Abstract

The objective of the current study is to elucidate the temporal dynamics of suspiciousness and hallucinations as they occur in daily life in the early stages of psychosis. Their prevalence and co-occurrence, as well as their temporal relation to affect and delusions, were compared between patients with a first psychotic episode (FEP) and individuals at clinical high risk for psychosis (CHRp). The Experience Sampling Method was used to investigate suspiciousness and hallucinatory experiences, delusions, and affect at semi-random moments throughout six days in 33 CHRp and 34 FEP. Overall, 91% of CHRp and 59% of FEP reported suspiciousness, and 24% and 39% reported hallucinations, respectively. Hallucinations almost always co-occurred with suspiciousness, whereas suspiciousness was often present without hallucinations. Suspicious episodes in CHRp occurred with marked increases in delusional intensity, while hallucinatory experiences were mostly absent. In FEP, a decrease of positive affect preceded suspicious episodes, while an increase of negative affect preceded hallucinatory episodes. Our results indicated the presence of a delusional mood (atmosphere) in CHRp as an experience in itself, without co-occurring or following hallucinations, thus refuting the anomalous experience hypothesis of psychosis. Abstract

Within psychosis, impairments in social cognition have been hypothesized to be a driving force behind daily-life social impairments. However, traditional measures of social cognition have low ecological validity, as evidenced by a weak correlation between social cognition and social functioning, and the limited usefulness of treatments aimed at improving social cognition. In order to obtain a more comprehensive view of social impairments, assessments are needed that take into account the multifaceted, dynamic, contextualized, and interactive nature of social interactions. In this chapter, criteria will be discussed that are necessary for achieving high ecological validity. In addition, the use of virtual reality (VR) and the experience sampling method (ESM) will be discussed in the context of the contribution so far, and future potential, of these techniques in order to better understand and intervene in the mechanisms contributing to social impairments in psychosis.

Abstract

Surface associated pneumococcal proteins alpha-enolase (Eno), immunoglobulin A1 protease (Iga), streptococcal lipoprotein rotamase A (SlrA), and putative proteinase maturation protein A (PpmA) have potential as candidates for future protein-based anti-pneumococcal vaccines. The immunogenicity of these proteins were studied in a cohort of 329 children during their first two years of life. During the first recorded episode of otitis media, acute and convalescent phase sera were available from 151 children. Concentrations of antibodies against Eno, Iga, SlrA and PpmA were measured by EIA and detected in 99% (300/302), 95% (288/302), 95% (288/302), and 83% (251/302) of the sera, respectively. There were no statistically significant differences between the groups of children with and without a history of pneumococcal contact or with respect to the type of pneumococcal contact. Despite a mean overall decrease in the antibody titers in the convalescent sera following AOM, several children were able to respond with a more than twofold increase in antibody titer in response to AOM. The majority of the children with increased antibody concentrations appeared in the groups, which were colonized with pneumococci at the time of serum collection, but were recorded as having no prior contact with pneumococci. In conclusion, SlrA, PpmA, Eno and Iga are immunogenic proteins that elicit antibody responses early in life. No significant correlation between antibody titers to these proteins and pneumococcal carriage or infection was found. Presumably, this results from the presence of cross-reactive epitopes on commensal bacteria.

Abstract

The surface proteome of a pathogen is an essential toolbox for its ability to colonize and persist in its host. Streptococcus pneumoniae expresses different families of surface-exposed proteins, distinguished by their mode of attachment to the cell: lipoproteins, choline-binding proteins, LPxTG-anchored proteins, the pneumococcal histidine triad family proteins, and non-classical surface proteins. These proteins have diverse roles in the interaction of the pneumococcus with its host, such as adhesion to epithelial cells and extracellular matrix proteins, substrate transport, immune evasion, and bacterial fitness. Here we will focus on a subset of surface proteins of S. pneumoniae, namely those whose predicted function in pneumococcal pathogenesis is not directly related to cellular adhesion.

Abstract

Environmental decision making involving multiple stakeholders can benefit from the use of a formal process to structure stakeholder interactions, leading to more successful outcomes than traditional discursive decision processes. There are many tools available to handle complex decision making. Here we illustrate the use of a multicriteria decision analysis (MCDA) outranking tool (PROMETHEE) to facilitate decision making at the watershed scale, involving multiple stakeholders, multiple criteria, and multiple objectives. We compare various MCDA methods and their theoretical underpinnings, examining methods that most realistically model complex decision problems in ways that are understandable and transparent to stakeholders.

