in innervation density; PGP-immunoreactive nerve fibers/bundles were reduced in grafted epidermis (p<0.026) and superficial dermis (p<0.001). The numbers of sweat glands (p<0.006) and hair follicles (p<0.001) were also reduced. The number of innervated blood vessels did not differ significantly on the two sides. There were significant correlations between sensory thresholds and the neuroanatomical variables: thresholds of cold and touch were correlated with the number of sweat glands in both grafted and normal skin (r2=0.56 and 0.50, respectively; p<0.001), while warmth thresholds were significantly correlated with the number of innervated blood vessels in grafted skin (r2=0.62, p<0.001). Encapsulated mechanoreceptors were not encountered in this study of hairy skin., Conclusions: Touch, cold and warmth thresholds and magnitude estimations do not return to normal levels after skin grafting in burn survivors. The elevation of thresholds and reduction of sensory intensity is accompanied by a general decrease in the density of nerve terminals. The lack, or numerical reduction, of sweat glands and innervated blood vessels was also indicative of diminished sensation on grafted skin.

We recently demonstrated that the accumulation of extracellular matrix in post-burn hypertrophic scarring (HSc) tissues is, in part, caused by an overexpression of mRNA for fibronectin, type I, and type III procollagen. Here, we report that five different fibroblast cell strains derived from HSc tissues are deficient in collagenase activity relative to paired fibroblasts from normal skin of the same patients. Quantitative analysis demonstrated significantly lower (52.5 +/- 16.8% vs 100 +/- 8.3%; n = 9; p < 0.05) collagenase activity in conditioned medium from HSc fibroblasts relative to that obtained from the control. The expression of collagenase mRNA was also significantly depressed (51 +/- 7% vs 100 +/- 11%; n = 5; p < 0.05) in four of five strains of HSc fibroblasts examined. The level of mRNA for collagenase in both HSc and normal fibroblasts increased with serial passage, but at any given passage number, the expression of this transcript was lower in HSc fibroblasts. Insulin-like growth factor 1 (IGF-1), which is present at the site of HSc in high quantity, reduced collagenase mRNA but increased type I collagen mRNA expression in a time-dependent manner. The collagenase activity in conditioned medium derived from IGF-1-treated normal dermal fibroblasts was reduced (23.1 +/- 7.81% vs 100 +/- 6.6%; n = 7; p < 0.05). A significant reduction in collagenase mRNA and activity was also found in HSc fibroblasts following IGF-1 treatment. These findings suggest that IGF-1-induced suppression of collagenase mRNA and activity may be a mechanism by which IGF-1 promotes the development of post-burn HSc.

Wound contraction is an essential component of wound healing. However, the development of scar contractures in tissues and organs disrupts normal organ integrity and produces functional deformities. Although interferons alpha and gamma inhibit extracellular matrix protein production by fibroblasts, their effects on cytoskeletal protein mediated-wound contraction are as yet unclear. The fibroblast-populated collagen lattice is an in vitro assay that simulates wound contraction. When matched pairs of human hypertrophic scar and normal dermal fibroblast cultures established from patients recovering from a thermal injury were used, interferon-alpha 2b exposure before lattice formation was found to significantly inhibit contraction in a treatment time-dependent manner (p < 0.05). Fibroblasts generated contractile forces that were triphasic and serum sensitive (p < 0.01). Comparison of hypertrophic scar and normal dermal fibroblasts revealed no significant differences in ability to induce lattice contraction. Northern blot analysis of mRNAs for the intracellular contractile proteins revealed that interferon-alpha 2b significantly down-regulated mRNA levels of the actin isoforms beta and gamma (50% to 60%) but had no significant effect on alpha-tubulin, vimentin, and alpha-actinin. Fibroblast-populated collagen lattices were stained with rhodamine-labeled phalloidin to reveal filamentous actin proteins. Marked morphologic alterations of the stress fibers were associated with reductions in lattice contraction after interferon-alpha 2b treatment. Thus interferon-alpha 2b's inhibition of wound contraction in vitro is associated with reductions in mRNA for beta and gamma actin and distinct morphologic alterations in fibroblast stress fiber morphology.