Abstract

Summary: Background: Clinical trials feature centrally in the development of drugs and vaccines to determine safety and efficacy. Clinical development can be slow and may have a duration of more than ten years. Global public health threats such as Ebola virus disease (EVD) and COVID-19 have demonstrated that it is possible to accelerate clinical trials while maintaining safety and efficacy. We investigated acceleration in clinical trials over the past decade and identified factors associated with acceleration for drugs targeting infectious diseases. Methods: A cross-sectional study was performed of all medicinal compounds targeting infectious diseases that received marketing authorisation by the European Medicines Agency (EMA) between 2012 and 2022. We calculated median clinical development time in years between the first phase 1 trial enrolment date and the authorisation date. Multivariable linear regression analysis was performed to identify factors associated with shorter development times. Findings: Eighty-one trajectories were included. The median clinical development time was 7.3 years (IQR 4.4–12.3). The fastest times belonged to drugs and vaccines targeting COVID-19 (1.3 years, IQR 0.8–1.6), EVD (5.5 years, IQR 5.1–5.8), and Hepatitis A-E (5.5 years, IQR 3.9–8.2). Factors associated with shorter development times were outbreak setting (−5.4 years [95% CI, −8.2 to −2.6]), accelerated assessment status (−4.0 years [95% CI, −7.6 to −0.5]), and drugs with combined compounds (−2.7 years [95% CI, −4.9 to −0.4]). Interpretation: Clinical development time for infectious disease-related drugs and vaccines was relatively short, and outbreak setting and accelerated EMA assessment were associated with shorter development times. Abstract

Summary: Human genetic studies of critical COVID-19 pneumonia have revealed the essential role of type I interferon-dependent innate immunity to SARS-CoV-2 infection. Conversely, an association between the HLA-B∗15:01 allele and asymptomatic SARS-CoV-2 infection in unvaccinated individuals was recently reported, suggesting a contribution of pre-existing T cell-dependent adaptive immunity. We report a lack of association of classical HLA alleles, including HLA-B∗15:01, with pre-omicron asymptomatic SARS-CoV-2 infection in unvaccinated participants in a prospective population-based study in the United States (191 asymptomatic vs. 945 symptomatic COVID-19 cases). Moreover, we found no such association in the international COVID Human Genetic Effort cohort (206 asymptomatic vs. 574 mild or moderate COVID-19 cases and 1,625 severe or critical COVID-19 cases). Finally, in the Human Challenge Characterisation study, the three HLA-B∗15:01 individuals infected with SARS-CoV-2 developed symptoms. As with other acute primary infections studied, no classical HLA alleles favoring an asymptomatic course of SARS-CoV-2 infection were identified.

Abstract

We describe the case of a 70-year-old immunocompromised patient who developed a severe disseminated Mycobacterium bovis BCG infection with several infected prosthetic joints and soft tissue abscesses following intravesical BCG instillation for bladder cancer. This case report addresses the risk of severe complications in immunocompromised patients and the challenges in the management of this potentially life-threatening condition.

Abstract

Summary: Regenerative agriculture (RA) is gaining traction globally as an approach for meeting growing food demands while avoiding, or even remediating, the detrimental environmental consequences associated with conventional farming. Momentum is building for science to provide evidence for, or against, the putative ecosystem benefits of RA practices relative to conventional farming. In this perspective article, we advance the argument that consideration of the soil microbiome in RA research is crucial for disentangling the varied and complex relationships RA practices have with the biotic and abiotic environment, outline the expected changes in soil microbiomes under RA, and make recommendations for designing research that will answer the outstanding questions on the soil microbiome under RA. Ultimately, deeper insights into the role of microbial communities in RA soils will allow the development of biologically relevant monitoring tools which will support land managers in addressing the key environmental issues associated with agriculture.

Abstract

Arthropods have long been appreciated as useful and important ecological indicators of beneficial or detrimental changes occurring in their environment, especially those driven by anthropogenic activity. However, morphological identification, especially of key groups such as soil arthropods, is laborious and requires high-level expertise, limiting the spatiotemporal scales of such monitoring. Molecular methods based on environmental DNA (eDNA) sampling have potential for scaling up sampling of arthropod biodiversity for environmental monitoring, due to the high-throughput capabilities of such methods. However, we still do not have a clear understanding of how well molecular methods detect variation in arthropod biodiversity over space and time, particularly at fine grains. We therefore conducted a study employing a standard eDNA metabarcoding approach to monitor the composition of subterranean arthropod communities in a homogeneous forest (pine plantation) and a heterogeneous forest (regenerating native woody vegetation). We sampled the two habitats at a range of spatial scales (cm to m) and with temporal replicates over ten months to capture seasonal variation. Our analysis of almost 800 samples showed that arthropod community composition differed significantly between the two forest types and habitat explained 16.7% of the variation observed overall. However, we did not observe strong temporal or seasonal change in either richness or composition. We attribute the perceived lack of fine-grained spatiotemporal pattern to current limitations of eDNA metabarcoding, which captured inadequate biodiversity to detect changes at these fine grains. We show that increased sampling and replicate DNA extractions result in more biodiversity being captured and reveal amplification biases and a lack of taxonomic assignments. Identifying these weaknesses highlights where efforts to improve eDNA metabarcoding should be focused. The many benefits that eDNA metabarcoding can bring to biodiversity based ecological monitoring mean the efforts still required to improve the reliability and reproducibility of these methods are undoubtedly worthwhile priorities.

Abstract

Background: Mortality rates are high among hospitalized patients with COVID-19, especially in those intubated on the ICU. Insight in pathways associated with unfavourable outcome may lead to new treatment strategies. Methods: We performed a prospective cohort study of patients with COVID-19 admitted to general ward or ICU who underwent serial blood sampling. To provide insight in the pathways involved in disease progression, associations were estimated between outcome risk and serial measurements of 64 biomarkers in potential important pathways of COVID-19 infection (inflammation, tissue damage, complement system, coagulation and fibrinolysis) using joint models combining Cox regression and linear mixed-effects models. For patients admitted to the general ward, the primary outcome was admission to the ICU or mortality (unfavourable outcome). For patients admitted to the ICU, the primary outcome was 12-week mortality. Findings: A total of 219 patients were included: 136 (62%) on the ward and 119 patients (54%) on the ICU; 36 patients (26%) were included in both cohorts because they were transferred from general ward to ICU. On the general ward, 54 of 136 patients (40%) had an unfavourable outcome and 31 (23%) patients died. On the ICU, 54 out of 119 patients (45%) died. Unfavourable outcome on the general ward was associated with changes in concentrations of IL-6, IL-8, IL-10, soluble Receptor for Advanced Glycation End Products (sRAGE), vascular cell adhesion molecule 1 (VCAM-1) and Pentraxin-3. Death on the ICU was associated with changes in IL-6, IL-8, IL-10, sRAGE, VCAM-1, Pentraxin-3, urokinase-type plasminogen activator receptor, IL-1-receptor antagonist, CD14, procalcitonin, tumor necrosis factor alfa, tissue factor, complement component 5a, Growth arrest–specific 6, angiopoietin 2, and lactoferrin. Pathway analysis showed that unfavourable outcome on the ward was mainly driven by chemotaxis and interleukin production, whereas death on ICU was associated with a variety of pathways including chemotaxis, cell-cell adhesion, innate host response mechanisms, including the complement system, viral life cycle regulation, angiogenesis, wound healing and response to corticosteroids. Interpretation: Clinical deterioration in patients with severe COVID-19 involves multiple pathways, including chemotaxis and interleukin production, but also endothelial dysfunction, the complement system, and immunothrombosis. Prognostic markers showed considerable overlap between general ward and ICU patients, but we identified distinct differences between groups that should be considered in the development and timing of interventional therapies in COVID-19. Abstract

Catalytic activity in the partial oxidation (CPO) of methane of some catalysts containing Ni or Rh/Ni active metals obtained by the calcination and reduction of hydrotalcite-like compounds was investigated. In particular, two hydrotalcite-like compounds subjected to the microwave-hydrothermal method (MWHT) were studied in order to evaluate the role of the synthesis method of the precursors on the catalytic activity and catalyst stability as compared to catalysts prepared by the conventional method. The tests carried out at 750°C do not evidence any difference among catalysts. However, when the temperature is reduced to 500°C a better catalytic performance is observed for the microwave-assited catalysts containing nickel, whereas for the bimetallic catalyst the best activity is achieved with the conventional method.

16. Ebullition dominates methane emissions in stratified coastal waters.

Abstract

Coastal areas are an important source of methane (CH 4 ). However, the exact origins of CH 4 in the surface waters of coastal regions, which in turn drive sea-air emissions, remain uncertain. To gain a comprehensive understanding of the current and future climate change feedbacks, it is crucial to identify these CH 4 sources and processes that regulate its formation and oxidation. This study investigated coastal CH 4 dynamics by comparing water column data from six stations located in the brackish Tvärminne Archipelago, Baltic Sea. The sediment biogeochemistry and microbiology were further investigated at two stations (i.e., nearshore and offshore). These stations differed in terms of stratification, bottom water redox conditions, and organic matter loading. At the nearshore station, CH 4 diffusion from the sediment into the water column was negligible, because nearly all CH 4 was oxidized within the upper sediment column before reaching the sediment surface. On the other hand, at the offshore station, there was significant benthic diffusion of CH 4 , albeit the majority underwent oxidation before reaching the sediment-water interface, due to shoaling of the sulfate methane transition zone (SMTZ). The potential contribution of CH 4 production in the water column was evaluated and was found to be negligible. After examining the isotopic signatures of δ 13 C-CH 4 across the sediment and water column, it became apparent that the surface water δ 13 C-CH 4 values observed in areas with thermal stratification could not be explained by diffusion, advective fluxes, nor production in the water column. In fact, these values bore a remarkable resemblance to those detected below the SMTZ. 17. Elevated internal phosphorus loading from shallow areas of eutrophic boreal lakes: Insights from porewater geochemistry.

Abstract

Internal phosphorus (P) loading is widely recognized as a major cause of lake eutrophication. One conventional paradigm states that the magnitude of internal loading through P diffusion is constrained by the presence of iron (Fe) oxides in surface sediments under oxic conditions near the sediment-water interface (SWI). However, biogeochemical P dynamics in Fe-rich sedimentary systems are still not fully understood, especially in eutrophic lakes where intensively coupled organic matter (OM) remineralization and reductive dissolution of Fe-bound P (Fe-P) exist concurrently. Here, we assess the diagenetic processes that govern sedimentary P cycling in two eutrophic Fe-rich lakes in southern Finland, Lake Hiidenvesi and Lake Kytäjärvi, using a combination of porewater and solid-phase analyses. Coupled reductive dissolution of Fe-P and OM remineralization controlled P regeneration in both lakes, with Fe-P acting as the dominant source for porewater P. Vivianite formation likely immobilized sedimentary P in the deepest basin of Hiidenvesi. Elevated P diffusion rates were observed at shallow sites under oxic bottom water conditions in summer in both lakes, stimulated by enhanced remineralization of both freshly- (mostly phytoplankton-origin) and earlier-deposited OM under elevated temperatures. Areas overlain by oxic bottom water contributed more benthic P fluxes to the water column compared to anoxic/hypoxic areas in both lakes during all sampling seasons. Our study suggests that in shallow eutrophic settings with high OM deposition and elevated temperatures, remineralization in upper sediments regenerates P efficiently enough to support a significant amount of P release to the water column even under sedimentary molar Fe/P ratios >20. Abstract

Background: Adverse events are common in high-risk surgical patients, but early detection is difficult. Recent innovations have resulted in wireless and 'wearable' sensors, which may capture patient deterioration at an early stage, but little is known regarding their ability to timely detect events. The objective of this study is to describe the ability of currently available wireless sensors to detect adverse events in high-risk patients., Methods: A descriptive analysis was performed of all vital signs trend data obtained during an observational comparison study of wearable sensors for vital signs monitoring in high-risk surgical patients during the initial days of recovery at a surgical step-down unit (SDU) and subsequent traumatology or surgical oncology ward. Heart rate (HR), respiratory rate (RR) and oxygen saturation (SpO 2 ) were continuously recorded. Vital sign trend patterns of patients that developed adverse events were described and compared to vital sign recordings of patients without occurrence of adverse events. Two wearable patch sensors were used (SensiumVitals and HealthPatch), a bed-based mattress sensor (EarlySense) and a patient-worn monitor (Masimo Radius-7)., Results: Twenty adverse events occurred in 11 of the 31 patients included. Atrial fibrillation (AF) was most common (20%). The onset of AF was recognizable as a sudden increase in HR in all recordings, and all patients with new-onset AF after esophagectomy developed other postoperative complications. Patients who developed respiratory insufficiency showed an increase in RR and a decrease in SpO 2 , but an increase in HR was not always visible. In patients without adverse events, temporary periods of high HR and RR are observed as well, but these were transient and less frequent., Conclusions: Current systems for remote wireless patient monitoring on the ward are capable of detecting abnormalities in vital sign patterns in patients who develop adverse events. Abstract

Aims: To evaluate the real-world effectiveness of vildagliptin and vildagliptin/metformin, combined with patient engagement, on glycemic outcomes. Patient engagement included both clinicians' engaging patients through education and counseling; and patients' self-engagement through disease awareness, lifestyle changes, and medication adherence., Methods: Prospective, observational, open-label, multi-center, pharmacoepidemiologic study of type 2 diabetes mellitus (T2DM) patients treated de novo with vildagliptin or vildagliptin/metformin. Data were collected at baseline (treatment initiation), 105±15d, and ≥145d., Results: The evaluable sample included 896 mainly male (58%), overweight (mean±SD BMI=30.3±5.4kg/m 2 ), in later middle age (mean±SD age=64±11years) patients. Over the three visits, mean(±SD) HbA1c levels declined from 8.1%(±1.0) to 7.3%(±1.0) to 7.2%(±0.9); HbA1c control rates rose from 7% to 36% to 43%. Mean±SD FPG levels decreased from 170(±49) to 141(±41) to 139(±42)mg/dL; control rates increased from 12% to 39% to 43% (all p<0.0001). Weight decreased nominally by 2kg (p=0.0290) and BMI by 0.8kg/m 2 (p<0.0001). Modeling showed patient engagement activities by clinicians and by patients to be major determinants of glycemic outcomes. Abstract

To identify issues of sample mix-ups, various molecular techniques are currently used. These techniques, however, are time consuming and require experience and/or DNA sequencing equipment or have a relatively high risk of errors because of contamination. Therefore, a quick and straightforward single nucleotide polymorphism (SNP) profiling assay was developed to link human tissues to a source. SNPs are common sequence variations in the human genome, and each individual has a unique combination of these nucleotide variations. Using potentially mislabeled paraffin-embedded tissues, DNA was extracted and SNP profiles were determined by real-time polymerase chain reaction analysis of the purified DNA using a selection of 10 commercially available SNP amplification assays. These profiles were compared with profiles of the supposed owners. All issues (34 in total) of potential sample mix-ups during the last 3 years were adequately solved, with six cases described here. The SNP profiling assay provides a quick (within 24 hours), easy, and reliable way to link human samples to a source, without polymerase chain reaction postprocessing. The chance for two randomly chosen individuals to have an identical profile is 1 in 18,000. Solving potential sample mix-ups will secure downstream evaluations and critical decisions concerning the patients involved.

22. Pathologic findings at radical prostatectomy: risk factors for failure and death.

Abstract

Background: Failure after radical prostatectomy can occur even out to 25 years after surgery. Therefore, it is important that studies have sufficient follow-up to determine more accurately the risk of failure. We evaluated a large cohort of patients for pathologic findings and risk of failure with a median follow-up of 9.5 years., Material and Methods: Between 1985 and 1995, 719 patients underwent radical prostatectomy for lymph node negative prostate cancer. The prostate was inked and evaluated for: (1) positive bladder neck or urethral margin, (2) positive seminal vesicle, (3) into capsule, (4) through capsule, and (5) positive margin. These were considered positive pathologic findings., Results: Overall, 264 (37%) of the patients had biochemical recurrence. For those patients with failure, median time to biochemical recurrence was 2.4 years. Five and 10-year biochemical failure rates were 28% and 38%, respectively. Pathologic stage of disease significantly (<0.0001) predicted for subsequent failure. If there were no positive pathology findings, the recurrence rate was 25%, compared to 63% for any of the 3 findings. Overall, 212 (29%) of the patients have died. Five and 10-year survival were 91% and 75%, respectively. A total of 45 patients (6%) died of prostate cancer. For patients with negative pathology findings, 3% died as a direct consequence of prostate cancer, compared to 13% if the pathology was positive. Of the patients with positive seminal vesicle, 28% died of cancer., Conclusion: Patients with any of the following factors have a risk of failure exceeding 40% and are candidates for studies of adjuvant therapy: seminal vesicle involvement, extension through the capsule, or margin involvement.

Abstract

Background/methods: Primary non-Hodgkin's lymphomas of the liver are rare. One specific clinico-pathological entity has been identified as hepatosplenic gamma/delta T-cell lymphoma. Recently, another distinct primary lymphoma of the liver has been recognised as primary low-grade hepatic B-cell lymphoma of mucosa-associated lymphoid tissue (MALT), based on a study comprising four cases. We analysed two additional cases of this particular non-Hodgkin's lymphoma, not only by morphology and phenotyping, but also by genotyping and cytogenetic analysis., Results: This type of non-Hodgkin's lymphoma is characterised by a dense lymphoid infiltrate, localised in the portal tracts, and is associated with lympho-epithelial lesions of the bile ducts, thereby mimicking hepatitis or an inflammatory bile duct disorder. In one of our cases, translocation t(3;14)(q27;q32) was identified as the sole cytogenetic abnormality. A high incidence of trisomy 3 has been associated with marginal zone B-cell lymphomas, and fluorescence in situ hybridisation as well as comparative genomic hybridisation studies have shown frequent involvement of the long arm of chromosome 3. Nevertheless, t(3;14)(q27;q32) involving BCL6 gene, located at 3q27, has not yet been found., Conclusion: Our findings suggest a role for the BCL6 gene in the histogenesis of this particular lymphoma.
